Paroksismal atriyal fibrilasyon epizotlarının rastgele zaman ve periyotlarda oluşması, hastanın hastaneye ulaşmasından çok daha kısa sürede kalp atışlarının normal sinüs ritmine dönmesi gibi sebepler paroksismal atriyal fibrilasyonun standart elektrokardiyogram kaydı ile teşhisini zorlaştırır. Bu sebeple, teşhis için geleneksel yöntemlerden daha etkili yöntemler geliştirilmelidir. Tezde “elektrokardiyogram ve el bileği nabız sinyali birlikte değerlendirilerek paroksismal atriyal fibrilasyonun günlük yaşamda kolay, hızlı ve güvenilir şekilde teşhisine yönelik bir yöntem geliştirilebilir mi; geliştirilmesi durumunda bu yöntem klinik araştırma süreci ile test edilerek pratik şekilde kullanılabilecek düzeyde rutin kullanıma sunulabilir mi?” sorusu yanıtlanmıştır. Bu amaçla, deneklerden eş zamanlı olarak elektrokardiyogram kaydı ve el bilek nabız sinyali kaydı alınmıştır. Elektrokardiyogramdaki ardışık R-Tepesi başlangıç anı ve el bileği nabız sinyali başlangıç anı arasındaki gecikme süreleri hesaplanmıştır. Bu bilgi ve hesaplanan diğer bilgiler analiz edilmiştir. İnceleme sonucunda, hasta ve sağlıklı denekler arasında bazı farklılıklar tespit edilmiştir. Ancak bu farklılıkların paroksismal atriyal fibrilasyona işaret edip etmediği konusunda kesin sonuçlara ulaşılamamıştır., The reasons such as the occurrence of paroxysmal atrial fibrillation episodes at random times and periods, and the return of the heartbeat to normal sinus rhythm in a much shorter time than the patient's arrival at the hospital make it difficult to diagnose paroxysmal atrial fibrillation with standard electrocardiogram recording. For this reason, more effective methods for diagnosis than traditional methods should be developed. In this thesis, “Can a method for the easy, fast and reliable diagnosis of paroxysmal atrial fibrillation in daily life be developed by evaluating the electrocardiogram and wrist pulse signal together; If this method is developed, can this method be tested with the clinical research process and put into routine use at a level that can be used practically?” The question has been answered. For this purpose, electrocardiogram recording and wrist pulse signal were recorded simultaneously from the subjects. The lag times between the sequential RPeak onset moment and the wrist pulse signal onset moment in the electrocardiogram were calculated. This information and other calculated information were analyzed. This information and other calculated information were analyzed. As a result of the review, some differences were detected between the patients and healthy subjects. But no definite conclusions were reached, whether or not these differences indicate paroxysmal atrial fibrillation.

In this study, it is aimed to determine whether patients can be warned before PAF events. 30-minute HRV data used in this study. Each piece of data was divided into 10 pieces of 5-minute parts. Time domain measurements from linear measurements of HRV and Poincare measurements from nonlinear measurements of HRV were used for each segment. Detecting performances were measured for each segment using k-nearest neighbor classifier. Objectives: Atrial fibrillation (AF), the most common arrhythmia in clinical practice, is a major cause of stroke. Atrial fibrillation increases morbidity and mortality. Nowadays cryoablation therapy is being performed efficiently and safely worldwide. In this paper the clinical outcomes of the patients whom were treated with cryoablation were discussed. Methods: Between June 2012 and March 2014, patients with paroxysmal AF who were treated with cryoablation were included in the study. The medical records of the patients were retrospectively reviewed. Patients were called to receive information about the symptom recurrence. Results: Twelve patients were included in the study. Half of the patients were female. The mean age was 48 ± 15 years. Acute procedural success rate of cryoablation was 100%. In one patient transient right phrenic nerve injury occurred which was returned to normal after discontinuation of cryoablation therapy. One patient developed AF episode for 10 minutes in the 4th month of follow-up period. Conclusion: In patients with paroxysmal atrial fibrillation cryoablation effectively decrease symptoms consistent with the literature. J Clin Exp Invest 2014; 5 (4): 599-603, Amaç: Atriyal fibrilasyon (AF) klinik pratikte en sık karşılaşılan aritmi olmakla beraber inmenin en önemli nedenidir. Atriyal fibrilasyon morbiditeyi ve mortaliteyi artırmaktadır. Kriyoablasyon tedavisi günümüzde bütün dünyada yaygın bir şekilde etkin ve güvenli bir şekilde yapılmaktadır. Bu yazıda kriyoablasyon tedavisi yaptığımız hastaların klinik sonuçları tartışıldı. Yöntemler: Haziran 2012 ve Mart 2014 tarihleri arasında kriyoablasyon tedavisi uygulanmış paroksismal AF\'li hastalar çalışmaya dahil edildi. Hasta dosyalarındaki veriler retrospektif olarak incelendi. Telefonla ulaşılan hastalar semptomlar açısından sorgulandı. Bulgular: Çalışmada 12 hasta değerlendirildi. Hastaların %50\'si kadındı. Yaş ortalaması 48 ± 15 yıl idi. Kriyoablasyon akut işlem başarısı %100 idi. İşlem sırasında komplikasyon olarak sadece bir hastada geçici sağ taraf frenik sinir hasarı gelişti ve işleme son verildi. Bir hastamızda 4. ay takibinde 10 dakika süren AF atağı gelişti. Sonuç: Bu çalışma ile literatür ile uyumlu olarak paroksismal atriyal fibrilasyonlu hastalarda kriyoablasyon tedavisi ile güvenli ve etkin bir şekilde semptomlardaki iyileşme gösterilmiştir

Medical Technologies National Conference (TIPTEKNO) -- OCT 27-29, 2016 -- Antalya, TURKEY, WOS: 000455003600049, Paroxysmal Atrial Fibrillation (PAF) is a very common heart disease caused by irregular impulses of atrial tissue in adult. Diagnosing in the early stages of this disorder is very important for the patients to stop the progression of the disease and to improve the life quality. In this study, it is aimed to predict the PAF event before the realization of the PAF which in 5 minutes for the PAF patients. 30-minute data used in the study were divided into 5-minute parts. Fast Fourier Transform of frequency domain measures of heart rate variability obtained easily and practically is used for each part. The statistical significances among segments and discriminating performances of k-Nearest Neighbors classifier were obtained for each segment using these measurements. Consequently, As a result of statistical analysis, it is shown that patients may be warned 12.5 minutes earlier than a PAF attack.
