CAPITAL gains , *INFORMATION & communication technologies , *MEDICAL care , *INDUSTRY 4.0 , *SOCIAL evolution , *CHIEF information officers - Abstract
Industry 4.0, which has completely transformed the production process, is not just about smart and connected systems. While industry 4.0 includes the intertwining and fusion of digital technologies in many fields from sequencing to nanotechnology, and also includes the mutual interactions, those digital technological developments are accompanied by the neoliberal globalization process. With the practices of privatization, marketization and deregulation which are at the center of neoliberal policies, states have either relatively withdrawn from service provision or have transformed their service provision in these areas, while the process was decisive on economic, political, social, cultural and spatial transformations, Thus, areas such as health, education, etc. have been restructured by basing on market dynamics and 'health' became a means of capital accumulation. Therefore, in the process of the development of digital technologies, the globalization of capitalism and the reshaping of the state with a neoliberal approach, health care delivery should be re-examined altogether with its agents and actors. As a matter of fact, transformations are taking place in the digital hospital concept through information and communication technologies. The legitimacy of the developments produced by the transformations has the potential to produce some controversial dynamics (such as the loss of professionalism, new surveillance and control practices, violations of privacy). In this regard, it is examined in this paper that how digitalization and neoliberal capitalism transform the hospital as a space, the patient and the medical profession. Accordingly, the transformation of medical practices and the loss of professionalism, new surveillance and control practices and privacy are discussed. [ABSTRACT FROM AUTHOR]

PROGRAM transformation , *HEALTH programs , *STATISTICS , *ECONOMIC development , *MEDICAL care , *DATA envelopment analysis - Abstract
Turkey has gone through great changes with "Health Transformation Program" and made a substantial transformation. However, much statistical data have been recorded regarding the practices and behavior of hospitals in Turkey since the implementation of health transformation program. While these records pointed to various problems arising from the reforms, a methodological analysis is rarely found. Based on five years of province level health care data, this paper analyzes the productive efficiency of Turkey's health care sector in the post reform period. Efficiency measures are calculated by a nonparametric approach known as data envelopment analysis. Productivity between the years 2011 and 2015 is measured by the Malmquist index. Furthermore, in order to classify the provinces with the assessment value of 1; the super-efficiency model is applied for further sorting. According to the computed Malmquist index, results show that 51 provinces increased their productivity however 30 provinces decreased their productivity in Turkey's health care sector. On the other hand, by the evaluation of the results, it is seen that the level of economic development and the efficiency performance of hospitals could not necessarily act cooperatively. Data on health levels are the primary development indicators for countries. Efforts made by governments to improve their health systems include transformations in health services. Investments made by using information technologies are one of the methods to perform these transformations. In an effort to enhance health services and increase satisfaction in Turkey, e-prescribing system (eRx) was initiated in Turkey in 2013 as a hospital information system to enhance electronic services and to increase service quality. E-prescribing application enables pharmacies, physicians and hospitals to access patient information over system that is open to joint use without using paper. The purpose of this study is to measure the success of e-prescribing system used by pharmacy personnel. We found that the users had high satisfaction levels from this system. [ABSTRACT FROM AUTHOR]

MEDICAL care costs , *MEDICAL care , *HEALTH services administration , *DATA envelopment analysis - Abstract
Efficient use of resources plays a key role in health sector management and health care costs in Organisation of Economic Co-Operation and Development (OECD) member countries account approximately for 9.8% of their GDP. In this paper, after determining the input and output variables from an extensive literature study and using the most recent health care data available, health care efficiency of 34 OECD members are evaluated via Data Envelopment Analysis (DEA). In order to avoid any missing interpretation during the evaluation process, number of countries is initially reduced to 22 based on an elimination process by using Pierce's Criterion, and these countries are re-evaluated, 10 countries out of 34 are listed as efficient, while only seven states in 22 were found efficient in the re-evaluation process. Lastly, points of interest on reviews and prospective improvements are stated. [ABSTRACT FROM AUTHOR]

INCOME , *DECISION making , *COST , *MEDICAL care , *ORGANIZATIONAL structure , *PAYMENT - Abstract
In this paper, it is aimed to calculate the division incomes and expenses which will guide the decisions managers of the hospital enterprise take. Division incomes are separated as service, medicine and medical expenditure material, and division expenses are separated as fixed and variable. In this study 35 divisions of the medical faculty of research hospital are examined. In the end of the examination it is found that the most revenue is got by the health services provided. Fixed expenses of the divisions are found higher than variable expenses. The reason of fixed expenses' being higher is the fixed payments made to personnel. Also, medicine expenses are the most important variable expense in the division. [ABSTRACT FROM AUTHOR]

AIM: Keeping health records regularly is important for determining the health status of the population and planning health services. It is observed that the existing health record systems in Istanbul are insufficient in determining the number of pregnant women, puerperants, newborns and preschool children. METHODS: Therefore, an intervention study was planned by Istanbul Directorate of Health in order to provide systematic monitoring of pregnant women, puerperants, newborns and preschool children, and ensure that equity in the distribution of qualified prenatal and postnatal health care is maintained. This project called GEBLIZ has been started to be used since September 2008. RESULTS: According to this project all public and private health centers in which pregnant and child care are given have the responsibility to transfer necessary information about the patients to an electronic database. Through this computerized system, a connection between primary, secondary and tertiary health care settings has been maintained, and deficiencies of paper records have been completed. Health records have become more consistent. CONLUSION: Compared to one year before the start of the intervention, there have been important increases in the number of pregnant women, puerperants, newborns and infants detected by primary health care units. Besides, "home visits" which have been neglected were started actively by primary health care personnel, and preventive health service who stayed in the shadow of therapeutic health services came to life again. DISEASES , *DIAGNOSIS , *THERAPEUTICS , *PSYCHIATRY , *MEDICAL care - Abstract
Cyclothymic disorder is one of the most difficult disorder to be recognized and be detailed in the psychiatric area. Cyclothymia begins usually insidiously early ages and according to DSM-IV-TR it is a form of bipolar disorders that is characterized by frequent and short cycles of subsendromal depression and hypomania. However for diagnosis of cyclothymia, it is considered that changes in affectivity, activity and cognition is more illustrative than DSM-IV-TR criteria. Additionally because of the conceptual confusion in the definition of hypomania, some difficulties are seen in diagnosing. Different models have been suggested in the studies for the etiology of cyclothymic disorder. Recently, genetic and family studies are particulary considerable. Results of etiology*oriented studies to suggest that there are some biological associations between cydothymie disorder and bipolar disorders. Particulary, it has been suggested that cyclothymic mood would be a genetic-familial predisposition for bipolar II disorder. Cyclothymia has been researched insufficiently because of the reasons like as; early onset, the difficulties in diagnosis, variety of the clinical appearance, difficulties in the determination of its symptoms, being accepted as a border disorder and rarely seeking for the treatment. However recently, there has been an increasing number of etiology, diagnose validity and reliability-oriented studies. In Turkish literature there are few studies in which cyclothymia is discussed generally. In this paper, our aim is to evaluate and enlighten the history, epidemiology, etiology, differential diagnosis and treatment of cyclothymia in a holistic way. [ABSTRACT FROM AUTHOR]

The consequences of immobility and bed rest in the elderly are multiple, severe, quick to develop and difficult to reverse. In the elderly with limited functional reserve, immobility may quickly lead to impairments in activities of daily life and disability. Misconceptions on indications and health consequences of bed rest are not uncommon both among health care professionals and in the society. Unfortunately our knowledge on health effects of bed rest as a treatment modality are based on a limited number of studies and there is a paucity of emphasis on the topic in the classical health care curriculum. However, health care workers need to be well-aware of diverse consequences of bed-rest and immobility so that they can actively participate in prevention and education of others. In this paper following a case presentation, we review consequences of bed rest and immobility and emphasize some important aspects in diagnosing the underlying problems, prevention and treatment of immobility. EVIDENCE-based medicine , *DECISION making in clinical medicine , *HEALTH outcome assessment , *MEDICAL care , *PATIENTS - Abstract
Today, evidence-based practice, its concepts, methods and implementation is perhaps the most discussed development in both the medical and political world. It aims to optimize health care outcomes and benefit the patient by integrating the best external evidence, clinical expertise and patient outcomes. It is rapidly raising value in health care. However, there is concern about the methods employed for designing best evidence, with evidence-based practice having ever greater resource implications for institutions. This paper examines what evidence-based practice is, what its origins and historical background are, what its advantages and challenges are and what are its current criticisms. [ABSTRACT FROM AUTHOR]
