Background: Colorectal cancer is the third leading cause of cancer deaths worldwide. Given the recent increasing trends in colorectal cancer incidence globally, up-to-date information on the colorectal cancer burden could guide screening, early detection, and treatment strategies, and help effectively allocate resources. We examined the temporal patterns of the global, regional, and national burden of colorectal cancer and its risk factors in 204 countries and territories across the past three decades. Methods: Estimates of incidence, mortality, and disability-adjusted life years (DALYs) for colorectal cancer were generated as a part of the Global Burden of Diseases, Injuries and Risk Factors Study (GBD) 2019 by age, sex, and geographical location for the period 1990-2019. Mortality estimates were produced using the cause of death ensemble model. We also calculated DALYs attributable to risk factors that had evidence of causation with colorectal cancer. Findings: Globally, between 1990 and 2019, colorectal cancer incident cases more than doubled, from 842 098 (95% uncertainty interval [UI] 810 408-868 574) to 2·17 million (2·00-2·34), and deaths increased from 518 126 (493 682-537 877) to 1·09 million (1·02-1·15). The global age-standardised incidence rate increased from 22·2 (95% UI 21·3-23·0) per 100 000 to 26·7 (24·6-28·9) per 100 000, whereas the age-standardised mortality rate decreased from 14·3 (13·5-14·9) per 100 000 to 13·7 (12·6-14·5) per 100 000 and the age-standardised DALY rate decreased from 308·5 (294·7-320·7) per 100 000 to 295·5 (275·2-313·0) per 100 000 from 1990 through 2019. Taiwan (province of China; 62·0 [48·9-80·0] per 100 000), Monaco (60·7 [48·5-73·6] per 100 000), and Andorra (56·6 [42·8-71·9] per 100 000) had the highest age-standardised incidence rates, while Greenland (31·4 [26·0-37·1] per 100 000), Brunei (30·3 [26·6-34·1] per 100 000), and Hungary (28·6 [23·6-34·0] per 100 000) had the highest age-standardised mortality rates. Abstract: Nowadays, nano-platforms designed for drug delivery systems (DDSs) such as polymers, liposomes, and micelles have been demonstrated to be clinically efficient. The sustained drug release is one of the advantages of DDSs, especially polymer-based nanoparticles. The formulation could enhance the drug's durability, in which the biodegradable polymers are the most interesting building blocks of DDSs. Nano-carriers could circumvent many issues by localized drug delivery and release via certain internalization routes such as intracellular endocytosis paths and increasing biocompatibility. Polymeric nanoparticles and their nanocomposite are one of the most important classes of materials that can be used for the assembly of nanocarriers that can form complex, conjugated and encapsulated forms. The site-specific drug delivery may arise from the ability of nanocarriers to pass through the biological barrier, their specific interactions with receptors, and passive targeting. The better circulation, uptake, and stability along with targeting attributes lead to lesser side effects and damage to normal cells. Hence, in this review, the most recent achievements on polycaprolactone-based or -modified nanoparticles in drug delivery systems (DDSs) for 5-fluorouracil (5-FU) are presented.

Introduction: It is reported that migraine may be a risk factor for brain cancers. Since one of the best ways to assess this possible relationship is to study the molecular mechanism, here the common central dysregulated proteins between these diseases are investigated via network analysis. Methods: The dysregulated proteins of migraine and gliosarcoma are extracted from the STRING database and interacted via Cytoscape software, version 3.7.2. to form two separate networks. Central nodes of the networks are compared to find the common central district proteins. First neighbors of the common central proteins are studied. Results: The number of 11 hub bottlenecks was identified for each of the migraine and gliosarcoma cancer networks. Albumin (ALB) and interleukin 6 (IL6) were introduced as common differentially expressed central proteins. Kininogen 1 (KNG1), vascular endothelial growth factor A (VEGFA), and neurofibromatosis type I (NF1) the first neighbors of ALB-IL6 were connected to the central nodes of networks of the two studied diseases. Conclusion: ALB and IL6 can be considered molecular links between migraine and brain cancers.

Objectives: Inflammation has been proposed to be one of the main causes of musculoskeletal pain. Diet is a lifestyle factor that plays an important role in managing inflammation; thus, we assessed the inflammatory potential of diets using the empirical dietary inflammatory index (EDII) to investigate the relationship between diet and musculoskeletal pain.Methods: This cross-sectional study included 212 elderly individuals who were selected from health centers in Tehran, Iran. Dietary intake was evaluated using a valid and reliable 147-item food frequency questionnaire. To measure the intensity of pain, a visual analogue scale was used. Multiple linear regression was applied to assess the association between the EDII and musculoskeletal pain.Results: In total, 62.7% and 37.3% of participants had mild and severe pain, respectively. The EDII values were 0.97±0.72 and 1.10±0.66, respectively, in those with mild and severe pain. Background: Updated data on chronic respiratory diseases (CRDs) are vital in their prevention, control, and treatment in the path to achieving the third UN Sustainable Development Goals (SDGs), a one-third reduction in premature mortality from non-communicable diseases by 2030. We provided global, regional, and national estimates of the burden of CRDs and their attributable risks from 1990 to 2019.Methods: Using data from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019, we estimated mortality, years lived with disability, years of life lost, disability-adjusted life years (DALYs), prevalence, and incidence of CRDs, i.e. chronic obstructive pulmonary disease (COPD), asthma, pneumoconiosis, interstitial lung disease and pulmonary sarcoidosis, and other CRDs, from 1990 to 2019 by sex, age, region, and Socio-demographic Index (SDI) in 204 countries and territories. Deaths and DALYs from CRDs attributable to each risk factor were estimated according to relative risks, risk exposure, and the theoretical minimum risk exposure level input. Findings: In 2019, CRDs were the third leading cause of death responsible for 4.0 million deaths (95% uncertainty interval 3.6–4.3) with a prevalence of 454.6 million cases (417.4–499.1) globally. While the total deaths and prevalence of CRDs have increased by 28.5% and 39.8%, the age-standardised rates have dropped by 41.7% and 16.9% from 1990 to 2019, respectively. COPD, with 212.3 million (200.4–225.1) prevalent cases, was the primary cause of deaths from CRDs, accounting for 3.3 million (2.9–3.6) deaths. With 262.4 million (224.1–309.5) prevalent cases, asthma had the highest prevalence among CRDs. The age-standardised rates of all burden measures of COPD, asthma, and pneumoconiosis have reduced globally from 1990 to 2019. Nevertheless, the age-standardised rates of incidence and prevalence of interstitial lung disease and pulmonary sarcoidosis have increased throughout this period. Low- and low-middle SDI countries had the highest age-standardised death and DALYs rates while the high SDI quintile had the highest prevalence rate of CRDs. The highest deaths and DALYs from CRDs were attributed to smoking globally, followed by air pollution and occupational risks. Non-optimal temperature and high body-mass index were additional risk factors for COPD and asthma, respectively.Interpretation: Albeit the age-standardised prevalence, death, and DALYs rates of CRDs have decreased, they still cause a substantial burden and deaths worldwide. The high death and DALYs rates in low and low-middle SDI countries highlights the urgent need for improved preventive, diagnostic, and therapeutic measures. Global strategies for tobacco control, enhancing air quality, reducing occupational hazards, and fostering clean cooking fuels are crucial steps in reducing the burden of CRDs, especially in low- and lower-middle income countries. BACKGROUND: The academic literature has not arrived at a consensus on the importance of body mass index (BMI) as an indicator of surgical feasibility and risk. This study evaluates board-certified plastic surgeons' and trainees' knowledge, experiences, and concerns around performing benign breast surgeries in high-BMI patients. METHODS: An online survey instrument was developed and shared with plastic surgeons and plastic surgery trainees from December 2021 to January 2022. RESULTS: There were 30 respondents (18 from Israel, 11 from the United States, and 1 from Turkey). Aim: The present systematic review and meta-analysis was designed to assess the impact of Helicobacter pylori infection on the efficacy of immune checkpoint inhibitors (ICIs). Materials & methods: PubMed, Scopus, Web of Science and EMBASE databases were systematically searched up to 1 February 2023. Results: Three studies comprising 263 patients treated with ICIs were included. The results of pooled analysis showed that H. pylori infection was associated with reduced overall survival and progression-free survival. Furthermore, the rate of progressive disease after administration of ICIs was higher in H. pylori-positive patients relative to H. pylori-negative patients. Conclusion: H. pylori infection status is a novel potential response biomarker for predicting the efficacy of ICIs in different cancers.

Objectives: Hanging is a common method of attempted suicide. This study investigated the epidemiological profile of attempted and completed suicides by hanging in southern Iran.Methods: This cross-sectional study was performed on 1167 suicide attempts by hanging between 2011 and 2019. All data related to suicide attempts by hanging were collected from the Fars Suicide Surveillance System. The trends in suicide cases and the mean age of attempted and completed suicides were plotted. The chi-square test was used to identify suicide-related factors. Crude rates of incidence, mortality, and standardized fatality during the study period were calculated. Finally, logistic regression was used to identify the predictors of death in individuals who attempted suicide.Results: The mean age of those who attempted suicide was 33.21±16.82 years; the majority were male (80.5%). The rate of attempted and completed suicide by hanging were 3.50 and 2.79 per 100 000 people, respectively. The case-fatality rate was calculated as 79.34%. The results of our study indicated an increasing trend in suicide attempts by hanging. The likelihood of death was 2.28 times higher in individuals with a previous history of suicide attempts and 1.85 times higher in those with a psychological disorder.Conclusions: The findings of this study suggest an increasing trend in attempted and completed suicide by hanging, especially among individuals with a history of suicide attempts and psychological disorders. It is necessary to take action to reduce the rate of suicide attempts and identify the underlying causes of suicide attempts by hanging.

BACKGROUND AND PURPOSE: Infants’ sleep disorders and parents’ insufficient sleep are common problems in the infant care. The current study was conducted to assess the effectiveness of infant massage on infants’ night-time sleep condition and mothers’ sleep quality. PROCEDURES: 140 infants were randomly put into two different groups, experimental group with fifteen-minute bedtime messages for two weeks and the control group with normal infant routine care. The Brief Infant Sleep Questionnaire, a personal information submission form, and Pittsburgh Sleep Quality Index for the mothers were the tools used to gather data in this study. RESULTS: Infants in experimental group showed meaningful differences in variables such as, sleep latency (P < 0001, eta = 0.099), number of night waking (P = 0.03, eta = 0.027) and longest continuous sleep period (P = 0.03, eta = 0.026). As for other variables no meaningful differences were observed. There wasn’t meaningful difference in the mother’s overall night-time sleep quality between the two groups (P = 0.184, eta = 0.012) except for the duration of the mother’s night-time sleep (P = 0.028, eta = 0.026) and the reduction of maternal sleep disorder (P = 0.020 eta = 0.029). CONCLUSION: The findings indicated that infants’ bedtime massages would improve some of the sleep markers of mothers and infants, and therefore, can be suggested as a practical, harmless, and cost-free method to improve sleep.

19. Monitoring Trends of CO, NO2, SO2, and O3 Pollutants Using Time-Series Sentinel-5 Images Based on Google Earth Engine

Air pollution (AP) is a significant risk factor for public health, and its impact is becoming increasingly concerning in developing countries where it is causing a growing number of health issues. It is therefore essential to map and monitor AP sources in order to facilitate local action against them. This study aims at assessing the suitability of Sentinel-5 AP products based on Google Earth Engine (GEE) to monitor air pollutants, including CO, NO2, SO2, and O3 in Arak city, Iran from 2018 to 2019. Our process involved feeding satellite images to a cloud-free GEE platform that identified pollutant-affected areas monthly, seasonally, and annually. By coding in the JavaScript language in the GEE, four pollution parameters of Sentinel-5 satellite images were obtained. Following that, images with clouds were filtered by defining cloud filters, and average maps were extracted by defining average filters for both years. The employed model, which solely used Sentinel-5 AP products, was tested and assessed using ground data collected from the Environmental Organization of Central Province. Our findings revealed that annual CO, NO2, SO2, and O3 were estimated with RMSE of 0.13, 2.58, 4.62, and 2.36, respectively, for the year 2018. The annual CO, NO2, SO2, and O3 for the year 2019 were also calculated with RMSE of 0.17, 2.41, 4.31, and 4.6, respectively. The results demonstrated that seasonal AP was estimated with RMSE of 0.09, 5.39, 0.70, and 7.81 for CO, NO2, SO2, and O3, respectively, for the year 2018. Seasonal AP was also estimated with RMSE of 0.12, 4.99, 1.33, and 1.27 for CO, NO2, SO2, and O3, respectively, for the year 2019. The results of this study revealed that Sentinel-5 data combined with automated-based approaches, such as GEE, can perform better than traditional approaches (e.g., pollution measuring stations) for AP mapping and monitoring since they are capable of providing spatially distributed data that is sufficiently accurate.

21. Impact of Region-of-Interest Delineation on Stability and Reproducibility of Liver SNR Measurements in 68Ga-PSMA PET/CT

Objective This study aims to assess the impact of various regions of interest (ROIs) and volumes of interest (VOIs) delineations on the reproducibility of liver signal-to-noise-ratio (SNRliver) measurements, as well as to find the most reproducible way to estimate it in gallium-68 positron emission tomography (68Ga-PET) imaging. We also investigated the SNRliver-weight relationship for these ROIs and VOIs delineations. Methods A cohort of 40 patients (40 males; mean weight: 76.5 kg [58–115 kg]) with prostate cancer were included. 68Ga-PET/CT imaging (mean injected activity: 91.4 MBq [51.2 MBq to 134.1 MBq] was performed on a 5-ring bismuth germanium oxide-based Discovery IQ PET/CT using ordered subset expectation maximization image reconstruction algorithm. Afterward, circular ROIs and spherical VOIs with two different diameters of 30 and 40 mm were drawn on the right lobe of the livers. The performance of the various defined regions was evaluated by the average standardized uptake value (SUVmean), standard deviation (SD) of the SUV (SUVSD), SNRliver, and SD of the SNRliver metrics. Results There were no significant differences in SUVmean among the various ROIs and VOIs (p > 0.05). On the other hand, the lower SUVSD was obtained by spherical VOI with diameter of 30 mm. The largest SNRliver was obtained by ROI (30 mm). The SD of SNRliver with ROI (30 mm) was also the largest, while the lowest SD of SNRliver was observed for VOI (40 mm). There is a higher correlation coefficient between the patient-dependent parameter of weight and the image quality parameter of SNRliver for both VOI (30 mm) and VOI (40 mm) compared to the ROIs. Conclusion Our results indicate that SNRliver measurements are affected by the size and shape of the respective ROIs and VOIs. The spherical VOI with a 40 mm diameter leads to more stable and reproducible SNR measurement in the liver.

In recent years, the level of acceptance of autonomous vehicles (AVs) has changed with the advent of new sensor technologies and the proportional increase in market perception of these vehicles. Our study provides an overview of the relevant existing studies in order to consolidate current knowledge and pave the way for future studies in this area. The paper first reviews studies investigating the market acceptance of AVs. We identify the nonbehavioural factors that account for the level of acceptance and examine these in detail by cross-referencing the results of relevant papers published between 2014 and 2021 to reach a consensus on the perceived benefits and concerns. The findings showed that previous studies have found legal liability, safety, privacy, security, traffic conditions, and cost to be key external factors influencing the acceptance or rejection of AVs, and that the upsides of adopting AVs in regard to improving traffic conditions and safety outweigh the risks identified in relation to these areas. This resulted in an overall weighted average of 65% market acceptance of AVs among the 11,057 people surveyed in this regard. However, the remaining respondents were not very favourably disposed towards adopting AVs because of unresolved issues related to data privacy, security breaches, and legal liability in the event of accidents. In addition, our evaluation showed that the worldwide market purchasing power for an AV, based on 2022 prices, is around $38k, which is significantly below the current anticipated price of $100k.

Monkeypox is a zoonotic virus that has recently affected different countries worldwide. On July 23, 2022, the WHO declared the outbreak of monkeypox as a public health emergency of international concern. Surveillance studies conducted in Central Africa in the 1980s and later during outbreaks in the same region showed smallpox vaccines to be clinically somewhat effective against Monkeypox virus. However, there is no specific vaccine against this virus. This research used bioinformatics techniques to establish a novel multi-epitope vaccine candidate against Monkeypox that can induce a strong immune response. Five well-known antigenic proteins (E8L, A30L, A35R, A29L, and B21R) of the virus were picked and assessed as possible immunogenic peptides. Two suitable peptide candidates were selected according to bio-informatics analysis. Based upon in silico evaluation, two multi-epitope vaccine candidates (ALALAR and ALAL) were built with rich-epitope domains consisting of high-ranking T and B-cell epitopes. After predicting and evaluating the 3D structure of the protein candidates, the most efficient 3D models were considered for docking studies with Toll-like receptor 4 (TLR4) and the HLA-A * 11:01, HLA-A*01:01, HLA-A*02:01, HLA-A*03:01, HLA-A*07:02, HLA-A*15:01, HLA-A*30:01 receptors. Subsequently, molecular dynamics (MD) simulation of up to 150 nanoseconds was employed to assess the durability of the interaction of the vaccine candidates with immune receptors. MD studies showed that M5-HLA-A*11:01, ALAL-TLR4, and ALALAR-TLR4 complexes were stable during simulation. Analysis of the in silico outcomes indicates that the M5 peptide and ALAL and ALALAR proteins may be suitable vaccine candidates against the Monkeypox virus. Communicated by Ramaswamy H. Sarma

We study a multi-objective multi-armed bandit problem in a dynamic environment. The problem portrays a decision-maker that sequentially selects an arm from a given set. If selected, each action produces a reward vector, where every element follows a piecewise-stationary Bernoulli distribution. The agent aims at choosing an arm among the Pareto optimal set of arms to minimize its regret. We propose a Pareto generic upper confidence bound (UCB)-based algorithm with change detection to solve this problem. By developing the essential inequalities for multi-dimensional spaces, we establish that our proposal guarantees a regret bound in the order of $\gamma_T\log(T/{\gamma_T})$ when the number of breakpoints $\gamma_T$ is known. Without this assumption, the regret bound of our algorithm is $\gamma_T\log(T)$. Finally, we formulate an energy-efficient waveform design problem in an integrated communication and sensing system as a toy example. Numerical experiments on the toy example and synthetic and real-world datasets demonstrate the efficiency of our policy compared to the current methods.

Background:The scale of the 2022 global mpox (formerly known as monkeypox) outbreak has been unprecedented. In less than 6 months, non-endemic countries have reported more than 67 000 cases of a disease that had previously been rare outside of Africa. Mortality has been reported as rare but hospital admission has been relatively common. We aimed to describe the clinical and laboratory characteristics and outcomes of individuals admitted to hospital with mpox and associated complications, including tecovirimat recipients. Methods:In this cohort study, we undertook retrospective review of electronic clinical records and pathology data for all individuals admitted between May 6, and Aug 3, 2022, to 16 hospitals from the Specialist and High Consequence Infectious Diseases Network for Monkeypox. The hospitals were located in ten cities in England and Northern Ireland. Inclusion criteria were clinical signs consistent with mpox and MPXV DNA detected from at least one clinical sample by PCR testing. Patients admitted solely for isolation purposes were excluded from the study. Key outcomes included admission indication, complications (including pain, secondary infection, and mortality) and use of antibiotic and anti-viral treatments. Routine biochemistry, haematology, microbiology, and virology data were also collected. Outcomes were assessed in all patients with available data. Findings:156 individuals were admitted to hospital with complicated mpox during the study period. 153 (98%) were male and three (2%) were female, with a median age of 35 years (IQR 30–44). Gender data were collected from electronic patient records, which encompassed full formal review of clincian notes. The prespecified options for data collection for gender were male, female, trans, non-binary, or unknown. 105 (71%) of 148 participants with available ethnicity data were of White ethnicity and 47 (30%) of 155 were living with HIV with a median CD4 count of 510 cells per mm3(IQR 349–828). Rectal or perianal pain (including proctitis) was the most common indication for hospital admission (44 [28%] of 156). Severe pain was reported in 89 (57%) of 156, and secondary bacterial infection in 82 (58%) of 142 individuals with available data. Median admission duration was 5 days (IQR 2–9). Ten individuals required surgery and two cases of encephalitis were reported. 38 (24%) of the 156 individuals received tecovirimat with early cessation in four cases (two owing to hepatic transaminitis, one to rapid treatment response, and one to patient choice). No deaths occurred during the study period. Interpretation:Although life-threatening mpox appears rare in hospitalised populations during the current outbreak, severe mpox and associated complications can occur in immunocompetent individuals. Analgesia and management of superimposed bacterial infection are priorities for patients admitted to hospital.

Central nervous system cancers (CNS cancers) impose a significant burden upon healthcare systems worldwide. Currently, the lack of a comprehensive study to assess various epidemiological indexes of CNS cancers on national and subnational scales in Iran can hamper healthcare planning and resource allocation in this regard. This study aims to fill this gap by providing estimates of CNS cancer epidemiological measures on national and subnational levels in Iran from 1990 to 2019.This study is a part of Global Burden of Disease (GBD) 2019 that contains epidemiological measures including prevalence, incidence, mortality, Disability-Adjusted Life Years (DALYs), Years Lived with Disability (YLDs), and Years of Life Lost (YLLs) of CNS cancers. Age standardization was utilized for comparing different provinces.In 2019, 5811 (95% Uncertainty Interval: 2942-7046) national new cases and 3494 (1751-4173) deaths due to CNS cancers were reported. National age-standardized incidence (ASIR), deaths (ASDR), and DALYs rates were 7.3 (3.7-8.8), 4.6 (2.3-5.5), and 156.4 (82.0-187.0) per 100,000 in 2019, respectively. Subnational results revealed that ASDR and ASIR have increased in the past 30 years in all provinces. Although incidence rates have increased in all age groups and genders since 1990, death rates have remained the same for most age groups and genders except for young patients aged under 15, where a decrease in mortality and YLLs can be observed.The incidence, deaths, and DALYs of CNS cancers increased at national and subnational levels. These findings should be considered for planning and resource allocation.

Hypertension is one of the major modifiable risk factors in developing cardiovascular diseases (CVD). Hence, we aimed to ascertain age- and sex-specific population attributable fraction (PAF) for CVD in different blood pressure levels to implement efficient preventive strategies at the population level.Participants' data were obtained from the Iranian stepwise approach for surveillance of noncommunicable disease risk factors (STEPs) survey to calculate PAF in four subsequent phases. In phase 0, PAF was measured, irrespective of the diagnosis status. In phase 1, the theoretical minimum range of 115 ≤SBP less than 130 mmHg was considered as the low-risk and measurements equal to or higher than 130 mmHg as the high-risk group. Across phase 2, patients were divided into normal and hypertensive groups based on the American College of Cardiology/American Heart Association guideline. In phase 3, patients were divided into two categories based on treatment coverage.A total number of 27 165 participants aged ≥25 years had valid blood pressure measurements and were enrolled. Phase 0: PAF generally had an upward trend with age advancing. Phase 1: participants with BP ≥130 mmHg comprised the largest PAF, extending from 0.31 (0.25-0.37) in older male individuals to 0.85 (0.79-0.91) in younger females. Phase 2: higher values were found in younger ages for hypertension. Phase 3 represented that attributable fractions among hypertensive patients who received treatment were much lower than drug-naïve hypertensive participants.Our study enlightens the necessity for implementing effective screening strategies for the younger generation and providing adequate access to antihypertensive medications for the low-risk population.

Introduction. The variety of the genetic factors playing role in development of dental caries calls for further research in this regard. The aim of the present study was to investigate the differences between caries-free adults and adults with dental caries in terms of polymorphism of caries-related genes (AMELX and ENAM). Methods. The present case-control study was performed on 81 adults aged 18–24 years, 41 caries free, and 40 with a DMFT ≥ 4. A questionnaire containing background and demographic information (such as age, gender, time and type of latest dental check-ups, parent’s education, oral self-care, and the place of residence in the first 12 years of life) was completed by participants at the time of examination. The blood sample was taken from each participant in the EDTA tube, and PCR was performed. Gene diversity of AMELX and ENAM genes was compared between the two groups. Results. Regarding AMELX gene, in the caries-free group 33 (80.5%) and in the group with DMFT ≥ 4, 33 (82.5%) students had TT genotype, but this difference was insignificant. For ENAM gene, in the caries-free group 34 (82.9%) and in the group with DMFT ≥ 4, 39 (97.5%) students had TT genotype, but this difference was insignificant ( P value = 0.048, CI 95%:0.02–1.27, and OR = 0.145). Conclusion. There was no relationship between TT and TC genotypes of single nucleotide polymorphism of AMELX and ENAM gene and susceptibility to dental caries, but with increasing sample size, there may be a relationship between SNP of ENAM gene and being caries free.

Scene understanding plays a crucial role in autonomous driving by utilizing sensory data for contextual information extraction and decision making. Beyond modeling advances, the enabler for vehicles to become aware of their surroundings is the availability of visual sensory data, which expand the vehicular perception and realizes vehicular contextual awareness in real-world environments. Research directions for scene understanding pursued by related studies include person/vehicle detection and segmentation, their transition analysis, lane change, and turns detection, among many others Unfortunately, these tasks seem insufficient to completely develop fully-autonomous vehicles i.e. achieving level-5 autonomy, travelling just like human-controlled cars. This latter statement is among the conclusions drawn from this review paper: scene understanding for autonomous driving cars using vision sensors still requires significant improvements. With this motivation, this survey defines, analyzes, and reviews the current achievements of the scene understanding research area that mostly rely on computationally complex deep learning models. Furthermore, it covers the generic scene understanding pipeline, investigates the performance reported by the state-of-the-art, informs about the time complexity analysis of avant garde modeling choices, and highlights major triumphs and noted limitations encountered by current research efforts. The survey also includes a comprehensive discussion on the available datasets, and the challenges that, even if lately confronted by researchers, still remain open to date. Finally, our work outlines future research directions to welcome researchers and practitioners to this exciting domain. The main challenges of compressed natural gas (CNG) engine fuelling in terms of methane abatement in the aftertreatment system are addressed in this study using differently loaded platinum group metal (pgm) catalysts. A dual-fuel injection strategy of methane-gasoline was implemented where methane gas was port-injected into the intake in stoichiometric conditions at levels corresponding to 20% and 40% energy density replacement of gasoline fuel. High, medium and low loaded palladium-rhodium catalysts were used and compared to study the effect of pgm loading on the catalyst light-off activity for methane. Results indicate that increasing the palladium loading led to significantly earlier light-off temperatures achieved at relatively lower temperatures of 340°C, 350°C and 395°C respectively. However, the benefit diminishes above palladium loading >142.5 g ft‐3. The study has also demonstrated that ammonia is formed over the CNG catalyst due to steam-reforming reactions from the increased levels of methane in the exhaust with dual-fuelling. Hence aftertreatment technologies such as selective catalytic reduction (SCR) should be adopted to remove them. This further highlights the need to regulate the harmful ammonia emissions from future passenger cars fuelled with CNG. In addition, the benefits of the dual-fuel system in terms of lower engine output carbon dioxide, non-methane hydrocarbon (NMHC) and particulate matter (PM) emissions compared to the gasoline direct injection (GDI) mode alone are presented.

Background. Primary ciliary dyskinesia (PCD), also known as the immotile-cilia syndrome, is a clinically and genetically heterogeneous syndrome. Improper function of the cilia causes impaired mucociliary clearance. Neonatal respiratory distress, rhinosinusitis, recurrent chest infections, wet cough, and otitis media are respiratory presentations of this disease. It could also manifest as infertility in males as well as laterality defects in both sexes, such as situs abnormalities (Kartagener syndrome). During the past decade, numerous pathogenic variants in 40 genes have been identified as the causatives of primary ciliary dyskinesia. DNAH11 (dynein axonemal heavy chain 11) is a gene that is responsible for the production of cilia’s protein and encodes the outer dynein arm. Dynein heavy chains are motor proteins of the outer dynein arms and play an essential role in ciliary motility. Case Presentation. A 3-year-old boy, the offspring of consanguineous parents, was referred to the pediatric clinical immunology outpatient department with a history of recurrent respiratory tract infections and periodic fever. Furthermore, on medical examination, situs inversus was recognized. His lab results revealed elevated levels of erythrocyte sedimentation rate (ESR) and C reactive protein (CRP). Serum IgG, IgM, and IgA levels were normal, while IgE levels were elevated. Whole exome sequencing (WES) was performed for the patient. WES demonstrated a novel homozygous nonsense variant in DNAH11 (c.5247G > A; p. Trp1749Ter). Conclusion. We reported a novel homozygous nonsense variant in DNAH11 in a 3-year-old boy with primary ciliary dyskinesia. Biallelic pathogenic variants in one of the many coding genes involved in the process of ciliogenesis lead to PCD.

41. Solar-powered Parking Analytics System Using Deep Reinforcement Learning

Advances in deep vision techniques and the ubiquity of smart cameras will drive the next generation of video analytics. However, video analytics applications consume vast amounts of energy as deep learning techniques are power-hungry. In this article, we focus on a parking video analytics platform and propose RL-CamSleep, a deep reinforcement learning-based technique, to actuate the cameras to reduce the energy footprint while retaining the system’s utility. Our key insight is that many video-analytics applications do not need to be always operational, and we can design policies to activate video analytics only when necessary. We design two modes of operation for the reinforcement learning (RL) controller: (i) cloud-based mode and (ii) grid-isolated solar-powered mode. In the cloud-based mode, the controller runs on the cloud to control the cameras, whereas, in the solar-powered mode, the RL controller is constrained by the energy produced by solar. We evaluate our approach on a city-scale parking dataset having 76 streets spread across a city. Our analysis shows RL-CamSleep can learn an adaptive policy that reduces the average energy consumption by 76% and achieves an average accuracy of 98%. For the grid-isolated mode, RL-CamSleep outperforms other baseline techniques demonstrating the need for adaptive policy in energy-constrained environments.

OBJECTIVE To determine the prevalence, progression, and modifiable risk factors associated with the development of diabetic retinopathy (DR) in a population-based cohort of youth-onset diabetes. RESEARCH DESIGN AND METHODS We conducted a multicenter, population-based prospective cohort study (2002–2019) of youth and young adults with youth-onset type 1 diabetes (n = 2,519) and type 2 diabetes (n = 447). Modifiable factors included baseline and change from baseline to follow-up in BMI z score, waist/height ratio, systolic and diastolic blood pressure z score, and A1C. DR included evidence of mild or moderate nonproliferative DR or proliferative retinopathy. Prevalence estimates were standardized to estimate the burden of DR, and inverse probability weighting for censoring was applied for estimating risk factors for DR at two points of follow-up. RESULTS DR in youth-onset type 1 and type 2 diabetes is highly prevalent, with 52% of those with type 1 diabetes and 56% of those with type 2 diabetes demonstrating retinal changes at follow-up (mean [SD] 12.5 [2.2] years from diagnosis). Higher baseline A1C, increase in A1C across follow-up, and increase in diastolic and systolic blood pressure were associated with the observation of DR at follow-up for both diabetes types. Increase in A1C across follow-up was associated with retinopathy progression. BMI z score and waist/height ratio were inconsistently associated, with both positive and inverse associations noted. CONCLUSIONS Extrapolated to all youth-onset diabetes in the U.S., we estimate 110,051 cases of DR developing within ∼12 years postdiagnosis. Tight glucose and blood pressure management may offer the opportunity to mitigate development and progression of DR in youth-onset diabetes.

The entry of heavy metals due to industrial activities into the environment is one of the major problems in this century. Nickel and chromium(VI) are the toxin elements that are used in various industries. In this study, lignocellulose nanofiber was purchased from the Nano-Novin Polymer Company (Gorgan, Iran) and used as an adsorbent for the removal of nickel and chromium(VI) ions from aqueous solutions in a batch system. The effects of pH, initial concentration, adsorbent dose, contact time, and temperature were investigated. The adsorption mechanism was determined by Langmuir, Freundlich, kinetic, and thermodynamic models. Investigation of equilibrium isotherms nickel and chromium(VI) showed that the isotherm fitted well with the Freundlich model. The pseudo-second-order model with the larger correlation coefficient had a greater fitness against experimental data in the kinetic studies. Thermodynamic parameters of both nickel and chromium such as Gibbs free energy, enthalpy, and entropy were calculated which indicated spontaneous, endothermic, and random processes, respectively. Lignocellulose nanofiber can be suggested as a good adsorbent that is highly capable of adsorbing nickel and chromium(VI) from aqueous solutions.
