Irrigation of root canal system is of great significance to the success of endodontic treatment, where sodium hypochlorite (NaOCl) is the most widely used irrigant in chemical preparation. NaOCl functions by eliminating bacterial biofilms and dissolving organic tissue, which may vary according to several factors such as the microbiology of root canal infection and the concentration of the irrigant. It has been proposed that the effectiveness of NaOCl could be enhanced via several methods, including heating the irrigant, applying in conjunction with certain reagents, or activating by agitation techniques. Despite its antibacterial and tissue-dissolving capacities, NaOCl should be used with caution to avoid detrimental effect due to its cytotoxicity and negative effect on dentin properties. In this narrative review, we discussed the factors that affect the properties of NaOCl, the methods to improve its efficacy, and the side effects that might occur in clinical practice.

To explore the short- and long-term outcomes in patients with nasopharyngeal carcinoma (NPC) with magnetic resonance imaging (MRI)-detected residual disease at 3 months post-treatment who received intervention either promptly (0 month) or following observation (after an additional 3 months).A total of 272 patients with residual disease at 3 months post-treatment (observation [observation for additional 3 months]: 122, intervention [prompt intervention]: 150) were analyzed. Univariate and multivariate analyses were performed to examine the survival. Adverse events were analyzed in all patients.Patients in the observation group had a lower 3-year overall survival (77.1% vs. 85.2%), progression-free survival (10.2% vs. 18.1%), and locoregional relapse-free survival (10.2% vs. 20.6%) (allPatients with NPC with MRI-detected residual disease 3 months post-radiotherapy should be encouraged to undergo prompt intervention rather than adopting a passive wait-and-see policy.

Portal vein tumor thrombosis (PVTT), a common complication of advanced hepatocellular carcinoma (HCC), remains the bottleneck of the treatments. Liver cancer cells potentially experienced multi-steps during PVTT process, including cancer cells leave from cancer nest, migrate in extracellular matrix, invade the vascular barrier, and colonize in the portal vein. Accumulated evidences have revealed numerous of molecular mechanisms including genetic and epigenetic regulation, cancer stem cells, immunosuppressive microenvironment, hypoxia, et al. contributed to the PVTT formation. In this review, we discuss state-of-the-art PVTT research on the potential molecular mechanisms and experimental models. In addition, we summarize PVTT-associated clinical trials and current treatments for PVTT and suppose perspectives exploring the molecular mechanisms and improving PVTT-related treatment for the future.

Gallbladder neuroendocrine carcinoma (GB-NEC) has a low incidence rate; therefore, its clinical characteristics, diagnosis, treatment and prognosis are not well explored.To review recent research and analyze corresponding data in the Surveillance Epidemiology and End Results (SEER) database.Data of GB-NEC (The GB-NEC incidence rate was 1.6% (of all gallbladder carcinomas), male to female ratio was 1:2 and the median survival time was 7 mo. The 1-, 2-, 3- and 5-year overall survival (OS) was 36.6%, 17.8%, 13.2% and 7.3% respectively. Serum chromogranin A levels may be a specific tumor marker for the diagnosis of GB-NEC. Elevated carcinoembryonic antigen, carbohydrate antigen (CA)-19-9 and CA-125 levels were associated with poor prognosis. Age [hazard ratio (HR) = 1.027, 95% confidence interval (CI): 1.006-1.047,The clinical manifestations and prognosis of GB-NEC are similar to GB-ADC, but the treatment is completely different. Early diagnosis and treatment are the top priorities.

A novel mechanochemically assisted persulfate activation method was proposed in this study to enhance the leaching of valuable metals from lithium-ion batteries by combining ball-milling, advanced oxidation processes and sucrose reduction. By optimizing leaching parameters including temperature, pH, milling time and solid-to-liquid ratio, high leaching efficiencies of 97.1%, 94.0%, 87.6% and 93.8% can be achieved for Li, Ni, Co and Mn respectively. Lithium-ion batteries are increasingly important for providing energy storage solutions. In the drive to improve the energy density at the cell level, optimizing the electrode architecture is crucial in addition to researching new materials. Binder-free (BF) electrodes include porous pellets only containing battery electroactive materials. These electrodes can provide advantages with regard to mechanical stability and alleviated ion transport limitations relative to composite approaches for very thick and energy-dense electrodes. However, the absence of conductive additives often limits suitable material candidates for BF battery electrodes. TiNb2O7 (TNO) is a promising BF electrode material from a gravimetric and volumetric capacity standpoint, but phase pure TNO has relatively low electronic conductivity. Herein, a sucrose precursor coating method for TNO materials was implemented to process the TNO materials into BF electrodes. The sucrose served as a source to generate carbon in the electrodes, where the carbon coating resulted in an increase in rate capability, discharge voltage, and cycle life.

Background To develop and validate a predictive nomogram for tumor residue 3–6 months after treatment based on postradiotherapy plasma Epstein–Barr virus (EBV) deoxyribonucleic acid (DNA), clinical stage, and radiotherapy (RT) dose in patients with stage II–IVA nasopharyngeal carcinoma (NPC) treated with intensity-modulated radiation therapy (IMRT). Methods In this retrospective study, 1050 eligible patients with stage II–IVA NPC, who completed curative IMRT and underwent pretreatment and postradiotherapy (-7 to +28 days after IMRT) EBV DNA testing, were enrolled from 2012 to 2017. The prognostic value of the residue was explored using Cox regression analysis in patients (n=1050). A nomogram for predicting tumor residues after 3–6 months was developed using logistic regression analyses in the development cohort (n=736) and validated in an internal cohort (n=314). Results Tumor residue was an independent inferior prognostic factor for 5-year overall survival, progression-free survival, locoregional recurrence-free survival and distant metastasis-free survival (all P Conclusions We developed and validated a nomogram model integrating clinical characteristics at the end of IMRT for predicting whether tumor will residue or not after 3–6 months. Thus, high-risk NPC patients who might benefit from immediate additional intervention could be identified by the model, and the probability of residue can be reduced in the future.

Context Childhood obesity increases the risk of chronic disease in adulthood. Objective To construct an early predictive model for a growth trajectory that is highly related to childhood overweight/obesity. Design Prospective cohort study. Settings Shanghai Birth Cohort (SBC) and US Collaborative Perinatal Project (CPP). Participants A total of 848 mother-child pairs in the SBC (2013-2016) and 22 691 pairs in the CPP (1959-1965) with 2- and 7-year follow-up, respectively. Main Outcome Measures A high-risk postnatal growth trajectory intimately associated with childhood overweight/obesity and its predictive model. Results We demonstrated that the shifts of postnatal body mass index (BMI) percentile had been completed around 1 year of age and identified a high-risk growth trajectory that was closely related to overweight/obesity [odds ratio 6.5 (95% CI 5.9, 7.2)] at 7 years old. Children with this trajectory presented with a consistent BMI around the 85th percentile after the age of 1 year. It could be recognized early after birth using a predictive model with 4 metabolites (tyrosine, glycine, octenoylcarnitine, and stearoylcarnitine), combined with sex, birth weight, and maternal prepregnancy BMI. The model had an area under the receiver operating characteristic curve of 0.869 (95% CI 0.779, 0.932), a sensitivity of 83.3% (95% CI 51.6%, 97.9%), and a specificity of 81.1% (95% CI 70.3%, 89.3%) in the validation data set. Conclusion Children with postnatal high-risk growth trajectories were significantly associated with subsequent overweight/obesity at 7 years old. Metabolite profiles at birth combined with clinical measures were able to predict at-risk children before overweight/obesity occurrence.

Different from the existing train delay studies that had strived to explore sophisticated algorithms, this paper focuses on finding the bound of improvements on predicting multi-scenario train delays with different machine learning methods. Motivated by the observation of deep learning methods failing to improve the prediction performance if the delay occurs rarely, we present a novel augmented machine learning approach to improve the overall prediction accuracy further. Our solution proposes a rule-driven automation (RDA) method, including a delay status labeling (DSL) algorithm, and the resilience of section (RSE) and resilience of station (RST) indicators to generate the forecast for train delays. The experiment results demonstrate that the Random Forest based implementation of our RDA method (RF-RDA) can significantly improve the generalization ability of multivariate multi-step forecast models for multi-scenario train delay prediction. The proposed solution surpasses state-of-art baselines based on real-world traffic datasets, which treat various real-time delays differently. Even when the predictability of conventional deep learning methods decreases, the performance of our method is still acceptable for practical use to provide accurate forecasts.

In the past few years, cloud edge system from conception to gradually improve, develop to today has become a mature side of cloud model. Compared with simple cloud computing, cloud edge system has lower network latency and is suitable for application scenarios with higher real-time requirements. Although it can make up for some shortcomings of cloud computing, it also faces many problems in practical application, how to realize the safe transmission of information in the open network environment is one of the difficulties. Since the data transmission process between the edge and the cloud exists in the form of plaintext transmission, this allows attackers to easily disrupt normal data interactions. It is an important research point to ensure the stable operation of the cloud edge system while maximizing the security of communication. To this end, the following work has been done in the direction of secure data transmission in this paper：1. Analyzed the potential risks of the cloud edge system communication network from the per-spective of data source security and data transmission security. 2. Summarizes the requirements and limitations of secure communication, introduces a light-weight SM9 encryption algorithm with low implementation cost and low resource consumption. And compared with other encryption algorithms. 3. Provide algorithmic foundation for establish a framework for secure transmission of light-weight data and achieve data confidentiality protection. Objective:To investigate liver histologic disease in chronic hepatitis B (CHB) patients with normal ALT (Methods:We retrospectively examined 1352 patients who underwent liver biopsy from 2017 to 2021 and then obtain their 1-year follow up data to establish a predict model of 1-year HBVDNA clearance rate through Logistic regression. Results:The proportion of patient with significant histological disease (significant liver necroinflammation or fibrosis) in the high nALT group was significantly higher than that in the low nALT group (56.43% vs 43.82%, p < 0.001, Table 1). HbeAg negative, GGT and ALT value (OR 0.024, 95% CI 0.011–0.055, p < 0.001; OR 1.027, 95% CI 1.006–1.050, p = 0.013; OR 1.993,95% CI 1.115–3.560, p = 0.020, Table 4) were independent predict factors. High normal ALT is an indicator of 1-year HBVDNA clearance after antiviral treatment. Conclusion: CHB patients with high-normal ALT have significantly higher histological disease rates, and these patients may benefit better from antiviral therapy. We suggest should undergo histological evaluation of liver disease as soon as possible. We also propose these patients should be offered antiviral therapy when feasible.

BackgroundLiver fibrosis is a reversible wound-healing response that can lead to end-stage liver diseases without effective treatment, in which HBV infection is a major cause. However, the underlying mechanisms for the development of HBV-induced fibrosis remains elusive, and efficacious therapies for this disease are still lacking. In present investigation, we investigated the effect and mechanism of green tea polyphenol epigallocatechin-3-gallate (EGCG) on HBV-induced liver injury and fibrosis.MethodsThe effect of EGCG on liver fibrosis was examined in a recombinant cccDNA (rcccDNA) chronic HBV mouse model by immunohistochemical staining, Sirius red and Masson’s trichrome staining. The functional relevance between high mobility group box 1 (HMGB1) and inflammasome activation and the role of EGCG in it were analyzed by Western blotting. The effect of EGCG on autophagic flux was determined by Western blotting and flow cytometric analysis.ResultsEGCG treatment efficiently was found to alleviate HBV-induced liver injury and fibrosis in a recombinant cccDNA (rcccDNA) chronic HBV mouse model, a proven suitable research platform for HBV-induced fibrosis. Mechanistically, EGCG was revealed to repress the activation of macrophage NLRP3 inflammasome, a critical trigger of HBV-induced liver fibrosis. Further study revealed that EGCG suppressed macrophage inflammasome through downregulating the level of extracellular HMGB1. Furthermore, our data demonstrated that EGCG treatment downregulated the levels of extracellular HMGB1 through activating autophagic degradation of cytoplasmic HMGB1 in hepatocytes. Accordingly, autophagy blockade was revealed to significantly reverse EGCG-mediated inhibition on extracellular HMGB1-activated macrophage inflammasome and thus suppress the therapeutic effect of EGCG on HBV-induced liver injury and fibrosis.ConclusionEGCG ameliorates HBV-induced liver injury and fibrosis via autophagic degradation of cytoplasmic HMGB1 and the subsequent suppression of macrophage inflammasome activation. These data provided a new pathogenic mechanism for HBV-induced liver fibrosis involving the extracellular HMGB1-mediated macrophage inflammasome activation, and also suggested EGCG administration as a promising therapeutic strategy for this disease.

BackgroundThe nerve-sparing (NS) effect of robot-assisted radical prostatectomy (RARP) on patients with a high-risk prostate cancer remains unclear. The objective of this study was to compare the urinary continence, erectile function and oncology outcomes of the nerve-sparing and non-nerve-sparing (NNS) group during RARP surgeries.MethodsWe systematically searched databases including PubMed, Embase, Cochrane Library and Web of Science to identify relevant studies published in English up to December 2022. Newcastle-Ottawa Scale (NOS) was used as a quality evaluation tool to evaluate the quality of the literature parameters involved, including urinary continence, erectile function and oncologic outcomes, which were compared using the Stata 15.1 software (StataSE, USA).ResultsA total of 8 cohort studies involving 2499 patients were included. A meta-analysis of results showed that the NS group was beneficial to the recovery of urinary continence (RR 0.46, 95%CI 0.22, 0.96; p=0.045ConclusionsThe current study results indicate that intraoperative NS during RARP is beneficial to long-term postoperative functional recovery and tumor prognosis of patients with high-risk prostate cancers. Purpose:Previous studies suggest that a cumulative cisplatin dose of 200 mg/m2 might be adequate in the intensity-modulated radiation therapy (IMRT) era for locoregionally advanced nasopharyngeal carcinoma (LANPC). However, two cycles of once-every-3-weeks cisplatin at 100 mg/m2 has never been prospectively compared with standard once-a-week cisplatin regimen.Patients and Methods:This trial was conducted at three hospitals from 2011 to 2016. Patients who met the eligibility criteria were recruited (ChiCTR-TRC-12001979) and randomly assigned (1:1) via a computer-generated sequence to receive once-every-3-weeks cisplatin at 100 mg/m2 for two cycles or once-a-week cisplatin at 40 mg/m2 for six cycles concurrently with IMRT. Primary endpoint was failure-free survival and between-group absolute difference of 10% as the noninferiority margin.Results:A total of 510 patients were enrolled. Median follow-up time was 58.3 months with 85.4% of 3-year failure-free survival in the once-every-3-weeks group and 85.6% in the once-a-week group. An absolute difference of −0.2% (95% confidence interval, −6.3 to 5.9; Pnoninferiority = 0.0016). Acute toxicities of grade 3 or higher occurred in 55.8% in the once-every-3-weeks group and 66.3% in the once-a-week group (P = 0.015). The most common acute toxicities were hematologic abnormalities, including leukopenia (16% vs. 27%; P = 0.0022) and thrombocytopenia (1% vs. 5%; P = 0.015). The late grade 3–4 auditory loss rate was significantly lower in the once-every-3-weeks group than the once-a-week group (6% vs. 13%; P = 0.0039).Conclusions:Once-every-3-weeks cisplatin as concurrent chemoradiotherapy is noninferior to once-a-week cisplatin in the treatment efficacy in the LANPC. Although both regimens are well tolerated, severe acute toxicities and late-onset auditory loss are higher in the once-a-week group.

Background Polycomb protein suppressor of zeste 12 (SUZ12) has been widely revealed involving in development and diverse physiopathology processes. However, the roles of SUZ12 in hepatocellular carcinoma (HCC) remain largely unknown. Methods Tissue microarray was used to examine the expression level of SUZ12. The gain- and loss- of function analysis were conducted to evaluate the effects of SUZ12 on the proliferation, migration and invasion of HCC cells. Meanwhile, luciferase reporter assay and RT-PCR assay were conducted to examine the effect of SUZ12 on transcriptional activity of chemokine receptors 7 (CXCR7). Results The expression level of SUZ12 was positively associated with HCC development, as revealed by tissue microarray analysis. Further gain- and loss- of function analysis demonstrated that SUZ12 promoted the proliferation, migration and invasion of HCC. Mechanistically, we found that SUZ12 could upregulate the expression of CXCR7 at the transcriptional level in HCC cells, and CXCR7 was revealed to contribute to the tumor-promoting roles of SUZ12. Of interest, luciferase reporter assay revealed SUZ12 positively controlled the CXCR7 via direct promoter combination but not epigenetic suppression. Moreover, high SUZ12 expression was positively correlated with CXCR7 in advanced patients with portal vein tumor thrombus. Conclusion Our findings indicate that SUZ12 plays pro-oncogenic roles in the progression of HCC, partially by activating CXCR7 signaling, especially in HCC patients with portal vein tumor thrombus. The SUZ12/CXCR7 axis may serve as a potential therapeutic target for treatments of advanced HCC patients.

Pancreatic cancer (PC) is one of the most fatal and chemoresistant malignancies with a poor prognosis. The current therapeutic options for PC have not achieved satisfactory results due to drug resistance. Therefore, it is urgent to develop novel treatment strategies with enhanced efficacy. This study sought to investigate the anticancer effect of gemcitabine and XCT790, an estrogen-related receptor alpha (ERRα) inverse agonist, as monotherapies or in combination for the treatment of PC. Here we demonstrated that the drug combination synergistically suppressed PC cell viability, its proliferative, migratory, invasive, apoptotic activities, and epithelial-to-mesenchymal transition (EMT), and it triggered G0/G1 cell cycle arrest and programmed cell death in vitro. In addition, in vivo assays using xenograft and mini-PDX (patient-derived xenograft) models further confirmed the synergistic antitumor effect between gemcitabine and XCT790 on PC. Mechanistically, gemcitabine and XCT790 suppressed PC by inhibiting ERRα and MEK/ERK signaling pathway. In conclusion, our current study demonstrated for the first time that gemcitabine combined with XCT790 displayed synergistic anticancer activities against PC, suggesting that their combination might be a promising treatment strategy for the therapy of PC.

To achieve higher power and energy density batteries, electrodes are often designed towards increased current densities and thicknesses. Under such conditions, electrode microstructure can become an important factor. Electrode microstructure is dependent on many factors, but one factor that can facilitate anisotropic microstructures is having anisotropic lithium-ion battery active material particles. Thus, robust methods to produce active materials with anisotropic particle morphologies are desirable. This manuscript will describe the use of an inhibitor to direct anisotropic morphologies of lithium-ion battery precursor particles. This anisotropic secondary platelet morphology was retained after conversion to final active material particles. The synthesis method should be generally applicable to producing a variety of transition metal oxide compositions with anisotropic morphologies, however, the inhibitor can have significant impacts on the rate of precipitation of the transition metals during precursor synthesis. Three exemplar materials will be described towards targeted synthesis of anisotropic cathode particles.

The advent of autonomous vehicles (AVs) is likely to introduce new mobility experiences for travelers. In particular, AVs would allow travelers to get off at the destinations and then drive themselves elsewhere to park rather than cruise for parking or park at a location with a high parking fee. The self-parking capability is likely to increase the utility of private-owned AVs (PAVs) and make this mobility option more attractive than human-driven vehicles. The present study investigates the dynamics of travelers shifting to PAVs from other transport modes and its negative impact on road traffic congestion. To this end, we propose an agent-based demand model which considers different travel cost components depending on crucial travel attributes such as trip purpose and activity duration. The estimated demand is then fed into a mesoscopic traffic simulation model to examine the resulting road traffic conditions. As charging private vehicles for the congestion they cause is an effective tool for demand management and congestion alleviation, we also integrate a distance-based pricing scheme into the overall modeling framework to investigate its impact on mode choice and transport network performance. A case study is conducted in Melbourne, Australia to demonstrate the proposed methodology. The results indicate that the distance-based pricing scheme can effectively limit the usage of PAVs and reduce traffic congestion, especially in the city center and peripheral suburbs.

Optical interferometric sensors have been widely used for highly sensitive detection of various physical quantities, and most of them operate in simple intensity interrogation mode. The deviation between the sensor's operating point and its quadrature point (Q-point) can cause signal distortion and sensitivity reduction. In this work, a new method for judging and compensating the above-mentioned deviation is proposed for improving sensor performance. Judgment of the degree of deviation is performed using the amplitude of the second harmonic (SH) component derived from the sensor's output signal, and the deviation compensation is achieved through wavelength tuning. The wavelength tuning direction is determined by the relationship between the two phases of fundamental frequency (FF) and SH components, which eliminates the ambiguity of the sign of wavelength change and expedites the tuning speed. The method was fully verified by simulation and experiment based on a Fabry-Perot interferometer (FPI) based fiber-optic microphone with a tunable laser. Experimental results show that the FF and SH amplitudes oppositely changes with changing the ambient temperature after fixing the laser wavelength and fast tuning of the laser wavelength minimizes the SH amplitude close to zero and simultaneously maximizes the FF amplitude, consequently maintaining the operating point of the microphone at its Q-point. Moreover, the proposed method shows high precision and strong robustness to the laser power fluctuation. According to the criterion that the total harmonic distortion allowed is less than 3%, the phase deviation between the microphone's operating point and its Q-point should be smaller than 0.004°, which can be easily satisfied by the method proposed in this work.

Purpose This study aims to explore the relation among US audit partners’ characteristics, their career advancement and audit quality. Design/methodology/approach This study uses data from Public Company Accounting Oversight Board Form AP, Auditor Reporting of Certain Audit Participants, and publicly available online data sources. The hand-collected data on audit partners’ personal characteristics include gender, work experience and educational background. The measures for audit quality include restatements and audit fees. Findings The authors find that audit partner characteristics matter for the time it takes an individual to reach partnership after completing a bachelor’s degree. There are significant differences in work experience and educational background between partners in the largest (Big N) audit firms and smaller (non-Big N) audit firms. Audit partner traits are related to audit quality, and the effects differ between Big N and non-Big N partners. Originality/value The literature has examined audit partners’ career paths using international data. However, little empirical academic research has examined the career advancement of US audit partners. This study provides initial insights on the career advancement of US partners on a large scale and complements the recent research that examines audit partner characteristics and audit quality in the US market.
