Purpose: To determine the clinical efficacy of epirubicin plus docetaxel or paclitaxel in the management of breast cancer. Methods: This study was a randomized controlled trial that recruited 78 patients with breast cancer treated in The First People’s Hospital of Anqing between December 2018 and March 2021. The participants were randomized to receive either epirubicin plus docetaxel (control group) or epirubicin plus paclitaxel (study group), with 39 cases in each group. Clinical outcomes, adverse events, quality of life, and prognosis of the two groups were compared. Results: Epirubicin plus paclitaxel produced greater efficacy than epirubicin plus docetaxel (p < 0.05). The combination also exhibited a higher safety profile versus epirubicin with docetaxel, as evidenced by lower incidences of nausea and vomiting, intestinal discomfort, muscle pain, and dyspnea reactions (p < 0.05). Epirubicin with paclitaxel resulted in greater improvement in quality of life in patients than epirubicin plus docetaxel (p < 0.05) as well as provided more benefits in terms of survival and recurrence control than epirubicin plus docetaxel (p < 0.05). Conclusion: Epirubicin/paclitaxel combination alleviates clinical symptoms, reduces adverse events, enhances patient prognosis, lowers the risk of postoperative recurrence, and provides superior quality of life when compared to epirubicin/docetaxel combination in breast cancer patients. Nonetheless, large-scale and better-randomized clinical trials are required to validate the findings of this study.

Deep learning has been widely used in single image rain removal and demonstrated favorable universality. However, it is still challenging to remove rain streaks,especially in the nightscape rain map which exists heavy rain and rain streak accumulation. To solve this problem, a single image nightscape rain removal algorithm based on Multi-scale Fusion Residual Network is proposed in this paper. Firstly, based on the motion blur model, evenly distributed rain streaks are generated and the dataset is reconstructed to solve the lack of nightscape rain map datasets. Secondly, according to the characteristics of the night rain map, multi-scale residual blocks are drawn on to reuse and propagate the feature, so as to exploit the rain streaks details representation. Meanwhile, the linear sequential connection structure of multi-scale residual blocks is changed to a u-shaped codec structure, which tackles the problem that features cannot be extracted effectively due to insufficient scale. Finally, the features of different scales are combined with the global self-attention mechanism to get different rain streak components, then a cleaner restored image is obtained. The quantitative and qualitative results show that, compared to the existing algorithms, the proposed algorithm can effectively remove rain streaks while retaining detailed information and ensuring the integrity of image information. &nbsp

It is of great importance to treat a bacterial-infected wound by a smart dressing capable of delivering antibiotics in a smart manner without causing drug resistance. The construction of smart release nanocontainers responsive to near-infrared (NIR) laser irradiation in an on-demand and stepwise way is a promising strategy for avoiding the emergence of multidrug-resistant bacteria. Here, we develop a hydrogel composite made of alginate and nanotubes with an efficient NIR-triggered release of rifampicin and outstanding antibacterial ability. This composite hydrogel is prepared through co-encapsulating antibacterial drug (rifampicin), NIR-absorbing dye (indocyanine green), and phase-change materials (a eutectic mixture of fatty acids) into halloysite nanotubes, followed by incorporation into alginate hydrogels, allowing the in-situ gelation at room temperature and maintaining the integrity of drug-loaded nanotubes. Among them, the eutectic mixture with a melting point of 39 °C serves as the biocompatible phase-change material to facilitate the NIR-triggered drug release. This paper focuses on the role of the new East Coast Rail Links (ECRL) project in Malaysia and how it will improve logistics efficiency, economic development, and environmental friendliness to meet users' expectations. The ECRL project aims to enhance connectivity between East and West Coast regions to stimulate industrial, commercial, and tourism development. The ECRL is a large-scale and heavily constructed railroad construction project. ECRL has not been a smooth project, with the government proposing the program in 2016 and construction beginning in 2017. However, due to various factors, it was suspended in 2018 and continued with a new rail route in 2019. Users' expectations for ECRL are mainly to reduce traffic congestion problems on the road, reduce high road transport costs, reduce pollution to the environment, drive business development and promote employment in the surrounding areas along the route. Because all these problems are becoming severe in Malaysia now, we will study ECRL and collect data from the respondents to point out the users' expectations of ECRL in terms of improving logistics efficiency, economic development, and environmental friendliness. After analysing the data, our study concludes with the result that all three variables positively influence users' expectations of ECRL, which, as a new railroad in Malaysia, will play a crucial role in the future development of the East Coast of Malaysia and Malaysia as a whole.

Arc volcanic rocks erupted above oceanic subduction zones generally have heavy molybdenum (Mo) isotopic compositions, which are attributed to contributions from oceanic slab–derived fluids with isotopically heavy Mo at subarc depths. Given that mafic igneous rocks with light Mo isotopic compositions have been rarely identified, it is unclear where light Mo isotope reservoirs reside, as these must exist due to oceanic subduction through geologic time. We present Mo isotope data from Mesozoic–Cenozoic mafic igneous rocks from the eastern North China craton, which were affected by continuous subduction of the oceanic slab. The Mesozoic mafic igneous rocks have island-arc basalt (IAB)–like features and high δ98Mo values of –0.15‰ to +0.09‰. In contrast, the Cenozoic mafic igneous rocks have ocean-island basalt (OIB)–like features and low δ98Mo values of –0.53‰ to –0.19‰. In addition, these two suites of rocks exhibit systematic differences in Sr-Nd isotopic compositions and fluid- and melt-mobile element contents, indicating that their mantle sources contained crustal components derived from different depths. The heavy Mo and weakly enriched Sr-Nd isotopic compositions and IAB-like trace-element signatures were inherited from slab fluids (including oceanic crust–derived aqueous solutions and seafloor sediment–derived hydrous melts) liberated at subarc depths. The light Mo and depleted Sr-Nd isotopic compositions and OIB-like trace-element signatures were inherited from dehydrated slab–derived melts (with rutile breakdown) at postarc depths. Therefore, Mo isotope systematics of mafic igneous rocks are a powerful means with which to trace the geochemical fluxes from subducting oceanic slabs to the mantle wedge at different depths.

Aims: Progression of disease within 24 months (POD24) is associated with poor survival in some subtypes of lymphoma.The aim is to identify high-risk patients with localized extranodal natural killer/T-cell lymphoma (ENKTL) and to define clinical factors associated with the risk of early recurrence after antitumor treatment. Methods: The authors retrospectively analyzed 330 cases with localized ENKTL, of which 89 experienced POD24. Results: The 5-year overall survival of the POD24 group was extremely inferior to that of the non-POD24 group. Risk factors for POD24 were Eastern Cooperative Oncology Group performance status ≥2, response evaluation (non-complete remission) after first-line treatment and elevated lactate dehydrogenase concentrations. Also, higher Epstein–Barr virus DNA titer was related to POD24. Based on these data with or without the availability of Epstein–Barr virus DNA, the authors conducted two nomograms to predict POD24, which showed good accuracy with high C statistics. Conclusion: The results showed that POD24 could serve as a marker to identify patients whose medical needs were unmet in ENKTL.

Selenium deficiency can be associated with increased susceptibility to some viral infections and even more severe diseases. In this study, we aimed to examine whether this association applies to severe fever with thrombocytopenia syndrome (SFTS).An observational study was conducted based on the data of 13,305 human SFTS cases reported in mainland China from 2010 to 2020. The associations among incidence, case fatality rate of SFTS, and crop selenium concentration at the county level were explored. The selenium level in a cohort of patients with SFTS was tested, and its relationship with clinical outcomes was evaluated.The association between selenium-deficient crops and the incidence rate of SFTS was confirmed by multivariate Poisson analysis, with an estimated incidence rate ratio (IRR, 95% confidence interval [CI]) of 4.549 (4.215-4.916) for moderate selenium-deficient counties and 16.002 (14.706-17.431) for severe selenium-deficient counties. In addition, a higher mortality rate was also observed in severe selenium-deficient counties with an IRR of 1.409 (95% CI: 1.061-1.909). A clinical study on 120 patients with SFTS showed an association between serum selenium deficiency and severe SFTS (odds ratio, OR: 2.94; 95% CI: 1.00-8.67) or fatal SFTS (OR: 7.55; 95% CI: 1.14-50.16).Selenium deficiency is associated with increased susceptibility to SFTS and poor clinical outcomes.

Lymphoma-associated hemophagocytic syndrome (LAHS) is a rare and life-threatening clinical syndrome with rapidly deteriorating health and high mortality. We retrospectively analyzed clinical features and prognostic factors from 117 patients diagnosed with LAHS. The cumulative incidence rate of LAHS was 4.0% (117/2906). Patients were classified into B-cell LAHS (B-LAHS, n = 22) and T/natural killer (NK)-cell LAHS (T/NK-LAHS, n = 95) groups. Patients with T/NK-LAHS were younger and had lower neutrophil counts and fibrinogen values, higher LDH and transaminase levels, and were more likely to develop hemophagocytic syndrome (HPS) during the clinical course than those with B-LAHS. The median survival time for the entire cohort was 57 days, and for the T/NK-LAHS and B-LAHS groups, it was 52 and 154 days, respectively, after the diagnosis of LAHS. Two undescribed dammarane triterpenoid saponins, cypaliurusides O and P (1 and 2), were isolated from the ethanol extracts of the leaves of Cyclocarya paliurus. Bioactivity assay results showed that compound 1 has potential cytotoxic activities against selected human cancer cell lines in vitro, with IC50 values ranging from 14.55 ± 0.55 to 22.75 ± 1.54 µM. Compound 1 showed better antitumor activity against HepG2 cells with IC50 of 14.55 ± 0.55 µM. In addition, compound 2 showed no obvious antitumor activity.

Carbonate and silicate metasomatism occurring in subduction zones is an important mechanism underlying mantle heterogeneity and compositional diversity of mantle-derived rocks. However, distinguishing between the two kinds of metasomatism is often difficult. Lithium (Li) and its isotopes have great potential in this regard because of the different Li isotopic compositions of recycled marine carbonate and silicate components. We report Li isotopic data from Cenozoic and Mesozoic alkali basalts of the West Qinling orogen in central China. Relative to those for normal basalts, very high δ7Li values (up to +11.2‰) were observed for the Cenozoic alkali basalts, but significantly and systematically lower values (as low as −3.3‰) were estimated for the Mesozoic alkali basalts. Their abnormal Li isotopic compositions, combined with major- and trace-element contents and Sr-Mg isotope ratios, indicate that the Cenozoic and Mesozoic alkali basalts originated from carbonated and silicated mantle sources, respectively, reflecting metasomatism of the mantle by slab-derived carbonate and silicate melts during Paleotethyan oceanic subduction. Interactions of such melts with the mantle peridotite in subduction channels can account for the elemental and isotopic differences of the studied alkali basalts. The present study demonstrates an effective way to distinguish between carbonate and silicate metasomatism in subduction zones by studying Li isotopic compositions of alkali basalts and highlights the prospect of Li isotopes in tracing the deep carbon cycle.

Post-collisional alkaline intrusive rocks from the Dabie orogen were studied for their whole-rock major-trace elements and Sr-Nd-Hf-Pb isotopes, as well as zircon U-Pb ages and Hf-O isotopes. The results provide geochemical constraints on the nature of their mantle sources and thus insight into crust-mantle interaction in the continental collision zone. The alkaline intrusive rocks are composed of syenite and nepheline syenite. Syn-magmatic zircon U-Pb dating by LA-ICP-MS for them yielded Early Cretaceous ages of 131.3 ± 1.4 Ma to 122.6 ± 0.6 Ma, coeval with the post-collisional magmatism in the Dabie orogen. One relict zircon with U-Pb age of 211 Ma is consistent with the timing of metamorphism for the ultrahigh-pressure (UHP) metamorphic rocks in this orogen. They have arc-like trace element distribution patterns, such as enrichment in LILE (large ion lithophile element) and LREE (light rare earth element) but depletion in HFSE (high field strength element), and enriched whole-rock Sr-Nd-Hf isotope compositions with high (87Sr/86Sr)i ratios of 0.7077–0.7131 but negative εNd(t) values of −16.0 to −9.4 and εHf(t) values of −17.5 to −12.7. Their syn-magmatic zircons have three groups of Hf-O isotope compositions comparable to those of UHP metamorphic rocks in Central-South Dabie and North Dabie, which represent the upper and middle continental crust of the subducted South China Block, respectively. In this regard, slab–mantle interaction is evident during the Triassic continental collision. We suggest that the melts derived from the subducted South China Block reacted with the lithospheric mantle wedge of the North China Block, resulting in phlogopite-bearing metasomatites, whose partial melting would generate the post-collisional alkaline intrusive rocks during the Early Cretaceous.

Eight Gram-negative, aerobic, motile with paired polar flagella and rod-shaped bacteria were isolated from six tobacco fields in Yunnan, PR China. 16S rRNA gene sequence analysis revealed that all the strains belonged to the genus Ralstonia. Among them, strain 22TCCZM03-6 had an identical 16S rRNA sequence to that of R. wenshanensis 56D2T, and the other strains were closely related to R. pickettii DSM 6297T (98.34–99.86%), R. wenshanensis 56D2T (98.70–99.64%), and R. insidiosa CCUG 46789T (97.34–98.56%). Genome sequencing yielded sizes ranging from 5.17 to 5.72 Mb, with overall G + C contents of 63.3–64.1%. Pairwise genome comparisons showed that strain 22TCCZM03-6 shared average nucleotide identity (ANI) and digital DNA–DNA hybridization (dDDH) values above the species cut-off with R. wenshanensis 56D2T, suggesting that strain 22TCCZM03-6 is a special strain of the R. wenshanensis. Five strains, including 21MJYT02-10T, 21LDWP02-16, 22TCJT01-1, 22TCCZM01-4, and 22TCJT01-2, had ANI values >95% and dDDH values >70% when compared with each other. These five strains had ANI values of 73.32–94.17% and dDDH of 22.0–55.20% with the type strains of the genus Ralstonia individually, supporting these five strains as a novel species in the genus Ralstonia. In addition, strains 21YRMH01-3T and 21MJYT02-11T represent two independent species. They both had ANI and dDDH values below the thresholds for species delineation when compared with the type species of the genus Ralstonia. In strains 21YRMH01-3T and 21MJYT02-10T, the main fatty acids were summed features 3, 8, and C16:0; however, strain 21MJYT02-11T contained C16:0, cyclo-C17:0, and summed features 3 as major fatty acids. The main polar lipids, including diphosphatidylglycerol, phosphatidylglycerol, and phosphatidylethanolamine, were identified from strains 21YRMH01-3T, 21MJYT02-10T, and 21MJYT02-11T. The ubiquinones Q-7 and Q-8 were also detected in these strains, with Q-8 being the predominant quinone. Based on the above data, we propose that the eight strains represent one known species and three novel species in the genus Ralstonia, for which the names Ralstonia chuxiongensis sp. nov., Ralstonia mojiangensis sp. nov., and Ralstonia soli sp. nov. are proposed. The type strains are 21YRMH01-3T (=GDMCC 1.3534T = JCM 35818T), 21MJYT02-10T (=GDMCC 1.3531T = JCM 35816T), and 21MJYT02-11T (=GDMCC 1.3532T = JCM 35817T), respectively.

Background Surgical repair is indicated for patellar tendon ruptures that result in loss of knee extensor mechanism function. However, biomechanical studies report conflicting results when comparing transosseous suture versus suture anchor repair techniques. This discrepancy may be due to inconsistencies in experimental design as these studies use various numbers of suture strands. Therefore, the main objective of this study is to compare the ultimate load of four- versus six-strand transosseous suture repair. Secondary objectives are to compare gap formation after cyclical loading and mode of failure. Methods Six pairs of fresh-frozen cadaveric specimen were randomly allocated to either four- or six-strand transosseous suture repair. Specimen underwent preconditioning cyclical loading and then load to failure. Results The six-strand repair had a significantly higher maximum load to failure compared with the four-strand repair (mean difference = 319.3 N [57.9%], p = 0.03). There was no significant difference in gap length after cyclical loading or at max load. There were no significant differences in mode of failure. Conclusion Utilizing a six-stand transosseous patella tendon repair construct with one additional suture increases overall construct strength by over 50% compared with a four-strand construct.

Nasopharyngeal carcinoma (NPC) is prevalent in Southern China and Southeast Asia, and autoantibody signatures may improve early detection of NPC. In this study, serum levels of autoantibodies against a panel of six tumor-associated antigens (p53, NY-ESO-1, MMP-7, Hsp70, Prx VI, and Bmi-1) and Epstein–Barr virus capsid antigen-IgA (VCA-IgA) were tested by enzyme-linked immunosorbent assay in a training set (220 NPC patients and 150 controls) and validated in a validation set (90 NPC patients and 68 controls). We used receiver-operating characteristics (ROC) to calculate diagnostic accuracy. ROC curves showed that use of these 6 autoantibody assays provided an area under curve (AUC) of 0.855 [95% confidence interval (CI), 0.818–0.892], 68.2% sensitivity, and 90.0% specificity in the training set and an AUC of 0.873 (95% CI, 0.821–0.925), 62.2% sensitivity, and 91.2% specificity in the validation set. Moreover, the autoantibody panel maintained diagnostic accuracy for VCA-IgA–negative NPC patients [0.854 (0.809–0.899), 67.8%, and 90.0% in the training set; 0.879 (0.815–0.942), 67.4%, and 91.2% in the validation set]. Importantly, combination of the autoantibody panel and VCA-IgA improved diagnostic accuracy for NPC versus controls compared with the autoantibody panel alone [0.911 (0.881–0.940), 81.4%, and 90.0% in the training set; 0.919 (0.878–0.959), 78.9%, and 91.2% in the validation set), as well as for early-stage NPC (0.944 (0.894–0.994), 87.9%, and 94.0% in the training set; 0.922 (0.808–1.000), 80.0%, and 92.6% in the validation set]. These results reveal autoantibody signatures in an optimized panel that could improve the identification of VCA-IgA–negative NPC patients, may aid screening and diagnosis of NPC, especially when combined with VCA-IgA. Objective. To systematically evaluate the efficacy and safety of pembrolizumab (PD-1/PD-L inhibitor) and adjuvant chemotherapy to treat NSCLC and provide evidence-based reference for clinical use. Methods. By searching the Cochrane Library, EMBASE, PubMed, and Web of Science, according to the inclusion criteria, literature selection, data extraction, and quality evaluation were carried out for the included literature. The I2 test was used to evaluate heterogeneity between studies, and the meta-analysis was performed using RevMan 5.3 software provided by Cochrane. Results. Finally, 14 relevant documents meeting the standards were included. It is a statistical difference in one-year survival rate [ OR = 1.50 , 95% CI (1.28, 1.76), P < 0.00001 , I 2 = 0 %, Z = 4.99 ]; overall response rate[OR =1.57, 95% CI (1.29, 1.90), P < 0.00001 , I 2 = 0 %, Z = 4.58 ]; progression-free survival [ OR = 2.99 , 95% CI (2.29, 3.91), P < 0.00001 , I 2 = 26 %, Z = 8.00 ]; and overall survival [ OR = 1.38 , 95% CI (1.07, 1.78), P = 0.01 , I 2 = 46 %, Z = 2.50 ] and reduces the incidence of adverse drug reactions [ OR = 2.54 , 95% CI (1.99, 3.25), P < 0.00001 , I 2 = 69 %, Z = 7.43 ]. Conclusion. Pembrolizumab adjuvant chemotherapy is effective in the treatment of advanced NSCLC, but attention should be paid to the occurrence of adverse reactions in clinical. Due to the limitations of the methodology included in the study, this conclusion required more validation of large-sample RCT.

BACKGROUND: C-X-C chemokine receptor type 4 (CXCR4) has a certain effect on renal fibrosis, and there are few specific studies in cells. We want to investigate the impact of suppressing CXCR4 activity on the expression of renal fibrosis-related genes in primary glomerular endothelial cells, mesangial cells, and podocytes. METHODS: Immunofluorescence assays were used to determine the purity of isolated glomerular endothelial cells, mesangial cells, and podocytes. CXCR4 knockdown cell lines were established by transfecting the short hairpin (sh)RNA against CXCR4. T140 and AMD3100 were used to inhibit the activity of CXCR4. LY294002 was used to inhibit the activity of phosphoinositide 3-kinase (PI3K). The mRNA expression of CXCR4 was determined by real-time quantitative reverse transcription polymerase chain reaction (qRT-PCR). The protein expression of CXCR4, collagen IV, matrix metallopeptidase (MMP)-9, PI3K, Rac1, and vascular cell adhesion protein 1 (VCAM-1) was evaluated by Western blot analysis. RESULTS: High purity was observed on isolated primary glomerular endothelial cells and podocytes. However, the purity of isolated mesangial cells was relatively low. The mRNA expression of CXCR4 was significantly suppressed by the transfection of shRNA. Compared to control cells, the expression of CXCR4, collagen IV, MMP-9, PI3K, Rac1, and VCAM-1 were dramatically downregulated in cell lines transfected with shRNA against CXCR4. Furthermore, cell lines treated with T140, AMD3100, or LY294002 also showed downregulated expression of these proteins compared to untreated cells. No significant differences were observed in the protein expression of these proteins between control cells and cells transfected with the shRNA negative control (NC). CONCLUSIONS: Suppressing the activity of CXCR4 downregulated the expression of renal fibrosis-related genes in primary glomerular cells, even under a non-inflammatory state.
