Domestic violence destroys the basic values on which the family should be built. It has many effects and serious consequences on family members such as death, injury, trauma, suicide, abuse etc. In terms of public management, it can lead to effects such as damage to public trust, inefficiency of workforce, increase in public expenditures and decrease in public revenues etc. In this paper, it will be emphasized whether Supreme Audit Institutions (SAIs) have roles and responsibilities in the management of fighting against domestic violence in public administrations, and if they have, how they carry out these roles and responsibilities based on international practice examples. The purpose of this paper is to raise awareness about the audit practices of supreme audit institutions in public administrations regarding a social problem such as domestic violence and the good management principles they point to. It can be concluded that SAIs, which have a key position in the protection and observance of financial discipline in the public sector, undertake an important function in the effective management of the fight against domestic violence in public administrations through their audit reports, opinions and recommendations. Objective:In this study, we aimed to compare the clinical and polysomnographic features of rapid eye movement sleep-dependent obstructive sleep apnoea syndrome (REM-OSAS) and positional OSAS (pos-OSAS).Materials and Methods:This study included all patients diagnosed with REM-OSAS and pos-OSAS who were examined at our sleep and disorders unit between January 2019 and November 2020. All patients underwent a full-night polysomnographic examination.Results:A total of 300 patients with OSAS were included in the study, of which 60 (20%) were diagnosed with REM-OSAS and 75 (25%) with pos-OSAS. Objective: Supine Percutaneous Nephrolithotomy (PCNL) is a minimally invasive method that is frequently used in the treatment of kidney stones larger than 2 cm. It has been suggested that one of the risk factors that may affect the success of PCNL is the stone to skin distance (SSD). We aimed to investigate the effect of SSD on surgical outcomes in patients who underwent supine PCNL surgery. Material and Method: One hundred and thirteen patients who underwent PCNL operation in the Galdakao modified supine Valvidia position between January 2017 and October 2021 were included in this study. Patients' demographic information, stone characteristics, SSD, operation time, fluoroscopy time, complications and hospital stay data were collected retrospectively from the hospital database. The patients were divided into 2 groups according to the median skin to stone distance (11.80 cm (min-max: 8.20-13.10)). Patients with SSD ≤11.80 cm formed Group 1 (n = 64) and patients with SSD>11.80 cm formed Group 2 (n =49). Results: The mean age of 113 patients included in the study was 45.17±15.2 years. BMI in Group-1 was significantly lower than Group-2 (28.37; 29.89, p <0.0001). It was found to be significantly shorter in SSD than in Group-1 (9.77 mm; 12.48 mm, p <0.0001). Although stone-free rates were high in Group-1, this was not statistically significant (84.3%; 79.6%, p =0.663). There was no significant difference between the groups in secondary interventions (p >0.05). Conclusion: According to our data, SSD has no effect on success rate, complications, hospitalization time, fluoroscopy time and re-operation rates in supine PCNL surgeries. [ABSTRACT FROM AUTHOR]

Aim:To determine the effects of positioning on the adaptation to spontaneous breathing in premature infants after weaning from mechanical ventilation.Materials and Methods:This randomized controlled experimental study was conducted with 60 (study group=30; control group=30) premature infants in the neonatal intensive care unit. The infants in the study group and the control group were in a prone position and a supine position, respectively, during the first 120 minutes when spontaneous breathing started after they were weaned from mechanical ventilation.Results:There were significant intragroup differences in mean heart rate between repeated measurements at different time points. The differences were not found to be due to positioning. The mean SpO2 was higher in the study group than in the control group, although the difference was not significant. This finding suggested that positioning did not have an influence on SpO2. Two infants in the control group failed to maintain spontaneous breathing. Although the study group had a significantly higher respiratory rate than the control group, the difference was not significant. Therefore, positioning was not found to affect respiratory rate.Conclusion:There was no effect of positioning on the adaptation to spontaneous breathing in premature infants after weaning from mechanical ventilation.

Purpose: The aim of this study was to compare the effect of supine and prone positions on physiological parameters of preterm infants under nasal continuous positive airway pressure (N-CPAP). Materials and Methods: This cross-over clinical trial was conducted on 62 hospitalized preterm infants under N-CPAP in the neonatal intensive care unit of Imam Reza Hospital Kermanshah, Iran. Neonates were placed in supine position at first for 180 minutes and then their position was changed into prone position for another 180 minutes. During this period, heart rate (HR), respiratory rate (RR) and arterial oxygen saturation (So2) were assessed once every 15 minutes in two positions. Results: There was significant difference observed in the means of HR in two positions. The RR in the prone position was significantly lower than that in the supine position; however, the So2 in the prone position was significantly higher than the supine position.Conclusion: The results of this study showed in preterm infants with respiratory distress syndrome under N-CPAP, there was lower RR and higher So2 in prone position when compared to the supine position. Therefore, preterm infants have better physiological stability in prone position.

Objective: The method of operation carried out by clip application in the supine position and using a single port for thoracoscopic sympathectomy has been defined in recent years. Since lesser tissue damage occurs with this method, lower perception of pain may be anticipated. The aim of this study is to investigate whether intravenous morphine alone provides effective postoperative analgesia in thoracoscopic sympathectomy performed with clips method using a single port with the patient in the supine position. Method: Files of 28 patients who underwent thoracoscopic sympathectomy between January 2017-December 2019 were retrospectively screened . Intrathecal morphine was administered to a group at a dose of 0.6 mg in 3 mL physiologic saline (Group ITM, n=14), and the other group received IV morphine in the same doses only by the PCA method (Group PCA, n=14). Both groups received postoperative Patient Controlled Analgesia (PCA) with IV morphine, as 1 mg bolus within 30 min-lock- in time. VAS values and postoperative IV total morphine consumption were recorded at the time of extubation and at the 6th, 24th and 48th hours postoperatively Results: Any statistically significant difference was not detected between patient groups as for demographic, and operative data. After extubation and at postoperative 6th hour, pain scores were statistically significantly lower in Group ITM than the Group PCA. Total IV morphine consumption at the postoperative 24th and 48th hours was statistically significant in the Group that did not receive ITM. In the postopoperative 48-hour follow-up, no VAS>4 value was observed in any one of the patients. Any intergroup difference was not detected as for length of hospital stay and complications associated with the method of analgesia used. Conclusion: In conclusion, even though an adequate analgesia is ensured through ITM in thoracoscopic sympathectomy operations realized using clips method and a single port with the patient in the supine position,we think that admiinistration of morphine using IV PCA method may provide clinically acceptable analgesic efficacy, and a regional administration may not be required in patients who will be operated with this surgical technique. A safe sleep environment is important in reducing sleep-related infant mortality. Sudden Infant Death syndrome is defined as a sudden and unexpected death of a baby under one year old during sleep. Safe sleep environments should be established to reduce and remove risk factors for Sudden Infant Death syndrome. Risk factors include a supine position while the baby is sleeping on a soft bed and presence of soft objects in the baby's sleep environment. Thus, nurses and health professionals should provide education and counselling to parents on relevant risk factors to prevent Sudden Infant Death syndrome. [ABSTRACT FROM AUTHOR]

Background: Different positions given for the surgical procedure can cause changes in tissue perfusion and cardiac output. In this study, we aimed to investigate the changes of pleth variability index (PVI), perfusion index (PI) and noninvasive continuous cardiac output measurement method (EscCO) in different surgical positions in participants with spontaneous breathing. Materials and Methods: A sample of 20 healthy individuals was included in the prospective observational this study. Participants held six different positions: supine, prone, 30 degrees upside, 45 degrees upside, trendelenburg (20-degrees head down), and reverse trendelenburg (20-degrees head up). The time was kept 10 minutes between each position. Before each position change, volunteers were placed in the supine position and after five minutes of rest, a new position was given. After the position change, PI, PVI, EscCO, peripheral capillary oxygen saturation values at 5th minutes were recorded. Results: No statistically significant difference was found in PI, PVI and EscCO values measured while the participants were in different positions (p >0.05). Conclusion: This study revealed that PI, PVI and EscCO did not change in all surgical positions, and this parameters were not affected by position in young healthy individuals with spontaneous breathing. Objective: Hızla yapılabilen ve noninvaziv bir işlem olan toraks USG yöntemi, ÇLT pozisyonunu değerlendirmede klinik olarak karar vermeye katkı sağlayabilir. Methods: One hundred and thirteen patients who required OLV and intubated using left sided DLT were enrolled in the study. After accuracy estimates obtained with RCA and USG based on blind intubation without using fiberoptic bronchoscopy (FOB), conventional clinical assessments including SpO2 >90%, peak airway pressure <35 cmH2O and plateau airway pressure <25 cmH2O were performed with the patient in the supine position, and the position of DLT was deemed to be optimal. While the RCA method was performed by using inspection and auscultation of thorax, the USG method was achieved by evaluating lung sliding and lung pulse. Thoracic USG was performed in all patients after RCA method, without changing the position of DLT. The results of these two methods were compared as for the prediction of DLT position. Results: The success rates of predicting accurate position of DLT using USG, and RCA methods were 83.2%, and 77.9%, respectively. The sensitivity and negative predictive value (NPV) of USG were higher when compared with RCA. DLT was in correct position in 71.7% of the cases, while malposition rate was 28.3%. When malposition was conceived, DLT was repositioned. Conclusion: While malposition was corrected with a single manipulation in 14 (43.7%) patients, multiple manipulations were needed in 18 (56.3%) patients. If repositioning of DLT was not successful, malposition of DLT was considered and position of DLT was corrected with the aid of FOB in 6 (5.3%) cases with the patients in supine position. Objective: In this study, using a series of supine and prone positions T1 and T2 with weighted axial and sagittal magnetic resonance imaging, the movement interval of the conus medullaris stimulated by the supine-prone position change was assessed in 40 healthy subjects. Method: We measured the distance between the conus medularis and the front dural surface and the width of the dural sac using electronic scales at the tip level of the conus medullaris in each subject. The data obtained was evaluated using SPSS 15. Results: It was determined that the spinal canal width was decreased by 30% (p:0.0001) as the average value when the conical tip was brought to the prone position from supine. Age (0-18 and >19 years) and gender diversity were not statistically significant factors in the movement of the conus medullaris. Conclusion: Our study offers a reliable diagnostic reference range in cases of normal extension conus with clinically tense spinal cord suspicion. [ABSTRACT FROM AUTHOR]

Objective: The aim of the study was to evaluate the safe sleeping habits of the children, knowledge level of parents about safe sleep and related risk factors. Material and Method: The study carried out in Istanbul University Istanbul Medical Faculty Hospital, Well Child Clinic and General Pediatric Clinic, between September and November 2016. The population of the study consisted o f the children between 6-59 months of age and their parents, The survey questions about sleeping habits were asked to the parents during a face-to-face interview. The weight and height of parents were measured and body mass indexes were also calculated. Results: The mean age of the children was 30.0±18.1 months. For the first 6 months 67.4% (n=139) of the babies were breastfed for the first 6 months of their lives was (n=139). At the end of the evaluations it was found that 88.3% (n=18) of the parents shared their rooms with their children, and 27.6% (n=57) of the children slept in supine position. More than half of the parents (60.7%) who shared their beds with their children smoked. Conclusion: Our study revealed the need to provide detailed information about safe steep habits when providing breastfeeding counseling after birth. [ABSTRACT FROM AUTHOR]

Background: Although the muscular bridge (MB) which seen in the coronary arteries is known to be a benign condition, it is reported to be associated with adverse cardiac events in some studies. Although some condition such as coronary ischemia have been blamed in the occurrence of these adverse events, it is thought that the negative effects of myocardial electrical conduction will contribute to this situation.In this study, the relationship between electrocardiography parameters that demonstrate the electrical activity of the heart and the muscular bridgewas investigated. Material and Methods: This study included 36 patients (26 males, mean age 56 years) who were diagnosed with MB and 36 patients with normal coronary artery (30 male, mean age 51 years). 12-lead electrocardiography (ECG) was performed in supine position and QT interval, QT corrected (QTc), Tp-Te interval, Tp-Te/QT and Tp-Te/QTc measurements were performed. These parameters were compared between groups. Results: There were no significant differences between MB group and control group in terms of clinical and baseline demographic characteristics However, QT interval, QTc, Tp-Te interval, Tp-Te / QT and Tp-Te / QTc were significantly higher in the MB group. Multivariate analysis was performed to determine the independent predictors of the Tp-e interval. The length of MB was detected as an independent predictor of Tp-Te interval. Objective: Vernal keratoconjunctivitis is more common in children and young adults having an atopic background. The aim of the present study was to determine the interleukin- 16 (IL-16) levels in tear fluids of patients with vernal keratoconjunctivitis (VKC). Methods: Tear fluid samples were collected from 20 patients with VKC and 10 healthy subjects. Tear fluid samples were collected with microcapillary tubes for hematocrit at the lateral canthus of patients in the supine position without any anesthesia. Tear levels of IL-16 were measured by ELISA kit. Results: The mean levels of IL-16 among the patients (514±135 pg/ml) was significantly higher than among controls (358±139 pg/ml) (p=0.04). Symptomatic acquired diaphragmatic eventration is an uncommon finding in the clinical settings. A 47-year-old man admitted to our clinic with the complaint of dyspnea occurring during exercise and in supine position for the last two months. Although his chest X-ray taken 20 days ago showed no abnormality, a right diapl-u'agmatic eventration was detected on his current chest radiograph. No significant patho!ogy explaining a recent unilateral diaphragmatic eventration was detected in ragiologic imaging studies of thoraco-abdominal and cervico-cranial regions. The patient has been followed up for two years with idiopathic acquired right diaphragmatic eventration and presented with a literature search due to this rare entity as a cause of dyspnea. Percutaneous nephrolithotomy (PNL) is the gold standard for the treatment of kidney stones larger than 2 cm. In patients with anatomical anomalies or musculoskeletal deformities PNL can be challenging due to abnormal anatomy. Kyphoscoliosis is a musculoskeletal disorder in which there is abnormal relationship of kidney to surrounding organs. Background and Design: The sympathetic skin response (SSR) is a electrophysiological test used as an index of sudomotor sympathetic function. The aim of this study was to evaluate possible sympathetic nervous system dysfunction in vitiligo patients with the sympathetic skin response. Material and Method: Sympathetic skin response was studied in 26 patients (20 female and 6 male) with clinical definitely generalized vitiligo and 23 healthy controls (18 female and 5 male). This study was performed in a semi-darkened room while the patients were in supine position. SSR recordings in all of the subjects were performed by a Medelec electroneuromyograph. Results: The average age of patients and controls were 30.6±12.8 and 29.7±10.5, respectively. No demographic differences existed statistically between patients and controls (p>0.05). The mean latency of SSR in vitiligo patients [mean SSR latency in patients, 1.48±0.58 ms vs controls, 1.52±0.46 ms (p=0.944)] was not significantly different compared with the controls. The mean amplitude of SSR in vitiligo patients (mean SSR amplitude in patients, 3.83±2.95 mV vs controls, 5.09±3.60 mV) was smaller compared with the controls, but this difference was not significant (p=0.100). Objectives: This study aims to investigate the possible relationship between passive rhinomanometry measurements in sitting and supine position and obstructive sleep apnea syndrome., Patients and Methods: Between January 2011 and December 2013, 88 male patients (mean age 46.8 years; range 18 to 79 years) underwent passive rhinomanometry in sitting and supine position following history, physical examination and fiberoptic endoscopic examination. 1.5 lt/sn air flow was pumped into the nose of each patient via a silicone nasal mask. Meanwhile, flow and pressure values were measured., Results: There was no correlation between the resistance values and apnea hypopnea index (AHI) scores and polysomnography and physical examination parameters (p>0.05). The differential resistance was correlated with AHI (r=0.325, p<0.05), body mass index (r=0.324, p<0.05), neck circumference (r=0.421, p<0.01), waist circumference (r=0.444, p<0.01), modified Mallampati score (r=0.356, p<0.05), and retropalatal grade (r=0.438, p<0.01)., Conclusion: The correlation between the differential resistance and physical examination parameters and AHI scores support the hypothesis that position-related retropalatal segment alterations can be measured by passive rhinomanometry while awake.

Recent advances in surgical techniques for thoracic have led to an increased use of one lung ventilation techniques. One lung ventilation is performed by doubled lumen tubes, fogarty catheters, Univent tubes and bronchial blockers. In this paper our bronchial blocker experiences were presented in fifteen thoracic surgery patients. Bronchial blockers were placed in fifteen patients who needed one-lung ventilation during thoracic surgery between January-April in 2007. Type of the bronchial blockers were selected randomly and total eight Cohen and seven Arndt bronchial blocker were placed. Following endotracheal intubation some of the bronchial blockers were performed at supine and the others were in lateral position to patients. Fiberoptic investigation was performed in all of the patients following lateral position. Bronchial blocker were placed in fifteen patients. In one patient who had congenital tracheal bronchus, one lung ventilation could not have been achieved by bronchial blocker. Following lateral positioning bronchial blocker of two patients were malpositioned and they were repositoned. Successfull one lung ventilation was performed by both bronchial blocker type. While the patient's airway position, postoperative period and type of the surgery have been considered, bronchial blockers may be an alternative airway device for one-lung ventilation.

Purpose: The purpose of this study was to evaluate the role of the subject's position and two different maneuvers in the reflux measurements in femoral veins., Materials and Methods: Venous reflux times in the common femoral vein (CFV) and superficial femoral vein (SFV) were studied in 50 healthy subjects (25 women, 25 male) aged between 22-57 years-old while the subjects were in supine and standing positions. Reflux was provocated only by Valsalva maneuver (VM) when the subject was lying down, whereas it was provocated by (1) Valsalva maneuver and (2) compression and release maneuver, respectively, when the subject was standing. Reflux times were recorded with each of the three methods., Results: Regarding CFV, a reflux time of 1 second or more was recorded in 29 limbs (29%) only in the supine position. Of these 29 measurements, 19 were between 1 and 1.9 sec and 8 between 2 and 2.9 sec. When the subjects were examined in the standing position, the prevalence of reflux lasting more than 1 sec substantially decreased (5%). The prevalence of reflux lasting less than 0.5 sec was 97% with standing compression maneuver, 84% with standing VM and 27% with supine VM. Similarly, prevalence of reflux of longer than 0.5 sec or 1 sec was lower in the standing position than in the supine position in SFV. In CFV, mean reflux time was 1.3 sec in the supine position, whereas it was 0.7 sec and 0.4 sec with standing compression and standing Valsalva maneuvers, respectively. Corresponding values in SFV were 0.5 sec, 0.3 sec and 0.2 sec, respectively., Conclusion: The probability of making a false positive diagnosis of pathological reflux is high with supine examinations. When an examination in the supine position reveals a reflux time of longer than 1 second, making the patient stand up is expected to yield more reliable results. Compression and release maneuver, with its lowest false positive reflux prevalence, is the most reliable method in the evaluation of venous insufficiency.
