Resumen La Gangrena de Fournier es una forma poco frecuente de fascitis necrotizante que puede llegar a ser fatal si no es tratada adecuadamente. Las áreas habitualmente afectadas son el periné y los genitales externos y con frecuencia requieren amplios desbridamientos quirúrgicos que pueden dejar defectos de cobertura extensos. La reconstrucción del tronco del pene requiere una reparación libre de tensión que permita mantener la capacidad eréctil del tejido y evite la formación de una cicatriz retráctil. La viabilidad de la técnica empleada, su resultado estético y el grosor del pene reconstruido son otros factores a tener en cuenta. Las principales opciones para proporcionar cobertura son los colgajos pediculados locales y los injertos de piel de espesor parcial o total. El colgajo fasciocutáneo pudendo se emplea comúnmente en la cirugía reparadora de periné y vagina pero, de acuerdo con lo observado en la literatura, apenas existen 2 casos descritos de su uso en reconstrucción del tronco del pene, con variaciones respecto al diseño tradicional del colgajo. Describimos una secuela de Gangrena de Fournier en la que logramos con éxito la cobertura del tronco del pene gracias a una novedosa modificación en el diseño del colgajo fasciocutáneo pudendo.

Inadequate tissue blood supply (e.g., in a wound or a poorly vascularised graft) can result in tissue ischemia and necrosis. As revascularization is a slow process relative to the proliferation of bacteria and the onset and spread of tissue necrosis, extensive tissue damage and loss can occur. Necrosis can spread rapidly, and treatment options are limited such that loss of tissue in ischemic tissue following necrosis onset is considered unavoidable and irreversible.Oxygen delivery from biomaterials exploiting aqueous decomposition of peroxy-compounds has shown some potential in overcoming the supply limitations caused by quite short oxygen diffusion distances in tissues by creating higher concentration gradients than can be attained by air saturated solutions or by distributing oxygen supply throughout a scaffold or construct by using particulate formulations. These have found application in tissue preservation, bioinks, creation of 3D tissue analogues etc. In preclinical models among the more exciting reports was a single study demonstrating reduction of ischemic skin necrosis albeit only short term using short term sub dermal delivery of oxygen below ischemic skin flaps. To explore this effect further, we developed an implantable solid peroxide-biomaterial based system with reduced hydrogen peroxide release by virtue of incorporation of minerals to catalytically decompose it in a much longer flap than examined previously. Blood flow in this flap reduced from essentially normal to essentially zero, along its 9cm length. Without treatment ∼50% of the total flap was necrotic in 2-4 days. In both groups, complete necrosis in the distal third of the flap with no observable flood flow was observed. But in the middle low blood flow region of the flap, treatment did prevent necrosis. This study indicated that subdermal oxygen delivery alone cannot completely mitigate dermal necrosis if no blood flow is present, but it could improve the survival of partially tissue at least in the short term which could find application to augment conventional treatments or to gain time until surgical intervention.

Perineal carcinoma of unknown primary origin (CUP) is a rare entity and represents a diagnostic and therapeutic challenge. These tumors may respond well to a combination of surgical resection, when feasible, local radiotherapy, and platinum-based systemic chemotherapy. A 67-year-old male patient consulted for urinary discomfort associated with perineal abscess. The perineal abscess was drained, and the patient was diagnosed with perineal carcinoma of unknown origin. Computed tomography (CT) scan shows a large perineal mass that involves both corpora cavernosa and corpus spongiosum. CT and bone scan excluded the metastatic spread of the disease. Inguinal lymph nodes were not identified. The surgical treatment consisted of two different phases. Phase I: the perineal mass exeresis with total penectomy. Phase II: perineal neourethrostomy with double oral mucosa graft. Coverage of the perineal defect and morphological reconstruction of the penis with fasciocutaneous and gracilis flap. The surgical intervention lasted for nine hours and the estimated blood loss was 500cc. The patient was discharged twelve days after the intervention. Later he received adjuvant treatment with immunotherapy (Cemiplimab) because was not subsidiary to treatment with platinum for renal failure. The patient died after nine months of follow-up due to pulmonary and abdominal metastatic spread.

After sagittal split osteotomy, the mandibular distal and proximal fragments do not always align themselves passively to one another, resulting in bony interferences and subsequent anomalous settlement of the condyles. Predicting these interferences could be an important ancillary procedure for avoiding intra- and postoperative surgical complications, rendering orthognathic surgery more effective and safer. This study evaluated the relevance of virtual surgical planning in assessing the displacement of the proximal segments after virtual distal segment repositioning, for predicting bony interferences between the segments and thus avoiding related intra- and postoperative surgical complications. The presence of interferences between the distal and proximal segments was compared between virtually predicted (computer-assisted simulation surgery, Dolphin software) and real cases in 100 consecutive patients diagnosed with dentofacial deformities who underwent orthognathic surgery with mandibular repositioning (using a short lingual osteotomy (SLO)). The results indicated that clockwise rotation of the mandible was the mandibular movement most prone to segment interference. Furthermore, virtual planning was sensitive (100%) but had low specificity (51.6%) in predicting proximal and distal segment interferences. This low specificity was due to the software-based automated design of the mandibular osteotomy, where the length of the distal segment was longer than the real SLO, and the mandibular ramus sagittal split was located just behind Spix's spine. Thus, more precise simulated osteotomies are needed to further validate the accuracy of virtual planning for this purpose.

In the late years, the use of acellular dermal matrix (ADM) has extended rapidly in the field of breast reconstruction. However, with the follow-up visits, we discovered some deformities on the lateral part of the lower pole, due to the folding of the ADM. The aim of this letter is to share some tricks developed in our clinical practice that would permit the surgeon to avoid getting these irregularities in the aesthetic result of breast reconstruction with ADM. ResumenLas lesiones en el dorso los dedos de la mano, debidas a traumatismos, infecciones o quemaduras, son causa frecuente de exposición de estructuras nobles óseas, articulares y/o tendinosas. El dorso de la mano ofrece la posibilidad de cobertura donante con una gran similitud de calidad de tejido. Por ello, una opción sencilla y práctica es la cobertura con un colgajo tipo Propeller de perforantes de la arteria intermetacarpiana dorsal, conocido en la literatura médica anglosajona como “Quaba flap,” y sus variantes. Presentamos una serie de 3 pacientes, con afectación y exposición noble del dorso de los dedos, desde F1, PIP Y F2, ocurridos tras infección, quemadura y traumatismo, respectivamente, que se vieron beneficiados de esa técnica, empleando, en cada caso, un colgajo de distinto espacio intermetacarpiano dorsal. Especificamos los detalles de la técnica quirúrgica con referencias anatómicas.
