To perform a systematic review and meta-analysis on the association between knee extensor strength and the risk of structural, symptomatic, or functional deterioration in individuals with or at risk of knee osteoarthritis (KOA).We systematically identified and methodologically appraised all longitudinal studies (≥1-year followup) reporting an association between knee extensor strength and structural (tibiofemoral, patellofemoral), symptomatic (self-reported, knee replacement), or functional (subjective, objective) decline in individuals with or at risk of radiographic or symptomatic KOA. Results were pooled for each of the above associations using meta-analysis, or if necessary, summarized according to a best-evidence synthesis.Fifteen studies were included, evaluating8,000 participants (51% female), with a followup time between 1.5 and 8 years. Meta-analysis revealed that lower knee extensor strength was associated with an increased risk of symptomatic (Western Ontario and McMaster Universities Osteoarthritis Index [WOMAC] pain: odds ratio [OR] 1.35, 95% confidence interval [95% CI] 1.10-1.67) and functional decline (WOMAC function: OR 1.38, 95% CI 1.00-1.89, and chair-stand task: OR 1.03, 95% CI 1.03-1.04), but not increased risk of radiographic tibiofemoral joint space narrowing (JSN) (OR 1.15, 95% CI 0.84-1.56). No trend in risk was observed for KOA status (present versus absent). Best-evidence synthesis showed inconclusive evidence for lower knee extensor strength being associated with increased risk of patellofemoral deterioration.Meta-analysis showed that lower knee extensor strength is associated with an increased risk of symptomatic and functional deterioration, but not tibiofemoral JSN. The risk of patellofemoral deterioration in the presence of knee extensor strength deficits is inconclusive.

In this second line the Vico legacy is introduced to explain a conflict in Danish literature concerning its lack of an epic centre of historical and aesthetical understanding. Benjamin’s defence of the epicity in the work of Brecht is similarly discussed in this second part of the study. The third line of reasoning is presenting some older investigations on Danish prose into this question of what an epic dimension in the residual Danish culture might have been about in the last century. But all the investigations presented failed to get to the point. The point of dissidence was too weak and the point of national-socialism too clever to be manifest: It could easily hide behind the general cover up of theological aesthetics dominating Lutheran Denmark.So the fourth line of reasoning deals with political theology as a sort of interiorised state of mind in Denmark.The fifth line of reasoning presents two examples of something radically different and rather excluded in the political culture of Denmark: The Danish Council of Freedom (Danmarks Frihedsråd) during WWII which failed when it came to attacking the collaboration between Danish democracy and the Third Reich; and the Danish School of Writing (Forfatterskolen) which has been attacked by the local establishment since it was born 25 years ago.

Institutionalisation of medical innovations is often based on low scientific evidence. According to McKinlay a medical innovation follows a process of seven typical stages from a promising report to either implementation or denunciation. This process is exemplified with percutaneous vertebroplasty, an invasive procedure for painful vertebral fracture in patients with osteoporosis.

The prevalence of positional plagiocephaly is increasing. A systematic database search (Cochrane, MEDLINE, Embase, Cinahl and PEDro), critical appraisal and systematic evaluation of the quality of 14 selected studies show that the following are certain risk factors for the development of positional plagiocephaly: supine sleeping position, positional preference, one-sided handling of the child, male sex, delayed motor development, reduced neck mobility, too little "tummy time", firstborn, multiple births and premature children. Recommendations for prevention are provided.

A total of 570 injuries in handball players were pro spectively registered in a well-defined geographic area of 124,321 inhabitants. The incidence of handball inju ries was 46 per 10,000 inhabitants per year. The inci dence in women (61 per 10,000 per year) was double that of men (31 per 10,000 per year). Sixty-two percent of the injuries were ligament sprains and tears and 12% were fractures. A total of 7% of the patients were hospitalized and the total loss of income because of sick leave was $3870. Sixty-eight percent of the injured handball players were absent from handball more than 1 week. Surprisingly, 8% of the minor injuries resulted in a sick leave of more than 6 days.
