Anksiyete tanımlanması zor bir endişe ve korku halidir. Preoperatif hastaların büyük bölümünde anksiyete ve stres mevcuttur. Oluşan bu durum da hem ameliyat öncesi hem sonrasında, istenmeyen etkilere neden olamakta, fizyolojik ve psikolojik patolojileri tetiklemektedir. Ameliyat öncesi yüksek anksiyete düzeylerinin postop mortalite ve morbiditeye etkileri de yüksektir. Bu nedenle ameliyat öncesi anksiyete kontrolü büyük önem taşımaktadır. Bu durumu önlemek amacıyla çeşitli yöntemler kullanılmaktadır. Bu yöntemlerden birisi de etkinliği kanıtlanmış olan akupunktur tedavisidir. Akupunkturun tek başına veya diğer yöntemlerle birlikte kullanımı preop anksiyete tedavisinde yarar sağlayacaktır.

4. Koroner Arter Baypas Cerrahisi Uygulanacak Hastalarda Ameliyat Öncesi Anksiyete Prevalansı ve Yorgunluk Düzeyi: Tanımlayıcı Kesitsel Bir Araştırma.

Objective: The study was conducted as descriptive cross-sectional to determine the prevalence of preoperative anxiety and fatigue level among patients undergoing coronary artery bypass greft surgery. Material and Methods: The study was conducted with 116 patients who will be undergone coronary artery bypass graft between April 1 2019-April 1 2020. Data were collected with personal information form, State-Trait Anxiety Inventory and Multidimensional Assessment of Fatigue (MAF) Scale. Mann-Whitney U, Kruskal-Wallis and Spearman correlation tests were used in the analysis of the data. Results: In the study, it was determined that the preoperative anxiety level of the individuals was 44.94±1.36, 64.7% had moderate-to-high level of anxiety, and the MAF Scale total score was 35.12±6.99. It was found that the preoperative anxiety level was statistically significantly higher in individuals who were single, unemployed and who evaluated their sleep quality as poor, and the fatigue level was statistically significantly higher in individuals who were female, single, smoking-alcohol users, worker and who evaluated their sleep quality as poor. In the study, it was also determined that there was a positive and moderately significant relationship between preoperative anxiety, fatigue sub-dimensions and total fatigue level (p<0.05). Conclusion: It was determined that the majority of patients who will be undergone coronary artery bypass grafting experienced preoperative anxiety, and the level of fatigue increased as the level of preoperative anxiety increased. Therefore, cardiac surgery patients should be evaluated holistically in the preoperative period and evidence-based nursing interventions should be applied to reduce preoperative anxiety and fatigue levels. Bu araştırmada ilk kez ameliyat olacak hastalara ameliyat sürecine yönelik yapılan görsel bilgilendirmenin durumluk -sürekli kaygı düzeylerine etkisinin belirlenmesi amaçlanmaktadır. Araştırmanın tasarımı, son test kontrol gruplu yarı deneysel araştırma olarak planlandı. Araştırma Bursa Uludağ Üniversitesi Sağlık Araştırma ve Uygulama Merkezi Genel Cerrahi Kliniğinde Aralık 2019 - Haziran 2021 tarihleri arasında 60 hasta ile gerçekleştirildi. Araştırmada 1 müdahale grubu ve 2 kontrol grubu mevcuttu. Müdahale ve birinci kontrol grubuna daha önce ameliyat deneyimi olmayan hastalar, ikinci kontrol grubuna ise daha önce ameliyat deneyimi olan hastalar dahil edildi. Araştırmada müdahale grubundaki hastalara görsel bilgilendirme yapılırken kontrol gruplarındaki hastalar klinikte yürütülmekte olan standart bilgilendirme süreçlerine dahil edildi. İstatistiksel analizler için SPSS 21.0 programı kullanıldı ve istatistiksel analizlerde tip I hata düzeyi %5 olarak kabul edildi. Araştırmaya dahil edilen gruplar demografik özellikler açısından benzerdi (p>0,05). Müdahale grubundaki hastaların sürekli ve durumluk kaygı puanları ameliyathaneye yönelik düşüncelerine göre incelendiğinde; olumlu düşünceye sahip olan hastaların kaygı puanları kontrol gruplarındaki hastalardan daha düşüktü (p=0,015 ve p=0,037). Erkeklerin kadınlara göre ameliyathaneye yönelik düşünceleri daha olumluydu (p=0,002). Sürekli ve durumluk kaygı puanlarına göre gruplar arasında farklılık yoktu (p=0,284 ve p=0,292). Üç grup arasında durumluk kaygı puanı ve sürekli kaygı puanı açısından anlamlı bir fark olmamakla birlikte, ameliyat öncesinde video ile eğitim verilen müdahale grubunda ameliyathaneye ilişkin düşünceleri olumlu olanların kaygı puanları daha düşüktü. Araştırma devam ederken Covid-19 salgınının ortaya çıkmasının araştırmayı olumsuz etkileyen bir faktör olduğu düşünüldü. Ameliyat öncesi kaygıyı azaltmayı amaçlayan araştırmalardan daha ayrıntılı ve kesin sonuçlar elde edebilmek için Covid-19 salgınına daha adapte şartlarda yeni araştırmalara ihtiyaç vardır. This study aims to determine the effect of providing visual information pertaining to the operation process on the state-trait anxiety levels of patients who are to undergo surgery for the first time. The design of the study was planned as a quasi-experimental research with two control groups. The research was carried out in Bursa Uludağ University Health Research and Application Center General Surgery Clinic between December 2019 - June 2021 with 60 patients. There were 1 intervention group and 2 control groups in the study. Patients with no previous surgical experience were included in the intervention and first control group, and patients with previous surgical experience were included in the second control group. Patients in the intervention group were subjected to visual information, control groups were solely undergone through standard care procedures. SPSS 21.0 program was used for statistical analysis and type I error level was accepted as 5% in statistical analysis. The groups were similar according to their demographic variables (p>0.05). The trait and state anxiety scores of the intervention group patients with positive perceptios on operating room were lower than the patients in the control groups (p=0.015 and p=0.037). Males had more positive perceptions related to operating theatre than women (p=0.002). There was no difference between the groups in terms of trait and state anxiety scores (p=0.284 and p=0.292). Although no significance was observed between the three groups according to their anxiety scores. Patients in intervention group, subjected to visual information on surgical process before their surgery, had positive perceptions related to operating room. We think that the emergence of the Covid-19 outbreak while the research was conducted, influenced the outcomes of this study. In order to obtain more detailed and precise results from studies aiming to reduce pre-operative anxiety, new studies are needed in conditions more adapted to the Covid-19 epidemic.

- Abstract
Objective: In this study, the effect of the anaesthesia method on preoperative anxiety in parturients scheduled to undergo elective caesarean surgery was investigated. Methods: After Dicle University Ethics Committee approval, 100 American Society of Anesthesiologists (ASA) II parturients, aged between 18 and 45 years, were included in this study from 2011 to 2012 at the Dicle University Faculty of Medicine, Department of Anaesthesiology. The parturients were divided into two groups, with 50 patients in each group: general anaesthesia (GA) and spinal anaesthesia (SA). Patients were evaluated during the preoperative visit. Demographic data and STAI TX-1 form to measure preoperative anxiety were recorded. Results: There were no significant differences between the two groups in maternal age, gestational age, level of education and number of children (p>0.05). The average anxiety score was significantly higher in the SA group than in the GA group (p<0.05). Conclusion: In conclusion, spinal anaesthesia is associated with a higher preoperative anxiety level than general anaesthesia in obstetric patients. Therefore, it is necessary to take prevention against preoperative anxiety for the patients undergoing caesarean section surgeries under spinal anaesthesia. [ABSTRACT FROM AUTHOR]

Objective: We aimed to determine the reasons about the anxiety of anesthesia , measure the preoperative and postoperative anxiety levels and investigate the relationship between patient satisfaction and anxiety on patient who is undergoing C/S. This poll study was performed in order to determine preoperative anxiety levels and the reasons of anxiety, and to compare the State-Trait Anxiety Inventory (STAI) test, which is the standard test for measuring anxiety. Material and Methods: 800 pregnancy of the patients who took the poll 1days before the operation were taken for evaluation. The first part of the poll form included demographic information. In the second part; they were asked to grade 12 anxiety reasons about anesthesia. In the third part; STAI and a questionnaire composed of 12 questions which purpose to determine the reasons about anxiety of anesthesia and QoR-40 questionnaire which thequantity, the quality of postoperative recovery. The patients were met on preoperative and postoperative periods and demanded to fill questionnare forms. At the end of the study, the most frequently reasons of anesthesia anxiety was never been awoken again, postoperative pain and stay in intensive care unit and postoperative anxiety was determined the relation with patient satisfaction and pain control. Objective: Different degrees of anxiety and fear are seen in the majority of pre-operative patients. An interview conducted by the anaesthesiologist during the preoperative period and the information given in this interview may reduce anxiety levels. In this study we aimed to evaluate the preoperative and postoperative anxiety levels and the impact of the preoperative information on the level of postoperative anxiety using the STAI anxiety scale. Methods: After approval of the ethics committee, the "STAI FORM TX- 1" questionnaire, which is used to measure situational anxiety, was administered to 107 patients via an interview in order to measure their levels of preoperative anxiety. Standard general anaesthesiology was performed after all patients received information on the surgical operation and general anaesthesiology. In order to determine postoperative anxiety levels in patients in the postoperative period, the STAI FORM TX-1 questionnaire was repeated at postoperative 4-6 hours. Results: A significant decrease (p<0.05) in anxiety was observed between the 1st and 2nd questionnaire. Levels of anxiety in women were significantly higher than in male patients (p<0.05) preoperatively. Postoperatively, women's anxiety levels were higher than in male patients but this difference was not statistically significant (p=0.05). Conclusion: The level of preoperative anxiety in women is higher than in men as resurgence of anxiety in women leaving the family is more intense and women express their concerns more easily than men. Aim: It has been shown that smoking is chronically anxiogenic. The aim of the study is to investigate the effect of smoking on preoperative anxiety in patients undergoing elective surgery. Material and Method: Three hundred patients, aged 18-65 years, ASA I-II who was scheduled for elective surgery were enrolled in the study. The patients smoked 10 or more cigarettes per day for at least 2 years were enrolled in Group S (n=150) while non-smokers were included in Group K (n=150). No premedication was given to the patients. Demographic data, type of operation and anesthesia, history of previous surgery, systemic disease and status of education of the patients were noted. Cigarettes per day, years smoking and abstinence time were noted in Group S. The patients' state and trait anxiety scores (STAI-S, STAI-T) were measured by using State-Trait Anxiety Inventory (STAI). After the completion of STAI form, patients were requested to asses their stress levels according to the numerical stress score (NRS). Results: There was no difference between two groups for demographic data. It was seen that STAI-T and NSS values were significantly higher in Group S when compared to Group K. It was found no correlation between abstinence time and STAI-S, STAI-T and NSS. Conclusion: It was concluded that trait anxiety level is higher in smokers than non-smokers and cessation of cigarette smoking a day before surgery does not affect preoperative anxiety. Bu araştırma, Yale Modifiye Preoperatif Anksiyete Ölçeği (Kısaltılmış Versiyonu) Çocuk Formu’nun (YMPAÖÇF) Türkçe uyarlamasını yapmak ve çocukların ve ebeveynlerinin preoperatif anksiyetelerinin azaltılmasında bir mizahi girişim olan hastane palyaçoluğunun etkisinin değerlendirilmesi amacıyla yapılmıştır. İlk aşamada, Bülent Ecevit Üniversitesi Sağlık Uygulama ve Araştırma Merkezi ile Zonguldak Kadın Doğum ve Çocuk Hastalıkları Hastanesi’nde Eylül 2017-Eylül 2018 tarihleri arasında YMPAÖÇF Türkçe geçerlik ve güvenirlik çalışması gerçekleştirilmiştir. Ön uygulamada 10, ölçeğin uygulanmasında 40 çocuk ile çalışılmıştır. Cronbach alfa değeri 0.911 olarak tespit edilmiş ve ölçeğin güvenirliği ve geçerliği sağlanmıştır. İkinci aşamada, günübirlik cerrahi müdahale geçirecek olan bir çocuğun ve ebeveyninin ameliyat öncesi ve sonrasında anksiyetesini azaltabilmek için girişimde bulunulmuştur. Bu aşama aynı hastanelerde Eylül 2017-Eylül 2018 tarihleri arasında 105 çocuk ve ebeveynleri ile yapılmıştır. Çocuk ve ebeveynler; kontrol (rutin ameliyat öncesi ve sonrası tedavi süreci), deney 1 (hemşirenin oyuncak bebek üzerinde ameliyat öncesi bilgi verdiği) ve deney 2 (hastane palyaçosunun terapötik oyun uyguladığı) grubuna randomize atanmıştır. The study was made to analyze the effect of hospital clowns, which is a humorous attempt to decrease preoperative anxiety of children and their parents besides study Turkish adapiton of Short Version of the Modified Yale Preoperative Anxiety Scale in Young Children. In the first step, the reliability and validity study of mYPAS-SF in young children was conducted Zonguldak Women's Obstetrics and Gynecology Hospital and Bulent Ecevit University Health Application and Research Center between September 2017- September 2018. 10 children for pre-application and 40 children for the appliaction of the scale were included in the study. The Cronbach’s alpha value was found 0.911, and the reliability and validity of the YMPAOCF was established. In the second step, an attempt was made to reduce the anxiety of children and their parents who will undergo day-to-day surgery preoperative and postoperative. This step was conducted with 105 children and their parents who underwent surgery between September 2017-September 2018 in the same hospitals. Children assigned to control (routine care), experiment 1 (the nurse gave preoperative information on the doll) and experiment 2 (hospital clown's playing therapeutic play) groups as randomize. Bu çalışma, acil ve elektif cerrahi girişim geçiren hastaların ameliyat öncesi ve sonrası anksiyete ve hemşirelik bakım memnuniyetin değerlendirilmesi amacı ile tanımlayıcı olarak yapıldı. Araştırma evrenini 1 Kasım 2015-1 Mart 2016 arasında acil ve elektif ameliyata alınan bireyler, örneklemini ise araştırmakriterlerini karşılayan ve araştırmanın amacı açıklanarak bilgilendirildikten sonra izin alınan toplam 281 hasta oluşturdu. Verilerin toplamasında '' Hasta Tanıtım Formu'', ''State- Trait Anxiety Inventory (STAI) I-II'' ve ''New Casttle Hemşirelik Bakım Memnuniyet Ölçeği (NHBMÖ)'' kullanıldı. Verilerin değerlendirilmesinde SPSS 21.0 paket programı ile tanımlayıcı istatistiksel metotlarının (Frekans, Yüzde, Ortalama, Standart sapma) yanı sıra Kolmogorov - Smirnov, Mann Whitney U test, Spearman Korelasyon Analizi ve Wilcoxon işaret testi kullanıldı. Hastaların ameliyat öncesi STAI puan ortalamalarında acil grubu 47,496±2,901, elektif grubu ise; 43,558±2,993 olarak saptandı. Acil ve elektif grubun ameliyat önce STAI puanları arasında anlamlılık mevcuttu. Elde edilen sonuçlara göre acil ameliyata alınan hastaların daha fazla anksiyete yaşadığı ve her iki grupta da ameliyat öncesi anksiyete düzeyinin daha yüksek olduğu, her iki grubunda ameliyat öncesi anksiyete düzeyinin ameliyat sonrasında anlamlı şekilde azaldığı saptandı. NHBMÖ'nde ise acil grubu 76,43±10,531 elektif grubu 78,651±9,481 olup, aralarında anlamlı bir ilişki saptanmadı. STAI II puanı ile ameliyat sonrası STAI I puanının HBMÖ ile korelasyonun da anlamlı ilişki saptandı. Bu sonuçlara göre hemşirelikten memnuniyet düzeyi arttıkça anksiyetenin azaldığı belirlendi. Araştırma sonuçlarına göre ameliyat öncesi ve sonrasında anksiyeteyi azaltmaya yönelik hemşirelik girişimlerinin yapılması önerilmektedir. This study was performed descriptively in order to evaluate preoperative and postoperative anxiety and the nursing care satisfaction of the patients undergoing emergency and elective surgery. The research population was composed of individuals who were taken under emergency and elective surgery between 1 November 2015-1 March 2016; the study sample was composed of a total of 281 individuals who met the criteria and whose permissions were received after being informed by explaining the purpose of the study. ''Patient Identification Form'', ''State-Trait Anxiety Inventory (STAI) I-II'' and ''New Casttle Nursing Care Satisfaction Scale (NSNS)'' were used for the collection of the data. Kolmogorov - Smirnov, Mann-Whitney U test, Spearman correlation analysis and Wilcoxon signed rank test were used as well as SPSS 21.0 software package and descriptive statistical methods (frequency, percentage, mean, standard deviation) for the evaluation of the data. Preoperative STAI average score of the patients was detected as 47.496 ± 2.901 for the emergency group and 43.558 ± 2.993 for the elective group. Also, statistically significant decrease in preoperative and postoperative STAI average scores of both groups was determined. According to the obtained results, it was detected that patients who were taken under emergency surgery experienced more anxiety; both groups had higher anxiety level before the surgery, and preoperative anxiety level of both groups significantly decreased after the surgery. NSNS was found to be 76.43 ±10.531 for the emergency group and 78.651 ± 9.481 for the elective group and no significant relationship was found between them. Significant relationship was detected in correlation of STAI II and postoperative STAI I score with NSNS. According to these results, it was determined that anxiety decreased by the increase in satisfaction level of nursing. According to the results of the study, it is suggested that nursing approaches to reduce preoperative and postoperative anxiety are carried out. 101

Anesteziyoloji çalışmalarında anestezinin kalitesini belirlemek için anket çalışmaları eskiden beri yapılmaktadır; ancak bu çalışmalar yeterli sayıda ve detayda değildir. Bu nedenle bu çalışmayla elektif cerrahi planlanan kadın hastalarda sezeryan ve diğer cerrahi klinik hastalarının demografik özelliklerini tespit etmek, bunların anksiyete ve bilgi edinme düzeyi üzerine etkisini belirlemek, sezeryan hastaları ile diğer klinik cerrahi hastaları arasında anksiyete farkını araştırmayı istedik. Anestezi polikliniğine müracaat eden hastalardan preoperatif dönemde anket formlarını doldurmaları istenmiştir. Anket formu iki bölümden oluşmaktadır. Birinci bölümde yaş, cinsiyet, eğitim, medeni durum, çocuk sayısı gibi demografik bilgilerle planlanan operasyon türü, daha önceden anestezi ile karşılaştı mı? Bölgesel anesteziyi biliyor mu? sorularının sorulması planlandı. İkinci bölümde anestezi ve cerrahiye bağlı anksiyeteyi ve bilgi edinme isteğini ölçmek için Amsterdam Preoprative Anxiety and Information Scale (APAIS) testi kullanılarak hastaların anksiyete düzeyinin ölçülmesi planlandı. Çalışmamızın sonuncunda; cerrahi operasyon ve anestezi uygulamalarının hastalar için önemli bir anksiyete faktörü olduğu, preoperatif dönemde hastaların kaygılarının yüksek olduğu, preoperatif anksiyete düzeyinin eğitim düzeyi yüksek olanlarda ve sezeryan hastalarında anlamlı olarak daha fazla olduğu saptandı. Çalışma grubumuzda eğitim seviyesinin lisans ve lisansüstü olması, preoperatif anksiyetesi olan hastalar ve planlanan ameliyatın sezeryan oluşu bilgi edinme isteklerinde anlamlı olarak artmaya neden olduğu tespit edildi. Therefore, with this study we wanted to determine the demographic characteristics of caesarean section and other clinical patients, determine the effects of these on anxiety and information level, investigate anxiety difference between elective surgery and cesarean section in female patients who scheduled for elective surgery. Filling the questionnaire was asked from the patients who were admitted anesthesia clinic preoperatively. The questionnaire consists of two parts. In the first section It was planned to be asked the demographic data such as age, gender, education, marital status, number of children, and questions such as the type of operation planned for, you have previously met with anesthesia, do you know regional anesthesia? In the second section it was planned to use Amsterdam Preoprativ Anxiety and Information Scale (apais) to measure anesthesia and surgery-related anxiety level and information requests As a result of our work; it was found that surgery and anesthesia is a major anxiety factor for patients, patients have high anxiety during preoperative period, preoperative anxiety level is significantly higher in high level of education and in cesarean patients. In our study group, it was detected that undergraduate and graduate education level, patients with preoperative anxiety and because of scheduled surgery is caesarean section caused a significant increase of information requests. Bu çalışmada; elektif sezeryan cerrahisi planlanan hastalarda hastaların yaşı, eğitim durumu, gebelik haftası, yaşayan çocuk sayısı ve anestezi tipinin preoperatif anksiyete düzeylerine olan etkisi incelenmeye çalışılmıştır. Çalışma Dicle Üniversitesi Tıp Fakültesi Anesteziyoloji ve Reanimasyon Anabilim Dalına başvuran;hastanın isteğine bağlı olarak genel anestezi ya da spinal anestezi altında, elektif şartlarda operasyonu planlanan; 18-45 yaş arasındaki 100 olguda planlanmıştır. Hastalar, preoperatif vizitte ziyaret edilmiş, anksiyete değerleri STAI Form TX-1 ile ölçülmüştür.. Sonuç olarak preoperatif dönemde hastaların anksiyete düzeylerinin yüksek olduğu; spinal anestezi grubunun genel anestezi grubuna göre anksiyete düzeyinin yüksek olduğu saptanmıştır. Anahtar sözcükler: preoperatif anksiyete, gebelik, anestezi tipi In this study, the effect of patients' age, educational status, gestational age, the number of children living, and the type of anesthesia on levels of preoperative anxiety with the patients scheduled for elective cesarean surgery was tried to be examined. The study was planned on 100 cases between the ages of 18-45, whose operations were scheduled under elective conditions with general anesthesia or spinal anesthesia dependent on the request of the patients, who applied to the Dicle University Faculty of Medicine, Anesthesiology and Reanimation Department. Patients were visited pre-operative, and the anxiety scores were measured with STAI Form TX-1. As a result it was established that, preoperatively, patients had higher levels of anxiety; and that spinal anesthesia group had higher levels of anxiety than the general anesthesia group.
