General Medicine - Abstract

- Abstract
One effort to improve nutritional status is consuming nutritious food and nutrition education for children to instill healthy eating habits from an early age. The purpose of this study was to analyze the effectiveness of providing snacks on the nutritional status and nutritional intake of adolescents. This type of research was a quasi-experimental design with a one-group pre-posttest design without a control group. The research subjects were adolescents aged 11-14 years. Snacks are given for 1 month that meets ± 20% energy and ± 8% protein and ± 15% fat. T-test to analyze the nutritional status of adolescents and nutrient intake before and after giving the distraction food. The results of this study showed that providing distracting food could increase body mass index (p-value=0,000), student weight (p-value=0,000), intake of protein (p-value=0,000), vitamin C (p-value=0,0008), and zinc (p-value=0,0004), but height, carbohydrate intake, and iron intake had no significant effect (p-value>0,05). This study concluded that the provision of snack food effectively increased students' nutritional knowledge, student weight, and intake of protein, vitamin C, and zinc, but height and body mass index, as well as carbohydrates and iron, did not increase significantly. The suggestion from this study is that distraction for adolescents is highly recommended to improve nutritional status so schools should be able to make a policy in providing distracting food as a program to improve adolescent nutrition at school.

General Medicine - Abstract

Objective: Today, stress monitoring on wearable devices is challenged by the tension between high-detection accuracy and battery lifetime driven by multimodal data acquisition and processing. Limited research has addressed the classification cost on multimodal wearable sensors, particularly when the features are cost-dependent. Thus, we design a Cost-Aware Feature Selection (CAFS) methodology that trades-off between prediction-power and energy-cost for multimodal stress monitoring. Methods: CAFS selects the most important features under different energy-constraints, which allows us to obtain energy-scalable stress monitoring models. We further propose a self-aware stress monitoring method that intelligently switches among the energy-scalable models, reducing energy consumption. Results: Using CAFS methodology on experimental data and simulation, we reduce the energy-cost of the stress model designed without energy constrains up to 94.37%. We obtain 90.98% and 95.74% as the best accuracy and confidence values, respectively, on unseen data, outperforming state-of-the-art studies. Analyzing our interpretable and energy-scalable models, we showed that simple models that use only heart rate (HR) or skin conductance level (SCL), confidently predict stress for HR >93.30 BPM and non-stress for SCL

24. Pension Policy and the State
- Author

Camila Arza
- Abstract
Over the 20th century, the state has acquired a prominent role in the administration of old-age pensions in Latin America, expanding legal coverage to the majority of the employed population. However, gaps in effective coverage remained large as a result of the high incidence of informal work. Over the last two decades of the 20th century, the almost exclusive role of the state in old-age protection was deeply challenged. Starting with Chile in 1981, several countries implemented structural pension reforms that shifted to individual accounts and private pension administration, thus completely reshaping the public-private balance in the pension system. Private schemes were presented as a new and more effective way to manage old-age security and promote economic growth. But these schemes also faced shortcomings, including high administrative costs, financial market risks, and problems of coverage, equity, and adequacy. Over the 2000s, as governments with a more progressive policy agenda took office in a context of favorable macroeconomic conditions, the state gained ground in the administration of old-age security. This more active role of the state has strengthened public pensions compared to two or three decades back, with better instruments for basic protection, new systems of administrative registry of contributors and pensioners, and broader coverage of the population across the territory.

- Abstract
This chapter presents the Latin American section and provides an overview of its main findings. It discusses the governance and management for social policy in a region with high levels of inequality, informal employment, low fiscal efforts, and major weaknesses in terms of social risks and state capacity. Besides these similarities, wide variations also exist in social policy regimes across countries in terms of governance, administrative arrangements, levels of decentralization, and mixes of public services and private services. Innovations in social policy over the 21st century have produced a range of mixed systems that are not always consistent, distributionally sound, or sustainable. The chapters in this section contribute to understanding these trends and their implications for the governance and management of social policy in Latin America.

26. The Slow and Reluctant Development of Social Citizenship in Latin America
- Author
-
Fernando Filgueira, Camila Arza, and Merike Blofield
- Abstract
This chapter puts the region’s social protection policy models into historical, political, and social context. It distinguishes four major stages in Latin American welfare state models, clearly associated with four stages in Latin American state-building: the minimalist “welfare” state of the primary export period; the contribution-based welfare state under the import substitution model; the turn to the reluctant welfare state and the introduction of the market into social affairs that characterized the Washington Consensus era; and finally, the expansionary stage of post-Washington Consensus until the mid-2010s. In the initial stages, the state role was minimal, focusing on primary education and boosting urban sanitation infrastructure. In the import substitution period, there was a strong push for modern social protection, tied firmly to protection for formal workers. However, informal labor markets and incomplete protection against a variety of risks produced a stratified model of social security with incomplete coverages. The liberal turn led to a focus on extreme poverty along with an increased reliance on market mechanisms in healthcare, education, and pensions. As the 20th century came to an end, pressures for more inclusive social policies promoted the expansion of coverage and benefits. The return and resilience of democracy, an export-oriented external stance, and the substantial economic growth of the first decade and a half of the 21st century were all factors that contributed to this new and still unstable social citizenship contract.

This study investigated the optimum transport condition for heart tissue to recover single-cell cardiomyocytes for future in-vitro or in-vivo studies. The heart tissues were obtained from removing excessive myocardium discharged during the repair surgery of an excessive right atrial hypertrophy due to a congenital disease. The transportation temperature studied was the most used temperature (4 °C) or the conventional condition, compared to a physiological temperature(37 °C). The heart tissues were transported from the operating theatre to the lab maintained less than 30 min consistently. Single-cell isolation was enzymatically and mechanically performed using collagenase-V (160 U/mg) and proteinase-XXIV (7-14 U/mg) following the previously described protocol. The impact of temperature differences was observed by the density of cells harvested per mg tissue, cell viability, and the senescence signals, identified by the p21, p53 and caspase-9 mRNA expressions. Results the heart tissue transported at 37 °C yielded significantly higher viable cell density (p 0.01) yielded viable cells significantly higher density (p 0.01) than the 4 °C; 2,335 ± 849 cells per mg tissue, and 732 ± 425 cells per mg tissue, respectively. The percentage of viable cells in both groups showed no difference. Although the 37 °C group expressed the apoptosis genes such as p21, p53 and caspase9 by 2.5-, 5.41-, 5-fold respectively (p 0.05). Nonetheless, the Nk×2.5 and MHC genes were expressed 1,7- and 3.56-fold higher than the 4 °C. and the c-Kit+ expression was 17.56-fold, however, statistically insignificant. Conclusion When needed for single-cell isolation, a heart tissue transported at 37 °C yielded higher cell density per mg tissue compared to at 4 °C, while other indicators of gene expressions for apoptosis, cardiac structural proteins, cardiac progenitor cells showed no difference. Further investigations of the isolated cells at different temperature conditions towards their proliferation and differentiation capacities in a 3-D scaffold would be essential.

Compared to mechanical and bioprosthetic valves which have apparent disadvantages, this technique has been an alternative with a promising safety and efficacy result. A comprehensive search was carried out in 4 databases (Pubmed, Cochrane Library, Proquest, Scopus) from February to March 2021 using search terms “autologous pericardium”, “aortic valve replacement”, and “aortic valve reconstruction”. Outcomes measured in this study were mortality, freedom of operation, thromboembolic and endocarditis event, and echocardiography finding. Risk bias of all studies was measured using MINORS criteria. A total of 12 studies involving 1427 subjects were included. The mean age was 64.95 years and 52.1% subjects were male. Mortality due to cardiac and noncardiac cause was 1.75%. Reoperation was needed in 1.12% subjects. Thromboembolic and endocarditis events occurred in 0.21% and 0.91% respectively. All studies reported lower average peak pressure gradient after surgery. Aortic valve replacement using autologous pericardium has a tolerable safety and efficacy.

- Abstract
A t(9;11)(p22;q23) translocation produces the MLL-AF9 fusion protein, which is found in up to 25% of de novo AML cases in children. Despite major advances, obtaining a comprehensive understanding of context-dependent MLL-AF9-mediated gene programs during early hematopoiesis is challenging. Here, we generated a human inducible pluripotent stem cell (hiPSC) model with doxycycline dose-dependent MLL-AF9 expression. We exploit MLL-AF9 expression as an oncogenic hit to uncover epigenetic and transcriptomic effects on iPSC-derived hematopoietic development and the transformation into (pre-)leukemic states. In doing so, we observed disruption of early myelomonocytic development and expansion of a CD34+ early hematopoietic progenitor compartment upon MLL-AF9 activation. In agreement, we identified gene profiles consistent with primary MLL-AF9 AML and uncovered highly confident MLL-AF9-associated core genes that faithfully represent primary MLL-AF9 AML, including known and thus far unknown factors. Our system allows for careful chemically controlled and stepwise in vitro hiPSC-derived differentiation under serum-free and feeder-free conditions. For a disease that currently lacks effective precision medicine, our system provides a novel entry-point into exploring potential novel biomarkers and targets for personalized therapeutic strategies.

Earth can act as a transducer to convert ultralight bosonic dark matter (axions and hidden photons) into an oscillating magnetic field with a characteristic pattern across its surface. Here we describe the first results of a dedicated experiment, the Search for Non-Interacting Particles Experimental Hunt (SNIPE Hunt), that aims to detect such dark-matter-induced magnetic-field patterns by performing correlated measurements with a network of magnetometers in relatively quiet magnetic environments (in the wilderness far from human-generated magnetic noise). Our experiment constrains parameter space describing hidden-photon and axion dark matter with Compton frequencies in the 0.5-5.0 Hz range. Limits on the kinetic-mixing parameter for hidden-photon dark matter represent the best experimental bounds to date in this frequency range.

Cardiovascular disease is the second highest cause of death across the globe. Myocardial infarction is one of the heart diseases that cause permanent impairment of the heart wall leads to heart failure. Cellular therapy might give hope to regenerate the damaged myocardium. Single cells isolated from an excess heart tissue obtained from the correction of the right ventricular hypertrophy in patients with Tetralogy of Fallot for future heart study were investigated. Methods: Once resected, the heart tissues were transported at 37 °C, in Dulbecco's Modified Eagle's medium/ DMEM (4.5 g.L−1, antibiotic–antimycotic 3x, PRP10% (v/v)), to reach the lab within 30 min, weighted and grouped into less than 500 mg and more than 1000 mg (n = 4). Each sample was digested with 250 U.mL−1 Collagenase type V and 4U.mL−1 Proteinase XXIV in the MACS™ C-tube (Milltenyi, Germany), then dissociated using the MACS™ Octo Dissociator with Heater (Milltenyi, Germany) for 60 min at 37 °C. Results: All cells isolated were rod-shaped cells; viability was up to 90%. The cell density obtained from the 500 mg group were 4,867 ± 899 cells.mg−1 tissue weight, significantly higher compared to the 1,000 mg group; had 557 ± 490 cells.mg−1 tissue weight (mean of (n = 3) ± 95% C.l). The isolated cells were analyzed using FACs BD Flowcytometer, expressed cTnT + 13.38%, PECAM-1 + /VCAM-1- 32.25%, cKit + 7.85%, ICAM + 85.53%, indicating the cardiomyocyte progenitor cells. Conclusion: Cardiomyocytes taken from the wasted heart tissue might be a candidate of cardiomyocytes source to study interventions to the heart as it contained up to 13.38% cardiomyocytes, and 32.25% of cardiac progenitor cells. Moreover, perhaps when cardiac cell therapy needs autologous cardiomyocytes, less than 500 mg tissue weight can be considered as sufficient.

This study was to determine the influence of intellectual capital and corporate social responsibility on firm value. This research method is quantitative. The population in this study is the mining sector companies on the Indonesia Stock Exchange from 2018 to 2020. The sample of this study was determined by purposive sampling. Data analysis techniques for hypothesis testing using multiple regression analysis. The results show that intellectual capital has no significant effect as well as corporate social responsibility on firm value. This is due to the low interest of investors in other aspects outside of physical and financial assets. investors are more focused on maximizing profit as a short term goal. it can be suggested that: (1) Companies should increase their assets to increase company size so that companies have more financial funds to manage their IC performance and create more added value, and (2) For future researchers, it is recommended to increase the number of sample companies, adding other variables and using other proxies in further research.

Cognitive workload affects operators' performance principally in high-risk or time-demanding situations and when multitasking is required. An online cognitive workload monitoring system can provide valuable inputs to decision-making instances, such as the operator's state of mind and resulting performance. Therefore, it can allow potential adaptive support to the operator. This work presents a new design of a wearable embedded system for online cognitive workload monitoring. This new wearable system consists of, on the hardware side, a multi-channel physiological signals acquisition (respiration cycles, heart rate, skin temperature, and pulse waveform) and a low-power processing platform. Further, on the software side, our wearable embedded system includes a novel energy-aware bio-signal processing algorithm. We also use the concept of application self-awareness to enable energy-scalable embedded machine learning algorithms and methods for online subjects' cognitive workload monitoring. Our results show that this new wearable system can continuously monitor multiple bio-signals, compute their key features, and provide reliable detection of high and low cognitive workload levels with a time resolution of 1 minute and a battery lifetime of 14.58 h in our experimental conditions. It achieves a detection accuracy of 76.6% (2.6% lower than analogous offline computer-based analysis) with a sensitivity of 77.04% and a specificity of 81.75%, on a simulated drone rescue mission task. Moreover, by applying our self-aware monitoring to exploit different energy-scalable modes, we can increase battery lifetime by 51.6% (up to 22.11 hours) while incurring an insignificant accuracy loss of 1.07%.

Infectious Diseases ,Virology - Abstract
The emergence of SARS-CoV-2 has caused worldwide pandemic of COVID-19. Infection is difficult to diagnose early as some patients remain asymptomatic and may carry this virus to other people. Currently, qRT-PCR is the widely accepted mode for detection. However, the need for sophisticated instrument and trained personnel may hinder its application, especially in remote and facility-lacking areas. Reverse transcription loop-mediated isothermal amplification (RT-LAMP) may serve as a potential approach for detection of SARS-CoV-2 as the resources needed in its application is far less complex than those of qRT-PCR. Herein, we evaluated RT-LAMP based analytical method (COVIDNow), relative to qRT-PCR, in detecting SARS-CoV-2 by using 63 clinical respiratory samples. Based on our finding, COVIDNow exhibited sensitivity and specificity values of 87.5% and 80.6%, respectively. Taken together, RT-LAMP based detection of SARS-CoV-2 by utilizing COVIDNow might serves as a valuable diagnostic tool in the management of global COVID-19 pandemic condition.
