ObjectiveAs phase III trials have shown interest in innovative but expensive drugs in the treatment of neuromyelitis optica spectrum disorder (NMOSD), data are needed to clarify strategies in the treatment of neuromyelitis optica (NMO). This meta-analysis compares the efficacy of first-line strategies using rituximab (RTX), mycophenolate mofetil (MMF), or azathioprine (AZA), which are still widely used.MethodsStudies identified by the systematic review of Huang etal. (2019) were selected if they considered at least two first-line immunosuppressants among RTX, MMF, and AZA. We updated this review. The Medline, Cochrane Central Register of Controlled Trials, Embase, and ClinicalTrials databases were queried between November 2018 and April 2020. To be included, the hazard ratio (HR) [95% CI] for the time to first relapse after first-line immunosuppression had to be available, calculable, or provided by the authors.ResultsWe gathered data from 919 NMO patients (232 RTX-, 294 MMF-, and 393 AZA-treated patients). The risk of first relapse after first-line immunosuppression was 1.55 [1.04, 2.31] (p=0.03) for MMF compared with RTX, 1.42 [0.87, 2.30] (p=0.16) for AZA compared with RTX, and 0.94 [0.58, 1.54] (p=0.08) for MMF compared with AZA.InterpretationThe findings suggest that RTX is more efficient than MMF as a first-line therapy. Even if the results of our meta-analysis cannot conclude that RTX has a better efficacy in delaying the first relapse than AZA, the observed effect difference between both treatments combined with the results of previous studies using as outcome the annualized relapse rate may be in favor of RTX.

bBackground/Aims:/bA meta-analysis of randomized trials performed in healthy asymptomatic individuals suggested that overall mortality may increase afteriHelicobacter pylori/ieradication despite a significant decrease in the gastric cancer incidence and mortality rates. This retrospective population-based cohort study investigated ifiH. pylori/itreatment is associated with an increase in overall mortality in patients with hypertension.bMethods:/bFrom the database of the Korean National Health Insurance Sample Cohort, we selected 198,487 patients treated for hypertension between 2002 and 2010. Those who receivediH. pylori/itreatment (iH. pylori/itreatment cohort, 5,541 patients) were matched to those who did not (nontreatment cohort, 11,082 patients) at the ratio of 1 to 2. The primary outcome was the risk of overall mortality. The secondary outcomes were the risks of mortality due to cardiovascular disease, cerebrovascular disease, and cancer. The outcomes were evaluated from 6 months afteriH. pylori/itreatment to December 2013. A Cox proportional hazard model was used to estimate the hazard ratios (HRs).bResults:/bDuring a median follow-up period of 4.8 years, death from any cause was reported in 4.1% of the patients in theiH. pylori/itreatment cohort and 5.5% of the patients in the nontreatment cohort. The adjusted HR (aHR) for overall mortality in theiH. pylori/itreatment cohort was 0.70 (95% confidence interval [CI], 0.60 to 0.82; p0.001). With regard to cause-specific mortality, compared with the nontreatment cohort, theiH. pylori/itreatment cohort had a lower risk of mortality due to cerebrovascular disease (aHR, 0.46; 95% CI, 0.26 to 0.81; p=0.007). The risks of mortality due to cancer and cardiovascular disease were not different between the cohorts.bConclusions:/biH. pylori/itreatment is not associated with an increase in overall mortality in patients treated for hypertension.

The purpose of this study was to norm-referenced criteria of upper limb for Korean middle school baseball player. Ninety five male middle school students with age ranging 14 to 15 years were participated in this study. They didn’t have any medical problem. HUMAC NORM (Stoughton, MA, USA) equipment system was used to obtain the values of peak torque, peak torque to body weight ratio. The results were presented as a norm-referenced criterion value using 5-point scale, by 5 groups (5 percent, 15 percent, 60 percent, 15 percent and 5 percent) and a few suggestions from this study as follows; • For the evaluation of strength, peak torque % body weights more than peak torque appeared to be more valuable information. • The provided criterion of peak torque % body weights are very information for baseball coach, athletic trainer, baseball player, sports injury rehabilitation specialists.

Positive fluid balance is a risk factor for mortality in critically ill patients, especially those requiring continuous renal replacement therapy (CRRT). However, the association between daily fluid balance and various organ impairments remains unclear. This study investigated the impacts of daily fluid balance prior to CRRT on organ dysfunction, as well as mortality in critically ill patients. We identified daily fluid balance between intensive care unit (ICU) admission and CRRT initiation. According to daily fluid balance, the time to CRRT initiation and the rate of organ failure based on the sequential organ failure assessment (SOFA) score were assessed. We recruited 100 patients who experienced CRRT for acute kidney injury. CRRT was initiated within 2 [0, 4] days. The time to CRRT initiation was shortened in proportion to daily fluid balance, even after the adjustment for the renal SOFA score at ICU admission (HR 1.14, P = 0.007). Based on the SOFA score, positive daily fluid balance was associated with respiratory, cardiovascular, nervous, and coagulation failure, independent of each initial SOFA score at ICU admission (HR 1.36, 1.26, 1.24 and 2.26, all P < 0.05). Ultimately, we found that positive fluid balance was related with an increase in the rate of 28-day mortality (HR 1.14, P = 0.012). Positive daily fluid balance may accelerate the requirement for CRRT, moreover, it can be associated with an increased risk of multiple organ failure in critically ill patients., Graphical Abstract
