Targeted therapies are validated to be efficient in non-small cell lung cancer (NSCLC) patients with driver gene mutations, but the emergence and development of immune checkpoint inhibitors (ICIs) in the last decades offers new insight into the therapeutic decisions of patients harboring driver gene mutations. The appropriate application of ICIs-based strategies in these patients remains to be assessed. Here, we present a patient with chest tightness and nonproductive cough. A primary site was found in left lower lobe with pleural effusion, pericardium effusion and multiple enlarged lymph nodes. He was diagnosed with advanced NSCLC and responded well to the first line pembrolizumab combined with pemetrexed and carboplatin with a progression free survival of 26 months. Immunohistochemistry analysis of relapsed 4R lymph node indicated ALK-positive. Surprisingly, identical form of ALK gene rearrangement was observed in re-biopsy sample and the specimen 2 years ago. The patient was again dramatically benefit from 2L alectinib. No adverse event was presented and the progression-free survival is not reached until this case report. Hence, the current case suggested that combined immunotherapy and chemotherapy might be an efficient and tolerable alternative at 1L or after resistance to standard of care in patients with oncogene mutant NSCLC. Further data is required to make this a practical ambition.

We analyzed data from the China Pulmonary Health (CPH) study to assess the prevalence of anxiety and depression in COPD patients and their relationship with disease severity.Patients and Methods: A total of 57,779 subjects aged 20 years or older were recruited in the CPH study. All participants were assessed using a standard questionnaire and underwent pulmonary function tests before and after the use of a bronchodilator in local health centers. The Hospital Anxiety and Depression Scale (HADS) questionnaire with a cutoff score of 8 was used to define anxiety and depression. The prevalence of anxiety and depression in patients of COPD were investigated. Multivariate logistic regression was used to investigate the effects of COPD and lung function on anxiety and depression.Results: A total of 49,053 participants (20,661 men and 28,392 women) completed the questionnaire with reliable post-bronchodilator pulmonary function test results and were included in the final analysis, of which 4686 (9.55%) were diagnosed with COPD. Of the patients with COPD, 10.79% had anxiety, 13.65% had depression, and 7.08% had anxiety and depression concomitantly. In the multivariate logistic regression analysis, COPD was not significantly associated with anxiety, depression, or both. After adjusting for confounders in model 1, patients in the GOLD III–IV group had a significantly higher risk of anxiety, depression, and their coincidence. After further adjusting for respiratory symptoms in model 2, lung function impairment in the GOLD III–IV group was only significantly associated with a higher risk of depression or at least one of anxiety and depression.Conclusion: Anxiety and depression are prevalent in patients with COPD in China. Background: Although asthmatic patients with chest tightness as their only presenting symptom (chest tightness variant asthma, CTVA) have clinical characteristics of eosinophilic airway inflammation similar to those of classic asthma (CA), the response of CTVA to treatment regimens used for CA is unclear. Objective: The response of 76 CTVA patients to standard asthma treatments with inhaled corticosteroids with long-acting beta-agonists was explored. Methods: The study was a multicenter, prospective, real-life study conducted in 16 centers in China. The primary efficacy endpoint was the changes in 5-point asthma control questionnaire (ACQ-5) score before and after treatment. Results: After 52 weeks of treatment with therapy regimens used for CA, the ACQ-5 scores decreased markedly from 1.38±0.91 (first administration) to 0.74±0.75 (26 weeks), and 0.71±0.78 (52 weeks) (P

The epidemic of COVID-19 has now spread globally and affected over 110 countries. As of Mar 10th, using publicly available data and official news reports in Henan province, we tracked a total of 1272 cases and a retrospective study was conducted to investigate the related factors in COVID-19 spread and control. We confirmed 554 primary patients had travel or residential history of Wuhan in the recent 2 weeks. Secondary cases accounted for 77.9% (141/181) among all the patients aged 61 or older, in whom contacted with unconfirmed returnees from Wuhan was responsible for 27.0% (38/141). The median incubate period is 7 (IQR, 4-10) days by analyzing time information in 469 cases. For 442 patients with discharge dates, the duration from onset to cure is 19 (IQR, 15-23) days. The time from onset to seeking care at a hospital varied in age groups, and differed between primary and secondary cases. Patients visiting different hospitals affected the time from seeking care to cure. Thus, our results showed the spread of COVID-19 and factors associated with outcomes of patients in Henan province, which helps to understand the epidemiological features outside of the epidemic area and control the disease in other regions and countries.
