BACKGROUND: Among patients with stable coronary disease and moderate or severe ischemia, whether clinical outcomes are better in those who receive an invasive intervention plus medical therapy than in those who receive medical therapy alone is uncertain. METHODS: We randomly assigned 5179 patients with moderate or severe ischemia to an initial invasive strategy (angiography and revascularization when feasible) and medical therapy or to an initial conservative strategy of medical therapy alone and angiography if medical therapy failed. The primary outcome was a composite of death from cardiovascular causes, myocardial infarction, or hospitalization for unstable angina, heart failure, or resuscitated cardiac arrest. A key secondary outcome was death from cardiovascular causes or myocardial infarction. RESULTS: Over a median of 3.2 years, 318 primary outcome events occurred in the invasive-strategy group and 352 occurred in the conservative-strategy group. At 6 months, the cumulative event rate was 5.3% in the invasive-strategy group and 3.4% in the conservative-strategy group (difference, 1.9 percentage points; 95% confidence interval [CI], 0.8 to 3.0); at 5 years, the cumulative event rate was 16.4% and 18.2%, respectively (difference, -1.8 percentage points; 95% CI, -4.7 to 1.0). Results were similar with respect to the key secondary outcome. The incidence of the primary outcome was sensitive to the definition of myocardial infarction; a secondary analysis yielded more procedural myocardial infarctions of uncertain clinical importance. There were 145 deaths in the invasive-strategy group and 144 deaths in the conservative-strategy group (hazard ratio, 1.05; 95% CI, 0.83 to 1.32). CONCLUSIONS: Among patients with stable coronary disease and moderate or severe ischemia, we did not find evidence that an initial invasive strategy, as compared with an initial conservative strategy, reduced the risk of ischemic cardiovascular events or death from any cause over a median of 3.2 years. The trial findings were sensitive to the definition of myocardial infarction that was used. Walking gait data acquired with force platforms may be used for person re-identification (re-ID) in various authentication, surveillance, and forensics applications. Current force platform-based re-ID systems classify a fixed set of identities (IDs), which presents a problem when IDs are added or removed from the database. We formulated force platform-based re-ID as a deep metric learning (DML) task, whereby a deep neural network learns a feature representation that can be compared between inputs using a distance metric. The force platform dataset used in this study is one of the largest and the most comprehensive of its kind, containing 193 IDs with significant variations in clothing, footwear, walking speed, and time between trials. Several DML model architectures were evaluated in a challenging setting where none of the IDs were seen during training (i.e., zero-shot re-ID) and there was only one prior sample per ID to compare with each query sample. The best architecture was 85% accurate in this setting, though an analysis of changes in walking speed and footwear between measurement instances revealed that accuracy was 28% higher on same-speed, same-footwear comparisons, compared to cross-speed, cross-footwear comparisons. These results demonstrate the potential of DML algorithms for zero-shot re-ID using force platform data, and highlight challenging cases.

7. Disconnected: What Can We Learn from Individuals with Very Low Nature Connection?

While nature connection, which describes a positive relationship between humans and the rest of the natural world, has been a focus of numerous research studies in the last few decades, relatively little attention has been paid to nature disconnection. While the majority of the populations reported in most studies tend to be highly connected, there is a small percentage of those who feel they have no connection to the natural world. In this paper, we examine this novel construct of nature disconnection through secondary analysis of existing data from the Monitor of Engagement with the Natural Environment survey (MENE) by Natural England. From our analysis of this disconnected population, we can see that they are more likely to be young (16–24 years old), male, not employed and living in rented accommodation. We also observe that they have lower levels of life satisfaction and pro-environmental behaviours. We go on to present an initial theoretical discussion as to the origins of disconnection and propose further research directions to tackle the under-theorisation of this construct.

Background Knowledge graphs (KGs) play a key role to enable explainable artificial intelligence (AI) applications in healthcare. Constructing clinical knowledge graphs (CKGs) against heterogeneous electronic health records (EHRs) has been desired by the research and healthcare AI communities. From the standardization perspective, community-based standards such as the Fast Healthcare Interoperability Resources (FHIR) and the Observational Medical Outcomes Partnership (OMOP) Common Data Model (CDM) are increasingly used to represent and standardize EHR data for clinical data analytics, however, the potential of such a standard on building CKG has not been well investigated. Objective To develop and evaluate methods and tools that expose the OMOP CDM-based clinical data repositories into virtual clinical KGs that are compliant with FHIR Resource Description Framework (RDF) specification. Methods We developed a system called FHIR-Ontop-OMOP to generate virtual clinical KGs from the OMOP relational databases. We leveraged an OMOP CDM-based Medical Information Mart for Intensive Care (MIMIC-III) data repository to evaluate the FHIR-Ontop-OMOP system in terms of the faithfulness of data transformation and the conformance of the generated CKGs to the FHIR RDF specification. Results A beta version of the system has been released. A total of more than 100 data element mappings from 11 OMOP CDM clinical data, health system and vocabulary tables were implemented in the system, covering 11 FHIR resources. The generated virtual CKG from MIMIC-III contains 46,520 instances of FHIR Patient, 716,595 instances of Condition, 1,063,525 instances of Procedure, 24,934,751 instances of MedicationStatement, 365,181,104 instances of Observations, and 4,779,672 instances of CodeableConcept. Patient counts identified by five pairs of SQL (over the MIMIC database) and SPARQL (over the virtual CKG) queries were identical, ensuring the faithfulness of the data transformation. Generated CKG in RDF triples for 100 patients were fully conformant with the FHIR RDF specification. Conclusion The FHIR-Ontop-OMOP system can expose OMOP database as a FHIR-compliant RDF graph. It provides a meaningful use case demonstrating the potentials that can be enabled by the interoperability between FHIR and OMOP CDM. Excessive sea turtle nest predation is a problem for conservation management of sea turtle populations. For green turtles (Chelonia mydas) nesting at Chagar Hutang beach, Redang Island in Malaysia, Asian water monitor lizards (Varanus salvator) are nest predator. To help deter water monitors from attacking nest, plastic mesh is placed on top of sea turtle nests, although this is not always successful in preventing predation. Due to human food waste is frequently dumped near sea turtle nesting area, no studies have documented the if Asian water monitor lizards are attracted by human food waste at Chagar Hutang beach. This study assessed the prevalence of Asian water monitors on the sea turtle nesting beach using passive track-count plots (2×1m) every 50 m along the beach 5-10 m above the high tide mark in the area used by sea turtles to construct their nests. Results indicated Asian water monitors were the only vertebrate nest predator at Chagar Hutang and were widely distributed along the beach and had a Passive Activity Index (PAI) of 1.25. The stomach contents of 20 Asian water monitors were examined by stomach flushing, and 76.8% of stomach contents were human food wastes, 21.2% were turtle egg and hatchling and only 1% were from other natural food sources. This study suggests human food waste attract Asian water monitor to Chagar Hutang beach and that this may result in water monitors visiting sea turtle nests at a higher rate compared to a situation if human food waste was not available.

Gregarious behaviours are common in animals and provide various benefits such as food acquisition and protection against predators. Many gregarious tropical species are shifting poleward under current ocean warming, creating novel species and social interactions with local temperate taxa. However, how the dynamics of these novel shoals might be altered by future ocean warming and acidification remains untested. Here we evaluate how novel species interactions, ocean acidification and warming affect shoaling dynamics, motor lateralization and boldness of range-extending tropical and co-shoaling temperate fishes under controlled laboratory conditions. Fishes were exposed to 1 of 12 treatments (combinations of three temperature levels, two pCO2 levels and two shoal type levels: mixed species or temperate only) for 38 days. Lateralization (a measure of asymmetric expression of cognitive function in group coordination and predator escape) of tropical and temperate species was right-side biased under present-day conditions, but side bias significantly diminished in tropical and temperate fishes under ocean acidification. Ocean acidification also decreased shoal cohesion irrespective of shoaling type, with mixed-species shoals showing significantly lower cohesion than temperate-only shoals irrespective of climate stressors. Tropical fish became bolder under ocean acidification (after 4 weeks), and temperate fish became bolder with increasing temperature, while ocean acidification dampened temperate fish boldness. Our findings highlight the direct effect of climate stressors on fish behaviour and the interplay with the indirect effects of novel species interactions. Because strong shoal cohesion and lateralization are key determinants of species fitness, their degradation under ocean warming and acidification could adversely affect species performance in novel assemblages in a future ocean, and might slow down tropical species range extensions.

Walking gait data measured using force platforms is a promising means for person re-identification in authentication and surveillance scenarios. We aimed to determine the most discriminant components of force platform data using a two-stream Convolutional Recurrent Neural Network (KineticNet). Each network in the two-stream architecture extracts features pertaining to a single stance phase and then these features are fused to represent the entire gait cycle. Over two sessions, ground reaction forces (Fx, Fy, Fz), moments (Mx, My, Mz), and center of pressure coordinates (Cx, Cy) were acquired from 118 participants as they walked our laboratory five times at preferred speed. For each participant and each session, up to three samples were reserved for network training, leaving one sample for network validation and one sample for network testing. KineticNet’s performance was evaluated using both individual component and multi-component inputs before ablation studies were conducted on its architecture. Fz was the most discriminant individual component, and re-identification using Fz, Fy, and Cy together was the most accurate overall at 96.02%. These results warrant further investigation into the utility of force platforms as an accessory or alternative to video cameras for gait based person re-identification.

This protocol details the preparation and execution of CRISPR/Cas9 genome editing in S. rosetta. The protocol builds on a method to transfect macromolecules into S. rosetta for delivering a purified Cas9 ribonucleoprotein from Streptomyces pyogenes (SpCas9 RNP) into S. rosetta. Upon cleaving the S. rosetta genome at locations specified by the guide RNA (gRNA) of the SpCas9 RNP, S. rosetta can use DNA oligonucleotides as templates to repair the double-stranded break. Those repair templates can encode foreign sequences and mutations for editing the S. rosetta genome, so long as DNA oligonucleotides have >30 bases of sequence that is homologous to both sides of the Cas9 cleavage site.

Visual Abstract, Highlights • The impact of breastfeeding on prolactin, cellular immune activation, and myocardial recovery was analyzed in 100 women with peripartum cardiomyopathy • Cardiac function was assessed by echocardiography at presentation and at serial intervals over the first year postpartum • The levels of circulating prolactin were assessed by ELISA, and cellular immunophenotyping by flow cytometry, and compared between breastfeeding and nonbreastfeeding women • Prolactin levels were higher in breastfeeding women and correlated with significant increases in CD8+ T cells • Despite significantly higher prolactin levels and increased CD8+ cells, myocardial recovery was similar in breastfeeding and nonbreastfeeding women, Summary The etiology of peripartum cardiomyopathy remains unknown. One hypothesis is that an increase in the 16-kDa form of prolactin is pathogenic and suggests that breastfeeding may worsen peripartum cardiomyopathy by increasing prolactin, while bromocriptine, which blocks prolactin release, may be therapeutic. An autoimmune etiology has also been proposed. The authors investigated the impact of breastfeeding on cellular immunity and myocardial recovery for women with peripartum cardiomyopathy in the IPAC (Investigations in Pregnancy Associated Cardiomyopathy) study. Women who breastfed had elevated prolactin, and prolactin levels correlated with elevations in CD8+ T cells. However, despite elevated prolactin and cytotoxic T cell subsets, myocardial recovery was not impaired in breastfeeding women.

Deep learning algorithms have shown remarkable performance in classification tasks, however, they typically perform poorly with small training datasets due to overfitting. Overfitting occurs for all data types, although for the purposes of this study we are interested in time-based signals. This study introduces a novel technique to simulate time series signals from a dataset of categorically labeled data which can be used to train a deep neural network. The objective is to improve the predictive accuracy of a deep neural network on a separate validation dataset. To demonstrate the simulation methodology and improvements to the model's performance, a small dataset of ground reaction forces was used with the goal of identifying a person based on the raw signal. Our results show that the simulation method presented improves validation accuracy and reduces model training time for each of the three signal types.

This study developed and evaluated a JSON-LD 1.1 approach to automate the Resource Description Framework (RDF) serialization and deserialization of Fast Healthcare Interoperability Resources (FHIR) data, in preparation for updating the FHIR RDF standard. We first demonstrated that this JSON-LD 1.1 approach can produce the same output as the current FHIR RDF standard. We then used it to test, document and validate several proposed changes to the FHIR RDF specification, to address usability issues that were uncovered during trial use. This JSON-LD 1.1 approach was found to be effective and more declarative than the existing custom-code-based approach, in converting FHIR data from JSON to RDF and vice versa. This approach should enable future FHIR RDF servers to be implemented and maintained more easily.

Background Patients with chronic kidney disease, especially those receiving hemodialysis (HD), are at risk of hyperkalemia (HK). This systematic review aimed to evaluate the prevalence of HK in patients with renal disease receiving HD and collate evidence on the effect of HK and differing HD patterns (i.e., long vs. short inter-dialytic intervals [LIDI and SIDI, respectively] in a thrice weekly schedule) on mortality. Methods Comprehensive searches were conducted across six databases and selected conference proceedings by two independent reviewers up to September 2020. A hundred and two studies reporting frequency of HK, mortality, or cardiovascular (CV) outcomes in adult patients with acute, chronic or end-stage renal disease in receipt of HD were included. Narrative synthesis of results was undertaken with key findings presented in tables and figures. Results Median prevalence of HK in patients with renal disease receiving HD was 21.6% and increased in patients receiving concomitant medications – mainly renin–angiotensin–aldosterone system inhibitors and potassium-sparing diuretics. Associations between elevated potassium levels and increased risk of both all-cause and CV mortality in the HD population were consistent across the included studies. In addition, there was a rise in all-cause and CV mortality on the day following LIDI compared with the day after the two SIDIs in patients on HD. Conclusions Evidence identified in this systematic review indicates a relationship between HK and LIDI with mortality in patients with renal disease receiving HD, emphasizing the need for effective monitoring and management to control potassium levels both in emergency and chronic HD settings.

22. Effect of Sand Types on the Early Swimming Performance of Green Turtle Hatchlings

- Abstract
The selection of the nest site is a critical factor whereby female turtles may choose to nest at the site that minimizes energy expenditure of hatchlings and time spent to construct the nest. Nest substrate such as grain size is hypothesized to influence hatchling fitness by affecting the amount of energy reserves used by the hatchlings during emergence. Throughout Malaysia more than half a million green turtle hatchlings are released from hatcheries annually, however the question of whether this practice is producing high-quality hatchlings remains unanswered. It is crucial to hatchery management to identify which types of sand are most suitable for egg incubation and beneficial for hatchlings during nest emergence. We measured the influence of sand types on hatchlings fitness in terms of self-righting ability and swimming performance which emerged from the coarse and fine grained sand. Ten nests which had reach 45 days of incubation duration were selected as the developing embryos would be robust enough to be transported from the nesting to the laboratory for further incubation. Eggs from each clutch were split and allowed to continuing incubating in two different experimental conditions, fine and coarse grain. After hatched, the morphological characteristics and self-righting ability of hatchlings were measured then placed inside the glass aquarium to test the swimming performance for 18 hours. Results revealed that sand types only influenced the straight carapace width (SCW) of hatchlings but had no effect on their swimming. Cooler nest temperatures and longer incubation periods in nests constructed in fine sand caused the hatchlings to have larger straight carapace width than hatchlings from coarse sand. We suggest that the relocation of eggs in the late incubation period within Chagar Hutang beach is fine. Hence, we concluded that green turtle hatchlings emerging from nests in either fine or coarse grain sands of Chagar Hutang beach did not have a significant impact on their swimming performance.

Access to knowledge has never been easier in the internet age, and so it is important that students develop skills to discriminate undependable information from reliably investigated research. We have created an exercise that teaches good research practice by exploring the history, ethics, and design of clinical trials. Students apply their understanding of these principles through an assessed systematic review and meta-analysis (SRMA) exercise. Here, a clinically themed hypothesis is tested using a structured literature search in conjunction with an eligibility matrix to map study design, ethics, subject selection, randomization and blinding, methodological standards, study power, and other potential sources of interstudy heterogeneity. Data extracted from selected studies are used to produce a forest plot with an aggregated effect size, confidence range, and measure of interstudy heterogeneity. A funnel plot is then used in conjunction with the eligibility matrix to evaluate study bias tendency, and, in this way, students reflect on the factors that promote disparate conclusion-making among studies with a common research focus. This exercise produced a normally distributed grade-profile across three academic-year cohorts, and comparison of individual exercise grade with year-long aggregated average suggested students who performed less well on conventional assignments engaged successfully with the systematic nature of this assessment. Those opting to use this format for their final-year capstone project also performed above their grade point average from the preceding year. We suggest that SRMA offers a readily applied method for students to quantitatively explore how differences in experimental research practices influence study dependability.

Power electronic devices are the standard solution for lowvoltage (LV) heater control; however, cost and harmonic issues are amplified for medium-voltage (MV) heater applications. To provide an MV alternative at a significantly lower cost, this article details modifications and testing that will evolve part-range and load-tap-changing voltage regulators into full-range variable voltage transformers (VVT) for MV heater control. From the initial minor modifications that converted autotransformers into 16-tap isolation transformers, the development moves on to the inclusion of secondary base windings that bring 32 taps into play for refined control. Three-phase solutions and the addition of MV interrupters from the utility industry provide an economical solution for a stand-alone MV heater operation that avoids the harmonic issues of other heater control technologies.

Resource Description Framework (RDF) is one of the three standardized data formats in the HL7 Fast Healthcare Interoperability Resources (FHIR) specification and is being used by healthcare and research organizations to join FHIR and non-FHIR data. However, RDF previously had not been integrated into popular FHIR tooling packages, hindering the adoption of FHIR RDF in the semantic web and other communities. The objective of the study is to develop and evaluate a Java based FHIR RDF data transformation toolkit to facilitate the use and validation of FHIR RDF data. We extended the popular HAPI FHIR tooling to add RDF support, thus enabling FHIR data in XML or JSON to be transformed to or from RDF. We also developed an RDF Shape Expression (ShEx)-based validation framework to verify conformance of FHIR RDF data to the ShEx schemas provided in the FHIR specification for FHIR versions R4 and R5. The effectiveness of ShEx validation was demonstrated by testing it against 2693 FHIR R4 examples and 2197 FHIR R5 examples that are included in the FHIR specification. A total of 5 types of errors including missing properties, unknown element, missing resource Type, invalid attribute value, and unknown resource name in the R5 examples were revealed, demonstrating the value of the ShEx in the quality assurance of the evolving R5 development. This FHIR RDF data transformation and validation framework, based on HAPI and ShEx, is robust and ready for community use in adopting FHIR RDF, improving FHIR data quality, and evolving the FHIR specification.

This paper investigates the digital privacy provisions in the island of Jersey. This is mainly done by examining the two key pieces of legislation with which any workplace must comply with. Those legislations are: The Employment (Jersey) Law 2003, and the Data Protection (Jersey) Law 2005. Neither law has digital privacy provisions per se, but these are the two prevalent pieces of legislation governing employer-employee relations in the island of Jersey at present. The study relates the local legislation in the island of Jersey to its UK and European counterparts identifying the missing parts. To investigate how much compliance to those legislations in the island's workplace, a typical workplace of Lysaght's, the current employer for the second author, is chosen as a case study. The study highlights the areas of concerns and provides specific recommendations for the employer. This work can be used as the basis for steering the future judicial review of this area, to ensure the island of Jersey remains in compliance with its peers both domestically and internationally.

OBJECTIVE: The aim of this work was to evaluate the hypothesis that the distribution of circulating immune cell subsets, or their activation state, is significantly different between peripartum cardiomyopathy (PPCM) and healthy postpartum (HP) women. BACKGROUND: PPCM is a major cause of maternal morbidity and mortality, and an immune-mediated etiology has been hypothesized. Purpose – The purpose of this paper is to explore the economic benefits of Individual Placement with Support programmes commissioned by NHS North in the North West and Yorkshire and Humber regions. Design/methodology/approach – A literature review was conducted and data collected from supported employment programmes in four localities. An econometric analysis was performed to evaluate likely savings for local commissioners and return on investment for the Treasury. Findings – Integration of employment support within mental health services is central to success. Econometric analysis showed that local commissioners could save £1,400 per additional job outcome by commissioning evidence-based interventions and there is a positive return on investment to the Treasury for every £1 spent there is a return to the Treasury of £1.04. Originality/value – This paper demonstrates the economic and social value of evidence-based supported employment for people with severe mental illness. The economic data generated could be helpful in encouraging investment in effective employment support in other areas. In Africa as in Asia, will successful agricultural transformation happen first in countries whose rulers are driven by concerns to avert fundamental rural-based political threats? This article explores this question with reference to Rwanda, where the political incentives are found to be different from those in comparable African countries. Whilst this did not immediately lead to the adoption of an appropriate agricultural strategy, following a major shock and some serious rethinking, policy has now turned a corner and the results are promising. This experience has revealed that the political economy of agricultural policy in Rwanda is distinguished by a capacity for learning from errors as well as a seriousness about implementation that are not widely observed elsewhere in the region.

Many articles have been written about arc-flash events, mostly with a single focus. This article brings forth a wider view with a time/distance analysis of different arc abatement technologies in medium-voltage, metal-clad switchgear. The IEEE C37.20.7 [8] actual test results data, obtained from a certified high-voltage test facility, offers the reader a true snapshot of pressures developed within standard metal-clad switchgear enclosure using the latest blend of arc abatement topologies. Comparisons are made between the three prevalent technologies, which address an arc flash by time-magnitude approach boundaries. This article also addresses the phenomena associated with electrical arcs within medium-voltage switchgear by developing an introductory understanding of the generation of an arc and the pressure damages that both equipment and personnel may encounter.

This article considers the possible long‐run implications of the liberalisation programme currently under way throughout Tropical Africa. A strengthening of private‐sector institutions and a corresponding shift in the relationship between the state and civil society is clearly on the agenda, but – the paper argues – it would not be fruitful for research to remain focused on the public‐private or state‐society balances as such. The more challenging and important questions have to do with (i) the substantially different kinds of private‐sector and civil society development which might result from the current policies, and (ii) the implications of any strengthening of civil societies for the emergence of more autonomous and effective states.

The 2005 Paris Declaration on Aid Effectiveness, reaffirmed in Busan in 2011, grew out of a consensus on the importance of ‘country ownership’ for the success of development efforts. But in Paris and again in Busan it was tacitly assumed that most aid-recipient countries already have development-oriented political leaderships. This article considers that assumption untenable and asks whether donors have any role in assisting the emergence of developmental regimes. Aid as such is probably on balance bad for the institutional fabric of poor developing countries. Therefore, firstly, more attention should be given to reforming the non-aid policies of donor countries which are known to affect for the worse the economic and political systems of developing countries. And secondly, more thought should be given to using aid to help country actors overcome the collective-action problems that prevent them from moving towards more developmental politics.

Academic debate on Rwanda has significant thematic gaps, and does not usually make use of a theoretically informed comparative framework. This article addresses one thematic gap – the distinctive approach of the RPF-led regime to political involvement in the private sector of the economy. It does so using the framework of a cross-national study which aims to distinguish between more and less developmental forms of neopatrimonial politics. The article analyses the RPF’s private business operations centred on the holding company known successively as TriStar Investments and Crystal Ventures Ltd. These operations are shown to involve the kind of centralized generation and management of economic rents that has distinguished the more developmental regimes of Asia and Africa. The operations of the military investment company Horizon and of the public–private consortium Rwanda Investment Group may be seen in a similar light. With some qualifications, we conclude that Rwanda should be seen as a developmental patrimonial state.
