Background Intimate partner violence (IPV) against women is pervasive throughout the world, with profound consequences for women’s health. Research to understand the extent, causes and consequences of IPV relies on self-reported data on violence, and yet there is a paucity of research into the consistency with which women report lifetime IPV over time. Methods We use data from the control group of the cluster randomised trial and a follow-on longitudinal study in Tanzania to examine discrepancies in women’s reported experience of lifetime physical IPV and sexual IPV over three time-points (T0, T29, T53 months). Among those reporting lifetime history of IPV at T0, we calculate the proportion who subsequently report no lifetime history at T29 and/or T53 (‘discrepant’ reporting). We use logistic regression to explore associations between discrepant reporting and respondent baseline characteristics, the nature of their IPV experiences at baseline, and situational factors at T53. Results Complete IPV data were available for 301 women. At T0, 154 (51%) women reported lifetime history of physical IPV, of whom 62% gave a discrepant ‘never’ report in a subsequent round. Among 93 (31%) with lifetime history of sexual IPV at T0, 73% provided a subsequent discrepant report. 73% of women reported lifetime physical IPV, and 55% lifetime sexual IPV in at least one survey round. For both IPV outcomes, women were less likely to provide discrepant reports if they had recent IPV at baseline, poor mental health (T53) and poor communication with partner (T53). For physical IPV only, reduced discrepant reporting was also associated with baseline household-level financial hardship and more severe or extensive experience of IPV. Conclusions A large proportion of women provided discrepant reports over the course of the study. Prevalence estimates of lifetime IPV from one-off cross-sectional surveys are likely to be underestimates, biased towards more recent and severe cases. To improve the stability of IPV measures, researchers should explicitly clarify the meaning of reference periods such as ‘ever’, consider using shorter reference periods (e.g. past-year), and avoid filter questions that use positive reports of lifetime IPV as a gateway to asking about more recent experiences. Trial registration: Maisha CRT01 registered at ClinicalTrials.gov #NCT02592252, registered retrospectively (13/08/2015).

PURPOSE: Acquisition of human papillomavirus (HPV) in women occurs predominantly through vaginal sex. However, HPV has been detected in girls reporting no previous sex. We aimed to determine incidence and risk factors for HPV acquisition in girls who report no previous sex in Tanzania, a country with high HPV prevalence and cervical cancer incidence. METHODS: We followed 503 adolescent girls aged 15-16 years in Mwanza, Tanzania, with face-to-face interviews and self-administered vaginal swabs every 3 months for 18 months; 397 girls reported no sex before enrollment or during follow-up; of whom, 120 were randomly selected. Samples from enrollment, 6-, 12-, and 18-month visits were tested for 37 HPV genotypes. Incidence, clearance, point prevalence, and duration of any HPV and genotype-specific infections were calculated and associated factors were evaluated. RESULTS: Of 120 girls who reported no previous sex, 119 were included, contributing 438 samples. HPV was detected in 51 (11.6%) samples. The overall incidence of new HPV infections was 29.4/100 person-years (95% confidence interval: 15.9-54.2). The point prevalence of vaccine types HPV-6,-11,-16, and -18 was .9%, .9%, 2.0%, and 0%, respectively. Spending a night away from home and using the Internet were associated with incident HPV, and reporting having seen a pornographic movie was inversely associated with HPV incidence. CONCLUSIONS: Incident HPV infections were detected frequently in adolescent girls who reported no previous sex over 18 months. This is likely to reflect under-reporting of sex. A low-point prevalence of HPV genotypes in licensed vaccines was seen, indicating that vaccination of these girls might still be effective.

Immune activation, characterized by polyclonal B-cell activation, accelerated T-cell turnover, dendritic cell depletion, and elevated proinflammatory cytokine levels, is a hallmark of human immunodeficiency virus type 1 (HIV-1) infection [1]. Immune stimulation by HIV-1 contributes to the depletion of CD4+ T cells, rather than to the purging of virus, and has been associated with accelerated progression of HIV-1 disease [2–5]. The dysregulation of cytokines, important markers of immune activation, has been shown to contribute to viral replication and disease progression [6, 7]. While the relationship between HIV-1 pathogenesis and persistent immune activation has been well described [1, 2], the role of systemic inflammatory response and immune activation in HIV-1 transmission is less understood. Increased immune activation in persons infected with HIV-1 could result in increased viral replication that could facilitate HIV-1 infectiousness and onward transmission [8, 9], but no data have directly explored this hypothesis. Furthermore, for HIV-1–uninfected persons, immune activation and an elevated inflammatory response before infection has also been hypothesized to potentially heighten susceptibility and facilitate HIV-1 acquisition; however, few studies have directly compared systemic immune activation between those who acquire HIV-1 and those who are exposed but remain uninfected with HIV-1. Naranbhai et al found significant association between markers of systemic immune activation, including cytokine concentrations, and HIV-1 acquisition among women [10], but few other data are available on the role of systemic immune function in HIV-1 acquisition or transmission. Insight into the roles of innate and adaptive immune functions and HIV-1 susceptibility and infectiousness is an important factor in the development of effective prophylactic and therapeutic HIV-1 vaccines, as well as strategies for HIV-1 prevention, such as preexposure prophylaxis and microbicides. Findings from the Step HIV-1 vaccine trial showed that, in a prime-boost HIV-1 vaccine strategy based on an adenovirus type 5 (Ad5) vector, there was a trend toward increased HIV-1 acquisition among Ad5-seropositive individuals. This finding indicated that the amnestic immune response elicited was harmful [11], suggesting a challenge for vaccine development and a critical need to better understand how immune activation affects HIV-1 transmission. In African populations, where both HIV-1 infection and other infectious diseases are frequently prevalent, chronic immune activation may be heightened by the presence of other infections, both systemic and mucosal, that could contribute to increased susceptibility and infectiousness [12]. The aim of the present study was to assess whether differences in markers of immune activation, as measured by a data from a panel of cytokines, were associated with an increased risk of HIV-1 transmission among heterosexual HIV-1–serodiscordant couples. Assessment of these factors in cases of virologically linked HIV-1 transmission in a prospective cohort of HIV-1–serodiscordant couples permitted simultaneous evaluation of the relationship between systemic immune activation and both the heightened susceptibility of HIV-1–uninfected partners and the infectiousness of HIV-1–infected partners.

We evaluated the efficacy of a brief motivational intervention (MI) counseling in reducing alcohol consumption among persons living with HIV/AIDS in Kampala, Uganda. Persons living with HIV/AIDS with Alcohol Use Disorders Identification Tool (AUDIT) score ≥3 points were randomized to either standardized positive prevention counseling alone or in combination with alcohol brief MI counseling. The mean change in AUDIT-C scores over 6 months was compared by treatment arm. The mean (standard deviation [SD]) AUDIT-C scores were 6.3 (2.3) and 6.8 (2.3) for control and MI arms ( P = .1) at baseline, respectively, and change in mean AUDIT-C score was not statistically different between arms over the 6 months ( P = .8). However, there was a statistically significant decrease in mean AUDIT-C score (-1.10; 95% confidence interval: -2.19 to -0.02, P = .046) among women in the MI arm. There was a nondifferential reduction in alcohol consumption overall, but MI appeared effective among women only. Studies with more than 1 counseling session and evaluation of gender differences in treatment response are needed.

BackgroundThe efficacy of condoms for protection against transmission of herpes simplex virus type 2 (HSV-2) has been examined in a variety of populations with different effect measures. Often the efficacy has been assessed as change in hazard of transmission with consistent vs inconsistent use, independent of the number of acts. Condom efficacy has not previously measured on a per-act basis.MethodsWe examined the per-act HSV-2 transmission rates with and without condom use among 911 African HSV-2 and human immunodeficiency virus type 1 (HIV-1) serodiscordant couples followed for an average of 18 months in an HIV prevention study. Infectivity models were used to associate the log10 probability of HSV-2 transmission over monthly risk periods with reported numbers of protected and unprotected sex acts. Condom efficacy was computed as the proportionate reduction in transmission risk for protected relative to unprotected sex acts.ResultsTransmission of HSV-2 occurred in 68 couples, including 17 with susceptible women and 51 with susceptible men. The highest rate of transmission was from men to women: 28.5 transmissions per 1000 unprotected sex acts. We found that condoms were differentially protective against HSV-2 transmission by sex; condom use reduced per-act risk of transmission from men to women by 96% (P < .001) and marginally from women to men by 65% (P = .060).ConclusionsCondoms are recommended as an effective preventive method for heterosexual transmission of HSV-2.

INTRODUCTION: Alcohol use by persons living with HIV/AIDS (PLWHA) negatively impacts the public health benefits of antiretroviral therapy (ART). Using a standardized alcohol assessment tool, we estimate the prevalence of alcohol use, identify associated factors, and test the association of alcohol misuse with sexual risk behaviors among PLWHA in Uganda. METHODS: A cross-section of PLWHA in Kampala were interviewed regarding their sexual behavior and self-reported alcohol consumption in the previous 6 months. Alcohol use was assessed using the alcohol use disorders identification test (AUDIT). Gender-stratified log binomial regression analyses were used to identify independent factors associated with alcohol misuse and to test whether alcohol misuse was associated with risky sexual behaviors. RESULTS: Of the 725 subjects enrolled, 235 (33%) reported any alcohol use and 135 (18.6%) reported alcohol misuse, while 38 (5.2%) drank hazardous levels of alcohol. Alcohol misuse was more likely among subjects not yet on ART (adjusted prevalence ratio [aPR] was 1.65 p=0.043 for males and 1.79, p=0.019 for females) and those with self-reported poor adherence (aPR for males=1.56, p=0.052, and for females=1.93, p=0.0189).
