The instability and uncertainty of international relations accentuate the role of new and inclusive platforms of interaction and cooperation. Eurasian Economic Union (EEU) alongside its regional and transregional partners represent an example of such a platform based on new principles and values, an element of international relation configuration.Aim. Assess the prospects for a zone (cluster) within the planned and potential transregional EEU’s expansion beyond the CIS borders.Task. To evaluate the dynamics of the consolidation concerning, first, current free trade EEU agreements (FTAs or the EEU FTAs) with Serbia, Iran, Vietnam and Singapore, second, potential agreements with listed in the EEU official strategy, third, other developing states with relevant economic and/or demographic potential.Methods. The authors use additional consolidation (share of intra-bloc trade) as a metric. It is calculated as the difference between the share of EEU intra-trade and within the existing or potential EEU FTA partnerships.Results. Based on the assessment of additional consolidation, the research revealed that the existing EEU FTAs with Iran and Vietnam are effective, while trade integration with Serbia, Singapore and China are ineffective. The presence of additional consolidation was revealed for Turkey, Egypt, and Bangladesh.Conclusions. The implementation of the transregional integration track is an important development step for the Eurasian Economic Union. However, trade partnerships have different trade efficiency.

The difficulties of diagnosis and management of patients with alcohol-induced heart lesions, features of the clinic of alcoholic cardiomyopathy are highlighted. Excessive and prolonged alcohol consumption increases the risk of developing acute and chronic heart failure, cardiac arrhythmias and aggravates existing cardiovascular diseases. At the same time, due to insufficient assessment of the origin of cardiac manifestations (patients often hide or downplay the fact of alcohol abuse), patients do not always receive specific treatment. The management of such patients presents significant difficulties, taking into account, among other things, the defeat of the gastrointestinal tract, central and peripheral nervous system and should be carried out jointly with doctors of other specialties.

At present, comorbidity is one of the current interdisciplinary problems of clinical medicine. At the same time, in the modern literature the number of teaching and study guides, monographs and other scientific publications reflecting the essence of comorbidity and its prognostic significance for the patient, is clearly inadequate. Also, no national clinical guidelines exist for managing patients with comorbid diseases. The aim of the review is to draw practical physicians’ attention to the importance of timely detection of comorbid nature of the internal disease necessary for the correct choice of therapeutic measures for patients suffering from multiple diseases, early prevention of possible complications and the formation of a favorable prognosis. Symptomatic gastroduodenal ulcer lesions were chosen as one of the examples of comorbid pathology, late diagnosis and absence of early prevention of which can become the causes of gastrointestinal bleeding and related deaths in patients. The article reflects the definition of comorbidity, existing synonyms, its main types, and methods of evaluation. The causes and factors of the development of comorbid diseases, their prevalence in the population are analyzed. Attention is focused on the rules for formulation of clinical diagnosis for a comorbid patient, the need to take into account comorbidity when choosing an algorithm for integrated diagnosis and rational treatment of the disease. The importance of assessing the risk of drug interactions in the pharmacotherapy of comorbid diseases and possible complications are emphasized. The epidemiology of symptomatic gastroduodenal ulcers, their pathogenesis, common clinical presentation and its features depending on a specific type, complexity of diagnosis are analyzed in detail.

Periodic disease or Familial Mediterranean fever is an inherited monogenic disease widespread among the ancient peoples of the countries of Mediterranean region: Armenians, Jews, Turks, Arabs. MEFV gene (Familian Mediterranean Fever Gene) located on chromosome 16 in the region 16р13.3, is responsible for the development of the disease. MEFV gene encodes marenostrin protein (Pirin) involved in the regulation of inflammatory processes. Typical manifestations of the disease are recurrent fever, peritonitis, synovitis, pleuritis, and rarely pericarditis, meningitis, erysipeloid skin lesions, development of amyloidosis, mainly kidneys, is also characteristic. In the described clinical case the disease has manifested as recurrent interstitial lung disease and morphea-like skin lesion. Over 5 years the lung pathology was considered as cryptogenic organizing pneumonia, so the patient was administered corticosteroids (with no effect). In 2013, molecular genetic study detected mutations V726A and A744S in compaud-heterozygous state, the diagnosis of periodic disease was confirmed. Receiving colchicine for over 3.5 years induced remission of the disease: there were no episodes of febrile fever and exacerbations of pneumonia, radiologic regression of lung lesions and elimination of morphea-like skin lesion was observed. Clinical symptoms of periodic disease in this case are rarely encountered.

Study purpose - to assess clinical and hemodynamic parameters in patients with ischemic heart disease (IHD) with and without use of metabolic therapy with trimetazidine MB. MATERIALS Seventy four patients with IHD, 40 of whom in addition to the main therapy for 3 months received metabolic preparation trimetazidine MB. RESULTS Number of anginal attacks and inhalations of nitroglycerin significantly decreased and parameters of hemodynamics improved after 3 months in patients who received trimetazidine MB. CONCLUSION Long-term use of trimetazidine MB in patients with IHD is advisable.

The paper deals with the data on biology of Aerococcus, a slightly studied group of microorganisms. Physiological-biochemical properties of Aerococcus are described, data of their distribution in nature are given. Peculiar attention is paid to the estimate of the role of Aerococcus in human microbiocenoses. As a result of the profound and all-round study of this group of microorganisms the authors have developed new bacterial drug "A-bakterin" based on the aerococcus strain. Data presented about the results of clinical tests of "A-bakterin" are presented, a possibility to use Aerococcus lysate in the elaboration of new drugs is discussed.

On the basis of experiments carried out in the course of this study the conclusion can be made that one of the mechanisms of the preventive effect of aerococci, the representatives of the indigenous microflora of the gastrointestinal tract, lies in their capacity for adhesion to and colonization of the mucous membrane, this enhancing the resistance of the enteric tract to infections. These properties of anterococci substantiate good prospects of using M-bacterin (live lyophilized aerococcal culture) for the control of enteric infections and dysbacteriosis.

P. aeruginosa wound infection was induced in white mice to test new preparations against P. aeruginosa. This model ensures the nearest approximation to the course of P. aeruginosa chronic infection, i.e. it reproduces the focus of inflammation and the prolonged course of the disease (the positive decision on application No. 4, 324, 555 of November 2, 1987, has been obtained). The essence of the method consists in obtaining the model of P. aeruginosa wound infection by a combined trauma of the skin (burn and incision): P. aeruginosa is introduced in a dose of 6 X 10(9)-8 X 10(9) microbial cells into the burn blister through the incision made 3 hours after the burn and then 20-24 hours later in a dose of 10(9)-2 X 10(9) microbial cells, introduced under the crust formed by that time.

Representatives of the normal microflora from genus Aerococcus, in particular strain Aerococcus viridans 167 isolated from breast milk are studied for their effect on biological properties of Staphylococcus aureus in vitro and in vivo. It is established that the number of viable cells of the staphylococcus cultivated in the presence of antagonists in the beef-extract agar decreases progressively with each following passage, the population dying after the seventh-eight passage. Electronograms fix deep changes in the cell ultrastructure. A degree of changes in biological properties depends on the duration of the antagonist action. The results obtained reveal one of the mechanisms of the antagonistic action of aerococci-antagonists producing hydrogen peroxide.
