ZUSAMMENFASSUNGIn einer Gruppe aus Expertinnen und Experten aus dem Bereich der Gynäkologie und der Kopfschmerzmedizin wurden die verschiedenen Aspekte des Zusammenhangs zwischen weiblichen Geschlechtshormonen und Kopfschmerzen, insbesondere Migräne, diskutiert. Auf Grundlage einer systematischen aktuellen Literaturrecherche wurde dann ein gemeinsames Konsensuspapier erarbeitet, das diese verschiedenen Aspekte in klinischer und therapeutischer Hinsicht umfasst. Insbesondere werden Informationen und Empfehlungen zur Einnahme der oralen hormonalen Kontrazeption bei Frauen mit Migräne, zur menstruellen Migräne, zur Situation in der Schwangerschaft und Stillzeit, zu postmenopausalen Kopfschmerzen und zu anderen hormonellen Situationen gegeben. Es handelt sich ausdrücklich nicht um eine Leitlinie, sondern um eine selbst initiierte Konsensusfindung.

ZUSAMMENFASSUNGMeist bessert sich die Migräne in der Schwangerschaft von allein. Sollte eine Akuttherapie der Migräne in der Schwangerschaft notwendig sein, sind Paracetamol und Ibuprofen (letzteres bis Woche 28) Mittel der Wahl. Alle vorliegenden Daten sprechen gegen ernsthafte Hinweise auf Teratogenität oder Embryotoxizität beim Menschen. ASS ist in der Schwangerschaft Analgetikum der zweiten Wahl. Eine „Low-dose“-Behandlung kann bei entsprechender Indikation in der ganzen Schwangerschaft durchgeführt werden. Auf die Verwendung von Metamizol sollte während der Schwangerschaft möglichst verzichtet werden. Sumatriptan ist Mittel der Wahl aus der Gruppe der Triptane zur Therapie von Migräneattacken, falls ein Triptan verwendet werden muss. Es ersetzt nicht die medikamentöse Migräneprophylaxe. Wenn eine medikamentöse Prophylaxe während einer Schwangerschaft notwendig ist, sind Betablocker Substanzen der ersten Wahl (mit der geringsten Gefährdung des Embryos oder Fötus). Amitriptylin ist Mittel der zweiten Wahl in dieser Situation, insbesondere bei komorbiden Depressionen. Alle anderen prophylaktischen Substanzen sollten nur mit äußerster Zurückhaltung eingesetzt werden.

ZUSAMMENFASSUNGBei der menstruellen Migräne gibt es verschiedene spezifische Therapieoptionen. So kann ca. 4–5 Tage vor dem erwarteten Menstruationsbeginn ein Östradiolpflaster mit 50 µg pro Tag für wenigstens 7 Tage als Kurzzeitprophylaxe ausprobiert werden; bei Unwirksamkeit kann dies mit 100 µg pro Tag wiederholt werden. Eine hormonelle Kontrazeption mit einem Mischpräparat oder Mehrphasenpräparat kann probatorisch abgesetzt werden. In besonderen Fällen ist eine durchgehende hormonelle Kontrazeption bis zu 2 Jahre sinnvoll.Patientinnen mit Migräne und Endometriose zeigen während der Menstruation höhere CGRP-Werte als in der Zyklusmitte, wohingegen dies bei Frauen ohne Migräne und Kontrollprobandinnen umgekehrt ist. Dies deutet auf eine mögliche Rolle von CGRP in den perimenstruellen Schmerzattacken hin. Weiterhin ist die Lebensqualität bei Patientinnen mit Migräne und Endometriose deutlich stärker beeinträchtigt als bei nur einer Erkrankung. Daher ist es klinisch relevant, diese Komorbidität zu berücksichtigen.Eine hormonelle Frau-zu-Mann-Transition geht mit signifikant mehr Kopfschmerzen einher als die umgekehrte Transition. Bei der Mann-zu-Frau-Transition scheint sich dagegen häufiger eine Migräne zu bilden, insbesondere mit Aura. Die Behandlung der Kopfschmerzen und der Migräne erfolgt wie bei anderen Betroffenen auch, allerdings muss die erhöhte Thromboseneigung durch die Hormonbehandlung berücksichtigt werden.

Little is known about health status outcomes reported by patients with heart failure with preserved ejection fraction (HFpEF) in non-clinical trial settings.Purpose: To better understand patient burden of HFpEF in terms of HF-specific functional and symptom status, HRQOL, healthcare resource utilization (HCRU) and costs in a US-based commercial and Medicare Advantage insured population.Patients and methods: We conducted a cross-sectional survey of patients with HFpEF and linked their survey and administrative claims data. Consenting, eligible patients completed a survey that included the 23-item Kansas City Cardiomyopathy Questionnaire (KCCQ-23) and the PROMIS Global Health-10 (GH-10) questionnaire, as well as clinical and demographic questions. HF medication use, HCRU and costs during the 12-month baseline period before the survey were determined from claims data. Generalized linear regression was used to assess the associations between baseline characteristics and the KCCQ-23 overall summary score.Results: Of 598 survey respondents with survey and claims data, 54.7% were female with mean age 74.0 years. The KCCQ-23 overall summary and clinical summary scores were 64.8 and 63.0, respectively, and the GH-10 physical and mental health summary scores were 39.9 and 45.5. Factors related to lower KCCQ-23 overall summary scores were HF treatment and symptom changes during the past 4-weeks before the survey, hospital admission during the past year, low household income, high comorbidity index, and morbid obesity (BMI> 40). Total all-cause healthcare costs were $38,243 during the year prior to the survey, of which 42% were HF-related.Conclusion: Patient-reported outcome measure scores indicated impairment due to HF symptoms and physical limitations in this real-world sample of patients with HFpEF, highlighting a need to assess patient-reported outcomes as well as the clinical and economic outcomes traditionally assessed by clinicians, health systems and payers.Keywords: heart failure with preserved ejection fraction, HFpEF, patient-reported health related quality of life, healthcare resource utilization, healthcare costs

The peat compaction method is currently adopted by Malaysia oil palm companies to mitigate the uprising environmental issues. This method is claimed to be effective in minimising the risk of fire through enhancement of soil moisture due to capillary effect. In this review, the authors discussed on the peatland function in global perspective, the important of peat soil compaction, emergence of potential peat compaction in oil palm plantation establishment, peat compaction processes and the effects of compaction on physicochemical properties and carbon emission via peat surface and fire. Authors also found that compaction terminology on tropical peatland should be defined wisely, as it closely depends on the initial water table level and the peat quality. Thus, the retrieved information could serve as basic platform to further probe into the highlighted aspects and may as well function as a guide for management of these sensitive ecosystems, particularly in light of carbon loss mitigation.

People have a strong preference for fairness. For many, fairness means equal reward and punishments for equal efforts and offences. However, this belief does not specify the units in which equality should be expressed. We show that people generally fail to take the interchangeability of units into account when judging and assigning fair punishments and reward. Therefore, judgments about and distributions of resources are strongly influenced by arbitrary decisions about which unit to express them in. For example, if points represent different monetary values for different recipients, people attempt to distribute money equally if money is salient but attempt to distribute points equally if points are salient. Because beliefs about fairness are a fundamental principle in many domains, the implications of these findings are broad. BackgroundThe think-aloud (TA) approach is a qualitative research method that allows for gaining insight into thoughts and cognitive processes. It can be used to incorporate a respondent's perspective when developing resource-use measurement (RUM) instruments. Currently, the application of TA methods in RUM research is limited, and so is the guidance on how to use them. Transparent publication of TA methods for RUM in health economics studies, which is the aim of this paper, can contribute to reducing the aforementioned gap.MethodsMethods for conducting TA interviews were iteratively developed by a multi-national working group of health economists and additional qualitative research expertise was sought. TA interviews were conducted in four countries to support this process. A ten-step process was outlined in three parts: Part A 'before the interview' (including translation, recruitment, training), Part B 'during the interview' (including setting, opening, completing the instrument, open-ended questions, closing), and part C 'after the interview' (including transcription and data analysis, trustworthiness).ConclusionsThis manuscript describes the step-by-step approach for conducting multi-national TA interviews with potential respondents of the PECUNIA RUM instrument. It increases the methodological transparency in RUM development and reduces the knowledge gap of using qualitative research methods in health economics.

14. An International Systematic Review Concerning the Effect of Social Media Exposure on Public Compliance with Infection Prevention and Control Measures During the COVID-19 Pandemic

Background Effective health communication is crucial for achieving positive population health outcomes. This is important for infection prevention and control (IPC), which relies on widespread public compliance to be effective. Whilst social media (SM) platforms disseminate health information, there is conflicting evidence to suggest their influence on compliance with public health guidelines. Objective To appraise the available evidence and determine the extent to which social media exposure contributes to public compliance with IPC measures. Methods A systematic protocol based on the MOOSE and PRISMA guidelines was followed. Observational studies were identified through systematic searches of four electronic databases and a hand search. Following data extraction, eligible studies were quality assessed, and the findings were integrated using narrative synthesis. Findings 15 studies were included. The studies located presented heterogeneous designs and findings. A weak positive association was identified, with 60% of the studies observing a positive association between social media and compliance with IPC measures. 26.7% identified a negative correlation, and 13.3% identified no correlation. Several factors were identified to mediate the relationship, including knowledge and conspiracy beliefs. Discussion This systematic review identified a weak positive correlation between exposure to social media and public compliance with recommended IPC measures, suggesting that social media should be utilised in future IPC related communication strategies. Social media provides a cost-effective, publicly accessible platform to disseminate accurate information, and neutralise misinformation. Recommendations are made for further research to reduce the uncertainty created by the identification of heterogenous studies.

The serval (Leptailurus serval) is a medium-sized felid that is widespread throughout Sub-Saharan Africa. We report on the first scientific record of the servaline morph in a serval in Akagera National Park, Rwanda. Though this morph has been documented in the forested ecosystems of West and Central Africa, the servaline morph has not been recorded in a predominantly savannah ecosystem. Further research into the prevalence of this morph outside of forest ecosystems and the adaptive advantage of the servaline coat pattern in servals is recommended. Our findings highlight the importance of conserving savannah ecosystems to preserve morphological diversity in a species.

In the late nineteenth century, San Francisco was booming and its merchant princes built grand stores such as City of Paris, Gump’s, The White House, I. Magnin, and The Emporium in the Union Square area. These stores were integral to the city’s emergence as a world-class commercial hub, starting with the mad scramble for supplies during the gold rush, and its transformation into a tourist mecca in the decades after World War II. This article looks at three of those retail enterprises: City of Paris, which satisfied desires for anything French; the I. Magnin empire, which catered to a selective upper-class clientele; and the Emporium department store, aimed at the middle class with its Bargain Basement and Santa for the kids.

20. Sometimes Your Heart Says ‘I Don't Know’: Insights From Parents of Undervaccinated Children

To explore drivers of suboptimal vaccination rates by understanding why parents without strong antivaccine beliefs do not fully vaccinate their children.Four focus groups were conducted in Washington state with 41 parents of children aged 24 to 48 months who delayed, declined, or missed some but not all vaccines. During the focus groups, parents were asked about reasons their child was undervaccinated, information that might address their concerns, and additional support they needed. Transcripts were analyzed using template analysis with deductive and inductive codes.Focus groups identified multiple reasons for parents deciding to delay or decline vaccines for their children, including issues of individualism and control. The groups emphasized the impact of personal circumstances, such as changes in insurance, on whether children were vaccinated. Our data also shed light on the reasons that parents do vaccinate their children, including school vaccination requirements, negative experiences with vaccine-preventable diseases, and a family tradition of vaccinating. Focus group participants offered suggestions for improving vaccine communication with parents such as having more parent/patient-friendly vaccine information, providing forums to discuss their concerns, and offering vaccination information in advance of well-child appointments.To achieve the full benefit of vaccines on individual and community health, we need better ways to address vaccine hesitancy and decrease barriers. We suggest that many hesitant parents would benefit from more dialog with health care providers about vaccines, more approachable educational materials, and enforcement of existing policies requiring vaccines in schools and childcare facilities.

Introduction Acute kidney injury (AKI) has been identified as one of the most common and significant problems in hospitalized patients with COVID-19. However, studies examining the relationship between COVID-19 and AKI in low- and low-middle income countries (LLMIC) are lacking. Given that AKI is known to carry a higher mortality rate in these countries, it is important to understand differences in this population. Methods This prospective, observational study examines the AKI incidence and characteristics of 32,210 patients with COVID-19 from 49 countries across all income levels who were admitted to an intensive care unit during their hospital stay. Results Among patients with COVID-19 admitted to the intensive care unit (ICU), AKI incidence was highest in patients in LLMIC, followed by patients in upper-middle income countries (UMIC) and high-income countries (HIC) (53%, 38%, and 30%, respectively), whereas dialysis rates were lowest among patients with AKI from LLMIC and highest among those from HIC (27% vs. 45%). Patients with AKI in LLMIC had the largest proportion of community-acquired AKI (CA-AKI) and highest rate of in-hospital death (79% vs. 54% in HIC and 66% in UMIC). The association between AKI, being from LLMIC and in-hospital death persisted even after adjusting for disease severity. Conclusions AKI is a particularly devastating complication of COVID-19 among patients from poorer nations where the gaps in accessibility and quality of healthcare delivery have a major impact on patient outcomes.

23. Predicting changes in the pharmacokinetics of CYP3A‐metabolized drugs in hepatic impairment and insights into factors driving these changes

Physiologically based pharmacokinetic models, populated with drug-metabolizing enzyme and transporter (DMET) abundance, can be used to predict the impact of hepatic impairment (HI) on the pharmacokinetics (PK) of drugs. To increase confidence in the predictive power of such models, they must be validated by comparing the predicted and observed PK of drugs in HI obtained by phenotyping (or probe drug) studies. Therefore, we first predicted the effect of all stages of HI (mild to severe) on the PK of drugs primarily metabolized by cytochrome P450 (CYP) 3A enzymes using the default HI module of Simcyp Version 21, populated with hepatic and intestinal CYP3A abundance data. Then, we validated the predictions using CYP3A probe drug phenotyping studies conducted in HI. Seven CYP3A substrates, metabolized primarily via CYP3A (fraction metabolized, 0.7-0.95), with low to high hepatic availability, were studied. For all stages of HI, the predicted PK parameters of drugs were within twofold of the observed data. This successful validation increases confidence in using the DMET abundance data in HI to predict the changes in the PK of drugs cleared by DMET for which phenotyping studies in HI are not available or cannot be conducted. In addition, using CYP3A drugs as an example, through simulations, we identified the salient PK factors that drive the major changes in exposure (area under the plasma concentration-time profile curve) to drugs in HI. This theoretical framework can be applied to any drug and DMET to quickly determine the likely magnitude of change in drug PK due to HI.

25. An Exploration of the Role of Refined Value Importance in Consumers’ Sustainable Disposal Behaviors

- Abstract
Research shows that consumers’ personal values influence their pro-environmental behaviors. However, studies of values and proenvironmental behavior have not examined how refined values relate to a range of sustainable disposal behaviors. In the current study, we examine associations between consumers’ refined value priorities and their behavior in four sustainable product disposal domains (recycling, donating, composting, and membership of giving/sharing groups) in a large and diverse sample of Australian adults. We show clear evidence of systematic relations between refined values and behaviors in each of these domains. While we show that motivationally aligned refined self-transcendence values are positively and self-enhancement values negatively associated with recycling, donating, and composting, we find the reverse relations for membership of giving/sharing groups. Our findings suggest that the study of consumers’ refined values across a range of behaviors can offer a more nuanced understanding of what motivates them to engage in sustainable product disposal behaviors, with the potential to inform the development of more targeted policies and initiatives encouraging consumer participation.

Background: Health-related quality of life (HRQoL) assessment in patients with acne is recommended by several national guidelines. There are several acne-specific HRQoL instruments. Objectives: Participants of the European Academy of Dermatology and Venereology (EADV) Task Forces (TFs) on QoL and Patient Oriented Outcomes (PO) and Acne, Rosacea, and Hidradenitis Suppurativa (ARHS) agreed to scrutinize aspects of existing acne-specific HRQoL instruments for their relevance in international study. Methods: Consensus agreement on items related to QoL was reached after an independent assessment by seven experts from the EADV TFs on QoL and PO, and a list of 97 items was prepared and proposed to a group of acne patients. In order to have data from patients to check if any important topics were overseen, another group of acne patients from participating countries was asked to list how acne influenced different aspects of their lives. Results: Based on results obtained from 601 acne patients from nine countries, most of the items and topics showed low relevance for acne patients especially during the previous month or shorter time periods. Based on percentage of relevance and factor analysis, short (6 items) and long (45 items) lists of the most relevant topics were formed. Conclusion: Most of the items and topics from the initial list showed low relevance for acne patients. None of the identified acne-specific HRQoL instruments contain all the items that were deemed most relevant to acne patients. For this reason, participating members of the EADV TFs on QoL and PO, and ARHs are in the process of developing a new acne-specific HRQoL instrument.

Plants have evolved to adapt to their neighbours through plastic trait responses. In intercrop systems, plant growth occurs at different spatial and temporal dimensions, creating a competitive light environment where aboveground plasticity may support complementarity in light-use efficiency, realizing yield gains per unit area compared with monoculture systems. Physiological and architectural plasticity including the consequences for light-use efficiency and yield in a maize-soybean solar corridor intercrop system was compared, empirically, with the standard monoculture systems of the Midwest, USA. The impact of reducing maize plant density on yield was investigated in the following year. Intercropped maize favoured physiological plasticity over architectural plasticity, which maintained harvest index (HI) but reduced light interception efficiency (ɛi) and conversion efficiency (ɛc). Intercropped soybean invested in both plasticity responses, which maintained ɛi, but HI and ɛc decreased. Reducing maize plant density within the solar corridor rows did not improve yields under monoculture and intercrop systems. Overall, the intercrop decreased land-use efficiency by 9%–19% and uncoordinated investment in aboveground plasticity by each crop under high maize plant density does not support complementarity in light-use efficiency. Nonetheless, the mechanistic understanding gained from this study may improve crop cultivars and intercrop designs for the Midwest to increase yield.

Youth in the juvenile justice system are a vulnerable, high-risk population. While the role of pediatricians and mental health professionals in providing care for these children is well studied, the surgical needs of this population are not well understood. We sought to characterize the physical trauma and surgical subspecialty needs of this population.A retrospective chart review was performed of all children transported under custody to a stand-alone urban children's hospital. Demographic information and inpatient and outpatient encounter data were collected and analyzed.Between January 2020 and March 2021, 74 patients were transported for 199 subspecialty evaluations. Sixty-nine (93%) were male, 66 (89%) identified as Black, and the median age was 16 y (range, 13-20). Of all patients, 19% had at least one documented medical condition, 43% had behavioral health history, and 73% had previous arrest. Of the 199 encounters, 137 were for physical trauma (65%). Of these, 47 (34%) were for physical trauma incurred at the time of their arrest. Sixty-three patients (85%) experienced previous physical trauma (69% blunt, 12% penetrating, and 7% both), 54% had documented head trauma, 23% had a history of self-harm, and 60% of girls had experienced sexual trauma. Of the 54 children with a previous arrest, 91% had a history of physical trauma compared to 70% who were not previously incarcerated (P = 0.03).Most subspecialty and emergency encounters for incarcerated children are for physical trauma, revealing an opportunity for trauma-focused care in this vulnerable population. Pediatric surgeons and emergency physicians play a major role in the care of incarcerated children.

Starting from Hannah Arendt’s reflections on the openness of the future and the function of promises for political action, this essay offers a new understanding of the political pedagogy at work in The Aesthetics of Resistance. Rather than being narrated by the author’s proxy, as much of the novel’s reception assumes, Weiss plays subversively with mask narration. The narrator’s selective reading of Théodore Géricault’s The Raft of the Medusa guides readers to a complex assessment of the costs and deformations necessitated by the narrator’s promise to remain part of the antifascist resistance. Via its poetics of looking away, the novel discloses a probing of history, politics, and collective action that contrasts sharply with the narrator’s stated poetics and teleological expectations. The Aesthetics of Resistance prepares its future readings as an open, not-yet-determined pursuit of aesthetic and political education.

In this introduction the editors first trace the reception of Peter Weiss’s Aesthetics of Resistance in (West) Germany and the United States. Focusing on the relation between politics and aesthetics, some critics characterized this relation as fraught, arguing that the novel’s supposedly outdated politics ultimately rendered it obsolete. Others approached the novel as a writing experiment articulating politics and aesthetics in tense but productive ways. Proceeding and departing from these established views of Weiss’s text, the editors then propose a reading that understands the novel as deliberately opening itself up to future readings, a text conceived and structured to engage readers of not-yet-determined futures.

34. Impact of saline infusion and citrate infusion rate during donor plasmapheresis

The purpose of our research was to evaluate the effects of saline infusion to counteract incidents of severe hypotension during donor plasmapheresis (DP; synonym with plasma donation), and on immunoglobulin G (IgG) content in collected plasma, and on donor hematological values. It examines the clearance (Cl) of IgG during DP without saline to calculate the distribution of citrate and citrate infusion rate (CIR) during DP. It is based on three studies: an observation of saline infusion in donors with severe drops in blood pressure at DP, and two crossover trials of 32 repeat plasma donors each on saline infusion during DP. We observed 50 cases with a significant drop of mean systolic blood pressure ≤90 mm Hg in 1 year, predominantly in young, female first-time donors. In trial 1, there was a reduction of IgG concentration in collected plasma related to the average IgG serum concentration without saline to 85.5% vs to 80.5% with saline (P .001). In trial 2, there was an important 11%-12% increase of the donor hematological values without saline during DP. An average of 84.6% of citrate was removed in retained plasma; our calculated minimal CIR of 0.16 ± 0.17 mg/kg/min was confirmed by other empirical studies. Our research has led to significant improvements in the practice of DP: (a) Saline should be infused at DP as clinically indicated in agreement with the donor. (b) Young, female first-time donors require special care. (c) The long-time effects of minimal amounts of citrate on bone metabolism are improbable.

35. Mental health of autistic adults during the COVID-19 pandemic: The impact of perceived stress, intolerance of uncertainty, and coping style

Autistic individuals are at particular risk of experiencing mental health problems during the COVID-19 pandemic, yet little is known about personal characteristics that may underlie this vulnerability. This longitudinal questionnaire study with 149 autistic and 147 non-autistic adults investigated the association between perceived stress and anxiety and depressive symptoms, measured 4 months later. In addition, the moderating impact of intolerance of uncertainty and coping styles on this association was examined. Confirming previous studies, autistic adults reported poorer mental health than non-autistic individuals. Results demonstrated similar moderation models across individuals with versus without autism. Perceived stress at timepoint 1 was associated with anxiety and depression at timepoint 2. Across both participant groups, individuals with maladaptive coping strategies and higher levels of intolerance of uncertainty at timepoint 1 showed more internalizing symptoms at timepoint 2. The findings demonstrate the burden of the pandemic on the mental health of autistic adults, especially on those with high levels of perceived stress, maladaptive coping strategies, or intolerance of uncertainty. Interventions to support autistic adults during and after the pandemic might involve providing psychoeducation about the impact of stress, coping, and intolerance of uncertainty on internalizing symptoms, and teaching more adaptive ways to cope with difficult circumstances. Lay abstract More and more research shows us that autistic individuals are at risk of experiencing mental health problems in response to the COVID-19 pandemic. However, little is known about why this is the case. At two timepoints during the pandemic, we asked 149 autistic and 147 non-autistic adults about feelings of anxiety, depression, and stress, and about characteristics that may explain why some (autistic) people have a larger chance of developing anxiety and depression during this pandemic. In our study, autistic adults experienced more anxiety and depression than non-autistic adults. Across autistic and non-autistic individuals, the people who experienced more stress at timepoint 1 experienced more anxiety and depression 4 months later. This was especially the case for those individuals who use maladaptive coping styles, such as denial or venting, and for those who have difficulties dealing with uncertain situations. Our findings show the burden of the COVID-19 pandemic on the mental health of autistic adults. Interventions to support autistic adults during and after the pandemic are needed, and they may want to focus on the negative impact of stress and teach autistic (and non-autistic) adults more adaptive ways to cope with stressful circumstances.

36. Feasibility of pair-housing of rats after cranial implant surgery

Rat models employing cranial implants are increasingly employed to facilitate neural stimulation and recording in freely moving animals. Due to possible damage to wound, implant or attached devices, rats with cranial implants are traditionally housed singly, and little information is available on group- or pair-housing. Here we describe a protocol for pair-housing rats following cranial implant surgery and describe our experience with pair-housing during post-surgical recovery and up to 16 weeks following surgery. Thirty-six adult Wistar rats of both sexes were implanted with deep brain stimulation electrodes. Ten rats were equipped with an additional wireless headstage. Rats were housed in stable pairs before surgery and re-introduced 0–18 h post-surgery. Rat grimace scores did not indicate pain after conclusion of the analgesia protocol, physiological parameters were in the normal range three days post-surgery and weight loss did not exceed 10%. Rats with a cement cap only were pair-housed continuously without damage to the headcap. Rats carrying an additional fragile headstage had to be separated during lights-off periods to prevent headstage damage but could be pair-housed during lights-on periods. Pair-housing is a feasible and effective method to facilitate the rats’ need for social companionship following cranial implant surgery.

Drug combination therapy is a promising strategy to enhance the desired therapeutic effect, while reducing side effects. High-throughput pairwise drug combination screening is a commonly used method for discovering favorable drug interactions, but is time-consuming and costly. Here, we investigate the use of reaction network topology-guided design of combination therapy as a predictive in silico drug-drug interaction screening approach. We focused on three-node enzymatic networks, with general Michaelis–Menten kinetics. The results revealed that drug-drug interactions critically depend on the choice of target arrangement in a given topology, the nature of the drug, and the desired level of change in the network output. The results showed a negative correlation between antagonistic interactions and the dosage of drugs. Overall, the negative feedback loops showed the highest synergistic interactions (the lowest average combination index) and, intriguingly, required the highest drug doses compared to other topologies under the same condition.

Background An infodemic is excess information, including false or misleading information, that spreads in digital and physical environments during a public health emergency. The COVID-19 pandemic has been accompanied by an unprecedented global infodemic that has led to confusion about the benefits of medical and public health interventions, with substantial impact on risk-taking and health-seeking behaviors, eroding trust in health authorities and compromising the effectiveness of public health responses and policies. Standardized measures are needed to quantify the harmful impacts of the infodemic in a systematic and methodologically robust manner, as well as harmonizing highly divergent approaches currently explored for this purpose. This can serve as a foundation for a systematic, evidence-based approach to monitoring, identifying, and mitigating future infodemic harms in emergency preparedness and prevention. Objective In this paper, we summarize the Fifth World Health Organization (WHO) Infodemic Management Conference structure, proceedings, outcomes, and proposed actions seeking to identify the interdisciplinary approaches and frameworks needed to enable the measurement of the burden of infodemics. Methods An iterative human-centered design (HCD) approach and concept mapping were used to facilitate focused discussions and allow for the generation of actionable outcomes and recommendations. The discussions included 86 participants representing diverse scientific disciplines and health authorities from 28 countries across all WHO regions, along with observers from civil society and global public health–implementing partners. A thematic map capturing the concepts matching the key contributing factors to the public health burden of infodemics was used throughout the conference to frame and contextualize discussions. Five key areas for immediate action were identified. Results The 5 key areas for the development of metrics to assess the burden of infodemics and associated interventions included (1) developing standardized definitions and ensuring the adoption thereof; (2) improving the map of concepts influencing the burden of infodemics; (3) conducting a review of evidence, tools, and data sources; (4) setting up a technical working group; and (5) addressing immediate priorities for postpandemic recovery and resilience building. The summary report consolidated group input toward a common vocabulary with standardized terms, concepts, study designs, measures, and tools to estimate the burden of infodemics and the effectiveness of infodemic management interventions. Conclusions Standardizing measurement is the basis for documenting the burden of infodemics on health systems and population health during emergencies. Investment is needed into the development of practical, affordable, evidence-based, and systematic methods that are legally and ethically balanced for monitoring infodemics; generating diagnostics, infodemic insights, and recommendations; and developing interventions, action-oriented guidance, policies, support options, mechanisms, and tools for infodemic managers and emergency program managers.

39. Forces on a Vertical Dam due to Solitary Impulse Wave Run-Up and Overtopping

Impulse waves feature tsunami-like characteristics and are generated by very rapid mass wasting, including landslides and avalanches, into water bodies. In engineered reservoirs, these waves may run up and overtop the dam, thereby exerting hydrostatic and dynamic forces on its structure. For this work, solitary waves were applied as a proxy for impulse waves and the forces acting on vertical structures during wave run-up and overtopping were investigated with hydraulic laboratory experiments. The solitary waves were generated in a wave channel with a piston-type wave generator and the horizontal pressure forces at a vertical wall as well as a vertical dam-like structure were measured with multiple pressure sensors. The discrete pressure data were used to interpolate pressure distributions and the resulting horizontal forces. Empirical equations were derived, approximating the measured maximum forces and their respective centers of pressure within ±7% for both cases with and without wave overtopping. For small freeboards, already small relative wave amplitudes may exert forces similar to those induced by an earthquake as estimated with a pseudo-static approach., Journal of Hydraulic Engineering, 149 (7), ISSN:0733-9429, ISSN:1943-7900

How do policymakers discover their energy security interests abroad? Conventional wisdom assumes states have an inherent interest in securing an affordable and steady supply of oil. In this paper, I show that policymakers often fail to realize such vital interests on their own. Instead, multinational actors like international oil corporations (IOCs) educate policymakers on their state’s security interests abroad. By integrating prior scholarship on corporate power with insights on lobbying in American politics, I theorize that multinational corporations like IOCs can influence security policy when two conditions are met: first, policymakers demand information on security policy because of issue complexities, bureaucratic inefficiencies, and structural holes in the international system; and second, these corporations possess social ties that grant them the access, trust, and legitimacy to supply those policymakers with information. In the context of energy security, IOCs provide information on foreign sources of oil, threats posed to access, and anticipatory strategies for protecting access. I apply the theory to the origins of U.S. lend-lease aid to Saudi Arabia in 1943. Through sequential analysis, process-tracing, and comparative counterfactual reasoning, I argue an American IOC hastened U.S. interests in securing Saudi oil by using its ties to lobby the Roosevelt Administration at a time when the Administration lacked information on the country. The theory and findings broaden the state-centric view of energy security, contribute new evidence to historiography on US–Saudi relations, and fill an important gap in our understanding of corporate lobbying in security policy.

Trata-se de um estudo conduzido pelo método da problematização fundamentada no Arco de Maguerez, articulado a revisão integrativa. Os artigos foram coletados na Base de Dados da Biblioteca Virtual em Saúde (BVS) com publicações entre 2015 a 2020. Foram selecionados 14 artigos no total nos idiomas português, inglês e espanhol, os quais foram representados no fluxograma de identificação e seleção dos artigos. Observou-se certa escassez de estudos relacionados ao tema proposto, sendo que a forma de atuação profissional, em demasiado, circunda o foco na doença. Dessa maneira, conclui-se a necessidade da assistência biopsicossocial.

Polo-like kinase 1 (Plk1) plays an important role in cell-cycle regulation. Recent work has suggested that Plk1 could be a biomarker of gemcitabine response in pancreatic ductal adenocarcinoma (PDAC). Although targeting Plk1 to treat PDAC has been attempted in clinical trials, the results were not promising, and the mechanisms of resistance to Plk1 inhibition is poorly understood. In addition, the role of Plk1 in PDAC progression requires further elucidation. Here, we showed that Plk1 was associated with poor outcomes in patients with PDAC. In an inducible transgenic mouse line with specific expression of Plk1 in the pancreas, Plk1 overexpression significantly inhibited caerulein-induced acute pancreatitis and delayed development of acinar-to-ductal metaplasia and pancreatic intraepithelial neoplasia. Bioinformatics analyses identified the regulatory networks in which Plk1 is involved in PDAC disease progression, including multiple inflammation-related pathways. Unexpectedly, inhibition or depletion of Plk1 resulted in upregulation of PD-L1 via activation of the NF-κB pathway. Mechanistically, Plk1-mediated phosphorylation of RB at S758 inhibited the translocation of NF-κB to nucleus, inactivating the pathway. Inhibition of Plk1 sensitized PDAC to immune checkpoint blockade therapy through activation of an antitumor immune response. Together, Plk1 suppresses PDAC progression and inhibits NF-κB activity, and targeting Plk1 can potentiate the efficacy of immunotherapy in PDAC. Significance: Inhibition of Plk1 induces upregulation of PD-L1 expression in pancreatic ductal adenocarcinoma, stimulating antitumor immunity and sensitizing tumors to immunotherapy.

Zusammenfassung Zielsetzung Bisher konnte der Ressourcenverbrauch bei einem Kontakt mit dem Justizsektor in gesundheitsökonomischen Evaluationen aus gesellschaftlicher Perspektive nicht monetär bewertet werden. Methodik Es wurden 12 Kostensätze (KS) zur Messung des Ressourcenverbrauchs basierend auf Definitionen des PECUNIA-Projekts ermittelt. KS wurden mittels Literatur- und Internetrecherche, direkter Korrespondenz mit Kostenträgern oder Proxys erhoben. Ergebnisse Es wurden KS für Polizei, Feuerwehr, Vandalismus, Gefängnisaufenthalt, Polizeigewahrsam, einfachen und schweren Diebstahl, geringfügigen und schweren Vandalismus an Immobilien und Anwaltskosten vor Gericht kalkuliert. Schlussfolgerung Bisher fehlten KS für den Justizsektor, die für ökonomische Evaluationen aus gesellschaftlicher Perspektive erforderlich sind. Die Nutzbarkeit der KS in Forschungsfeldern außerhalb der Gesundheitsökonomie betont die Relevanz dieser Arbeit. - Abstract
Head and neck cancer patients often undergo a demanding treatment schedule requiring radiation and chemotherapy. Adherence to these treatment schedules is affected by several issues, including socioeconomic factors, characteristics of the disease and treatment plan, symptoms, and side effects. Oncology advanced practitioners (APs) work directly with patients with head and neck cancer (HNC) during active treatment, often screening for and managing side effects and providing referrals. Evidence-based interventions and innovative strategies for the oncology AP to improve HNC patients’ quality of life and treatment adherence include frequent distress screening, AP-led educational interventions and symptom management clinics, and incorporating technology to allow for close contact with patients during treatment.

The 15 January 2022 climactic eruption of Hunga volcano, Tonga, produced an explosion in the atmosphere of a size that has not been documented in the modern geophysical record. The event generated a broad range of atmospheric waves observed globally by various ground-based and spaceborne instrumentation networks. Most prominent was the surface-guided Lamb wave (≲0.01 hertz), which we observed propagating for four (plus three antipodal) passages around Earth over 6 days. As measured by the Lamb wave amplitudes, the climactic Hunga explosion was comparable in size to that of the 1883 Krakatau eruption. The Hunga eruption produced remarkable globally detected infrasound (0.01 to 20 hertz), long-range (~10,000 kilometers) audible sound, and ionospheric perturbations. Seismometers worldwide recorded pure seismic and air-to-ground coupled waves. Air-to-sea coupling likely contributed to fast-arriving tsunamis. Here, we highlight exceptional observations of the atmospheric waves.

50. Kann man Super-Shrinks ausbilden?

- Abstract
ZusammenfassungEs werden die Ergebnisse einer von der Deutschen Forschungsgemeinschaft (DFG) geförderten längsschnittlichen Studie zur Kompetenzentwicklung von Psychotherapeut*innen in Ausbildung zusammenfassend dargestellt. Die Studie verfolgte das Ziel, die Entwicklung globaler verfahrensübergreifender Kompetenzen über einen Zeitraum von 3 Jahren zu untersuchen. Der Studie wurde das Kompetenzmodell der Bundespsychotherapeutenkammer zugrunde gelegt, das zwischen fachlichen, interpersonalen und personalen Kompetenzen unterscheidet. Die Kompetenzentwicklung wurde von Psycholog*innen zu Beginn der therapeutischen Ausbildung in kognitiver Verhaltenstherapie (KVT), tiefenpsychologisch fundierter Psychotherapie (TfP) oder Psychoanalyse (PA) im Vergleich zu einer Kontrollgruppe auf der Grundlage von Selbstberichten, Tests und Fremdauswertungen untersucht. Die Ergebnisse zeigten eine Verbesserung in Fachwissen, Fallformulierung, „healing involvement“ und der affiliativen Kommunikation mit Patient*innen, jedoch eine Stagnation und teilweise Verschlechterung im therapeutischen Arbeitserleben, in der Attributionskomplexität und der Introjektaffiliation. Veränderungen des positiven therapeutischen Arbeitserlebens wurden eher von Aspekten der Ausbildung beeinflusst, während Veränderungen im negativen therapeutischen Arbeitserleben von Aspekten der individuellen Persönlichkeit bestimmt wurden. Veränderungen im Introjekt wurden durch die Bindungsstile der Teilnehmenden erklärt. Die Studie zeigt, dass die erfolgreiche Kompetenzentwicklung durch eine stärkere Förderung globaler Kompetenzen, die als bedeutsam für das Erreichen eines Super-Shrink-Level eingeschätzt werden, ausgebaut werden könnte. Hierbei erscheinen die Aspekte Persönlichkeit und Bindungsstil als wichtige Bereiche der individuellen Förderung und einer Verringerung des ausbildungsbezogenen Stresslevels.
