Introduction: In this study, we aimed to evaluate the development of 0-6 year old children with Denver II and to examine the factors affecting the results. Materials and Methods: The population of the descriptive and retrospective, in which the surveillance data were analyzed, consisted of 391 children who applied to healthy life center of district health directorate. SPSS 22.0 package program was used to analyze the data. The data was examined by using ANOVA, Kruskal- Wallis, Fisher's exact test and Pearson chi-square, p<0.05 was considered as statistically significant. Results: 49.9% of the participants in our study were girl, average age was 3.0±1.4 year and 100% was between 0-6 age. As a result of the study, 241 (61.6%) out of 391 children identified with DDST II have been found to be normal, 53 (13.6%) of them suspicious and 97 (24.8%) children are found to be abnormal. A statistically significant difference was found between the decrease in the education level of the parents, the fact that the mother was a housewife and the child's gender was male and the probability of abnormal DDST II results (p<0.05). Conclusion: In the study, it was determined that almost one fourth of the children showed abnormal development. Introduction: Despite advances in obstetric care, the prematurity rate is increasing all over the world with the use of assisted reproductive techniques. However, the survival rates of these babies have increased thanks to the developments in neonatal and perinatal care. The specific problems and complications seen in early prematurity, determine the long-term neurodevelopmental outcome. One of the most important determinants of neurodevelopmental outcome is intraventricular hemorrhage (IVH). In this study, risk factors that could cause IVH and severe IVH were investigated in premature infants. Materials and Methods: In this study, inpatient files in the neonatal intensive care unit of our hospital in a 10-year period were analyzed retrospectively. As the patient group, 40 patients with a gestational age below 37 weeks and diagnosed with IVH; the control group was selected from patients who were hospitalized on similar dates, did not have IVH, and whose body weight and gestational age were similar to the IVH group. Results: Prolonged mechanical ventilation, hypoxemia, hypercarbia and recurrent hypercarbia attacks, hypotension, recurrent surfactant need, erythrocyte transfusion, bicarbonate therapy, use of volume expander and pregnancy with assisted reproductive techniques were found to be risk factors for IVH. Mortality rate was significantly higher in IVH group and severe IVH (stage III-IV bleeding) group compared to the control group. In IVH group, periventricular leukomalacia was observed in 4 patients and hydrocephalus in 10 patients. Conclusion: IVH is a complication of prematurity that increases long-term morbidity and mortality. Use of blood transfusions, volume expanders and bicarbonate therapy which are identified as risk factors in this study should be minimized in neonatal intensive care. Introduction: Though predicting the emerging hyperbiluribinemia is the rational approach, timely exchange transfusion in significant neonatal hyperbilirubinemia is vital. The aim of this study is to evaluate the socio-demographic and clinical characteristics of newborns with significant hyperbilirubinemia, to evaluate risk factors for exchange transfusion and long-term neurodevelopmental status. Materials and Methods: Newborns who were admitted with bilirubin levels above the excahge transfusion thresholds (American Academy of Pediatrics, guideline 2004) total 104 cases were enrolled to the study. A total of 65 cases at 12-36 months were evaluated with Guide for Monitoring Child Development. The clinical and demographic characteristics, risk factors, treatment modalities, etiology of the groups and long-term neurodevelopmental status were compared between the groups with and without exchange transfusion, Results: Exchange transfusion was performed in 19 (18.3%) patients. Main factors that increase the risk of exchange transfusion were bilirubin level and bilirubin/albumin ratio. Cut-off bilirubin level and bilirubin/albumin ratio which increase the risk for exchage transfusion were 26.43mg/dl and 7.43mg/g respectively. The underlying etiologies were hemolytic disease (29.8%), dehydration and breast milk jaundice (28.8%), prematurity (26.9%). Late comers (postnatal >96 hours) were more likely to have bilirubin level ≥25 mg/dl. Development delays were detected in 13.8% of 65 cases. Conclusions: Admission bilirubin level and the ratio of bilirubin to albumin was among the factors that increase the risk of exchange transfusion. Late admission with significant hyperbiluribinemia may be prevented by identification of neonates at risk for developing significant hyperbilirubinemia before discharge or early in follow-up. Aim: Breast milk is the most important component of nutrition in terms of health and development of infants and children. The aim of this study is to determine the level of knowledge of breast milk and breastfeeding of mothers who gave birth in Düzce University Research and Education Hospital. Material and Method: The cross-sectional study was conducted with 137 mothers in the time of December 1, 2018 and December 31, 2018 at Düzce University Research and Education Hospital. The entire population was targeted and 80 (58.3%) mothers were reached. The data collection tool consisted of 'Information and Attitudes on Breastfeeding and Human Milk Questionnaire' which was developed by the researchers and composed of 27 questions and LATCH Breastfeeding Assesment Tool (LATCH). Data was evaluated with with SPSS for Windows 22.0 package program. Student-t test, ANOVA, Mann-Whitney U, Kruskall-Wallis, Pearson Correlation Analysis, linear regression analysis were used in the data assessment. Results: The mean age of the mothers participating in the study was 28.30±5.22. It was determined that 98.8% of the mothers knew that breastmilk was the ideal nutrient for the baby and 72.5% did not know that breastfeeding immediately after birth reduces postpartum hemorrhage. The mean LATCH scale score of the mothers was 6.64±1.91, and it was found that the scores taken from the scale showed significant differences with previous live birth, breastfeeding experience, type of delivery and receiving anesthesia at delivery (p=0.01), (p<0.001), (p=0.01), (p=0.01). In multivariate analysis, it was found that the experience of breastfeeding (ß=1.297 p=0.003) and the normal way of delivery (ß=0.919 p=0.043) increased breastfeeding success. Conclusion: It is thought that raising the awareness of mothers that they should avoid non-indication C-section is important in order to increase breastfeeding success. [ABSTRACT FROM AUTHOR]

Germinal matrix-intraventricular hemorrhage (GM-IVH) is a major complication of prematurity and inversely associated with gestational age and birth weight. The hemorrhage originates from the germinal matrix with an immature capillary bed where vascularization is intense and active cell proliferation is high. It occurs in around 20% of very low-birth-weight preterm neonates. Germinal matrix-intraventricular hemorrhage is less common in females, the black race, and with antenatal steroid use, but is more common in the presence of mechanical ventilation, respiratory distress, pulmonary bleeding, pneumothorax, chorioamnionitis, asphyxia, and sepsis. Ultrasonography is the diagnostic tool of choice for intraventricular hemorrhage and its complications. Approximately 25-50% of the germinal matrix-intraventricular hemorrhage cases are asymptomatic and diagnosed during routine screening. These cases are usually patients with low-grade hemorrhage. Neurologic findings are prominent in severe intraventricular hemorrhage cases. The major complications of the germinal matrix-intraventricular hemorrhage in preterm babies are periventricular hemorrhagic infarction, posthemorrhagic ventricular dilatation, periventricular leukomalacia, and cerebellar hemorrhage. It is an important cause of mortality and morbidity. The management of hemodynamics and ventilation of patients, appropriate follow-up, and early diagnosis and treatment can minimize morbidity. Prognosis in intraventricular hemorrhage is related to the severity of bleeding, parenchymal damage, and the presence of seizures and shunt surgery. The main determinant of prognosis is periventricular hemorrhagic infarction and its severity. Moderate-severe intraventricular hemorrhage can cause posthemorrhagic hydrocephalus, cerebral palsy, and mental retardation. Even mild germinal matrix-intraventricular hemorrhage can result in developmental disorders. Long-term problems such as neurodevelopmental disorders and cerebral palsy are as important as short-term problems. Improving the quality of life of these babies should be aimed through appropriate treatment and follow-up. In this review, intraventricular hemorrhage and complications are discussed. Objective: This study was planned to evaluate the growth-development and nutritional status of infants aged 6-12 months. Methods: The study included 300 mother-infant pairs in Çekmeköy, Istanbul, between June and September 2014. Information on 300 babies and their families who participated in the study were taken by a questionnaire, one-day nutritional expenditures of the babies were recorded, and weight and height lengths were measured by nurses. The weight and height of infants were evaluated according to a Z score and growth curves for our country; energy and nutrient intake were evaluated with TUBER. Results: Of the infants, 45.3% were female, and their average age was 10.1±1.9 months. The weight and height of infants at birth were 3.2±0.4 kg, 49.8±2.4 cm and current weight and height were from 9.3±1.5 kg and 72.4±5.9 cm, respectively. Body weight, height and BMI ratios of infants with -1 and +1 SD Z scores at birth were 69.0%, 47.4%, 52.9% and current values were 59.1%, 44.9% and 42.4% respectively. The increase in body weight and BMI were found to be statistically significant (p>0.05). It was observed that mothers preferred yogurt (33.1%), pudding (15.7%), fruit puree (13.4%), baby biscuits (8.4%) and eggs (6.7%) as the first complementary foods. It was determined that yogurt was started at week 24.7±4.6; fruits and vegetables at 24.9±5.3; eggs at 26.6±6.1; meat and poultry at week 29.6±8.3.It was observed that infants received 970.6±274.3 kcal/day of energy and 27.8±12.0 g/day of protein. All energy and nutrients except iron were consumed well enough an even more. Discussion: The study showed that 6-12 month babies overconsume many nutrients including daily energy and BMIs were found to be higher when compared with their birth. The first 3 years age, is called as infancy period, is a very important period for learning ability and basis for the skills concerning the developmental stages. Gross motor, fine motor, cognitive, social, language and oral motor development are examined as developmental parameters and occur according to a typical sequence, pattern and timing. Each stage of the motor developmental sequence has purpose and contribute to the overall development of the child. Reflexes are seen in the first stage of development and these reflexes are replaced by reactions over time. The motor developmental sequence offers a basis for understanding not only typical development, but also atypical development of the infant, this developmental sequence could be used as a basis for evaluating and treating motor delays and deficiencies in infants. Investigation of normal developmental milestones in infancy as a whole in terms of motor, cognitive, social, language and oral motor are important for early diagnosis and intervention of deviations from normal/typical development. Atypical development in one or more of these parameters is a sign of risk for other parameters. Therefore, the finding of atypical development seen in these parameters is critical in terms of evaluating the infant as a whole and referring it to the relevant health professional when necessary. Aim: To investigate the relationship between motor and sensory development in the first 4 months of life in preterm infants born at 32 gestational weeks and below. Material and Methods: The study consisted of 56 high-risk infants with a corrected age of 1 month who were born at 32 gestational weeks and stayed in the neonatal intensive care unit for at least 15 days. Neuro Sensory Motor Developmental Assessment and Infant Sensory Profile-2 were used for evaluation. These assessments were applied to preterm infants at the 1st and 4th months. The results of assessments were analyzed using the Wilcoxon test. The relationship between the results of motor and sensory assessments was analyzed using Spearman's correlation test. Results: The mean gestational age of the infants was 29.58±2.09 weeks, their birth weights were 1233.87±251.22 grams, and their duration of stay in the neonatal intensive care unit was 26.48±9.58 days. There was a statistically significant difference between the N euro Sensory Motor Developmental Assessment and Infant Sensory Profile-2 scores between the 1st and 4th months (p<0.05). It was found that there was a risk in terms of sensory development in 86-91% of the preterm infants at the 1st month and in 69-85% at the 4th month. There was moderate-strong degree of significant relationship between motor and sensory development. Conclusion: Considering the findings of our study, preterm infants are at risk for motor and sensory development. Teething is a physiological process that all children experience. Even though this period is a normal part of babies development, interestingly little is known about management of symptoms. Dry cough, crabbing, sleeping disorder, gingival inflammation, and salivation are among the most common symptoms observed during milk dentition. However, even though fever, diarrhea, and several numbers of other complaints are not adequate evidences, teething period is commonly considered responsible. Although pain and discomfort related to baby teething can be managed by using pharmacological and non-pharmacological methods, inadequate evidence regarding these draws attention. The main function of fats as essential part of the diet is known to provide energy. However, in recent years, fats have been determined to have other important functions, particularly in infancy. While amount and calorie intake/weight gain constituted the focus of previous studies, recent studies concerning the content and properties of fats have showed the impact of fat quality on growth and development as well as long-term health outcome. Due to the fact that palmitic acid content of palm oil used in infant formulas structurally differs from the human milk content of palmitic acid, infant formulas containing palm oil may lead to digestion and absorption problems among infants. These problems may be mainly as follows: a) reduction in fat absorption and calorie intake; b) adverse effects on stool consistency and frequency; c) occurrence of digestive problems such as colic and regurgitation; d) adverse effects on bone mineralization and bone mass due to reduction in calcium absorption. Identifying developmental disorders in early childhood can lead to timely intervention and better outcomes. The aim of this study was to develop a new screening instrument for the infancy period (the Social-Communication Area Screening Test for Infants - SCASI). The research study was carried out at the Istanbul University School of Medicine. A total of 432 children aged between 6 and 24 months and their families constituted the study group. The SCASI is a parent-reported test with 43 items and a two-factor structure. It has strong internal consistency; α is 0.962, and 0.961 and 0.811 for factors I and II, respectively. Overall, 84 out of 310 children (28.2%) in the normative sample were identified as children "at risk" for developmental disorders. The SCASI is believed to be a promising instrument for both pediatricians and other clinicians to identify children at risk for developmental disorders in infancy, and plan further intervention programs. Although breast-milk is the most essential nutrient required for the growth and development of infants, there are still some problems related to breastfeeding in our country. In this study, we sought to evaluate the knowledge and practices of mothers from different socioeconomic groups living in Istanbul about infant feeding. The study included 202 mothers from three different socioeconomic groups, and a 42-item questionnaire was completed during face-to-face interviews. Mothers from median and upper socioeconomic groups exclusively breastfed their babies longer than the others. The mothers from the low socioeconomic group had received information about infant feeding mainly from their parents and neighbors, while the other two groups had received information from healthcare workers. Mothers from the low socioeconomic group had mainly used vitamin D alone, while the others had used multivitamins. We concluded that the income and education level of mothers significantly affect food content and the starting time of supplementary foods. [ABSTRACT FROM AUTHOR]

Healthy attachment is a key concept in creating a positive environment for the development of the infant after birth. The earlier and the healthier the mother creates a communication and interaction with her baby, a stronger maternal-infant attachment will be developed. The time period immediately after birth is the most convenient time for the initiation of positive mother-infant relationship as it is the most intense period of attachment. The aim of this article was to define mother-infant attachment and to examine the evidence-based practices for midwives and nurses who play a role in the development of this attachment in the early postpartum period. Objectives: Breastfeeding is the ideal nutrition method for healthy growth and development of infants. In this retrospective study it was aimed to investigate the socio-demographic factors affecting the breastfeeding status in the neonatal period. Material and Methods: The study was conducted on 121 babies, who regularly attended the "Well Baby Clinic", and records of the children were reviewed retrospectively. The sociodemographic characteristics of parents, duration of gestation, birth weight of children, and feeding regimen on the 15th days were recorded. All statistical data were analyzed using SPSS 13.0 for Windows program. Statistical significance was defined as p<0.05. Results: Of the infants who were included in the study, 49.6% were female (60), and 50.4% (61) were male. The mean age of the mothers and fathers was 27.5±5.1 (17-40) and 32.1±5.5 (23-51) years, respectively. Sixty-three of the children (77) were born by cesarean section. Of the infants 79.3% (96) were given exclusively breast milk, whereas 20.7% (25) were given formula with breast milk. The exclusive breastfeeding rate was lower among mothers who had babies less than 2500 g (p=0.010). Duration of pregnancy was shorter in "partially-breastfed" babies (p=0.002). There was no statistically significant relation between sociodemographic factors and breastfeeding status (p>0.05). Conclusion: These data suggest that breast-feeding initiation in the neonatal period is affected by the factors such as duration of pregnancy and birth weight. It is concluded that to prevent premature births and determine the causes of low birth weight and taking the necessary measures may play an important role in increasing exclusively breastfeeding in the neonatal period. Breastmilk feeding provides growth, physical and mental development of babies; therefore it is important to feed newborns with breastmilk that contains sufficient amount of nutrients. Nevertheless, newborns that cannot be fed with breastmilk are generally fed with cow milk protein based formulae. However some babies suffering from lactose intolerance, protein intolerance and galactosemia cannot consume these cow milk protein based formulae. Soy protein based formulae, several protein hydrolysates or amino acid derived formulae are prepared for these babies. Besides, locally available food sources, such as maize, lupin, cowpea, chickpea, sorghum, are gaining attraction to produce low-cost infant foods. In this review, brief information about protein sources used in infant foods was given and methods developed to produce formulations were discussed. Objectives: The aim of this study was to investigate the effects of different feeding type on the physical growth of infants in the first 12 months of life. Materials and methods: Seventy-five infants who visited the children outpatient clinics or/and vaccination clinics regularly were recruited for the study. They were classified into breast feeding group (N=28, 14 male and 14 female), Formula feeding group (N=22, 11 male, 11 female), and mixed feeding group (N=25, 14 male, 11 female) according to the feedings types before 6-months-old. The growth indices were measured and standardized growth charts for Turkish children were used for comparison. Results: The body weight and lengths of infants in the all three groups showed no statistical difference for the first 4 months according to the feeding types (p>0.05). However, growth of the infants with formula feeding and mixed feeding began to exceed the breast feed group gradually from the 4 months on. The differences of weight are statistically significant from 6-12 month and 4-12 month respectively, compared with breast feeding group (p<0.05). Conclusion: The infants with mixed or formula feeding are exposed a higher risk of overweight between 6-12 months of the first year.
