Amaç: Talasemi major (TM) hastalarında selenyum düzeyi ve glutatyon peroksidaz 3 (GPX3) aktivitesinin ölçüldüğü çalışmalar mevcuttur. Ancak, selenyumun depo ve taşınmasında görevli olan Selenoprotein P (SePP)'nin talasemi hastalarında çalışıldığı bir çalışmaya rastlanılamamıştır. Bu çalışmanın amacı, talasemi major hastalarının tiroid fonksiyonları ile SePP ve GPX3 düzeylerini karşılaştırarak selenoproteinlerin tiroid fonksiyonlarına etkisinin olup olmadığını değerlendirmektir.Yöntem: Bu çalışmaya 80 olgu (40 kontrol, 40 TM) dahil edildi. Kontrol ve hasta gruplarındaki GPX3 ve Selenoprotein P konsantrasyonları sandwich ELISA yöntemi ile ölçüldü. Hemogram Advia 120 cihazı ile ölçüldü. Ferritin, T4, T3 ve TSH immunolojik yöntemle çalışıldı. Demir Advia Chemistry XPT cihazı ile spektrofotometrik olarak ölçüldü. Anti-TPO ve Anti-hTG Immulite 2000 cihazında immunoassay yöntem ile çalışıldı. İdrarda iyot spektrofotometrik olarak ölçüldü.Bulgular: Talasemi major grubunun SePP konsantrasyonu kontrol grubundan daha yüksek bulundu. Talasemi major hastalarının GPX3 konsantrasyonları ile kontrol grubundaki bireylerin GPX3 konsantrasyonları arasında istatistiksel olarak anlamlı bir fark bulunamadı. Tüm bireylerin ferritin konsantrasyonu ile serbest T4 ve T3 konsantrasyonları arasında negatif korelasyon bulundu. Tüm bireylerin GPX3 konsantrasyonları ile serbest T4 ve serbest T3 konsantrasyonları arasında negatif korelasyon bulundu. Sağlıklı bireylerde SePP konsantrasyonu ile TSH arasında negatif korelasyon bulundu.Sonuç: Bu çalışma, talasemi major hastalarında SePP ve GPX3 konsantrasyonlarının birlikte ölçüldüğü ilk araştırmadır. Çalışmamızda, SePP düzeyleri TM hastalarında kontrol grubuna göre daha yüksek olarak bulundu ancak grupların GPX3 düzeyleri arasında fark bulunamadı. Demir birikimi ile tiroid hormonları arasında ilişki olabileceği gösterildi. Tiroid hormonları ile selenoproteinler arasında bulunan korelasyonlar selenyumun tiroid fonksiyonları için gerekli olduğunu gösterebilir. Talasemilerde SePP'nin tiroid metabolizmasındaki rolünün aydınlatılabilmesi için detaylı çalışmaların yapılması gerekmektedir. However, selenoprotein P (SePP), which is responsible for the storage and transport of selenium, has not been studied in thalassemia patients. The aim of this study is to compare the thyroid functions of the thalassemia major patients with the levels of SePP and GPX3 and to evaluate the effect of selenoproteins on thyroid function.Methods: Eighty patients (40 controls, 40 TM) were included in this study. GPX3 and Selenoprotein P concentrations in the control and patient groups were measured by sandwich ELISA. The hemogram was measured with the Advia 120 device. Ferritin, T4, T3 and TSH samples were analyzed by using immunological method. Iron was measured spectrophotometrically using Advia Chemistry XPT. Anti-TPO and Anti-hTG were studied by immunoassay method using the immulite 2000 device. Iodine was measured spectrophotometrically in urine.Results: SePP concentration of thalassemia major group was higher than control group. There was no statistically significant difference between the GPX3 concentrations of the TM patients and the GPX3 concentrations in the control group. A negative correlation was found between ferritin concentrations of all individuals and concentrations of free T4 and T3. A negative correlation was found between the GPX3 concentrations of all subjects and free T4 and T3 concentrations. There was a negative correlation between SePP and TSH concentrations in healthy subjects.Conclusion: This is the first study in which the concentrations of SePP and GPX3 were measured together in thalassemia patients. In our study, SePP levels were higher in TM patients than in the control group but there was no difference in GPX3 levels of the groups. It was shown that there may be a relationship between iron overload and thyroid hormones. Correlations between thyroid hormones and selenoproteins may indicate that selenium is necessary for thyroid functions. Diabetes Mellitus (DM) genetik ve çevresel faktörlere bağlı kompleks bir hastalıktır. Selenoprotein P (SEPP) DM'nin etiyopatogenezinde önemli bir rol oynamaktadır. Yapılan çalışmalar SEPP'nin glikoz metabolizmasının ve yeni bir hepatokin olarak insülin duyarlılığının düzenlenmesinde önemli bir rol oynadığı gösterilmiştir. Bu çalışmada amaç, Selenoprotein P1'in (SEPP1) yaygın varyasyonlarının Gestasyonel Diabetes Mellitus (GDM) riskini değiştirip değiştirmediğini saptamaktır. Buna bağlı olarak aynı coğrafik bölgeden 72 GDM'li gebe ve 64 sağlıklı gebe çalışmaya dahil edildi. SEPP1 geninin polimorfizmini (rs3877899) belirlemek üzere allel-özgü Polimeraz Zincir Reaksiyonu (ASPCR) analizi kullanıldı. Gestasyonel diyabetik gebe grubunda açlık glikozu, insülini, İnsülin direnci testi (HOMA IR testi), Hemoglobin A1c (HbA1c), total kolesterol düzeyleri ve fetal ağırlık sağlıklı gebe kadın gruba göre daha yüksek bulundu. AA, GA ve GG genotiplerinin frekansları, GDM'li gebelerde sırasıyla % 28, % 43 ve% 29, sağlıklı gebelerde % 24, % 50 ve % 26 olarak saptandı. Bulgularımız, SEPP1 genotiplerinin ve alellerinin dağılımının, GDM'si olan ve olmayan kişiler arasında anlamlı farklılık göstermediğini ortaya koymuştur (p> 0.05). SEPP, glikoz metabolizması ve insülin duyarlılığının düzenlenmesinde anahtar rol oynamasına rağmen, SEPP1 polimorfizmi popülasyonumuz da GDM oluşumunu değiştirmemiştir. GDM etiyopatogenezinde farklı mekanizmalar yer alabilir. Bununla birlikte, daha geniş popülasyonlarda yapılan daha ileri çalışmalara gereksinim vardır. SEPP1 (rs3877899) polimorfizminin ise Türk kadınlarında gestasyonel diyabet gelişiminde rolü bulunmadığı düşünülmektedir. Selenoprotein P (SeP) appears to play a key role in the etiopathogenesis of DM. Recently, it has been demonstrated that SeP played an important role in glucose metabolism and the regulation of insulin sensitivity as a new hepatokine. The purpose of this study was to determine whether common variations in selenoprotein P1 (SEPP1) alter the risk of Gestational Diabetes Mellitus (GDM). 72 pregnant women with GDM and 64 healthy pregnant women from the same geographic region were included in the study. Allelespecific Polymerase Chain Reaction (ASPCR) analysis was used to identify polymorphisms of the SEPP1 gene (rs3877899). Results: We found that fasting glucose, insulin, HOMA-IR, HbA1c, total cholesterol levels and weight of fetus were higher in gestational diabetic pregnants compared group to healthy pregnant women group. The frequencies of the AA, GA and GG genotypes were found as 28 %, 43 % and 29 % in pregnant women with GDM and 24 %, 50 % and 26 % in healthy pregnant women, respectively. Our results indicated that the distribution of the SePP1 genotypes and alleles did not differ signifi cantly among subjects with or without GDM (p>0.05). Although SeP plays a key role in glucose metabolism and the regulation of insulin sensitivity, the SEPP1 polymorphism did not changed occurrence of GDM in our population. Different mechanisms may be involved in etiopathogenesis of GDM. However, it should be clarifi ed with further studies in larger populations.
