Dit is niet optimaal om de uitdagingen ten gevolge van de vergrijzing en de toename van chronische ziekten het hoofd te bieden. Het is nodig om te evolueren naar geïntegreerde zorg. We verstaan hieronder een zorgsysteem dat beter afgestemd is op de multi-dimensionele noden van mensen met chronische aandoeningen, dat rekening houdt met de behoeften van mensen gedurende hun hele leven, en dat over de verschillende zorglijnen heen. In België lopen er hierover al verschillende initiatieven, zowel op federaal niveau als op het niveau van de deelstaten. Maar de actoren op het terrein hebben de indruk dat deze nog niet voldoende gecoördineerd worden. Background Rapid response systems aim to achieve a timely response to the deteriorating patient; however, the existing literature varies on whether timing of escalation directly affects patient outcomes. Prior studies have been limited to using ‘decision to admit’ to critical care, or arrival in the emergency department as ‘time zero’, rather than the onset of physiological deterioration. The aim of this study is to establish if duration of abnormal physiology prior to critical care admission [‘Score to Door’ (STD) time] impacts on patient outcomes. Methods A retrospective cross-sectional analysis of data from pooled electronic medical records from a multi-site academic hospital was performed. All unplanned adult admissions to critical care from the ward with persistent physiological derangement [defined as sustained high National Early Warning Score (NEWS) > / = 7 that did not decrease below 5] were eligible for inclusion. The primary outcome was critical care mortality. Secondary outcomes were length of critical care admission and hospital mortality. The impact of STD time was adjusted for patient factors (demographics, sickness severity, frailty, and co-morbidity) and logistic factors (timing of high NEWS, and out of hours status) utilising logistic and linear regression models. Results Six hundred and thirty-two patients were included over the 4-year study period, 16.3% died in critical care. STD time demonstrated a small but significant association with critical care mortality [adjusted odds ratio of 1.02 (95% CI 1.0–1.04, p = 0.01)]. It was also associated with hospital mortality (adjusted OR 1.02, 95% CI 1.0–1.04, p = 0.026), and critical care length of stay. Each hour from onset of physiological derangement increased critical care length of stay by 1.2%. STD time was influenced by the initial NEWS, but not by logistic factors such as out-of-hours status, or pre-existing patient factors such as co-morbidity or frailty. Conclusion In a strictly defined population of high NEWS patients, the time from onset of sustained physiological derangement to critical care admission was associated with increased critical care and hospital mortality. If corroborated in further studies, this cohort definition could be utilised alongside the ‘Score to Door’ concept as a clinical indicator within rapid response systems.

To date, there has not been a systematic assessment of the status of first response systems in Nepal. An online survey was cascaded through government, non-governmental organisations and academic networks to identify first response providers across Nepal. Identified organisations were invited to complete a questionnaire to explore the services, personnel, equipment, and resources in these organisations, their first aid training activities and whether the organisation evaluated their first response services and training. Of 28 organisations identified, 17 (61%) completed the questionnaire. The range of services offered varied considerably; 15 (88.2%) provided first aid training, 9 (52.9%) provided treatment at the scene and 5 (29.4%) provided full emergency medical services with assessment, treatment and transport to a health facility. Only 8 (47.1%) of providers had an ambulance, with 6 (35.3%) offering transportation without an ambulance. Of 13 first aid training providers, 7 (53.8%) evaluated skill retention and 6 (46.2%) assessed health outcomes of patients. The length of a training course varied from 1 to 16 days and costs from US$4.0 to 430.0 per participant. There was a variation among training providers in who they train, how they train, and whether they evaluate that training. No standardisation existed for either first aid training or provision of care at the scene of an injury. This survey suggests that coordination and leadership will be required to develop an effective first response system across the country. Resumen Objetivo Describir el proceso de planificacion y ejecucion de un estudio masivo de seroprevalencia para SARS-CoV-2 en los profesionales del Hospital Universitario Fundacion Alcorcon (HUFA) (Espana). Metodo Se efectua una descripcion del plan disenado y desarrollado en el HUFA para la realizacion de las extracciones de las muestras para serologia en el total de los profesionales que prestan servicio en el hospital, entre el 14 y el 29 de abril del 2020. Se lleva a cabo un analisis descriptivo de la participacion de los profesionales en el estudio. De igual manera, se planificaron zonas de extraccion, personal sanitario y tiempos de ejecucion. Se asignaron 2.326 extracciones a personal sanitario, el resto fueron a trabajadores de las empresas externas. Resultados Participaron en el estudio 2.641 trabajadores (90,5%) de 2.918 candidatos. La categoria profesional que mas se analizo globalmente fue la de enfermeria con un 28,3% (n = 590). El porcentaje de cumplimiento de la organizacion de los horarios fue del 28,6%. Se planifico hasta un maximo de 298 extracciones diarias. El dia con mas afluencia fue el cuarto con 399 extracciones. Conclusiones El hecho de organizar un dispositivo tan grande con un alcance al 100% de los trabajadores que prestan servicio en el hospital, y con una respuesta tan mayoritaria por parte de ellos, ha permitido que el estudio de seroprevalencia realizado obtenga unos resultados con alta fiabilidad. Aunque el porcentaje de participacion fue muy alto, el grado de cumplimiento de la planificacion fue reducido.

Background Physician turnover is a concern in many health care systems globally. A better understanding of physicians’ reasons for leaving their job may inform organisational policies to retain key personnel. The aim of this study was to investigate hospital physicians’ intention to leave their current job, and to investigate if such intentions are associated with how physicians assess their leaders and the organisational context. Methods Data was derived from a survey of 971 physicians working in public hospitals in Norway in 2016. The data was analysed using descriptive statistics and multivariate analysis. Results We found that 21.0% of all hospital physicians expressed an intention to leave their current job for another job. An additional 20.3% of physicians had not made up their mind whether to stay or leave. Physicians’ perceptions of their leaders and the organisational context influence their intention to leave their hospital. Respondents who perceived their leaders as professional-supportive had a significantly lower probability of reporting an intention to leave their job. The analysis suggests that organisational context, such as department mergers, weigh in on physicians’ considerations about leaving their current job. Social climate and commitment are important reasons why physician stay. Conclusions A professional-supportive leadership style may have a positive influence on retention of physicians in public hospitals. Further research should investigate how retention of physicians is associated with performance related to organisational and leadership style.

Resumen: Este trabajo describe el proceso de revisión del catálogo de servicios de salud pública de la ciudad de Barcelona para responder a la reducción presupuestaria producida a partir de 2010 en las administraciones públicas en España. Se basó en un grupo de trabajo que revisó las diferentes líneas de actividad, teniendo en cuenta los gastos y los ingresos asociados y su razón de ser, valorando factores como la obligación legal de desempeño, la financiación finalista, la petición explícita por las administraciones fundadoras y la existencia o no de otros actores capaces de asumirla. Se describen los cambios y sus consecuencias. Se valora que el nuevo catálogo está consolidado y se considera ratificado por los órganos de gobierno, que se han renovado tras los relevos políticos. Creemos que esto es fruto de haberlo basado en el consenso profesional y los criterios de gestión, claves para el buen funcionamiento de un organismo autónomo ejecutivo de carácter público. We conclude that this is because it was based on professional consensus and management criteria, which are key for the smooth operation of a public autonomous executive organisation. OBJECTIVE: We aim to describe medical services provided by Nordic general practitioners (GPs), and to explore possible differences between the countries.DESIGN AND SETTING: We did a comparative analysis of selected data from the Nordic part of the study Quality and Costs of Primary Care in Europe (QUALICOPC).SUBJECTS: A total of 875 Nordic GPs (198 Norwegian, 80 Icelandic, 97 Swedish, 212 Danish and 288 Finnish) answered identical questionnaires regarding their practices.MAIN OUTCOME MEASURES: The GPs indicated which equipment they used in practice, which procedures that were carried out, and to what extent they were involved in treatment/follow-up of a selection of diagnoses.RESULTS: The Danish GPs performed minor surgical procedures significantly less frequent than GPs in all other countries, although they inserted IUDs significantly more often than GPs in Iceland, Sweden and Finland. Finnish GPs performed a majority of the medical procedures more frequently than GPs in the other countries. The GPs in Iceland reported involvement in a more narrow selection of conditions than the GPs in the other countries. The Finnish GPs had more advanced technical equipment than GPs in all other Nordic countries.CONCLUSIONS: GPs in all Nordic countries are well equipped and offer a wide range of medical services, yet with a substantial variation between countries. There was no clear pattern of GPs in one country doing consistently more procedures, having consistently more equipment and treating a larger diversity of medical conditions than GPs in the other countries. However, structural factors seemed to affect the services offered.

IntroductionFinancing of primary healthcare (PHC) is the key to the provision of equitable universal care. We aimed to identify and prioritise the perceived needs of PHC practitioners and researchers for new research in low- and middle-income countries (LMIC) about financing of PHC.MethodsThree-round expert panel consultation using web-based surveys of LMIC PHC practitioners, academics and policy-makers sampled from global networks. Iterative literature review conducted in parallel. First round (Pre-Delphi survey) elicited possible research questions to address knowledge gaps about financing. Responses were independently coded, collapsed and synthesised to two lists of questions. Round 2 (Delphi Round 1) invited panellists to rate importance of each question. In Round 3 (Delphi Round 2), panellists ranked questions in order of importance.ResultsA diverse range of PHC practitioners, academics and policy-makers in LMIC representing all global regions identified 479 knowledge gaps as potentially critical to improving PHC financing. Round 2 provided 31 synthesised questions on financing for rating. The top 16 were ranked in Round 3e to produce four prioritised research questions.ConclusionsThis novel exercise created an expansive and prioritised list of critical knowledge gaps in PHC financing research questions. This offers valuable guidance to global supporters of primary care evaluation and implementation, including research funders and academics seeking research priorities. The source and context specificity of this research, informed by LMIC practitioners and academics on a global and local basis, should increase the likelihood of local relevance and eventual success in implementing the findings.

In Italy, there are 19 regions and two autonomous provinces, and each of these 21 jurisdictions has local responsibility for health care. As regards the COVID-19 pandemic, these 21 jurisdictions have been classified into three classes of risk (mild, moderate, and high) associated with three colours (green/yellow, orange, and red). More than 20 indicators are routinely collected in these jurisdictions, and this information determines, every 2 weeks, the colour assigned and the consequent restrictions in everyday life. This study analyses which administrative body local councils use to carry out their basic public health responsibilities. The study sample includes data from municipalities with less than 10,000 residents, which we believe is a first for studies published in academic journals in Spain. The data used was obtained by means of a survey administered by trained personnel. 93.7% of all the municipalities in the province of Girona, the area under study, responded to the survey. The analysis shows that there is a statistically significant difference between municipalities with more and less than 10,000 residents with regards to which administrative body local councils use for managing public health responsibilities. The results of this study suggest that in the ongoing debate over the streamlining of local government, the current situation regarding public health responsibilities in municipalities with less than 10,000 residents needs to be taken into account. Este trabajo analiza cuál es el ente administrativo de referencia al que acuden los ayuntamientos para el desarrollo de sus competencias mínimas en salud pública. La muestra del estudio incorpora, creemos que por primera vez en los estudios académicos en Espana, ¿ datos de municipios de menos de 10.000habitantes. Los datos provienen de un cuestionario administrado por personal entrenado y se obtuvo respuesta del 93,7% de los municipios de la provincia de Girona. El análisis muestra que la gestión municipal de las competencias de salud pública difiere de forma estadísticamente significativa, en cuanto al ente de referencia al que acuden los ayuntamientos, entre los municipios de más y de menos de 10.000 habitantes. Los resultados indican la necesidad de tener en cuenta la situación actual del desarrollo de las competencias municipales en salud pública en los municipios de menos de 10.000 habitantes en el debate sobre la racionalización de la Administración local.

The management of chronic diseases is a prime challenge of most 21st century health care systems. Many Western countries have invested heavily in care plans oriented towards specific conditions and diseases, such as dementia and cancer. The major downside of this narrowly focused approach is that treatment of multimorbidity is ignored. This paper describes the development and main stance of a national position that proposes streamlined reforms of the Belgian health care system to improve care for patients with multiple chronic diseases. We used a combination of methods to develop this stance: literature review and stakeholders’ consultation. The latter identified areas for improvement: efficiency of the health care system, coordination of care, investments in human care resources, informal caregivers’ support, better accessibility, and changes in the financial payment system. The position paper list 20 recommendations that are translated into about 50 action points to reform the health care system. Chronic care tailored to the patient's needs, including implementation of multidisciplinary teamwork, new functions, task delegation in primary care, and empowerment of the patient and informal caregivers are some major areas discussed. In addition, improved support, revised payment mechanisms, and setting up a quality system, along with the tailoring of patient care, can all facilitate delivery of high quality care in patients with chronic comorbidities.

The best systems create little extra work for staff and can be implemented almost without thinking in the course of a busy day. The system used in the stock room of our eye clinic in Ivory Coast illustrates this well.

In 2008 werd het nationaal programma “Prioriteit aan de chronisch zieken” gelanceerd, dat sterk de nadruk legde op goede informatie en op de toegankelijkheid van de zorg. In een volgende stap vroeg de minister van Volksgezondheid het Federaal Kenniscentrum voor de Gezondheidszorg (KCE) om samen met de FOD Volksgezondheid en het RIZIV een toekomstvisie voor de chronische zorg te ontwikkelen. Het KCE bestudeerde de internationale, wetenschappelijke literatuur, en keek naar het werk dat in binnen-en buitenland reeds rond chronische zorg gebeurde. Het betrok ook uitgebreid de mensen van het terrein, het beleid en de koepels van patiëntenverenigingen bij het onderzoek. 15 p. ill. Er is geen enkel wetenschappelijk bewijs dat homeopathie werkt. Toch wordt het veel gebruikt voor allerlei kwalen. De positieve ervaringen van de gebruikers kunnen te maken hebben met het ‘placebo-effect’, dus omdat patiënt en therapeut een effect verwachten. Tegelijk is er wel een risico dat men een noodzakelijke klassieke medische behandeling niet of te laat opstart, door een gemiste of verkeerde diagnose. Daarom wordt de uitoefening van homeopathie best beperkt tot artsen, die nu al de meerderheid van de homeopaten uitmaken. Dit concludeert het Federaal Kenniscentrum voor de gezondheidszorg (KCE) in zijn derde studie over de situatie van niet-conventionele therapieën in België.
