ÖZETÇalışmanın amacı ratlarda böbrek iskemi/reperfüzyon hasarında, ELABELA, Apelin 13, Sistatin-C ve Nitrikoksit düzeylerine bakmak, karşılaştırmak ve N-Asetilsistein in uygulamasının iskemi/reperfüzyon üzerine olan etkilerini gerek histolojik olarak gerekse adı geçen moleküller üzerinden belirlemektir.Mevcut çalışma Fırat Üniversitesi Deneysel Araştırmalar Merkezi'nden temin edilen 8 haftalık 68 adet erkek Wistar-albino türü erkek sıçan üzerinde Fırat Üniversitesi Deneysel Araştırmalar Merkezi (FÜDAM) ve Fırat Üniversitesi Tıp fakültesi Histoloji ve Embriyoloji Anabilim Dalı Laboratuvarında gerçekleştirildi. Denekler randomize olarak 7 gruba ayrıldı. Grup 1 (Kontrol grubu) (n=8), Grup 2 (Sham GRUBU) (n=10), Grup 3 (Tek Taraflı Renal İR grubu) (n=10), Grup 4 (Bilateral Renal İR grubu) (n=10), Grup 5 (Tek Taraflı Renal İR eşliğinde NAC grubu) (n=10), Grup 6 (Bilateral Renal İR eşliğinde NAC grubu) (n=10), Grup 7 (NAC Grubu) (n=10) olarak belirlendi.Gruplar incelendiğinde Kontrol, Sham ve NAC grubu arasında biyokimyasal ve histopatolojik açıdan anlamlı farklılık izlenmedi. Üre ve kreatinin değerleri Bilateral Renal İR grubu ve 6 da kontrol grubuna göre artmış olarak tespit edildi. Tek Taraflı Renal İR grubu'nda ise kontrol grubuna göre üre değerinde anlamlı artış tespit edilmemişken, kreatinin düzeyinde anlamlı artış tespit edildi. Bilateral Renal İR grubu'de ise üre ve kreatinin düzeylerinde Tek Taraflı Renal İR grubuna göre istatistiksel olarak anlamlı artış tespit edildi. Tek Taraflı Renal İR grubu ile Tek Taraflı Renal İR eşliğinde NAC grubu arasında üre ve kreatinin arasında anlamlı farklılık tespit edilmedi. Bilateral Renal İR eşliğinde NAC grubunda ise Bilateral Renal İR grubu'e göre üre açısından anlamlı farklılık tespit edilmemiş olup, kreatinin değerinde istatistiksel olarak anlamlı azalma tespit edilmiştir. Kontrol grubuna göre, Tek Taraflı Renal İR grubu, Bilateral Renal İR grubu, Tek Taraflı Renal İR eşliğinde NAC grubu ve Bilateral Renal İR eşliğinde NAC grubu'da anlamlı olarak TAS düzeylerinde azalma ve TOS düzeylerinde artış tespit edildi. Bilateral Renal İR grubu'de Tek Taraflı Renal İR grubuna göre TAS, ELA, Apelin-13 istatistiksel olarak anlamlı şekilde azalırken; TOS, Sistatin-C ve İNOS seviyelerinde anlamlı artış tespit edilmiştir. Tek Taraflı Renal İR eşliğinde NAC grubu'de ise Tek Taraflı Renal İR grubu'e göre TAS, ELA ve Apelin-13 seviyelerinde istatistiksel olarak anlamlı artış tespit edildi. Grup6'da Bilateral Renal İR grubu'e göre kreatinin, TOS, Sistatin-C ve İNOS'da anlamlı azalma; TAS, ELA ve Apelin-13'de anlamlı artış tespit edildi.Sonuç olarak bu çalışmada, ELA ve Apelin-13 ün iskemiye sekonder azaldıkları, İNOS ve Sistatin-C'nin ise anlamlı olarak arttığı görüldü. Üre ve kreatinin iskemi reperfüzyon hasarında bu moleküller kadar spesifik olmadığı değerlendirildi. N-asetilsistein tedavisi sonrası gerek doku da gerekse serum örneklerinde iyileşmeyi müteakiben ELA ve Apelin-13 artışı ile İNOS ve Sistatin-C'nin azaldığı tespit edildi. Bu moleküllerden özellikle Apelin-13 ve ELA'nın tedavide de kullanıldığı çalışmalar yapılmakta olup, ilerleyen dönemlerde bu çalışmaların artışı renal iskemi ve reperfüzyon hasarlarının erken dönemde tespitine ve tedavisine imkan sağlamasına ışık tutacağı düşünülmektedir.Anahtar kelimeler: Renal iskemi reperfüzyon, Elabela, apelin, sistatin-c, İNOS ABSTRACTIN EXPERIMENTAL KIDNEY ISCHEMIA REPERFUSION INJURY ON RATS ELABELA, APELIN-13, CYSTATIN-C AND NITRIC OXIDE LEVELS AND THE PROTECTİVE EFFECT OF N-ACETYLCYSTEINEThe aim of this study was to look at and compare the levels of ELABELA, Apelin 13, Cystatin-C and Nitricoxide in renal ischemia / reperfusion injury in rats and to determine the effects of N-Acetylcysteine on ischemia / reperfusion by histologically and by the mentioned molecules.The present study was performed on 8 weeks 68 male Wistar-albino male rats obtained from Firat University Experimental Research Center in Firat University Experimental Research Center (FÜDAM) and Firat University Faculty of Medicine Histology Embryology Department Laboratory.The subjects were randomly divided into 7 groups. Groups are determined as Group 1 (control group) (n = 8), Group 2 (Sham Group) (n = 10), Group 3 (Unilateral Renal I / R group) (n = 10), Group 4 (Bilateral renal I / R group) (n = 10), Group 5 (NAC group with Unilateral Renal I / R) (n = 10), Group 6 (NAC group with Bilateral Renal I / R) (n = 10), Group 7 (NAC Group) (n = 10).When the groups were examined, no significant difference was observed between Control, Sham and Group 7 in terms of biochemical and histopathological features. Urea and creatinine levels were found to be increased in Group 4 and 6 compared to the control group. In group 3, while there was no significant increase in urea but a significant increase in creatinine level was detected according to the control group. In group 4, urea and creatinine levels were significantly higher than Group 3. No significant difference was found between Group3 and Group 5 according to urea and creatinine levels. In Group 6, no significant difference was found in urea and a statistically significant decrease in creatinine value was determined according to Group 4.According to the control group, significant decrease in TAS levels and increase in TOS levels were found in Group 3, Group 4, Group 5 and Group 6. In Group 4, TAS, ELA and Apelin-13 decreased significantly compared to Group 3; while significant increase in TOS, Cystatin-C and INOS levels were detected. In Group 5, there was a statistically significant increase in TAS, ELA and Apelin-13 levels according to Group 3. In Group 6 according to Group 4 significant reduction in creatinine, TOS, cystatin-C and INOS; significant increase in TAS, ELA and Apelin-13 was found.As a result, in this study, it was seen that ELA and Apelin-13 decreased and significantly increased INOS and Cystatin-C secondary to ischemia. Urea and creatinine were evaluated as not as specific as this molecüles in ischemia reperfusion injury. After N-acetylcysteine treatment, both INOS and Cystatin-C decreased with the increase in ELA and Apelin-13 following improvement in both tissue and serum samples. Studies of these molecules, especially Apelin-13 and ELA, are used in the treatment, and it is thought that the increase in these studies will shed light on the early detection and treatment of renal ischemia and reperfusion injury.Key words: Renal ischemia reperfusion, Elabela, apelin, cystatin-c, INOS 90
