To evaluate the real-world treatment outcomes in patients with neovascular age-related macular degeneration (nAMD) in Korea, focusing on retinal fluid resolution. This multi-institutional retrospective chart review study, analyzed medical records of patients with nAMD (age ≥ 50 years) who received their first anti-vascular endothelial growth factor (VEGF) treatment in ophthalmology clinics across South Korea between January 2017 and March 2019. The primary endpoint was the proportion of patients with retinal fluid after 12 months of anti-VEGF treatment. The association between fluid-free period and VA gains was also evaluated. A total of 600 patients were enrolled. At baseline, 97.16% of patients had retinal fluid; after 12 months of anti-VEGF treatment, 58.10% of patients had persistent retinal fluid. To investigate the risk of glaucoma development in patients with atrial fibrillation (A-fib) using Korean National Health Insurance Service data.The present study used a National Sample Cohort consisting of approximately one million random subjects who were tracked from 2002 to 2013 (12 years). Newly diagnosed glaucoma and A-fib were included based on the Korean Classification of Disease codes. The A-fib group consisted of patients who received an initial A-fib diagnosis between January 2003 and December 2007 as an index period (n = 8765). The control group (n = 43,352) was selected using a 1:5 propensity-score matching for social and demographic factors. Each subject was followed up until 2013. Multivariate Cox proportional hazard regression analysis was performed to compare the risk of glaucoma development between the A-fib group and the control group.The rate of glaucoma development was 3.54% in the A-fib group and 2.96% in the control group (P 0.0001). A-fib increased the risk of glaucoma development [hazard ratio = 1.31; 95% confidence interval (CI): 1.15 to 1.48] after adjusting for age, sex, comorbidities, residence, household income, and year of enrollment. In multivariable Cox regression analysis, patients with comorbidity of diabetes mellitus and chronic renal failure and those aged ≥50 years showed significantly higher risk of glaucoma development (all P 0.001).A-fib was significantly associated with the development of glaucoma after adjusting for potential confounding factors. Physicians may need to monitor patients with A-fib carefully for possible glaucoma development.

Background Patients who survive an acute phase of stroke are at risk of falls and fractures afterwards. However, it is largely unknown how frequent fractures occur in the Asian stroke population. Methods Patients with acute (< 7 days) ischemic stroke who were hospitalized between January 2011 and November 2013 were identified from a prospective multicenter stroke registry in Korea, and were linked to the National Health Insurance Service claim database. The incidences of fractures were investigated during the first 4 years after index stroke. The cumulative incidence functions (CIFs) were estimated by the Gray's test for competing risk data. Fine and Gray model for competing risk data was applied for exploring risk factors of post-stroke fractures. Results Among a total of 11,522 patients, 1,616 fracture events were identified: 712 spine fractures, 397 hip fractures and 714 other fractures. The CIFs of any fractures were 2.63% at 6 months, 4.43% at 1 year, 8.09% at 2 years and 13.00% at 4 years. Those of spine/hip fractures were 1.11%/0.61%, 1.88%/1.03%, 3.28%/1.86% and 5.79%/3.15%, respectively. Age by a 10-year increment (hazard ratio [HR], 1.23; 95% confidence interval [CI], 1.17–1.30), women (HR, 1.74; 95% CI, 1.54–1.97), previous fracture (HR, 1.72; 95% CI, 1.54–1.92) and osteoporosis (HR, 1.44; 95% CI, 1.27–1.63) were independent risk factors of post-stroke fracture. Conclusion The CIFs of fractures are about 8% at 2 years and 13% at 4 years after acute ischemic stroke in Korea. Measuring blood pressure (BP) at home and remote monitoring can improve the patient's adherence to BP control and vascular outcomes. This study evaluated the feasibility of a trial regarding the effects of an intensive mobile BP management strategy versus usual care in acute ischemic stroke patients. A feasibility-testing, randomized, open-labeled controlled trial was conducted. Remote BP measurement, data transmission, storage, and centralized monitoring system were organized through a Bluetooth-equipped sphygmomanometer paired to the participants' smartphones. Participants were randomized equally into intensive management (behavioral intensification to measure BP at home by texting, direct telephone call, or breakthrough visit) and control (usual care) groups. The primary feasibility outcomes were: 1) recruitment time for the pre-specified number of participants, 2) retention of participants, 3) frequency of breakthrough visit calls, 4) response to breakthrough visit call, and 5) proportions satisfying BP measurement criteria. Sixty participants were randomly assigned to the intensive management (n = 31) and control (n = 29) groups, of which 57 participants were included in the primary analysis with comparable baseline characteristics. Recruitment time from the first to the last participant was 350 days, and 95% of randomized participants completed the final visit (intensive, 94%; control, 98%). Eight breakthrough visit calls were made to 7 participants (23%), with complete and immediate responses within 3 ± 4 days. The median of half-day blocks fulfilling the BP measurement criteria per patient were 91% in the intensive group and 83% in the control group (difference, 12.2; 95% confidence interval, 2.2-22.2). No adverse events related to the trial procedures were reported. The intensive monitoring, including remote BP measurement, data transfer, and centralized monitoring system, engaged with behavioral intensification was feasible if the patients complied with the intervention. However, the device utilized would need further improvement prior to a large trial.

이 연구는 성인문해 중학학력인정과정에 참여한 ‘학습자의 관점’에서 성인문해교육제도의 의미와 한계를 탐구하는 것을 목적으로 하였다. 이를 위해 서울에서 최초로 A 평생학습관의 중학 3단계 과정에 재학 중인 9명의 문해학습자를 대상으로 2014년 12월에 반구조화된 면담을 실시하였다. 분석 결과, 문해학습자의 중학과정 경험은 네 국면으로 구분되었다. 중학과정이라는 제도화된 경로에의 ‘진입’, 구조화된 교육과정을 경험하는 ‘참여’, 교사의 제한적 지지를 통해 학업을 ‘지속’하는 국면, 그리고 원하지 않는 종결을 맞이하는 ‘수료’가 그것이다. 이와 같은 학습자의 경험에 비추어 볼 때, 중학학력인정제도는 몇 가지 한계를 노정한다. 첫째, 교육제도로서의 ‘공식성’은 지녔으나 이에 대한 사회적 지원을 제공하는 ‘공공성’은 결여하고 있다. 둘째, 성인학습자를 위하여 분량, 난이도 등을 조정하여 ‘교육 내용의 적정성’은 확보하였으나 학습자의 특성을 반영한 ‘적합성’은 낮은 편이다. 셋째, 과목별 구조화와 전문교원을 통해 ‘전문성’은 강조하지만, 전담교원과 시설 등은 취약하다. 넷째, 초등과정과 연계된 단계별 교육을 제공하는 ‘체계성’은 있으나, 중학과정 이후의 학습과정을 제공하는 ‘계속성’은 미비하다. 이러한 한계는 문해교육과 학력 간 결합의 의미를 재고찰할 필요가 있음을 시사한다.

이 연구는 저소득층 가정배경에도 불구하고 성공적으로 명문대에 입학한 대학생들의 대학생활 적응과정을 분석함으로써, 대학교육의 한계와 개선 방향을 모색하기 위한 것이다. 이러한 연구 목적을 실현하기 위하여 삼성꿈장학재단의 차세대리더육성 장학금을 지원받은 7명의 저소득층 대학생들을 대상으로 사례연구를 수행하였다. 분석 결과, 연구 참여자들은 개인적 강점과 학교교육의 시너지 효과에 힘입어 사회적 약자 배려 전형 등을 통해 명문대 진학에 성공했지만, 입학 후에는 출발선 격차와 이를 강화하는 대학교육 환경 등으로 인하여 학업적, 사회적으로 소속 대학에 원만하게 적응하지 못하고 있었다. 이는 대학들이 기회균등을 학생선발 과정에만 한정하고, 교육의 과정과 결과 면에서 보장하기 위한 노력을 기울이지 않은 채 주류 학생 중심(majorities-oriented)의 교육을 실시하고 있기 때문이다. 이러한 결과는 대학교육의 책무범위에 대한 근본적인 문제를 제기하며, 대학교육을 기관이나 교수가 아닌 학생 중심으로 전환함으로써 대학 내 약자를 배려하는 교육시스템을 구축할 필요성을 시사한다.

This article explores the implications of the recent increase in older students at Korean universities dominated by younger students. A case study design is used to gain an in-depth understanding of experiences of older students in a Korean university. Semi-structured interviews of twelve older students, three professors, two administrators, and four younger students at the university were conducted for the date collection. The results suggest that the experiences of Korean older students differed from those of counterparts in western countries since academic and extracurricular services of Korean universities were geared towards younger students and thus were unhelpful for older students with multiple responsibilities at work and home. Also, the results show that traditional hierarchical relationships between younger and older students created strains for older students who had to rely heavily on younger counterparts. Korea has invested heavily in education and the development of an education system aligned with its national development plans by adopting a sequential approach, both in terms of school levels and the quantity and quality of education. Working on one educational level at a time, Korea has focused on developing its educational system, beginning with primary education in the 1950s, secondary education in the 1970s and 1980s, and higher education in the 1990s and 2000s. The first goals pertained to meeting demand for the quantity of education provided. The universalization of primary, secondary and higher education was achieved in 1957, 1999 and 2000, respectively. Now, Korea has started to invest in factors that aim to improve the quality of education, using metrics such as pupil-teacher ratios, class size, student satisfaction with school, and research and development outcomes of higher education institutions (HEIs). The analysis of Korea's experiences and continuous endeavors to help its students lead happy and productive lives, and contribute to building a better nation can provide insights and meaningful lessons to the countries in Latin America and the Caribbean, too. LAC countries are also invited to share their educational experiences with Korea, given their own strengths in education, including the high satisfaction with school that students in LAC countries report. This type of knowledge sharing between Korea and the LAC countries would contribute to improve the education systems of both sides, which ultimately supports them to raise their students to be happier and more capable adults.

Korean Abstract: 체제전환국인 라오스는 2000년대 이후 우리나라의 ODA 지원이 크게 증가한 지역이다. 2000년대 초반 150만 달러에 불과하였던 우리나라의 대라오스 ODA 규모는 2011년 3천 3백만 달러로 20배가량 증가하였다. 우리나라 라오스 국가협력전략(CPS)은 인적자원개발을 중점협력 분야로 지정하고 있다. 본 연구의 목적은 라오스 교육분야 ODA 사업 개선을 위한 정책방안을 제시하고, 향후 라오스 CPS 개선 시 고려해야 할 시사점을 제공하는 것이다. 이를 위해 세계은행, ADB 등 국제기구의 발간 자료와 라오스 교육협력에 관한 연구자료 및 선행연구를 분석하였다. 라오스의 교육분야 개발수요를 파악하고 라오스 교육 및 직업훈련 부서, ODA 담당부서의 정부관계자를 대상으로 현지면담조사를 실시하여 최신 자료와 데이터를 수집하였다. 사회주의 국가인 라오스는 1986년 이후 계획경제에서 시장경제로 탈바꿈하였다. 개혁ㆍ개방정책을 통해 2010년 이후 연평균 7% 이상의 경제성장률을 달성하였고, 2011년 저중소득국(LMIC) 그룹에 진입하였다. 이러한 변화에 추동력을 확보하기 위해 라오스 정부는 2015년까지 1인당 국민소득 1,700달러 달성과 2020년까지 현대화와 산업화, 사회경제 발전을 목표로 제7차 국가사회경제개발계획을 추진하고 있다. 그럼에도 불구하고 2012년 기준 라오스의 평균 교육연한은 4.6년에 불과하여 라오스 경제ㆍ사회의 발전을 저해하는 걸림돌로 작용하고 있다. 라오스의 교육 및 산업ㆍ고용 통계, 라오스 정부의 발전계획 및 교육개혁 그리고 선행연구를 분석한 결과, 초중등교육, 특히 초등교육과 전기중등교육에 관한 개발수요가 가장 많은 것으로 나타났다. 이는 라오스의 기초교육 수준이 대부분의 아시아 국가보다 열악하기 때문이다. 다음으로 고숙련 및 중급 기술자의 비율이 주변 개도국인 캄보디아, 몽골, 파키스탄, 필리핀, 베트남 등 보다 낮으므로 직업교육훈련에 대한 잠재적 수요가 큰 것으로 나타났다. 마지막으로 경제성장과 더불어 증가하고 있는 고등교육 개발수요에 효율적으로 대응하기 위한 전략이 필요한 것으로 보인다. 라오스의 초중등교육이 전체적으로 개선됨에 따라 최근 고등교육에 대한 국가 및 사회적 수요가 현저하게 늘고 있기 때문이다. 이러한 변화는 우리나라의 라오스 교육개발협력 방안을 수립할 때 중요한 요소로서 고려되어야 할 것이다. 2008~12년 라오스의 총 ODA 중 교육 분야에 대한 지원은 13.3%(5천 5백만 달러)를 차지한다. 세계은행과 ADB의 지원이 활발한데, 중점 사업 분야는 기관별로 다소 차이가 있다. 세계은행의 경우 저소득층 및 소수민족 거주 지역의 초등교육 기회의 확대와 질적 제고에 집중하는 반면, ADB는 라오스의 중등교육과 고등교육 및 직업교육훈련에 역점을 두고 있다. 한편 DAC 회원국의 활동을 살펴보면, 이 연구에서 검토한 일본과 독일, 호주는 인적자원개발이 라오스 경제사회 발전의 핵심동인이라는 공통인식하에 교육분야 개발을 중점적으로 지원하지만 중점 사업 영역에는 차이를 가진다. 예를 들어 일본이 기초교육 영역 중, 과학ㆍ수학 교사의 역량 개발과 IT 및 법률과 관련한 인적자원개발에 집중하는 반면에, 독일은 자국의 강점이 있는 직업교육훈련을 통해 숙련 근로자를 육성하는 데에 중점을 둔다. 호주의 경우 라오스 초등교육의 질과 접근성을 개선하고 낙후 지역에 거주하는 여성과 소외계층 및 장애아의 교육기회 확대에 주력하는 특징이 있다. 인접국인 베트남과 비교할 때, 우리나라의 라오스에 대한 지원 규모는 크지 않다. 우리나라는 최근 5년 동안 라오스 교육 분야에 2천 3백만 달러를 지원했으며, 이는 식수 공급 및 위생 분야 다음으로 많은 수준이다. 원조시행기관별로 살펴보면, KOICA는 중등교육(69.2%), 직업교육훈련(25.6%), 아세안 이러닝 사업(4.2%), 초등학교 건립 사업(1%)의 순으로 지원하였다. EDCF의 경우 2004년에 추진된 수파누봉대학 설립 사업(2천 3백만 달러)이 유일한 교육차관 사업이다. 교육부는 정부초청장학생 사업, 글로벌 교육지원 사업, 이러닝 세계화 사업, 아세안사이버 대학 설립 사업 등을 통해서 라오스를 부분적으로 지원하고 있다. 라오스 교육분야 ODA 사업의 문제점은 다음과 같다. 무엇보다 교육분야 중장기 계획이 없이 한-라오스 정상회담 등에 의해 개별 사업이 단발성으로 추진된다는 점이다. 이로써 해당 사업이 종료되면 사업 효과가 단절되는 문제가 관찰되었다. 아울러 다른 개도국에서와 마찬가지로 체계적인 성과관리가 이루어지지 못하고 있다. 라오스 중등학교 교과서 지원 사업, 한-라오스 직업훈련원 건립 사업, 수파누봉대학 설립 사업 등과 같은 대표적인 사업계획에 성과관리 요소가 포함되지 않았고, 따라서 사업 종료 후에 체계적인 성과 측정이 이루어지지 못하는 문제가 발생하였다. 이외에 라오스 주재 해외원조기관과의 협조 및 조정 없이 국내 개발협력기관의 독자적인 사업 추진이 문제점으로 파악되었다. 이는 라오스에만 한정되지 않으므로 우리나라 교육분야 개발협력사업의 효율성 제고 차원에서 해법이 다각도로 탐색되어야 할 것이다.본 연구는 라오스 교육분야 개발협력 방안을 다음과 같이 제시하였다. ‘단계적 원조 확대를 통한 라오스 교육발전 목표 실현 지원’이라는 기본 목표하에 ① 단계적 사업 추진, ② 원조 중복의 최소화, ③ 우리나라 원조 경험의 적극적 활용을 추진전략으로 제시하였다. 개발협력 사업 운영원칙으로는 ① 체계적인 수요조사, ② 증거기반(evidence-based) 사업 추진, ③ 파트너십의 활성화, ④ 체계적 성과관리 원칙을 제안하였다. 중점 사업영역으로는 중등교육, 직업교육훈련, 고등교육을 제시하였다. 중점 사업영역에서 초등교육을 제외한 이유는 라오스의 초등교육 등록률이 이미 95%를 상회하며, 일본과 호주가 이미 질적 개선에 많은 지원을 제공하기 때문이다. 마지막으로 중점 사업영역별로 국내 기관이 수행 가능한 프로그램을 예시적으로 제시하였는데, 선정 기준은 다음과 같다. 첫째, 라오스 정부가 제7차 국가사회경제개발계획에서 강조한 중등교육과 직업교육훈련의 발전을 지원할 수 있는 프로그램을 선정하였다. 둘째, 라오스 정부의 교육부문 개발계획 실현과 관련하여 해외원조기관들의 중복을 최소화할 수 있는 프로그램을 선정하였다. 셋째, 라오스 및 기타 개도국에 대한 우리나라의 경험을 최대한 활용할 수 있는 프로그램을 선정하였다. 넷째, 라오스에 대한 우리나라의 원조 경험이 일천함을 감안하여 단계적으로 확대해나갈 수 있는 교육영역별 프로그램을 선정하였다. 이상의 기준을 토대로 이 연구는 중등교육, 직업교육훈련, 고등교육의 영역별로 총 12개의 프로그램을 제안하였다. English Abstract: The purpose of this study is to promote systematic development of the Laotian education sector for improvement of current education development cooperation projects in Laos (2013-2015) and to provide implications for the establishment of the Country Partnership Strategy (CPS) in 2015. The direction of the education sector development cooperation and major cooperation programs are proposed in this report are based on the Laotian government’s 7th National Social Economic Development Plan 2011-2015, analysis of needs in education development, and actual analysis on assistance from international organizations and major donor countries. To this end, published data from international organizations such as the World Bank and Asian Development Bank (ADB), research data and previous research on education cooperation were analyzed. Also, field research (July 5th-12th , 2014) was performed to identify the needs related to cooperation in education sector development and to collect the latest data in Laos. During the field research, interviews were conducted with government officials from ODA-related departments, in addition to departments in education and vocational training. In the First Chapter, the purpose of this study, its background, and research method are presented. Chapter Two presents the result of the research on the development plan and education development needs in Laos. During 1990-2012, industrial diversification was in progress as the agricultural sector, which was the country’s main industry in the past, decreased in terms of its share of the economy. To ensure the momentum of change and pursue modernization, industralization, social and economic development in Laos by 2010, the Laotian government promoted the 7th National Social Economic Development Plan (2011-2015). In promoting the education development cooperation in Laos, it is necessary to actively support human resource development which is vital in raising the overall level of the various communities. And in order to develop the plan, the result of the research shows that development needs should focus on primary education and early secondary education. After that, potential development needs were identified in vocational training which remains very weak at the moment. Finally, it seems necessary that strategies that effectively responds to needs in higher education development along with economic growth in Laos have to be formulated.The Third Chapter surveyed the current status of cooperation in education development as they relate to the international community in Laos and reviewed the development cooperation cases from major donors (Germany, Japan, Australia). Analysis showed that during 2008-2012, 13.3% (54.66 million US dollars) of total ODA went toward the support of the education sector .Chapter 4 described the performance of Korea’s education sector development cooperation by agency (e.g. KOICA, EDCF, Ministry of Education). The results showed that, in a recent five-year period (2007-2011), Korean government provided 23.12 million US dollars in support of education, which was the second largest next to drinking water supply and sanitation (29.95 million US dollars).Based on results from chapter 2-4, Chapter 5 proposes a plan for cooperation in education sector development in Laos which can be included in the Laos Country Partnership Strategy 2016-2020. The purpose of cooperation in the Lao education sector development is “Supporting Lao education development goals by gradually increasing assistance”, with strategies suggested as follows: 1) step by step project management, 2) minimization of assistance overlap, 3) maximizing the use of Korea’s development experience as a recipient. As for operating principles of education development cooperation projects, such strategies as 1) systematic needs analysis, 2) evidence-based project management, 3) strengthening partnership, 4) systematic performance management principles, were suggested. The major areas of assistance, including secondary education, vocational training, and higher education were identifiedFinally, Chapter Six summarized the research results mentioned in previous chapters and suggested implications which will help improve the current direction and project management with respect to education sector development cooperation of Korea. The implications of proposals made in this study for overall cooperation in education sector development are as follows:First, to improve the efficiency of education development cooperation, harmonization between Korea and other donor institutions that have served recipient countries for a long time should be considered as a critical principle. Second, education development cooperation projects should be pursued in concert with middle and long term Laotian education sector development cooperation. Third, policy measures which would enable development cooperation agencies to follow the education sector plans of the CPS need to be prepared. Fourth, stronger monitoring along with increased evaluation and follow-up training for education development cooperation projects is important. Fifth, to increase the level of quality and efficiency of education development cooperation, training of ‘education ODA advisers’ is needed who can stay in recipient countries and help develop joint projects in consultation with the Ministry of Education officials. Finally, ODA projects in education should favor gradual expansion over rapid quantitative expansion in order to improve the efficiency.

Korean Abstract: 본 연구의 목적은 ｢베트남 협력전략 2011~2015(관계부처 합동 2011)｣ 이후의 교육분야 개발협력방안을 모색할 때 필요한 시사점을 제시하기 위하여 ‘베트남의 교육분야 개발협력방안’을 직업교육훈련에 초점을 맞추어 제안하는 것이다. 교육분야 중에서 ‘직업교육훈련’을 중점 개발분야로 설정한 이유는 무엇보다도 베트남 정부가 추진 중인 ｢2011~2020 사회경제개발전략｣에서 현대화된 산업국가로 도약하기 위한 주요 전략으로 직업교육훈련을 강조하고 있기 때문이다. 아울러 그동안 우리나라에서 추진한 베트남 교육분야 개발협력사업 중 대부분이 직업교육훈련이어서 상대적으로 많은 노하우를 축적하고 있으며, ｢베트남 협력전략 2011~2015｣에서도 직업교육훈련을 중점 협력분야로 설정하였기 때문이다. 이에 본 연구에서는 베트남의 국가발전전략과 연계하여 교육분야 이슈를 전체적으로 조망하되, 직업교육훈련 중심으로 베트남과의 교육분야 개발협력방안을 제시하였다. 이를 위하여 국내외 선행연구를 대상으로 문헌분석을 하였고, 베트남의 교육개발 수요를 파악하고 관련 자료를 수집하기 위하여 베트남 현지에 직접 출장(2013년 7월)을 가서 교육 및 직업교육훈련, ODA 담당부서의 정부 관계자와 면담하였다. 이외에도 전문가협의회를 개최하여 베트남과의 교육분야 개발협력사업의 성과와 문제점 등을 논의하였다.English Abstract: This study aims to present the goal and strategies in plans of the Korean government for educational development and cooperation in Vietnam, focusing on vocational education and training (VET). The study focused on VET because Vietnam has set VET as a main strategic area in terms of becoming a modernized industrial country which was stipulated in its Socio-Economic Development Strategy 2011-2020. Besides, Korea has much experience in cooperation with Vietnam in the field of VET since its education ODA programs in Vietnam are mostly related with VET. In this context, this study has reviewed issues of education development and cooperation in Vietnam and then suggests a range of policy options to improve education ODA programs.
