5. Evaluation of Public Hospitals' Performance with Decision Tree Algorithms

Purpose: The study aims to evaluate a range of financial performance indicators calculated through structural, operational, and HVI measures for public hospitals in the Turkish healthcare sector using various decision tree algorithms. Methodology: The study comprises threa phases. In the first phase, financial ratios were calculated from the hospitals' financial statements using the ratio analysis method. In the second phase, these ratios were used to calculate the HVI. In the third phase, the selected operational and financial indicators were analyzed with decision tree algorithms. The ID3, C4.5 and CART decision tree algorithms and AUC were used for predicting operational and financial indicators and performance assessment of decision trees.Findings: It has been observed that decision trees created using the ID3 algorithm exhibit higher performance compared to other algorithms (AUC = 0.93). According to the results of the study, the number of beds significantly predicts the operational and financial performance of public hospitals and can be explained by the hospital scale. In addition, a strong relationship was found between operational and financial performance indicators with training status.Originality: The study is original in demonstrating the effectiveness of the ID3 decision tree algorithm in predicting the performance of public hospitals.

The research was conducted to measure the effects of transformational leadership, transactional leadership, and laissez-faire leadership on workplace conflict. “Convenience sampling method” was used in the research. Survey technique was used to collect research data. Within the scope of the research, 406 valid surveys were collected from healthcare professionals who work in public and private hospitals in Gaziantep and do not have administrative duties. “Partial Least Squares” (PLS) and “Structural Equation Modeling” (SEM) were conducted to examine the theoretical hypotheses proposed between transformational, transactional, laissez-faire leadership and workplace conflict. According to the results of the structural equation model, it was concluded that inspirational motivation, idealized influence, intellectual stimulation, and individualized consideration, which are transformational leadership subdimensions, have negative effects on cognitive and relational conflict. The research also concluded that contingent reward, one of the transactional leadership sub-dimensions, has a negative effect on cognitive and relational conflict. In addition, it was concluded that the other sub-dimensions of transactional leadership, management by exception-active and management by exception-passive have a positive effect on cognitive and relational conflict. Finally, the study concluded that the laissez-faire leadership style does not have any effect on cognitive and relational conflict. [ABSTRACT FROM AUTHOR]

The aim of this research is to examine the relationship between perceived organizational support and organizational commitment and to determine the differences in perceived organizational support and organizational commitment based on demographic factors. The population of the research consists of all healthcare employees working in a public hospital in Erzurum. The sample of the research consists of 338 healthcare employees working in this public hospital. In collecting research data, a questionnaire was used, starting with a form measuring the demographic characteristics of the healthcare employees. Then, the Perceived Organizational Support Scale and Organizational Commitment Scale questions were included. In line with the research objectives, independent groups t-test and one-way analysis of variance (ANOVA) were performed, and correlation analysis was conducted to determine the direction of the relationship between the scale and subgroups. As a result of the research, a positive and significant relationship was found between perceived organizational support and organizational commitment and its sub-dimensions, emotional, normative and continuance commitment. A significant difference was found in the averages of working hours, working positions and marital status of healthcare professionals, organizational commitment and its sub-dimensions of normative commitment. No significant difference was found in the mean of gender, age, monthly income, organizational commitment, affective commitment, continuance commitment, normative commitment and perceived organizational support. This study aims to evaluate the Turkish validity and reliability of The Personal Growth Initiative Scale developed by Zahid et al. (2023) in healthcare professionals. This study was applied to 343 healthcare professionals in a public hospital. The sample was determined by the quota sampling method, considering the profession criterion. The data within the scope of the research was collected by survey method The survey consisted of questions containing participant characteristics and the Personal Growth Initiative Scale developed by Zahid et al (2023). SPSS 26 and AMOS 24 software were used to evaluate the data. After language validity, content validity was evaluated Exploratory Factor Analysis (EFA) and second level Confirmatory Factor Analysis (CFA) were used to evaluate the scale construct validity. Croribach Alpha (CA) and Composite Reliability (CR) coefficients were used to evaluate the scale reliability analysis. The Personal Growth Initiative Scale translated into Turkish was determined to have content (CVR>0.977) and construct validity (CMIN/DF=2,699; NFI=0,9I7, GFI=0,954, AGFI=9,4I, RMR=0,079). In addition, the Personal Growth Initiative Scale translated into Turkish had internal consistency. When the findings were examined, the participants' mean Personal Growth Initiative Scale score was 77.25-10.24 (min: 39.00; max: 95.00). The Personal Growth Initiative Scale, translated into Turkish, was determined to be a valid and reliable measurement tool in the research population. [ABSTRACT FROM AUTHOR]

Medical tourism is a rapidly growing industry globally and is considered a strategic investment area by many countries. Public health institutions, particularly in countries like Turkey, play a significant role in the development of medical tourism. Identifying the challenges faced in this sector is critical for its sustainable growth. This research aims to confidently identify and prioritize challenges related to the implementation of medical tourism in a Training and Research Hospital. Through the identification of 12 criteria, strategic recommendations will be developed to increase the potential of medical tourism. Data was collected from 10 healthcare professionals working in various units and professional groups in the field of medical tourism at the relevant hospital using a semi-structured interview form. The collected data was confidently analyzed using the Analytical Hierarchy Process (AHP) steps. Primary barriers to medical tourism in public hospitals include the hospital's image, availability of foreign language-speaking healthcare professionals, and the hospital's advertising and promotional activities. Research findings support this assertion. In this context, policymakers must consider the research results and implement comprehensive policies for social media, social networks, and related areas, especially regarding promotional and marketing activities. Furthermore, healthcare professionals must be strongly encouraged to learn a foreign language, and studies must be conducted to make arrangements for these staffs. [ABSTRACT FROM AUTHOR]

A pandemic is the spread of an infectious agent or disease over a wide area, between countries, continents, or the whole world. Crises are unexpected events that require an immediate response from the organization. In this study, it is aimed to examine the perspectives of health managers on crisis management and solution proposals during the pandemic period. Within the scope of the study, in-depth interviews were conducted with seven health managers working in the management part of a public hospital in Istanbul, and the qualitative data obtained were divided into various categories and analyzed by content analysis method. It has been determined that health managers provide uninterrupted communication in crisis management, examine events on the spot, share problems with the crisis center team and develop solutions, and they do not experience great difficulties during crisis management. Amaç: Türkiye’de gerek acil servislere gerekse polikliniklere çok sayıda başvuru mevcuttur. Acil servisler Türkiye’de hasta popülasyonunun önemli bir kısmını karşılayan tanı ve tedavi birimleridir. Çalışmamızda ülkemiz acil servislerine ve hastane polikliniklerine başvuran ve muayene edilen hasta sayıları ile tanı ve tedavi sürecinde istenen konsültasyon sayıları incelendi. Amaç ülkemiz kamu hastanelerine olan başvuruları saptamak, yıllara göre değişimlerini incelemek ve acil servis ile poliklinik hizmetlerinin son yıllardaki değişimini göstermektir. Materyal ve Metod: Retrospektif tanımlayıcı bir çalışma olarak planlanan bu çalışmada 1 Ocak 2016-1 Ocak 2022 tarihleri arasında Türkiye’de 2. ve 3. Basamak 836 kamu hastanesi polikliniklerine ve acil servislerine başvuran hasta sayılarına ait veriler ele alınmıştır. Veriler tablolama programı üzerine kaydedilmiş ve yüzdesel değişimleri tablolama programı istatistiksel formülleri üzerinden hesaplanmıştır. Bulgular: Ülkemizin nüfusu 2016’da yaklaşık 79,8 milyon iken 2021 yılı sonu itibariyle 84,6 milyona çıkmıştır. Bu süreçte acil servislerde muayene edilen hasta sayısı 2016’da 92,6 milyon kişi iken 2021 sonu itibariyle 129,5 milyona çıkmıştır. Aynı dönem içerisinde polikliniklerdeki muayene sayıları 199,5 milyondan 136,9 milyona düşmüştür. Bu yıllar arasında uzman hekim konsültasyon sayıları 2016’da 12,1 milyon hasta iken 2021 sonunda 14,6 milyon hastaya çıkmıştır. Sonuç: Sağlık hizmetlerinde yaşanan olumlu gelişmelerle birlikte hizmete erişebilirliğin de artmasıyla son yıllarda sağlık hizmeti alma talebinde ciddi artışlar yaşanmıştır. Bu taleplerden en çok etkilenen alan olan acil servislerde muayene edilen hasta sayısı ve tüm muayene oranları içerisindeki payı yıllar içerisinde büyük artış göstermiştir. Bu istatistikler ışığında gerek acil servis hizmetlerinin gerekse poliklinik hizmetlerinin günün şartlarına göre planlanması uygun olacaktır.

The aim of the study is to determine the effect of glass ceiling, power distance, occupational self-efficacy and role model in the career advancement of women. The study is designed in quantitative research design and relational survey model. The research is carried out on female employees working in public hospitals of Ordu Provincial Directorate of Health. The data collection process is collected between 2 March 2022 and 11 April 2022 through convenient sampling method. The sample consists of 324 female health employees. Data is analyzed using SPSS 25 and PROCESS macro statistics software. The relationship among the variables is identified by using correlation and regression analysis. Correlation and regression analysis are applied to determine the relationship between the variables. The research findings reveal positive relationships between female employees' career advancement, and occupational self-efficacy and role model. The findings also exhibit that there are negative relationships between female employees' career advancement, and glass ceiling and power distance. In addition, the findings show that there is a positive relationship between the glass ceiling perception of female employees and power distance, but a negative relationship with the role model. On the other hand, no significant relationship is found between the glass ceiling perception of female employees and occupational self-efficacy. According to the results of the mediation test conducted through the PROCESS macro, the glass ceiling perception of female employees has a mediating effect in the relationship between career advancement and, role model and power distance. The aim of this study is to examine the perceptions of patients on the concept of patient-friendliness in the provision of health services in hospitals and some variables that affect it. The population of the study consists of individuals over the age of 18 who have benefited from health services from any health institution in the last year, and the sample consists of 386 participants who volunteered to participate in the survey. Patient-Friendly Scale and Personal Information Form were used to collect the data of the study. T test and ANOVA test were used in the analysis of the data. There is a significant difference between the preferred hospital type in terms of trust and compassionate communication. There is a difference in the dimension of compassionate communication compared to the public hospital in individuals' preference for a private hospital. Individuals who prefer university hospitals have a higher level of trust than those who prefer public hospitals. The patient-friendliness of the hospitals in the provision of health services is significant in the hospital preference of the individuals. Considering that individuals take into account certain criteria in choosing a hospital while receiving health care, it is concluded that their level of knowledge and awareness about health care has increased. Although public hospitals are more preferred, it is seen that they do not meet the needs of individuals in terms of compassionate communication and trust. It can be concluded that patients focus more on the quality of health services rather than the duration of health care provided. [ABSTRACT FROM AUTHOR]
