Omental torsion leading on to omental infarction is an unusual cause of acute abdominal pain in adults. Often the condition mimics common causes of acute abdomen like acute cholecystitis, acute appendicitis or acute pancreatitis. A review of literature reveals that this enigmatic condition has been managed both non - operatively and by surgery in the past. We report the case of a 46 - year - old man who presented with a 4 - day history of severe right - sided abdominal pain mimicking acute cholecystitis. Abdominal CT scan revealed a right upper quadrant mass with a whirl - like appearance, suspicious for omental infarction. He was started on conservative management with analgesics and antibiotics. He improved symptomatically and was discharged

The explosive growth of the Internet traffic requires the design of next generation switching systems accommodating huge data-centric traffic at the packet level. All-optical packet switching (OPS) with greater flexibility, granularity and switching capacity is able to meet the complexity of traffic. An all-optical fiber-optic switching device maintains the signal as light, from input to output. Semiconductor optical amplifiers (SOAs) are widely studied as nonlinear elements for high bit rate all-optical switching applications, such as wavelength conversion and regeneration. All optical switches incorporating SOAs have peculiarities like small size, low switching energy requirement and high non linearity. Here simulation of a 4 x 4 optical packet space switch array based on Optically Amplified Suppressed Interference Switches (OASIS) with different data rate has been performed. The OptiSystem version-12 simulator is used to model the structure, it is also used to assess the behavior and performance of this switch array. The simulation of 4x4 space switch array with different bit rate and for different modulation format is done which gives the BER within the acceptable range. Implications of cascadability of this switch array will be analysed which improves the quality and capacity of the existing networks

This study reports the knowledge and perceptions of the people with regard to cause and transmission of chronic forms of lymphatic filariasis (LF) and its elimination programme during mass drug administration (MDA) under the programme to eliminate LF in an urban area of Orissa, India. The paper is based on both quantitative and qualitative surveys undertaken before and after the first MDA among different socio-economic strata of the urban area. The data revealed that the urban residents are aware of the chronic forms of LF and also felt that LF is a problem in their area. But most of them do not know the role of mosquitoes in the transmission of the disease. A few people explained the association between LF and hydrocele, and the role of mosquitoes. In the present study, it is observed that poor knowledge about LF is highly influenced by socioeconomic conditions. People from lower economic category were least aware of the cause of both lymphoedema and hydrocele. However, knowledge in some issues with regards to the disease and the awareness on MDA have increased significantly after MDA.

p53 aberrations are early events in the pathogenesis of betel- and tobacco-related oral malignancies. Accumulation of p53 protein in oral lesions may elicit a humoral immune response against p53 protein in these patients. p53 antibodies (Abs) were analyzed in 183 sera obtained from patients with premalignant or malignant oral lesions and normal individuals by enzyme-linked immunoassay using recombinant p53 protein as antigen. These results were correlated with accumulation of p53 protein in patients' matched oral tissue specimens. Circulating p53 Abs were observed in 24 of 70 (34%) cancer patients and 15 of 50 (30%) patients with premalignant oral lesions. p53 Abs showed a significant association with increase in tumor size and dedifferentiation of tumors, factors indicative of poor prognosis. Expression of p53 protein was analyzed in 43 matched oral lesions (18 premalignant and 25 malignant cases). All the p53-seropositive patients (7 leukoplakia and 11 squamous cell carcinoma) showed elevated levels of p53 protein in matched oral lesions. However, the total number of patients seropositive for p53 Abs was lesser than that of patients exhibiting p53 protein accumulation in oral lesions. Four of the 63 normal healthy individuals who were heavy consumers of tobacco (smoking/chewing) and betel were found to be positive for p53 Abs. Detection of circulating p53 Abs in patients with premalignant oral lesions suggests that humoral immune response against p53 protein is an early event in oral oncogenesis and may be a surrogate marker for both p53 alteration and preclinical cancer.

The level of Superoxide anion was found to be significantly elevated in polymorphonuclear leukocytes (PMNL) from diabetic subjects as compared with those from normal subjects. This elevation was attributed to the significant reduction in the activities of both cytoplasmic and mitochondrial Superoxide dismutase (SOD), the effect being more pronounced in the cytoplasmic fraction. Although the content of copper decreased considerably in the diabetic PMNL, the decrease in the zinc content was less significant, with an insignificant alteration in the content of manganese. PMNL obtained from insulin-treated diabetic patients showed considerable alleviation of SOD levels. The implication of these results are discussed herein.

The level of reduced glutathione in diabetic polymorphonuclear leukocytes was found to be significantly decreased compared to normal. Although the activity of glutathione reductase remained unchanged, the activity of glutathione peroxidase was found to be decreased in the diabetic state. Ketosis was not found to additionally aggravate the glutathione system. Polymorphonuclear leukocytes obtained from insulin-treated patients showed significant restoration.

Infective osteomyelitis of mastoid temporal bone was a frequent complication of middle ear disease before the advent of antibiotics and chemotherapy, but now it is quite rare (Kelemen, 1957, Misra & Prakash, 1968). Tubercular osteomyelitis is rarer still (Shambaugh, 1959). A case of tubercular osteomyelitis of the mastoid temporal bone is reported.
