Our aim was to asses the association between the decrease of FEV1 and the modifications of the indices (PEF, MEF25, MEF50 and MEF75% of FVC) during methacholine challenge. 105 patients of both sexes with mild bronchial asthma were included in this study and were subjected to methacholine test according to ATS standards. The results were analysed using analysis of variance (MANOVA), discriminant stepwise and multiple regression. The reduction of FEV1 induced by increasing dosage of methacholine was associated with a significant decrease of the other flow-volume curve indices but we can observe a different flow behaviour. The MEF50 was more significant during the first level of variations and the MEF75 and MEF25 being more significant during the subsequent phases. We conclude that the evaluation of the flow volume indices can be important for better study the site of action of methacholine.

Objective: The aim of this study was to determine which characteristics could better distinguish dementia with Lewy bodies (DLB) from Alzheimer's disease (AD) at the mild cognitive impairment (MCI) stage, with particular emphasis on visual space and object perception abilities. Methods: Fifty-three patients with mild cognitive deficits that were eventually diagnosed with probable DLB (MCI-DLB: n = 25) and AD (MCI-AD: n = 28) at a 3-year follow-up were retrospectively studied. At the first visit, the patients underwent cognitive assessment including the Qualitative Scoring Mini Mental State Examination Pentagon Test and the Visual Object and Space Perception Battery. The Neuropsychiatric Inventory Questionnaire, Unified Parkinson's Disease Rating Scale (UPDRS) and questionnaires for cognitive fluctuations and sleep disorders were also administered. Results: The best clinical predictor of DLB was the presence of soft extrapyramidal signs (mean UPDRS score: 4.04 ± 5.9) detected in 72% of patients, followed by REM sleep behavior disorder (60%) and fluctuations (60%). Wrong performances in the pentagon's number of angles were obtained in 44% of DLB and 3.7% of AD patients and correlated with speed of visual attention. Executive functions, visual attention and visuospatial abilities were worse in DLB, while verbal episodic memory impairment was greater in AD. Deficits in the visual-perceptual domain were present in both MCI-DLB and AD. Conclusions: Poor performance in the pentagon's number of angles is specific of DLB and correlates with speed of visual attention. The dorsal visual stream seems specifically more impaired in MCI-DLB with respect to the ventral visual stream, the latter being involved in both DLB and AD. These cognitive features, associated with subtle extrapyramidal signs, should alert clinicians to a diagnostic hypothesis of DLB.

The international literature suggests that the causes of weak financial sustainability of public organizations depend on both external and internal conditions. Whereas the external conditions are related to demographic and socio-economic factors, the internal conditions are associated with the political and managerial settings, and thus, the latter could be addressed by the local government with the aim to maintain financial sustainability over the long term. Based on a literature review, the authors explain the most consolidated trend of such variables with respect to the impact of those variables on financial sustainability, and simultaneously, they reveal a disproportion in what the literature has most recently analyzed. Ultimately, the authors highlight the need for further research regarding the managerial factors and additional cross-country comparisons of the roles of both external and internal conditions.

According to mental model theory, illusory inferences are a class of deductions in which individuals systematically go wrong. Mental model theory explains them invoking the principle of truth, which is a tendency not to represent models that falsify the premises. In this paper we focus on the illusory problems based on conditional sentences. In three experiments, we show that: (a) rather than not representing models that falsify the conditionals, participants have a different understanding of what falsifies a conditional (Experiment I); (b) specifically, participants think that a conditional with an impossible antecedent or consequent is false (Experiment 2); (c) if the domain of the conditionals in the illusory problems are expanded to show that their antecedents and consequents are possible, the participants find it easy to reach the correct conclusions (Experiment 3). According to our results, the illusory inferences based on conditional premises, differently from those based on disjunctive premises, are caused by a difference between the understanding of natural language factual conditionals and the truth table of the factual implication; the principle of truth is not necessary to explain them.

Visual-constructional apraxia is a prominent feature of dementia with Lewy bodies (DLB) that might help to clinically distinguish it from Alzheimer's disease (AD). The main goal of this study was to assess performance on the copy intersecting-pentagon item of the Mini-Mental State Examination with the new Qualitative Scoring method for the Pentagon copy Test (QSPT). In order to determine which aspects of the drawings might differentiate DLB from AD, pentagon drawings of autopsy-verified DLB (n = 16) and AD (n = 15) patients were assessed using the QSPT. The qualitative scoring encompasses the assessment of different parameters of the drawing, such as number of angles, distance/intersection, closure/opening, rotation, and closing-in. The QSPT scores were compared between groups using linear analyses and artificial neural network analyses at four different time points. Linear analyses showed that during the first evaluation, number of angles was the only parameter that showed a significant difference between DLB and AD patients. A gradual decline in other parameters and total pentagon score occurred in both groups during subsequent years, with greater decline for the DLB group. The artificial neural network analyses using auto-contractive maps showed that, with disease progression, DLB became related to relatively lower qualitative pentagon scores, whereas AD became related to relatively higher qualitative scores. These findings suggest that the QSPT might be a sensitive measure of visuo-constructive abilities able to differentiate DLB from AD at disease onset and as the diseases progress, but further studies on larger population are necessary in order to establish its clinical relevance. © 2014-IOS Press and the authors. To what extent the shift to Gov 2.0 technologies, embedded in the latest European governments’ strategies, is changing public service delivery in practice? Gov 2.0 technologies provide numerous opportunities for governments to create efficiencies and better serve the public. With the public sector facing intense scrutiny of its budgets it should clearly be exploring where these tools can ease financial pressure, guarantee democratic governance, and improve public service delivery. The interactive and collaborative nature of Gov 2.0 should be encouraged given the fact that public interaction is quickly moving to center stage on the public governance agenda, as it is increasingly recognized as a primary driver for innovation and value creation in both the private and public sectors. This chapter is structured as follows: The second section presents an overview of the strategic plans launched by the EU over the last decade in order to strengthen e-government services for citizens and analyzes to what extent the four major European countries have defined strategic objectives which imply the use of Gov 2.0 technologies. The extent to which the shift to Gov 2.0 technologies are changing public service delivery in practice is presented in the third section. Finally, in the conclusion we summarize the analysis highlighting advantages and challenges of Gov 2.0 development in Europe.

The purpose of this paper is to investigate the potential impact of IFRS on SMEs’ annual reporting, considering in particular: 1) the evaluation of their financial performance; 2) their creditworthiness and rating, according to Basel II. In the literature, little attention has been paid to the specific influence of IFRS on firm rating. The empirical evidence reported in this paper demonstrates the potentially important effect on the previous two issues. Accordingly, this research raises some strategic issues for individual firms and banks, as well as for the entire economic system. In fact, the current worldwide economic crisis highlights the perils of alternative sets of standards characterised by a different extent of conservatism. The paper describes the results of a multiple-case study, involving 11 firms and an Italian bank. The empirical evidence highlights the differences in these firms’ ratings, implementing alternatively the Italian or the IFRS model.
