A man in his mid-20s presented with a painless swelling over his right thigh, which had been progressively increasing over 3 years. He underwent an excisional biopsy for the same, which showed reactive lymphadenopathy. Since the last year and a half, he developed a lower abdominal wall swelling with mild redness over it. In addition, over the last few months before presentation to haematology outpatient clinic, he experienced bouts of fever, night sweats, anorexia, weight loss and right inguinal lymphadenopathy. On examination, splenomegaly was identified. In view of the patients’ symptoms, he underwent a positron emission tomography scan, which showed hypermetabolic activity in the subcutaneous tissue sparing the lymph nodes and spleen. A deep skin punch biopsy taken from his right thigh was consistent with the diagnosis of subcutaneous panniculitis-like T-cell lymphoma αβ T-cell phenotype. The patient was treated successfully with oral steroids and on routine follow-up, he is in remission for 5 years.

Background & objective: Inter-scalene brachial plexus block (ISB) is the gold standard for postoperative pain management in shoulder surgery. Although, this method has its side-effects and possibly complications. Supra-scapular nerve block and axillary nerve block have also been used in upper limb procedures. We compared ISB with the blockade of supra-scapular and axillary nerves (called shoulder block) for postoperative analgesia after shoulder arthroscopic surgical operation under ultrasound guidance (USG) and nerve stimulators. Methodology: It was a prospective, randomized, comparative study. Results: The VAS pain scores at different times postoperatively were not significantly different between the ISB and ShB groups (P = t 0.071, 0.28, 0.378, 0.358, 0.451 at 2, 4, 8, 16, and 24 h respectively. VAS 0 was significantly difference (P = 0.029) but still the VAS score was less than 3, so no pain killers were given. Conclusion: Ultrasound guided supra-scapular and axillary nerve blocks ae equally effective as inter-scalene brachial plexus block for postoperative analgesia in shoulder arthroscopic surgery with less side-effects. Abstract: New coumarin chalcones 3j–p were conveniently obtained in high yields via Claisen-Schmidt condensation reaction, when acetyl coumarin 1 reacted with 3-aryl-1-phenyl pyrazole-4-carbaldehydes 2j–p in boiling ethanol in the presence of triethyl amine as a catalyst. Also, two synthetic pathways were afforded for the synthesis of novel tetrazolo[1,5-a]pyrimidinyl-2H-chromen-2-ones 5a-p. The first pathway is a multistep process including formation and separation of chalcones, which then were allowed to react with 5-aminotetrazole 4. While, the second pathway is a highly efficient one-pot three-component condensation reaction of 3-acetyl coumarin 1, aromatic aldehydes 2a-p and 5-aminotetrazole 4 under green and mild reaction conditions by using acetic acid (AcOH) as a catalyst and solvent. The molecular structure of products was established on the basis of their NMRs, IR and elemental analysis data. Solvent optimization was carried out in the reaction producing 3-(5-Phenyl-4,5-dihydrotetrazolo[1,5-a]pyrimidin-7-yl)-2H-chromen-2-one (5a). Background: Background: Acute myocardial infarction (AMI) pathophysiology is mediated by systemic, intraplaque myocardial inflammatory processes that occur mainly due to coronary artery thrombosis in an atherosclerotic plaque area. The G-protein-coupled chemokine receptor (Ccr6) is displayed on the surface of many types of leukocytes, that have been found in atherosclerotic plaques. It is a novel mediator of inflammation and immune response. Objectives: To determine CCR6 lymphocyte expression in AMI patients and its association with disease severity using the Gensini scoring system. Methods: 25 AMI patients and 25 controls underwent flow cytometry to determine the percentage of circulating CCR6+ lymphocytes. To forecast AMI and determine how CCR6 expression relates to it, multivariate logistic regression analysis was used. Results and Discussion: There was a higher percentage of CCR6+ lymphocyte expression in AMI patients than in controls. In addition, CCR6 showed a significant positive correlation with the Gensini score (GS) in the AMI group then with the degree of coronary artery disease (CAD). Conclusion: The chemokine receptor Ccr6 is an independent biomarker for AMI and mayplay a role as a mediator of T lymphocyte recruitment, which is associated with coronary lesion destabilization.

Background: The lack of anti-COVID-19 treatment to date warrants urgent research into potential therapeutic targets. Virtual drug screening techniques enable the identification of novel compounds that target the Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) Main Protease (Mpro). Objective: The binding of the halogenated compounds to Mpro may inhibit the replication and transcription of SARS-CoV-2 and, ultimately, stop the viral life cycle. In times of dire need for anti- COVID-19 treatment, this study lays the groundwork for further experimental research to investigate these compounds' efficacy and potential medical uses to treat COVID-19. Method: New heterocyclic compounds were synthesized through the first reaction of cyclohexane- 1, 3-dione (1a) or dimedone (1b) with trichloroacetonitrile (2) to give the 2,2,2-trichloroethylidene) cyclohexane-1,3-dione derivatives 3a and 3b, respectively. The latter compounds underwent a series of heterocyclization reactions to produce biologically active compounds. Results: Novel compounds, including fused thiophene, pyrimidine and pyran derivatives, were synthesized and tested against human RNA N7-MTase (hRNMT) and selected viral N7-MTases such as SARS-CoV nsp14 and Vaccinia D1-D12 complex to evaluate their specificity and their molecular modeling was also studied in the aim of producing anti-covid-19 target molecules. Conclusion: The results showed that compounds 10a, 10b, 10c, 10e, 10f, 10g and 10h showed high % inhibitions against SARs-Covnsp 14. Whereas compounds 5a, 7a, 8b, 10a, 10b, 10c and 10i showed high inhibitions against hRNMT. This study explored the binding affinity of twenty-two halogenated compounds to the SARS-CoV-2 MPro and discovered fifteen compounds with higher binding affinity than Nelfinavir, of which three showed remarkable results. c-Met kinase inhibitions of 10a, 10f, 10g and 10h showed that all compounds exhibited higher inhibitions than the reference Foretinib.

In this study, a catalyst based on Montmorillonite clay was implemented for carbon nanotubes (CNTs) synthesizing. The kaolinite clay was used as a supporting material for iron-cobalt bimetallic catalytic nanoparticles. The CNTs have been synthesized by using atmospheric chemical vapor deposition (APCVD). To assess the quality of preparation both the catalyst and CNTs have been characterized by different techniques. The chemical bonding and interactions were verified by FT-IR. The general overview of microstructure was examined using SEM, while, the detailed structure and morphology were examined by HR-TEM, in addition to thermal analysis (DTA); surface area (BET); X-ray fluorescence (XRF), Raman spectroscopy, and XRD analysis. The results revealed that; Fe2O3 and Co3O4 NPs were uniformly assembled on the clay nanoplatelets. The specific BET surface area of the clay and catalyst was determined to be 46.12 and 57.06 m2/g respectively. Also, from XRD, the peaks at 26° and 42.7° confirm the presence of CNTs. The FTIR absorption bands, D, G, and G\ bands from the Raman spectrum confirm the hexagonal structure of the CNTs. The obtained results prove the high quality of CNTs preparation.

Introduction: Coronary bifurcation lesions are considered one of the challenging entities in the field of coronary intervention due to the risk of side branch loss and higher risk of stent thrombosis. However, there is limited data about the proper management of such lesions in the setting of myocardial infarction as most bifurcation lesion studies excluded patients with acute coronary syndromes (ACS). The aim of this study was to compare in-hospital and mid-term outcomes of single-stent and two-stents strategy in the management of bifurcation culprit lesions in patients presenting with anterior STEMI. Methods: This retrospective multi-center study included all patients presented with anterior STEMI who underwent primary PCI between January 2017 and December 2019, coronary angiography showed true bifurcation lesion with sizable side branch that can be managed by stenting. Patients with left main bifurcation, those indicated for urgent CABG, and patients in cardiogenic shock were excluded. Included patients were divided into two groups according to the stenting strategy either single or two stents. Six months follow up data were collected by telephone calls and by examination of medical records. Results: Out of 1355 anterior STEMI patients presented between January 2017 and December 2019, 158 patients (11.6%) were identified to have bifurcation culprit lesions with a sizable diagonal branch. 93 patients (59%) were treated by single stent while 65 patients (41%) were managed by two-stents strategy. Cancer is a challenging ailment and represents the main reason for death worldwide for humans and animals. Although great developments have hindered cancer progression, several adverse effects are associated with modern chemotherapy. Natural remedies, such as the usage of medicinal plant or their products in cancer treatment, may decrease prejudicial side properties. Recently, the modern research scheme and innovative screening practices for herbs or plants have enabled phytochemical discovery for the prevention and treatment of cancer. This criticism highlights herbs such as acacia, basil, black seeds, cedar, castus, ficus, garlic, ginger, indigo, onion, pomegranate, quince, and thyme, promising anticancer effects. The present review also revealed the mode of action of each herb as anticancer effects at level in vitro and in vivo studies. The item also totalizes the vital mechanisms and signaling molecules involved in preventing cancer diseases. This will fill the investigation gap in the exploration of using natural molecules and encourage researchers in clinical trials of anticancer agents from herbs for humans and animals.

Glycyrrhiza glabra and Sophora japonica (Fabaceae) are well-known medicinal plants with valuable secondary metabolites and pharmacological properties. The flavonoid-rich fractions of G. glabra roots and S. japonica leaves were prepared using Diaion column chromatography, and the confirmation of flavonoid richness was confirmed using UPLC-ESI-MS profiling and total phenolics and flavonoids assays. UPLC-ESI-MS profiling of the flavonoid-rich fraction of G. glabra roots and S. japonica leaves resulted in the tentative identification of 32 and 23 compounds, respectively. Additionally, the wound healing potential of topical preparations of each fraction, individually and in combination (1:1) ointment and gel preparations, were investigated in vivo, supported by histopathological examinations and biomarker evaluations, as well as molecular docking studies for the major constituents. The topical application of G. glabra ointment and gel, S. japonica ointment and gel and combination preparations significantly increase the wound healing rate and the reduction of oxidative stress in the wound area via MDA reduction and the elevation of reduced GSH and SOD levels as compared to the wound and Nolaver®-treated groups. The molecular docking study revealed that that major compounds in G. glabra and S. japonica can efficiently bind to the active sites of three proteins related to wound healing: glycogen synthase kinase 3-β (GSK3-β), matrix metalloproteinases-8 (MMP-8) and nitric oxide synthase (iNOS). Consequently, G. glabra roots and S. japonica leaves may be a rich source of bioactive metabolites with antioxidant, anti-inflammatory and wound healing properties.

Heart Disease is a complex and life-threatening ailment that poses a significant mortality risk around the world, with nearly a third of global deaths attributable to heart-related conditions. The early prediction and detection of heart disease are of utmost importance in the medical field, as they may lead to saving numerous lives. However, the lack of heart expertise in many countries and the high rates of misdiagnosis highlight the need for accurate and efficient prediction methods. Machine learning-based approaches have the potential to address this need, particularly in handling the large amounts of data generated by medical sectors and hospitals. In this study, the performance and accuracy of several supervised machine learning algorithms were compared for heart disease prediction using a dataset obtained from PhysioNet databases. The classifiers that were applied included Artificial Neural Network (ANN), Gradient Boosting, Decision Tree, Naive Bayes, and Random Forest. Results showed that the ANN algorithm achieved the highest Accuracy of 94.1%, with a sensitivity and specificity of 94.1%. The study thus concluded that supervised machine learning techniques can be utilized with great success to forecast heart disease, displaying exceptional potential for practical application and accuracy

Objectives: The study aimed to reveal the level of psychological security during Corona pandemic period for a sample of the University of Jeddah students. Methods: To achieve the goal of the study, a scale consisting of (33) items was constructed according to item response theory, specifically according to the rating scale model. The researcher used the following programs: (SPSS), (BIGSTEPS), and (BIOLOG-MG3) in analyzing the data of this scale, which was applied to a sample of (612) students from the University of Jeddah. During the first semester of the academic year (1441-1442 AH), corresponding to (2020-2021 AD), each of the following were estimated: psychological security scale parameters, the standard error of estimate, and items fitting to the rating scale model Results: In light of the results of model fitting, the psychological security scale consisted in its final form of (25) items. Also the results showed that the scale has good psychometric properties. Conclusion: Depending on the results, it becomes clear that the level of psychological security during the Corona pandemic was high among the students of the University of Jeddah.

Background: Cesarean Section on Maternal Request (CSMR) is a growing phenomenon whose literature needs to be appraised, and it is exemplified by a steady increase in the world neck and neck by high percentage of births by CS. It is even more luckless that giving birth by elective CS based on the pregnant woman’s choice has become the first place among the justifications, notwithstanding its direct and long-term complications. Methods: This retrospective study reviewed data of all CS deliveries during the year 2022 at our hospital using electronic medical records in the hospital information system. Retrieved data include baseline demographic characteristics, mode of delivery, indications, and the type of CS, aiming to clarify the reason for CS to challenge the percentage of CS based on the yearning of the pregnant woman without a medical reason; to identify, analyze and try to solve the ethical problem raised up by the pregnant woman’s request for CS. Results: The results revealed two significant facts; a sturdy noteworthy increase in the percentage of pregnant women delivering by CS compared to vaginal delivery at 54% versus 45.6%, and a sharp increase in the CS deliveries on maternal requests at 22.78%. The main reason for this shifting practice is the previous one CS followed by a decision that was taken on personal and family convictions. These harvested results revealed a significant increase in the percentage of pregnant women not receiving proper antenatal counseling about the appropriate method of delivery, with improper justification to jump over nature. Conclusion: Cesarean section should be signposted when on earth there is any indication or menace of detriment to the maternal and fetal binomial. If in earlier times "labor death" was a fact of life, nowadays it is astonishing and disgraceful the death of a mother due to pregnancy-delivery-postpartum. The proclamation that vaginal delivery is better because it is "natural" cannot and should not be taken to the last consequences under the risk of bad luck. The best form of birth is the safe one. To provide every pregnant woman with the right to choose her child's mode of delivery is to arbitrate for her sovereignty, yielding her respect and pride, nonetheless, it should be minimalistic and not absolute under this banner. Minimizing the rate of primary CS carries the secret key to ideal obstetrical care.
