Intra-articular ankle injuries to the talar body is one of the common cause of athletic disability that have been estimated to occur in approximately 6.5% of the ankle sprains and commonly affect the dome of the talus. Patients are commonly referred to the clinics with an acute injury to the ankle or with chronic ankle instability. This article describes the clinical presentation and imaging to diagnose of talar osteochondral defects. The treatment options are also reviewed to give consensus on optimal treatment of this condition. Bu çalışma ile eklem yüzlerinde oluşan osteochondral defektlerin, özellikle Osteochondritis Dissecans 'ta oluşan defektin Methylmethacrylate (Acrylic bone cement) ile doldurularak, eklem yüzünün normal yapısının kazandırılması amaçlandı. Deneysel çalışma materyali olarak klinikman sağlıklı 10 köpek kullanıldı. Osteochondral defekt el matkabı kullanılarak femur trochleo patellaris'i lateral çıkıntısı üzerinde, 0.5 cm çapında ve 1.0 cm derinlikte oluşturuldu. Daha sonra hamur kıvamında hazırlanan akrilik kemik çimentosu, sement bıçağı ve kaşığı kullanılarak defekte dolduruldu ve eklem yüzüne normal şekli verildi. Bütün olguların 2, 3, 4, 5, 6, 7, 8, 10 ve 14'üncü haftalarda klinik ve radyolojik kontrolleri yapıldı. Radyolojik muayeneler AP ve LM pozisyonlarda yapılan radyografilerle gerçekleştirildi. Postoperatif 3, 4, 5, 7, 10 ve 14'üncü haftalarda köpekler ötanezi yapılarak, ilgili eklemler histopatolojik muayene için laboratuvara gönderildi. Klinik olarak tüm eklemlerdeyangısel bozukluklara rastlanmadı. Birinci hafta sonunda eklem hareketleri ve ağırlık verilmesi normal bulundu ve ekstremitenin yürüyüş fonksiyonuna katkısı normal olarak gözlendi. Radyolojik olarak dejeneratif ve yangısel eklem bozukluklarına, atropati ve osteofit oluşumuna rastlanmadı. Histopatolojik bulgular klinik ve radyolojik bulguları destekler bulundu. Yalnız bir olguda 14'üncü haftada sement yüzeyinin fibröz bir doku tabakası ile örtüldüğü saptandı. Sonuç olarak akrilik kemik çimentosu ile osteochondral defektin dolgusunun yapılması, bu tür olgularda rekonstriktif amaçla potansiyel öneme sahip bir uygulama olabileceği ve bu yöntemin diğer yöntemler yanı sıra sağıtımda alternatif bir yöntem olabileceği kanısına varılmıştır. The purpose of this investigation was to resurface of the joint surface in osteochondral defect, especially in Osteochondritis Dissecans by using Methylmet hacrylate (Acrylic bone cement) in dogs. Clinically healthy 10 dogs were used for this purpose. Osteochondral defect was experimentally performed on lateral labium of the trochleo patellaris by using hand drill. Depth of the defect was 1.0 cm. and diameter was 0.5 cm. Than the osteochondral defect was filled acrylic bone cement by using cement knife and spoon and constructed normal joint surface. All the cases were controlled clinically and radiologically to 2, 3, 4, 5, 6, 7, 8, 10 and 14 weeks. Radiological examination was made AP and ML positions all of the cases. At the end of 3. 4, 5, 7, 10 and 14 weeks dogs were euthanesied and histopathological examination was made. Clinically inflamative disorder was not observed all of joints. Joint movements and weigih-bearing were found normal during one week after the operation. Any degenerative and inflamative joint disorders, arthropatie or osteophy were not seen radiologically. Histological findings supported the clinical and radiological findings. Only in one case the cement surface was covered by thin fibröz layer in 14th week. In conclusion it was seen that the cement plug has important potential application on reconstrictive purposes in the joint defects. For this reason, this method can be alternative of the other methods in the treatment of osteochodral defects.
