Abstract

The risk of ischemic events carried by different clusters of type 2 diabetes mellitus (DM) in the setting of secondary prevention is not definite and the association between DM and bleeding complications is controversial. We explored these issues in the START-ANTIPLATELET, a multicenter Italian registry including acute coronary syndrome (ACS) patients. Study outcome was 1-year incidence of the net composite endpoint including major adverse cardiovascular events (MACE) or any bleeding and its individual components across different DM strata (no DM, DM with or without insulin). Out of 951 patients, 20.0% had diabetes not on insulin and 2.5% had diabetes on insulin. The rate of the net composite endpoint was highest in patients receiving insulin (39.4 per 100 person-years vs 11.7 in diabetic patients not on insulin vs 14.0 in those without DM; p = 0.007). In DM, the higher risk of MACE was regardless of insulin use (p = 0.36). Conversely, the increase in bleeding complications was limited to patients on insulin (Hazard Ratio 2.31, 95% CI 0.93-5.71 vs no DM; p = 0.0105 across DM strata). On top of aspirin, the rates of the net composite endpoint were similar with ticagrelor/prasugrel or clopidogrel irrespective of DM status (p for interaction 0.63). In conclusion, in ACS patients, type 2 DM enhances the risk of MACE regardless of the DM cluster, whereas the propensity to bleeding related to DM seems confined to insulin-treated patients.

2. Le relazioni pericolose: il ruolo dell’infiammazione e delle comorbilità nell’infezione da HIV e da Covid-19

Abstract

Nelle persone che vivono con HIV (PLWH) i disturbi nella regolazione dell'espressione delle citochine possono influenzare in modo determinante l’esito della patologia favorendo, da un lato, l’insorgenza precoce di comorbilità e alterando, dall’altro, la risposta immune alle infezioni. Molte evidenze hanno dimostrato peraltro che l’esito della malattia da SARS-CoV-2 è strettamente correlato alla risposta immunitaria e alla preesistenza di comorbilità. È evidente che entrambi i virus sono in grado di attivare meccanismi infiammatori, acuti nel caso di SARS-CoV-2, cronici nel caso di HIV, mentre le comorbilità sembrano rappresentare nel primo caso una causa, nel secondo un effetto del danno dovuto al virus. La disregolazione immune dovuta ad HIV potrebbe influenzare in modo determinante la risposta a SARS-CoV-2 sebbene, al momento, non sia ancora chiaro se PLWH abbiano una prognosi peggiore o migliore in caso di COVID-19. Entrambi i casi pongono in evidenza quanto infiammazione e comorbilità siano ormai un aspetto fondamentale nella fisiopatologia di molte malattie infettive e debbano essere studiate con attenzione per le loro importanti implicazioni nella clinica e nella terapia.

Abstract

The relevance of cardiovascular role played by levels of serum uric acid is dramatically growing, especially as cardiovascular risk factor potentially able to exert either a direct deleterious impact or a synergic effect with other cardiovascular risk factors. At the present time, it still remains undefined the threshold level of serum uric acid able to contribute to the cardiovascular risk. Indeed, the available epidemiological case studies are not homogeneous, and some preliminary data suggest that the so-called "cardiovascular threshold limit" may substantially differ from that identified as a cut-off able to trigger the acute gout attack. In such scenario, there is the necessity to clarify and quantify this threshold value, to insert it in the stratification of risk algorithm scores and, in turn, to adopt proper prevention and correction strategies. The clarification of the relationship between circulating levels of uric acid and cardio-nephro-metabolic disorders in a broad sample representative of general population is critical to identify the threshold value of serum uric acid better discriminating the increased risk associated with uric acid. The Uric acid Right for heArt Health (URRAH) project has been designed to define, as primary objective, the level of uricemia above which the independent risk of cardiovascular disease may increase in a significantly manner in a general Italian population.

Abstract

AIMS Our aim was to report on a survey initiated by the European Association of Percutaneous Cardiovascular Interventions (EAPCI) concerning opinion on the evidence relating to dual antiplatelet therapy (DAPT) duration after coronary stenting. METHODS AND RESULTS Results from three randomised clinical trials were scheduled to be presented at the American Heart Association Scientific Sessions 2014 (AHA 2014). A web-based survey was distributed to all individuals registered in the EuroIntervention mailing list (n=15,200) both before and after AHA 2014. A total of 1,134 physicians responded to the first (i.e., before AHA 2014) and 542 to the second (i.e., after AHA 2014) survey. The majority of respondents interpreted trial results consistent with a substantial equipoise regarding the benefits and risks of an extended versus a standard DAPT strategy. Two respondents out of ten believed extended DAPT should be implemented in selected patients. After AHA 2014, 46.1% of participants expressed uncertainty about the available evidence on DAPT duration, and 40.0% the need for clinical guidance. CONCLUSIONS This EAPCI survey highlights considerable uncertainty within the medical community with regard to the optimal duration of DAPT after coronary stenting in the light of recent reported trial results. Updated recommendations for practising physicians to guide treatment decisions in routine clinical practice should be provided by international societies.

Abstract

To evaluate satisfaction rate, efficacy and complication rate of carbon dioxide (CO2) vs normal saline as uterine distension medium for outpatient diagnostic hysteroscopy in patients with suspicious endometrial cancer.Thirty-eight undergoing outpatient hysteroscopy performed with carbon dioxide (group A) or normal saline (group B). Quality of vision of uterine cavity, procedure time, complications, patient discomfort, satisfaction rate have been evaluated.In group B a significant reduction in abdominal pain, shoulder tip pain and a lower incidence of vasovagal reactions were observed in comparison to group A. In normal saline group we found a higher satisfaction rate and a lower operative time in comparison to the CO2 group.Uterine distension with normal saline seems to have less adverse effects and is better accepted by patients.

21. Effects of HIV 1 Tat protein on ion secretion and on cell proliferation in human intestinal epithelial cells

Abstract

BACKGROUND & AIMS: Severe diarrhea and enteropathy of unknown origin are frequent in patients infected with human immunodeficiency type 1 virus (HIV-1). The HIV-1 transactivating factor protein (Tat) is a key factor in the pathogenesis of acquired immunodeficiency syndrome. We investigated whether Tat could directly induce ion secretion and cell damage in enterocytes. METHODS: Electrical parameters (ion transport studies) were measured in Caco-2 cell monolayers and in human colonic mucosa specimens mounted in Ussing chambers. The effect of Tat on intestinal mucosa integrity was determined by monitoring the transepithelial electrical resistance of Caco-2 cell monolayers. (3)H-thymidine incorporation and cell count were used to evaluate the effect of Tat on cell growth. Intracellular calcium concentrations were measured at the single-cell level using microfluorometry technique. RESULTS: Tat protein induced ion secretion in Caco-2 cells and in human colonic mucosa similar to that induced by bacterial enterotoxins. It also significantly prevented enterocyte proliferation. In both instances, the effect of Tat was maximum at concentrations within the range detected in the sera of HIV-1-infected patients. Anti-Tat antibodies inhibited both effects. Ion secretion and the antiproliferative effects were mediated by L-type Ca(2+) channels. An increase in intracellular calcium concentration in Caco-2 cells was found after addition of Tat. CONCLUSIONS: These results indicate that Tat may be involved in HIV-1-related intestinal disease through direct interaction with enterocytes.

Abstract

Pentalogy of Cantrell is a rare congenital anomaly consisting of the following features: 1) midline supraumbilical abdominal wall defects; 2) deficiency of the anterior diaphragm; 3) defects in the diaphragmatic pericardium; 4) defects of the lower sternum; 5) congenital intracardiac defects. We report 3 cases of pentalogy of Cantrell diagnosed respectively at 13, 18 and 24 weeks of gestation. In case 1 Cantrell's pentalogy was diagnosed during the 1(st) trimester. Case 2 revealed the coexistence of cystic hygroma. Case 3 showed an association with dysplastic left kidney and mild pyelectasis of the right kidney. Our results confirm the possibility of an early detection of Cantrell's pentalogy and reveal the possibility of associations with other pathological findings.

Abstract

Background: Coronary drug-eluting stents have significantly reduced the rate of in-stent restenosis, but the rate of in-stent thrombosis seems increased. In this study, we tested whether Quinacrine (Q) might reduce smooth muscle cells (SMC) proliferation while exerting minor effects on endothelial cells proliferation (EC) or thrombosis. Methods: Human SMC and EC were treated with increasing concentrations of Q, and the effects on apoptosis or cell proliferation were tested. Next, we evaluated Q effects on tissue factor (TF) and cell adhesion molecules expression (CAMs) in EC. Finally, we tested Q effects on neointima formation and re-endothelialization in a rat model of carotid artery angioplasty. Results: In SMC, all tested Q concentrations reduced proliferation, increased p53 expression and apoptosis. In contrast, EC, pro-apoptotic effects and TF activation were only observed after prolonged treatment with the highest dose, while CAMs expression was never induced at all concentrations. In vivo, Q induced p53 levels and apoptosis in the neointima, and significantly reduced neointimal formation without affecting re-endothelialization. Conclusion: Q exerts pro-apoptotic effects with higher selectivity for SMC, without pro-thrombotic effects, and might represent a safer drug to prevent or treat artery restenosis after percutaneous interventions.
