Background Infection by the monkeypox virus classically causes a cutaneous rash that is preceded by fever and lymph node swelling, as well as other nonspecific systemic symptoms. A recent outbreak occurred and spread in Europe and other regions, especially among patients who declare themselves as men who have sex with men. Current reports have shown that cutaneous lesions may be limited to the anogenital area. We report on a case of proctitis caused by monkeypox virus, without visible typical lesions of this virus. Case presentation A 29-year-old Caucasian male presented with a monkeypox virus proctitis that recurred after treatment for a documented Neisseria gonorrhoeae and Chlamydia trachomatis coinfection, likely acquired at the same time. The proctitis was preceded by fever and a swollen inguinal lymph node, and was associated with a hemorrhoid. The monkeypox virus polymerase chain reaction of a rectal swab documented high viral loads, although no typical lesion was visible. After resolution of the rectitis, the patient developed a single dermatome herpes zoster, despite the absence of usual risk factors. The patient evolved well without further specific treatment. Conclusion This case shows that monkeypox virus can be responsible for proctitis, without any typical lesion, along with the important rectal shedding of the virus. It raises the concern of contagion during anal intercourse through body fluids and gives further credit that monkeypox virus can be a sexually transmitted infection. This should prompt routine rectal screening in patients with proctitis accompanied by fever and swollen lymph nodes, and in patients who have a history of unprotected receptive anal sex, even in presence of other sexually transmitted infections, and especially during a monkeypox virus outbreak. The potential link between monkeypox virus infection and shingles warrants further investigations.

O risco infecioso associado a terapêutica biológica é cada vez mais estudado e conhecido, existindo estratégias de rastreio e profilaxia bem estabelecidas. O mesmo não se verifica na utilização de corticoide sistémico nem dos tradicionais modificadores de doença autoimune, de que são exemplo o metotrexato e a ciclosporina, apesar de usados há largos anos. A dose e tempo de terapêutica com corticoide sistémico relacionam-se com a ocorrência de infeção oportunista. Doses superiores a 5 mg/dia associam-se a infeção bacteriana, superiores a 10 mg/dia a reativação de infeção por vírus herpes zoster (VHZ), e superiores a 15 mg/dia ou por mais de 2 a 4 semanas a reativação de tuberculose latente, o que implica rastreio e aplicação de quimioprofilaxia de forma adequada. O corticoide sistémico surge ainda como dos principais fatores de risco para o desenvolvimento de pneumocistose no doente não-VIH e doses prolongadas (por mais de 4 semanas) podem levar a reativação de infeção por vírus da hepatite B (VHB) e justificar a profilaxia com tenofovir disoproxil fumarato ou entecavir. Foram ainda descritos casos de estrongiloidíase complicada de síndrome de hiperinfeção em doentes sob corticoide. O grau de imunossupressão conferido pode contraindicar a realização de vacinas vivas atenuadas. O metotrexato e ciclosporina comportam baixo risco infecioso quando utilizados em monoterapia, sendo a vigilância de sintomas a principal estratégia preventiva. Não obstante, ambos são imunomoduladores, contraindicam a realização de vacinas vivas atenuadas e associam-se a risco de infeção bacteriana e reativação de infeção por VHZ, no caso da ciclosporina, ou reativação de infeção por VHZ e VHB no caso do metotrexato, sobretudo se administrado em dose >0,4 mg/kg/semana. Ambos se associam a evolução para tuberculose ativa quando em associação terapêutica com outros imunossupressores. Compreender e estudar o risco de infeção na utilização de terapêutica imunossupressora permite a sua aplicação de forma mais informada e segura.

Previously, we identified a Chlamydia trachomatis Lymphogranuloma venereum (LGV) recombinant strain possessing a unique non-LGV ompA genotype. Here, culture-independent genome sequencing confirms its circulation in Europe, Middle East and North America, and unveils genetic evidence of emergence of antibiotic resistance. Multi-country and systematic molecular surveillance is needed. The quadrivalent human papillomavirus (HPV) vaccine was introduced in the Portuguese National Immunization Program in October 2008, targeting 13-year-old girls. This study aimed at evaluating the impact of HPV vaccination on the epidemiology of genital warts (GWs) in Portugal.Observational, retrospective chart review study conducted at two free-of-charge walk-in sexually transmitted diseases (STD) clinics in Lisbon region. The medical records of all patients attending a first STD consultation at the study centers between May 2006 and December 2017 (observation period) were reviewed. The number of patients diagnosed with GWs and/or chlamydial infection at each year was documented and used to determine yearly prevalence of both conditions throughout the observation period. We broke down the observation period into pre-vaccination (May 2006 to December 2008) and vaccination (January 2009 to December 2017) periods.Most patients were male (69.5%) and aged ≥ 25 years (78.1%). The majority of male patients were men who have sex with women (62.0%). Marked decreases in the prevalence of GWs between the last year of the pre-vaccination period (2008) and the last year of the observation period (2017) were found for female patients aged ≤ 19 and 20-24 years (86.8% and 77.4%, respectively). Lower decreases were observed for male patients of the same age groups (38.5% and 19.3%, respectively). GWs prevalence increased among patients ≥ 25 years (9.7% and 14.7% among female and male patients, respectively). Overall prevalence of chlamydial infection increased by 75.9% between 2008 and 2017.This study contributes to the body of evidence showing that public HPV vaccination programs are effective in reducing the prevalence of GWs among vaccine-eligible patients. HPV vaccination program may significantly reduce the burden associated with GWs in Portugal.

Background: Lymphogranuloma venereum (LGV) is a sexual transmitted infection (STI), currently endemic within the population of men who have sex with men (MSM) of Western Countries. L2B variant has been reported as the predominant strain in the current LGV epidemics, although a shift towards L2-434 has been observed in some European countries. Objectives: To evaluate and characterize the population with LGV infection diagnosed in Centro Hospitalar Universitário de Lisboa Central, Lisbon, Portugal. Methods: A retrospective analysis of all LGV diagnoses between 2016 and 2019 was performed. The diagnosis was established through ompA-genotyping of samples yielding a positive result to Chlamydia trachomatis (CT). All considered samples were retrieved from the clinician activity, through swabbing and urine analysis and CT infection diagnosis was obtained using real-time PCR. Results: During the period studied 16 279 CT diagnostics tests were employed, with a striking increase from 2016 (n = 467) to 2019 (n = 9362). A total of 1602 diagnoses of CT were established, from which 168 (10.5%) corresponded to LGV, with both infections showing a rising evolution, between 2016 and 2019, of 2.9 and 2.7 times, respectively. The majority of the LGV strains were genotyped as L2/434 (67.3%; n = 113). LGV predominantly affected MSM and men who have sex with men and women (97.0%; n = 163). Anorectal infection was the most prevalent one (90.5%; n = 152), being proctitis the main clinical presentation (76.2%; n = 128). Absence of symptoms was reported in almost 15% of the cases (n = 24). The presence of concomitant infection with human immunodeficiency virus was dominant (73.2%; n = 123) and the prevalence of one or more STI co-infections was about 60.1% (n = 99). Conclusions: An increasing evolution of CT and LGV testing and diagnosing was observable throughout the studied period. Characteristics of the population are similar with those described within LGV epidemics. Syphilis is an old disease and a major public health concern, which is re-emerging in the western world. Atypical syphi- litic presentations can be challenging to diagnose. We present a 23-year-old, HIV-positive man who has sex with men who presented with a 2-week history of an asymptomatic lesion on the glans. Previous topical and systemic treatments proved unsuccessful. Physical examination demonstrated a slightly indurated, ill-defined erythematous plaque over most of the glans. Enlarged, non-tender, bilateral inguinal lymph nodes were palpable. Serological tests revealed a fourfold increase in the VDRL titre (1:8, previously 1:2). A diagnosis of syphilitic balanitis of Follmann was made and complete resolution was achieved with an intramuscular single dose of benzathine penicillin 2.4 MU. Syphilitic balanitis of Follmann represents an uncommon manifestation of primary syphilis, with less than 100 cases reported. Untrea- ted, it will resolve spontaneously, but the patient will remain highly infectious and at-risk for long-term morbidity. In an era in which the number of syphilis cases is increasing sharply both in Western Europe and the United States, all physicians should be aware for the typical and atypical early syphilitic manifestations as they represent opportunities of paramount importance for treatment and epide- mic control.

Introduction: Sexually transmitted infections (STIs) are a significant public health problem worldwide. They are more frequent in young men; however the prevalence in older individuals is increasing. The aim of this study was to access the prevalence of STIs diagnosis in men aged 50 years and older. We also aimed to compare the demographic and behavioral characteristics of those diagnosed with STIs and those with non-STI conditions.Methods: We conducted a retrospective study including all men aged 50 years and older who have attended a Sexual Transmitted Diseases (STD) clinic of a central hospital in Lisbon, Portugal, between July 2015 and December 2017.Results: Between July 2015 and December of 2017, 1722 men attended the STD clinic, of which 323 (18.8%) were aged 50 years and older. Out of these 323 men, 137 (41.4%) were diagnosed with a STI, more often anogenital warts (32.9%), genital herpes simplex virus infection (19.7%) and early syphilis (19.0%). The mean age of the patients with a STI was 59.9 years and almost all (90.5%) reported an inconsistent use of a condom (90.5%). The number of men that have sex with men (MSM) was significantly higher in patients with a STI (p = 0,003) than those with a non-STI diagnosis. Additionally, 40 (29.2%) of these STI patients had been previously infected with the human immunodeficiency virus (HIV) and 33 (24.1%) had a past history for syphilis, which were significantly higher than in patients in patients with a non-STI diagnosis (p = 0.015 and p = 0.001, respectively).Conclusion: Existing data on STI has mainly focused younger population with few studies performed in older individuals. Our study reports a large number of men aged 50 years and older that allows a better understanding of this age group. High levels of unsafe sex and previous diagnosis of HIV and syphilis emphasizes the importance of sexual health promotion in older individuals.

Recent studies worldwide reveal a significant prevalence of extragenital infections by Neisseria gonorrhoeae among men who have sex with men. We aimed to analyse the frequency and characteristics of extragenital gonococcal infections diagnosed in men who have sex with men in a walk-in Sexually Transmitted Infection clinic in Lisbon, Portugal.We conducted a cross-sectional, retrospective study of the anorectal and/or oropharyngeal Neisseria gonorrhoeae infections in men who have sex with men, diagnosed in our Sexually Transmitted Infection clinic between January 2014 and December 2016.We found extragenital infection in 87 cases of gonorrhoea identified in men who have sex with men in this period, including: 49 cases of anorectal disease, 9 of oropharyngeal disease, 13 cases of infection at both extragenital sites, and 16 of simultaneous extragenital and urogenital gonorrhoea. Patients' ages ranged from 17 to 64 years (median: 28 years). Forty-seven (54%) of the patients did not present with any extragenital symptoms. Thirty (35%) were human immunodeficiency virus-1-positive.Since most extragenital Neisseria gonorrhoeae infections are asymptomatic, they may be missed and go untreated unless actively investigated. Current international guidelines recommend the screening of gonorrhoea at extragenital sites in men who have sex with men because anorectal and oropharyngeal infections constitute a potential disease reservoir, and may facilitate transmission and/or acquisition of human immunodeficiency virus infection.Our results highlight the relevance of testing men who have sex with men for Neisseria gonorrhoeae at extragenital sites, regardless of the existence of local complaints. The implementation of adequate screening programmes in Portugal should be considered. We also reinforce the need to raise awareness in the population regarding the adoption of prophylactic measures against transmission of sexually transmitted infections during anal and/or oral sexual exposure.Introdução: Estudos internacionais recentes revelam uma prevalência significativa de infeções extragenitais por Neisseria gonorrhoeae em homens que têm sexo com homens. Pretendemos analisar a frequência e caraterísticas das infeções gonocócicas extragenitais diagnosticadas em homens que têm sexo com homens numa consulta aberta de Infeções Sexualmente Transmissíveis em Lisboa, Portugal. Material e Métodos: Conduzimos um estudo observacional transversal, retrospetivo, das infeções anorrectais e/ou orofaríngeas por Neisseria gonorrhoeae em homens que têm sexo com homens, diagnosticadas na nossa consulta de Infeções Sexualmente Transmissíveis entre janeiro de 2014 e dezembro de 2016. Resultados: Detetámos infeção extragenital em 87 dos casos de gonorreia identificados em homens que têm sexo com homens no período analisado, incluindo: 49 casos de doença anorrectal, 9 de doença orofaríngea, 13 casos de infeção em ambas as localizações extragenitais, e 16 de gonorreia simultaneamente extragenital e urogenital. A idade dos doentes variou entre 17 e 64 anos (mediana: 28 anos). Quarenta e sete (54%) dos doentes não apresentavam qualquer sintoma extragenital. Trinta (35%) tinham também infeção pelo vírus da imunodeficiência humana-1. Discussão: Dado que a maioria das infeções extragenitais por Neisseria gonorrhoeae são assintomáticas, estas podem não ser diagnosticadas, e não tratadas, se não pesquisadas ativamente. Em várias normas de orientação clínica internacionais atuais é recomendado o rastreio da gonorreia em localizações extragenitais em homens que têm sexo com homens, visto que as infeções anorrectais e orofaríngeas constituem potenciais reservatórios da doença e podem facilitar a transmissão e/ou aquisição de infeção pelo vírus da imunodeficiência humana. Conclusão: Os nossos resultados apontam para a relevância de testar os homens que têm sexo com homens para a presença de Neisseria gonorrhoeae em localizações extragenitais, independentemente da existência de sintomas locais. A implementação de programas de rastreio adequados em Portugal deverá ser considerada. Relevamos também a necessidade de sensibilizar a população para a adoção de medidas profiláticas contra a transmissão de infeções sexualmente transmissíveis durante o contacto sexual anal e/ou oral.

A infecção genital por Chlamydia trachomatis (Ct) é frequentemente assintomática e quando não tratada precocemente constitui um problema major de saúde pública. Os autores realizaram um estudo cujo objectivo foi determinar a prevalência de infecção genital por Ct numa consulta de Infecções Sexualmente Transmissíveis (ISTs) e realizar uma análise de diferentes variáveis. Para tal foi realizado um estudo retrospectivo dos doentes com diagnóstico de infecção por Ct confirmado por técnica de amplificação de ácidos nucleicos, observados na consulta de ISTs do Serviço de Dermatologia e Venereologia do Hospital Curry Cabral entre Janeiro de 2006 e Dezembro de 2009. Encontrou-se uma prevalência de infecção de 10,5% (81 doentes com infecção genital por Ct em 771 indivíduos testados). Na população com infecção por Ct foram encontradas diferenças com significado estatístico entre o sexo masculino e feminino na idade média dos doentes (homens=28,9 vs mulheres=24,3) p= 0,018; no número de companheiros sexuais nos últimos 6 meses (homens=3,32 vs mulheres=1,50) p=0,009; e no tempo em semanas decorrido entre o início dos sintomas e o diagnóstico (homens=2,65 vs mulheres=6,64) p= 0,032. Os doentes com co-infecção pelo Vírus da Imunodeficiência Humana (VIH) tinham um maior número de companheiros sexuais nos últimos 6 meses do que os não VIH p=0,003. Assim, existem diferenças demográficas, comportamentais e clínicas entre o sexo masculino e feminino e entre a população VIH e não VIH. Além disso, a elevada prevalência de infecção por Ct justifica os programas de diagnóstico precoce instituídos.

Background Portugal has the highest prevalence rate of HIV infection in Western Europe. The proportion of patients with a late diagnosis, carried out in full-blown AIDS stage, remains high. Skin and mucous membrane manifestations are not rare in these patients. Objective A demographic, clinical, and laboratorial characterization of patients with de novo HIV infection diagnosis made in the Department of Dermatology and Venereology of a central hospital in Lisbon, Portugal. Methods Retrospective review of medical records of adult patients newly diagnosed with HIV infection (reactive immunoassay for antibodies to HIV-1/HIV-2 or HIV p24 antigen) in the Dermatology and Venereology Department of a Portuguese central hospital in the period between January 2005 and December 2013. Results During the study period, 97 new cases were diagnosed, 70 men and 27 women. The median age at diagnosis was 36 years. Of the total, 50 cases were diagnosed with a concomitant sexually transmitted infection (STI), more frequently syphilis and ano-genital HPV infection. The remaining 47 patients were diagnosed with other dermatological conditions such as prurigo nodularis, psoriasis, and Kaposi’s sarcoma. The duration of complaints that lead to medical attention ranged from Conclusion This study grants a descriptive overview of de novo HIV infection diagnoses performed by dermatovenereologists in a central hospital in Lisbon, Portugal. As in the past, the importance of Dermato–Venereology for HIV infection diagnosis remains present in daily clinical practice.

COVID-19 lockdowns impose constraints on sexual health services and related medical activities. Our STI clinic favoured telemedicine as advised by specialist societies,1 illustrating the feasibility of maintaining STI care provision during a state of emergency. Recommendations regarding changes to sexual behaviour were also provided.2 At least theoretically, lockdown could lead to behavioural changes and a reduction in the risk of STIs. To date, published reports indicate that during the lockdown of Spring 2020, STI incidence was either comparable3 or reduced4 5 relative to previous years. Whether the apparent reduction indicates a true epidemiological change or rather an underestimation due to lack of presentation for diagnosis remains to be determined. Introdução: Assistiu-se nas últimas décadas a um aumento do número de casos de gonorreia notificados na Europa.Material e métodos: Foram revistos de forma retrospectiva os processos clínicos dos doentes observados na Consulta de IST do Hospital de Curry Cabral durante o período compreendido entre Janeiro de 2006 e Dezembro de 2013 (8 anos). O rastreio para a infecção por Neisseria gonorrhoeae é realizado em todos os doentes de forma sistemática através de técnica de diagnóstico molecular (BD Probetec-SDA system). Os autores procederam à caracterização epidemiológica e sociodemográfica bem como determinação da frequência da infecção por N. gonorrhoeae e respectivas localizações.Resultados: Durante o período em análise, num universo de 1735 doentes, foi diagnosticada gonorreia em 114 casos (6,6%), 104 em homens e 10 em mulheres, com uma mediana de idades de 29 anos. O diagnóstico de gonorreia verificou-se em 63 homens que têm sexo com homens (HSH) e 41 homens que têm sexo com mulheres. Dos doentes com diagnóstico de gonorreia, 26 eram seropositivos para o VIH (23%). A localização uro-genital foi a mais frequente, com 103 casos, seguindo-se a infecção do recto em 12 doentes e infecção da orofaringe em 9 doentes. De todos estes diagnósticos, 17% ocorreram em localizações nas quais os doentes não referiram sintomas (13 homens e 5 mulheres).Conclusões: Os resultados deste trabalho foram semelhantes aos dados publicados recentemente na literatura. Salienta-se o elevado número de casos em HSH. Neste contexto epidemiológico, a identificação deste agente em localizações sem queixas não é negligenciável, realçando a importância do rastreio extragenital em doentes assintomáticos nessas localizações.

A dermatoscopia constitui uma técnica de diagnóstico não-invasiva, in vivo, que permite complementar a observação clínica de lesões cutâneas não-pigmentadas de etiologia diversa. Nos tumores cutâneos não-pigmentados, a dermatoscopia facilita a observação de estruturas vasculares, aumentando a acuidade no seu diagnóstico, distinguindo tumores melanocíticos e não-melanocíticos, benignos e malignos. Na parte I deste artigo são discutidos os princípios básicos da avaliação dermatoscópica dos tumores cutâneos não-pigmentados, incluindo a morfologia vascular e os padrões de distribuição dos vasos, assim como de outras pistas adicionais.

O eritema exsudativo multiforme representa uma síndroma mucocutânea, imuno-mediada, resultante de mecanismos de hipersensibilidade associados a causas infecciosas ou farmacológicas. Nas primeiras verifica-se um predomínio da infecção pelo vírus herpes simples. A associação com a infecção por Mycobaterium tuberculosis é menos frequente. As Estatinas são um grupo farmacológico muito utilizado atualmente. São habitualmente bem toleradas, no entanto, podem estar implicadas na génese/agravamento de doenças auto-imunes como Lupus Eritematoso Cutâneo. Descreve-se o caso de uma doente caucasóide, de 74 anos que, 18 meses após início de terapêutica com sinvastatina, desenvolveu uma dermatose localizada à raiz dos membros superiores e dorso, constituída por pápulas e placas eritematosas, anulares e com superfície descamativa. Analiticamente era de salientar presença de ANA e anticorpos anti-SSA. O exame histopatológico foi consistente com a hipótese clínica de Lupus Eritematoso Cutâneo Subagudo. Após a suspensão da Sinvastatina assistiu-se à resolução completa do quadro clínico e laboratorial. O Lupus Eritematoso Cutâneo induzido por fármacos é uma entidade muitas vezes indistinguível das formas idiopáticas. Background: Dermatofibromas are common benign skin neoplasms. Main observations: The authors report the clinical, dermoscopic and histopathological features of a hemosiderotic dermatofibroma mimicking melanoma in an 85-year-old woman. Conclusions: Atypical dermoscopic patterns may prove difficult to differentiate from melanocytic lesions, and because of its polymorphic, melanoma-like presentation, definite diagnosis of hemosiderotic dermatofibroma can be reached by histopathological examination. Although infrequent, alopecia can be caused by benign cutaneous tumours. Neurofibromas are common benign tumors that originate in the peripheral nerve sheath. Diffuse neurofibroma is a rare variant of neurofibroma that is thought to occur mainly in the head and neck of children and young adults. Histology generally shows an infiltrative tumor, composed of spindle cells in the dermis and subcutaneous tissue. Despite being rare, malignant transformation has been reported. The association between diffuse neurofibromas and neurofibromatosis type 1 still remains to be determined since some authors question the low incidence usually described. We report the case of a 42-year-old man who had a diffuse neurofibroma on the scalp presenting as alopecic plaques.

Sweet's syndrome (SS) or acute febrile neutrophilic dermatosis is a reactive process that presents in different clinical settings and ranges from classical (idiopathic), malignancy associated or drug induced. The authors describe a 51-year-old Caucasian woman referred to our department with a 3-day history of pseudovesicular reddish papules on her neck, upper trunk and limbs. Two days prior to the eruption, aceclofenac 100 mg every 8 h was initiated for lower back pain. She also complained of high fever (39°C), arthralgias and general malaise. Laboratory evaluation showed an elevation of erythrocyte sedimentation rate and C reactive protein. A biopsy specimen of skin lesions showed throughout the upper reticular dermis a dense infiltrate of mature neutrophils. Aceclofenac was discontinued and oral prednisolone (0.5 mg/kg) was started. Fever resolved within 48 h, whereas cutaneous lesions cleared within the first week. No relapse was noted after a 6-month follow-up period. Drug-induced SS by aceclofenac diagnosis was sustained by the presence of all the five diagnostic criteria for drug-induced SS presented by Walker and Cohen in 1996. Several hundred cases of SS have been reported in the literature. However, drug-induced SS represent overall less than 5% of all cases, mostly as isolated clinical cases. Reports of nonsteroidal anti-inflammatory drug-induced SS include diclofenac, celecoxib and rofecoxib. Our patient represents the first case of aceclofenac-induced SS and illustrates the need to enquire about recent drugs in a patient with suspicion of SS.

Introdução: A resistência aos antibióticos em Neisseria gonorrhoeae tem-se revelado um importante problema de saúde pública mundial, estando a levantar grandes dificuldades em termos de opções terapêuticas em alguns países. Objectivo: Rever o panorama (nacional e internacional) da resistência aos antibióticos em Neisseria gonorrhoeae. Material e Métodos: Pesquisa de artigos em revistas nacionais e internacionais (estas últimas com in- dexação na Pubmed/Medline e redigidas em inglês). Utilizaram-se como palavras-chave: “Neisseria gonorrhoeae antibiotic resistance”. Resultados: As cefalosporinas de terceira geração (ceftriaxone e cefixima), associadas ou não à azitromicina, substituíram as fluoroquinolonas como fármacos de primeira linha no tratamento da gonorreia. Os rela- tos de resistência às cefalosporinas são ainda relativamente escassos; contudo, o aumento da concentração inibitória mínima (CIM), traduzido pela diminuição da sensibilidade a esta classe de antibióticos, tem vindo a ser regularmente descrita. Por outro lado, a resistência à azitromicina foi relatada em vários países. Em Portugal, já foram reportados casos de Neisseria gonorrhoeae resistentes à azitromicina mas não às cefalosporinas. Porém, estirpes com diminuição da sensibilidade às cefalosporinas foram já detetadas. Conclusão: Novas alternativas terapêuticas são indispensáveis para o tratamento das infeções por Neisseria gonorrhoeae, bem como condutas adequadas por parte dos médicos e dos doentes e seus contactantes.
