본 연구는 신문기사 자료를 활용하여 도시 관점에서 코로나19에 대한 이슈 흐름을 살펴보는데 목적이 있다. 연구대상은 국내에서 발행되는 총 47개의 언론사의 기사를 대상으로 하고, 검색 기간은 2020년 1월 20일부터 8월 2일까지 총 28주이다. 특히, 코로나19의 유행을 출현 단계(1월 20일~2월 17일), 확산 단계(2월 18일~5월 5일), 생활방역 단계(5월 6일~8월 2일)로 구분하여 분석에 활용하였다. 데이터베이스 구축은 빅카인즈를 활용했으며, ‘코로나’와 ‘도시’를 키워드로 검색하였다. 분석방법으로는 추이 그래프, 빈도분석, 분산분석, 워드클라우드, 의미연결망 분석 등을 활용하였다. 분석결과, 첫째, 코로나 출현 단계에서는 ‘중국’, ‘바이러스’, ‘신종’, ‘우한’, 확산 단계에서는 ‘확진자’, ‘대구’, 생활방역 단계에서는 ‘사업’, ‘산업’, ‘뉴딜’, ‘관광’ 등과 같은 키워드들이 많이 언급되었다. 둘째, 주요 키워드들의 출현 비중은 코로나19 유행 단계 그룹별로 통계적으로 유의미한 차이가 있었다. 셋째, 워드클라우드 및 의미연결망 분석결과, 출현 및 확산단계에서는 도시적 차원의 대응과 관련된 이슈가 거의 나타나지 않았으나, 생활방역 단계에서는 서서히 ‘계획’, ‘주택’, ‘공간’ 등과 같은 키워드들도 출현하기 시작하였다. 향후 코로나19에 대한 다양한 연구과 정책적 제안들을 제시하는데 본 연구가 활용될 수 있기를 기대한다.

Background: Due to the aging population in Korea, knee osteoarthritis (KOA) has become an increasingly common condition. Many patients with KOA prefer analgesics, herbal medicines, acupuncture, or exercise, rather than arthroscopic surgery or a knee replacement. Gyebutang (GB) granules are a herbal extract widely used to treat KOA in traditional Korean medicine, but there is insufficient evidence of its efficacy and safety.Methods: A multicenter, randomized, assessor-blinded, 2-armed parallel, controlled clinical trial has been designed to investigate the efficacy and safety of GB combined with acupuncture for the treatment of KOA. There will be 100 patients with KOA enrolled in the study from 3 traditional Korean medicine hospitals. The participants will be randomly allocated to an experimental group (GB and acupuncture) or a control group (celecoxib and acupuncture) in a 1:1 ratio. Both groups will receive acupuncture treatment once a week for 6 weeks; one group will receive GB and the other will receive celecoxib for the same duration.Results: The primary outcome will be the change of knee osteoarthritic pain, based on scores on a 100 mm visual analog scale. The secondary outcomes will be scores on a numeric rating scale, the Western Ontario and McMaster Universities osteoarthritis index, patient global assessment, European quality of life 5-dimension 5-level scale, and adverse events.Conclusion: The results of this study will provide evidence of efficacy and safety of GB as a treatment for patients with KOA.

Background Wolf-Hirschhorn syndrome (WHS) includes features such as growth restriction, mental retardation, congenital heart disease, convulsions as well as microcephaly and micrognathia. Thus, the anesthesiologists may have difficulties in airway management, neuromuscular relaxation, and in maintaining hemodynamic stability. Case A 24-year-old man with WHS underwent surgery for closed reduction and internal fixation of the right neck of femur. His face showed features typical of patients with WHS such as a prominent glabella, hypertelorism, micrognathia, low-set malformed ears, and a down-turned mouth. Since difficult airway management was expected, a video-assisted laryngoscope was used for successful intubation. The surgery terminated without any problems under total intravenous anesthesia. Conclusions A patient with WHS may have a variety of problems. Therefore, through careful evaluation of the airway, cardiovascular system, and cerebral nervous system and by making an appropriate anesthetic plan, anesthesiologists can perform a safe general anesthesia in patients with WHS.

현대의학기술의 비약적 발달은 사망에 임박한 회생가능성이 없는 사람의 죽음에 이르는 과정을 점차 길게 하였다. 그에 따라 오랫동안 사회 각층에서 연명치료가 오히려 인간의 존엄을 오히려 해치는 것은 아닌지, 연명치료를 중단하려면 어떠한 요건을 충족하여야 하는지에 관하여 많은 논의가 있었다. 우리는 2016. 2. 3. 연명의료결정법을 제정하였고 그간 학계 논의와 개별 사건에서 법원의 판단을 통해 해결하였던 연명치료 중단의 문제를 입법적으로 해결하고자 하였지만 현행의 연명의료결정법은 짧은 기간 동안 두 차례개정까지 하였음에도 여전히 의료현실에서 발생하는 문제들을 해결하기에는 추가적인 보완이 필요한 것으로 보인다. 의료현실에 대한 고려와 체계적인 법해석이 부족한 상태에서 사회적 필요에 따라 성급하게 입법화하는 과정에서 그동안 법원의 판단을 통해 해결하였던 문제 이상으로 의료현장에서 발생하는 혼란을 해소하기에는 부족한 법이 만들어진 것으로 보인다. 연명의료결정법이 밝히는 목적과 같이 환자의 최선의 이익을 보장하고, 자기결정을 존중하여 인간으로서의 존엄과 가치를 보호하기 위해서는 ①대상이 되는 환자의 범위를 ‘임종과정에 있는 환자’에 한정할 것이 아니라 말기의 중증환자나 지속적 식물인간 상태에 있는 환자로서 사실상 임종과정에 있는 환자에 준한다고 볼 수 있는 경우에도 그에 부합하는 의학적 판단과 법으로 정하는 절차에 따라 연명의료중단이 가능하게 하되, ②사전에 환자가 연명의료중단을 희망하는 의사를 표시하였음이 환자 본인이 작성한 문서, 녹음물, 녹화물 또는 이에 준하는 기록물 등에서 객관적으로 확인되는 경우 연명의료중단 시점에서 환자의 연명의료중단 의사를 추정하고, ③벌칙 규정과 관련하여 형법상 범죄와 연명의료결정법 제40조의 관계를 보다 명확히 하는 방향으로 입법적인 개선을 지속해 나간다면 현행 연명의료결정법이 가지고 있는 문제를 조금이라도 해소할 수 있을 것이라 생각한다. 앞으로도 심도 있는 논의와 지속적인 연구가 계속되길 기대해 본다.

Elevating neuroprotective proteins using adeno-associated virus (AAV)-mediated gene delivery shows great promise in combating devastating neurodegenerative diseases. Amyotrophic lateral sclerosis (ALS) is one such disease resulting from loss of upper and lower motor neurons (MNs) with 90-95% of cases sporadic (SALS) in nature. Due to the unknown etiology of SALS, interventions that afford neuronal protection and preservation are urgently needed. Caveolin-1 (Cav-1), a membrane/lipid rafts (MLRs) scaffolding and neuroprotective protein, and MLR-associated signaling components are decreased in degenerating neurons in postmortem human brains. We previously showed that, when crossing our SynCav1 transgenic mouse (TG) with the mutant human superoxide dismutase 1 (hSOD1G93A) mouse model of ALS, the double transgenic mouse (SynCav1 TG/hSOD1G93A) exhibited better motor function and longer survival. The objective of the current study was to test whether neuron-targeted Cav-1 upregulation in the spinal cord using AAV9-SynCav1 could improve motor function and extend longevity in mutant humanized mouse and rat (hSOD1G93A) models of familial (F)ALS. Methods: Motor function was assessed by voluntary running wheel (RW) in mice and forelimb grip strength (GS) and motor evoked potentials (MEP) in rats. Immunofluorescence (IF) microscopy for choline acetyltransferase (ChAT) was used to assess MN morphology. Neuromuscular junctions (NMJs) were measured by bungarotoxin-a (Btx-a) and synaptophysin IF. Body weight (BW) was measured weekly, and the survival curve was determined by Kaplan-Meier analysis. Results: Following subpial gene delivery to the lumbar spinal cord, male and female hSOD1G93A mice treated with SynCav1 exhibited delayed disease onset, greater running-wheel performance, preserved spinal alpha-motor neuron morphology and NMJ integrity, and 10% increased longevity, independent of affecting expression of the mutant hSOD1G93A protein. Cervical subpial SynCav1 delivery to hSOD1G93A rats preserved forelimb GS and MEPs in the brachial and gastrocnemius muscles. Conclusion: In summary, subpial delivery of SynCav1 protects and preserves spinal motor neurons, and extends longevity in a familial mouse model of ALS without reducing the toxic monogenic component. Furthermore, subpial SynCav1 delivery preserved neuromuscular function in a rat model of FALS. The latter findings strongly indicate the therapeutic applicability of SynCav1 to treat ALS attributed to monogenic (FALS) and potentially in sporadic cases (i.e., SALS).

Purpose: The purpose of this study was to identify the factors influencing professional self-concept of advanced practice nurses. Methods: Data were collected from 185 advanced practice nurses in B city. The collected data were analyzed by descriptive statistics, t-test, ANOVA, Pearson correlation coefficient, and simultaneous multiple regression using SPSS/WIN 21.0 program. Results: Professional self-concept of advanced practice nurses significantly differed according to age, work place, current position, total career and satisfaction to nursing. 본 연구는 대학생을 대상으로 아동기 정서적 학대와 성인기 우울증상의 관계에서 인지적 취약성과 경험회피가 순차적으로 매개하는지를 검증하였다. 이를 위해 325명의 대학생들은 아동기 정서적 학대, 인지적 취약성, 경험회피, 그리고 우울을 평가하는 설문지를 작성하였다. 연구 결과, 아동기 정서적 학대와 성인기 우울 증상의 관계를 인지적 취약성과 경험회피가 연속매개하였다. 또한 완전매개 모형과 부분매개모형을 비교한 결과, 완전매개모형이 보다 더 적합한 것으로 나타났다. 이러한 결과는 인지적 취약성에 초점을 맞추는 것뿐만 아니라 내적 경험 수용 능력을 발달시키는데 초점을 둔 개입의 중요성을 시사한다. 이후 본 연구의 제한점 및 후속 연구의 방향을 제시하였다.

본 연구는 전국의 청소년복지시설을 이용하거나 이곳에서 생활하는 만 15세 이상 24세 미만의 청소년 30명을 대상으로 인터뷰 조사를 실시하여 가정 밖 청소년들의 자립 모색 과정을 분석하였다. 수합된 자료는 Strauss & Corbin(1990)의 근거이론을 기반으로 개방코딩, 축코딩, 선택코딩의 분석과정을 거쳤다. 분석결과, 이들은 ‘1차 사회화 환경의 붕괴(인과적 조건)’로 인해 가정과 학교에서 제대로 된 보살핌을 받지 못하고 있었고 그 결과 집을 나와 길거리 생활, 아르바이트, 빚/범죄에의 연루 등 ‘한국사회의 통상적인 청소년 사회화 과정 밖에서의 삶(맥락적 조건)’을 살고 있었다. 이러한 생활을 하다가 ‘시설 생활을 시작(중심현상)’하여 신체적, 심리적 휴식 및 안정을 갖게 되면서 전환의 계기를 갖게 된다. 또한 시설을 통해 학업지도, 심리상담, 취업 관련 프로그램 등의 다양한 ‘지원 및 지지(중재적 조건)’를 받게 되면서 청소년 자신도 ‘자립을 위한 노력과 시도(작용/상호작용)’를 하게 되어 자립(결과)할 수 있는 기반을 다지게 된다. 그러나 모든 가정 밖 청소년들이 이러한 단계를 순차적으로 밟아 가면서 자립을 위한 기틀 마련이라는 성공적인 결과를 갖게 되는 것은 아니었다. 각 단계마다 가정 밖 청소년들이 그 다음 단계로 나아갈 수 없는 다양한 난관이 있으며 이에 부딪혀 좌절하고 방황하면서 자립 의지가 꺾이기도 한다. 그러므로 가정 밖 청소년들을 위한 정책제안은 이들의 자립과정을 지원하는 것을 목표로 하여 각 단계에 존재하는 난관과 장애물을 어떻게 통제할 것인가를 중심으로 이루어져야 할 것이다. 본 연구에서는 가정 밖 청소년들이 처한 다양한 상황을 바탕으로 이들의 자립을 지원 할 수 있는 정책을 제안하였다.
