Abstract: The ubiquity of the obesity condition in the United States, Europe and other regions with developed economies will associate to a significant adverse impact on public health. Numerous data indicate that social, behavioral, neuroendocrine, and metabolic factors may encourage compulsive eating behaviors thus increasing the risk of obesity. Several pathological conditions overlap with excess weight. Among the most common, there are binge eating disorder (BED) and food addiction (FA), which share several neurobiological and behavioral aspects with substance addictions. BED has many features in common with addictive behavior, such as loss of control and the need to frequently repeat the dysfunctional pattern despite negative consequences. The food addiction hypothesis assumes that exposure to highly palatable foods alters the reward circuits of the brain, resulting in a behavioral phenotype similar to substance addiction and facilitating dysfunctional eating behaviors, such as binge eating crises. In this review, over 100 publications, researched on MEDLINE from 2000 until march 2021, were included since they evaluate neuroendocrine changes, emotional homeostatic factors and the reward circuit, associating them with exposure to highly palatable foods, loss of control, the way we eat, the increase in impulsiveness and the inability to change eating behavior despite the negative consequences related to overweight and obesity. Finally, understanding the underlying neurobiological circuits of compulsive eating behaviors and food addiction could result in a great therapeutic potential for patients suffering from ailments nutrition and obesity.

This contribution aims at an understanding, at a molecular scale, of the state of CO2 molecules absorbed in glassy poly(ether imide) (PEI). This issue has been challenged by combining different approaches that tackle the problem from both the experimental and the theoretical viewpoints and providing a comprehensive physicochemical picture. In situ FTIR spectroscopy and gravimetry were exploited to gather relevant experimental information, while molecular dynamics (MD), density functional theory (DFT), and statistical thermodynamics approaches were used to model the behavior of the binary system at different scales. Based on the findings of FTIR spectroscopy and on DFT and MD calculations, it was determined that, among the possible interaction configurations, some are prevailing. In particular, the carbon atom of carbon dioxide molecules establishes relatively weak interactions prevalently with the carbonyl groups of PEI. A quantitative estimate of such interaction has been provided by MD calculations. The system was also analyzed using a lattice fluid model, specifically developed to deal with sorption of low molecular weight compounds in glassy polymers, that is rooted on statistical thermodynamics, determining the values of the isosteric heat of sorption and carbon-dioxide-polymer interaction energy. Finally, experimental data of CO2-PEI mutual diffusivity have been interpreted using a semiempirical theoretical model accounting for the effects of the penetrant concentration, of energy barriers associated with the occurrence of an effective diffusive jump, and of a thermodynamic factor.

Purpose This research aimed to investigate the socio-demographic, clinical, and psychological variables predictive of a greater functioning and quality of life in patients with gynecological cancer after their first cycle of carboplatin and taxol-based chemotherapy. Methods The sample of the present research consisted of 104 patients. The European Organization on Research and Treatment of Cancer QLQ-C30, the State-Trait Anxiety Inventory-Form Y, and the Multidimensional Scale of Perceived Social Support were administered to each participant. Results The analyses showed that higher state anxiety levels predicted a lower role, emotional, and social functioning and a lower general quality of life. Higher trait anxiety levels and social support perceived from one’s friends predicted a greater role functioning. Similarly, having a relationship predicted a greater physical, cognitive, and social functioning. On the contrary, the presence of relapsed cancer was negatively associated with these patients’ quality of life. Conclusions The present study highlighted the importance of identifying patients at higher risk of experiencing lower levels of functioning and worse general quality of life to implement tailored interventions from the beginning of treatment, thus improving the quality of life of these patients throughout the chemotherapy treatment.

Introdução: O cuidado com o paciente internado em Unidade de Terapia Intensiva abrange variadas esferas, incluindo a da higiene oral, frequentemente praticada por profissionais da equipe de enfermagem. Estes pacientes são constantemente expostos a fatores de risco que podem levar a complicações sistêmicas, como a pneumonia associada à ventilação mecânica. A execução diária da higienização oral contribui para a redução de patógenos da cavidade oral, trazendo segurança ao paciente. Sua prática adequada demanda conhecimento e padronização, e, para tanto, protocolos e treinamentos eficazes são necessários. Métodos: Foi realizado estudo diagnóstico situacional de caráter observacional, descritivo e transversal, com aplicação de questionário a equipe de enfermagem das Unidades de Terapia Intensiva de um hospital público de cardiologia do Estado de São Paulo. Resultados: Foram obtidos 103 questionários respondidos, sendo 49 técnicos de enfermagem, 29 enfermeiros, 20 auxiliares de enfermagem e 5 enfermeiros residentes. Do total de participantes, 39,8% afirmaram não realizar a higienização do tubo orotraqueal e 58,3% não recebeu treinamento em higienização oral. A consideração de que a higienização oral é adequada foi de 73,8% e demonstrou-se independente estatisticamente de fatores como a higienização do tubo orotraqueal, e recebimento de treinamento. Conclusão: O papel da equipe de enfermagem é essencial para a sistematização da prática da higiene oral, colaborando para a prevenção de infecções. A presença de protocolo sistematizado e treinamentos eficazes favorecem as boas práticas assistenciais e beneficia o tratamento do paciente. O cirurgiãodentista inserido na equipe multidisciplinar possibilita enriquecimento para o atendimento do paciente crítico.

Sexual and gender minority (SGM) patients with cancer experience significant health disparities requiring tailored care. Collecting sexual orientation and gender identity (SOGI) data in the electronic medical record (EMR) could allow care to be tailored and is in line with Radiation Oncology's mission to better serve diverse patients. Here, we describe a systematic method for collecting SOGI data for all patients starting radiation treatment in a Department of Radiation Oncology (DRO).During a 3-month experimental period, DRO staff administered a demographic questionnaire and attitude survey to new adult patients. SOGI demographic data, entered into the EMR by nursing staff, were extracted and analyzed for all patients from the experimental period and from the 3 months prior (control period). Descriptive and categorical data completion rates were compared between the experimental and control periods using independent samples t-tests and Pearson Chi-square tests.A total of 788 patients were included in this analysis: 368 in the control period and 420 in the experimental period. Of the 420 patients enrolled in the experimental period, 267 (63.6%) were offered a survey, of whom 211 (79.0%) completed a survey. There were higher rates of sexual orientation responses entered into the EMR for the experimental group compared to the control group (56.9% vs. 27.1%, p0.001), with the highest response rates for patients who completed a survey (82.9%). Ten patients (2.9%) identified as gay/lesbian and 100% identified as cisgender. The majority of patients were not upset by the form, with only 11 patients (5.2%) stating that any specific question caused them distress.Collecting SOGI data via a demographic form is feasible in an outpatient DRO. This approach was well-received by the majority of patients and could lead to provision of higher quality, tailored care.

Advances in time-resolved structural techniques, mainly in macromolecular crystallography and small-angle X-ray scattering (SAXS), allow for a detailed view of the dynamics of biological macromolecules and reactions between binding partners. Of particular promise, are mix-and-inject techniques, which offer a wide range of experimental possibility as microfluidic mixers are used to rapidly combine two species just prior to data collection. Most mix-and-inject approaches rely on diffusive mixers, which have been effectively used within crystallography and SAXS for a variety of systems, but their success is dependent on a specific set of conditions to facilitate fast diffusion for mixing. The use of a new chaotic advection mixer designed for microfluidic applications helps to further broaden the types of systems compatible with time-resolved mixing experiments. The chaotic advection mixer can create ultra-thin, alternating layers of liquid, enabling faster diffusion so that even more slowly diffusing molecules, like proteins or nucleic acids, can achieve fast mixing on timescales relevant to biological reactions. This mixer was first used in UV–vis absorbance and SAXS experiments with systems of a variety of molecular weights, and thus diffusion speeds. Careful effort was also dedicated to making a loop-loading sample-delivery system that consumes as little sample as possible, enabling the study of precious, laboratory-purified samples. The combination of the versatile mixer with low sample consumption opens the door to many new applications for mix-and-inject studies.

Technological developments are getting faster, as is the dissemination of existing information, especially on the capital market. In order for investors to avoid losses from the capital market, a method is needed that is able to analyze the movement of the stock price. This study focuses on the application of the Data Scales Analysis (DSA) method which uses a formula with the concept of the same amount of data between the first and second order average differences at levels n and n + 1 for predicting the stock price of issuers, in predicting stock prices in the capital market. The resulting formula is named JIC-FLY 2 which is a new formula used to predict stock prices in the capital market. The population used in this study are issuers who are members of IDX 30 from the banking sub-sector with the sample used is the issuer of BBCA (PT Bank Central Asia Tbk). The results of this study note that the DSA method with this formula is able to produce the best predictive value, namely DSA 12 with an error percentage of 0.035%.

Neste artigo, discute-se a metáfora do teto de vidro e do labirinto de cristal, confrontando tais referenciais com os dados do relatório Techo de cristal en comunicación. A partir dessas reflexões, busca-se conhecer a percepção dos profissionais da comunicação da América Latina a respeito de suas próprias realidades organizacionais. Percebe-se que, apesar de a área ter proximidade com as Ciências Sociais Aplicadas, com frequência as desigualdades de gênero estão aí reproduzidas.

Worldwide, the coronavirus 19 disease pandemic caused a worse chance of a timely diagnosis for cancer patients. We conducted a retrospective analysis of new diagnoses registered in the national pediatric oncology database, comparing the first lockdown period (March-May 2020) with the same period of 2015-2019. The total number of cases (0-19 years) dropped by 20.8% (from 441 between 2015 and 2019 to 349 in 2020). A major reduction was observed for adolescents (15-19 years) (-32.9%) and for adolescents with solid tumors (-56.4%, p = 0.03). Our data suggest that the enforced lockdown reduced the possibility for these already vulnerable patients to access the referral centers.

O objetivo deste artigo é analisar o padrão setorial de inovação da indústria brasileira, considerando a Taxonomia Pavitt. A hipótese é que há diferenças para a Taxonomia Pavitt em países em desenvolvimento. O trabalho então utilizou o banco de dados PINTEC 2014 complementado com os bancos de dados RAIS e PIA. O arcabouço metodológico foi a análise fatorial para encontrar fatores relevantes combinados à análise de cluster para criar grupos setoriais com as mesmas características de mudança tecnológica. Os fatores criados (impacto da inovação, gastos com inovação, apropriação de conhecimento e projetos industriais e gastos com marketing) foram bons o suficiente para detectar padrões setoriais de inovação na indústria brasileira. O artigo contribui para mostrar que o tamanho da empresa não é uma boa variável para investigar o padrão setorial da indústria brasileira. A indústria brasileira apresenta alguma convergência com a taxonomia Pavitt, mas apresenta algumas especificidades que também foram confirmadas por estudos anteriores.

BACKGROUND: Extracorporeal membrane oxygenation (ECMO) has been widely used in patients with COVID-19, but uncertainty remains about the determinants of in-hospital mortality and data on post-discharge outcomes are scarce. The aims of this study were to investigate the variables associated with in-hospital outcomes in patients who received ECMO during the first wave of COVID-19 and to describe the status of patients 6 months after ECMO initiation.METHODS: EuroECMO-COVID is a prospective, multicentre, observational study developed by the European Extracorporeal Life Support Organization. This study was based on data from patients aged 16 years or older who received ECMO support for refractory COVID-19 during the first wave of the pandemic-from March 1 to Sept 13, 2020-at 133 centres in 21 countries. In-hospital mortality and mortality 6 months after ECMO initiation were the primary outcomes. Mixed-Cox proportional hazards models were used to investigate associations between patient and management-related variables (eg, patient demographics, comorbidities, pre-ECMO status, and ECMO characteristics and complications) and in-hospital deaths. Survival status at 6 months was established through patient contact or institutional charts review. This study is registered with ClinicalTrials.gov, NCT04366921, and is ongoing.FINDINGS: Between March 1 and Sept 13, 2020, 1215 patients (942 [78%] men and 267 [22%] women; median age 53 years [IQR 46-60]) were included in the study. Median ECMO duration was 15 days (IQR 8-27). 602 (50%) of 1215 patients died in hospital, and 852 (74%) patients had at least one complication. Multiorgan failure was the leading cause of death (192 [36%] of 528 patients who died with available data). In mixed-Cox analyses, age of 60 years or older, use of inotropes and vasopressors before ECMO initiation, chronic renal failure, and time from intubation to ECMO initiation of 4 days or more were associated with higher in-hospital mortality. 613 patients did not die in hospital, and 547 (95%) of 577 patients for whom data were available were alive at 6 months. 102 (24%) of 431 patients had returned to full-time work at 6 months, and 57 (13%) of 428 patients had returned to part-time work. At 6 months, respiratory rehabilitation was required in 88 (17%) of 522 patients with available data, and the most common residual symptoms included dyspnoea (185 [35%] of 523 patients) and cardiac (52 [10%] of 514 patients) or neurocognitive (66 [13%] of 512 patients) symptoms.INTERPRETATION: Patient's age, timing of cannulation (FUNDING: None.

In Siddha literatures viral fevers can be compared to Vatha Kapha Suram and therefore decoction for this Suram is taken from classical Siddha text book and the ingredients are searched for pharmacological activity and toxicity study from 118 published journal papers from Pubmed, Scopus, Google Scholar, Research Gate and Science Direct. Collected data is entered in MS excel and analysed using RFC score. All together there are 51 pharmacological activities are published for these nine ingredients. In which Anti-inflammatory has RFC=1, other properties like antipyretic, anti-tussive, bronchodialator, anti-asthmatic, analgesic, neuro-regenerative, neuroprotective, anti-diarrhoeal, anti-emetic, neuroprotective, analgesic, sedative has more than 0.55 RFC score. More than this cardioprotective, renoprotective, hepatoprotective, and other protective properties are present in more than 50% of ingredients. In toxicity studies no toxic effects are seen in in vivo studies for the ingredients The Suvai, Thanmai, Pirivu, in Siddha aspect, of these ingredients can highly reduces Vatha and Kapha disorders. Therefore, the decoction shows significant pharmacological activity for the sign and symptoms of viral infections.

This report summarizes the incidence, relative frequency distributions and survival & mortality by age, sex, stage and grade, of adult invasive primary cancers of the lip in two entrant time-periods as recorded in the SEER Program of the National Cancer Institute for diagnosis years 1973-2014 (SEER Stat 8.3.5). While the occurrence rates and frequency are low in the United States, they are exceptionally important from a clinical and surgical standpoint because of the morphological and functional changes involved.

This paper proposes a taxonomy of power converter control schemes based on the recently proposed concept of complex frequency. This quantity captures local frequency variations due to the change of both the phase angle and amplitude of bus voltages and current injections. The paper derives the analytical expressions of the link between complex power variations and complex frequency of each converter controller as well as the identification of critical control parameters. The main contribution of this work is to provide a general framework that allows classifying converters synchronization mechanisms and controllers. This framework also allows comparing converters with synchronous machines. To validate the theoretical results, extensive simulations are performed using a modified version of the WSCC 9-bus system. Examples of how the theoretical formulations of the paper can be used to improve power converter control in power system applications are showcased., 10 pages, 22 figures

Despite decades of research and practical experience, developers have few tools for programming reliable distributed applications without resorting to expensive coordination techniques. Conflict-free replicated datatypes (CRDTs) are a promising line of work that enable coordination-free replication and offer certain eventual consistency guarantees in a relatively simple object-oriented API. Yet CRDT guarantees extend only to data updates; observations of CRDT state are unconstrained and unsafe. We propose an agenda that embraces the simplicity of CRDTs, but provides richer, more uniform guarantees. We extend CRDTs with a query model that reasons about which queries are safe without coordination by applying monotonicity results from the CALM Theorem, and lay out a larger agenda for developing CRDT data stores that let developers safely and efficiently interact with replicated application state.

Continuous invasive monitoring of intracranial pressure (ICP) is essential in neurocritical care for surveillance and management of raised ICP. Fluid-based systems and strain gauge microsensors remain the current standard. In the past few decades, several studies with wireless monitoring were developed aiming to reduce invasiveness and complications.To describe a novel Wi-Fi fiber-optic device for continuous ICP monitoring using smartphone in a swine model.Two ICP sensors (wireless prototype and wire-based reference) were implanted in the cerebral parenchyma of a swine model for a total of 120 minutes of continuous monitoring. Every 5 minutes, jugular veins compression was performed to evaluate ICP changes. The experimentation was divided in 3 phases for comparison and analysis.Phase 1 showed agreement in ICP changes for both sensors during jugular compression and releasing, with a positive and strong Spearman correlation (r = 0.829, P.001). Phase 2 started after inversion of the sensors in the burr holes; there was a positive and moderately weak Spearman correlation (r = 0.262, P.001). For phase 3, the sensors were returned to the first burr holes; the prototype behaved similarly to the reference sensor, presenting a positive and moderately strong Spearman correlation (r = 0.669, P.001).A Wi-Fi ICP monitoring system was demonstrated in a comprehensive and feasible way. It was possible to observe, using smartphone, an adequate correlation regarding ICP variations. Further adaptations are already being developed.

In their quest toward Exascale, High Performance Computing (HPC) systems are rapidly becoming larger and more complex, together with the issues concerning their maintenance. Luckily, many current HPC systems are endowed with data monitoring infrastructures that characterize the system state, and whose data can be used to train Deep Learning (DL) anomaly detection models, a very popular research area. However, the lack of labels describing the state of the system is a wide-spread issue, as annotating data is a costly task, generally falling on human system administrators and thus does not scale toward exascale. In this article we investigate the possibility to extract labels from a service monitoring tool (Nagios) currently used by HPC system administrators to flag the nodes which undergo maintenance operations. This allows to automatically annotate data collected by a fine-grained monitoring infrastructure; this labelled data is then used to train and validate a DL model for anomaly detection. We conduct the experimental evaluation on a tier-0 production supercomputer hosted at CINECA, Bologna, Italy. The results reveal that the DL model can accurately detect the real failures, and, moreover, it can predict the insurgency of anomalies, by systematically anticipating the actual labels (i.e., the moment when system administrators realize when an anomalous event happened); the average advance time computed on historical traces is around 45 minutes. The proposed technology can be easily scaled toward exascale systems to easy their maintenance.

Secretory carcinoma (SC) is a salivary gland tumor with a generally low grade microscopic appearance, a characteristic immunophenotype, and a recurrent translocation leading to ETV6::NTRK3 fusion gene. Rare cases are reported in children. The maxillary sinus is an unusual localization. SC have an overall favorable prognosis, but cases with high grade morphology have been described in adult population and are related to a more aggressive clinical course. We present a pediatric case of secretory carcinoma involving the maxillary sinus with high grade morphology, with a review of the literature of secretory carcinomas with high grade component.

We assessed the incidence, predictors and clinical correlates of de-novo aortic regurgitation (AR), which physiologically reduces left ventricular assist device (LVAD) effectiveness due to recirculation syndrome, in the MOMENTUM 3 trial portfolio of the fully magnetically levitated HeartMate 3 (HM3) pump using the randomized pivotal trial (PT) and post-trial continued access protocol (CAP).De-novo aortic regurgitation incidence at 2 years was analysed in the randomized PT and validated in the first 1000 implanted patients of the CAP. Patients with concomitant/prior aortic valve surgery or without baseline or post-implant echocardiograms were excluded from this analysis. AR severity was assessed qualitatively by site-adjudicated echocardiograms (significant AR was defined as moderate or severe grade on echocardiogram). Of 1028 patients enrolled in the PT, 918 were eligible for inclusion in this analysis (HM3, n = 465; HMII, n = 453). At 2 years of LVAD support, freedom from significant AR was greater in the HM3 (92%) than HMII (82%) (hazard ratio 0.45, 95% confidence interval 0.27-0.75, p 0.01). Of 907 HM3 patients analysed from the first 1000 implanted CAP patients, the rate of freedom from significant AR was 90%, consistent with the PT (p = 0.3). In the combined HM3 group (n = 1372), multivariable Cox modelling identified increasing age and female sex as significant predictors. Survival free of urgent transplant or AR corrective procedure was similar between HM3 patients with and without significant de-novo AR.The development of moderate or severe grade de-novo AR is reduced with the fully magnetically levitated HM3 LVAD compared to the axial-flow HMII pump. The occurrence of significant de-novo AR with the HM3 pump is not associated with a worse outcome at 2 years of follow-up.

Os autores propõe-se a comprovar quais os verdadeiros responsáveis pela desinfecção dos condutos radiculares. Para esse fim, selecionaram casos de canais de dentes monoradiculares infectados, comprovados pelo teste bacteriológico, os quais foram tratados mecânica e quimicamente. Os casos foram agrupados de acordo com a conduta seguida, como se vê: I Grupo — 6 casos — Instrumentação acompanhada por irrigação com água destilada seguida por medicação com Eugenol. II Grupo — 6 casos — Instrumentação acompanhada por irrigação com água destilada seguida por medicação com Néo-Grove. III Grupo 6 casos — Instrumentação acompanhada por irrigação com água destilada seguida por medicação com Para Mono Clorofenol Canforado. IV Grupo — 7 casos — Instrumentação acompanhada por irrigação com soda clorada e água oxigenada a 20 vol.V Grupo — 8 casos — Instrumentação acompanhada por irrigação com soda clorada e água destilada. VI Grupo — 7 casos — Instrumentação acompanhada por irrigação com soda clorada e água oxigenada a 20 vol., sob isolamento relativo. O controle dos resultados foi feito através de testes bacteriológicos.

Sterculia foetida derived biodiesel is a potential fuel for a diesel engine. The Sterculia foetida biodiesel required a pre-refining process called degumming and an acid pretreatment process before converting them to methyl ester using the transesterification process. This study blended fuel from Sterculia foetida biodiesel and diesel with different volume ratios (5% to 30% of biodiesel blend with 95% to 70% diesel fuel). Sterculia foetida biodiesel and blended fuels met the ASTM D6751 and EN 14214 standards. The blended fuel is examined to obtain its influences on the performance and emission when operating at a diesel engine (1300 rpm to 2400 rpm). From the outcome, the engine performance of the SFB5 blend shows better performance than diesel fuel in terms of BTE (28.84%) and BSFC (5.86%). Artificial neural networks and extreme learning machines were employed to predict engine performance and exhaust emissions. The developed models gave excellent results, where the coefficient of determination is more than 99% and 98% for BSFC and BTE, respectively. When the engine is operated with SFB5, there is a significant reduction in CO, HC, and smoke opacity emissions by 8.26%, 2.08%, and 3.08%, respectively, and at the same time, an increase in CO2 by 3.53% and NOX by 22.39%. The comparison is made with diesel fuel. The extreme learning machine modelling is powerful for predicting engine performance and exhaust emission compared to artificial neural networks in terms of prediction accuracy. Sterculia foetida biodiesel–diesel blends of 5% show its capability to replace diesel fuel by providing engine peak performance than diesel fuel.

Background Concerns regarding long-term durability of surgical repair for functional mitral regurgitation are based on short-term data, with few comparisons of ischemic (IMR) versus non-ischemic (NIFMR) etiology. Methods 788 consecutive patients receiving mitral repair for functional mitral regurgitation were evaluated from a prospectively maintained database. Patients with other surgical procedures were included. Propensity score matching was used to compare outcomes in IMR versus NIFMR. Results Unmatched IMR patients tended to be older men with greater comorbidities. 198 matched pairs of IMR versus NIFMR patients had similar demographics with relatively preserved ejection fraction 40±13% and end-systolic diameter 4.3±1.1cm. Concomitant coronary revascularization occurred in 70% of matched IMR patients. All patients received an annuloplasty ring, usually 24-26 mm. Heart failure class improved from 2.8 preop to 1.5 at 5 years (P =2+) mitral regurgitation (27±4% v 26±4%, P=0.4), severe regurgitation (10±3% v 8±2%, P=0.5), and mitral reoperation (3±1% v 3±1%, P=0.4) were not different between IMR v NIFMR. Recurrent moderate regurgitation was associated with heart failure readmission but not with mortality. Conclusions In propensity-matched patients, IMR versus NIFMR had worse survival but similar repair durability, with moderate regurgitation in 27% at 10 years and rare severe regurgitation or mitral reoperation. In selected patients with relatively preserved function, mitral repair for IMR or NIFMR can improve symptoms with durable mild regurgitation in most patients out to 10 years.

The prediction of right heart failure (RHF) after left ventricular assist device (LVAD) implantation remains a challenge. Recently, risk scores were derived from analysis of the European Registry for Patients with Mechanical Circulatory Support (EUROMACS) data, the EUROMACS-RHF, and the modified postoperative EUROMACS-RHF. The authors assessed the performance characteristics of these 2 risk score formulations in a continuous-flow LVAD cohort at their institution.A retrospective, observational study.At a tertiary-care academic medical center.Adult patients who underwent durable LVAD implantation between 2015 and 2018.None MEASUREMENTS AND MAIN RESULTS: Early post-LVAD RHF was defined as follows: (1) need for right ventricular assist device, or (2) inotropic or inhaled pulmonary vasodilator support for ≥14 postoperative days. The authors used logistic regression and examined receiver operating characteristic (ROC) curves to evaluate the ability of the 2 risk scores to distinguish between outcome groups. A total of 207 patients met the inclusion criteria. Of the patients, 16% developed RHF (33/207). The EUROMACS-RHF score was not predictive of RHF in the authors' cohort (odds ratio [OR] 1.25; 95% CI [0.99-1.60]; p = 0.06), but the postoperative EUROMACS-RHF CPB score was significantly associated (OR 1.38; 95% CI [1.03-1.89]; p = 0.03). The scores had similar ROC curves, with weak discriminatory performance: 0.601 (95% CI [0.509-0.692]) and 0.599 (95% CI [0.505-0.693]) for EUROMACS-RHF and postoperative EUROMACS-RHF, respectively.In the authors' single-center retrospective analysis, the EUROMACS-RHF risk score did not predict early RHF. An optimized risk score for the prediction of RHF after LVAD implantation remains an urgent unmet need.

Investigación que tuvo como objetivo caracterizar desde una reflexión teórica la incidencia de la inteligencia emocional como eje fundamental en la praxis transformadora de la gestión institucional educativa. Pesquisa abordada bajo el paradigma positivista, enfoque cuantitativo, desde una modalidad documental de nivel descriptivo, teniendo como instrumento de recolección de datos el fichaje; como técnica de análisis de los resultados se utilizó el análisis de contenido. Concluyéndose, que ser gerente en una dependencia educativa implica poder formar competencias que logren integrar adecuadamente la organización, pero para ello, quien funge de líder debe comportarse como persona adulta cuando logra emerger espontáneamente autónomo colaborativo; quien desde su respetarse, asume una actitud de respeto hacia el otro, acción que solo es posible al reconocer que solo desde el manejo adecuado de emociones y sentimientos puede generar transformación, desde el amor.

An unusual violent clash between religious, Christian and Muslim demonstrators and the Haifa Museum of Art took place on Thursday, January 9, 2019. The protesters amassed the entrance of the museum to express their distress against the exhibition Shop It! A handheld firebomb thrown towards the museum was but the first round for the violent reactions of the next several days. These included attacking police forces and attempts to break into the museum featuring works of art considered sacrilegious by the demonstrators. The protest developed into a multi-participant debate in the art scene, involving ordinary citizens, artists and art curators, local church leaders, the city mayor, the minister of culture and other politicians, civil rights organizations and the court, as each part sought to exercise civic, moral, ethical or practical authority to censor the position or the work of the other. The article seeks to analyze the different ways in which mechanisms of regularization, supervision and punishment, which have always been part of the world of art, are used not only by those perceived as apparatuses of the state or as ideological mechanisms of abstract political or institutional entity, but as standard practices in the discourse of creating, trading, curating and presenting art – a discourse which, in the Israeli-Palestinian context, is imbued with the notion of national conflict.

As intoxicações são causas frequentes de admissão hospitalar e possuem elevada mortalidade quando não são reconhecidas e tratadas de maneira adequada. Quanto à definição, a intoxicação aguda corresponde a um conjunto de sinais e sintomas que aparecem no organismo logo após a exposição a substâncias tóxicas; no mundo, as principais substâncias envolvidas em casos de intoxicação são o álcool, os opióides, os benzodiazepínicos e o acetaminofeno. Ademais, cada uma dessas drogas possuem fisiopatologia e manifestações clínicas próprias; contudo, alguns sinais e sintomas são comuns em algumas delas e, dessa forma, torna-se imprescindível aos profissionais de saúde saberem diferenciá-las para o adequado tratamento ao paciente. Além das medidas básicas como suporte ventilatório e estabilização do paciente, em muitas situações a lavagem gástrica e a hemodiálise tornam-se essenciais para a remoção destas substâncias do organismo. Concomitantemente, na maioria dos casos existem antídotos que podem ser utilizados para a redução da intoxicação e, por isso, faz-se necessário identificar qual a causa da intoxicação. Outrossim, tão importante quanto o tratamento, são a adoção de medidas preventivas, já que muitas vezes as intoxicações ocorrem devido à desinformação da população e ao fácil acesso a estas substâncias. Nesse sentido, o controle rígido na prescrição e oferta dessas drogas são responsabilidade das autoridades de saúde; além disso, quando indicado o uso dessas substâncias, as orientações devem ser claras quanto à posologia e vias de administração.

Loa loa is a filarial nematode responsible for loiasis, endemic to West–Central Africa south of the Sahara and transmitted by flies. This study reports a case of L. loa in the vitreous cavity of the eye of a young patient, along with an in-depth literature review. A 22-year-old woman from Cameroon who migrated from Cameroon to Italy was referred to the Emergency Ophthalmology Department at Policlinico di Bari in July 2021 with the presence of a moving parasite in the subconjunctiva of the left eye. A recent onset of a papular lesion on the dorsal surface of the right wrist and a nodular lesion in the scapular region were detected. L. loa filariasis was diagnosed based on anamnestic data, clinical and paraclinical signs, and a parasitological test confirming the presence of microfilariae in two blood samples collected in the morning of two different days. Because of the unavailability of diethylcarbamazine (DEC), albendazole (ALB) 200 mg twice daily was administered for 21 days. A mild exacerbation of pruritus occurred during treatment, but resolved with the use of an antihistamine. A single dose of 12 mg ivermectin was prescribed at the end of the treatment with albendazole. Unlike other endemic parasite infections, L. loa is not included in the Global Program to Eliminate Lymphatic Filariasis, because it is not mentioned in the WHO and CDC list of neglected tropical diseases. This can result in an overall risk of lack of attention and studies on loiasis, with lack of data on global burden of the disease.

O objetivo deste trabalho é analisar o Plano Inova Empresa no âmbito de políticas orientadas à missão. Para isso, foi utilizada a análise de conteúdo sob os editais do plano, com a criação de categorias de análise baseadas no referencial teórico apresentado. Os resultados obtidos apresentaram que mesmo o Plano Inova Empresa sendo considerado um grande esforço de política industrial e de inovação brasileiro, com formas pioneiras de liberação de recursos através de análise de Planos de Negócio, os editais não foram construídos sob objetivos de missões que poderiam transformar a economia brasileira e nem tiveram o objetivo de mudar a estrutura econômica, tornando-se apenas mais um esforço de política industrial sistêmico não se enquadrando perfeitamente no arcabouço de Política Orientada a Missão.
