Amaç: Çalışmamızda abdomen bölgesinde bulunan yapı ve organların çoğunun arteriel vaskülarizasyonunu sağlayan ve aorta abdominalis'in en önemli tek dalları olan; truncus coeliacus, arteria mesenterica superior, arteria mesenterica inferior varyasyonlarının ve aralarında bulunan mesafe ölçümlerinin belirlenmesi amaçlandı.Gereç ve Yöntem: Çalışmada; 2016 ile 2017 yılları arasında Dicle Üniversitesi Tıp Fakültesi'nde multidedektör bilgisayarlı tomografi tekniği ile elde edilmiş 261 (115 kadın, 146 erkek) hastanın görüntüleri retrospektif olarak incelendi. Çalışmaya 16-83 yaş aralığı dahil edildi. Çalışma sonucunda elde edilen veriler SPSS programında istatistiksel olarak değerlendirildi. Truncus coeliacus, arteria mesenterica superior ve arteria mesenterica inferior'da saptanan varyasyonlar; orjin noktasındaki varyasyonlar, ana dallarındaki varyasyonlar ve bu arterlerin ne çeşit dallanma gösterdiklerine yönelik varyasyonlar olmak üzere üç kategoride yapıldı.Bulgular: Truncus coeliacus ve ana dallarının oldukça farklı varyasyon tiplerine sahip olduğu anlaşıldı. Truncus coeliacus'un en fazla varyasyon gösteren dalı arteria gastrica sinistra olarak, en az varyasyon gösteren dalı ise arteria splenica (lienalis) olarak saptandı. Arteria mesenterica superior ve arteria mesenterica inferior'un truncus coeliacus'a göre daha az varyasyon gösterdiği belirlendi.Sonuç: Truncus coeliacus, arteria mesenterica superior ve arteria mesenterica inferior varyasyonlarının bilinerek yapılan abdominal teşhis ve tedavi girişimlerinin daha başarılı olacağı kanaatine varıldı.Anahtar kelimeler: Anatomik varyasyonlar, truncus coeliacus, arteria mesenterica superior, arteria mesenterica inferior, MDBT Aim: In this study, it was aimed to determine the variations and distance measurements between the coeliac trunk, superior mesenteric artery and inferior mesenteric artery which provide great deal of arterial vascularization for structures and organs in abdomen visceral and which are the most important single branches of aorta abdominals. Material and Method: In the study, the images of the 261 patients (115 female, 146 male), obtained by multidetector computed tomography technique in the Dicle University Medical Faculty, were retrospectively analysed between 2016 and 2017. The study included 16-83 age range. The data obtained from the study were statistically evaluated via SPSS program. The coeliac trunk, superior mesenteric artery and inferior mesenteric artery were evaluated in three categories, such as variations at the origin, variations in the main branches, and branching variations of these arteries.Results: The coeliac trunk and its main branches were found to have quite different variation types. It was revealed that the most variant branch of coeliac trunk was left gastric artery, while the least was the splenic artery. It was determined that the superior mesenteric artery and inferior mesenteric artery displayed lesser variation than coeliac trunk.Conclusion: It was concluded that the variations of the coeliac trunk, superior mesenteric artery and inferior mesenteric artery will be more successful in abdominal diagnosis and treatment attempts carried out by knowing how to do.Key Words: Anatomical variations, coeliac trunk, inferior mesenteric artery,superior mesenteric artery, MDCT 112

Normalde truncus coeliacus’un üç dalından biri olan arteria hepatica communis, duedonum’un birinci kısmının üst yüzünde arteria hepatica propria ve arteria gastroduedonalis dallarını verir. Arteria renalis ise arteria mesenterica superior’un hemen aşağısında, aorta’nın yan taraflarından dik açı ile ayrılan iki adet kalın arterdir. Mide fundusu adeno karsinomu şüphesi ile takip edilen 64 yaşındaki erkek hastaya çekilen Spiral Bilgisayarlı Üst Abdominal BT’sinde a. hepatica dextra, varyasyonel olarak a. mesenterica superior’un proksimalinden, arteria hepatica sinistra ise truncus coeliacus’tan orjin almaktaydı. Olgumuzda çift arteria renalis sinistra varyasyonu da izlendi. Hem arteria renalis hem de arteria hepatica’lardaki varyasyonlar, giderek artan girişimsel radyolojik işlemler, transplantasyon ve vasküler cerrahi işlemler nedeniyle daha fazla önem kazanmaktadır. Bu yüzden hastamızdaki gibi mide karsinomu olup gastrektomi ve lenfadenektomi planlanan olgularda aorta abdominalis’ten ayrılan arterlerin anatomisi ve varyasyonlarının iyi bilinmesi gereklidir. Common hepatic artery, normally is one of the three branches of coeliac trunk, gives off gastroduedonal artery and proper hepatic artery at the upper face of the first part of the duedonum. Renal arteries are two thick arteries which are located just below the mesenteric superior artery and separated by a right angle to the sides of the aorta. 64 year-old male patient with suspected adenocarcinoma of the fundus of the stomach was taken spiral computed upper abdominal CT. Right hepatic artery as a variational is originated from the superior mesenteric artery proximally and left hepatic artery is originated from coeliac trunk. In our case double left renal arteries were observed. Due to growing interventional radiological procedures, transplantation and vascular surgical procedures, variations of the renal arteries and the hepatic arteries are becoming more and more important. This is why patients who undergoing gastrectomy and lymphadenectomy in gastric carcinoma like our patient, a good knowledge of the anatomy and variations of the arteries leaving the abdominal aorta is required.
