Categories of nutraceuticals are dietary supplements, functional food, medicinal food, pharmaceuticals. Advance age is often characterized by a declined in large spectrum of cognitive abilities including reasoning, memory, perceptual speed and language. Alzheimer’s disease (AD) is the most common and feared from dementia representing circa 70% of all dementia cases and displaying a dramatic epidemics due to the enormous growth of the aged population worldwide. It is still unclear which factors lead to molecular cascade of neurode generation in AD, but along with genetic environmental factors vascular pathology and risk factor have been recently shown to play crucial role in AD pathogenesis .AD impacts dramatically on everyday life of older adults, being one of the main cause of disability in old age. There is clear evidence that a diet rich in specific nutritional food group (fruit, fish, vegetables) can reduce the incidence and prevalence of some of the main clinical outcomes, such as neurodegenerative disorders, cardiovascular diseases, diabetes, cancer. This specific nutritional food group are rich in micronutrients and vitamins are beneficial for health. The Mediterranean diet is characterized by a high consumption of plant food, fish olive oil as primary source of monounsaturated fat and moderate intake of wine. In this article, we focus our attention on group of substance proposed to prevent or treat Alzheimer’s disease.

The current research work focuses on the conventional bio-catalysed synthesis of amyl levulinate with immobilized Candida antarctica lipase B in the absence of a toxic solvent. The green catalyst and solvent free system offered considerable benefits like mild and gentle condition requirement, lesser-energy demand, easy down-streaming and reusability of the biocatalyst. The process parameters were examined, and the maximum conversion (73.2%) was obtained under the optimum condition of 50℃ temperature, 1:3 molar ratio of levulinic acid to amyl alcohol using 0.8% (w/v) biocatalyst at 250 rpm speed. The immobilized lipase was characterized by SEM, BET, XRD, FTIR and amyl levulinate was characterized by FTIR and GC-MS analysis. The enzyme lipase was effectively reused till 5 cycles for the formation of ester. The thermodynamic parameters (ΔH, ΔG, and ΔS) and energy of activation (Ea) were determined for the assessment of energy requirement. The activation energy was determined to be 48.42 kJ/mol. The research protocol strikes the development of biocatalytic synthetic route of amyl levulinate without using any toxic solvent, which may contribute to green chemistry approach.

The present study evaluates the potential use of ultrasound irradiation to synthesize decyl oleate using Fermase CALBTM10000 under the solvent-free system (SFS). The optimal condition to achieve a maximum yield of 97.14% was found to be 1:2 oleic acid:decanol ratio, 1.8% (w/w) enzyme loading, 45°C temperature, 200 rpm agitation speed, 50 W power input, 50% duty cycle, 22 kHz frequency and reaction time of 25 minutes. The thermodynamic study was done to determine the change in entropy, Gibb's free energy, and change in enthalpy at various temperatures. The experimental results and kinetic study showed that the reaction followed ordered bi-bi model with kinetic parameters as rate of reaction (V max ) = 35.02 M/min/g catalyst, Michaelis constant for acid (K A ) = 34.47 M, Michaelis constant for alcohol (K B ) = 3.31 M, Inhibition constant (Ki) = 4542.4 M and sum of square error (SSE) = 0.000334. The application of ultrasound irradiation combined with biocatalyst and the absence of solvent intensified the process compared to the traditional stirring method using hexane as solvent.

Objectives. To adapt and extend an existing typology of vaccine misinformation to classify the major topics of discussion across the total vaccine discourse on Twitter. Methods. Using 1.8 million vaccine-relevant tweets compiled from 2014 to 2017, we adapted an existing typology to Twitter data, first in a manual content analysis and then using latent Dirichlet allocation (LDA) topic modeling to extract 100 topics from the data set. Results. Manual annotation identified 22% of the data set as antivaccine, of which safety concerns and conspiracies were the most common themes. Seventeen percent of content was identified as provaccine, with roughly equal proportions of vaccine promotion, criticizing antivaccine beliefs, and vaccine safety and effectiveness. Of the 100 LDA topics, 48 contained provaccine sentiment and 28 contained antivaccine sentiment, with 9 containing both. Conclusions. Our updated typology successfully combines manual annotation with machine-learning methods to estimate the distribution of vaccine arguments, with greater detail on the most distinctive topics of discussion. With this information, communication efforts can be developed to better promote vaccines and avoid amplifying antivaccine rhetoric on Twitter.

Mosses have been known for millennia and highly esteemed all over the world as the rich source of bioactive compounds. The research targets on evolution of microbicidal potentialities of Rhodobryum roseum (extract) used against selected fungus (X. oryzae pv oryzae, S. enteric, P. multocida and M. plutonius) and bacteria (R. solani, S. rolfsii, F. oxysporum and T. indica) to assay antimicrobial activity. Impact of aqueous and undertaken organic viz., ethanol, acetone, choloform, petroleum ether, methanol extract of R. roseum, at varied concentrations and at different time intervals were examined against the growth of bacteria and fungus. All the aqueous extracts were proved to be infective against all the tested pathogens. The antimicrobial potential of six extracts was screened against undertaken bacteria and fungi using micro broth dilution assay. Out of the six (diverse organic and aqueous) extract of R. roseum in ethanol and acetone showed maximum inhibitory activity in S. rolfsii with the MIC value of 5.00 (µg/ml), along with MFC value of 6.25 (µg/ml) in acetone extract and the value of MBC was recorded utmost in X. oryzae with value 3.00 (µg/ml) extracted in ethanol. Over all, the organic extract of R. roseum has potent antimicrobial activity and could be possible source of lead molecules considered for the future development of microbicidal agent.

Nano-sized materials have been an important tool in basic and applied sciences. A novel, low cost, green and reproducible bacteria, Staphylococcus aureus mediated biosynthesis of titanium dioxide nanoparticles (TiO2 NPs) was reported in the present study. Initial conformational studies were done using UV-visible spectroscopy and confirmed the synthesis of TiO2 NPs in the broth. The detailed characterization of the TiO2NPs was carried out using SEM, XRD, FTIR and Raman spectroscopy. From the SEM, it was confirmed that the sample showed the NPs were smooth and spherical with an average diameter of about 20nm. From FTIR analysis, it was confirmed that the TiO2 nanoparticles are crystalline in nature, which was confirmed by the FTIR peak at 518cm-1 corresponds to the TiO2 vibration present in a crystalline structure. Additionally, the synthesized NPs were also characterized by Transmission Electron Microscopy (TEM) and Particle size analyzer. This study was aimed to determine the antibacterial and antibiofilm activity of Titanium oxide nanoparticles against both Gram-positive and Gram-negative bacterial species and significant positive results against Bacillus subtilis and Escherichia coli were observed.

Background: Studies concerning pediatric lichen sclerosus are limited, and, to date, there have been no studies comparing the course of lichen sclerosus in boys and girls. We sought to examine all publications on boys and girls with lichen sclerosus and assess and compare epidemiology, symptoms and signs, genetic background, risk factors, treatment, and prognosis. Methods: A systematic search was performed in the Embase, Medline, Cochrane, and Web of Science databases. Inclusion criteria were information on children ages 0–18 years and a clinical or histologic diagnosis of lichen sclerosus. Literature from 1985 to 2021 was reviewed. Results: A total of 1780 articles were retrieved from the search, of which 90 articles were eligible for inclusion. Boys and girls present similarly on many aspects; nonetheless, treatment and follow-up are approached differently. Conclusions: Though the clinical approach is often different, lichen sclerosus in boys and girls demonstrates many similarities. More research is needed, especially on follow-up, to gain a better understanding of the course of lichen sclerosus and establish an advanced management plan for children.

The conventional flavor synthesis method suffers from low yields, time inefficiency, and extreme reaction conditions. Therefore, there is a necessity for the green and novel synthesis approach to overcome these limitations. The current review presents a holistic insight into different aspects associated with the synthesis of flavor esters using the immobilized enzyme. The application of process intensification tools such as ultrasound and microwave irradiation can enhance the reaction efficiency because of higher product recovery, less formation of by-products, and decreased energy consumption. This review presents the process intensification of value-added flavor esters synthesis and the mechanism of ultrasound and microwave action on the enzyme to enhance the enzyme activity and increase the reaction rate. It also summarizes the role of process intensification in enzymatic flavor ester synthesis, followed by specific examples as reported in the literature.

Oyster mushroom( Pleurotus ostreatus) is belong to the group of healthy foods, as they contain high levels of proteins, vitamins and different classes of compounds, it is discovered that oyster mushrooms could play a key role in maintaining good health. Oyster mushroom (Pleurotus sp.) Class Basidiomycetes and Family Agaricaceae are widely known as ‘dhingri’ in India. Pleurotus Ostreatus have several medicinal properties including ; anti-arthritic , antitumor, immune modulatory , antioxidant, anticancer, anti-inflammatory, antigenotoxic, hypo-cholesterolaemic, antihyperglycaemic antihypertensive, antiplatelet aggregating, antiviral and antimicrobial activities.. In this paper studied that effects of chemical constituents of oyster mushroom(Pleurotus sp.) on DNA damaging protein which analyzed its activity of PARP inhibiting or vice – versa.For this analysis we choose the molecular docking technique to check the effects of different chemical constituents of oyster mushroom(Pleurotus sp.) on DNA damaging protein and compare their results to PARP inhibitory drugs which taken as standard . We perform the molecular docking in between chemical constituents of oyster mushroom(Pleurotus sp.) and 4UND protein compare to performance of molecular docking in between standard PARP inhibitory drugs and 4UND protein with the help of PyRx and BIOVIA Discovery studio software.The analysis of molecular docking shows that some chemical constituents of oyster mushroom(Pleurotus sp.) having more binding affinity than standard PARP inhibitory drugs .The Rutin shows better binding affinity than PARP inhibitory drugs on the same protein.

Fifteen different genotypes of greater yam (Dioscorea alata) NGY-1, NGY-2, NGY-3, NGY-4, NGY-5, NGY-6, NGY-7, NGY-8, NGY-9, NGY-10, NGY-11, NGY-12, NGY-13, NGY-14 and Da-199 were procured from different places of south Gujarat, like Valsad, Navsari, Surat and Anand. Among the biochemical parameters total carbohydrate ranged between 51.87 and 87.85% from different genotypes, starch ranged from 47 to 80.67%, crude fat ranged from 0.6 to 2.32%, crude fibre ranged between 1.10 and 4.09%, anthocyanin content of genotypes ranged from 1.01 to 3.25 mg/g, beta-carotene content ranged between 0.97 and 1.88 µg/g. The antinutrients namely Diosgenin and Tannin ranged from 0.28 to 0.93% and 0.923 mg/100 g to 2.447 mg/100 g, respectively. RAPD analysis was also done by the help of 18 RAPD markers: OPA1, OPA2, OPA3, OPA13, OPB1, OPB6, OPB7, OPM1, OPM2, OPM4, OPM7, OPM11, OPM12, OPM13, OPM15, OPM16, OPM17 and OPM19 from which an average of 9.7 loci were detected with an average of 4.72 polymorphic loci which is 48.65% polymorphism per loci and 48.70% average polymorphism. From the overall study, it can be conferred that the study revealed highest carbohydrate content in NGY-3 (87.85%), fat (2.32%) and crude fiber content (4.09%) in NGY-11, β-carotene in NGY-7 (1.88 µg/g), anthocyanin in NGY-4(3.25 mg/g). Lowest tannin (0.923 mg/100 g) and diosgenin (0.28%) was found in NGY-6, and NGY-7 respectively. For each of the biochemical parameters, the varieties with the optimum values may be cultivated. As the molecular studies revealed NGY-2 and NGY-3 have 96% similarities, they may be the duplicate of the same genotypes which can be studied further for better germplasm conservation.

Objective: To review phase 3 trials of sodium-glucose cotransporter (SGLT) inhibitors in type 1 diabetes mellitus (T1DM) patients. Data Sources: A literature search of Ovid MEDLINE databases (1946 through May 17, 2019) limited to English-language human clinical trials was conducted using the following terms: sodium-glucose transporter 2 inhibitors, canagliflozin, dapagliflozin, empagliflozin, sotagliflozin, ertugliflozin, ipragliflozin, or remogliflozin combined with type 1 diabetes mellitus. Results were verified via Google Scholar and clinicaltrials.gov. Study Selection and Data Extraction: Articles were included if they were phase 3 trials in adults with T1DM. Data Synthesis: Phase 3 trials are available for dapagliflozin, empagliflozin, and sotagliflozin. All 3 drugs demonstrated statistically significant reductions in hemoglobin A1C, weight, and total daily insulin dose without an increased risk of hypoglycemia in up to 52 weeks of therapy. The incidence of diabetic ketoacidosis (DKA) was higher in patients on a SGLT inhibitor at all doses, with the exception of empagliflozin 2.5 mg (0.8% vs 1.2% with placebo). Relevance to Patient Care and Clinical Practice: SGLT inhibitors are potential adjuncts to insulin in T1DM patients, providing clinically meaningful benefits. Regulatory bodies have either approved or are reviewing these agents for use in T1DM. Clinicians should be familiar with the DKA risk associated with SGLT inhibitors and utilize DKA risk-mitigation strategies. Empagliflozin 2.5 mg warrants additional investigation given its efficacy without an increased incidence of DKA. Conclusions: Phase 3 trial data of SGLT inhibitors provide evidence for sustained efficacy in T1DM patients. Appropriate patient selection for therapy and routine monitoring are essential to minimize associated risks.

Since ancient times, people explored the nature, chiefly plants in search of new drugs. This has resulted in the use of great number of medicinal plants with curative properties to treat a variety of diseases and hold medicinal significant bioactive compounds. Along with higher plants lower plants also posses this quality and bryophytes are just one of them as they are relatively free from microbial attacks which point toward their antimicrobial ability. Unfortunately, very little is known about the chemistry of bryophytes and information concerning research results is very scattered. Hence, preliminary phytochemical screening of different extracts (petroleum ether, acetone, methanol, chloroform, ethanol) of Plagiochasma appendiculatum and Dicranum scoparium was carried out along with total phenolic (GAE/ gm) and total flavonoid (mg QE/ gm) content of different extract of two tropical bryophytes, i.e. P. appendiculatum and D. scoparium using standard procedures, to identify the phytochemical constituents. The extracts were screened for the presence and quantities of total flavonoids and total phenols with a view to assess their therapeutic values in ethnomedicine. The results of the phytochemical screening revealed the presence of total flavonoids and total phenols in varying quantities in the two bryophytes. These results suggest that the undertaken bryophytes can be veritable and potential source of useful drugs in treatment of ailments.

Due to rapid expansion of urbanization, unhealthy diet habits and sedentary lifestyle, the incidence of DM is increasing. The chronic nature of DM causes significant personal suffering and economic difficulty to families. The was aimed at investigating the cost difference in various brands of the same oral anti-diabetic drug.Methods: The minimum and the maximum cost in rupees (INR) of a particular anti-diabetic drug manufactured by various pharmaceutical companies were obtained from current index of medical specialties (CIMS) website, Indian drug review (IDR) 2021 issue and National pharmaceutical pricing authority-pharma sahi daam. The cost ratio and percentage cost variation were noted for each brand.Results: Amongst single drug therapy, metformin 500 mg sustained release showed highest price variation (3668%). Minimum cost variation was found with glipizide 2.5 mg (65%). Amongst the fixed dose combinations, highest cost variation was seen with glimepiride 2 mg+metformin 1000 mg (2703%) while minimum cost variation was found with repaglinide 1 mg+voglibose 0.3 mg (29%).Conclusions: A noticeable cost variation was found in different brands of the same anti-diabetic drug. Prescribing a more expensive brand when a cheaper one is available can burden the patient financially and thus reduce patient compliance. Background Pharmacokinetic data show reduced mycophenolic acid levels in renal transplant recipients taking mycophenolate mofetil (MMF) and proton pump inhibitors (PPIs) concomitantly. This reduced exposure could increase rejection risk. The typical initial MMF dose post renal transplantation is 2 g/day, which often requires dose reduction secondary to side effects. Existing studies have not shown significant acute rejection differences for patients taking MMF-PPI versus patients taking MMF-ranitidine. Objective The purpose of this study was to evaluate clinical outcomes in renal transplant recipients receiving a lower MMF dose than previously studied (1.5 g/day) and either a PPI or histamine-2 receptor antagonist (H2RA). Methods This retrospective cohort study included adult subjects receiving a renal transplant between January 1, 2009, and June 30, 2013. Comparison groups were defined based on acid-suppressing therapy class prescribed at discharge from transplantation. The primary outcome was acute rejection incidence within 1 year posttransplantation. Results Of 728 renal transplant recipients screened, 522 were included: 183 taking a PPI and 339 taking an H2RA. There was no significant difference in acute rejection within 1 year (H2RA 19% versus PPI 14%, p=0.12) or 3 months (4% vs 5%, p=0.44, respectively) posttransplantation. Maintenance immunosuppression (MMF dose and tacrolimus troughs) was similar between groups at 3 months and 1 year. Graft and patient survivals were favorable (> 95%), and graft function at 1 year was stable and similar between groups. Conclusion Despite taking lower MMF doses than previously studied, subjects on a PPI compared to an H2RA were not at increased risk of acute rejection within 1 year posttransplantation.

Background The administration of intravenous immunoglobulin (IVIG) has demonstrated promise in the treatment of medically refractory inflammatory bowel diseases (IBD). We aimed to identify factors associated with IVIG failures in the treatment of refractory IBD. Methods Our historical cohort included patients with refractory IBD admitted to our inpatient service with an exacerbation and treated with at least 1 dose of IVIG (0.4 g/kg). Detailed clinical variables were recorded for subjects. Examined outcomes included changes in disease-specific severity indices, the duration of surgery-free survival after IVIG, infusion reactions, subsequent IBD-related emergency department visits, hospital readmissions, and mortality. Results Fifty-four subjects with refractory IBD (61% female, age 42 ± 16 yrs, 23 with Crohn's disease, 15 with ulcerative colitis, 16 with pouchitis) met the inclusion criteria. All disease severity scores were significantly improved after IVIG administration (Harvey-Bradshaw index P = 0.007, partial Mayo score P = 0.002, modified Pouchitis Disease Activity Index P = 0.008). Twenty-seven patients (50%) underwent surgery, with a mean surgery-free survival of 28.7 ± 3.7 months. In univariable analysis, patients with Clostridium difficile infection (CDI) had a 3-fold increased risk of bowel resection surgery after IVIG compared with those without (hazard ratio = 2.9, 95% confidence interval, 1.2-7.4; P = 0.023), and in subsequent multivariable analysis, CDI remained significant (hazard ratio = 3.0, 95% confidence interval, 1.2-7.6; P = 0.024). CDI was also associated with increased risk of hospital readmission (hazard ratio = 2.5, 95% confidence interval, 1.05-5.9; P = 0.038). Conclusions Our study demonstrates that IVIG is beneficial in patients with medically refractory IBD, and that concomitant CDI is a risk factor for the treatment failure of IVIG for refractory disease.

The current COVID-19 pandemic has affected our daily lives and routine to a large extent. Be it our over-burdened health related services or fear of economic loss on a larger scale, this pandemic has affected everyone in some way or the other. No doubt that those getting infected with this virus are being affected the most but still there is a huge population which is not infected with this virus. This group of population is suffering due to its pre-existing illness/disease which is not being addressed currently because of various unavoidable measures like lockdown taken up by many governments across the world. This stratum of population may be hidden as of now but it will eventually pop-out when the current pandemic gets over or is slowed down and this may lead to another health crisis post COVID-19 pandemic.&nbsp

A major driving force in the application and advancement of a biocatalytic process in the industries is the environmental concern and their related regulations. There is an ever-increasing demand for cleaner, novel technologies that cause less ecological problems for sustainable development. Enzymatic processes are the most effective as they operate under mild reaction condition with excellent selectivities and highly attractive to researchers in the area of green chemistry. Biocatalysis has been applied for organic synthesis to make the production pathways greener and sustainable. Considering the advantages of green chemistry, biocatalysis has significantly substituted conventional chemical approaches in numerous fields, and this replacement may strike even more fields due to recently developing innovations in enzyme engineering. The chapter addresses the employment of enzymes for organic synthesis. Fundamental groups of enzymes comprise of oxidoreductases, transferase, hydrolases, lyases, isomerases, and ligase. The difficulties and possibilities in expanding biocatalytic pathway are also presented.

If the polythene bags burned in agriculture land, they release harmful chemicals like Cd, Pb, CO, NOX and SO2, which causes many diseases in animals as well as in human beings. The accumulation of polythene bags also makes agricultural soil sterile/barren as it stops the gaseous exchange. In normal agricultural land, the concentration of oxygen is 3 ppm. Where polythene is present, the concentration of oxygen in soil decreases to 0.001 ppm. In addition, the polythene pollution can adversely affect agricultural lands, waterways, and oceans.

The enzymatic synthesis of cetyl caprate, a cosmetic grade ester was studied by esterification of cetyl alcohol and capric acid using Fermase CALB™10000 under solvent free condition. The process parameters namely, temperature, enzyme loading, molar ratio, agitation speed have been investigated and optimized to attain maximum yield of cetyl caprate. The maximum yield of 91.9% was obtained in 80 min under optimized conditions of 50 °C temperature, 3% enzyme loading, capric acid:cetyl alcohol ratio of 1:3, and 200 rpm stirring speed. The enzyme operational stability study reveals that enzyme can retain its original activity for the synthesis of cetyl caprate and could be reused up to five to six times without any notable loss of catalytic activity. The kinetics of the reaction was derived by employing non-linear regression consideration, and the experimental results and kinetic evaluation showed that reaction obeys ping-pong bi–bi model. The kinetic parameters, Vmax = 21.886 M/min/g catalyst, KA = 1.194 × 10−2 M, KB = 8.4512 M, KiA = 2.02 × 10−2 M, KiB = 3.55 × 10−4 M and SSE = 4.31 × 10−5 measured from initial rate data were used to replicate experimental results with good agreement. The thermodynamic parameters, i.e., change in enthalpy, Gibb’s free energy, change in entropy was evaluated at different operating temperatures. Microwave-assisted synthesis of ethyl laurate using lauric acid and ethanol catalyzed by Fermase CALB has been investigated. The effect of operating parameters like molar ratio (lauric acid: ethanol), enzyme loading, temperature, and molecular sieves was systematically studied. A maximum conversion of 98.2% was obtained in 10 ​min compared to the conventional method, where the reaction required 4 ​h to achieve 92.4% conversion. The optimum parameters for the microwave-assisted synthesis were 1:2 ​M ratio of lauric acid to ethanol, 45 ​°C temperature, 1.8% (w/w) enzyme amount, 1.5% (w/w) molecular sieves. The enzyme lipase was reused for up to seven successive cycles under microwave irradiation. The thermodynamic study was carried out to determine various thermodynamic parameters for the reaction. The esterification mechanism was proposed, and the impact of microwave irradiation on the immobilized enzyme after several reuse was studied by scanning electron microscopy.

Rasburicase is a recombinant urate oxidase enzyme administered for treatment of hyperuricemia associated with tumor lysis syndrome. Studies demonstrate effectiveness of single fixed-dose rasburicase as compared to the FDA-approved dose of 0.2 mg/kg intravenously daily for up to five days. Doses in these studies range from 1.5 mg to 7.5 mg. Our study evaluated outcomes in patients who received single 4.5 mg fixed-dose rasburicase. This retrospective, IRB-approved chart review evaluated adult oncology subjects who received fixed-dose rasburicase between January 2007 and April 2014. The primary outcome was percentage of patients with normalization of uric acid (level 2) versus non-overweight patients. Initial failure was defined as need for additional doses or progression to dialysis within one week of the initial fixed-dose. In the 128 patients included, the mean baseline uric acid level was 14.84 mg/dL. Of the 112 patients with a follow-up uric acid level, 68% achieved normalization within 24 h of rasburicase administration. Thirty-eight patients received additional treatment: 10 received additional dose(s) and 28 underwent dialysis. Normalization of uric acid in overweight versus non-overweight patients was 66% and 73%, respectively. Overall, a single 4.5 mg fixed-dose of rasburicase effectively normalized uric acid in 68% of patients within 24 h. Further studies are needed to determine the optimal single fixed-dose necessary for treatment response across all patients.

Communication parameters used in calculating trust value in most of existing trust based protocol (wireless network) are number of successful session or packet forwarded between two nodes, number of packet dropped or delayed, response time, battery life, mobility of node etc. This paper provides analysis of existing trust based routing by surveying current “sate of the art” work in this area. This paper also proposed a new parameter (number of route error message sent by a node) of a wireless node which can affect the network performance and can be used to calculate trust value.

Role of male circumcision (MC) as a tool to prevent sexually transmitted infections (STIs)/human immunodeficiency virus (HIV) was assessed. MC is reported to be beneficial in conditions such as traumatic injury, Balanitis Xerotica Obliterans, refractory balanoposthitis, and chronic, recurrent urinary tract infections. MC also reduces the chances of penile carcinoma by facilitating improved penile hygiene, lowering HPV/HIV transmission rates, and reducing chronic inflammatory conditions such as phimosis and balanitis. MC has been recommended by the WHO and UNAIDS in 2007 as an additional HIV prevention intervention in settings of high HIV prevalence. The antinutrients namely Diosgenin and Tannin ranged from 0.28 to 0.93% and 0.923 mg/100 g to 2.447 mg/100 g, respectively. RAPD analysis was also done by the help of 18 RAPD markers: OPA1, OPA2, OPA3, OPA13, OPB1, OPB6, OPB7, OPM1, OPM2, OPM4, OPM7, OPM11, OPM12, OPM13, OPM15, OPM16, OPM17 and OPM19 from which an average of 9.7 loci were detected with an average of 4.72 polymorphic loci which is 48.65% polymorphism per loci and 48.70% average polymorphism. The pre-processing technique used is Daubechies wavelet transform before calculating the statistical features. The statistical features calculated are variance, kurtosis, energy and entropy. Along with these four features, the three features are calculated by using Fast Independent Component Analysis (fast ICA). These features are averaged and plot in order to categorize between meditational and non-meditational group.

The present work highlights the effect of ultrasound on enzymatic synthesis of isobutyl propionate, a rum flavor by esterification of isobutanol and propionic acid in non-aqueous, solvent free system (SFS) using Fermase CALB™10000. The optimization study for different variables in presence of ultrasound showed a maximum conversion of 95.14% at 60 °C temperature, 4% w/w enzyme dose, 1:3 acid:alcohol ratio, 40 W power, 25 kHz frequency, 50% duty cycle and 150 rpm speed in 3 h as compared to 10 h of conventional method. The optimal enzyme loading was reduced to 4% w/w using ultrasound irradiation compared to 5% w/w of conventional. The efficiency of enzyme improved notably and can be reused up to seven cycles preserving its former activity. The application of ultrasound greatly enhanced esterification reactions by maintaining enzyme stability and improving the production yield. Background Solid organ transplant recipients are especially vulnerable to Clostridium difficile infection (CDI) due to cumulative risk factors including increased exposure to healthcare settings, persistent immunosuppression, and higher rates of antimicrobial exposure. We aimed to identify risk factors associated with CDI development in kidney transplant recipients including implications of immunosuppressive therapies and acid-suppressing agents. Methods This was a single-center, non-interventional, retrospective case-control study of adult subjects between June 1, 2009 and June 30, 2013. During this time, 728 patients underwent kidney transplantation. Overall, 22 developed CDI (cases) and were matched 1:3 with 66 controls. Cases and controls were also matched for induction agent, kidney allograft type (living or deceased), and time from transplant to CDI result (±60 days). Results The majority of subjects received a deceased donor kidney (77.3%) and basiliximab induction therapy (86.4%). The overall CDI incidence was 3%. Factors independently associated with CDI were average tacrolimus trough (AOR = 1.25, 95% CI = 1.00-1.56, P = .048) and antibiotic exposure for urinary tract infections (UTI) (AOR = 4.17, 95% CI = 1.12-15.54, P = .034). Proton pump inhibitor use was not associated with CDI (OR = 0.81, 95% CI = 0.29-2.29, P = .691). Conclusion Maintaining a clinically appropriate tacrolimus trough and judicious antibiotic use and selection for UTI treatment could potentially reduce CDI in the kidney transplant population.

Before sending data from source to destination node, the source node searches for route from source to destination. As no special routers available in such network each node acts as a router and cooperates in routing. Special routing protocols are available for MANET like DSDV, DSR, AODV, etc. Due to attacker nodes and selfish nodes these protocols are vulnerable to many types of malicious activities like packet drop, packet delay, packet modification, etc. In this paper, we have implemented a packet drop attack and study its effect on route discovery time of AODV and throughput. We modified AODV routing to detect such nodes and shows improvement in studied parameters.

Mobile ad hoc network is a network of wireless node connected using wireless media. Each node operates as a router for all other nodes in the network. Nodes in MANET are mobile and hence change their topology dynamically. From one source to destination node there may be multiple paths. Due to wireless link mobile ad hoc networks are susceptible to many attacks. One popular attack on mobile ad hoc network is packet drop attack (grey hole, black hole). The basic routing protocols used for mobile ad hoc network will handle the issues arise due to mobility and dynamic changing topology of network but do not have any mechanism to detect malicious activities of any node. The other possibility of packet drop is due to limited queue size. In this paper I find out ideal queue size for AODV routing in Mobile ad hoc network so that less chances of packet drop due to limited queue size. I set up an experiment in OPNET and implemented a WLAN with five wireless nodes and generated large amount of TCP traffic. I have created multiple scenarios with different queue size (1,3,5,8, Infinite) set in AODV protocol for each node and compare the throughput of the network and found the ideal queue size for AODV routing.

Methods: Ninety-four adult patients undergoing hysteroscopic procedures were divided into sevoflurane (S) group or desflurane (D) group. Patients were premedicated with midazolam 0.03 mg/kg and fentanyl 1 μ g/kg. Anaesthesia was induced with propofol 2.0–2.5 mg/kg, followed by insertion of a ProSeal™ LMA. Adverse airway responses such as cough, hiccups, laryngospasm and breathholding were recorded. In the post-operative period: time to awakening, response to verbal commands, orientation, ability to sit with support and the recovery room Aldrete score were recorded. Results: Three patients in group S (6.4%) and six patients (13.3%) in Group D had adverse airway events. The mean time to eye opening (Group S-10.75 ± 7.54 min, Group D-4.94 ± 1.74 min), obeying verbal commands (Group S-13.13 ± 8.75 min, Group D-6.55 ± 1.75 min), orientation (Group S-15.42 ± 8.46 min, Group D-6.23 ± 2.4 min) and to sit with support (Group S-36.09 ± 12.68 min, Group D-14.35 ± 3.75 min) were found to be lesser with desflurane than with sevoflurane (P < 0.001). The mean time to recovery was delayed in Group S-46.00 ± 12.86 min compared to Group D-26.44 ± 5.33 min (P < 0.001). Conclusion: Desflurane has faster awakening properties than sevoflurane without an increase in adverse airway events when used during spontaneous ventilation through a ProSeal™ LMA along with propofol and fentanyl.

The development of antiretroviral therapy (ART) has been one of the most dramatic progressions in the history of medicine. Aim: The aim was to study the mitochondrial toxicities in terms of peripheral neuropathy (PN), lipodystrophy (LD), hepatic steatosis, lactic academia (LA), and pancreatitis developing in AIDS cases on nucleoside analog reverse transcriptase inhibitors (NRTIs) based ART regimens. Materials and Methods: An observational study, which included 90 AIDS cases, receiving first line ART regimens containing two NRTIs (zidovudine [AZT]/stavudine [d4T] with lamivudine [3TC]) and one nonNRTIs (nevirapine/efavirenz) was conducted at Skin-VD outpatient department of a tertiary care hospital attached to a Medical College. Thorough history was taken, and clinical examination was done. Cases were subjected to measurements of abdominal girth and mid-arm circumference, liver function tests, blood sugar, lipid profile, serum lactate, and amylase levels. Results: Of 90 cases on ART, 66% were males and 34% were females. Mitochondrial toxicities developed in 26 (30%) cases out of 90, which included 3 (7%) out of 42 cases on AZT + 3TC and 23 (48%) out of 48 cases on d4T + 3TC. Most common toxicity was PN seen in 20 (22%) cases; male cases developed PN at a lower CD4 count than female cases. LD was observed in total of 13 (14.5%) cases; deposition of fat in the abdomen in seven cases and at the nape of the neck (buffalo hump) in one case while loss of fat from extremities was seen in seven cases and loss of buccal fat in seven cases. Women presented more with fat accumulation (breast and abdomen), while men with loss of fat (limbs and buttocks). Both PN and LD were more common in d4T based regimen. LA was reported in one case on d4T. Hepatic steatosis was seen in three cases and pancreatitis in one case receiving AZT. Conclusion: Regular monitoring and early diagnosis of mitochondrial toxicities with timely switch to safer alternatives is of utmost importance.
