Supersaturation with respect to calcium salts (oxalate and phosphate) is the driving force leading to crystalluria and nephrolithiasis. High-molecular-weight urinary inhibitors are recently described molecules capable of altering the process of kidney stone formation. By inhibition of crystal nucleation, growth and aggregation and by inhibition of crystal interaction with tubular cells, these proteins efficiently prevent stone formation and retention in the urinary tract. But in spite of considerable efforts, characterization of these proteins is still under way. Besides the pathophysiology of risk factors for calcium salts supersaturation such as idiopathic hypercalciuria or hyperoxaluria, the renal involvement of protein inhibitors is the most exciting field in the comprehensive approach of nephrolithiasis, a disease that affects up to 10% of people in Western countries.

The control of the formation of crystals in biological fluids is one of the most exciting field of research involving both organic and biochemical areas. Many organisms have evolved mechanisms which minimize or avoid the effects of nucleation and crystal growth formation. One of the most important mechanism is the interaction of specific proteins, called inhibitors, with crystals which alters their habits and leads to their elimination. This article, focused on saliva, pancreatic juice and bile, reviews our present knowledge on the structure-function relationships existing between these proteins and their ability to inhibit the growth of different calcium salt crystals.

1. The influence of spiramycin coadministration on cyclosporin pharmacokinetics was studied in five renal transplant patients. The plasma concentrations of cyclosporin were measured both by non-specific radioimmunoassay (RIA) and high-performance liquid chromatography (h.p.l.c.). 2. The kinetics of cyclosporin were followed before treatment, and after 1 day and then 2 weeks of oral treatment with spiramycin (3 X 10(6) iu, twice daily). The main pharmacokinetic parameters (the area under the plasma drug concentration-time curve, the maximum plasma drug concentration and the time to reach it) obtained both by RIA and h.p.l.c. were not modified by spiramycin cotreatment after 1 day, nor after 2 weeks of spiramycin administration. Therefore, the pharmacokinetics of cyclosporin (parent drug and parent drug plus metabolites) are not influenced by the coadministration of spiramycin macrolide at therapeutic dosage. 3. Spiramycin may be preferable to other macrolide antibiotics known to interact with cyclosporin such as erythromycin or josamycin.
