Anterolateral uyluk flebinin bölünüp, şimerik paternde geniş doku defektlerinin rekonstrüksiyonu son yıllarda popülerlik kazanmıştır. Olgumuzda 55 yaşında kadın hasta alt ekstremitesinde mevcut geniş açık yara ile kliniğimize başvurdu. Düzgün sınırları olmayan açık yara için, defekte uygun Anterolateral uyluk flebi dizayn edilerek bölündü. İyileşme sürecinde hastada herhangi bir problem yaşanmadı. Tatmin edici kozmetik ve fonksiyonel sonuç elde edildi. The reconstruction of large tissue defects by splitting the ALT flap and using the flap in a chimeric fashion become popular in recent years. A 55-year-old female with an open would in the lower extremity referred to our clinic. An ALT flap was designed and splitted for irregularly shaped open wound. The patient had an uneventful recovery with a satisfactory cosmetic and functional outcome.

Çalışmanın amacı; 4 yıllık bir süre boyunca ayak bileği bölgesinde meydana gelmiş kompleks yaraların tedavi süreci ve komplikasyonları ile ilgili faktörlerin analizidir. Mevcut retrospektif çalışma, Ocak 2011- Ocak 2015 yılları arasında ayak bileği defektlerinin serbest fleplerle onarımı yapılmış 20 hastayı kapsamaktadır. Bütün vakalar demografik yapı, oluş mekanizması, yerleşim, eşlik eden yaralanmalar, flep seçimi, flep boyutu, alıcı vasküler yapılar, anostomoz sayısı, hastanede yatış süresi ve post operatif komplikasyonlar açısından analiz edilmiştir. Çalışmamızda ayak bileği defektleri için 20 hastaya toplam 22 serbest flep yapılmıştır. 6 hastada anterolateral uyluk flebi, 6 hastaya lateral kol flebi, 4 hastaya latissimus dorsi kas deri flebi, 3 hastada radial ön kol flebi ve 1 hastada yüzeyel femoral arter perforatör serbest flebi kullanılmıştır. Vakalarda başarı oranının %85 olduğu görülmüştür. Alt ekstremite rekonstrüksiyonunda birincil amaç fonksiyonun geri kazanılması ve korunmasıdır. Uygun tedavi planının yapılması hayatidir ve bu tip yaralanmalarda serbest flep ile rekonstrüksiyon en ideal tercihtir. The purpose of this study is to analyze a 4-year experience of managing complex wounds in the ankle region focusing on factors associated with complications. The present retrospective study included 20 patients treated for defects in the ankle area with free flaps from January 2011 to January 2015. All cases were analyzed for the demographics, mechanism and location of the lower extremity injury, collateral injuries, flap selection, flap size, recipient vessels, number of anastomosis, hospital stay, postoperative complications. A total of 22 free flaps performed in 20 patients for tissue defects in the ankle region. There were 6 patients who underwent lower extremity reconstruction using an anterolateral thigh flap and 6 patients with a lateral arm free flap, 4 with a latissimus dorsi muscle-skin flap, 3 with a radial forearm free flap and one with superficial femoral artery perforator free flap. Our success rate in lower extremity reconstruction with free flaps was %85.The primary goal of lower extremity reconstruction is to recover function and maintain it. It is vital to make the appropriate treatment plan, and all this suggests that free flap would be the best treatment for patients with severe injuries to the lower extremities.

Çalışmanın amacı; kliniğimizde Ocak 2014 – Ocak 2015 tarihleri arasında gerçekleştirilen 16 Anterolateral uyluk (ALT) flebi vakasının retrospektif olarak incelenmesidir. Çalışmamızda 4’ü kadın, 12’si erkek hasta olmak üzere toplam 16 hasta değerlendirildi. Mevcut defektlerin rekonstrüksiyonunda Anterolateral uyluk flebinin,13 olguda serbest stil, 3 olguda pediküllü olarak uygulanmış olduğu görüldü. Serbest stil anterolateral uyluk flebinin; baş-boyun bölgesi defektleri ve alt ekstremite doku defektleri rekonstrüksiyonu için kullanıldığı görüldü. Uyluk bölgesi, trokanterik bölge ve perine gibi yakın bölgelerde ise pediküllü flebin tercih edilmiş olduğu görüldü. Anterolateral uyluk flebi serbest ve pediküllü olarak yumuşak doku rekonstrüksiyonunda önemli bir seçenek haline gelmiştir. Minimal donör alan morbiditesi yaratarak büyük defektlerin rekonstrüksiyonuna olanak sağlamaktadır. The purpose of our study is to evaluate 16 anterolateral thigh flap (ALT flap) cases performed between January 2014 and January 2015. 16 patients (4 female/ 12 male) were included in our study. Out of 16 cases, ALT flap was performed as a free flap in 13 cases, and was performed as a pedicled flap in 3 cases. It was seen that the free ALT flap was used especially for head-neck and lower extremity reconstcutions whereas the pedicled ALT flap was used for thigh, trochanteric and perineal region reconstructions. It was observed that the ALT flap has become an important choice for soft tissue reconstruction. It provides the reconstruction of large tissue defects with minimum donor site morbidity.
