Objective: evaluation of the clinical and economic efficiency of using Levilimab in the treatment of moderate and severe COVID-19 based on real world data (RWD).Material and methods. A single-center observational retrospective case-control study was performed. According to the matching algorithm, 834 pairs of patients with moderate and 347 pairs with severe infection were selected, similar in gender, age, vaccination status, severity of the disease and the level of C-reactive protein.Results. The clinical efficiency of Levilimab with respect to in-hospital mortality was demonstrated both for the moderate course (6% in the Levilimab group and 10% in the standard therapy group; odds ratio (OR) 1.71; 95% confidence interval (CI) 1.19–2.47; pConclusion. The use of Levilimab for the treatment of moderate and severe COVID-19 is feasible both from a clinical and economic points of view. Conducting RWD trials is an important tool to understand the effectiveness of medical technologies in real clinical practice.

DNA may be the most versatile molecule discovered to date. Beyond its well-known central role in genetics, DNA has the potential to be a remarkably usefultechnological material. It has been demonstrated as a scaffold for the assemblyof organic and inorganic nanomaterials [1]; a vehicle for drug delivery [2]; amedium for computation [3]; and a possible wire for transporting electricalsignals [4]. A key factor in exploiting DNA in these ways is the ability tointegrate DNA with other materials. In this paper, we review two approachesto forming DNA complexes with functional nanomaterials: (1) linking DNAwith single-wall carbon nanotubes (SWCNTs), which can then be used asnanoscale electrical contacts for probing electron transport in DNA; and (2)directed nanoassembly of Au nanoparticles using DNA/PNA (peptide nucleicacid) hybrid scaffolds.

Evans syndrome, a combination of autoimmune hemolytic anemia and immune thrombocytopenia, is a rare disease in children. In childhood, it may turn out to be one of the first manifestations of a primary immunodeficiency or an immune dysregulation syndrome. Here we present a clinical case of a patient who was initially diagnosed with Evans syndrome and did not respond well to therapy. Based on the results of genetic testing, the child was then diagnosed with primary immunodeficiency, namely, activated PI(3)kd syndrome. During follow-up, the patient developed lymphoma and had to undergo radical treatment (allogeneic hematopoietic stem cell transplantation). The patient's parents gave consent to the use of their child's data, including photographs, for research purposes and in publications.

Epithelioid hemangioendothelioma (EHE) is one of the rarest vascular tumor found in the brain. In the pediatric group of patients, the proportion of EHE is only 0.02% of all brain neoplasms, and therefore oncogenesis and the causes are currently not well understood. Intracranial localization of EHE is extremely rare, nevertheless, this type of neoplasm should be included in the differential diagnostic series when multiple small intracerebral masses with hypointense MR signal in T2 mode are detected on magnetic resonance imaging. No specific therapy is currently developed for this disease, the optimal treatment is surgical removal of the volumetric mass if this type of treatment is possible. This article presents a clinical case of a primary multiple CNS lesion in a 14-year-old child with a long history of epilepsy. According to our data, the presented clinical case is the only one registered in the world, with primary multiple CNS EHE of the brain in the pediatric population

Гамов зробив внесок у розвиток саме молекулярної біології, поставивши питання про розшифрування генетичного коду, тому необхідно у назву конференції додати – «molecular biology», щоб назва конференції виглядала наступним чином: «Аstronomy and Beyond: Astrophysics, Cosmology and Gravitation, Astroparticle physics, Rradioastronomy, Astrobiology and Molecular biology».

The association between rheumatoid arthritis (RA) and blood lipid levels has often been described as paradoxical, despite the strong association between RA and cardiovascular disease (CVD) risk. We aimed to clarify the genetic architecture that would explain the relationship between RA and blood-lipid levels, while considering inflammation as measured by C-reactive protein (CRP).Genome-wide association study (GWAS) summary statistics were collected from the CHARGE Consortium and Global Lipids Genetics Consortium. Blood-lipid levels includes HDL-C, LDL-C, triglycerides (TG), and total cholesterol (TC). Causality was examined by assessing Mendelian Randomization (MR) analysis. Pleiotropy, the identification of shared causal variants between traits, was assessed by conducting colocalization analyses.Using the MR Egger method, RA did not appear to causally predict alterations in lipid factors, rather the MR Egger intercept revealed that the genetic relationship between RA and HDL-C, LDL-C and TC may be explained by horizontal pleiotropy (p=0.003, 0.006, and 0.018, respectively). MR was suggestive of a horizontally pleiotropic relationship between CRP and lipid factors, while a causal relationship could not be ruled out. Recurring genes arising from shared causal genetic variants between RA and varying lipid factors included NAT2/PSD3, FADS2/FADS1, SH2B3, and YDJC.Horizontal pleiotropy appears to explain the genetic relationship between RA and blood-lipid levels. In addition, blood-lipid levels appear to suggest a horizontally pleiotropic relationship to CRP, if not mediated through RA as well. Consideration of the pleiotropic genes between RA and blood lipid levels may aid in enhancing diagnostic means to predict CVD.

Coronavirus disease (COVID-19) is an acute infectious disease of the respiratory tract caused by a new SARS-CoV-2 coronavirus. A global vaccination program against SARS-CoV-2 continues, and the incidence of COVID-19 worldwide is significantly decreasing. However, among millions of those who survived COVID-19, numerous groups will need assistance due to increased clinical consequences after COVID-19. Currently, there is a need to search for molecular biomarkers for monitoring the onset and progression of post-COVID syndrome. —This study describes a procedure for estimating the distribution of hydrocarbon resources by fields with different sizes of reserves and by the number of pools in each field. Such an evaluation is important in predicting the efficiency of exploration. The procedure first uses a simulation model for generating a population of hydrocarbon pool sizes. Then the spatial distribution of pools over a petroleum basin is modeled as a nonstationary Poisson point field. When we generate a field population in the basin, each field is considered a geometric object consisting of several pools according to the principle of their mutual geometric location. The proposed model was used to estimate a two-dimensional distribution of the number of fields and resources contained in them by resource sizes and the number of pools in each field. The proposed assessment was made for oil resources in the West Siberian petroleum province.

The Solidity programming language is the most widely used language for smart contract development. Improving smart contracts’ correctness, security, and performance has been the driving force for research in vulnerability detection, program analysis, and compiler techniques for Solidity. Similar to system-level languages such as C, Solidity enables the embedding of low-level code in programs, in the form of inline assembly code. Developers use inline assembly for low-level optimizations, extending the Solidity language through libraries, and using blockchain-specific opcodes only available through inline assembly. Nevertheless, inline assembly fragments are not well understood by an average developer and can introduce security threats as well as affect the optimizations that can be applied to programs by the compiler; it also significantly limits the effectiveness of source code static analyzers that operate on the Solidity level. A better understanding of how inline assembly is used in practice could in turn increase the performance, security, and support for inline assembly in Solidity. This paper presents a large-scale quantitative study of the use of inline assembly in 6.8M smart contracts deployed on the Ethereum blockchain. We find that 23% of the analyzed smart contracts contain inline assembly code, and that the use of inline assembly has become more widespread over time. We further performed a manual qualitative analysis for identifying usage patterns of inline assembly in Solidity smart contracts. Our findings are intended to help practitioners understand when they should use inline assembly and guide developers of Solidity tools in prioritizing which parts of inline assembly to implement first. Finally, the insights of this study could be used to enhance the Solidity language, improve the Solidity compiler, and to open up new research directions by driving future researchers to build appropriate methods and techniques for replacing inline assembly in Solidity programs when there is no real necessity to use it.

Drowning is one of the leading causes of death in the pediatric population. Patients arriving to the emergency department (ED) with submersion injuries are often asymptomatic and well-appearing, but can sometimes present critically ill and require prolonged resuscitation. The question of how long to continue resuscitation of a pediatric patient with a submersion injury is a difficult question to answer.We present a case of 6-year-old boy was found by his friends submerged in sea water for 10-15 min. The patient was rescued by lifeguards and evaluated by emergency medical personnel, who found him breathing spontaneously but unresponsive. En route to hospital, the patient became apneic, cardiopulmonary resuscitation (CPR) was started, and the patient was intubated. The patient arrived to the ED in cardiopulmonary arrest, CPR was continued and epinephrine was administered. Return of spontaneous circulation was achieved after 42 min in the ED. Initial laboratory test results showed severe acidosis and chest x-ray study showed diffuse interstitial edema. Ventilator settings were adjusted in accordance with lung protective ventilation strategies and the acidosis began to improve. Over the next several days, the patient was weaned to noninvasive ventilation modalities and eventually made a complete neurologic recovery and continued to be a straight-A student. Why Should an Emergency Physician Be Aware of This?We make the case that, in select drowning patients, duration of CPR longer than 30 min can potentially result in favorable neurologic outcomes. Prolonged CPR should be especially strongly considered in patients with a pulse at any point during evaluation. With the combination of prolonged CPR and judicious use of lung protective mechanical ventilation strategies, we were able to successfully treat the patient in our case.

The authors analyzed the problem of diuretic resistance (DR) in patients with chronic heart failure (CHF). Most of the symptoms and signs of CHF are associated with hypervolemia and vascular congestion in the systemic and pulmonary circulation. The severity of the latter is the main factor which negatively affects the overall assessment of life satisfaction in patients with CHF. Since the patient, even at the incurable stage of CHF, primarily expects a rapid decrease in the severity of manifestations of decompensation from the prescribed therapy, achieving euvolemia is the essence of its short-term objective. Without diuretics, these immediate effects, according to which most CHF patients judge the qualifications of the doctor, are almost impossible to achieve. Unfortunately, apparently, not a single clinician was able to avoid disappointment in the effectiveness of CHF therapy associated with DR in their practice. As a rule, DR reflects the progressive course of CHF and is often associated with a poor prognosis. The review consistently covers the issues of terminology, diagnosis, pathogenesis, and prevention of DR, which aggravates CHF, and discusses measures aimed at restoring sensitivity to diuretics.

Mutations in the TP53 tumor suppressor gene are common in many cancer types, including the acute myeloid leukemia (AML) subtype known as complex karyotype AML (CK-AML). Here, we identify a gain-of-function (GOF) Trp53 mutation that accelerates CK-AML initiation beyond p53 loss and, surprisingly, is required for disease maintenance. The Trp53R172H mutation (TP53R175H in humans) exhibits a neomorphic function by promoting aberrant self-renewal in leukemic cells, a phenotype that is present in hematopoietic stem and progenitor cells (HSPC) even prior to their transformation. We identify FOXH1 as a critical mediator of mutant p53 function that binds to and regulates stem cell–associated genes and transcriptional programs. Our results identify a context where mutant p53 acts as a bona fide oncogene that contributes to the pathogenesis of CK-AML and suggests a common biological theme for TP53 GOF in cancer.Significance:Our study demonstrates how a GOF p53 mutant can hijack an embryonic transcription factor to promote aberrant self-renewal. In this context, mutant Trp53 functions as an oncogene to both initiate and sustain myeloid leukemia and suggests a potential convergent activity of mutant Trp53 across cancer types.This article is highlighted in the In This Issue feature, p. 813

Metabolic syndrome (MetS) is diagnosed by the presence of high scores on three or more metabolic traits, including systolic and diastolic blood pressure (SBP, DBP), glucose and insulin levels, cholesterol and triglyceride (TG) levels, and central obesity. A diagnosis of MetS is associated with increased risk of cardiovascular disease and type 2 diabetes. The components of MetS have long been demonstrated to have substantial genetic components, but their genetic overlap is less well understood. The present paper takes a multi-prong approach to examining the extent of this genetic overlap. This includes the quantitative genetic and additive Bayesian network modeling of the large TwinsUK project and examination of the results of genome-wide association study (GWAS) of UK Biobank data through use of LD score regression and examination of the number of genes and pathways identified in the GWASes which overlap across MetS traits. Results demonstrate a modest genetic overlap, and the genetic correlations obtained from TwinsUK and UK Biobank are nearly identical. However, these correlations imply more genetic dissimilarity than similarity. Furthermore, examination of the extent of overlap in significant GWAS hits, both at the gene and pathway level, again demonstrates only modest but significant genetic overlap. This lends support to the idea that in clinical treatment of MetS, treating each of the components individually may be an important way to address MetS.

Explaining why a database query result is obtained is an essential task towards the goal of Explainable AI, especially nowadays where expressive database query languages such as Datalog play a critical role in the development of ontology-based applications. A standard way of explaining a query result is the so-called why-provenance, which essentially provides information about the witnesses to a query result in the form of subsets of the input database that are sufficient to derive that result. To our surprise, despite the fact that the notion of why-provenance for Datalog queries has been around for decades and intensively studied, its computational complexity remains unexplored. The goal of this work is to fill this apparent gap in the why-provenance literature. Towards this end, we pinpoint the data complexity of why-provenance for Datalog queries and key subclasses thereof. The takeaway of our work is that why-provenance for recursive queries, even if the recursion is limited to be linear, is an intractable problem, whereas for non-recursive queries is highly tractable. Having said that, we experimentally confirm, by exploiting SAT solvers, that making why-provenance for (recursive) Datalog queries work in practice is not an unrealistic goal.

In recent years, decentralized finance (DeFi) has experienced remarkable growth, with various protocols such as lending protocols and automated market makers (AMMs) emerging. Traditionally, these protocols employ off-chain governance, where token holders vote to modify parameters. However, manual parameter adjustment, often conducted by the protocol's core team, is vulnerable to collusion, compromising the integrity and security of the system. Furthermore, purely deterministic, algorithm-based approaches may expose the protocol to novel exploits and attacks. In this paper, we present "Auto.gov", a learning-based on-chain governance framework for DeFi that enhances security and reduces susceptibility to attacks. Our model leverages a deep Q- network (DQN) reinforcement learning approach to propose semi-automated, intuitive governance proposals with quantitative justifications. This methodology enables the system to efficiently adapt to and mitigate the negative impact of malicious behaviors, such as price oracle attacks, more effectively than benchmark models. Our evaluation demonstrates that Auto.gov offers a more reactive, objective, efficient, and resilient solution compared to existing manual processes, thereby significantly bolstering the security and, ultimately, enhancing the profitability of DeFi protocols.

Autoregressive (AR) encoder-decoder neural networks have proved successful in many NLP problems, including Semantic Parsing -- a task that translates natural language to machine-readable parse trees. However, the sequential prediction process of AR models can be slow. To accelerate AR for semantic parsing, we introduce a new technique called TreePiece that tokenizes a parse tree into subtrees and generates one subtree per decoding step. On TopV2 benchmark, TreePiece shows 4.6 times faster decoding speed than standard AR, and comparable speed but significantly higher accuracy compared to Non-Autoregressive (NAR)., Comment: 4 pages main body + 4 pages appendices

In Federated Learning (FL), accessing private client data incurs communication and privacy costs. As a result, FL deployments commonly prefinetune pretrained foundation models on a (large, possibly public) dataset that is held by the central server; they then FL-finetune the model on a private, federated dataset held by clients. Evaluating prefinetuning dataset quality reliably and privately is therefore of high importance. To this end, we propose FreD (Federated Private Fr\'echet Distance) -- a privately computed distance between a prefinetuning dataset and federated datasets. Intuitively, it privately computes and compares a Fr\'echet distance between embeddings generated by a large language model on both the central (public) dataset and the federated private client data. To make this computation privacy-preserving, we use distributed, differentially-private mean and covariance estimators. We show empirically that FreD accurately predicts the best prefinetuning dataset at minimal privacy cost. Altogether, using FreD we demonstrate a proof-of-concept for a new approach in private FL training: (1) customize a prefinetuning dataset to better match user data (2) prefinetune (3) perform FL-finetuning.

The use of blockchains for automated and adversarial trading has become commonplace. However, due to the transparent nature of blockchains, an adversary is able to observe any pending, not-yet-mined transactions, along with their execution logic. This transparency further enables a new type of adversary, which copies and front-runs profitable pending transactions in real-time, yielding significant financial gains. Shedding light on such "copy-paste" malpractice, this paper introduces the Blockchain Imitation Game and proposes a generalized imitation attack methodology called Ape. Leveraging dynamic program analysis techniques, Ape supports the automatic synthesis of adversarial smart contracts. Over a timeframe of one year (1st of August, 2021 to 31st of July, 2022), Ape could have yielded 148.96M USD in profit on Ethereum, and 42.70M USD on BNB Smart Chain (BSC). Not only as a malicious attack, we further show the potential of transaction and contract imitation as a defensive strategy. Within one year, we find that Ape could have successfully imitated 13 and 22 known Decentralized Finance (DeFi) attacks on Ethereum and BSC, respectively. Our findings suggest that blockchain validators can imitate attacks in real-time to prevent intrusions in DeFi.

The use of polymers in the agro-industrial complex has increased over the past decades. This is due to their use as structural materials in equipment for processing, storage and preparation of agricultural products. The article presents the overview of the state of existing control systems for the process of predicted polymerization, a literary and patent analysis of the presented mathematical models of chemical polymerization reactions. There are considered the methods of control of the processes of design, manufacture, modeling and construction of automated control systems. (Research purpose) The research purpose is to determine the promising directions for the development and research of the designing materials and products made of photopolymers with predetermined structural properties. (Materials and methods) The article presents software complexes for modeling polymer materials, algorithms and mathematical models of polymerization. Authors used a literary and patent review in the study. (Results and discussion) The analyze of the program complexes for modeling polymer materials shows that in an explicit, visualized form, the process of creating photopolymers with specified design parameters is not represented in them. The analysis of a typical technological process as an object of management was carried out using the example algorithm of a specific production unit, and have established the possibility of improving existing automated control systems for it. The article describes the possibility of constructing an automated control system for the technological process of photopolymerization, stabilization of physico-chemical processes in the production of polymer products. (Conclusions) The actual tasks of further research is creating a software package for modeling materials with specified structural properties; mathematical apparatus for calculating and modeling photopolymerization processes; construction of automated control systems for technological processes of manufacturing products from photopolymer structural materials.

The threat of antibiotic-resistant bacteria is an ever-increasing problem in public health. In this report, we examine the photochemical properties with a proof-of-principle biocidal assay for a novel series of regio-regular imidazolium derivative poly-(3-hexylthiophene)/sodium dodecyl sulfate (P3HT-Im/SDS) materials from ultrafast sub-ps dynamics to μs generation of reactive oxygen species (ROS) and 30 min biocidal reactivity with Escherichia coli (E. coli). This broad series encompassing pure P3HT-Im to cationic, neutral, and anionic P3HT-Im/SDS materials are all interrogated by a variety of techniques to characterize the physical material structure, electronic structure, and antimicrobial activity. Our results show that SDS complexation with P3HT-Im results in aggregate materials with reduced ROS generation and light-induced anti-microbial activity. However, our characterization reveals that the presence of non-aggregated or lightly SDS-covered polymer segments is still capable of ROS generation. Full encapsulation of the P3HT-Im polymer completely deactivates the light killing pathway. High SDS concentrations, near and above critical micelle concentration, further deactivate all anti-microbial activity (light and dark) even though the P3HT-Im regains its electronic properties to generate ROS.

The article analyzes the role of microcirculatory disorders in the pathogenesis of inflammatory complications (peri-implantitis) in patients with orthopedic constructions on dental implants and substantiates the possibility of a complex course application of low-intensity infrared laser therapy and low-frequency electrostatic therapy. It was shown that the initial state of patients with peri-implantitis is characterized by significantly low values of the level of general perfusion and its intensity, as well as the predominance of shunt mechanisms that reduce the values of the nutritive link of blood flow. The cause of the revealed disorders is the low level of functioning of the vasodilation mechanisms, caused by inflammatory and destructive processes in the peri-implant tissues. The use of a complex of physiotherapeutic exposure was characterized by a more pronounced corrective effect on the parameters of microcirculation and oxidative metabolism in comparison with standard dental treatment. The observed dynamics of microcirculatory systems of tissues of the peri-implant zone under the conditions of the complex influence of physiotherapeutic factors is aimed at the formation of adaptive reactions of local hemodynamics to the local needs of soft and bone tissues experiencing an inflammatory process around the implant, as well as strengthening the mechanisms of long-term adaptation of trophic relations in tissue peri-implant microregions. Taking into account the pathogenetic significance of microhemodynamic disorders in the formation of clinical manifestations of peri-implantitis, the revealed activating effect of the technology of the combined use of a laser and low-frequency electrostatic therapy serves as an objective proof of its therapeutic efficacy.

A comparative analysis of symptoms in arterial ischemic and stroke-like conditions in children, the development and analysis of the «Presumptive stroke in children» scale for prehospital diagnosis of ischemic strokes in children.This article analyzes cases of emergency hospitalization of children in a stroke center with suspected cerebrovascular pathology as well as the symptoms of patients with suspected ischemic stroke, compares the leading and final diagnoses, identifies the main «mimics» of stroke and their symptom complexes. A comparison of the obtained data with the results of previous studies was carried out.The results show the prevalence of motor disorders and coordination disorders in cases of ischemic stroke and cerebral symptoms in cases of stroke-like conditions in children. The most frequent stroke-mimic is migraine. Based on the results of the study, we propose a screening scale for pre-hospital diagnosis of ischemic stroke in children. A preliminary assessment of the effectiveness of the scale using examples of cases of confirmed stroke and stroke-mimic was performed.Timely hospitalization of children with stroke can not only reduce mortality, but also use modern diagnostic and treatment methods to reduce the volume of brain damage with a possible complete restoration of motor and cognitive functions. Further analysis of the sensitivity and specificity of the «Presumptive stroke in children» scale is required.Сравнительный анализ симптомов при артериальном ишемическом инсульте (АИИ) и инсультоподобных состояниях у детей, разработка и анализ шкалы предполагаемого инсульта у детей (Шкала) для догоспитальной диагностики АИИ у детей.Проанализированы случаи госпитализации детей скорой медицинской помощью в Центр инсульта с подозрением на цереброваскулярную патологию. Проведен анализ симптомов, выявляемых у больных с подозрением на АИИ, сравнение направляющего и окончательного диагнозов, определение основных «масок» инсульта и их симптомокомплексов. Проведено сравнение данных с результатами, полученными ранее в исследованиях.Результаты исследования позволяют говорить о превалировании двигательных и координаторных нарушений при АИИ и общемозговых симптомов при инсультоподобных состояниях у детей. Наиболее часто встречающейся «маской инсульта» является мигрень. На основании результатов исследования предложена Шкала, проведена предварительная оценка эффективности Шкалы на примерах подтвержденных случаев АИИ и «масок инсульта».Своевременная госпитализация детей с АИИ может не только уменьшить летальность, но и использовать современные методы диагностики и лечения для уменьшения объема поражения головного мозга с возможным полным восстановлением двигательных и когнитивных функций. Необходим дальнейший анализ чувствительности и специфичности предложенной Шкалы.

Single-molecule techniques provide important details supplementing the framework obtained from traditional bulk experiments. Many cellular processes such as DNA replication, DNA repair, and telomere maintenance involve interaction among multiple proteins, their co-factors, and DNA. To investigate such interactions and to differentiate the function of each component necessitate a technique that allows the combinatorial exposure of DNA to multiple proteins and co-factors as well as manipulation of the DNA configuration. We present a nanofluidic device with the capability of active combinatorial exchange of up to three buffers in real-time and dynamic manipulation of DNA under physiologically relevant conditions. We demonstrate its utility in monitoring compaction of DNA by telomeric proteins, DNA modification by an endonuclease, and DNA loop extrusion by cohesin.
