Não é de hoje que a comunidade internacional chama à ação e à responsabilidade os principais atores políticos e sociais para discussão e adoção de medidas de salvaguarda ambiental. O Brasil durante muito tempo se esforçou para tornar-se uma voz ativa frente à governança ambiental, no entanto, nos últimos anos, tem deixado de lado o protagonismo até então exercido nessa área. O Acordo de Paris foi o responsável pelo desenvolvimento de um novo sistema de participação voluntária das partes através das Contribuições Nacionalmente Determinadas. Uma nova ordem jurídica faz surgir novos paradigmas, nesse sentido, buscou-se por meio de pesquisa exploratória em meios físicos e eletrônicos, utilizando-se o método indutivo, verificar a possibilidade de a comunidade internacional responsabilizar um Estado pelo não cumprimento do Acordo de Paris, respeitando sua soberania. Como conclusão destaca-se que com base unicamente no Acordo de Paris não há possibilidade de responsabilização de um Estado parte pelo descumprimento das avenças, ficando, assim, à mercê da diplomacia climática estabelecida entre os diversos atores que compõe a comunidade internacional. A cidadania ativa e fiscalizadora, interna e externa, tem papel fundamental em compelir as partes para, ao menos se esforçarem a atingir metas através políticas públicas voltadas à mitigação dos efeitos climáticos.

During the last decade, immunotherapy has radically changed perspectives on anti-tumor treatments. However, solid tumor treatment by immunotherapy has not met expectations. Indeed, poor clinical response to treatment has highlighted the need to understand and avoid immunotherapy resistance. Cholangiocarcinoma (CCA) is the second cause of hepatic cancer-related deaths because of drug inefficacy and chemo-resistance in a majority of patients. Thus, intense research is ongoing to better understand the mechanisms involved in the chemo-resistance processes. The tumor microenvironment (TME) may be involved in tumor therapy resistance by limiting drug access. Indeed, cells such as cancer-associated fibroblasts (CAFs) alter TME by producing in excess an aberrant extracellular matrix (ECM). Interestingly, CAFs are the dominant stromal component in CCA that secrete large amounts of stiff ECM. Stiff ECM could contribute to immune exclusion by limiting anti-tumor T-cells drop-in. Herein, we summarize features, functions, and interactions among CAFs, tumor-associated ECM, and immune cells in TME. Moreover, we discuss the strategies targeting CAFs and the remodeling of the ECM to improve immunotherapy and drug therapies.

O presente artigo tem o propósito de refletir sobre o fornecimento gratuito de medicamentos pelo Estado e a limitação imposta pela reserva do possível. O problema proposto é o de identificar quais são os limites impostos pela reserva do possível à efetivação do direito à saúde, especificamente, ao fornecimento gratuito de medicamentos pelo Estado. Como hipótese, aventa-se que o Poder Público deverá garantir uma existência digna a todos os brasileiros, ainda que alegue a ausência de recursos financeiros e/ou previsão orçamentária. O objetivo geral foi o de analisar os limites da reserva do possível para o fornecimento gratuito de medicamentos pelo Estado, em detrimento do mínimo existencial com foco no aumento da judicialização de demandas dessa natureza nos estados brasileiros. Em relação aos objetivos específicos, buscou-se conceituar as políticas públicas de saúde, discutir sobre a dignidade da pessoa humana e o direito à saúde, compreender a dificuldade na efetivação do direito à saúde pelos entes públicos, por força de decisões judiciais, à luz do princípio da reserva do possível, e examinar os entendimentos do Supremo Tribunal Federal sobre o tema. Como metodologia, tem-se a análise sistemática da Constituição da República Federativa do Brasil em relação aos direitos fundamentais e sociais cumulativamente aos princípios constitucionais, bem como uma revisão bibliográfica e jurisprudencial, utilizou-se para tanto do método dedutivo. Como conclusão, destaca-se que a hipótese se confirmou, visto que o Estado deve garantir um mínimo existencial. Entretanto, deve observar os orçamentos públicos sob o viés do interesse coletivo, conforme delineado na teoria da reserva do possível.

O presente artigo tem como objetivo analisar os impactos sociais e jurídicos causados pelo uso da inteligência artificial na tomada de decisões judiciais. Para tanto, destaca-se como problema de pesquisa o questionamento: O uso da inteligência artificial nas decisões judiciais afronta os Direitos Fundamentais assegurados na Constituição Federal? Aventa-se como hipótese de pesquisa a necessidade de parâmetros assentados na Lei, uma vez que o Poder Judiciário, como órgão julgador, não se restringe, tão somente, a regulamentos éticos os quais, inclusive, apresentam falhas em diversas questões que serão elucidadas no decorrer do presente artigo. Assim, as discussões acerca do referido assunto, vão além das benesses proporcionadas pelas Legaltechs, empresas especializadas em softwares jurídicos. A metodologia adotada consiste na abordagem dedutiva, realizada através da ampla revisão bibliográfica e análise documental alusivas ao tema abordado. Com base nisso, desprende-se no decorrer da pesquisa a evolução tecnológica, as diferentes abordagens do aprendizado de máquina, a crise numérica no judiciário e os impactos relativos ao uso da inteligência artificial nas decisões judiciais. Por fim, conclui-se que, apesar dos avanços significativos prestados pela Resolução 332/20 do Conselho Nacional de Justiça, pouco se contribuiu para o preenchimento das lacunas jurídicas na produção e governança desta nova tecnologia quando empregadas no Poder Judiciário.

A prática da adoção intuitu personae busca manter a criança ou adolescente na família que já está habituada e já criou laços. Desta forma o presente artigo aventa o seguinte questionamento: em que medida a flexibilização do cadastro nacional de adoção em razão do instituto da adoção intuitu personae garante o melhor interesse da criança e do adolescente? Para tanto, busca-se como objetivo geral analisar frente ao texto constitucional e infraconstitucional bem como aos princípios constitucionais, os reflexos da flexibilização do cadastro nacional de adoção em razão da adoção intuitu personae e como objetivos específicos, apresentar conceitos chaves, os princípios base e a evolução histórica da adoção. Delimita-se a pesquisa na constituição federal, legislação civil brasileira e direito de família. A técnica de pesquisa utilizada foi a documental qualitativa com análise de doutrina, legislação e jurisprudência brasileira e o método de pesquisa o dialético cuja aplicação se deu pela observação da situação fática atual, analisando as contradições que transcendem, momento no qual se trabalha as novas contradições e suas possíveis soluções. Conclui-se que a adoção intuitu personae, não é uma forma de burlar e muito menos desrespeitar o Cadastro Nacional de Adoção, mas sim, uma forma de agilizar a legalização da adoção de uma criança, a qual já está sob os cuidados de uma determinada família e com eles já criou laços de amor, afeto e carinho, garantindo o melhor interesse da criança ou adolescente.

O presente artigo discorre sobre o reconhecimento da família multiespécie dentro do ordenamento jurídico brasileiro. Busca-se apresentar o conceito de família, quais os tipos de família previstos expressamente na Constituição da República Federativa do Brasil de 1988 ou no Código Civil Brasileiro e quais os princípios norteadores do direito da família. Ademais, busca-se demonstrar que o atual conceito de família se baseia no princípio da afetividade e, portanto, a família multiespécie é uma categoria que merece proteção do ordenamento jurídico brasileiro. Demonstrando que apesar do reconhecimento da doutrina, o Poder Judiciário não reconhece a família multiespécie em face da ausência de previsão legal. O método utilizado para realização do presente trabalho foi o dedutivo, no qual já se considera a existência da família multiespécie, enquanto que a técnica de pesquisa foi baseada em pesquisa de jurisprudências e doutrinas. Desse modo, conclui-se que a família multiespécie é uma forma de se constituir família.

Basic wood density (BWD) is a functional characteristic important to quantify carbon storage and is related to tree growth and its survival. We investigated BWD variability among tree species, populations, and forest types across the Atlantic Forest biome in Rio de Janeiro state, Brazil. We sampled and measured 576 trees of 83 species in 28 natural ecosystems in the Dense Ombrophilous Forest, Seasonal Semi-deciduous Forest, and the Restinga Forest. Stem wood from two different heights at each tree was acquired using a drill borer; discs were removed from branches in 20% of the height from 39 trees. BWD means increased from 0.42 for the most humid ecosystem to 0.675 g⋅cm−3 from the driest ecosystem. We identified changes in the wood as a result of climatic variables particular to different typologies, such as increased wood density in areas with lower precipitation and higher temperatures, were also observed within species, but with little difference in means. We identified three groups of species according to their BWD, different in mean height and diameter. The branches can be used to estimate tree wood density.

A presente pesquisa objetiva abordar o tema herança digital, que na contemporaneidade vem sendo alvo de várias discussões devido ao fato da inexistência de legislação pertinente e ao grande aumento de tecnologia à disposição da sociedade. Uma vez existente tal situação vem à tona a preocupação com direitos fundamentais (Privacidade e Intimidade) ofendidos em caso de transmissão de bens que antes não eram assim reconhecidos, como o caso das contas nas redes sociais, moedas virtuais, produtos digitais em geral. A sociedade evolui, o uso da Internet amplia-se, novas formas de comunicação são criadas, novas mídias, bancos de dados, contas de e-mail, moedas virtuais, o que tem levado à criação de um novo tipo de patrimônio, o digital. Mas o que acontecerá com esse tipo de propriedade depois que seu dono falecer? O que se percebe é uma possível ofensa especificamente ao direito fundamental da intimidade e da privacidade, visto que não há elementos claros sobre a disseminação de informações que, em vida, podem ser consideradas íntimas. Trata-se de um artigo de cunho bibliográfico, explicativo e descritivo, utilizando-se como método de abordagem o dedutivo, tendo em vista que parte-se da premissa que há uma ofensa aos princípios mencionados quando o de cujus não autoriza a transmissão de sua herança digital. A questão de qual é o destino post-mortem do conteúdo digital adquirido ou gerado por uma pessoa durante sua vida requer uma resposta altamente matizada que evita a simplicidade e imprecisões. Até o momento, em âmbito nacional, nem o debate social nem o jurídico foram intensos o suficiente para se chegar a conclusões maduras que orientassem o trabalho do legislador. Dessa forma, este artigo objetivou investigar qual deve ser a destinação dos bens digitais para quando da morte de seu proprietário. Conclui-se que a herança digital é um novo marco no campo do Direito, face ao desenvolvimento das mídias digitais. Sendo necessária a autorização do falecido, a fim de resguardar seu direito de privacidade e intimidade em relação a sua herança digital.

Cerebral ischemia due to hypoxia is a major cause of secondary brain injury and is associated with higher morbidity and mortality in patients with acute brain injury. Hyperoxia could improve energetic dysfunction in the brain in this setting. Our objectives were to perform a systematic review and meta-analysis of the current literature and to assess the impact of normobaric hyperoxia on brain metabolism by using cerebral microdialysis.We searched Medline and Scopus, following the recommendations of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement; we searched for retrospective and prospective observational studies, interventional studies, and randomized clinical trials that performed a hyperoxia challenge in patients with acute brain injury who were concomitantly monitored with cerebral microdialysis. This study was registered in PROSPERO (CRD420211295223).We included a total of 17 studies, with a total of 311 patients. A statistically significant reduction in cerebral lactate values (pooled standardized mean difference [SMD] - 0.38 [- 0.53 to - 0.23]) and lactate to pyruvate ratio values (pooled SMD - 0.20 [- 0.35 to - 0.05]) was observed after hyperoxia. However, glucose levels (pooled SMD - 0.08 [- 0.23 to 0.08]) remained unchanged after hyperoxia.Normobaric hyperoxia may improve cerebral metabolic disturbances in patients with acute brain injury. The clinical impact of such effects needs to be further elucidated.

Fracture-dislocation of the trapeziometacarpal (TM) joint in adolescent patients is a rare injury, with only 3 cases reported in literature to our knowledge. Its low incidence, together with the complexity of the anatomy and biomechanics of TM joint, may represent a challenge for surgeons in choosing the best treating option. Here, we report a case of a TM fracture dislocation in a 14-year-old boy treated with percutaneous Kirschner wire pinning. The results we obtained endorsed our choice in patients for whom closed reduction is achievable.

No ano de 2020, o Brasil foi surpreendido pela pandemia de covid-19, momento no qual foi necessário o isolamento social que refletiu em diversos fatores, como o fechamento de escolas e posterior ensino remoto, e a crescente inclusão digital nas escolas brasileiras. A agenda 2030 da Organização das Nações Unidas (ONU) estabelece objetivos e metas para que os países alcancem um futuro sustentável, dentre eles, o objetivo número quatro preza por assegurar a inclusão da educação de qualidade, além de promover oportunidades de aprendizagem para todas as pessoas ao longo da vida. O presente estudo justifica-se em razão da importância de analisar o direito fundamental à educação e sua efetividade no cenário pandêmico, em relação ao cumprimento do objetivo que trata da educação na agenda 2030 da ONU, para tanto questiona-se: em que medida é possível garantir o cumprimento do objetivo 4 da Agenda 2030 frente à pandemia Covid-19 com a inclusão digital nas escolas brasileiras? O método de abordagem utilizado foi o indutivo, em razão do emprego de um caso concreto, em que é observado um fenômeno que está ocorrendo. A pesquisa é do tipo documental indireta bibliográfico-doutrinária. Como conclusão destaca-se que para o cumprimento ODS nº 04, é fundamental que os poderes públicos em âmbito municipal, estadual e federal criem projetos e estratégicas para o acesso integral da internet a todos os indivíduos, reduzindo custos e provendo uma internet de qualidade, sendo importante também, o uso consciente e correto dos referidos meios tecnológicos, sobretudo por parte dos alunos.

Gaucher disease type 3 is a chronic neuronopathic disorder with wide-ranging effects, including hepatosplenomegaly, anaemia, thrombocytopenia, skeletal disease and diverse neurological manifestations. Biallelic mutations in GBA1 reduce lysosomal acid β-glucosidase activity, and its substrates, glucosylceramide and glucosylsphingosine, accumulate. Enzyme replacement therapy and substrate reduction therapy ameliorate systemic features of Gaucher disease, but no therapies are approved for neurological manifestations. Venglustat is an investigational, brain-penetrant, glucosylceramide synthase inhibitor with potential to improve the disease by rebalancing influx of glucosylceramide with impaired lysosomal recycling. The Phase 2, open-label LEAP trial (NCT02843035) evaluated orally administered venglustat 15 mg once-daily in combination with maintenance dose of imiglucerase enzyme replacement therapy during 1 year of treatment in 11 adults with Gaucher disease type 3. Primary endpoints were venglustat safety and tolerability and change in concentration of glucosylceramide and glucosylsphingosine in CSF from baseline to Weeks 26 and 52. Secondary endpoints included change in plasma concentrations of glucosylceramide and glucosylsphingosine, venglustat pharmacokinetics in plasma and CSF, neurologic function, infiltrative lung disease and systemic disease parameters. Exploratory endpoints included changes in brain volume assessed with volumetric MRI using tensor-based morphometry, and resting functional MRI analysis of regional brain activity and connectivity between resting state networks. Mean (SD) plasma venglustat AUC0-24 on Day 1 was 851 (282) ng•h/ml; Cmax of 58.1 (26.4) ng/ml was achieved at a median tmax 2.00 h. After once-daily venglustat, plasma concentrations (4 h post-dose) were higher compared with Day 1, indicating ∼2-fold accumulation. One participant (Patient 9) had low-to-undetectable venglustat exposure at Weeks 26 and 52. Based on mean plasma and CSF venglustat concentrations (excluding Patient 9), steady state appeared to be reached on or before Week 4. Mean (SD) venglustat concentration at Week 52 was 114 (65.8) ng/ml in plasma and 6.14 (3.44) ng/ml in CSF. After 1 year of treatment, median (inter-quartile range) glucosylceramide decreased 78% (72, 84) in plasma and 81% (77, 83) in CSF; median (inter-quartile range) glucosylsphingosine decreased 56% (41, 60) in plasma and 70% (46, 76) in CSF. Ataxia improved slightly in nine patients: mean (SD, range) total modified Scale for Assessment and Rating of Ataxia score decreased from 2.68 [1.54 (0.0 to 5.5)] at baseline to 1.55 [1.88 (0.0 to 5.0)] at Week 52 [mean change: −1.14 (95% CI: −2.06 to −0.21)]. Whole brain volume increased slightly in patients with venglustat exposure and biomarker reduction in CSF (306.7 ± 4253.3 mm3) and declined markedly in Patient 9 (−13894.8 mm3). Functional MRI indicated stronger connectivity at Weeks 26 and 52 relative to baseline between a broadly distributed set of brain regions in patients with venglustat exposure and biomarker reduction but not Patient 9, although neurocognition, assessed by Vineland II, deteriorated in all domains over time, which illustrates disease progression despite the intervention. There were no deaths, serious adverse events or discontinuations. In adults with Gaucher disease type 3 receiving imiglucerase, addition of once-daily venglustat showed acceptable safety and tolerability and preliminary evidence of clinical stability with intriguing but intrinsically inconsistent signals in selected biomarkers, which need to be validated and confirmed in future research.

The aim of this systematic review is to understand which surgical procedure provides better results in terms of pain relief and function in the treatment of chronic exertional compartment syndrome (CECS) of the forearm. We searched Medline (PubMed), Web of Science, Embase and Scopus databases on 8 July 2020. Twelve studies were included in this review. We assessed the quality of the studies using the Coleman Methodological Score. Data on demographic features, operative readings, diagnostic methods, follow-up periods, type and rates of complications, survivorship of the procedure, return to sport activity, and outcome measures were recorded. In conclusion, compared to the other techniques, endoscopic fasciotomy delivers similar success rates and lower incidence of complications. Cite this article: EFORT Open Rev 2021;6:101-106. Este trabalho busca alimentar o necessário debate sobre a patenteabilidade dos programas de computador e das tecnologias imateriais. Em geral, a doutrina majoritária considera patenteável apenas o software necessário para viabilizar um efeito técnico em um hardware. O questionamento aventado é saber qual o planejamento jurídico ideal cumulado com as estratégias defensivas a serem adotadas pelo autor para salvaguardar o direito intelectual neste grande mercado. Como objetivo geral destaca-se analisar as estratégias eficazes de planejamento jurídico defensivo da propriedade intelectual contida nos programas de computadores durante sua fase de desenvolvimento, introdução ao mercado e distribuição. Para tanto como objetivos específicos busca-se evidenciar a importância das cláusulas contratuais sigilosas no desenvolvimento de softwares para possibilitar a futura defesa de seus direitos autorais, demonstrar as divergências ocasionadas pela aplicação de leis idênticas às obras literárias e musicais aos programas de computadores, discutir casos reais e precedentes. O método de pesquisa utilizado foi o dialético, o procedimento foi o comparativo visto que se analisa precedentes judiciais estrangeiros e brasileiros sobre o tema. A técnica de pesquisa é a documental indireta. Como conclusão destaca-se que a situação ascende a necessidade da adaptação do direito autoral, para capacitá-lo a atender as criações contemporâneas imateriais, ou a implementação destas criações na propriedade industrial. Tais tecnologias tornaram-se tão valiosas quanto o equipamento que lhes dá suporte.

The heartwood formation process is little known in fast growing plantation woods. Therefore, the aims of this study were to determine how planting spacing and tree age affect the formation and proportion of heartwood and sapwood, as well as the density of eucalyptus wood. Trees from a eucalyptus clonal plantation (Eucalyptus grandis × E. urophylla hybrid) cultivated in three spacings (3 × 1, 3 × 2 and 3 × 3 m) were sampled at 14, 27, 32, 53, 64 and 76 months of age. Heartwood percentage was quantified with Dimethyl yellow indicator, while the wood density was determined by X-ray densitometry. The heartwood percentage, wood volume, heartwood density and wood density were correlated with the different growth rates. The heartwood formation process started between 32 and 53 months, regardless of spacing. The heartwood proportion doubled with increasing age in the widest spacing and increased about four times in 3 × 1 and 3 × 2 m spacing. The planting spacing influenced the growth rates of the trees, but did not affect the heartwood and sapwood percentage or density. The greatest increase in density values occurred between the first year of growth (14 months) and the beginning of heartwood formation (53 months).

O ordenamento jurídico brasileiro contém uma lacuna em relação a decisão sobre a destinação dos restos mortais do “de cujus” quando não há vontade expressa deixada em vida. Assim, o presente artigo busca responder ao seguinte problema: “frente a ausência de codificação específica a um direito funeral na legislação brasileira a destinação dos restos mortais do “de cujus” deve ser analisada frente ao direito sucessório, ou ao direito das coisas?” Como objetivo geral busca-se analisar a quem compete decidir sobre o destino dos retos mortais do “de cujus” frente as teses da linha sucessória e do direito de propriedade no âmbito do direito funeral. Para tanto, destaca-se como objetivos específicos: relacionar os direitos inerentes ao “de cujus”, conhecer os institutos principais do direito sucessório e do direito das coisas, estabelecer ligações entre a linha sucessória e/ou o direito das coisas em relação a quem compete a tomada de decisão quanto ao destino dos restos mortais do “de cujus”. A presente pesquisa é documental indireta e o método de pesquisa utilizado é o dialético. Como conclusão apresenta-se que a afetividade é a determinante para a tomada de decisão em relação a destinação do “de cujus”.

O instituto de adoção é de extrema importância para assegurar que crianças e adolescentes que, por algum motivo, se encontram fora da convivência familiar, possam ter tal direito assegurado. Entretanto, analisando números fornecidos pelo Conselho Nacional de Justiça e posições doutrinárias, percebe-se que há certa deficiência nos processos que versam sobre a adoção. Portanto, o presente artigo tem como objetivo examinar os fatores que ocasionam a morosidade do judiciário brasileiro em relação aos processos de adoção e se existe a possibilidade de responsabilizar o Estado pela demora na prestação jurisdicional. Acerca da pesquisa, se utiliza a documental descritiva com análise de doutrina, legislação e jurisprudência brasileira com abordagem qualitativa. O método de pesquisa utilizado foi o dialético, considerando-se que os fatos não podem ser considerados fora de um contexto social, buscando analisar contradições que se transcendem e buscam soluções. Para compreensão do tema, é abordado a definição e desenvolvimento histórico da família e do instituto de adoção, ambos conceitos sofreram diversas transformações com o passar dos anos. A discussão reside, então, na possibilidade do Estado ser responsabilizado civilmente pela morosidade processual em ações de adoção. Como conclusão é possível afirmar que a invocação da hipótese de responsabilidade civil objetiva do Estado, é um dos únicos meios de garantir que os danos sofridos por adotandos, sejam reparados.

Background Dietary interventions have been previously explored in children with ADHD. Elimination diets and supplementation can produce beneficial behaviour changes, but little is known about the mechanisms mediating change. We propose that these interventions may work, in part, by causing changes in the gut microbiota. A microbiome-targeted dietary intervention was developed, and its feasibility assessed. Methods A non-randomised feasibility study was conducted on nine non-medicated children with ADHD, aged 8–13 years (mean 10.39 years), using a prospective one-group pre-test/post-test design. Participants were recruited from ADHD support groups in London and took part in the 6-week microbiome-targeted dietary intervention, which was specifically designed to impact the composition of gut bacteria. Children were assessed pre- and post-intervention on measures of ADHD symptomatology, cognition, sleep, gut function and stool-sample microbiome analysis. The primary aim was to assess the study completion rate, with secondary aims assessing adherence, adverse events (aiming for no severe and minimal), acceptability and suitability of outcome measures. Results Recruitment proved to be challenging and despite targeting 230 participants directly through support groups, and many more through social media, nine families (of the planned 10) signed up for the trial. The completion rate for the study was excellent at 100%. Exploration of secondary aims revealed that (1) adherence to each aspect of the dietary protocol was very good; (2) two mild adverse events were reported; (3) parents rated the treatment as having good acceptability; (4) data collection and outcome measures were broadly feasible for use in an RCT with a few suggestions recommended; (5) descriptive data for outcome measures is presented and suggests that further exploration of gut microbiota, ADHD symptoms and sleep would be helpful in future research. Conclusions This study provides preliminary evidence for the feasibility of a microbiome-targeted dietary intervention in children with ADHD. Recruitment was challenging, but the diet itself was well-tolerated and adherence was very good. Families wishing to trial this diet may find it an acceptable intervention. However, recruitment, even for this small pilot study, was challenging. Because of the difficulty experienced recruiting participants, future randomised controlled trials may wish to adopt a simpler dietary approach which requires less parental time and engagement, in order to recruit the number of participants required to make meaningful statistical interpretations of efficacy. The prognostic role of the Pupillary Pain Index (PPI), derived from automated pupillometry, remains unknown in post-anoxic brain injury.Single-center retrospective study in adult comatose cardiac arrest (CA) patients. Quantitative PPI and Neurologic Pupil Index (NPi) were concomitantly recorded on day 1 and day 2 after CA. The primary outcome was to assess the prognostic value of PPI to predict 3-month unfavourable outcome (UO, defined as Cerebral Performance Category of 3-5). Secondary outcome was the agreement between PPI and NPi to predict unfavourable outcome.A total of 102 patients were included; patients with UO (n = 69, 68%) showed a lower NPi (4.2 [3.5-4.5] vs. 4.6 [4.3-4.7]; p 0.01 on day 1-4.3 [3.8-4.7] vs 4.6 [4.3-4.8] on day 2), and PPI (3 [1-6] vs. 6 [3-7]; p 0.01 on day 1-3 [1-6] vs 6 [4-8]; p 0.01 on day 2) than others. A PPI = 1 on day 2 showed a sensitivity of 26 [95% CI 16-38]% and a specificity of 100 [95% CI 89-100]% to predict UO (p = 0.003 vs. NPi ≤ 2). On day 2, a total of 6 patients had concomitant PPI = 1 and NPi ≤ 2, while 12 showed NPi 2 and PPI = 1; the coefficient of agreement was 0.42. Moreover, NPi and PPI values showed a moderate correlation both on day 1 and day 2.In this study, PPI = 1 on day 2 could predict UO in comatose CA patients with 100% specificity, but with a low sensitivity (yet higher than NPi). The agreement between PPI and NPi values was moderate.

The widely recognized problems of pharmacological strategies based on killing cancer cells demand a rethink of therapeutic approaches. Tumor reversion strategies that aim to shift cancer cells to a healthy differentiated state are a promising alternative. Although many studies have firmly demonstrated the possibility of reverting cancer to a normal differentiated state, we are still unable (with the exception of retinoic acid in a form of leukemia) to revert cancer cells to a stable differentiated healthy state. Here, we review the main biological bases of redifferentiation strategies and provide a description of the most promising research avenues.

The development of more efficient and environmentally friendly analytical methods represents a current focus for the fine chemical industry. In particular, microscale methodologies that are free of solvents/reagents. The headspace-GC/MS (HS-GC) methodology was employed in this study as a tool for monitoring a biocatalysed reaction of (4S)-(+)-carvone using Phoma sp., a filamentous fungus from human skin. Biocatalysis provides some advantages, such as high efficiency, high degrees of regioselectivity, chemoselectivity, and enantioselectivity. In order to optimize the small scale biocatalytic reaction of the (4S)-(+)-carvone by the filamentous fungus Phoma sp. was used headspace GC/MS methodology, factorial design of experiments and the response surface methodology (RSM) was performed using the biomass of the fungus, substrate mass and pH as parameters. It was observed that for all reactions conditions tested, forming the products (1 R,4S)-dihydrocarvone and (1S,4S)-dihydrocarvone. The most influential factor was pH, with the highest conversion rate (>95%) and diastereomeric excess (d.e.) (>80%) obtained at pH 5.0. Thus, it was demonstrated that human skin Phoma sp. fungus showed significant bioreduction activity and that headspace GC/MS is an efficient approach for real-time monitoring the biocatalysed reactions.

O presente artigo aborda a questão jurídica relacionada ao direito sucessório do filho concebido após a morte do pai. O problema proposto é saber que respaldo jurídico e principiológico brasileiro a criança gerada por meio da inseminação artificial homóloga post mortem possui. Como hipótese aventa-se que a interpretação análoga do artigo 1.597 do código civil (CC) juntamente com a interpretação extensiva do art. 1.799, CC, garante ao filho concebido via inseminação artificial post mortem o direito sucessório e as garantias legais. O objetivo geral consiste em analisar as possibilidades do direito sucessório ao concebido pelo método de inseminação artificial homóloga post mortem. Como objetivos específicos destacam-se: abordar de forma breve o direito das sucessões, principais conceitos (morte entre outros) e seus princípios norteadores; explanar sobre o planejamento familiar e discorrer sobre a reprodução assistida apresentando o primeiro caso no qual houve julgamento procedente do pedido de a autora realizar a inseminação artificial homóloga post mortem com os sêmens do cônjuge falecido. Como metodologia tem-se a análise sistemática dos artigos 1.597 e 1.799 ambos do Código Civil cumulativamente com os princípios constitucionais, bem como uma revisão bibliográfica com análise doutrinária e jurisprudencial, utilizando-se para tanto o método dedutivo. Como conclusão destaca-se que a hipótese restou confirmada visto que é necessária uma interpretação análoga e extensiva dos dispositivos estudados para garantir o direito sucessório nos casos de reprodução artificial post-mortem.

Sepsis-associated brain dysfunction is a frequent disorder in septic patients and has a multifactorial pathophysiology. Cholinergic pathways and brainstem dysfunction may result in pupillary alterations. The aim of this study was to evaluate whether early assessment of the Neurological Pupil Index (NPiRetrospective cohort study of adult critically ill septic patients admitted to the intensive care unit of a University Hospital; patients with acute or known brain damage were excluded. Patients' severity was assessed by the daily Sequential Organ Failure Assessment Score and the SOFAinfmax/inf(i.e., highest SOFA Score during the first five days) was computed. The worst NPi (i.e., lowest value from one eye) was collected daily and then computed over the first five days of assessment. Mortality was assessed at hospital discharge.A total of 75 patients were included over the study period (median age 67 [53-75] years and median SOFA Score at admission 10 [8-12]); 64 (85%) presented septic shock; 48 (64%) died at hospital discharge. The worst NPi during the first five days of sepsis was significantly lower in non-survivors compared to survivors (4.4 [3.6-4.6] vs. 4.5 [4.2-4.7]; P=0.042). The worst NPi was also significantly lower in high severity group (i.e., SOFAinfmax/inf≥12) when compared to others (4.4 [3.2- 4.5] vs. 4.5 [4.0-4.7] P=0.01). However, in the multivariate analyses, the NPi value was not independently associated with in-hospital mortality or high SOFAinfmax/inf.In this study, no independent prognostic role of NPi was observed in septic patients. Further larger prospective studies are needed to better evaluate the role of automated pupillometry in this setting.

High levels of procalcitonin (PCT) have been associated with a higher risk of mortality in COVID-19 patients. We explored the prognostic role of early PCT assessment in critically ill COVID-19 patients and whether PCT predictive performance would be influenced by immunosuppression.Retrospective multicentric analysis of prospective collected data in COVID-19 patients consecutively admitted to 36 intensive care units (ICUs) in Spain and Andorra from March to June 2020. Adult (18 years) patients with confirmed COVID-19 and available PCT values (72 hours from ICU admission) were included. Patients were considered as "no immunosuppression" (NI), "chronic immunosuppression" (CI) and "acute immunosuppression" (AIT if only tocilizumab; AIS if only steroids, AITS if both). The primary outcome was the ability of PCT to predict ICU mortality.Of the 1079 eligible patients, 777 patients were included in the analysis. Mortality occurred in 227 (28%) patients. In the NI group 144 (19%) patients were included, 67 (9%) in the CI group, 66 (8%) in the AIT group, 262 (34%) in the AIS group and 238 (31%) in the AITS group; PCT was significantly higher in non-survivors when compared with survivors (0.64 [0.17-1.44] vs. 0.23 [0.11-0.60] ng/mL; P0.01); however, in the multivariable analysis, PCT values was not independently associated with ICU mortality. PCT values and ICU mortality were significantly higher in patients in the NI and CI groups.PCT values are not independent predictors of ICU mortality in COVID-19 patients. Acute immunosuppression significantly reduced PCT values, although not influencing its predictive value.

We study the problem of learning good heuristic functions for classical planning tasks with neural networks based on samples that are states with their cost-to-goal estimates. It is well known that the learned model quality depends on the training data quality. Our main goal is to understand better the influence of sample generation strategies on the performance of a greedy best-first heuristic search guided by a learned heuristic function. In a set of controlled experiments, we find that two main factors determine the quality of the learned heuristic: the regions of the state space included in the samples and the quality of the cost-to-goal estimates. Also, these two factors are interdependent: having perfect estimates of cost-to-goal is insufficient if an unrepresentative part of the state space is included in the sample set. Additionally, we study the effects of restricting samples to only include states that could be evaluated when solving a given task and the effects of adding samples with high-value estimates. Based on our findings, we propose practical strategies to improve the quality of learned heuristics: three strategies that aim to generate more representative states and two strategies that improve the cost-to-goal estimates. Our resulting neural network heuristic has higher coverage than a basic satisficing heuristic. O presente trabalho concentra-se nos Refugiados que deixam seus países de origem ou residência e buscam uma nova vida em outro. Como problema destaca-se o questionamento: como são garantidos os direitos humanos dos refugiados na atualidade (século XXI) no Brasil? Como hipótese aventa-se que apesar do Instituto do Refúgio ter se estagnado no final do século XX, atualmente com a evolução legislativa e políticas públicas o Brasil vem buscando meios de reforçar a proteção do direito humanitário aos refugiados. O objetivo geral consiste em analisar se os direitos humanos estão sendo garantidos aos refugiados que chegam ao Brasil neste século. Os objetivos específicos são apresentar brevemente a evolução histórica dos direitos humanos a nível mundial, a legislação brasileira em relação ao refúgio, bem como o órgão orientador e coordenador de proteção ao refugiado (CONARE) e elucidar o desenvolvimento dos direitos humanos e as medidas tomadas a partir do século XXI para a proteção e o amparo dos refugiados no Estado Brasileiro. Os conceitos a serem utilizados têm como marco teórico o direito internacional e nacional dos refugiados, a doutrina e legislação internacional e brasileira sobre o tema. A metodologia utilizada foi a revisão bibliográfica, utilizando-se como delimitações: o espaço temporal do século XXI e o espaço geográfico dos refugiados que buscam se estabelecer no Brasil.

Chagas disease is caused by the protozoan Trypanosoma cruzi (T. cruzi). It remains the major parasitic disease in Latin America and is spreading worldwide, affecting over 10 million people. Hundreds of new compounds with trypanosomicidal action have been identified from different sources such as synthetic or natural molecules, but they have been deficient in several stages of drug development (toxicology, scaling-up, and pharmacokinetics). Previously, we described a series of compounds with simple structures, low cost, and environmentally friendly production with potent trypanosomicidal activity in vitro and in vivo. These molecules are from three different families: thiazolidenehydrazines, diarylideneketones, and steroids. From this collection, we explored their capacity to inhibit the triosephosphate isomerase and cruzipain of T. cruzi. Then, the mechanism of action was explored using NMR metabolomics and computational molecular dynamics. Moreover, the mechanism of death was studied by flow cytometry. Consequently, five compounds, 314, 793, 1018, 1019, and 1260, were pre-clinically studied and their pharmacologic profiles indicated low unspecific toxicity. Interestingly, synergetic effects of diarylideneketones 793 plus 1018 and 793 plus 1019 were evidenced in vitro and in vivo. In vivo, the combination of compounds 793 plus 1018 induced a reduction of more than 90% of the peak of parasitemia in the acute murine model of Chagas disease.

Introdução: A endocardite da válvula prostética (EVP) é uma infecção cardíaca do revestimento interno do coração nas válvulas cardíacas substituídas que são denominadas válvulas prostéticas. A EVP, um dos ramos da classificação de endocardite, possui subdivisões que dizem respeito ao tempo e ao procedimento clínico que o paciente tenha sido submetido previamente à infecção, e desta maneira traçam perfis microbiológicos distintos. Objetivo: Analisar em vies etiologico e epidemiologico a endocardite infecciosa da válvula prostética. Material e métodos: Foram utilizados os métodos de leitura crítica e de pesquisa bibliográfica. As análises foram realizadas através das principais fontes de bancos de dados e artigos científicos, entre eles: scholar google, SciELO, PubMed. Resultados: Cerca de 20% de todos os casos de endocardite se tratam de EVP e ocorre em cerca de 6% das pessoas que tem válvula prostética. A epidemiologia de EVP muda se a substituição da válvula foi por via cirúrgica ou por procedimento de implante transcateter, bem como se a infecção for precoce ou tardia. Em procedimentos cirúrgicos, a literatura revela que cerca de 76,7% dos casos de EVP ocorrem dentro de 120 dias após a cirurgia, enquanto o tempo mediano do reconhecimento e diagnóstico de PVE em procedimento transcateter é de 5 meses. Em quase todos os casos, bactérias Staphylococcus spp são os principais responsáveis pelas infecções que acarretam em EVP, seguido das bactérias Enterococci spp especificamente em casos de substituição da válvula aórtica via transcateter. A grande maioria dos afetados são mais comumente do sexo masculino, com idades variantes de 10 a 80 anos. EVP é comumente associada com grande chances óbito, atingindo porcentagens de até 34,4% de mortalidade, sendo a complicação que apresenta maior taxa de letalidade entre os tipos de endocardite infecciosa. Conclusão: Entende-se que a endocardite se encontra, ainda, caracterizada como uma doença relativamente incomum, entretanto, se apresemta em viés extremamente complexo, de forma a ser considerada uma doença de grande risco à vida.

Chronic exertional compartment syndrome of the forearm is a rare disease characterized by the recurrent onset of forearm pain and progressive weakness during exertion. Over the years many surgical techniques have been reported in literature: wide-open fasciotomy, mini-open fasciotomy, fasciotomy associated with fasciectomy, and mini-invasive endoscopic fasciotomy. Compared with traditional open techniques, mini-invasive techniques are characterized by minimal tissue trauma, less postoperative pain, faster recovery to normal activity and better esthetical result. We describe a mini-invasive endoscopic technique for the treatment of forearm chronic exertional compartment syndrome using the SmartRelease endoscopic tissue release system (MicroAire).

The aim of the present study is to discuss the imposition of the regime of separation of matrimonial property to those above seventy years old. The problem that we have proposed is to show how this limitation of choice imposed by article 1.641, II of Brazilian Civil Code might hurt Constitutional Principles. The methodology we have used was bibliographical and case-laws review. The criteria we have established to this work were the age of seventy to the engaged couple (bride or groom), Brazilian civil legislation, Brazilian case-laws and Brazilian Federal Constitution. By means of the analysis it is clear that a person with total discernment must not have restricted his or hers right of choice by the simple fact of being elderly. It is of great significance to emphasize that the said imposition to those above seventy years old violates one of the most valuable rights of the human life which is the autonomy of choosing their acts as well as their dignity among other Constitutional Principles. The raised hypothesis is at the end confirmed since it is unconstitutional the said imposition so much that the majority of the legal theory as well as some case-laws are already deciding and positioning in terms of the unconstitutionality of the said article 1.641 II of the Brazilian Civil Code.

Humanity finds itself in a period of great challenges. The memories need to be answered, because they are constant as transformations to face the century. The human catastrophes that live annually are not of a variable nature, but an urban phenomenon, with capacity to project and disrespect that allows the accomplishment of an environmentally integrated water project, the tributary of the Rio Santo Anastácio stream located in Presidente Prudente - SP. The methodology used is the choice of the area with a minimum of 1 km² of area and is a micro basin, an area demarcated through the use of Google Earth Pro software, where the hydrological aspects, the environmental impact and the basic sanitation were submitted. The results were similar to a basin of studied streams with high levels of degradation due to the silting and poor use of the soil of the surrounding properties, with few permanent preservation areas preserved, besides the irregular waste disposal. In this way, the results can be more favorable in relation to the question, and the same results corroborate the studies carried out in the streams of Cedro and Cedrinho

The deregulation of PI3K/Akt signaling is among the most causes in inducing the acquisition of a metastatic phenotype in breast cancer cells, leading to Epithelial-Mesenchymal Transition (EMT). Inhibition of the PI3K/Akt pathway is known to be beneficial in the clinical setting. However, the activation of secondary pathways and toxicity profiles of available inhibitors, hindering optimal therapeutic results. Preliminary studies showed that myo-Inositol inhibits the PI3K/Akt pathway by exerting a pleiotropic anti-tumor action.
