Mandatory vaccination has been a highly disputed policy for tackling infectious diseases. Here I argue that a universal mandatory vaccination policy for the general public against the COVID-19 pandemic is ethically preferable when grounded in the concept of solidarity, which is defined by Barbara Prainsack and Alena Buyx as an enacted commitment to a relevant respect recognized by a group of individuals with equal moral status. This approach is complementary to utilitarian accounts and could better address other reasonable oppositions to mandatory vaccination. From a solidaristic account, the recognized relevant respect is to end the COVID-19 pandemic as soon as possible. This group of individuals would be willing to carry costs to assist each other in this respect, and a mandatory vaccination policy could be their institutionalized mutual assistance. The costs to be carried include both the financial costs of vaccination and the health costs stemming from potential adverse events and scientific uncertainties. The proposed social health insurance similarity test suggests the degree of coercion the mandatory vaccination policy could undertake within each state’s specific legal and judicial context. The COVID-19 pandemic posed an unprecedented challenge to modern bioethical frameworks in the clinical setting. Now, as the pandemic stabilises and we learn to ‘live with COVID’, the medical community has a duty to evaluate its response to the challenge, and reassess our ethical reasoning, considering how we practise in the future. This article considers a number of clinical and bioethical challenges encountered by the author team and colleagues during the most severe waves of the pandemic. We argue that the changed clinical context may require reframing our ethical thought in such a manner as to adequately accommodate all parties in the clinical interaction. We argue that clinicians have become relatively disempowered by the ‘infodemic’, and do not necessarily have adequate skills or training to assess the scientific literature being published at an unprecedented rate. Conversely, we acknowledge that patients and families are more empowered by the infodemic, and bring this empowerment to bear on the clinical consultation. Sometimes these interactions can be unpleasant and threatening, and involve inviting clinicians to practise against best evidence or even illegally. Generally, these requests are framed within ‘patient autonomy’ (which some patients or families perceive to be unlimited), and several factors may prevent clinicians from adequately navigating these requests. In this article, we conclude that embracing a framework of shared decision-making (SDM), which openly acknowledges clinical expertise and in which patient and family autonomy is carefully balanced against other bioethics principles, could serve us well going forward. One such principle is the recognition of clinician expertise as holding weight in the clinical encounter, when framed in terms of non-maleficence and beneficence. Such a framework incorporates much of our learning and experience from advising and treating patients during the pandemic.

Resumen: El presente trabajo tiene por objetivo mostrar que la fuerza obligatoria del contrato se encuentra justificada en el respeto por la autonomía personal de los contratantes. Para ello, se efectúa un contraste entre las nociones de voluntad, promesas y autonomía que han protagonizado los esfuerzos para justificar el efecto vinculante del contrato en los estudios dogmáticos y de filosofía del derecho de contratos. Por último, se indican algunas ventajas de la autonomía personal sobre los otros parámetros en competencia, reforzando su pertinencia para fundar el principio de pacta sunt servanda. Abstract: This paper aims to show that the binding force of the contract is justified in respect for the personal autonomy of the contracting parties. To do this, a contrast is made between the notions of will, promises and autonomy that have led the efforts to justify the binding effect of the contract in dogmatic and philosophical studies of contract law. Finally, some advantages of personal autonomy over other competing parameters are indicated, reinforcing its relevance to support the principle pacta sunt servanda.

Introduction Media discourses about people living with dementia and carers contribute to the ways in which public, social spaces are designed, planned, and used. Negative media discourses play an important role in the socio-spatial exclusion of people living with dementia and stigmatising and dehumanising language prevents the achievement of genuine, rights-based dementia-friendly environments. Because the media plays a powerful role in shaping public attitudes, there is a need to understand media constructions of people living with dementia and carers in relation to their socio-spatial rights, which is the aim of this paper. Method A Foucauldian-inspired discourse analysis (FDA) was conducted on the public news media texts of one regional Australian city, to identify discourses relating to the socio-spatial rights of people living with dementia and carers. Lefebvre’s (1996) ‘right to the city’ concept was used as a conceptual framework, to define socio-spatial rights. Results Analysis revealed discourses relating to the right to urban citizenship, the right to difference and socio-spatial justice. Representations of participatory democracy, important to urban citizenship, are absent, as are the voices of people living with dementia. Through advocacy from others and a lens of citizenship, socio-spatial rights for people living with dementia, and in one instance carers, are recognised, in the context of dementia-awareness and dementia-friendly initiatives. While a lens of personhood constructs people living with dementia as historical and relational beings, a citizenship lens adds a spatial dimension. Conclusions Advocacy and the lenses of personhood and citizenship are important in creating positive constructions of people living with dementia, however, their participation in decision-making processes would confer an active citizenship status, and the inclusion of their authentic voices in media discourse would contribute to raising awareness towards rights-based dementia-friendly communities.

Objective: To identify how peer support interventions, for self-management of chronic pain, support basic psychological needs from a self-determination theory (SDT) perspective, using a systematic review.\ud \ud Methods: Ten databases were searched for studies reporting qualitative research about peer interactions in pain management interventions. ‘Best fit’ framework synthesis methodology was applied to identify strategies that support the satisfaction of competence, autonomy and motivation. These were matched to definitions of strategies provided by standardised taxonomies.\ud \ud Results: 18 studies were selected for inclusion. The synthesis resulted in a conceptual model, identifying 12 peer strategies that support psychological needs for self-management of chronic pain; 10 overlapped with existing taxonomies. \ud \ud Conclusion: This was the first known attempt to synthesise evidence about peer support strategies for people living with pain, using SDT as an a priori framework. The model demonstrates commonality between the motivation-promoting processes of peer support and those of other behaviour change interventions and identifies additional unique strategies provided by peers. This systematic classification of peer support strategies provides a means for future study of the efficacy and comprehensiveness of peer interventions.\ud \ud Practice Implications: The model could assist healthcare professionals and support groups to optimise the potential of peer processes.

Purpose: In the aphasia literature, motivation has been described as potentially influencing rehabilitation outcomes, and there are reports that researchers and clinicians have acted to promote it. However, studies directly investigating the range of beliefs and practices surrounding motivation do not exist currently. The purpose of this scoping review is to develop themes related to the beliefs and practices appearing in the recent aphasia literature. Method: Four databases (CINAHL, PsycINFO, PubMed, and Google Scholar) were searched using keywords aphasia and motivation (including derivatives such as motiv*) for articles published between 2009 and 2020. Searches returned 19,731 articles; after deleting duplicates and applying inclusionary criteria, 365 articles remained. In each article, text surrounding the term motivation was highlighted and thematic analysis was applied to these quotations. Results: Sixteen themes were developed through thematic analysis and placed into two groups. The first group contained five themes suggesting that researchers believed that motivation should be studied and recognized the value of motivation in person(s) with aphasia when participating in research or clinical activities. The second group contained 11 themes reporting diverse beliefs and practices in how motivation is incorporated in research and clinical activities. Conclusions: Results from this scoping review suggest that aphasia researchers, clinicians, and persons with aphasia hold beliefs about motivation that can influence clinical and research decisions. In general, beliefs and decisions related to motivation appeared to be guided by intuition rather than theories of motivation. These themes are discussed within the context of three psychological needs proposed by self-determination theory: competency, autonomy, and relatedness. Applying theories of motivation to future study in aphasia rehabilitation will guide work that can provide empirical support for these beliefs.

As cognitive enhancement research advances, important ethical questions regarding individual autonomy and freedom are raised. Advocates of cognitive enhancement frequently adopt a procedural approach to autonomy, arguing that enhancers improve an individual's reasoning capabilities, which are quintessential to being an autonomous agent. On the other hand, critics adopt a more nuanced approach by considering matters of authenticity and self-identity, which go beyond the mere assessment of one's reasoning capacities. Both positions, nevertheless, require further philosophical scrutiny. In this paper, we investigate the ethics of cognitive enhancement through the lenses of political and philosophical arguments about autonomy and freedom. In so doing, we contend that a substantive, relational account of individual autonomy offers a more holistic understanding of the ethical concerns of cognitive enhancement.

This article seeks to contribute to a better understanding of the relationship between Catholic and Protestant theories of mission by examining the influence of the Jesuit José de Acosta on the De conversione Indorum et gentilium (1669), one of the first comprehensive handbooks of Protestant missiology, written by Johannes Hoornbeeck. It is demonstrated that Acosta's Thomist emphasis on the willing acceptance of a new faith made his ideas particularly attractive to Hoornbeeck and are the reason why the latter preferred Jesuit sources to Franciscan thinkers and writings, since these were more Scotist in their outlook.

Although organizations increasingly offer wellness programs that enable employees to work out before or during work, it remains unknown what implications physical activity before or during the workday might have for work outcomes. Whereas a workout might be rewarding, especially for those who enjoy exercise, working out might also be draining, especially for those who are less intrinsically motivated to exercise. Integrating the Work-Home Resources model with self-determination theory, we develop and test theory which identifies how physical activity before the end of the workday might exert countervailing effects by impeding work focus through drained personal resources (i.e., ego depletion), while also improving work focus via enhanced personal resources (i.e., self-efficacy). We further theorized that motivation for exercise-whether it is intrinsically or extrinsically motivated-serves as a cross-level moderator of these relations. In a 5-day experience sampling study tracking 74 regularly exercising employees with Fitbit activity monitors, results indicated that physical activity was not significantly related to ego depletion. However, we found that light physical activity was positively related to self-efficacy and self-efficacy positively related to work focus (as rated by coworkers). Further, vigorous physical activity only resulted in better work focus among employees with an intrinsic (vs. extrinsic) motivation for exercise. Finally, moderate physical activity resulted in better work focus via self-efficacy among extrinsically motivated exercises, whereas this relation was negative for intrinsically motivated exercisers. Combined, our results highlight that physical activity can improve work focus when there is a match between physical activity intensity and exercise motivation. Resumo Este artigo trata de questões relacionadas à inseminação artificial homóloga post mortem. Tomando como referência normas éticas que asseguram a livre escolha do casal no planejamento familiar, objetivou-se descobrir de que forma tais regulamentações influenciariam na concretização desse projeto parental. A partir do método hipotético-dedutivo, realizou-se revisão de literatura em bioética e biodireito, além de pesquisa documental no sítio eletrônico do Conselho Federal de Medicina. Refletiu-se, então, sobre o princípio de autonomia dos pacientes submetidos às técnicas de reprodução assistida, levando em conta algumas das repercussões dessa técnica sobre o direito de família e sucessões. Por fim, descreveu-se o método de casuística clínica, utilizado pelas clínicas como parâmetro para tomar decisões e aconselhar o cônjuge sobrevivente acerca da problemática da concepção póstuma. Abstract This article discusses issues related to post mortem homologous artificial insemination. Taking as reference ethical norms that ensure the couple's free choice in family planning, the objective was to understand how such regulations would influence the accomplishment of this parental project. Using the hypothetical-deductive method, a literature review on bioethics and biolaw was carried out, in addition to a documentary research on the website of the Federal Council of Medicine. Then, reflections on the principle of autonomy of patients undergoing assisted reproduction techniques were made, considering some of the repercussions of this technique on family and succession law. Finally, a description of the method of clinical casuistry is presented, being used by clinics as a parameter to make decisions and advise the surviving spouse about the problem of posthumous conception. Resumen Este artículo trata aspectos relacionados a la inseminación artificial homóloga post mortem. Con base en la normativa ética que garantiza la libre elección de la pareja en la planificación familiar, el objetivo fue identificar la influencia de la legislación en la realización de este proyecto parental. A partir del método hipotético-deductivo, se realizó una revisión bibliográfica sobre bioética y bioderecho, además de una búsqueda documental en el sitio web del Consejo Federal de Medicina. Con esto, se reflexionó sobre el principio de autonomía de los pacientes sometidos a técnicas de reproducción asistida, teniendo en cuenta algunas de las repercusiones de esta técnica en el derecho de familia y de sucesiones. Por último, se describió el método de la casuística clínica utilizado por las clínicas como parámetro en la toma de decisiones y asesoramiento al cónyuge sobreviviente en el tema de la concepción póstuma.

OBJETIVO:Descrever com base na literatura científica, as vantagens da implementação do PP como instrumento de promoção de segurança e autonomia no processo de parturição.MÉTODO:Compreende um estudo bibliográfico, de caráter descritivo, realizado entre os meses de março a abril de 2023 pela Biblioteca Virtual em Saúde, utilizando os descritores :Gestação; Parto, Humanização e Autonomia pessoal. Dessa forma foram incluídos estudos em português e inglês, na íntegra e publicados nos últimos cinco anos. A busca inicial resultou em 126 estudos. Após aplicação dos critérios de inclusão e leitura exploratória apenas nove foram selecionados para compor esta pesquisa.RESULTADOS E DISCUSSÃO:Conforme análise da literatura, a implementação do PP configura-se como uma ferramenta importantíssima de garantia aos direitos e autonomia da mulher por favorecer uma melhor comunicação entre a mulher e os profissionais de saúde, oportunizando discussões de opções disponíveis e favorecendo controle, segurança e diminuindo os índices de violência obstétrica e intervenções desnecessárias, evidenciando o protagonismo da gestante durante o momento do parto.CONSIDERAÇÕES FINAIS:É pertinente a busca por mais conhecimento no que diz respeito ao PP como forma de oferecer às gestantes um maior suporte na orientação da construção do instrumento. Bem como, os profissionais devem disponibilizar estratégias para a criação do plano, não contemplando somente a ocasião da consulta de pré-natal, mas possibilitando a construção individual e/ou coletiva junto a família.

Sport performance depends in part on psychological factors. Self-determination theory emphasizes that the satisfisfaction of basic psychological needs and autonomous motivation may be linked to positive behavioral results and positive perceptions of performance. Our main objective in this study was to analyze the predictive power of such variables as perceived autonomy support, the satisfaction of psychological needs, and autonomous motivation on perceived sport performance. We recruited 447 young Portuguese athletes/participants (340 males, 107 females; Mage = 15.72 years, SD = 1.8). We had participants complete Portuguese sport versions of the Perceived Autonomy Support Exercise Climate Questionnaire, the Basic Psychological Needs in Exercise Scale, the Behavior Regulation Sport Questionnaire and the Perceived Performance in Sport Questionnaire. A gender invariant structural equation model showed that the coach’s autonomy support significantly predicted the satisfaction of psychological needs for autonomy, competence, and relatedness. The satisfaction of these three psychological needs predicted autonomous motivation. Satisfying the need for competence positively predicted perceived sport performance, but autonomous motivation was not significantly related to perceived performance.

As surgical care continues to transition to an outpatient setting, ambulatory surgery centers (ASCs) present favorable options for physician investment. As of 2017, more than 90% of ASCs have at least some physician ownership, with 64% solely physician-owned. Yet, physician ownership creates an inherent conflict of interest known as dual agency, where clinicians have a personal financial stake in addition to their obligation towards patient well-being. Here, we assess the ethical considerations surrounding dual agency in the setting of ASCs through the lens of beneficence, nonmaleficence, autonomy, and justice. We further propose strategies for appropriate navigation of such situations, including disclosure of ownership status, instruction on unfamiliar techniques, and adherence to accepted clinical practice guidelines for materials selection and surgical indications.

It is commonplace that care should be patient-centered. Nevertheless, no universally agreed-upon definition of patient-centered care exists. By consequence, the relation between patient-centered care as such and ethical principles cannot be investigated. However, some research has been performed on the relation between specific models of patient-centered care and ethical principles such as respect for autonomy and beneficence. In this article, I offer a detailed case study on the relationship between specific measures of patient-centered care and the ethical principle of respect for autonomy. Decision Quality Instruments (DQIs) are patient-centered care measures that were developed by Karen Sepucha and colleagues. The model of patient-centered care that guided the development of these DQIs pays special attention to the ethical principle of respect for autonomy. Using Jonathan Pugh's theory of rational autonomy, I will investigate how the DQIs relate to patient autonomy. After outlining Pugh's theory of rational autonomy and framing the DQIs accordingly (Part I), I will investigate whether the methodological choices made while developing these DQIs align with respect for autonomy (Part II). My analysis will indicate several tensions between DQIs and patient autonomy that could result in what I call "structural paternalism." These tensions offer us sufficient reasons, especially given the importance of the ethical principle of respect for autonomy, to initiate a more encompassing debate on the normative validity of Decision Quality Instruments. The aim of the present paper is to highlight the need for, and to offer a roadmap to, this debate.

The aim of a theoretically ideal process of informed consent is to promote the autonomy of the patient and to limit unethical physician paternalism. However, in practice, the nature of the medical profession requires physicians to act as ontological decision architects-based on the medical knowledge that they acquire through their experience and training, physicians ontologically determine a subset of viable courses of action for their patient. What is observed is not an unethical physician limitation or biasing of the patient towards certain treatment options that violates patient autonomy or consciously undermines informed consent, but rather a more foundational paternalism that is necessarily inherent to the physician-patient relationship. In this article we argue for a recognition of this underlying physician paternalism and posit that this necessary paternalism is not a foil to patient autonomy, but rather a foundational aspect of the duties of the medical professional within the physician-patient relationship.

Participation in sport can paradoxically be a source of psychological needs satisfaction and psychological needs frustration. Self-determination theory was applied to explain temporal relationships of athletes’ psychological needs satisfactions and psychological needs frustrations with burnout through a two-wave longitudinal study. Participants included 184 athletes (Mage = 24.04 years, SD = 5.56, 67.9% male) representing a range of competitive levels. A latent difference score model specifying longitudinal relationships between burnout and needs satisfactions and needs frustrations was tested. Significant within-variable changes were observed for all needs-satisfaction and needs-frustration variables. Phenomenology gives rise to certain ontological considerations that have far-reaching implications for standard conceptions of patient autonomy in medical ethics, and, as a result, the obligations of and to patients in clinical decision-making contexts. One such consideration is the phenomenological reduction in classical phenomenology, a core feature of which is the characterisation of our primary experiences as immediately and inherently meaningful. This paper builds on and extends the analyses of the phenomenological reduction in the works of Husserl, Heidegger, and Merleau-Ponty in order to identify and explain its implications for our current understanding of the principle of respect for patient autonomy and the norms of clinical decision making.

Background Care homes have been disproportionately affected during the COVID-19 pandemic. Practical challenges of enacting infection control measures in care home settings have been widely reported, but little is known about the ethical concerns of care home staff during the implementation of such measures. . Objectives To understand the ethical challenges perceived by care home staff during the early months of the COVID-19 pandemic. Research design An exploratory qualitative study. Participants and research context A purposive sample of 15 care home staff in different roles and ranks in Hong Kong was recruited to take part in semi-structured interviews between June and August 2020. Ethical considerations Ethical approval for this study was obtained. Participation was voluntary and anonymous. Participants had the right to withdraw from the study at any time without reprisal. Findings Three themes were identified: unclear legitimacy regarding infection control measures, limited autonomy in choices over infection control measures and inevitable harms to residents’ well-being. While the participants expected that they would have legitimated power to implement infection control measures, they were resistant when their right to self-determination of testing and vaccination was infringed. They also felt trapped between ethical duties to protect care home residents from infection risk and the anticipated detrimental effects of infection control measures. Conclusions The findings of this study reveal tensions among the ethical obligations of care home staff in response to a public health emergency. They highlight the importance of strengthening ethical sensitivity and ethical leadership in identifying and resolving the challenges of pandemic responses.

Findings demonstrating decision-related neural activity preceding volitional actions have dominated the discussion about how science can inform the free will debate. These discussions have largely ignored studies suggesting that decisions might be influenced or biased by various unconscious processes. If these effects are indeed real, do they render subjects' decisions less free or even unfree? Here, we argue that, while unconscious influences on decision-making do not threaten the existence of free will in general, they provide important information about limitations on freedom in specific circumstances. We demonstrate that aspects of this long-lasting controversy are empirically testable and provide insight into their bearing on degrees of freedom, laying the groundwork for future scientific-philosophical approaches.

Introduction: The responsibility for managing a long-term condition (LTC) such as chronic kidney disease (CKD) typically transfers from parent to child, as children become older. However, children can find it challenging to become independent at managing their LTC and evidence for how health care professionals (HCPs) support transfer of responsibility is limited. This study aimed to explore how young people with CKD assume responsibility for managing their condition and the HCP’s role during this process.Methods: Sampling, qualitative data collection and analysis were guided by a constructivist grounded theory approach. Individual and dyadic interviews, and focus groups, were conducted with 16 young people aged 13-17 years old with CKD, 13 parents and 20 HCPs.Findings: A grounded theory, shifting responsibilities, was developed that produces new insights into how young people’s, parents’ and HCPs’ constructions of the transfer of responsibility differed. These diverse constructions contributed to multiple uncertainties around the role of HCPs, when the process started and was completed, and whether the endpoint of the process was young people’s self-management or young person-parent shared management.Conclusion: Families would benefit from HCP support over a longer timeframe, that integrates assuming self-management responsibility with gaining independence in other areas of their lives and focuses on young people ‘doing’ self-management.Patient or Public Contribution: Patient and public involvement (PPI) was integrated throughout the study, with young adults with CKD and parents who had a child with CKD actively involved in the study’s design and delivery.

Citizenship has provided an important conceptual framework in dementia research and practice over the past fifteen years. To date, there has been no attempt to synthesize the multiple perspectives that have arisen in this literature. The purpose of this paper is to explore, reflect on, and contrast, the key concepts and trends in the citizenship discourse as it relates to people with dementia. Using a scoping review methodology, forty-nine articles were identified for review. Despite the use of different descriptors, thematic analysis revealed four core themes underpinning citizenship discourse: 1) the relationality of citizenship; 2) facilitated agency and autonomy; 3) attention to stigma, discrimination and exclusion; and 4) recognition of the possibilities of identity and growth. Overall, this scoping review found a major emphasis on expanding definitions of agency and autonomy to render citizenship unconditional and inclusive of the diverse life experiences of people living with dementia. Notably, there is recognition that a more intersectional lens for embedding the subjective experience within a broader socio-political context is needed. Whilst the adoption of a citizenship lens in dementia research and practice has had real-world implications for policy and research, its exploration and use continue to be led by academics, highlighting the importance that future research involve input form people with dementia.

Most people believe in free will. Past research has indicated that reducing this belief has numerous downstream consequences including everyday outcomes as well as neural and cognitive correlates associated with a reduction of self-control. However, the exact mechanisms through which a reduction in free will belief affects self-control are still a matter of investigation. In the present registered report, we used a task switching paradigm to examine whether reducing belief in free will makes people less controlled or whether it enhances their reliance on automatic impulses. Using Bayesian sequential analysis, we failed to conceptually replicate the previous link between free will belief and cognitive control. Our registered report plan mostly accumulated substantial evidence supporting the null hypothesis. That is, diminished belief in free will does neither impact control nor automaticity. Theoretical implications of this finding are discussed.

Aims Higher education systems around the world have enforced campus closures to combat the COVID‐19 pandemic. Such measures may threaten students' basic psychological needs for relatedness, competence and autonomy, and the development of intrinsic learning motivation. Little is known about whether the implementation of campus closures yielded negative developmental patterns for these motivational variables. We aim to close this research gap. Sample We investigated developmental patterns in longitudinal data spanning the first four semesters of undergraduate students in two cohorts at one German university starting in 2013 and 2019 (cohort 1: normal study conditions; cohort 2: affected by campus closure). We used propensity score modelling to ensure comparability between both cohorts resulting in a sample of 435 students each (total n = 870 students). Method We estimated conditional latent growth curve models (LGCM) to investigate developmental trends in need satisfaction and intrinsic learning motivation across the students' first four semesters and how these trends differed between the two cohorts. Results The results indicated a more maladaptive development of motivational variables for students of cohort 2 compared to students of cohort 1. More specifically, we found negative developmental trends following the implementation of campus closures for competence, relatedness and intrinsic learning motivation. Conclusions Our findings highlight the importance of considering side effects for students' psychological functioning when discussing the implementation or renewal of campus closures. It seems important to find ways to carefully reopen higher education institutions while also minimizing further risks for students and faculty.

An important aspect of the nineteenth century debate on the relationship between science and religion concerned the popularity of deterministic views among scientists. An integral part of Comte's positivism, the idea of immutable laws that determined natural and social phenomena became an increasingly prevalent component of scientific perspectives in the Darwinian era. Referring to this tendency as 'scientific fatalism,' critics likened it to Calvinist predestination, which transformed the debate into one involving polemics about different branches of Christianity as well. This paper focuses on a neglected aspect of this debate, namely, the role that references to Islam and Turks played in it. 'Mohammedan fatalism,' already a common theme in justifications of colonialism, promptly became a tool with which to condemn new scientific views. Comparing French, British, and American writings on the topic, the paper illustrates that while there emerged approaches that praised the fatalism of Muslims while making a case for scientific determinism, most scientists and thinkers resorted to condemning the fatalism of Muslims in order to distinguish their views from it. In this respect, the paper demonstrates how political and religious discourses played a significant part in the shaping of scientific discourse in the Victorian era.

This paper examines the problem of dying alone in the context of no-visitors hospital policy during the COVID-19 pandemic. It critically analyses a rights-based solution, offering a democratized visitors policy alternative, premised on the value of legal justice. While an inclusive, participatory, and thoroughly justified visitors' policy, which takes into account the good of all stakeholders in the process, is indeed the right alternative to the paternalistic, top-down no-visitors policy, I argue that the democratized visitors' policy alternative ought to be grounded on reasons of both justice and love. Legal justice and claimable individual rights, though important, are limited and cannot fully capture the vicissitudes of mutual vulnerabilities and the moral stringency of duties of mutual care. In the context of suffering and death, instances of extreme vulnerability and interdependence, individual rights of autonomy and self-determination prove insufficient to meet our most basic needs for love, human presence, and accompaniment.

Recovery, a model of care aimed at patient-led nursing practice emphasizing autonomy, hope and self-determination, has in recent years been adapted for the secure forensic psychiatric setting. Often referred to as 'secure recovery', this model suggests the aims of recovery are achievable even in highly restrictive settings. This paper will adopt a Foucauldian perspective to offer a critical analysis of recovery in forensic settings. In providing recovery-oriented care, nurses utilize pastoral power in guiding patients to institutionally preferred outcomes. Akin to Christian religious conversion, nurses engage in a neo-religious conversion of patients to a neoliberal subjectivity of homo-economicus. This path of recovery is grounded in an ethos of personal responsibility and self-government, inseparable from the greater context of neoliberal governmentality. Despite attempts at transforming forensic nursing practice into more egalitarian directions, recovery remains a coercive practice, and fails to meet the overall goals of this paradigm in secure settings.

INTRODUCTION: To address the rising presence of opioid use disorder in the United States, states have begun to implement specialized opioid intervention courts to provide immediate support for individuals at risk of opioid overdose. The present study sought to understand the motivations of women to engage in treatment while enrolled in an opioid intervention court. METHODS: We conducted 31 in-depth, qualitative interviews with women enrolled in an opioid intervention court in Buffalo, NY, to better understand their motivation regarding opioid use treatment. The data indicated a combined social-ecological and self-determination theory framework. RESULTS: Thematic analysis revealed four themes across the Social-Ecological Model that aligned with motivation-related needs of autonomy, competence, and relatedness, as defined by Self-Determination Theory. Themes at each level of the Social-Ecological Model described either support for or undermining of women’s motivation for treatment: (1) individual level: personal motivation for change, (2) interpersonal level: support for OUD treatment-related autonomy, competence, and relatedness, (3) community level: court systems provide pathways to treatment, and (4) society level: insufficient social resources can undermine competence. CONCLUSIONS: The findings provide new insights into various factors across all levels of the Social-Ecological Model that influence motivation for opioid use disorder treatment among women enrolled in opioid intervention court. Results support the possibility to integrate programs rooted in Self-Determination Theory to support opioid use disorder treatment among justice-involved persons.

There has been growing research interest in the effects that motivation plays in motor learning, and specifically how autonomy, competence, and social relatedness may directly benefit the learning process. Here, we present a preregistered manipulation of autonomy-support by providing learners with choice during the practice of a speed cup-stacking task. One group was given control over when a video demonstration was provided and the viewing speed. A yoked control group received an identical demonstration schedule, but without choice (as their schedule was matched to a participant with choice). Critically, we addressed a gap in the literature by adding a yoked group who was explicitly told that they were being denied choice and that their schedule was chosen by another participant. We found no statistically significant learning differences between groups, despite finding evidence that providing choice increased perceived autonomy. Equivalence tests further showed that although the groups were not statistically equivalent, the effect size is likely too small to practically study the effects of autonomy-support through choice in most motor learning labs. These findings add to a growing body of research that questions a causal role of autonomy-support on motor learning, and the robustness of the so-called self-controlled learning advantage. Self-determination theory (SDT) of human motivation was used to examine associations between different forms of motivation in Argus II retinal prosthesis users and their engagement and satisfaction with the Argus device.Nine subjects were administered: 1) a Situational Motivation Scale (SIMS) questionnaire to measure intrinsic motivation, identified regulation, external regulation, and amotivation, and 2) the Argus questionnaire (AQ) which was organized into 5 categories to measure 'Decision to get an Argus implant,' 'Self-perception as an Argus user', 'Utility of Argus,' 'Perceived competence,' and 'Family support.' Spearman correlations (Nine subjects completed both questionnaires. Statistically significant associations were observed between identified regulation and AQ items from categories: Decision to get Argus, Self-perception, Utility of Argus, and Perceived competence; and between intrinsic motivation and AQ items from Self-perception and Utility. External regulation was negatively associated with Family support, and amotivation was associated with one item from Self-perception. Engagement with the device and satisfaction were associated to both identified regulation and intrinsic motivation. There was no significant relationship between external regulation and adherence to the device.The SDT model can be used to investigate the types of motivation that influence uptake and engagement of the Argus device. Clinicians can use this knowledge to improve outcomes by supporting confidence in users and by encouraging them to maintain internalization and continued commitment to adherence.

Machine intelligence already helps medical staff with a number of tasks. Ethical decision-making, however, has not been handed over to computers. In this proof-of-concept study, we show how an algorithm based on Beauchamp and Childress' prima-facie principles could be employed to advise on a range of moral dilemma situations that occur in medical institutions. We explain why we chose fuzzy cognitive maps to set up the advisory system and how we utilized machine learning to train it. We report on the difficult task of operationalizing the principles of beneficence, non-maleficence and patient autonomy, and describe how we selected suitable input parameters that we extracted from a training dataset of clinical cases. The first performance results are promising, but an algorithmic approach to ethics also comes with several weaknesses and limitations. Should one really entrust the sensitive domain of clinical ethics to machine intelligence?

For years, the federal government has failed to uphold its promises to provide health care to Native Americans. These promises are echoed in treaties, the Constitution, and judicially-created law. As a result of this breach of promise and chronically underfunding, there are significant health disparities between indigenous populations and other Americans. In a recent 2020 case, McGirt v. Oklahoma, the U.S. Supreme Court held that both the federal government and individual states must follow the terms of a treaty made with a tribe, encouraging the possibility of direct health care funding. This reform, however, means little without tribal sovereignty and self-determination, which give dignity and decisionmaking capabilities back to a group that has long been without them. This Note explores two examples of self-determination in Native American health care, the Alaskan Native health care system and the recent vaccine rollout, proposing a framework for increasing self-determination in health care to provide support for funding reform, which becomes increasingly necessary as Native Americans continue to struggle to access health care.

INTRODUCTION Self-determination theory proposes that autonomy-supportive environments produce better health outcomes, and evidence demonstrating the importance of autonomy support for health behavior change has become increasingly clear. The bulk of the previous research has focused on the effects of receiving autonomy support, whereas the present study focuses on the potential impact of providing autonomy support for improving the weight loss and relationship satisfaction of the support provider. METHOD Sixty-four couples were randomized to a standard weight loss intervention or to an intervention that included training to enhance autonomy support behavior (e.g., empathic responding, accepting personal choices, etc.). Groups met weekly for 6 months, with assessments at baseline, 3, 6, and 12 months. The assessment of provided autonomy support was based on ratings of videotaped interactions of the couple discussing weight loss issues and on partner report of the autonomy support they received. RESULTS The results did not show any condition effects; however, the results for both conditions indicated that beyond receiving autonomy support, providing autonomy support was associated with greater sustained weight loss for the support provider, and for men greater relationship satisfaction over time. DISCUSSION The results confirm the importance of couples-based interventions and autonomy support for behavior change. In addition, it appears that providing autonomy support is useful for both the recipient and the provider. This paper explores contemporary approaches to balancing truth with the provision of hope during the disclosure of a dementia diagnosis. We discuss the ethical significance of these practices as they relate to each member of the triad – the person, the carer and the clinician – at the point of diagnosis and beyond. The process of disclosing a diagnosis of dementia is complex. It encompasses breaking bad news while balancing hope, with truth about a progressive life-limiting condition. The process of receiving the diagnosis likewise challenges the person who may be unprepared for the diagnosis, while carers seek information and supports. The impact of receiving a diagnosis of dementia can be life-changing and harmful at the personal level – for both the person and carer. This risk of harm becomes a critical consideration for clinicians when deciding on the level of truth: what information should be relayed and to whom? That risk is also balanced against the ethical issue of patient autonomy, which includes the right to know (or not) and make informed decisions about therapeutic interventions. While the consensus is that the autonomy of the person living with dementia must be upheld, controversy exists regarding the extent to which this should occur. For instance, at diagnosis, it is common for clinicians to use euphemisms rather than the word dementia to maintain hope, even though people and carers prefer to know the diagnosis. This practice of therapeutic lying is a pervasive ethical issue in dementia care, made more acceptable by its roots in diagnosis disclosure.

In debates over the regulation of communication related to dual-use research, the risks that such communication creates must be weighed against against the value of scientific autonomy. The censorship of such communication seems justifiable in certain cases, given the potentially catastrophic applications of some dual-use research. This conclusion however, gives rise to another kind of danger: that regulators will use overly simplistic cost-benefit analysis to rationalize excessive regulation of scientific research. In response to this, we show how institutional design principles and normative frameworks from free speech theory can be used to help extend the argument for regulating dangerous dual-use research beyond overly simplistic cost-benefit reasoning, but without reverting to an implausibly absolutist view of scientific autonomy.

Health behaviors are the cornerstone of cardiovascular risk reduction but change is challenging and maintenance is uncommon. The use of Self-Determination Theory (SDT) can promote long-term change however the pathway is uncertain related to cardiovascular risk reduction. A multi-disciplinary clinical team trained in SDT counseled 294 individuals with high cardiovascular risk. Our participants had a significant decrease in cholesterol as well as a significant increase healthy diet and motivation measures. Autonomous motivation was a critical element in the pathways. We demonstrated that a SDT team-based clinical intervention can promote cardiovascular risk reduction through autonomous motivation.

Life and health as fundamental matters are major concerns for every human being. To this end, he / she should be assisted by the community in which they live. If the foundation of social life is the personalistic principle, the recognition of the primacy of the dignity of the person, among other values, is constructed on the principles of subsidiarity and solidarity. It is the role of the healthcare institution to create such conditions, in which the safest and the most effective methods of treatment and prevention are available, and the individual can consciously select which of these realises his or her good to the greatest extent, in accordance with the recognised hierarchy of values, the accepted worldview, and life goals. The aim of this article is to analyse the relationship between the community and the individual, in the field of healthcare, under normal conditions and during epidemics. The existence of specific threats may suggest that exceptions to the fundamental rules of social life: autonomy, subsidiarity, and social solidarity, are permissible. However, the recognition of the fundamental value of the dignity of the person, requires the community to apply the same rules as in normal times, although many activities of public institutions are intensified in times of more serious danger.

Resumo Esta pesquisa destaca a moradia como uma das dimensões da vida de indivíduos que vivenciam sofrimentos psíquicos graves, que reverbera em seu cuidado e plano terapêutico singular. O objetivo deste artigo é apreender os sentidos das experiências de morar para os usuários dos Centros de Atenção Psicossocial (CAPS) do município de Santos, estado de São Paulo. Pesquisa qualitativa realizada com usuários de diferentes serviços por meio de entrevistas semiestruturadas submetidas a análise de conteúdo temática. O estudo identificou que a noção de casa remete a proteção, individualidade e memórias afetivas que corroboram a construção de uma vida concreta ampliada no viver em liberdade na cidade, apresentando moradias plurais, localizadas em territórios distintos. As formas de apropriação dos espaços demonstraram as possibilidades e os desafios próprios das trocas sociais e vivências cotidianas. Estar e habitar uma casa representa ter acesso a bens materiais e subjetivos, assim como a garantia do cuidado em liberdade. Entretanto, ainda existem limitações no exercício da efetiva participação social. Abstract This research highlights housing as one of the dimensions of the lives of individuals who experience severe psychological suffering, which reverberates in their care and unique therapeutic plan. This paper aims to grasp the Psychosocial Care Centers (CAPS) users’ meanings of their experiences of living in Santos (SP), Brazil. This qualitative research was conducted with users of different services through semi-structured interviews submitted to the thematic content analysis. This study identified how the conception of housing relates to protection, individuality, and affective memories that corroborate the construction of an expanded concrete life regarding living in freedom in the city, presenting plural housing located in different territories. Taking ownership of spaces reveals the possibilities and challenges inherent to social exchanges and daily experiences. Being and living in a house means having access to material and subjective goods, besides assuring care in freedom. However, there are still limitations in the effective exercise of social participation.

Although adults who sustain a severe traumatic brain injury (TBI) require support to make decisions in their lives, little is known about their experience of this process. The aim of this study was to explore how participation in decision making contributes to self-conceptualization in adults with severe TBI.We used constructivist grounded theory methods. Data included 20 in-depth interviews with adults with severe TBI. Through a process of constant comparison, analysis involved open and focused coding until clear categories emerged and data saturation was achieved.Self-conceptualization emerged as a complex and multifaceted process, as individuals with TBI aimed to reestablish a sense of autonomy. We describe a recursive relationship in which decision-making participation assists the dynamic construction of self, and self-concept contributes to the experience of making decisions. The role of an individual's social support network in acting as a bridge between participation and self-conceptualization is presented.Findings emphasize that contributing to decisions about one's own goals across a range of life areas can reinforce a positive self-concept. It is vital that supporters understand that participation in decision making provides a pathway to conceptualizing self and aim to maximize the person's participation in the decision-making process. Implications for Rehabilitation Previous research has identified that the experience of sustaining TBI has a significant impact on a person's conceptualization of self. This study identified that decision-making experiences play an important role in the ongoing process of self-conceptualization after injury. Decision-making experiences can reinforce a person's self-concept or lead them to revise (positively or negatively) their sense of self. By maximizing the person's decision-making participation, those around them can support them to develop positive self-attributes and contribute to shaping their future goals.

Resumo O presente trabalho analisa como a autonomia e a dignidade do indivíduo em casos de terminalidade pode ser preservada por meio das diretivas antecipadas de vontade. Entende-se que se deve buscar uma forma, ainda não encontrada e implantada no Brasil, de fazer que o indivíduo, mesmo incapaz de se comunicar, fique seguro do conhecimento de sua vontade por parte dos profissionais de saúde. Como método, o estudo busca aprofundar conceitos utilizados pela doutrina, resoluções e leis. Conclui-se que é preciso dar mais publicidade às diretivas antecipadas de vontade, informando a população sobre sua existência e permitindo que as pessoas expressem seus desejos relativos à saúde. Ademais, é necessário criar um banco de dados que permita o compartilhamento da manifestação de vontade do indivíduo com a rede hospitalar. Abstract This article analyzes how the autonomy and dignity of individuals in cases of terminality can be preserved using the advance directives of will. It is understood that a procedure, yet to be found or implemented in Brazil, should be sought to ensure that individuals, even when incapable of communicating, can be assured that their will by health professionals. As a method, this study seeks to deepen concepts used by doctrine, resolutions and laws. It is concluded more publicity should be assigned to the advance directives of will, informing the population about their existence and allowing people to express their desires regarding health. Furthermore, it is necessary to create a database that allows sharing an individual’s expression of will with the hospital network. Resumen El presente trabajo analiza cómo la autonomía y dignidad del individuo en casos de terminalidad puede ser preservada a través de las directivas anticipadas de la voluntad. Se entiende que se debe buscar un camino, aún no encontrado e implantado en Brasil, para que el individuo, incluso incapaz de comunicarse, esté seguro del conocimiento de su voluntad por parte de los profesionales de la salud. Como método, el estudio busca profundizar conceptos utilizados por doctrina, resoluciones y leyes. Se concluye que es necesario dar más publicidad a las directivas anticipadas de voluntad, informando a la población sobre su existencia y permitiendo que las personas expresen sus deseos con respecto a la salud. Además, es necesario crear una base de datos que permita compartir la manifestación de la voluntad del individuo con la red hospitalaria.

Resumo Embasado em Axel Honneth, este estudo teórico descreve elementos da teoria do reconhecimento e suas interfaces com a autonomia de usuários(as) no cuidado em saúde, retratando as esferas do direito, do amor e da solidariedade em suas interfaces com a autonomia. A partir de considerações reflexivas sobre a prática profissional e o cuidado ofertado, conclui-se que a teoria do reconhecimento pode servir como estrutura que amplia a compreensão de situações do contexto de saúde, especialmente aquelas em que os elementos da esfera de reconhecimento são suprimidos ou prejudicados. A translação da teoria para o campo do cuidado de saúde abre perspectivas interessantes para compreender o valor do direito, do amor, da solidariedade na saúde e de possíveis consequências de sua inobservância. Abstract Based on Axel Honneth, this theoretical study describes elements of the theory of recognition and its interfaces with user autonomy in healthcare, describing how the spheres of rights, love, and solidarity intersect with autonomy. From reflections on professional practice and the care offered, one concludes that the theory of recognition can serve as a framework that expands the understanding of healthcare situations, especially those in which elements of recognition are suppressed or impaired. Applying the theory to the field of healthcare opens interesting perspectives for understanding the value of rights, love, and solidarity in healthcare and the possible consequences of their non-observance. Resumen Basado en Axel Honneth, este estudio teórico describe elementos de la teoría del reconocimiento y sus interfaces con la autonomía de los(las) usuarios(as) en el cuidado en salud, retratando las esferas del derecho, del amor y de la solidaridad en sus interfaces con la autonomía. Con base en las consideraciones reflexivas sobre la práctica profesional y el cuidado ofrecido, se concluye que la teoría del reconocimiento puede servir como un marco que amplíe la comprensión de las situaciones en el contexto sanitario, especialmente aquellas en las que los elementos de la esfera del reconocimiento están suprimidos o perjudicados. La traslación de la teoría al ámbito del cuidado de la salud abre interesantes perspectivas para comprender el valor del derecho, del amor, de la solidaridad en la salud, y las posibles consecuencias de su desconocimiento.

Obstetric anesthesiologists can use the tenets of principlism to identify ethically supportable decisions. This approach begins with recognizing and upholding the fiduciary obligations that every physician has to their patient. For the pregnant patient, these obligations are both autonomy- and beneficence-based. Additional beneficence-based obligations arise in cases where the fetus is also considered a patient. Ethical dilemmas result from conflict between any of these obligations. A shared decision-making model that incorporates the foundational principles of ethics can be used to resolve these conflicts, leaving clinicians and patients confident that a sound decision-making process has taken place and an ethically supportable decision has been reached.

Individual patient autonomy is a core ethical consideration in Western bioethics, which has traditionally referred to respecting the ability of capable patients to formulate their own medical decisions. Increasing cultural diversity within the US warrants transition to medical models aligned with moral pluralism and a relational approach to autonomy, focused on the patient as embedded within a social network, with family as a central component of decision making. Standardized cultural assessments and increased provider training in cultural competence and humility can assist in addressing cultural gaps in mental health care to reduce barriers in mental health care access among minorities.

Resumo Em razão das possibilidades oferecidas pela tecnociência, a obstinação terapêutica se tornou frequente. Buscando evitar tal prática, profissionais de saúde vivenciam dilemas éticos de manutenção ou suspensão de tratamentos considerados inúteis. O estudo descreve a percepção do profissional de saúde diante da distanásia e reflete sobre aspectos bioéticos envolvidos em questões inerentes ao ser humano. Trata-se de revisão integrativa da literatura realizada a partir de trabalhos publicados nas bases de dados científicos SciELO e BVSalud, no período de 2010 a 2020. Cuidados paliativos e princípios bioéticos são os principais aliados para a recuperação da dignidade do paciente, sendo necessária legislação específica para respaldo do profissional e do paciente. A distanásia consiste em prolongar a vida de enfermos considerados incuráveis, o que, além de não estar de acordo com o princípio da beneficência, resulta em maleficência, devido à exposição à grande incidência de dor e desconforto. Abstract The possibilities offered by technoscience turned therapeutic obstinacy into a frequent occurrence. Seeking to avoid this practice, health professionals experience ethical dilemmas of maintenance or suspension of treatments considered useless. This study describes the health professional’s perception of dysthanasia and reflects on bioethical aspects involved in issues inherent to human beings. This is an integrative literature review carried out from articles published in the scientific databases SciELO and BVSalud, from 2010 to 2020. Palliative care and bioethical principles are the main allies for the recovery of a patient’s dignity, requiring specific legislation to support the professional and the patient. Dysthanasia consists of prolonging the life of patients considered incurable, which, in addition to not being in accordance with the principle of beneficence, results in maleficence due to exposure of the patient to a high incidence of pain and discomfort. Resumen En virtud de las posibilidades ofrecidas por la tecnociencia, la obstinación terapéutica se hizo frecuente. Para evitar dicha práctica, profesionales de la salud viven dilemas éticos con el mantenimiento o la suspensión de tratamientos considerados inútiles. El estudio describe la percepción del profesional de la salud ante la distanasia y reflexiona sobre aspectos bioéticos intervinientes en cuestiones inherentes al ser humano. Se trata de una revisión integradora de la literatura elaborada a partir de trabajos publicados en las bases de datos científicas SciELO y BVSalud, durante el período entre 2010 y 2020. Los cuidados paliativos y los principios bioéticos son los principales aliados para la recuperación de la dignidad del paciente, aunque se necesita una legislación específica para respaldar al profesional y al paciente. La distanasia consiste en prolongar la vida de enfermos considerados incurables, lo que, además de no estar de acuerdo con el principio de la beneficencia, tiene como resultado la maleficencia, debido a que están expuestos a una gran incidencia de dolor y molestias.

Within bioethics, Kant’s conception of autonomy is often portrayed as excessively rationalistic, abstract, and individualistic, and, therefore, far removed from the reality of patients’ needs. Drawing on recent contributions in Kantian philosophy, we argue that specific features of Kantian autonomy remain relevant for medical ethics and for patient experience. We use contemporary end-of-life illness narratives—a resource that has not been analyzed with respect to autonomy—and show how they illustrate important Kantian themes, namely, the duty to know oneself, the interest in elaborating universalizable principles, and the emphasis on ideals as points of orientation that guide behavior without ever being fully realized. As Kant does, the patient-authors discussed here perceive the end of life as a moment to reflect on the constitutive principles which have governed that life, thereby offering a privileged moment to pursue self-knowledge. We argue that bioethical conceptions of autonomy stand to gain if they revise their conception of Kantian moral philosophy as too formal, abstract, and detached from emotions and personal relationships to be helpfully applied.

Recycling is a process carried out by various organizations and individuals to enhance the environment’s long-term sustainability. Some youth think that recycling is a monotonous action as it may seem inconvenient, less aware of the environmental issues and more time-consuming than they think and rather go for video games. Therefore, this study investigates the relationship between motivation and recycling intention in gamified learning among youth. To address the research aim, this study uses gamification as a motivational driver for a game-like learning experience to improve recycling intentions among youth. Self-determination theory (SDT) and the theory of Planned Behavior (TPB) will be this study's main motivational and behavioral theories. (n = 124) high schools and college students were invited to take part in an online gamified recycling activity, Edcraft Gamified Learning (EGL), consisting of two levels of gamified unused plastic-crafting recycling activities. After the activity, the participants will answer a post-event questionnaire and the data collected were analyzed. The result shows that controlled motivation (CM) and autonomous motivation (AM) positively influenced youth attitudes and social norms. Besides, attitude is the only psychosocial determinant that positively influences the recycling intention of the youth. Gamification only moderates positively between attitude and recycling intention. This study has clearly shown the effectiveness of gamified learning activity towards recycling intention directly and as a component that moderates the relationship between attitude and recycling intention, which shows a favorable evaluation towards recycling intention with gamified learning involved. Moreover, the findings showed that not all relationships are positive in a gamified learning environment, and it gives a good view on the weakness and strengths with the guideline of SDT and TPB.

BackgroundAlthough prior research shows supportive evidence that parental practices are associated with adolescents' prosocial behaviors, limited evidence exists on the effects of parents' use of social and material rewards on distinct forms of prosocial behaviors, and the mediating effects of sociocognitive and socioemotive traits in these relations.AimsThe present study was designed to examine the longitudinal relations among parents' use of social and material rewards, youth prosocial traits, and prosocial behaviors.Materials & methodsParticipants were 417 adolescents (M age = 14.70 years; 225 girls) from Valencia, Spain who completed surveys on parents' use of social and material reward practices, prosocial moral reasoning, empathic concern, and six types of prosocial behaviors.ResultsPath analyses showed that parents' use of social rewards was indirectly, positively related to emotional, dire, altruistic, public (negatively), and compliant prosocial behaviors via empathic concern. The use of social rewards was also indirectly positively linked to altruistic prosocial behaviors via both empathic concern and prosocial moral reasoning. In contrast, parents' use of material rewards predicted less prosocial moral reasoning, which in turn, was linked to more altruistic, prosocial behaviors.Discussion and conclusionsThe implications for parental socialization and self-determination theories of prosocial and moral development are discussed.

For undergraduate nursing students, an integral mode of learning is their clinical placements. Learning within the clinical setting is influenced by factors such as the facilitators, peers, unit, or department the student is placed on. What is not known is the role of a social context that is grounded in Self-Determination Theory and how this influences the development of therapeutic relationship skills among undergraduate nursing students. The aim of this study was to examine the role of autonomy support in the professional learning of undergraduate nursing students. This study employed a two-group pre-test and post-test design, with undergraduate nursing students placed in either an autonomy-supportive group or a comparison group in an Australian context. Therapeutic relationship skills were assessed at the beginning and end of their compulsory mental health clinical placement. Participants were 210 (n = 210) Australian undergraduate nursing students. Using a tertile split, 140 students were placed into one of two groups based on their perception of autonomy support at the post-test time point. Pre- and post-test therapeutic relationship data were collected for both groups. A repeated measures analysis of variance was used to examine whether engagement in different clinical placements would illicit significant differences in therapeutic relationship skills. Undergraduate nursing students who were engaged in the autonomy-supportive setting had significantly higher scores associated with the therapeutic relationship subscales: Positive Collaboration and Positive Clinical Input. The importance of the social context is underscored, which is strongly influenced by the educational leader, typically referred to as a nurse facilitator.
