Background: Parastomal hernia is defined as a defect at the level of the abdominal wall related to the presence of an enterostomy. It is a common complication in stoma formation. Sugarbaker's laparoscopic repair technique consists of reinforcing the posterior wall with a flat mesh that rides over the intestinal loop of the stoma as a bridge, tunneling a portion of it. Case report: We present the case of a 42-year-old patient with the presence of a 10 × 10 cm parastomal hernia and an uncomplicated umbilical hernia, undergoing repair of the abdominal wall with placement of a double-component mesh using a laparoscopic approach with the Sugarbaker technique. Conclusion: The laparoscopic approach for parastomal hernia repair using the Sugarbaker technique is safe and associated with favorable short-term outcomes. [ABSTRACT FROM AUTHOR]

Introduction: Umbilical hernia surgery is a procedure that has aroused the interest of surgeons worldwide due to its high prevalence, advances in knowledge and techniques in the abdominal wall. The objective of this study is to carry out a critical review of the literature from an analytical and reflective position in order to generate a document that promotes understanding among surgeons. Methods: A search of the available literature in English and Spanish since 1990 (time interval: 30 years) was performed using PubMed, the Cochrane Library, and a journal search. Search terms included 'umbilical hernia', 'treatment' and 'surgery'. Results: They are presented in response to the questions: How has umbilical hernia surgery evolved? What is the current status of treatment? What is the role of biomaterials? What are the expectations? What are your treatment options? Conclusions: New advances in umbilical hernia surgery create opportunities for their implementation by surgeons and institutions in order to adapt and improve our clinical practice. Introduction: omphalocele is one of the most frequent defects of anterior abdominal wall. Objectives: to determine the adjusted prevalence rate and to describe the effectiveness of prenatal diagnostic methods, chromosomal abnormalities and congenital defects most frequently associated with omphalocele. Methods: a retrospective descriptive observational study was carried out in Villa Clara Province between January 2010 and December 2019. Data were obtained from the Cuban registry of congenital malformations. The universe was constituted by the 26 cases diagnosed pre or postnatally with omphalocele. The reference population was 75 397 live newborns and 448 stillbirths during the period. Results: the adjusted prevalence rate was 3.4 per 10 000 births. Prenatal diagnosis was made in 96% of cases, the sensitivity of maternal serum alpha fetoprotein determination was 13.8%, while prenatal ultrasound had a sensitivity of 96%. In 12 of the 26 cases with omphalocele (46%) some other associated major congenital defect was found, the most frequently identified were musculoskeletal (31.8%) and cardiovascular (22.7%). Conclusions: the adjusted prevalence observed was slightly higher than that reported in the literature, which could be due to the inclusion of all cases resulting from gestational interruptions. Ultrasound was the most effective method of prenatal diagnosis. Musculoskeletal and cardiovascular defects were the most frequently associated, especially in cases with chromosomal aneuploidy. The case report of a 26 years pregnant woman and gestacional age of 34.4 weeks is described, who was referred from General Hospital of Rusape to Harare Central Hospital, both in Zimbabwe, due to an umbilical hernia that afflicted her since the childhood. The patient had suffered from that hernial volume in 3 previous pregnancies whose childbirths were normal. After the pertinent studies that demonstrated the diagnosis, and evaluate the case in multidisciplinary team, it was decided to practice an emergency Cesarean section. During the surgical procedure it was found that the whole uterus was inside the hernial sack covered by omentum; the new born was extracted, who possessed good vitality, Apgar index 9-10 and 2590 grams weight; later on, the redundant hernial tissue was dried up. Data of 23,449 piglets (2,399 litters) of a comercial farm in the Northwest parto f Mexico were analyzed. All litters were from hybrid shows and purebreed boars, registered Duroc, Yorkshire, Chester-White and Hampshire. Looking at the frequency on cryptorchidism, umbilical hernia and pseudohermaphroditsm, the expected value of boars being double and triple carriers of a single gene, the Independence hypothesis is rejected when it is compared with the observed values. An hypothesis of a single gene affecting the middle line is proposed. The consideration must be taken when selecting breeding animals in order to avoid the presentation of these and probably other middle line deffects. [ABSTRACT FROM AUTHOR]

Preeclampsia (PE) is a hypertensive disorder induced by pregnancy where there is a reduction in the uterine perfusion pressure. Research supports the use of low dose aspirin (LDAAS) and its usefulness in the prevention of PE in pregnant women with risk factors. Their benefits in animal models subject to RUPP are not determined. The objective of the investigation was to analyze the systemic blood pressure and the morphological findings at renal level in fetuses of rats with reduction of uterine perfusion pressure (RUPP) exposed to LDAAS compared to those not exposed. Four groups of pregnant female rats Sprague Dawley (n=5) were formed. At 14.5 days post-conception (dpc), surgical RUPP was induced, ligating uterine arteries, with the RUPP group and RUPP+LDAAS group being given 5 mg/kg/day of aspirin orally. The control group was made up of those not operated and the LDAAS group was administered aspirin in the same dose from 14.5 dpc. A 18.5 dpc, prior to euthanasia systemic blood pressure was measured with flow plethysmograph Insight v2.11 and fetuses were extracted. The cephalo-caudal length (CCL) was measured, processed and stained with hematoxylin-eosin, describing transverse histological sections at the kidney level. It was determined that in the mean arterial pressure, there were significant differences between the group RUPP and RUPP+LDAAS (p <0.05). The size of the fetuses was lower in the RUPP group (p <0.0001), where one fetus presented congenital umbilical hernia. The quantification of renal vesicles was also lower (p <0.005). In conclusion, the administration of LDAAS decreases the effects induced by RUPP in terms of fetal size, renal morphology and congenital malformations such as umbilical hernia. BACKGROUND: Umbilical herniorrhaphy is one of the most performed surgical interventions in pediatric patients; his treatment has changed little in the last 100 years. External compression was often recommended and insisted on the importance of preserving the appearance of the navel, rather than excision. The complications are exceptional, so the spontaneous rupture of the umbilical hernia is extremely rare. CLINICAL CASE: Female infant of two months, previously healthy, with rupture of an umbilical hernial sac associated with the placement of a foreign body, with evisceration of the small intestine. The clinical picture was of short evolution because the surgical repair was immediate, with postoperative recovery without setbacks; It required brief intrahospital treatment. CONCLUSIONS: Although the indications for surgical repair of umbilical hernia have been debated and may vary among pediatric surgeons, the risk of complications is very low; The cases of mortality reported are few. Ascites is the most common complication in patients with liver cirrhosis. Umbilical hernias (HU) occur in 20% of these patients and 40% in those with severe ascites. HU occurs due to increased intra-abdominal pressure, weakening of the abdominal fascia and loss of muscle mass. In addition, they have a tendency to enlarge rapidly and present high risk of complications that threaten the patient's life. The treatment of the uncomplicated HU is controversial, both the surgical management (herniorrhaphy) and the conservative management (control of ascites) present high rate of complications, consequently high morbidity and mortality. Currently, umbilical herniorrhaphy is recommended with prior control of ascites in uncomplicated HU management, it reduces the risk of surgical wound infection, evisceration, ascites drainage, peritonitis, and it reduces up to 41% of HU recurrence. The success of this approach also depends on the degree of liver dysfunction. The treatment of complicated HU is surgical (herniorrhaphy without mesh), with lower mortality rate compared to conservative management. Studies reveal advantages of umbilical herniorrhaphy laparoscopy (minimally invasive and stress-free) compared to open surgery; however there is still no evidence about it. [ABSTRACT FROM AUTHOR]

The case report of a 47 years adult is described who went to the emergency room of "Saturnino Lora Torres" Teaching Clinical Surgical Provincial Hospital in Santiago de Cuba due to epigastric distress of insidious beginning for 3 days that disappeared in the last 24 hours, appearing again in the right iliac cavity and mesogastrium, but this time it was transformed in a fixed, sharp, of moderate intensity pain that was not relieved with analgesics, and associated with nausea; equally, he referred the presence of an umbilical hernia that was not able to decrease and it was corroborated in the physical exam. The patient was operated on and a strangulated umbilical Littre hernia was found. A resection and anastomosis of ilium at the Meckel diverticulum level was carried out, as well as umbilical hernia repair. Background: in 1958, Cantrell et al. described a rare syndrome in neonates. The description of the syndrome was characterized by a midline supraumbilical defect encompassing a thoracoabdominal omphalocele and ectopia cordis, a defect of the lower sternum, ventral diaphragmatic and pericardial defects, and other congenital cardiac anomalies. Clinical cases: 4 neonates with the uncommon Cantrell's pentalogy were seen in our hospital within a 5-year period without having any apparent factor in common. This paper describes the clinical course and surgical correction. Introduction: The incomplete closure of the anterior abdominal wall affects the formation of defects such as gastroschisis. Today most surgeons use silastic sheets for the closure of large omphalocele and gastroschisis, creating an extra-abdominal reservoir. Prenatal diagnosis allows for prompt and orderly preparation for primary closure and umbilicoplastia. En our institution traditionally gastrosqusis patients were placed silastic mesh and recently has begun closing umbilicoplastia. El primary and fundamental purpose of this paper is to to the experience in our institution umbilicoplasty primary closure. Material and Methods: In a prospective study in March 2002 to April 2003, treated 16 patients with gastosquisis, all patients underwent decompression from the digestive tract by orogastric tube, enemas or irrigations with 12 Fr Levin tube before surgery, guided by hand through the handles exposed. Results: Of 16 patients treated at 13 study employed primary closure. There were four deaths, attributed the cause of death after the autopsy, sepsis in two cases, intraventricular hemorrhage in one and one at chylopericardium. Discussion and Conclusions: Decompression of the digestive tract favoring primary closure in 81.25%, in the cases not achieving primary closure was due to the saturation effect on venous return and cardiac status was improved secondary to exposure toxoinfeccioso handles, power beginning in the short term with good tolerance, improve the appearance umbilicoplasty in all patients. Objective. To know the clinical features and results of the surgical treatment the neonatal survival of infants with isolated gastroschisis and omphalocele in a general hospital. Subjects and methods. The hospital files of 14 neonates with gastroschisis and seven with omphalocele and hospitalized and treated during four years were review. Results. The birth weight of patients with gastroschisis was 2,177 ± 539 g and the other with omphalocele was 2,536 ± 469 g they have similar gestational age but eight of the newborns with gastroschisis were small for the gestational age at birth. Polyvinyl silo was installed in all of them, primary surgical closing was not realized. Auxiliary plastic patch was installed days after in three of them. Three of these neonates died (42.8%). The case of a newborn girl with diagnosis of Cantrell's syndrome done before borne, is reported. The initial management of the abdominal wall defect was to cover the thoracic ectopia cordis and another midline defects. When she had 11 days of life underwent to correct the omphalocele and diaphragmatic defect and cover the structures with latex.
