RESUMEN Introducción: Las enfermedades cardiovasculares, son la principal causa de muerte a nivel mundial, su aparición está influida por factores psicosociales uno de ellos es el estrés. Objetivo: Analizar la relación que existe entre el nivel de estrés laboral y la percepción de síntomas de enfermedades cardiovasculares en trabajadores de la Región de Ñuble, en Chile. Material y Métodos: Estudio analítico de corte transversal, desarrollado a través de encuestas on line (antecedentes sociodemográficos, laborales, de salud y nivel de estrés laboral) a trabajadores de la región de Ñuble. Resultados: De los 260 trabajadores evaluados, el 57,3% reportó síntomas de enfermedad cardiovascular, que se relacionan al nivel de estrés de los trabajadores y también al sexo, edad, nivel de estudios, rubro laboral y percepción de carga laboral excesiva. Conclusiones: Este estudio concluye que la percepción de síntomas de enfermedad cardiovascular, esta relacionada al nivel y gravedad del estrés laboral .

En Chile, el cancer infantil ocupa el 2° lugar en mortalidad en ninos mayores de 5 anos. Dentro del cancer infantil, el mas incidente es la Leucemia Linfoblastica Aguda. En relacion al tratamiento de la Leucemia, el Trasplante de Medula Osea puede ser indicacion en determinados casos de Leucemia como ultima opcion terapeutica. Al trabajar en el area de la salud, podemos atender a ninos con cancer con condiciones especiales. Dentro de ellos, tenemos a los ninos con Sindrome de Down (SD), que por ser portadores de la Trisomia 21, tienen mayor riesgo de desarrollar Leucemia. En el siguiente articulo analizaremos el caso de una adolescente con Sindrome de Down, que al tener una Leucemia de alto riesgo debera ser trasplantada de medula osea, este procedimiento es de alto riesgo, unica opcion curativa para la paciente. Por ser el trasplante un tratamiento que puede conllevar graves complicaciones y ser los ninos con Sindrome de Down particularmente fragiles al tratamiento previo al trasplante, analizaremos el caso desde una perspectiva bioetica en base a un modelo de toma de decisiones en salud del Hastings Center de Filadelfia, a partir del siguiente problema etico: Dadas las consecuencias nocivas que podria conllevar el trasplante de medula para la paciente, considerando su condicion de Sindrome de Down, la paciente tendra una mejor calidad de vida postrasplante? ?es eticamente correcto no trasplantar a la paciente?

Summary Background Patients with chronic obstructive pulmonary disease (COPD) have few options for treatment. The efficacy and safety of the phosphodiesterase-4 inhibitor roflumilast have been investigated in studies of patients with moderate-to-severe COPD, but not in those concomitantly treated with longacting inhaled bronchodilators. The effect of roflumilast on lung function in patients with COPD that is moderate to severe who are already being treated with salmeterol or tiotropium was investigated. Methods In two double-blind, multicentre studies done in an outpatient setting, after a 4-week run-in, patients older than 40 years with moderate-to-severe COPD were randomly assigned to oral roflumilast 500 μg or placebo once a day for 24 weeks, in addition to salmeterol (M2-127 study) or tiotropium (M2-128 study). The primary endpoint was change in prebronchodilator forced expiratory volume in 1 s (FEV 1 ). Analysis was by intention to treat. The studies are registered with ClinicalTrials.gov, number NCT00313209 for M2-127, and NCT00424268 for M2-128. Findings In the salmeterol plus roflumilast trial, 466 patients were assigned to and treated with roflumilast and 467 with placebo; in the tiotropium plus roflumilast trial, 371 patients were assigned to and treated with roflumilast and 372 with placebo. Compared with placebo, roflumilast consistently improved mean prebronchodilator FEV 1 by 49 mL (p 1 was noted in both groups. Furthermore, roflumilast had beneficial effects on other lung function measurements and on selected patient-reported outcomes in both groups. Nausea, diarrhoea, weight loss, and, to a lesser extent, headache were more frequent in patients in the roflumilast groups. These adverse events were associated with increased patient withdrawal. Interpretation Roflumilast improves lung function in patients with COPD treated with salmeterol or tiotropium, and could become an important treatment for these patients. Funding Nycomed.

A secondary hyperparathyroidism resulting from decreased intestinal calcium (Ca) absorption has been proposed as a contributory factor to glucocorticoid-induced osteoporosis. Inhaled steroids do not usually suppress adrenal gland function unless daily doses above 1,500 microgram are used. A recent study, however, has shown a reduced total body calcium in patients on regular beclomethasone treatment. In theory, osteopenia in these patients could be due to a direct effect of inhaled steroids on bone or due to an impaired intestinal calcium absorption. In this study, Ca absorption and parathyroid hormone (PTH) secretion were evaluated in three groups: 1) asthmatics on continuous oral and inhaled steroid treatment (11.3 +/- 4.4, range 5-33.5 mg.day-1 prednisone and 660 +/- 265, range 400-1,600 microgram.day-1 beclomethasone, respectively); 2) asthmatics on regular beclomethasone therapy (585 +/- 210, range 400-1,200 microgram.day-1); and 3) healthy subjects. The prevalence of vertebral fractures was evaluated by a spinal X-ray. No differences were found in either Ca absorption or PTH serum levels between asthmatics and healthy subjects (analysis of variance-ANOVA). Vertebral fractures were significantly more frequent in patients from group 1 (14 of 25) than in those from group 2 (2 or 25). We conclude that both prolonged oral steroid treatment and inhaled steroids, at doses lower than 1,600 microgram.day-1 do not cause Ca malabsorption, and that hyperparathyroidism does not contribute to osteoporosis in these patients.

BACKGROUND Hepatitis B comprises one of the major health problems worldwide. One of the measures for the prevention thereof is the massive vaccination of adolescents. The purpose of our study is that of assessing the degree of coverage of the systematic hepatitis B prevention campaign among the 8th-grade population in the province of Guadalajara in addition to studying the immune response of said population, relating said response to other variables. METHODS Cross-sectional study. The degree of coverage of the vaccination campaign was studied throughout the entire target population in the province in question. For the study of the immune response, a sample of 338 students was taken by systematic random sampling. The titres of anti-HB's were quantified 6 months following the third dosage, levels of 10 mU/ml or above being considered as being levels providing protection. RESULTS The percentage of students properly vaccinated totaled 82.7%. The overall serum protection rate was 97.5%, and 46.7% of the sample responded showing titres of over 100 mU/ml. A statistically significant inverse relationship (p < 0.001) existed between the level of anti-HB's and the Quetelet level, we not having found any significant differences between the two sexes on comparing the postvaccination titres of anti-HB's. CONCLUSIONS We consider the degree of coverage provided to be satisfactory and the immunogenic aspect of the vaccine to be good. Obesity is a factor providing a prior indication of a minor response to the same. This response is not influenced by the subject's sex.

We assess the attitudes of Spanish doctors towards immigrant patients.cross-sectional descriptive study carried out in January-June 2003.Family doctors and third-year residents of 15 health care centers in Zaragoza, Spain.specifically designed interview answered in anonymously and on a voluntary basis.A total of 62.3 % of the sample responded. Nearly 75 % of the doctors had received training on immigration, mainly from a theoretical point of view. Almost 25 % of immigrant consultations are not related to health matters. Doctors think that immigrant patients somatize more than Spanish ones but they do not suffer from more psychiatric illness. They also think immigrants show less treatment compliance and are more demanding. They disagree about possible racial biases in diagnosis or treatment of these patients. Spanish doctors consider that the different ethnic groups present similar management difficulties. The group with the most negative stereotype was the patients from Muslim North African countries.These findings are discussed from a psychological and anthropological perspective.

This article summarizes the main results and conclusions presented in the Symposium "Nutritional supplementation: evidences and experiences" that took place in the XXIIIrd SENPE Congress (2008). Protein energy malnutrition, that can affect 30-50% of hospitalized patients, increases both time of hospitalization and costs of medical care of this kind of patients. There is a lot of scientific evidences demonstrating that the use of nutritional supplementation improves nutritional status or prevents malnutrition in those patients who do not meet their nutritional needs with a conventional diet or an adapted one with no replacing intake from normal food. This is strengthened by the results that demonstrate the rol of nutritional supplements improving nutritional and functional parameters. Current bibliographic reviews focused on certain clinical frameworks (i.g. geriatrics, oncology), prove that nutritional supplements reduce complications related to pathology and to nutritional status, and also reduce length of hospitalization and mortality. More studies regarding to efficacy of oral nutritional supplements are needed. These studies should be carried out with a period of follow-up longer than the current published studies have. As well as effective, nutritional supplements become a save therapeutic intervention with no important adverse events that, according to bibliography, improve patient's functionality and quality of life. It is worth mentioning that nutritional supplements can be effective on certain kind of patients, for instance, malnourished elderly or elderly in risk of malnourishment, and hospitalized surgical patients. Scientific literature refers that it is necessary to carry out more studies, with an accurate methodology, which assess the effect of nutritional supplements on quality of life and its cost-effectiveness on malnourished patients regarding specific clinical situations. That would allow physicians to make clinical decisions based on evidences and cost analysis. Yes El presente artículo resume los resultados y conclusiones presentados en el Simposio “Suplementación nutricional: evidencias y experiencias” celebrado en el XXIII Congreso Nacional de la SENPE (2008). La malnutrición calórico-proteica, que puede afectar al 30-50% de los pacientes hospitalizados, aumenta el tiempo de hospitalización y el coste de la atención médica en aquellos que la padecen. Un importante volumen de evidencias científicas avala que la administración de suplementos nutricionales (SN) mejora el estado nutricional o previene la aparición de malnutrición en pacientes que no cubren sus necesidades nutricionales con la dieta convencional o con la dieta adaptada, sin que, por ello, se produzca desplazamiento de la ingesta. Todo ello viene reforzado por los resultados que evidencian el papel de los SN en la mejora de parámetros tanto nutricionales como funcionales. La revisión de la literatura existente en determinados contextos clínicos (geriatría u oncología), evidencia que los SN reducen la aparición de complicaciones propias, tanto de la patología de base como del estado de desnutrición, así como que favorecen la reducción de la estancia hospitalaria y de la mortalidad. A pesar de ello, son necesarios más estudios sobre la eficacia de los SN orales en los que se realice un seguimiento más prolongado de lo que ofrecen las publicaciones disponibles actualmente. Más allá de su eficacia, los SN constituyen una intervención terapéutica segura y sin efectos adversos clínicos relevantes que, según la literatura, mejoran la funcionalidad del paciente y su calidad de vida. Cabe añadir que los SN pueden ser coste-efectivos en ciertos perfiles de paciente (ancianos malnutridos o en riesgo de desnutrición y en pacientes quirúrgicos hospitalizados). La revisión de la literatura evidencia la necesidad de realizar más estudios, con la metodología adecuada, que valoren el efecto sobre la calidad de vida de los SN así como su coste-efectividad sobre pacientes desnutridos en el contexto de situaciones clínicas concretas. Ello permitiría al facultativo la toma de decisiones clínicas basadas en la evidencia y los análisis de costes.
