2. Modular construction and circularity. A case study of a mass timber design studio

The increased use of wood-based materials such as CLT (Cross Laminated Timber) can reduce the GHG emissions of the construction sector. Likewise, offsite and modular construction methods can lead to more efficient material use, reducing construction-generated solid waste. However, it is worth noting that employing mass timber and modular construction is not automatically beneficial under all circumstances. The transition from the current linear, high-impact, and wasteful construction practices to a circular, regenerative one can offer an alternative solution to the problem. Moreover, high education institutions can play an influential role in this transition. However, there is a knowledge gap regarding education for circularity in architectural design. This paper aims to address this gap. It presents an educational approach integrating circular design principles with mass timber and modular construction in the setting of an architectural design studio. This paper analyses the pedagogical methods employed and the learning outcomes of the design studio. The results showed students successfully integrated architectural design and knowledge of modular mass timber technology with an innovative circular rationale and exceeded the learning outcomes in two cases.

- Abstract
Background: An endogenous increase in advanced glycation products (AGE - advanced glycation end product) has been correlated with coronary artery disease (CAD) for some time, but the role of exogenous AGEs consumed through foods with any degree of processing that involves temperature increase, has not yet been fully elucidated. We aimed to verify if the dietary intake of AGE (dAGE) and the formation of fluorescent AGE in the skin correlate with the presence of endothelial dysfunction and subclinical atherosclerosis during the aging process. Design: Observational, quantitative, descriptive cross-sectional study Methods: 30 adults (age 54 + 9.7 years; non-smokers, without diabetes mellitus (DM), and without coronary artery disease were stratified by Body Mass Index (BMI) range (normal weight 30 Kg/m2), and evaluated about dietary intake and AGE formation and cardiovascular parameters related to endothelial function, and subclinical atherosclerosis. We used Carotid Doppler Ultrasound to calculate intimal medial thickness (IMT), flow-mediated dilation (FMD) to verify endothelial function, Skin Autofluorescence (SAF) AGE reading to measure AGE formation. We collected 03 24-hour dietary recalls on different days to calculate dietary AGE intake. Results: dAGE and SAF was homogeneous in the 3 evaluated groups (eutrophic = 17743 + 6088 kU/day ; 3,0 + 0,7 AU , overweight = 20555 + 4901 kU/day ; 2,7 + 0,5 AU, obese = 18424 + 8778 kU/day; 2,7 + 0,7 AU; p=0.63; p=0,58). dAGE was inversely correlated with the FMD in the eutrophic group (r=-0.679, p

A região mais antiga da cidade de Santos é retratada através de um passeio aquarelado que possibilita experimentar cenas do cotidiano do Centro Histórico de Santos, que por muitas vezes passam despercebidas. Desta forma, frações de momentos visuais por mais simples e comuns que pareçam são evidenciados, recebendo uma visão mais sensível e curiosa para com esses patrimônios e a região. A técnica utilizada é aquarela sobre papel. Os locais retratados, respectivamente são: Casa da Frontaria Azulejada, Pátio Interno da Casa da Frontaria Azulejada, Antiga Estação Railway SantosJundiaí, Convento do Carmo com vista da Rua Itororó, Catedral de Santos Nossa Senhora do Rosário, Igreja Nossa Senhora do Rosário, Monumento ao Trabalhador Portuário, e Ruínas dos antigos armazéns do Porto de Santos. As imagens fizeram parte do projeto “Postais Culturais”, com fomento contemplado via edital de “Produção de Exposições Inéditas de Artes Visuais” do Programa de Ação Cultural do Governo do Estado de São Paulo (ProAC 10/2019). O projeto contou com a exposição de 33 obras, visita guiada, rodas de conversa e oficinas de aquarela.

Negli ultimi anni l'Ordine delle Psicologhe e degli Psicologi della Liguria ha indagato la salute psicologica dei cittadini liguri, a fronte di un crescente malessere psicologico e un eleva-to utilizzo di antidepressivi nella popolazione ligure rispetto alla popolazione nazionale (Rap-porti SISM 2015 ? 2018). Lo studio attuale riporta i dati di due rilevazioni su campioni indi-pendenti relative al benessere psicologico dei cittadini liguri prima e durante la pandemia da COVID-19, tramite l'utilizzo di una batteria di test psicometrici online. I risultati suggeriscono che la pandemia abbia avuto un impatto sul benessere dei cittadini. In particolar modo si è os-servato un aumento di possibili disturbi depressivi e la presenza di disturbo post-traumatico da stress. Nonostante una diminuzione del benessere psicologico percepito, non si è osservato un aumento della proporzione di individui che riferiscono di essersi rivolti a servizi di supporto psicologico, in particolare per quanto riguarda gli uomini, che si rivolgono a tali servizi in mi-sura nettamente minore rispetto alle donne. É stato inoltre rilevato che le fasce più scolarizzate della popolazione tendono a mostrare una intenzione maggiore di rivolgersi a professionisti della salute mentale.

Atualmente um vírus proveniente de países Africanos tem se disseminado rapidamente pelo mundo. Trata-se de um vírus de DNA semelhante a erradicada varíola humana, denominado de Monkeypox vírus, sendo em macacos, em 1958, na Dinamarca. Assim, o objetivo deste trabalho foi evidenciar, com o auxílio da revisão de literatura, os aspectos gerais do MPXV enfatizando a atuação do médico veterinário nesse contexto. Segundo dados da OMS, mais de 3,7 mil casos foram confirmados em humanos até o dia 22 de agosto de 2022. Os sintomas observados após a contaminação são cefaleia, febre, dor no corpo, linfadenopatia grave e erupção de pústulas espalhadas pela pele. Ainda não está esclarecida a fonte animal de disseminação, entretanto, atualmente a principal fonte de transmissão parece ser a interhumana, sendo a maioria dos infectados homens homossexuais. A ANVISA aprovou a utilização, pelo Ministério da Saúde, da medicação Tecovirimat e de uma vacina específica. O papel do médico veterinário no controle de zoonoses é imperativo, uma vez que a influência desse profissional na saúde na humana é um fator determinante pela larga dimensão de atuação e competência profissional, apresentando importância no controle de zoonoses no âmbito da saúde humana e animal.

8. Accuracy of Magnetic Resonance Imaging in the Diagnosis of Multiple Ligament Knee Injuries: A Multicenter Study of 178 Patients

Background: Magnetic resonance imaging (MRI) has shown limited diagnostic accuracy for multiple ligament knee injuries (MLKIs), especially posterolateral corner (PLC) injuries. Hypothesis: The diagnostic accuracy of MRI for MLKIs will only be moderate for some knee structures. Patient-related factors and injury patterns could modify the diagnostic accuracy of MRI. Study Design: Cohort study; Level of evidence, 3. Methods: All patients with MLKIs surgically treated between January 2014 and December 2020 in the centers participating in the study were reviewed. We recorded sex, age, mechanism of injury, time from injury to MRI, and vascular and neurological associated lesions. Lesions to the anterior cruciate ligament (ACL), posterior cruciate ligament, medial collateral ligament, lateral collateral ligament (LCL), popliteus tendon, popliteofibular ligament, iliotibial band, biceps tendon, medial and lateral meniscus, and articular cartilage from MRI reports and surgical records were also collected. The sensitivity, specificity, positive predictive value, negative predictive value, diagnostic accuracy, diagnostic odds ratio, positive and negative likelihood ratio, and intraclass correlation coefficient of MRI were calculated for each knee structure. With logistic regression, associations between patient and injury characteristics and MRI accuracy were assessed. Results: A total of 178 patients (127 male; mean age, 33.1 years) were included. High-energy trauma was the most common mechanism of injury (50.6%), followed by sports trauma (38.8%) and low-energy trauma (8.4%). The ACL was the structure with the best diagnostic accuracy, diagnostic odds ratio, and positive predictive value (94.4%, 113.2, and 96.8%, respectively). PLC structures displayed the worst diagnostic accuracy among knee ligaments (popliteus tendon: 76.2%; LCL: 80.3%) and diagnostic odds ratio (popliteus tendon: 9.9; LCL: 17.0; popliteofibular ligament: 17.5). MRI was more reliable in detecting the absence of meniscal and chondral lesions than in identifying them. Logistic regression found that the diagnostic accuracy was affected by the Schenck classification, with higher Schenck grades having worse diagnostic accuracy for peripheral structures (iliotibial band, popliteus tendon, and biceps tendon) and improved diagnostic accuracy for the ACL and posterior cruciate ligament. Conclusion: The diagnostic accuracy of MRI for MLKIs largely varied among knee structures, with many of them at risk of a misdiagnosis, especially PLC, meniscal, and chondral lesions. The severity of MLKIs lowered the diagnostic accuracy of MRI for peripheral structures.

A síndrome do idoso frágil se origina a partir do declínio das funções fisiológicas dos idosos. Observa-se entre estes pacientes a diminuição de atividades físicas, perda de peso, queda da velocidade de mobilidade e redução das funções cognitivas - sendo todas estas situações avaliadas a partir dos critérios de fragilidade de Fried – especialmente em idosos com doenças crônicas, neoplasias e doenças demenciais. Atualmente, verifica-se a prevalência maior de idosos pré-frágeis. Com o envelhecimento da população, espera-se que ocorra o aumento de idosos frágeis. A sarcopenia, o risco de quedas e os maus tratos sofridos com violência física e psicológicas devem ser estudados, de forma a compreender como eles auxiliam na degeneração da funcionalidade dos idosos. Nesse sentido, o objetivo deste trabalho é analisar estudos que tenham como objeto de pesquisa a influência da redução de massa muscular, de quedas e de maus-tratos na qualidade de vida de idosos com fragilidade. Identificou-se que a sarcopenia aumenta com a idade, estabilizando-se aos 80 anos e sendo mais comum em mulheres, idosos institucionalizados e idosos hospitalizados. Somado a isso, identificou-se que a prática de exercícios físicos, estimulada desde a vida adulta, é capaz de impedir que idosos pré-frágeis se tornem frágeis. Com relação à queda, ela é a variável clínica mais presente nos idosos frágeis, sendo um histórico comum em boa parte deles, com uma média de três quedas em histórias pregressas. Os maus-tratos foram objeto de poucos estudos, porém conclui-se que idosos homens e depressivos são as maiores vítimas de violência física e verbal.

13. Challenges of prehospital silver trauma patients

- Abstract
Silver trauma patients, defined as those aged >65 years who have traumatic injuries, are a complex group because physiology alters with age leading to difficulties with assessment. This is exacerbated within the prehospital setting as environmental factors and ambulance delays increase the risk of complications. This article focuses on the pathophysiology and application of clinical guidelines on three aspects of prehospital silver trauma: neurology, osteology and haemorrhage. Neurologically, silver trauma patients have a higher risk of traumatic brain injury and are harder to assess because of age-related cognitive decline. Regarding osteology, older people, particularly postmenopausal women, are at a higher risk of vertebral fracture, with many going undiagnosed. Haemorrhage is also influenced, as geriatric patients typically experience occult bleeding or rebleeding several days after an event when crews have left. Despite these risks, prehospital guidelines often offer little support for clinicians in making holistic, clinically sound decisions for their patients.

15. The undisclosed disclosures: Conflicts of interest in studies related to robotics in hepatobiliary and pancreatic surgery

The use of robotics in hepatobiliary and pancreatic surgery has increased. With this increased collaboration, there has also been a push toward improving the transparency of conflicts of interest in terms of funding provided by robotics companies.Studies with ≥1 American author published between 2016 and 2020 discussing robotic hepatobiliary and pancreatic surgery were included in the analysis. The Centers for Medicare and Medicaid Services Open Payments Program was used to evaluate the accuracy of industry payment disclosures. Conflict of interest was defined as a lack of disclosure of ≥$100 funding from any robotics company in the United States. The primary outcome of this study was to determine the efficacy of the current standard conflict of interest reporting in surgery.A total of 355 studies (2,413 authors) were included. Of the studies that received robotics funding (n = 133), 20.3% did not disclose their conflicts of interest, whereas 79.7% had a conflict of interest disclosure. However, most of the disclosures (76.5%) were inaccurate.The findings of this study require an update in journal policies to ensure the accurate disclosure of conflicts of interest in robotic hepatobiliary and pancreatic surgery studies. Similarly, the investigators should ensure that their disclosures are accurate. Finally, surgeons must be more on top of their relationships with these companies and consider their vulnerability to bias.

A lack of bone volume may compromise a correct three-dimensional implant placement. This study was designed to evaluate the clinical and radiographic outcomes of simultaneous horizontal guided bone regeneration (GBR) performed using autogenous bone or blood-derived products mixed with a bone xenograft. The study population consisted of patients operated on using one of two clinical protocols for GBR: group A, which used autogenous bone mixed with a bone xenograft, and group B, which used advanced platelet-rich fibrin (A-PRF) mixed with a bone xenograft. The primary outcome was the clinical gain in the peri-implant defect. The secondary outcomes included an analysis of the postoperative healing, periodontal parameters, marginal bone loss, and occurrence of adverse events. All of the surgeries were carried out successfully. One patient in each group experienced a case of early implant loss, and three patients (one in group A and two in group B) presented biologic complications. The mean peri-implant vertical defect heights at baseline in group A and group B were 3.6 ± 0.9 mm and 4 ± 1.5 mm, respectively (P = .382). No statistically significant differences in the mean residual defect heights (P = .521) or in the postoperative wound healing (P = .611) were observed. Stable peri-implant marginal bone levels were recorded after loading in both groups. The use of A-PRF combined with a particulate bone xenograft and covered with a fixed collagen membrane may provide clinical results similar to those obtained via autogenous bone mixed with bone xenograft.

18. Mortality in Elderly Patients Taking Furosemide: Prospective Cohorts Study

Objectives. Low blood pressure (BP) has been proposed as a risk factor of death in elderly patients. However, this association could be partially accounted for by the deleterious effects of BP-lowering drugs. We analyzed whether these drugs are associated to an increased risk of death in elderly patients taking multiple potential confounders into account. Design. This is a prospective cohort study. Setting and Participants. Probabilistic sample of 772 community-dwelling patients aged >65 years living in Spain, who were appointed for an initial clinical visit and followed up through telephone calls 4, 6, 9, 12, and 60 months afterwards. Methods. At baseline visit, BP was measured using standardized methods, and BP medications and risk factors of death in elderly patients (BMI, oxygen saturation, toxic habits, comorbidity, muscular strength, and functional and cognitive capacity) were collected. During the follow-up, the vital status of patients and the date of death were ascertained. Results. During a median 5-year follow-up, 226 all-cause deaths occurred among the 686 participants included in the analysis. In a Cox regression model that included all the BP drug classes, diuretics and nitrites were significantly associated with mortality ( p < 0.005 ). Within diuretics, furosemide was found to be responsible for the association of the group. In multivariable Cox regression models adjusted for BP and the rest of the mortality risk factors, furosemide remained as the only BP drug that was independently associated with mortality (hazard ratio 2.34; p < 0.01 ). Conclusions. Furosemide was prospectively associated with increased mortality in older people. If confirmed, this drug should be taken into account by prescribers and considered a confounder in BP studies.

21. Early outcome of humeral head replacement with glenoid reaming arthroplasty (Ream and Run) for treatment of advanced glenohumeral osteoarthritis

Long-term outcomes of anatomic total shoulder arthroplasty (aTSA) can be compromised by glenoid loosening and failure. The purpose of this study was to evaluate the short- and midterm outcomes of humeral head replacement with glenoid reaming arthroplasty (Ream and Run) for the treatment of advanced glenohumeral osteoarthritis, and to identify associations between preoperative factors and outcomes.Forty-nine shoulders (mean age 60 ± 7 years) with minimum 2-year follow-up (mean 4.6 ± 1.7) were evaluated. Forty-three (87.8%) were male. Thirteen (26.5%) had previous nonarthroplasty shoulder surgery. There were 19 (38.8%) Walch type A and 30 (61.2%) type B glenoids. Pre- and postoperative shoulder motion, patient-reported outcomes (PROMs), and health-related quality of life (HRQoL) were assessed. Pre- and postoperative plain radiographs were evaluated. Mixed effects models were used to investigate factors associated with outcomes.Active forward elevation and active external rotation improved from 111.7° ± 23.8° to 139.2° ± 21.1° and 13.3° ± 20.7° to 38.7° ± 14.7°, respectively (P .001). The mean American Shoulder and Elbow Surgeons Standardized Shoulder Assessment Form (ASES), Disabilities of the Arm, Shoulder, and Hand questionnaire (DASH), Simple Shoulder Test (SST), and visual analog scale (VAS) shoulder pain scores at the most recent follow-up were 86.6 ± 19.9, 10.1 ± 14.1, 10.5 ± 2.2, and 1.5 ± 2.3, respectively. The mean changes of PROMs were significant and exceeded the minimal clinically important difference for aTSA. The percentage maximal possible improvement for ASES, DASH, and SST were greater than 75%. Male sex (P .008) and not having prior shoulder surgery (P .04) were significantly associated with better absolute and greater change in PROMs. Age, preoperative shoulder motion, and Walch glenoid classification were not associated with PROMs. Five (10.2%, 95% confidence interval [CI] 1.8%-18.6%) patients underwent revision for pain. Three additional patients were dissatisfied at final follow-up without undergoing revision, resulting in 16.7% (95% CI 6.1%-27.2%) of patients being dissatisfied with their outcome after their initial RnR. Mean Short Form-6 Dimensions scores improved from 0.66 ± 0.12 to 0.77 ± 0.13 and mean EuroQol-5 Dimensions scores improved from 0.68 ± 0.20 to 0.85 ± 0.17 (P .001). Improvement in HRQoL was significantly associated with nondominant arm treatment, increased age, and greater preoperative SST score. Annual medialization of the humeral head center of rotation was 0.56 ± 1.6 mm/yr. No radiographic measure was associated with long-term PROMs.Ream and Run can provide significant and clinically important improvement in PROMs and HRQoL for a high percentage of patients at short- and midterm follow-up. This procedure is an appropriate alternative to aTSA in select patients.

Background: Access to high-quality dietary intake data is central to many nutrition, epidemiology, economic, environmental, and policy applications. When data on individual nutrient intakes are available, they have not been consistently disaggregated by sex and age groups, and their parameters and full distributions are often not publicly available. Objectives: We sought to derive usual intake distributions for as many nutrients and population subgroups as possible, use these distributions to estimate nutrient intake inadequacy, compare these distributions and evaluate the implications of their shapes on the estimation of inadequacy, and make these distributions publicly available. Methods: We compiled dietary data sets from 31 geographically diverse countries, modeled usual intake distributions for 32 micronutrients and 21 macronutrients, and disaggregated these distributions by sex and age groups. We compared the variability and skewness of the distributions and evaluated their similarity across countries, sex, and age groups. We estimated intake inadequacy for 16 nutrients based on a harmonized set of nutrient requirements and bioavailability estimates. Last, we created an R package-nutriR-to make these distributions freely available for users to apply in their own analyses. Results: Usual intake distributions were rarely symmetric and differed widely in variability and skewness across nutrients and countries. Vitamin intake distributions were more variable and skewed and exhibited less similarity among countries than other nutrients. Inadequate intakes were high and geographically concentrated, as well as generally higher for females than males. We found that the shape of usual intake distributions strongly affects estimates of the prevalence of inadequate intakes. Conclusions: The shape of nutrient intake distributions differs based on nutrient and subgroup and strongly influences estimates of nutrient intake inadequacy. This research represents an important contribution to the availability and application of dietary intake data for diverse subpopulations around the world.

This study aimed to analyze factors related to adolescent mothers' satisfaction with childbirth.Prospective and cross-sectional study PARTICIPANTS: Fifty adolescent mothers with maternal age from 14 to 19 years and delivery of a single and live newborn at term SETTING: Low-risk maternity hospital INTERVENTION: The participants were invited to answer a questionnaire (North Bristol modified version of the Mackey Childbirth Satisfaction Rating Scale [mMCSRS]) with 18 items measuring childbirth satisfaction. Each item was to be rated on a 5-point Likert scale (very dissatisfied to very satisfied).The main outcome measure was the total score on the questionnaire.The median maternal age was 18 years (95% CI, 11-25), and the median maternal satisfaction score of adolescent mothers was 88 (95% CI, 83-90). There was a significant difference in the total scores on the mMCSRS regarding the following factors: "oral fluid and food intake during labor" (yes = 84.0 vs no = 78.0, P = 0.044); "professional who attended the birth" (physician = 78.0 vs midwife = 86.0, P = 0.022); "skin-to-skin contact" (yes = 83.0 vs no = 71.0, P = 0.004); and "breastfeeding at the first hour" (yes = 84.5 vs no = 75.5, P = 0.008). Multiple regression with a stepwise procedure identified the following independent factors: "gestational age" (coefficient = 2.14, P = 0.03), "oral fluid and food intake during labor" (coefficient = 5.30, P = 0.013), and "skin-to-skin contact" (coefficient = 11.2, P0.001).Satisfaction with childbirth in adolescent mothers is associated with measures that can be easily implemented in the health care system. They are chiefly the provision of oral fluid and food during labor and skin-to-skin contact. Specific strategies are thus needed to increase adolescents' satisfaction with childbirth.

The rapid response of phytoplankton communities to environmental changes makes this compartment one of the most important biological variable to consider in dealing with ecology and trophodynamic of coastal areas, especially in relation with ongoing climate changes and increased human pressures. In the last 30 years, the use of chemotaxonomy has improved the capability to detect phytoplankton community composition in terms of chemofunctional groups, especially for small size species requiring specific expertise and techniques, as well as time-consuming observation with light microscope. In this study, the temporal variability of chemotaxonomic functional groups has been investigated in surface waters of a LTER station in the Gulf of Naples over 12-year weekly sampling activities (2003–2015). Data reveal increasing trends in the percentage contribution of diatoms, prasinophytes, cryptophytes to the total phytoplankton biomass, while other groups show a decreasing trend. Nevertheless, strong differences in the intervals of periodicity for each chemofunctional group have emerged, both in terms of timescale and strength of periodicity over time, implying that temporal dynamics of groups were not stationary over the time and that they have different relationships with environmental variability. Our results contribute to enrich scientific knowledge on the ecology of this LTER site and underline the importance of chemotaxonomy as a monitoring tool in coastal marine systems.

Purpose of Review Aquatic foods are increasingly being recognized as a diverse, bioavailable source of nutrients, highlighting the importance of fisheries and aquaculture for human nutrition. However, studies focusing on the nutrient supply of aquatic foods often differ in the nutrients they examine, potentially biasing their contribution to nutrition security and leading to ineffective policies or management decisions. Recent Findings We create a decision framework to effectively select nutrients in aquatic food research based on three key domains: human physiological importance, nutritional needs of the target population (demand), and nutrient availability in aquatic foods compared to other accessible dietary sources (supply). We highlight 41 nutrients that are physiologically important, exemplify the importance of aquatic foods relative to other food groups in the food system in terms of concentration per 100 g and apparent consumption, and provide future research pathways that we consider of high importance for aquatic food nutrition. Summary Overall, our study provides a framework to select focal nutrients in aquatic food research and ensures a methodical approach to quantifying the importance of aquatic foods for nutrition security and public health.

29. Performance comparison of newly developed hydro-mechanical (hybrid) models for real-time induced seismicity forecasting

- Abstract
The Adaptive Traffic Light System (ATLS) is a seismic risk mitigation tool that can be used during geoenergy exploitation projects such as in the creation of Enhanced Geothermal Systems (EGS). In real-time applications, ATLS needs to include a data assimilation scheme in order to produce adaptive and time-dependent probabilistic seismicity forecasts by using the maximum available information at the moment of the assessment during such industrial operation. Numerical models capable to robustly forecast in real-time the temporal evolution of induced seismicity while properly accounting for uncertainties are the core elements of a functioning ATLS. In this respect, hydro-mechanical (HM) hybrid models are suitable models since they combine both statistical and physics-based assumptions to forecast induced seismicity. They include an accurate modeling of the physical processes involved in the generation of seismicity caused by human activity by keeping the associated computational cost low. In this work, we have developed two classes of simplified hybrid models that are based on 1D radial pore-fluid diffusion and fluid injection. The first class (HM0) accounts for linear pore-fluid diffusion from the fluid source, while the second class (HM1) accounts for the non-linear response of the medium due to pressure-dependent permeability variations upon fluid injection. Each class is sequentially coupled to two stochastic seismicity models that trigger seismicity if the respective space-time pressure solution reaches a critical value according to the Mohr-Coulomb criterion. However, both seismicity models differ in the way they simulate seismicity. One model uses an analytical approach based on probability density functions of model parameters to simulate seismicity (CAPS), while the other model simulates seismicity via stochastic seed approach (SEED). In a hindcast experiment, we test the models’ real-time forecasting capabilities within a data assimilation scheme using datasets from in-situ injection experiments that encompass different spatial scales. An essential step towards building a reliable ATLS is to properly weight each candidate model according to its respective contribution to the seismic hazard calculation. Therefore, an important part in our calculations is to evaluate and compare the forecasting performance of each model for each of the presented cases. Our findings show that models that take into account reliable distribution of uncertainties on a-priori selected model parameters as well as models accounting for a physically more accurate pressure solution in space and time generate better forecasting performances.

30. Earthquakes illuminate the incipient collision and subduction of Loyalty Ridge at Vanuatu subduction zone

Bathymetric highs resist subduction producing large- to small-scale change in the morphology of the subduction zone: Increase of the outer-rise curvature, trench indentation and large-scale slides and slumps in the fore-arc region. At the plate interface, bathymetric highs induce geometrical and frictional changes that can produce increase or decrease of the local coupling, and thus having an effect on the likelihood of occurrence of large megathrust earthquakes. Numerical models predict complex strain and stress patterns arising from subduction of such relief mainly driven by the size and relative geometry of trench and subducted high. However, the collision and subduction of bathymetric highs is investigated mainly via geophysical and geological surveys since seismic sequences have rarely illuminated the subduction of seafloor relief. Here, we report of a year-long and very energetic earthquake activity (10 Mw 6.5-7.5) at the Loyalty Ridge – Vanuatu trench at both the plate interface and in the outer-rise region. The spatio-temporal and magnitude of the earthquakes revealed complex release of the accrued flexural strain along the outer-rise and a pronounced segmentation of the interface with repeating M7 earthquakes, low aftershock activity and a large “aseismic” zone. The collision and subduction of the Loyalty Ridge along the Vanuatu trench seem to indicate a frictionally segmented interface where large megathrust earthquakes are unlikely to occur.

31. Stress Inversion and Forecast of Future Vent Locations in Calderas: Combining a Monte Carlo Algorithm with a Physics-based Model of Dike Propagation

Forecast of vent opening locations in volcanic regions is typically performed on the basis of the spatial density of past eruptive vents, without accounting for the physics of magma propagation. As sophisticated as the statistical analysis can be, such methods are difficult to apply to settings with scarce and spatially sparse data. An alternative approach has been recently proposed that combines a two-dimensional mechanical model of stress-driven dike pathways in the subsurface with a Monte Carlo stress optimization method. Here, we extend that strategy to three dimensions. We present a model of crustal stress in calderas accounting for tectonic processes and gravitational loading/unloading associated to topography. Then, we introduce a model of dike propagation that is able to capture the complexity of three-dimensional magma trajectories with low run times and may also backtrack dikes from a vent to the magma storage region. We test these models on synthetic scenarios inspired by real calderas, producing sets of dikes and vents for a given stress field and magma reservoir. Then, we use such scenarios to test a stress inversion strategy such that dike trajectories backtracked from the known vents are consistent with the assumed location of a magma reservoir. We eventually exploit the results from the stress inversions to produce probability maps of future vent locations.

Copper(I) [Cu2(μ-Br)2(tBuImCH2pyCH2L)]n (L = OMe, NEt2, NHtBu) compounds supported by flexible functionalized NHC-based polydentate ligands have been prepared in a one-pot procedure by reacting the corresponding imidazolium salt with an excess of copper powder and Ag2O. An X-ray diffraction analysis has revealed that [Cu2(μ-Br)2(tBuImCH2pyCH2NEt2)]n is a linear coordination polymer formed by bimetallic [Cu(μ-Br)]2 units linked by the lutidine-based NHC-py-NEt2 ligand, which acts as a heteroditopic ligand with a 1κC-2κ2N,N′ coordination mode. We propose that the polymeric compounds break down in the solution into more compact tetranuclear [Cu2(μ-Br)2(tBuImCH2pyCH2L)]2 compounds with a coordination mode identical to the functionalized NHC ligands. These compounds have been found to exhibit high catalytic activity in the Cu-catalyzed azide–alkyne cycloaddition (CuAAC) reaction. In particular, [Cu2(μ-Br)2(tBuImCH2pyCH2NEt2)]2 efficiently catalyzes the click reaction of a range of azides and alkynes, under an inert atmosphere at room temperature in neat conditions at a very low catalyst loading, to quantitatively afford the corresponding 1,4-disubstituted 1,2,3-triazole derivatives in a few minutes. The cycloaddition reaction of benzyl azide to phenylacetylene can be performed at 25–50 ppm catalyst loading by increasing the reaction time and/or temperature. Reactivity studies have shown that the activation of the polynuclear catalyst precursor involves the alkyne deprotonation by the NHC moiety of the polydentate ligand to afford a copper(I)-alkynyl species bearing a functionalized imidazolium ligand. DFT calculations support the participation of the dinuclear species [(CuBr)2(μ-tBuImCH2pyCH2NEt2)], resulting from the fragmentation of the tetranuclear compound, as the catalytically active species. Objetivo: Relatar a experiência na elaboração e implantação do protocolo de extravasamento e infiltração de antineoplásicos em acesso venoso central. Método: estudo descritivo, tipo relato de experiência realizado de fevereiro a maio de 2021. Resultados: Após a busca de evidências na literatura, foi elaborado um fluxograma referente aos cuidados de enfermagem realizados durante e após o extravasamento/infiltração. Além das medidas iniciais padronizou-se o uso da hialuronidase tópica e da fotobiomodulação (1 a 3 joules, com potência de 100 miliwats). A área deveria ser demarcada e se possível fotografada. Após a validação do protocolo por um grupo de enfermeiros especialistas e pelo escritório de qualidade, foi implantado em um Ambulatório de Oncologia de Hospital Público do interior do Estado de São Paulo. Conclusão: Apesar de raro, o extravasamento em acesso central pode acontecer e a equipe precisa de um protocolo assistencial para padronizar a conduta e garantir o sucesso da terapêutica.

37. A Decade of Youth Behavior Studies Regarding ICT's Impacts: Notes on the Experience of School of the Future Research Laboratory at the University of São Paulo - Brazil

- Abstract
School of the Future Research Laboratory at the University of São Paulo – USP is 32 years old and since 2010 has been devoted to map and discuss new behaviors among different virtual communities seeking of media and information literacy (MIL), new ways of learning and producing contents on digital platforms. On order to do so we have been developing surveys, virtual ethnography and netnographic studies to map new attitudes and behaviors of contemporary networked actors. This article brings a summary of several findings in the research conducted in the last ten years, demonstrating how the pioneering work of School of the Future researchers points out indicators on the behavior of young communities that are valid to this day and still raise contemporary discussions. In order to do so we will focus on our most extensive netnographic studies developed with young students throughout Brazil in 2012-2014.

Despite substantial observational and experimental evidence that aspirin use can provide protection against the development of colorectal neoplasia, our understanding of the molecular mechanisms involved is inadequate and limits our ability to use this drug effectively and safely for chemoprevention. We employed an untargeted plasma metabolomics approach using liquid chromatography with high-resolution mass spectroscopy to explore novel metabolites that may contribute to the chemopreventive effects of aspirin. Associations between levels of metabolic features in plasma and aspirin treatment were investigated among 523 participants in a randomized placebo-controlled clinical trial of two doses of aspirin (81 or 325 mg/day) and were linked to risk of colorectal adenoma occurrence over 3 years of follow-up. Metabolic pathways that were altered with aspirin treatment included linoleate and glycerophospholipid metabolism for the 81-mg dose and carnitine shuttle for both doses. Metabolites whose levels increased with 81 mg/day aspirin treatment and were also associated with decreased risk of adenomas during follow-up included certain forms of lysophosphatidylcholine and lysophosphatidylethanolamine as well as trihydroxyoctadecenoic acid, which is a derivative of linoleic acid and is upstream of cyclooxygenase inhibition by aspirin in the linoleate and arachidonic acid metabolism pathways. In conclusion, our findings regarding lysophospholipids and metabolites in the linoleate metabolism pathway may provide novel insights into the chemopreventive effects of aspirin in the colorectum, although they should be considered hypothesis-generating at this time. Prevention Relevance: This research used metabolomics, an innovative discovery-based approach, to identify molecular changes in human blood that may help to explain how aspirin use reduces the risk of colorectal neoplasia in some individuals. Ultimately, this work could have important implications for optimizing aspirin use in the prevention of colorectal cancer.

A set of neutral [IrBr(L2)(κC-tBuImCH2PyCH2OMe)] and cationic [Ir(L2)(κ2C,N-tBuImCH2PyCH2OMe)]PF6 (L2 = cod, (CO)2) Ir(I) compounds featuring a flexible lutidine-derived polydentate ligand having NHC and −OMe as donor functions have been evaluated as catalyst precursors for the N-methylation of aniline using methanol both as a reducing agent and a C1 source. The carbonyl complexes are somewhat more active than the related diene compounds with the neutral compound [IrBr(CO)2(κC-tBuImCH2PyCH2OMe)] being the more active. A range of aromatic primary amines, including heterocyclic amines, have been selectively transformed into the corresponding N-methylamino derivatives using this catalyst at a low catalyst loading (0.1 mol %) and substoichiometric amounts of Cs2CO3 (half equiv) as a base, in methanol at 423 K. For aliphatic primary amines, selective N,N-dimethylation was achieved under the same catalytic conditions. The unselective deprotonation of the methylene linkers in [IrBr(CO)2(κC-tBuImCH2PyCH2OMe)] affords two isomeric neutral complexes featuring a coordinated dearomatized pyridine core, which were converted into [Ir(OMe)(CO)2(κC-tBuImCH2PyCH2OMe)] upon addition of methanol. This compound undergoes thermal activation of a C–H bond of the tert-butyl group to give the cyclometalated iridium(I) complex [Ir(CO)2{κ2C,C-(-CH2Me2C-ImCH2PyCH2OMe)}] featuring a bidentate C,C-coordinated NHC ligand. Mechanistic investigations support a borrowing hydrogen mechanism proceeding through iridium(I) intermediates with the methoxo complex as the catalytic active species and the cyclometalated complex as the catalyst resting state. Infections complicate 5%-10% of cerebrospinal fluid (CSF) shunts. We aimed to describe the characteristics and contemporary pathogens of shunt infections in children in Canada and the United States.Descriptive case series at tertiary care hospitals in Canada (N = 8) and the United States (N = 3) of children up to 18 years of age with CSF shunt infections from July 1, 2013, through June 30, 2019.There were 154 children (43% female, median age 2.7 years, 50% premature) with ≥1 CSF shunt infections. Median time between shunt placement and infection was 54 days (interquartile range, 24 days-2.3 years). Common pathogens were coagulase-negative staphylococci (N = 42; 28%), methicillin-susceptible Staphylococcus aureus (N = 24; 16%), methicillin-resistant S. aureus (N = 9; 5.9%), Pseudomonas aeruginosa (N = 9; 5.9%) and other Gram-negative bacilli (N = 14; 9.0%). Significant differences between pathogens were observed, including timing of infection (P = 0.023) and CSF leukocyte count (P = 0.0019); however, differences were not sufficient to reliably predict the causative organism based on the timing of infection or discriminate P. aeruginosa from other pathogens based on clinical features. Empiric antibiotic regimens, which included vancomycin (71%), cefotaxime or ceftriaxone (29%) and antipseudomonal beta-lactams (33%), were discordant with the pathogen isolated in five cases. There was variability between sites in the distribution of pathogens and choice of empiric antibiotics. Nine children died; 4 (44%) deaths were attributed to shunt infection.Staphylococci remain the most common cause of CSF shunt infections, although antibiotic resistant Gram-negative bacilli occur and cannot be reliably predicted based on clinical characteristics.

The dorsal visual stream, the cortical circuit that in the primate brain is mainly dedicated to the visual control of actions, is split into two routes, a lateral and a medial one, both involved in coding different aspects of sensorimotor control of actions. The lateral route, named “lateral grasping network”, is mainly involved in the control of the distal part of prehension, namely grasping and manipulation. The medial route, named “reach-to-grasp network”, is involved in the control of the full deployment of prehension act, from the direction of arm movement to the shaping of the hand according to the object to be grasped. In macaque monkeys, the reach-to-grasp network (the target of this review) includes areas of the superior parietal lobule (SPL) that hosts visual and somatosensory neurons well suited to control goal-directed limb movements toward stationary as well as moving objects. After a brief summary of the neuronal functional properties of these areas, we will analyze their cortical and thalamic inputs thanks to retrograde neuronal tracers separately injected into the SPL areas V6, V6A, PEc, and PE. These areas receive visual and somatosensory information distributed in a caudorostral, visuosomatic trend, and some of them are directly connected with the dorsal premotor cortex. This review is particularly focused on the origin and type of visual information reaching the SPL, and on the functional role this information can play in guiding limb interaction with objects in structured and dynamic environments.

Combined angular deformities of the knee and foot are challenging clinical presentations for the orthopedic surgeon. We describe a surgical technique used in a 58-year-old man diagnosed with genu varum associated with progressive collapsing foot deformity and severe hallux valgus. The surgical team and the patient decided to perform a one-stage correction of all deformities. The procedure included corrective osteotomies with bone grafting and soft tissue reconstruction. The varus knee, hindfoot valgus, and hallux valgus were properly corrected intraoperatively with the surgical techniques described here. Satisfactory functional outcomes and excellent maintenance of the realignment were achieved at short-term follow-up. Level of Evidence V; Therapeutic Studies; Expert Opinion.

Introduction Dogs with chronic kidney disease (CKD) may have alterations in the glomerular filtration barrier, including podocyte loss. Detection of podocyte mRNA in urine could be useful for assessing podocyturia in dogs with kidney disease. The objective of this study was to evaluate the presence of nephrin mRNA (NPHS1) and podocin mRNA (NPHS2) in urine sediments of dogs with naturally occurring CKD and healthy dogs. Material and Methods Twenty-four dogs, 14 with CKD and 10 as healthy controls, underwent clinical evaluation. The dogs with CKD were divided into two groups, according to the International Renal Interest Society criteria: stage 1 or 2 CKD (n = 5) and stage 3 or 4 CKD (n = 9). Urine was collected by catheterisation or free catch and RNA isolation from the urine sediments was optimised using glycogen as a co-precipitant. Detection of NPHS1 and NPHS2 in the sediment samples was performed using quantitative real-time PCR. Results Both types of mRNA were detected in samples from all groups, but the percentages of detection were higher in the group of dogs with stage 1 or 2 CKD and lower in the group of dogs with stage 3 or 4 disease. Conclusion Physiological podocyturia was observed in healthy dogs, and the results suggest differential podocyturia in dogs with CKD, according to the stage of the disease, i.e. an increase in podocyturia in dogs at stage 1 or 2 and a reduction in podocyturia in dogs at stage 3 or 4.

Plastic tuning of multicell superconducting accelerating cavities is crucial in the development cycle of cavities for particle accelerators. Cavities must meet stringent requirements regarding the operating mode frequency, field flatness, and eccentricity before lining them up in a cryomodule string. After dressing bare cavities with helium vessels, the welded vessel prevents access to individual cavity cells disallowing any further localized tuning. Currently, there is no straightforward way to tune dressed cavities other than cutting the vessel and then tuning the bare cavity and dressing it back, which would significantly impact cost and schedule. In this paper, we present a novel tuning technique for already jacketed cavities that is non-invasive and cost-effective. The proposed scheme employs pressurized balloons to be temporarily deployed inside the cavity as a means to localize mechanical deformation in specific cells. The proposed tuning technique was successfully utilized to recover a 9-cell 1.3 GHz tesla-style cavity.

Tumourigenesis and cancer progression require enhanced global protein translation1-3. Such enhanced translation is caused by oncogenic and tumour-suppressive events that drive the synthesis and activity of translational machinery4,5. Here we report the surprising observation that leucyl-tRNA synthetase (LARS) becomes repressed during mammary cell transformation and in human breast cancer. Monoallelic genetic deletion of LARS in mouse mammary glands enhanced breast cancer tumour formation and proliferation. LARS repression reduced the abundance of select leucine tRNA isoacceptors, leading to impaired leucine codon-dependent translation of growth suppressive genes, including epithelial membrane protein 3 (EMP3) and gamma-glutamyltransferase 5 (GGT5). Our findings uncover a tumour-suppressive tRNA synthetase and reveal that dynamic repression of a specific tRNA synthetase-along with its downstream cognate tRNAs-elicits a downstream codon-biased translational gene network response that enhances breast tumour formation and growth.
