Weighted shifts are an important concrete class of operators in linear dynamics. In particular, they are an essential tool in distinguishing variety dynamical properties. Recently, a systematic study of dynamical properties of composition operators on $L^p$ spaces has been initiated. This class of operators includes weighted shifts and also allows flexibility in construction of other concrete examples. In this article, we study one such concrete class of operators, namely composition operators induced by measures on odometers. In this paper, we explore the effects of excimer laser irradiation on heavily Aluminum (Al)-implanted silicon carbide (4H-SiC) layer. 4H-SiC layers were exposed to UV-laser radiation (308 nm, 160 ns), at different laser fluences and the effects of the laser exposure surface were evaluated from morphological, micro-structural and nano-electrical standpoints. Depending on the irradiation condition, significant near-surface changes were observed. Moreover, the electrical characteristics of the implanted layer, evaluated by means of transmission line method, gave a sheet-resistance of 1.62×104 kW/sq for the irradiated layer, linked to a poor activation of the p-type dopant and/or a low mobility of the carriers in the laser-modified 4H-SiC layer. This study can be useful for a fundamental understanding of laser annealing treatments of 4H-SiC implanted layers.

Cystic neutrophilic granulomatous mastitis is a rarely reported, benign, mastitis that is not associated with lactation. The pathophysiology is still unknown; however, there is often a relationship with Corynebacterium kroppenstedtii. Cystic neutrophilic mastitis can have deep seated tender nodules and cutaneous ulceration which can clinically resemble pyoderma gangrenous. It can be treatment refractory and can progress to a point where mastectomy is required. This case series reports two patients treated with adalimumab with remission of disease. One patient first received dapsone with remission of symptoms, but incompatible side effects resulted in discontinuation. Both dapsone and adalimumab appear to provide disease remission in patients with cystic neutrophilic granulomatous mastitis.

The National Cancer Institute of Aviano, Italy, recently launched a new program to educate the cancer population, bringing patients as close as possible to the world of physical activity during the cancer rehabilitation phase. A personalized assessment of the patient using the latest technologies allows to create a personalized electronic record of the state of physical performance to be shared among the patient, the staff at the institute (including the medical doctors and physiotherapists in charge), and the fitness instructors, who deal with of the recovery tasks. The outcome of the program can allow the development of a personalized rehabilitation path that includes physical fitness to be monitored remotely.

In questo articolo si descrivono e discutono le caratteristiche principali della lingua quechua relative alla distinzione tra ‘maschio’ e ‘femmina’. La prima parte del testo indica il modo in cui il quechua codifica questa distinzione attraverso il lessico, ponendo l’attenzione sui termini di parentela; la seconda parte mostra i sostantivi, aggettivi e verbi indicanti persone, comportamenti e abitudini considerati come tipicamente femminili o maschili. Sia la prima parte che la seconda sono vòlte a evidenziare alcuni fattori di tipo socio-culturale associati alla distinzione linguistica tra genere femminile e maschile.

The impact behavior of a carbon-Kevlar hybrid composite, widely used in sport car manufacturing, was evaluated. To highlight the hybridization effect, comparative analyses were performed with the basic CFRP laminate having the same lay-up. Tensile, bending and low velocity impact tests, followed by nondestructive inspections, highlighted that Kevlar first lamina hybridization leads to an increment in specific impact strength, up to 55%. To assess the most reliable technique to detect the impact damage, nondestructive evaluation was performed by pulsed thermography, phased array ultrasonic technique, computed tomography and digital radiography. Phased array ultrasonic technique can be considered the most appropriate technique.

After about 16 years since its first outburst, the transient neutron star low-mass X-ray binary XTE J1701$-$462 turned on again in September 2022, allowing for the first study of its X-ray polarimetric characteristics by a dedicated observing program with the Imaging X-ray Polarimeter Explorer (IXPE). Polarimetric studies of XTE J1701$-$462 have been expected to improve our understanding of accreting weakly magnetized neutron stars, in particular, the physics and the geometry of the hot inner regions close to the compact object. The IXPE data of two triggered observations were analyzed using time-resolved spectroscopic and polarimetric techniques, following the source along its Z-track of the color-color diagram. During the first pointing on 2022 September 29, an average 2-8 keV polarization degree of 4.6$\pm$ 0.4\% was measured, the highest value found up to now for this class of sources. Conversely, only a $\sim$0.6\% average degree was obtained during the second pointing ten days later. The polarimetric signal appears to be strictly related to the higher energy blackbody component associated with the boundary layer (BL) emission and its reflection from the inner accretion disk, and it is as strong as 6.1\% and 1.2\% ($>95\%$ significant) above 3-4 keV for the two measurements, respectively. The variable polarimetric signal is apparently related to the spectral characteristics of XTE J1701$-$462, which is the strongest when the source was in the horizontal branch of its Z-track and the weakest in the normal branch. These IXPE results provide new important observational constraints on the physical models and geometry of the Z-sources. We report the first $> 99\%$ confidence detection of X-ray polarization in BL Lacertae. During a recent X-ray/$\gamma$-ray outburst, a 287 ksec observation (2022 November 27-30) was taken using the Imaging X-ray Polarimetry Explorer ({\it IXPE}), together with contemporaneous multiwavelength observations from the Neil Gehrels {\it Swift} observatory and {\it XMM-Newton} in soft X-rays (0.3--10~keV), {\it NuSTAR} in hard X-rays (3--70~keV), and optical polarization from the Calar Alto, and Perkins Telescope observatories. Our contemporaneous X-ray data suggest that the {\it IXPE} energy band is at the crossover between the low- and high-frequency blazar emission humps. The source displays significant variability during the observation, and we measure polarization in three separate time bins. Contemporaneous X-ray spectra allow us to determine the relative contribution from each emission hump. We find $>99\%$ confidence X-ray polarization $\Pi_{2-4{\rm keV}} = 21.7^{+5.6}_{-7.9}\%$ and electric vector polarization angle $\psi_{2-4{\rm keV}} = -28.7 \pm 8.7^{\circ}$ in the time bin with highest estimated synchrotron flux contribution. We discuss possible implications of our observations, including previous {\it IXPE} BL Lacertae pointings, tentatively concluding that synchrotron self-Compton emission dominates over hadronic emission processes during the observed epochs., Comment: Accepted to ApJL. 16 pages, 8 figures

However, many questions regarding the physical conditions at shock fronts, such as the magnetic-field morphology close to the particle acceleration sites, remain open. Here we report the detection of a localized polarization signal from some synchrotron X-ray emitting regions of Tycho's supernova remnant made by the Imaging X-ray Polarimetry Explorer. The derived degree of polarization of the X-ray synchrotron emission is 9% ± 2% averaged over the whole remnant, and 12% ± 2% at the rim, higher than the value of polarization of 7%–8% observed in the radio band. In the west region, the degree of polarization is 23% ± 4%. The degree of X-ray polarization in Tycho is higher than for Cassiopeia A, suggesting a more ordered magnetic field or a larger maximum turbulence scale. The measured tangential direction of polarization corresponds to the radial magnetic field, and is consistent with that observed in the radio band. These results are compatible with the expectation of turbulence produced by an anisotropic cascade of a radial magnetic field near the shock, where we derive a magnetic-field amplification factor of 3.4 ± 0.3. The fact that this value is significantly smaller than those expected from acceleration models is indicative of highly anisotropic magnetic-field turbulence, or that the emitting electrons either favor regions of lower turbulence, or accumulate close to where the orientation of the magnetic field is preferentially radially oriented due to hydrodynamical instabilities. © 2023. However, phase-resolved and energy-resolved polarimetry of X-ray pulsars is required in order to test different models and to shed light on the emission processes and the geometry of the emission region. Here we present the first results of the observations of the accreting X-ray pulsar Vela X-1 performed with the Imaging X-ray Polarimetry Explorer. Vela X-1 is considered to be the archetypal example of a wind-accreting, high-mass X-ray binary system, consisting of a highly magnetized neutron star accreting matter from its supergiant stellar companion. The spectropolarimetric analysis of the phase-averaged data for Vela X-1 reveals a polarization degree (PD) of 2.3% ± 0.4% at the polarization angle (PA) of −47fdg3 ± 5fdg4. A low PD is consistent with the results obtained for other X-ray pulsars and is likely related to the inverse temperature structure of the neutron star atmosphere. The energy-resolved analysis shows the PD above 5 keV reaching 6%–10% and a ∼90° difference in the PA compared to the data in the 2–3 keV range. The phase-resolved spectropolarimetric analysis finds a PD in the range 0%–9% with the PA varying between −80° and 40°. © 2023. The Author(s). Postoperative atrial fibrillation (POAF) is a frequently reported postcardiac surgery complication leading to increased in-hospital and long-term mortality rates. Many randomized controlled trials (RCTs) have recently suggested using statins to protect against POAF. Therefore, we performed a systematic literature search and meta-analysis in electronic databases for eligible studies published between January 2006 and January 2022. The principal inclusion criteria were as follows: RCTs' study design, statin-naive patients, total study participants ≥50 units, and statin pretreatment started no more than 21 days before cardiac surgery. In the primary analysis, statin pretreatment reduced the incidence of POAF compared with placebo. Analyzing different molecules, atorvastatin was associated with lower incidence of POAF but rosuvastatin was not. We therefore performed a sensitivity analysis excluding RCTs affected by important risk of biases. Thus, studies whose participants were ≥199 were those eligible for the secondary analysis. No statistically significant difference between statin pretreatment and placebo (OR 0.87; 95% CI: 0.71-1.07, P = 0.18) as well as for atorvastatin (OR 0.88; 95% CI: 0.61-1.28; P = 0.48; I 2 = 84%) and rosuvastatin (OR 0.87; 95% CI: 0.68-1.12, P = 0.29) was observed. To conclude, statin pretreatment before cardiac surgery is not associated with a significant reduction in POAF occurrence.

In this paper I will describe and interpret some data from Southern Quechua and Southern Aymara spoken texts recorded during traditional divinatory sessions. This analysis is aimed to show that divination can be considered as an example of how ritual speech, often described as a phenomenon based on fixed sequences, is characterized by variable elements too, being the relationship between a particular “context of situation” and the divinatory speech event very strong. In particular, the analysis focuses on a specific kind of speech acts, which seem to be built with the help of morphological devices that are normally used to indicate the source of information in the Quechua and Aymara language families. The same devices are used with a specific performative function in the context of divination: To “officialise” the message of the oracle and to oppose this to the speech acts of the diviner.

Humans have unlimited wants. This foundational economic principle and widely accepted assumption about human nature poses considerable challenges to addressing sustainability because pursuing wealth and economic growth to meet unlimited wants increases resource use and pollution. Here we show evidence that this principle is not universal, and actually applies only to a minority of people. Across 42 community samples (N = 7,860) from 33 countries spanning 6 continents, we examined how much money people wanted in their absolutely ideal life. In 86% of countries the majority of people thought they would achieve their absolutely ideal lives with US$10 million or less, and in some countries as little as US$1 million or less. However, a substantial minority (8–39% across countries) wanted as much money as they could obtain, indicating unlimited wants. Limited and unlimited wealth ideals were not related to country differences in economic development, but those with unlimited wants tended to be younger, city-dwelling people who valued power, success and independence, and lived in countries with a greater collective focus and acceptance of power differences. The results suggest that transformative approaches relying on limiting wealth and growth to achieve sustainability may be more consistent with human ideals and aspirations than commonly believed.

Stray current is a relevant phenomenon in particular for DC electrified transportation systems, affecting the track and infrastructure within the right of way and other structures and installations nearby. It worsens with time and the level of protection depend on timely maintenance, besides correct design choices. The assessment of track insulation is the starting point for both stray current monitoring systems and at commissioning or upon major changes. Standardized methods (ref. EN 50122-2, or IEC 62128-2) are almost unchanged in the last 20 years, but suffer from accuracy issues and variability due to parameters and conditions not under operator’s control. A critical discussion and analysis of the sources of variability and practical constraints is proposed, followed by the evaluation of uncertainty, with the objective not only to assess the accuracy of provided results, but to foster research on innovative, more flexible and accurate, methods.

Background Ventricular preexcitation (VPE) due to atrio-ventricular accessory pathways (AP) has been associated to a higher incidence of sudden cardiac death (1,2). The incidence of major arrhythmic events (MAE) may be higher in children (1,3), but most of the past studies recruited a high proportion of patients with congenital heart disease (CHD), treated with outdated pharmacological therapy (1). Transcatheter ablation (TCA) of the AP is a safe and effective therapy and it is nowadays the standard of care (1,2). However, it may be less effective and more dangerous in smaller and younger children (1,3). It is described that patients may spontaneously lose VPE during childhood (1), but the natural history of VPE is not well understood yet. Purpose To study the natural history of VPE. We focused especially on the likelihood of spontaneous resolution of VPE during growth, the possible associated factors, and the incidence of MAE during a long follow up in a contemporary paediatric cohort. Methods We retrospectively recruited patients diagnosed with VPE at age 0 to 12, referred from 1993 to 2021 to two tertiary care hospitals. Significant CHD and double AP were excluded. The primary outcome was the spontaneous resolution of VPE, defined by the loss of short PR and delta wave seen in at least two consecutive ECG and/or on 24-hour Holter monitoring. The follow up stopped when the patients reached 16 years of age, met the primary outcome or effective TCA was performed. Given the time-to-event dependency of the condition, the survival analysis of VPE considered the left truncation resulted from the recruitment of patients after the first days of life. Results We recruited 153 patients with a median age of diagnosis of 4.9 (0.2 – 8.4). The median follow-up time was 4.9 (1.6 – 8) years. During follow-up, 42 (28%) patients spontaneously loss VPE. The left truncated Kaplan-Meier survival curve of VPE is shown in Picture 1. It estimates that VPE would persist in 53% (40.1-70.2) and 33.8% (23.7-48.4) of patients at the age of 1 and 16, respectively. Symptoms and intermittent preexcitation were the only variables significantly associated with lower (OR 0.2) and higher probability (OR 3.45) of SR of VPE. The vast majority of patients with symptoms was treated with drug therapy before the age of 8, and TCA was safely delayed thereafter. None of the patients experienced a MAE during follow up. Conclusion According to our data, a large proportion of paediatric patients are likely to spontaneously lose VPE within the first year of life, and a spontaneous resolution may occur later during childhood in a non-negligible number of cases. The absence of symptoms and/or intermittent preexcitation are significant predictors of loss of VPE. The results of our study are of the utmost clinical relevance, since they support a wait-and-see strategy, especially in very young children with VPE.

In an X-ray pulsar, a neutron star accretes matter from a stellar companion through an accretion disk. The high magnetic field of the rotating neutron star disrupts the inner edge of the disc, funneling the gas to flow onto the magnetic poles on its surface. Hercules X-1 is in many ways the prototypical X-ray pulsar; it shows persistent X-ray emission and it resides with its companion HZ Her, a two-solar-mass star, at about 7~kpc from Earth1. Its emission varies on three distinct timescales2 the neutron star rotates every 1.2~seconds, it is eclipsed by its companion each 1.7~days, and the system exhibits a superorbital period of 35~days which has remained remarkably stable since its discovery3. Several lines of evidence point to the source of this variation as the precession of the accretion disc4,5, the precession of the neutron star6,7 or both8. Despite the many hints over the past fifty years, the precession of the neutron star itself has yet not been confirmed or refuted. We here present X-ray polarization measurements with the Imaging X-ray Polarimetry Explorer9 (IXPE) which probe the spin geometry of the neutron star. These observations provide direct evidence that the 35-day-period is set by the free precession of the neutron star crust, which has the important implication that its moment of inertia eigenvalues differ fractionally by a few parts per ten million10. Furthermore, we find indications that the basic spin geometry of the neutron star is altered by torques on timescale of a few hundred days.

Given sustained high vaccination coverage and enhanced surveillance for measles, Spain has been free of endemic measles transmission since 2014, achieving elimination certification from the World Health Organization in 2017. In November 2017, measles was introduced through an imported case travelling to the Valencian Community, causing an interregional outbreak. Here, we describe the outbreak using data reported to the national epidemiological surveillance network. The outbreak involved 154 cases (67 males, 87 females) notified in four regions; 148 were laboratory-confirmed and six epidemiologically linked. Most cases were adults aged 30–39 (n = 62, 40.3%) years. Sixty-two cases were hospitalised (40.3%) and 35 presented complications (22.7%). Two thirds of the cases (n = 102) were unvaccinated including 11 infants (≤ 1 year) not yet eligible for vaccination. The main route of transmission was nosocomial; at least six healthcare facilities and 41 healthcare workers and support personnel were affected. Sequencing of the viral nucleoprotein C-terminus (N450) identified genotype B3, belonging to the circulating MVs/Dublin.IRL/8.16-variant. Control measures were implemented, and the outbreak was contained in July 2018. The outbreak highlighted that raising awareness about measles and improving the vaccination coverage in under-vaccinated subgroups and personnel of healthcare facilities are key measures for prevention of future outbreaks.

Purpose:Despite standard of care (SOC) established by Stupp, glioblastoma remains a uniformly poor prognosis. We evaluated IGV-001, which combines autologous glioblastoma tumor cells and an antisense oligonucleotide against IGF type 1 receptor (IMV-001), in newly diagnosed glioblastoma.Patients and Methods:This open-label protocol was approved by the Institutional Review Board at Thomas Jefferson University. Tumor cells collected during resection were treated ex vivo with IMV-001, encapsulated in biodiffusion chambers with additional IMV-001, irradiated, then implanted in abdominal acceptor sites. Patients were randomized to four exposure levels, and SOC was initiated 4–6 weeks later. On the basis of clinical improvements, randomization was halted after patient 23, and subsequent patients received only the highest exposure. Safety and tumor progression were primary and secondary objectives, respectively. Time-to-event outcomes were compared with the SOC arms of published studies.Results:Thirty-three patients were enrolled, and median follow-up was 3.1 years. Six patients had adverse events (grade ≤3) possibly related to IGV-001. Median progression-free survival (PFS) was 9.8 months in the intent-to-treat population (vs. SOC, 6.5 months; P = 0.0003). In IGV-001–treated patients who met Stupp-eligible criteria, PFS was 11.6 months overall (n = 22; P = 0.001) and 17.1 months at the highest exposure (n = 10; P = 0.0025). The greatest overall survival was observed in Stupp-eligible patients receiving the highest exposure (median, 38.2 months; P = 0.044). Stupp-eligible patients with methylated O6-methylguanine–DNA methyltransferase promoter (n = 10) demonstrated median PFS of 38.4 months (P = 0.0008). Evidence of immune activation was noted.Conclusions:IGV-001 was well tolerated, PFS compared favorably with SOC, and evidence suggested an immune-mediated mechanism (ClinicalTrials.gov: NCT02507583).

Phagosomal escape and intracellular survival, often accompanied by Small Colony Variants (SCVs) formation, are typical features of infections caused by S. aureus. The survival in macro-phages favours S. aureus dissemination and complicates treatment. RAW 264.7 murine macro-phages infected with S. aureus USA300 and treated with erythromycin and 20mM carnosine, alone and in combination, were used as experimental model. SCVs were isolated from all treat-ment conditions, but only those undergoing the pressure of combined erythromycin and carnosine for 48 hours were stable for at least six passages on blood agar. Nucleic acid extraction was car-ried out for S. aureus USA300 wild-type and stable SCVs. Whole Genome Sequencing (WGS) was performed using Illumina DNA Prep and Illumina MiSeq, and quantitative reverse transcription PCR was performed. WGS analysis did not yield mutations pointing to differences between S. au-reus USA300 and stable SCVs, therefore the focus was shifted to evaluating gene expression vari-ations. Genes such as zur, mntR, uhpt, fur, sdrE were shown to be significantly up-regulated in SCVs compared to S. aureus USA300 wild-type, suggesting a global change that allows adapta-tion to intracellular persistence, including protection from inflammatory response and evasion of the immune system.

Polarization measurements could provide information on their magnetic fields and surface properties. We observed polarized x-rays from the magnetar 4U 0142+61 using the Imaging X-ray Polarimetry Explorer and found a linear polarization degree of 13.5 ± 0.8% averaged over the 2– to 8–kilo–electron volt band. The polarization changes with energy: The degree is 15.0 ± 1.0% at 2 to 4 kilo–electron volts, drops below the instrumental sensitivity ~4 to 5 kilo–electron volts, and rises to 35.2 ± 7.1% at 5.5 to 8 kilo–electron volts. The polarization angle also changes by 90° at ~4 to 5 kilo–electron volts. The pomegranate (Punica granatum) is an ancient perennial plant species of the Punicaceae family. Its seeds are consumed as food or as juice. Previous studies have noted that pomegranate juice encompasses many active compounds with beneficial effects. The main goals of this work were to study the phenolic components of freeze-dried and reconstituted pomegranate juices obtained from 13 pomegranate genotypes growing in Morocco.We analyzed several pomegranate juices using high-performance liquid chromatography and high-resolution mass spectrometry to determine phenolic compounds. Twenty-seven bio-phenols, belonging to four different classes (phenolic acids, hydrolyzable tannins, anthocyanins, and flavonoids), were identified based on their accurate mass measurements, and quantified. Some encouraging results were obtained. Even though the freeze-drying process introduced a marked degradation of bio-phenols, substantially lowering their levels in the reconstituted fruit juices, these fruit juices were still rich enough in bio-phenols to compete with some fresh fruit juices. The reconstituted juices obtained by rehydration of the lyophilized material still differed enough to enable a statistical classification based on their polyphenol content. A correlation analysis was applied to the polyphenol data to explore correlations and similarities between genotypes.The results showed that freeze-drying and reconstitution of juices introduced some degradation of the polyphenol content. The overall polyphenolic pattern within the same cultivar, in two different harvesting years, was maintained, however, suggesting the composition stability of the freeze-dried juices produced in this time span. © 2022 Society of Chemical Industry.

Laser annealing process for ohmic contact formation on 4H-SiC has attracted increasing attention in the last years, because it enables the fabrication of SiC power devices on very thin substrates. We have investigated the formation of Nickel-based ohmic contact on 4H-SiC by using a Yb:YAG laser in scanning mode, with a wavelength of 515 nm and a pulse duration of 1200 ns. A 100 nm thick Ni layer has been deposited on SiC and irradiated at different process conditions. The reaction process has been studied, as a function of fluence and scan number of laser annealing, by means of X-Ray Diffraction (XRD) and Transmission Electron Microscopy (TEM) analyses. The electrical properties of the annealed layers have been evaluated on Schottky Barrier Diodes (SBDs) devices, confirming the ohmic behavior of the reacted contact and showing improved performances respect to RTA approach. The compatibility of thermal budget of the process in the front side has been verified by means process simulation. A strong relationship between structural properties of reacted layers and electrical behavior of SBDs devices has been revealed. Solid-state laser annealing process, with wavelength in green light region, can indeed represent a suitable solution for ohmic contact formation of 4H-SiC power devices, fabricated on thin substrates.

Silicon dioxide (SiO2) layers deposited on 4H-SiC and subjected to different post deposition annealing (PDA) in NO and N2O were studied to identify the key factors influencing the channel mobility and threshold voltage stability in lateral implanted 4H-SiC MOSFETs. Cyclic gate bias stress measurements allowed to separate the contributions of interface states (Nit) and near interface oxide traps (NIOTs) in the two oxides. The reduction of these traps in the NO annealed sample is due to the lower amounts of sub-stoichiometric silicon oxide (~1nm) and carbon-related defects (<1nm) at the interface, as could be demonstrated by Electron Energy Loss Spectroscopy. The experimental results indicate that limiting the SiC re-oxidation during post-deposition annealing in MOSFET technology is a key factor to improve the mobility and threshold voltage stability.

A 19-year-old female with a history of pre-B cell acute lymphocytic leukemia (ALL) presented with two aggressive cutaneous squamous cell carcinomas (C-SCC) in the right hand. The patient was diagnosed with pre-B cell ALL at four years of age. She underwent chemotherapy with initial remission. However, recurrence of the pre-B cell ALL required an unrelated allogeneic cord hematopoietic stem cell transplant (alloHSCT). Post-transplant, the patient developed Graft-Versus-Host Disease (GVHD), which was treated with immunosuppressant therapy for six years until resolution. Fourteen years following the transplant, the patient developed a morbilliform drug eruption secondary to clindamycin. She consequently received prednisone treatment. During the treatment period, the patient developed a new ulcerated and tender nodule on the dorsal aspect of her right hand. Further histopathological biopsy confirmed the diagnosis of C-SCC, which required excision. Ten months following the excision, the patient developed an additional C-SCC nodule on the same right hand, separated by 2.6 cm from the prior C-SCC. She was referred for a ray resection procedure. This case illustrates a patient with multiple risk factors that may have contributed to the continued development of C-SCC. Such risk factors include: a prolonged course of immunosuppressant medications and voriconazole treatment. Additional research is needed to investigate the etiologies and risks of C-SCC development in patients who require a transplant and long-duration immunosuppressive therapy.

It is generally acknowledged that the first morphological change of hidradenitis suppurativa/acne inversa (HS/AI) consists of infundibular plugging of the folliculosebaceous apocrine apparatus, which is followed by acute and chronic inflammation, cysts with sinus formation, and fibrosis. Alternatively, it has been hypothesized that HS/AI is primarily a neutrophilic autoinflammatory disease and that the follicular plugging typical of this disease is secondary to inflammation.To review the sequence of the changes that mark the disease development, we have performed a histopathologic study on the surgical material from a series of axillary and inguinal/perineal cases.The histologic material from surgery on Hurley's second and third stage HS/AI was retrieved and collected with the patients' clinical images. The virtually uninvolved skin peripheral to the lesions was studied together with the main inflammatory foci on vertical sections stained with hematoxylin-eosin and immunohistochemistry for the follicle sheaths.The fully developed lesions showed acute and chronic, suppurative and granulomatous inflammation overlapping fibrosis, cysts, and sinuses. Instead, the skin adjacent to florid inflammation showed plugging and dysmorphic alterations of the hair follicles associated with immunopathological changes of the inner root sheath keratin expression.Our observations coincide with the classical pathological studies on the progressive changes of HS/AI; however, in our specimens, the virtually normal skin peripheral to the fully developed lesions show seemingly initial follicular changes that suggest development error. This finding would support the hypothesis of combined mutation-induced epithelial differentiative defects and immunological derangement in HS/AI pathogenesis.GRUNDLAGEN: Es wird allgemein akzeptiert, dass die ersten morphologischen Veränderungen einer Hidradenitis suppurativa/Acne inversa (HS/AI) infundibuläre Keratosen des apokrinen follikulosebaceösen Apparates darstellen. Dieser Verschluss der Ostien wird gefolgt von akuter und chronischer Entzündung, Zysten mit Sinusformationen und Fibrose. Alternativ wird die HS/AI als eine primär neutrophile autoinflammatorische Erkrankung angesehen, bei der die Infundibulumkeratosen als Sekundärphänomen auftreten.Wir führten eine histopathologische Untersuchung an chirurgischen Gewebeproben axillärer und anogenitaler HS/AI durch, um die Reihenfolge morphologischer Veränderungen im Verlauf der Erkrankung zu erfassen.Das Gewebematerial stammte von Patienten mit HS/AI im Hurley-Stadium II und III, die sich einer chirurgischen Behandlung unterzogen hatten. Klinische Abbildungen lagen ebenfalls vor. Wir analysierten sowohl klinisch gesunde Haut in der Peripherie der Exzidate als auch die verschiedenen inflammatorischen Läsionen mittels vertikaler Gewebeschnitte. Hierzu wurden Hämatoxylin-Eosin und immunhistologische Färbungen der Haarscheiden verwendet.Die vollausgebildeten Läsionen zeigten akute und chronische, suppurative und granulomatöse Entzündungszeichen, die sich mit Fibrose, Zysten und Sinusen überlappten. Die benachbarte scheinbar gesunde Haut wies Infundibulumverschlüsse und dysmorphe Veränderungen der Haarfollikel auf. Es fanden sich immunpathologische Alterationen der Keratinexpression der inneren Haarwurzelscheiden.Unsere Beobachtungen stimmen mit den klassischen Studien zu progredienten entzündlichen Veränderungen der HS/AI überein. Wir konnten jedoch auch initiale Alterationen der Haarfollikel in der scheinbar gesunden Haut finden, die auf eine Entwicklungsstörung hinweisen. Diese Befunde stützen die Hypothese einer Kombination von durch Mutationen bedingten Störungen der epithelialen Differenzierung und immunologischen Komponenten in der HS/AI-Pathogenese.

International audience; Despite numerous technological applications associated to nickel silicide thin films, their formation mechanisms are still far from being understood. We combined experimental and numerical approaches to unravel the early stages of nickel silicide formation with an atomistic precision. In particular, we employed first principles calculations, X-ray reflectivity as well as high-resolution scanning transmission electron microscopy analyses. Altogether, our work demonstrates that during nickel deposition on top of a silicon surface, an interface alloyed layer is formed even at room temperature before any thermal activation. Moreover, we managed to determine that this interfacial layer has a nickel-rich Ni3Si composition which is favored by the ability of nickel atoms to penetrate the surface layers of the silicon substrate.
