Contains fulltext : 287226.pdf (Publisher’s version ) (Closed access) The Female Sexual Subjectivity Inventory (FSSI) is a 20-item self-report measure to assess whether women feel entitled to express and satisfy their sexual needs. Despite the relevance of sexual subjectivity for female sexual wellbeing, the FSSI has not yet received scientific attention in Italy. For this reason, the purpose of the present study was to evaluate the psychometric properties of the Italian version of the FSSI. A community sample of 1404 women (mean age = 22.25±3.42 years; age range 18-35 years) was recruited in Italy. Exploratory factor analysis revealed that the Italian FSSI had a five-factor structure, similar to the original version, named Sexual body-esteem, Sense of entitlement to sexual pleasure from self, Sense of entitlement to sexual pleasure from partner, Self-efficacy in achieving sexual pleasure, and Sexual self-reflection. The confirmatory factor analysis indicated that the five-factor structure of the Italian FSSI had good fit. The Italian FSSI showed good internal reliability (Cronbach's alphas for the five factors were .74, .77, .77, .85, and .76 respectively) and good convergent validity. Overall, results indicate that the Italian FSSI has good psychometric properties and can be used to assess sexual subjectivity among young Italian women. 16 p.

Abstract

Context:Optical interferometry is at a key development stage. ESO's VLTI has established a stable, robust infrastructure for long-baseline interferometry for general astronomical observers. The present second-generation instruments offer a wide wavelength coverage and improved performance. Their sensitivity and measurement accuracy lead to data and images of high reliability. Aims:We have developed MATISSE, the Multi AperTure mid-Infrared SpectroScopic Experiment, to access high resolution imaging in a wide spectral domain and explore topics such: stellar activity and mass loss; planet formation and evolution in the gas and dust disks around young stars; accretion processes around super massive black holes in AGN. Methods:The instrument is a spectro-interferometric imager covering three atmospheric bands (L,M,N) from 2.8 to 13.0 mu, combining four optical beams from the VLTI's telscopes. Its concept, related observing procedure, data reduction and calibration approach are the product of 30 years of instrumental research. The instrument utilizes a multi-axial beam combination that delivers spectrally dispersed fringes. The signal provides the following quantities at several spectral resolutions: photometric flux, coherent fluxes, visibilities, closure phases, wavelength differential visibilities and phases, and aperture-synthesis imaging. Results:We provide an overview of the physical principle of the instrument and its functionalities, the characteristics of the delivered signal, a description of the observing modes and of their performance limits. An ensemble of data and reconstructed images are illustrating the first acquired key observations. Conclusion:The instrument has been in operation at Cerro Paranal, ESO, Chile since 2018, and has been open for science use by the international community since April 2019. Abstract

According to evidence-based medicine (EBM), physicians must be able to assess and understand scientific evidence generated from biomedical research. Among the many statistical methods involved in the proof of the evidence, an essential area concerns how to select and interpret diagnostic tests to confirm or exclude a diagnosis. Rather surprisingly, these statistical methods are not always correctly reported or satisfactorily explained in some EBM reference books, which leads to misunderstandings and incorrect analyses of published studies in the biomedical literature. In this paper, the authors consider the methodology for performing diagnostic test studies and a correct analysis of the diagnostic test results by explaining it for a clinical audience with a working example.

Abstract

The young stellar object MWC 297 has been observed with the VLT interferometer equipped with the AMBER instrument. MWC 297 has been spatially resolved in the continuum with a visibility of 0.50 as well as in the Br gamma emission line where the visibility decrease to a lower value of 0.33. This change in the visibility with the wavelength can be interpreted by the presence of an optically thick disk responsible for the visibility in the continuum and of a stellar wind traced by Br gamma and whose apparent size is 40% larger. We validate this interpretation by building a model of the stellar environment that combines a geometrically thin, optically thick accretion disk model consisting of gas and dust, and a latitude-dependent stellar wind outflowing above the disk surface. The continuum emission and visibilities obtained from this model are fully consistent with the interferometric AMBER data. They agree also with existing optical, near-infrared spectra and other broad-band near-infrared interferometric visibilities. A picture emerges in which MWC 297 is surrounded by an equatorial flat disk that is possibly still accreting and an outflowing wind which has a much higher velocity in the polar region than at the equator. Abstract

Part II of this work addresses the use of likelihood ratios for the calculation of a positive predictive value (PPV) and a negative predictive value (NPV) for patient diagnosis. These two values are the most useful indices in the application of a diagnostic test in order to make a diagnosis for a patient. Furthermore, the relevance of sample size is also addressed by providing background theory and pertinent tables. These basic topics for assessing the validity of a diagnostic test and for performing appropriate and valid case vs control studies are explained in a simple way for a clinical audience with a working example.

Abstract

The results of meta-analyses on the effectiveness of high positive end-expiratory pressure (PEEP) and prone positioning in acute lung injury (ALI)/acute respiratory distress syndrome (ARDS) are not consistent. In addition, the meta-analyses on the activated protein C in patients with sepsis combine trials with discordant results. Therefore, the aim of this paper was to give a critical review of these meta-analyses. All relevant meta-analyses were identified by a computerized search of PubMed using combinations of the following terms: acute lung injury, acute respiratory distress syndrome, positive end-expiratory pressure, mechanical ventilation, prone position, drotrecogin, activated protein C, sepsis, and septic patients. A high level of PEEP and prone ventilation was shown to reduce the mortality in patients with severe acute hypoxemic respiratory failure. Although the evidence for the efficacy of activated protein C is not conclusive, it should be considered in patients that are at a high risk for death without any contraindications related to bleeding risk. Meta-analysis models can be very useful for clinical decisions if they include all of the similar papers on a medical topic and are correct from the methodological point of view; however, these results must be checked by a careful and well-informed reader.

Abstract

Background Nivolumab has shown a survival benefit compared with docetaxel as second-line treatment for patients with previously treated advanced squamous non-small cell lung cancer (NSCLC) in a randomized phase III trial. The experiences of patients and physicians in routine clinical practice are often different from those in a controlled clinical trial setting. We present data from the entire Italian cohort of patients with squamous NSCLC enrolled in a worldwide nivolumab NSCLC expanded access program. Patients and Methods Patients with pretreated advanced squamous NSCLC received nivolumab 3 mg/kg every 2 weeks for up to 24 months. Safety was monitored throughout; efficacy data collected included objective tumor response, date of progression, and survival information. Results The Italian cohort comprised 371 patients who received at least one dose of nivolumab. In the overall population, the objective response rate (ORR) was 18%, the disease control rate (DCR) was 47%, and median overall survival (OS) was 7.9 months (95% confidence interval 6.2–9.6). In subgroup analyses, ORR, DCR, and median OS were, respectively, 17%, 48%, and 9.1 months in patients previously treated with two or more lines of therapy (n = 209) and 8%, 40%, and 10.0 months in patients treated beyond disease progression (n = 65). In the overall population, the rate of any-grade and grade 3–4 adverse events was 29% and 6%, respectively. Treatment-related adverse events led to treatment discontinuation in 14 patients (5%). There were no treatment-related deaths. Conclusion To date, this report represents the most extensive clinical experience with nivolumab in advanced squamous NSCLC in current practice outside the controlled clinical trial setting. These data suggest that the efficacy and safety profiles of nivolumab in a broad, real-world setting are consistent with those obtained in clinical trials. Implications for Practice Nivolumab is approved in the U.S. and Europe for the treatment of advanced non-small cell lung cancer (NSCLC) after failure of prior platinum-based chemotherapy. In this cohort of Italian patients with previously treated, advanced squamous NSCLC treated in a real-world setting as part of the nivolumab expanded access program, the efficacy and safety of nivolumab was consistent with that previously reported in nivolumab clinical trials.

Abstract

Purpose Small-cell lung cancer (SCLC) is increasingly diagnosed in elderly patients, who are at higher risk of treatment-related morbidity and mortality. We conducted a randomized two-stage phase II study to assess the therapeutic index of two different platinum/etoposide regimens, attenuated-dose (AD) and full-dose (FD) plus prophylactic lenograstim. Patients and Methods SCLC patients older than 70 years were randomized to receive four courses of cisplatin 25 mg/m2 on days 1 and 2, and etoposide 60 mg/m2 on days 1, 2, and 3 every 3 weeks (AD); or cisplatin 40 mg/m2 on days 1 and 2, and etoposide 100 mg/m2 on days 1, 2, and 3 every 3 weeks, plus lenograstim 5 mg/kg days 5 through 12, every 3 weeks (FD). A combined primary end point named therapeutic success (TS), which took into account activity, toxicity, and compliance, was used. Results Ninety-five patients were enrolled. Seventy-five percent and 72% of the patients in the AD and FD arms, respectively, completed the treatment as per protocol. Response rate was 39% and 69% in the AD and FD arms, respectively, and 1-year survival probability was 18% and 39%, respectively. Treatment was well tolerated in both groups, with no grade 3 to 4 myelotoxicity in the AD arm, and 12% myelotoxicity in the FD arm. Overall, the observed TSs were 10 (36%) of 28 patients and 42 (63%) of 67 patients for AD and FD treatments, respectively. Conclusion In elderly patients with SCLC a full-dose cisplatin/etoposide regimen combined with prophylactic lenograstim is active and feasible, while attenuated doses of the same regimen are associated with a poor therapeutic outcome.
