This essay contends that the red MAGA hat in the modern conservative movement plays off Donald Trump's populist construction of his supporters as victims by creating opportunities for the performance of what we call “rehearsed victimhood.” As an enactment of vulnerability that allows individuals in historically powerful positions to claim marginalized status by manifesting material evidence of their subjugation, we argue that rehearsed victimhood relies on weaponized polysemy to bait critics, the cooptation of civil rights rhetoric, and acts of humiliation and violent self-sacrifice. We illustrate the performance of rehearsed victimhood through a close reading of media coverage of numerous incidents where Trump supporters claimed discrimination after being attacked, fired, or ridiculed for wearing a MAGA hat.

The Chilean water penny genus Tychepsephus Waterhouse, 1876 is revised, with descriptions and photographic illustrations of life stages including two larval morphotypes, the pupa of one morphotype, and adults of two species. The pupa of Tychepsephus has not been reported previously. Tychepsephus cekalovicisp. nov. is described, and Ectopria (Chilectopria) grandis Pic, 1947, syn. nov. is proposed as a new synonym of Tychepsephus felix Waterhouse, 1876, which is redescribed. Taxonomic treatment of the adults of both species includes images of the habitus of males and females, morphological variation, and male and female genitalia. Males and females are sexually dimorphic. Information on the habitat of Tychepsephus is provided and illustrated with photographs, and the known geographic distribution of the two species is mapped. The occurrence of Tychepsephus in Argentina is reported; therefore, the genus no longer can be considered endemic to Chile. The taxonomic status and geographic distribution in South America of other species of Psephenidae, particularly members of the subfamily Eubriinae, is reviewed.

Background. Time to treatment initiation (TTI) is a quality metric in cancer care. The purpose of this study is to determine the accuracy of TTI data from a single cancer center registry that reports to the National Cancer Database (NCDB) for sarcoma diagnoses. Methods. A retrospective analysis of a single Commission on Cancer (CoC)-accredited cancer center’s tumor registry between 2006 and 2016 identified 402 patients who underwent treatment of a musculoskeletal soft tissue sarcoma and had TTI data available. Registry-reported TTI was extracted from the tumor registry. Effective TTI was manually calculated by medical record review as the number of days from the date of tissue diagnosis to initiation of first effective treatment. Effective treatment was defined as oncologic surgical excision or initiation of radiation therapy or chemotherapy. Registry-reported TTI and effective TTI values were compared for concordance in all patients. Results. In the entire cohort, 25% (99/402) of patients had TTI data discordance, all related to surgical treatment definition. For patients with a registry-reported value of TTI = 0 days, 74% (87/118) had a diagnostic surgical procedure coded as their first treatment event, with 73 unplanned incomplete excision procedures and 14 incisional biopsies. In these patients, effective TTI was on average 59 days ( P < 0.001 ). For patients with a registry-reported value of TTI >0 days, only 4% (12/284) had discordant TTI values. Conclusions. Nearly three-fourths of patients with a registry-reported value of TTI = 0 days in a large, CoC-accredited cancer center registry had a diagnostic procedure coded as their first treatment event, though their effective treatment had not yet started. These data suggest that TTI is likely longer than what is reported to the NCDB. Redefinition of what constitutes surgical treatment should be considered to improve the accuracy of data used in measuring TTI in sarcoma.

Climate change is altering species’ range limits and transforming ecosystems. For example, warming temperatures are leading to the range expansion of tropical, cold-sensitive species at the expense of their cold-tolerant counterparts. In some temperate and subtropical coastal wetlands, warming winters are enabling mangrove forest encroachment into salt marsh, which is a major regime shift that has significant ecological and societal ramifications. Here, we synthesized existing data and expert knowledge to assess the distribution of mangroves near rapidly changing range limits in the southeastern USA. We used expert elicitation to identify data limitations and highlight knowledge gaps for advancing understanding of past, current, and future range dynamics. Mangroves near poleward range limits are often shorter, wider, and more shrublike compared to their tropical counterparts that grow as tall forests in freeze-free, resource-rich environments. The northern range limits of mangroves in the southeastern USA are particularly dynamic and climate sensitive due to abundance of suitable coastal wetland habitat and the exposure of mangroves to winter temperature extremes that are much colder than comparable range limits on other continents. Thus, there is need for methodological refinements and improved spatiotemporal data regarding changes in mangrove structure and abundance near northern range limits in the southeastern USA. Advancing understanding of rapidly changing range limits is critical for foundation plant species such as mangroves, as it provides a basis for anticipating and preparing for the cascading effects of climate-induced species redistribution on ecosystems and the human communities that depend on their ecosystem services.

The evidence to support the benefits of exercise for people living with and beyond cancer is robust. Still, exercise oncology interventions in the United States are only eligible for coverage by third-party payers within the restrictions of cancer rehabilitation settings. Without expanded coverage, access will remain highly inequitable, tending toward the most well-resourced. This article describes the pathway to third-party coverage for 3 programs that address a chronic disease and utilize exercise professionals: the Diabetes Prevention Program, Supervised Exercise Training for Peripheral Artery Disease, and Cancer Rehabilitation. Lessons learned will be applied toward expanding third-party coverage for exercise oncology programming.

Mosquito host-feeding behavior is an important parameter for determining the vector potential of mosquito species in a given locale. Despite the recent discovery of Uranotaenia sapphirina Osten Sacken feeding on annelid hosts in Florida, host association studies for this mosquito species in the United States remain limited. To investigate the blood-feeding pattern of Ur. sapphirina in the northeastern United States, mosquitoes were collected from Massachusetts, Connecticut, and New Jersey using CDC miniature light traps, peat fiber resting boxes, gravid traps, and backpack aspirators. Vertebrate and invertebrate hosts of this mosquito species were identified through PCR amplification and nucleotide sequencing of portions of the mitochondrial cytochrome b gene and the 28S ribosomal RNA gene, respectively. Of 21 (24.7%) specimens successfully identified to host species, 47.6% contained solely annelid blood, 14.3% mammalian blood, 14.3% avian blood, and 23.8% with mixed blood of annelid and avian origin. The mud earthworm, Sparganophilus tennesseensis Reynolds (Haplotaxida: Sparganophilidae), was identified as the most common host (n = 14, including mixed bloods), followed by American robin, Turdus migratorius (n = 7, including mixed bloods). Testing of these blood engorged mosquitoes for West Nile virus and eastern equine encephalitis virus did not result in any positive specimens. This is the first report of Ur. sapphirina feeding on annelids and on both vertebrate and invertebrate hosts in mixed bloodmeals in the northeastern United States. Our findings support the recent report of Ur. sapphirina feeding on invertebrates and further emphasizes the inclination of some mosquito species to feed on a wider range of hosts spanning nontraditional taxonomic groups.

Commercial phosphorus pentoxide reacts with some N-donor bases to give the adducts P(2)O(5)L(2) and P(4)O(10)L(3) (L = DABCO, pyridine, 4-tert-butylpyridine). The DABCO adducts were structurally characterized by single-crystal X-ray diffraction. It is proposed that P(2)O(5)L(2) and P(4)O(10)L(3) undergo interconversion through a “phosphate-walk” mechanism, which was evaluated using DFT calculations. P(2)O(5)(pyridine)(2) (1) efficiently transfers monomeric diphosphorus pentoxide to phosphorus oxyanion nucleophiles, yielding substituted trimetaphosphates and cyclo-phosphonate-diphosphates (P(3)O(8)R)(2−) (R(1) = nucleosidyl, phosphoryl, alkyl, aryl, vinyl, alkynyl, H, F). Hydrolytic ring-opening of these compounds forms linear derivatives [R(1)(PO(3))(2)PO(3)H](3−) and nucleophilic ring-opening gives linear disubstituted [R(1)(PO(3))(2)PO(2)R(2)](3−) compounds.

Insurance markets often feature consumer sorting along both an extensive margin (whether to buy) and an intensive margin (which plan to buy). We present a new graphical theoretical framework that extends a workhorse model to incorporate both selection margins simultaneously. A key insight from our framework is that policies aimed at addressing one margin of selection often involve an economically meaningful trade-off on the other margin in terms of prices, enrollment, and welfare. Using data from Massachusetts, we illustrate these trade-offs in an empirical sufficient statistics approach that is tightly linked to the graphical framework we develop.

Purpose There is a paucity in the literature regarding the characteristics and attitudes of social media (SM) utilization in a professional manner by neurosurgical oncologists. Methods A 34-question electronic survey was created using Google Forms and disseminated via email to members of the AANS/CNS Joint Section on Tumors. Demographic data were compared amongst those who utilize social media versus those who do not. Factors associated with positive outcomes of professional SM use and with having more followers on SM were analyzed. Results The survey received 94 responses, of which 64.9% reported that they currently use SM in a professional manner. Age

Context In recent years, patient-centered healthcare has become a primary concern for researchers and healthcare professionals. When included in randomized controlled trials (RCTs), patient-reported outcome (PRO) measures serve a critical role in supplementing efficacy outcomes with a patient perspective. Objectives The goals of this study are to evaluate the reporting completeness of PROs within literature concerning carpal tunnel syndrome (CTS) utilizing the Consolidated Standards of Reporting Trials Patient-Reported Outcomes (CONSORT-PRO) extension. Methods We searched MEDLINE, Embase, and Cochrane Central Register of Controlled Trials (CENTRAL) for published RCTs relating to CTS with at least one PRO measure from 2006 to 2020. Two investigators screened all RCTs for inclusion utilizing Rayyan (https://rayyan.qcri.org/), a systematic review screening platform. In an independent, masked fashion, investigators then evaluated all RCTs utilizing the CONSORT-PRO adaptation and Cochrane Collaboration Risk of Bias (RoB) 2.0 tool. Bivariate regression analyses were utilized to assess relationships between trial characteristics and completeness of reporting. Results Our search returned 374 publications, yet only 31 unique RCTs met the inclusion criteria. The mean overall percent of adherence for CONSORT-PRO was 41%. Our secondary outcome—assessing study characteristics—indicated significantly higher completeness of reporting in the absence of a conflict of interest statement (p Conclusions Our study indicated that the completeness of CONSORT-PRO reporting was deficient within CTS trials. Because of the importance placed on PROs in clinical practice, we recommend adherence to CONSORT-PRO prior to publication of RCTs to increase the understanding of various interventions on patients’ quality of life (QoL).

The study investigated mobile banking and commercial banks’ performance nexus in Zimbabwe for the period 2011-2021. The investigation employed a pragmatic approach and a descriptive design. Primary data as well as secondary data were used to achieve the study objectives. Questionnaires, interviews and documentary guides were used to collect primary and secondary data respectively. There were 108 questionnaires distributed to staff members and customers of 6 sampled commercial banks in Zimbabwe having mobile banking experience. The interview guide was administered to bank managers to get a deeper understanding of the topic under research and as a mitigation strategy for addressing the flaws of the questionnaires. Analysis of quantitative data utilised descriptive analysis tools, correlations analysis tools as well as regression analysis tools whereas theme analysis was used for qualitative data. SPSS version 26 was used for this purpose. The study implications established that mobile banking and commercial banks’ performance nexus in Zimbabwe was significant and also enhanced mobile banking security capabilities and strengthened commercial banks’ performance. The investigation concluded that mobile banking’s success in enhancing commercial banks’ performance hinged on its ability to allow the user to get easier, cheaper and timely access to services. However, the study results showed that the cost of mobile banking services, government policies and intense competition inhibited the uptake of banking services. Commercial banks are recommended to continue strengthening the security of mobile banking services and to boost their uptake by bank customers in Zimbabwe.

Information characterising site fidelity and abundance for common bottlenose dolphins (Tursiops truncatus) along the southwest coast of Florida is important for defining stock structure for management purposes. Long-term site fidelity and ranging patterns of bottlenose dolphins in Charlotte Harbor and Pine Island Sound, Florida were investigated using photo-ID data collected during 566 boat-based surveys from 1982 through 2007. Seasonal abundance estimates were generated from seven multi-week field seasons during 2001 through 2006, before and after a major hurricane and red tide event occurred in the area. In total, 1,154 distinctive dolphins were identified up to 34 times each with 84% of individuals resighted on more than one day. Multiple year residency rates were high with 81% of dolphins sighted in at least two years and 30% over ten or more years. Seventy-six percent of individuals with sightings on two or more days were observed in both summer and winter. Of 249 dolphins sighted on ten or more days in the study area, 83% were never observed outside the study area, indicating strong site-fidelity. Two years after a devastating Category 4 hurricane in 2004 and following two years of Karenia brevis harmful algal blooms, 94% of dolphins were observed in the same region within the study area and abundance estimates remained stable. Documenting range and site fidelity patterns of individuals over long periods of time is helpful for characterizing population structure and for examining changes attributable to environmental factors and perturbations such as hurricanes, harmful algal blooms and climate change.

Leaf miners, Liriomyza spp., have developed as a severe pest in recent years, reducing the production of several ornamental and crops in Indonesia. As a result, monitoring and control mechanisms have been proposed. This research aimed to investigate community structure and identify Liriomyza spp., which impacts crops and natural parasitoids in Bali Province. Leaf samples of vegetables, ornamental plants, and related species were collected in all regencies/cities of Bali Province in 2019 and 2020 and preserved in the laboratory for observation and counting of leaf miners and related parasitoids. The findings revealed that four Liriomyza spp. were detected in host plants, namely Liriomyza sativae Blanchard (Diptera: Agromyzidae), Liriomyza huidobrensis Blanchard (Diptera: Agromyzidae), Liriomyza chinensis Kato (Diptera: Agromyzidae), and Liriomyza trifolii Burgess (Diptera: Agromyzidae). Asteraceae species were the most common host plants attacked by leaf miners in the field. The same occurrence was discovered at both heights (high and lowlands). The dry season is the most crucial component in the abundance of Liriomyza spp. In addition, parasitoids of the type Opius cromatomiyae Belokobylskij and Wharton (Hymenoptera: Braconidae) and Hemiptarsenun varicornis Girault (Hymenoptera: Eulophidae) were identified in abundance in Asteraceae plants. Similarly, the parasitization rate of the two parasitoids was found to be high in plants of the Asteraceae and Brassicae families. This approach highlights the decision-making process for controlling Liriomyza spp. on diverse host plant families by developing ecologically beneficial and sustainable parasitoids.

Bacterial response to two-dimensional nanomaterials is dependent on the type and concentration of the material and the growth stage of the bacteria.

With the prevalence of carbon fiber reinforced polymers (CFRPs) in aerospace platforms, there is a need to better understand radiative heat transport through the material. A laboratory experiment was constructed and computational Monte Carlo (MC) model optimized to quantify and understand the laser-scattering properties of CFRPs. The transmission data were analyzed by a zonal MC (ZMC) method to determine the material’s albedo and extinction coefficient, which can then be used for platform-level aerospace models to predict heat transfer more accurately through CFRP structures. This paper considers the effects of nodal, substrate, and detector plane sizing, as well as laser beam parameters, on matching albedo and extinction coefficient predictions from the ZMC method to experimental test data. Average albedo values for IM7/977-3 CFRP using the anchored ZMC method are 0.78 and 0.81 with one-ply and two-ply samples, respectively, having standard deviations of 0.11 and 0.09. Extinction coefficient predictions are 109.4 and [Formula: see text] with standard deviations of 28.3 and [Formula: see text] for one-ply and two-ply samples.

We have developed and used high-time-resolution, single-molecule field-effect transistors (smFETs) to characterize the conformational free-energy landscape of RNA stem-loops. Stem-loops are some of the most common RNA structural motifs and serve as building blocks for the formation of more complex RNA structures. Given their prevalence and integral role in RNA folding, the kinetics of stem-loop (un)folding has been extensively characterized using both experimental and computational approaches. Interestingly, these studies have reported vastly disparate timescales of (un)folding, which has been recently interpreted as evidence that (un)folding of even simple stem-loops occurs on a highly rugged conformational energy landscape. Because smFETs do not rely on fluorophore reporters of conformation or on the application of mechanical (un)folding forces, they provide a unique and complementary approach that has allowed us to directly monitor tens of thousands of (un)folding events of individual stem-loops at a 200 μs time resolution. Our results show that under our experimental conditions, stem-loops fold and unfold over a 1-200 ms timescale during which they transition between ensembles of unfolded and folded conformations, the latter of which is composed of at least two sub-populations. The 1-200 ms timescale of (un)folding we observe here indicates that smFETs report on complete (un)folding trajectories in which unfolded conformations of the RNA spend long periods of time wandering the free-energy landscape before sampling one of several misfolded conformations or, alternatively, the natively folded conformation. Our findings demonstrate how the combination of single-molecule sensitivity and high time resolution makes smFETs unique and powerful tools for characterizing the conformational free-energy landscape of RNA and highlight the extremely rugged landscape on which even the simplest RNA structural elements fold.

Access to palliative care clinicians is a limited resource. Expanding the reach of existing palliative care expertise by utilizing virtual care is one strategy to reach areas that lack access. We delivered virtual services through a centralized hub across multiple health settings and tracked outcomes.Through a centralized virtual palliative care hub based in an urban academic health center, access to specialty palliative care was offered across homes, critical access hospitals (CAHs), and extended care facilities (ECFs) in the state of Indiana. Webpage-based platforms were used, and hardware included a cart on wheels for rural hospital sites. Data specific to palliative care were monitored for each encounter.Over one year, 372 patients were seen for virtual palliative care consultations, of whom 275 (73.9%) were seen in the outpatient setting (where the patient was at home or in an ECF) and 97 (26.1%) were inpatient visits performed in CAHs. Visits occurred with patients from almost all counties in Indiana. Advance directives were established for 286 (76.9%) patients seen, and 107 (28.8%) patients were referred to hospice.Specialty palliative care is a limited resource that has been further constrained by the COVID-19 pandemic. Our experience demonstrates that centralized virtual hub-based palliative care can be leveraged to provide effective, patient-centered, and compassionate care in regions without a specialist and has the potential to improve access to specialty palliative care.

Renal blood flow represents >20% of total cardiac output and with this comes the great responsibility of maintaining homeostasis through the intricate regulation of solute handling. Through the processes of filtration, reabsorption, and secretion, the kidneys ensure that solutes and other small molecules are either returned to circulation, catabolized within renal epithelial cells, or excreted through the process of urination. Although this occurs throughout the renal nephron, one segment is tasked with the bulk of solute reabsorption—the proximal tubule. Among others, the renal proximal tubule is entirely responsible for the reabsorption of glucose, a critical source of energy that fuels the body. In addition, it is the only other site of gluconeogenesis outside of the liver. When these processes go awry, pathophysiological conditions such as diabetes and acidosis result. In this review, we highlight the recent advances made in understanding these processes that occur within the renal proximal tubule. We focus on the physiological mechanisms at play regarding glucose reabsorption and glucose metabolism, emphasize the conditions that occur under diseased states, and explore the emerging class of therapeutics that are responsible for restoring homeostasis.

This study explored how discovering misattributed paternity in adulthood after commercial DNA testing impacts psychological well-being and identity formation. Thirty-three subjects participated in this phenomenological qualitative study. Eighteen had experienced misattributed paternity, while the other 15 subjects discovered misattributed paternity due to being donor conceived. Three fundamental themes emerged in both the misattributed group and donor-conceived group that could be perceived as the core themes are: (a) sadness, grief, and loss; (b) seeking connection and belonging; and (c) betrayal and anger. Other significant themes revealed by both groups included: Otherness, Curiosity, Relief and Comfort, Surprise, Acceptance, and Empathy and Rationalization. Additional themes also revealed by the donor-conceived group, but not revealed in the misattributed group are: (a) existential concerns, (b) self-assuredness, and (c) right to know and advocacy. Overall, findings in this study reveal unique experiences between both groups, suggesting the circumstances around conception and discovery vary depending on misattributed status.

Highlights Abstract Femoral central venous catheter (CVC) placement is a common procedure performed in the pediatric intensive care unit. Femoral CVCs have a relatively low intraprocedural complication rate compared with other access sites. We describe a case of guidewire-induced transient complete heart block during femoral CVC insertion. This complication is rare in the pediatric population. The incidence of its occurrence and guidance to prevent this complication are discussed.

Rhithrops capensis gen. et sp. nov. is described from the Matjies River, in the Cedeberg range of the Western Cape Province, South Africa. The new genus is apparently related to the South African endemic genera Rapnus Grouvelle, 1899 and Strina Redtenbacher, 1867, but exhibits a number of distinct character states, including gibbosities outside the sublateral pronotal furrows and a transversely upturned labral apex, unique within the family. Rhithrops gen. nov. was collected from around the roots of aquatic plants in fast flowing deep water runs, as well as being sampled in nocturnal drift nets. The new species has relatively long legs and well-developed tarsal claws, and appears to be adapted to an entirely aquatic, benthic existence. A revised key is provided to the dryopid genera of South Africa.

Child Advocacy Centers (CACs) are well-positioned to identify children with mental health needs and facilitate access to evidence-based treatment. However, use of evidence-based screening tools and referral protocols varies across CACs. Understanding barriers and facilitators can inform efforts to implement mental health screening and referral protocols in CACs. We describe statewide efforts implementing a standardized screening and referral protocol, the Care Process Model for Pediatric Traumatic Stress (CPM-PTS), in CACs. Twenty-three CACs were invited to implement the CPM-PTS. We used mixed methods to evaluate the first two years of implementation. We quantitatively assessed adoption, reach, and acceptability; qualitatively assessed facilitators and barriers; and integrated quantitative and qualitative data to understand implementation of mental health screening in CACs. Eighteen CACs adopted the CPM-PTS. Across CACs, screening rates ranged from 10% to 100%. Caregiver ratings indicated high acceptability. Facilitators and barriers were identified within domains of the Consolidated Framework for Implementation Research. Qualitative findings provided insight into adoption, reach, and caregivers' responses. Our findings suggest screening for traumatic stress and suicidality in CACs is valued, acceptable, and feasible. Implementation of mental health screening and referral protocols in CACs may improve identification of children with mental health needs and support treatment engagement.

Beatriz Elena Arias López, Berit Bliesemann de Guevara, and Berena Torres Marín reflect on textile initiatives undertaken by peace signatories of the former guerrilla group FARC. Mathilda Shepard analyzes the work of the Tejedoras de Mampuján, asking what it means to speak of justice, reparations, and reconciliation in the wake of the plantation. Danielle House reflects on Mexican embroidery. She considers violence in Mexico and the way the victims it consumes have been framed as “ungrievable.” Mónica Salazar discusses symbolic resistance in participatory needlework projects about the US-Mexico border. Dillon writes about Chilean arpilleras and Colombian testimonial textiles. She draws parallels among the different artistic practices discussed in the collected essays, tracing the emergence of a supranational artistic movement that generates meaning through a multiplicity of practices. The Dialogues demonstrate the new signifying processes that these needlework groups use, as well as the way they articulate meanings that extend beyond the stitches and semiotics of their clothwork.

Multiple sclerosis (MS) is an autoimmune disease driven by lymphocyte activation against myelin autoantigens in the central nervous system leading to demyelination and neurodegeneration. The deoxyribonucleoside salvage pathway with the rate-limiting enzyme deoxycytidine kinase (dCK) captures extracellular deoxyribonucleosides for use in intracellular deoxyribonucleotide metabolism. Previous studies have shown that deoxyribonucleoside salvage activity is enriched in lymphocytes and required for early lymphocyte development. However, specific roles for the deoxyribonucleoside salvage pathway and dCK in autoimmune diseases such as MS are unknown. Here we demonstrate that dCK activity is necessary for the development of clinical symptoms in the MOG

Raynaud’s phenomenon of the tongue after radiation therapy with or without chemotherapy is an exceedingly rare complication. Symptoms are similar to Raynaud’s disease of other sites and involve pallor and discomfort on exposure to cold temperatures that resolve with rewarming. Presentation occurs approximately 18-24 months after radiotherapy on average and can usually be managed effectively with lifestyle modification and pharmacotherapy. Here, we present a case of lingual Raynaud’s following surgery and adjuvant radiation therapy in a patient with squamous cell carcinoma of the oral cavity.
