2. ACTINIC CHEILITIS: EPIDEMIOLOGICAL STUDY AMONG RURAL WORKERS

- Abstract
To identify the epidemiological aspects of AK in rural workers, 60 patients, residents of rural areas, over 18 years of age, who perform or have performed work activities with exposure to solar radiation participated in this research. Aspects such as sex, sun exposure, literacy level (divided into literate and non-literate), use of lip sunscreen, smoking, alcohol consumption and clinical characteristics were evaluated. Sixty individuals were examined between October and December 2021, 45 (75.0%) were male and 15 (25.0%) were female. As for sun exposure, 38 (63.3%) were exposed to the sun for an average of 8 hours a day, and only 17 individuals (28.3%) used lip balm. The remaining 71.6% do not use and have never used lip balm. Regarding smoking and alcohol consumption, 22 (36.7%) used cigarettes daily, and 14 (23.3%) used alcohol frequently. The literacy level of the individuals examined is high, where most individuals (80.0%) have completed elementary school. These data reveal a relationship between the accumulated working time outdoors and the presence of the pathology, highlighting the need to raise awareness of the use of sun protection and awareness campaigns for the Prevention of Oral Cancer.

Objectives: Martini et al. developed a nomogram to predict significant (>25%) renal function loss after robot-assisted partial nephrectomy and identified four risk categories. We aimed to externally validate Martini’s nomogram on a large, national, multi-institutional data set including open, laparoscopic, and robot-assisted partial nephrectomy. Methods: Data of 2584 patients treated with partial nephrectomy for renal masses at 26 urological Italian centers (RECORD2 project) were collected. Renal function was assessed at baseline, on third postoperative day, and then at 6, 12, 24, and 48 months postoperatively. Multivariable models accounting for variables included in the Martini’s nomogram were applied to each approach predicting renal function loss at all the specific timeframes. Results: Multivariable models showed high area under the curve for robot-assisted partial nephrectomy at 6- and 12-month (87.3% and 83.6%) and for laparoscopic partial nephrectomy (83.2% and 75.4%), whereas area under the curves were lower in open partial nephrectomy (78.4% and 75.2%). The predictive ability of the model decreased in all the surgical approaches at 48 months from surgery. Each Martini risk group showed an increasing percentage of patients developing a significant renal function reduction in the open, laparoscopic and robot-assisted partial nephrectomy group, as well as an increased probability to develop a significant estimated glomerular filtration rate reduction in the considered time cutoffs, although the predictive ability of the classes was

Introdução: Os odontomas são considerados um tipo de tumor odontogênico hamartoma (anomalia de desenvolvimento), com etiologia ainda incerta, sendo assintomático e descoberto por exames de rotinas, é um dos tumores mais encontrados na cavidade bucal. Objetivo: O objetivo do presente trabalho é apresentar um relato de caso de um odontoma composto erupcionado em mandíbula, analisando o seu diagnóstico e tratamento. Metodologia: Neste artigo faz-se uma revisão da literatura sobre ele, seguido de um caso clínico relatado sobre odontoma composto na região anterior da mandíbula. Relato de Caso: Paciente do sexo masculino, com 22 anos de idade, compareceu ao Centro de Especialidades Odontológicas (CEO) de Fernandópolis/SP encaminhado pelo ortodontista. Ao exame extrabucal nada digno de nota foi verificado. Ao exame intrabucal verificou-se múltiplas massas esbranquiçadas na região lingual dos dentes 33, 34 e 35, sem sintomatologia dolorosa. O exame radiográfico panorâmico trazido pelo paciente mostrou a presença de um conjunto de estruturas semelhantes a dentes, de forma e tamanho variáveis, envoltos por uma zona radiolúcida, sugerindo o diagnóstico de odontoma composto. Foi solicitada tomografia computadorizada para auxiliar no planejamento da intervenção cirúrgica. O material coletado durante a cirurgia foi encaminhado para análise anatomopatológica e o diagnóstico definitivo foi de odontoma composto. Conclusão: Conclui-se que o diagnóstico precoce de Odontoma Composto possibilita maiores chances de intervenções cirúrgicas bem-sucedidas, evitando grandes complicações, e permitindo assim um processo de cicatrização adequado, poupando estruturas nobres e uma completa reabilitação da saúde bucal do paciente.

Bladder cancer (BCa) is one of the most diagnosed urological malignancies. A timely and accurate diagnosis is crucial at the first assessment as well as at the follow up after curative treatments. Moreover, in the era of precision medicine, proper molecular characterization and pathological evaluation are key drivers of a patient-tailored management. However, currently available diagnostic tools still suffer from significant operator-dependent variability. To fill this gap, physicians have shown a constantly increasing interest towards new resources able to enhance diagnostic performances. In this regard, several reports have highlighted how artificial intelligence (AI) can produce promising results in the BCa field. In this narrative review, we aimed to analyze the most recent literature exploring current experiences and future perspectives on the role of AI in the BCa scenario. We summarized the most recently investigated applications of AI in BCa management, focusing on how this technology could impact physicians’ accuracy in three widespread diagnostic areas: cystoscopy, clinical tumor (cT) staging, and pathological diagnosis. Our results showed the wide potential of AI in BCa, although larger prospective and well-designed trials are pending to draw definitive conclusions allowing AI to be routinely applied to everyday clinical practice.

9. PANDEMIA DE COVID-19 E FAMÍLIAS: IMPACTOS DA CRISE E DA RENDA BÁSICA EMERGENCIAL

- Abstract
São dois os objetivos deste trabalho: I) avaliar como cenários de contração econômica e consequente queda no emprego impactam as famílias por classes de renda; e II) projetar o impacto do auxílio emergencial nas famílias e na economia. Para cumprir com esses objetivos, utilizou-se um modelo de equilíbrio geral computável (EGC) para o Brasil com dinâmica recursiva e periodicidade trimestral. Esse modelo é uma adaptação do modelo Brazilian Social Accounting – General Equilibrium Model for Income Generation, Households and Transfers (Bright), apresentado em Cardoso (2016; 2020). O modelo utilizado neste trabalho está especialmente capacitado para questões relacionadas a transferências, tributação, além de geração, apropriação e distribuição da renda na economia brasileira, como em Cardoso (2016; 2020). Além disso, traz modificações, como a adaptação da teoria, e banco de dados em uma abordagem trimestral, como usado em outros artigos que analisam impactos de epidemias (Arndt e Lewis, 2001; Dixon et al., 2010; Geard et al., 2016). Para atender ao primeiro objetivo, projetaram-se os impactos de queda em 0,1% sobre o emprego da economia brasileira sobre a renda disponível das famílias, por onze classes de renda. A queda ad hoc de 0,1% possibilita que se estabeleça um parâmetro em relação à distribuição dos impactos na renda familiar. Em seguida, estimou-se o público-alvo do auxílio emergencial segundo os critérios de elegibilidade do programa, utilizando dados do Cadastro Único para Programas Sociais do Governo Federal (Cadastro Único) e da Pesquisa Nacional por Amostra de Domicílios (PNAD) Contínua, e simulou-se a inserção do auxílio, avaliando seus impactos.

10. PACIENTE COM XEROSTOMIA PÓS-COVID-19 EM TRATAMENTO COM LASERTERAPIA: RELATO DE CASO

- Abstract
A COVID-19 é uma infecção aguda e crônica, tornou-se uma pandemia em março de 2020, decretado pela Organização Mundial de Saúde (OMS), sendo considerada um grande problema para a saúde pública, desde então muitos sintomas associados a COVID-19 vem surgindo, além de problemas sistêmicos. A cavidade oral é uma porta de entrada a patógenos e doenças autoimunes, contudo após a infecção pela COVID-19 surgiram relatos de pacientes que se queixavam de sensação de boca seca, decorrente da redução da produção de saliva, sendo diagnosticados com xerostomia. Esta alteração tem etiologia multifatorial, relacionado à fatores como a presença de doenças autoimunes, o uso de medicamentos, a radioterapia e a quimioterapia, com maior predisposição no sexo feminino. Desta forma, o objetivo do presente trabalho foi relatar o caso clínico de uma paciente que foi diagnosticada no Centro de Especialidades Odontológicas (CEO), no município de Fernandópolis/SP, com xerostomia pós-covid-19. Paciente de 46 anos de idade, sexo feminino, leucoderma, que durante a anamnese relatou o uso de medicamentos psicotrópicos, e ter portado teste positivo para COVID-19, durante o exame físico intraoral, observou-se ressecamento de toda a mucosa bucal, que se apresentava avermelhada e brilhante. Além disso, foi possível verificar a ausência do acúmulo de saliva no assoalho da boca. Considerando o estudo do caso clínico foi realizada uma busca na literatura sobre a relação de pacientes com o sintoma citado e práticas atuais de prevenção e tratamento. O tratamento indicado após pesquisa e diagnóstico de xerostomia foi o uso de laser de baixa potência, associado a troca medicamentosa do antidepressivo. Apesar dos estudos relatando a associação da COVID-19 as manifestações orais vêm aumentando, ainda não é possível uma certeza que relacione a etiologia da doença à infecção por COVID-19, contudo são necessários mais estudos contundentes sobre essa relação. Ressalta-se, no entanto, que o tratamento de laserterapia teve resposta significativa.

Esse artigo buscou compreender a pandemia pelo SARS-CoV-2 na qual, fez com que o cirurgião-dentista possibilitasse algumas mudanças na Odontologia, ocasionando uma alteração expressiva na forma de atendimento e da rotina dos consultórios odontológicos. O objetivo deste trabalho foi realizar uma revisão de literatura sobre as principais manifestações bucais da COVID-19. As pesquisas bibliográficas foram realizadas nas bases de dados Google Scholar, PubMed e SciELO, utilizando as palavras–chaves: manifestações bucais e COVID-19. Os resultados obtidos ao final desse levantamento foi de que muitas manifestações em boca foram diagnosticadas em pacientes com COVID-19, como: xerostomia, lesões vesicobolhosas, petéquias, lesões ulcerativas e eritematosas. No entanto, muitas dúvidas existem com relação a essas lesões no sentido de entender se são manifestações primárias da doença ou manifestações secundárias a infecções oportunidades, por exemplo. Dessa forma, conclui-se que independente da origem das manifestações bucais nos pacientes com COVID-19 o acompanhamento pelo cirurgião-dentista se faz necessário tanto durante quanto após o tratamento da doença, pois ele poderá diagnosticar, tratar e prevenir tais lesões a fim de melhorar a qualidade de vida desses pacientes.

- Abstract
O presente artigo analisa a suficiência das medidas de proteção social adotadas pelo Estado Brasileiro em função da pandemia causada pelo novo Coronavírus (SARS-CoV 2) para garantir o mínimo existencial. É dado enfoque nas medidas adotadas pela Lei nº 13.982, de 02 de abril de 2020, mais precisamente a criação do auxílio emergencial e a possibilidade de antecipação dos benefícios por incapacidade permanente e do benefício de prestação continuada (BPC), dado o fechamento das agências da previdência social e, por isso, a impossibilidade de realização das perícias necessárias para concessão desse benefício previdenciário e desse benefício assistencial, respectivamente.Para análise da suficiência das medidas adotadas, sob a ótica do mínimo existencial, são apresentados os valores que foram pagos ou antecipados em comparação com o salário-mínimo nacional. Com isso, é apresentada a seguinte reflexão. Essas medidas garantiram apenas a sobrevivência ou garantiram uma vida digna. O método utilizado será o dedutivo, realizando pesquisa bibliográfica, partindo da coleta de conceitos doutrinários e informações jurídicas e legais.

14. Direct dotterising or angioplasty of acute stroke due to tandem atherosclerotic occlusions

- Abstract
BackgroundTandem occlusions cause 10–15% of LVO acute ischemic strokes but are difficult to treat endovascularly and frequently excluded from clinical trials. The optimum endovascular method is still debated, however going directly through the carotid occlusion can speed up the procedure and reduce procedural risk by eliminating an exchange maneuver.MethodUsing retrospective data from three centers, we compared treating atherosclerotic tandem occlusions using a 0.035'-guidewire and direct dotterisation or angioplasty with a peripheral vascular balloon suitable for the wire, vs. the usual technique of an 0.014'wire. We compared the successful recanalization (mTICI 2b-3) rates, 90 days' functional outcomes (mRS 0–2), and puncture-to-recanalization times between both procedures.ResultsForty-two consecutive patients with atherosclerotic tandem occlusions were included; 25 were treated with the 0.014'wire technique and 17 with the 0.035'-guidewire and direct dotterisation or angioplasty with a peripheral vascular balloon technique. The direct technique achieved a higher rate of successful recanalization (100 vs. 72%, P = 0.018), better functional outcome (88.4 vs. 48.0%, P = 0.044), and faster procedure times (mean 65.1 mins vs. 114.8 mins, P < 0.001). The number of attempts was similar between both groups (median 2 vs 3 attempts, P = 0.101). There was no significant difference in the complication rate between both groups (5.9 vs. 12.0%, P = 0.462).ConclusionCompared to previous endovascular techniques for treating atherosclerotic tandem occlusions, the direct technique using standard 0.035' guidewires and dotterisation or a peripheral vascular balloon is significantly faster with better outcomes. However, this will require further external validation in larger cohorts.

Ex vivo lung perfusion (EVLP) is a relevant procedure to increase the lung donor pool but could potentially increase the airway tree ischemic injury risk.This study aimed to evaluate the direct effect of EVLP on the airway tree by evaluating bronchial cell vitality and tissue signs of injury on a series of 117 bronchial rings collected from 40 conventional and 19 EVLP-treated lung grafts. Bronchial rings and related scraped bronchial epithelial cells were collected before the EVLP procedure and surgical anastomosis.The preimplantation interval was significantly increased in the EVLP graft group (p 0.01). Conventional grafts presented cell viability percentages of 47.07 ± 23.41 and 49.65 ± 21.25 in the first and second grafts which did not differ significantly from the EVLP group (first graft 50.54 ± 25.83 and second graft 50.22 ± 20.90 cell viability percentage). No significant differences in terms of histopathological features (edema, inflammatory infiltrate, and mucosa ulceration) were observed comparing conventional and EVLP samples. A comparison of bronchial cell viability and histopathology of EVLP samples retrieved at different time intervals revealed no significant differences. Accordingly, major bronchial complications after lung transplant were not observed in both groups.Based on these data, we observed that EVLP did not significantly impact bronchial cell vitality and airway tissue preservation nor interfere with bronchial anastomosis healing, further supporting it as a safe and useful procedure.

BACKGROUND Postoperative pain after cardiac surgery is a very important issue and affects recovery, risk of postoperative complications and quality of life. The pain management has been traditionally based on intravenous opioids with growing evidence suggesting the use of opioid-free and opioid-sparing techniques to reduce its adverse effects. CASE PRESENTATION We report the case of a 75-years-old frail patient underwent awake mediastinal revision with subxiphoid access due to deep sternal wound infection using a Pectoralis-Intercostal Rectus Sheath (PIRS) plane block. During the procedure the patient never reported pain receiving acetaminophen 1 g every 8 hours for postoperative pain management without others pain relievers. CONCLUSION Ultrasound guided PIRS block could be an effective and safe analgesic technique to manage sternal and subxiphoid drainage pain in patients undergoing cardiac surgery via subxiphoid approach.

Systemic medications used during the treatment of onychomycosis caused by Trichophyton rubrum may have relapse, making them costly and insignificant for the patient. Photodynamic therapy (PDT) is an advantageous therapeutic option for disease control, mainly due to the absence of risk of microbial resistance. The action of PDT is combined by three elements: photosensitizer (PS), visible light (VL) and molecular oxygen, leading to the formation of reactive oxygen species (ROS) or singlet oxygen formation (1O2), both ROS (type I mechanism) as 1O2 (type II mechanism) induce damage and death to microbial cells. This research was proposed as a study through a non-systematic review, to investigate the action of PS "Protoporphyrin IX" (Pp IX) associated with visible light on T. rubrum, contemplating clinical and relevant data regarding the treatment of onychomycosis by PDT. PubMed survey was conducted from June 2021 to April 2022. The research strategy included clinical trials, randomized trials, systematic reviews, meta-analyses and reviews in English. Pp IX has affinity for T. rubrum, which is justified by the incorporation of Pp IX into the cell membrane, which led to inhibition by LV irradiation. Thus, the photodynamic process of Pp IX may lead to cell death by type I and II mechanisms. The data found are promising, however, new studies in vitro and in vivo are suggested, since few studies have been found related to the theme.

O nervo trigêmeo é o quinto (V) nervo craniano, se dividido em três ramos que saem do crânio através do rosto onde transmitem o sentido do rosto e os movimentos da mandíbula: A divisão oftálmica (V1) transmite sensibilidade da testa e dos olhos. divisão maxilar transmite a sensibilidade da justiça, lábio superior e bucal. A divisão mandibular (V3) transmite a sensibilidade da mandíbula e do lábio inferior; também permite o movimento dos envolvidos na mastigação e deglutição. A Neuralgia trigeminal é uma doença rara, mas muito dolorosa. Dor semelhante a um choque elétrico, que ocorrem sem aviso e são, mas frequentemente desencadeados por áreas específicas do rosto. O paciente pode apresentar crises inicialmente curtas e leves, com períodos de remissão relativamente longos. Mas a neuralgia do trigêmeo pode progredir, causando ataques mais longos e frequências de dor intensa. Embora a causa exata da neuralgia não seja totalmente compreendida, um vaso sanguíneo é frequentemente encontrado comprimindo o trigêmeo. Medicamentos, termo coagulação, cirurgia e medicamentos podem ser usados para tratar a dor e cada tratamento tem seus benefícios.

A interação entre a Odontologia e a Fonoaudiologia vem se aperfeiçoando satisfatoriamente, fundamentada, entre outros aspectos, no interesse partilhado na saúde e fisiologia do sistema estomatognático. A atuação de ambas as áreas está intimamente relacionada, uma vez que a intervenção de uma interfere e depende diretamente do trabalho da outra. O objetivo deste trabalho foi discorrer acerca da importância da ortodontia na fonoaudiologia para o atendimento do paciente infantil. Para o desenvolvimento desta revisão de literatura, foi realizada uma pesquisa nas bases de dados: SciELO e PubMed/Medline, utilizando as palavras-chaves “ortodontia”, “criança” e “fonoaudiologia”. A relação entre ambas as áreas é de suma importância para o tratamento e avaliação da prioridade de intervenções em cada caso, podendo o fonoaudiólogo atuar simultaneamente com o cirurgião-dentista, antes, durante ou após a terapia ortodôntica. Portanto, conclui-se, que a atuação conjunta destas especialidades é fundamental em todos os sentidos, pois o trabalho interdisciplinar fornece estabilidade e sucesso terapêutico, promovendo satisfação e saúde aos pacientes.

Os odontomas são tumores benignos da mandíbula compostos por derivados do epitélio odontogênico e ectomesênquima. Eles são divididos histologicamente em dois grupos: odontoma composto e odontoma complexo. O composto é formado por muitos dentes, pequenos, agrupados em um saco cuja parede conjuntiva é idêntica à do folículo dentário normal. A etiologia do odontoma é desconhecida, podem aparecer após um trauma antigo. A maioria dessas lesões são assintomáticas e são frequentemente detectadas em radiografias de rotina. O tratamento consiste na extração cirúrgica de o odontoma sozinho ou seguido de tratamento ortodôntico no caso de inclusão de um dente.

30. LEVANTAMENTO DOS PACIENTES COM CÂNCER DE BOCA ATENDIDOS NO CEO DO MUNICÍPIO DE FERNANDÓPOLIS/SP 2014-2021

O câncer de boca é uma doença crônica não transmissível. Pode ser desencadeado por alguns motivos, como o tabaco e bebidas alcoólicas quando consumidas de maneira exacerbada. Podemos citar ainda: fatores genéticos, infecções por vírus como o Papilomavírus Humano (HPV) e o vírus Epstein-Barr (EBV), condições socioeconômicas, idade, exposição profissional (como trabalhadores rurais expostos com muita frequência a radiação solar), baixo índice de massa corporal, hábitos de higiene oral, dentre outros. Acomete mais pessoas do sexo masculino, portanto, homens idosos, na maior parte das vezes com o diagnóstico de carcinoma de células escamosas ou carcinoma espinocelular. Investir em prevenção é a melhor maneira de se evitar o câncer de boca. O objetivo desta pesquisa, foi realizar um levantamento epidemiológico dos pacientes com câncer de boca atendidos no Centro de Especialidades Odontológicas (CEO) do município de Fernandópolis/SP no período de 2014-2021. A amostra deste levantamento foi obtida por meio da análise dos prontuários contidos nos arquivos da especialidade de Estomatologia do CEO do munícipio de Fernandópolis. Dos 48 pacientes, 32 foram diagnosticados com carcinoma epidermóide e apenas 1 com carcinoma verrucoso. A faixa etária variou entre 36 e 94 anos e 30 deles faziam o uso de tabaco, álcool ou ambos. Nessa pesquisa, as lesões foram predominantes nas seguintes regiões: rebordo alveolar inferior com 21% (11), palato mole com 17% (9) e borda lateral de língua com 17% (9). Apenas 23% dos pacientes contabilizados eram do sexo feminino, sendo os outros 77%, homens. Foi possível concluir que, houve maior número de diagnósticos de carcinoma epidermóide em homens de meia idade que possuíam hábitos tabagistas e/ou elitistas.

O câncer é um problema de saúde pública mundial, apresentando cerca de 12,7 milhões de casos em todo o mundo. Os pacientes com câncer de cabeça e pescoço que recebem tratamento radioterápico tendem apresentar complicações bucais como efeitos colaterais do seu tratamento. O objetivo deste artigo foi identificar as principais complicações bucais decorrentes do tratamento radioterápico do câncer de cabeça e pescoço e as condutas indicadas para prevenir e controlar essas complicações. O estudo constitui-se em uma revisão de literatura nas bases de dados Pubmed/Medline e Biblioteca Virtual em Saúde, utilizando os descritores “câncer de cabeça e pescoço”, “radioterapia” e “saúde bucal”. Foram identificados que as principais complicações bucais decorrentes do tratamento antineoplásico relatadas pela literatura são a mucosite, xerostomia, candidíase, cárie de radiação, osteorradionecrose e trismo. As condutas terapêuticas utilizadas pelo cirurgião-dentista são indicadas antes, durante e após o tratamento. Considerando-se a ocorrência de múltiplos sintomas no paciente oncológico, é necessário intervir para ter boas condições de saúde bucal antes, durante e após o tratamento, diminuindo assim, a ocorrência e a severidade das complicações apresentadas.

- Abstract
A osteorradionecrose (ORN) dos maxilares é uma consequência secundária das mais graves em pacientes submetidos a radioterapia de cabeça e pescoço. Trata-se de uma doença na qual o osso irradiado torna-se desvitalizado e exposto através da perda da integridade da pele e da mucosa, persistindo sem cicatrização. O objetivo do presente trabalho foi revisar a literatura sobre o manejo preventivo e terapêutico da ORN dos maxilares. O estudo constitui-se em uma revisão de literatura elaborada através do levantamento bibliográfico nas bases de dados da PubMed e Biblioteca Virtual da Saúde (BVS), utilizando os termos descritores “osteorradionecrose”, “radioterapia” e “assistência odontológica”. Foram identificados aspectos relacionados à prevenção e as diversas formas de tratamento da ORN, tais como oxigenoterapia hiperbárica, abordagem farmacológica e cirúrgica. Concluiu-se, por meio das informações obtidas na literatura, que a prevenção ainda é a melhor conduta, sendo o cirurgião-dentista um dos profissionais fundamentais nesse contexto. Com relação ao tratamento, observa-se que normalmente há uma combinação de medidas conservadoras e ressecção cirúrgica, sendo que a mais utilizada é a antibioticoterapia tópica ou sistêmica.

Para os pacientes com câncer, a manutenção de um estado de saúde sem maiores complicações é essencial e o bem-estar da boca está intimamente relacionado com uma condição de boa saúde. Deve-se considerar também que o paciente com câncer pode ter consequências negativas sobre os dentes devido aos efeitos da quimioterapia ou radioterapia. O objetivo deste trabalho foi demonstrar a importância do manejo odontológico em cuidados paliativos de pacientes com câncer bucal, por meio de uma revisão da literatura. As equipes de cuidados paliativos são formadas por profissionais de diferentes áreas da saúde para alcançar uma abordagem abrangente. O cirurgião-dentista pode desempenhar um papel importante, uma vez que a boca, órgão de expressão, é frequentemente acometida nas fases tardias das doenças oncológicas. Além disso, considera-se que o cirurgião-dentista desempenha um papel fundamental na educação dos outras pessoas envolvidas nesta gestão, a saúde bucal é uma obrigação funcional e emocional fortemente relacionada com a qualidade de vida, sendo essencial a integração de mais cirurgiões-dentistas no plano de cuidados continuados.

O carcinoma adenoide cístico (CAC) é a neoplasia maligna mais frequente das glândulas salivares menores, caracteriza-se clinicamente por ser de crescimento lento, a localização mais frequente é o palato duro. Objetivo: O objetivo do presente trabalho será discorrer sobre as principais características do CAC. Materiais e Métodos: Um levantamento bibliográfico foi realizado nas seguintes bases de dados: Scielo, Bireme, Pubmed, empregando-se os termos descritores: “carcinoma”, "glândulas salivares” e “tumor”, associados entre si pelo operador booleano “AND”. Resultados: O CAC é um tipo de câncer (tumor basalóide), com início nas glândulas salivares, geralmente são pequenos, com crescimento lento. Sintomas incluem uma úlcera no palato, um nódulo na face ou pescoço, causando dor ou incapacidade de mover certos músculos. O diagnóstico conta com anamnese e exame patológico (biópsia, FNA). Sua terapia é baseada na ressecção cirúrgica. O carcinoma adenoide cístico é uma neoplasia maligna de difícil diagnóstico e com elevada morbimortalidade associada. Mesmo após o tratamento o paciente deve ser constantemente avaliado. Conclusão: Foi possível concluir que o CAC é considerado o quinto lugar das lesões epiteliais malignas das glândulas salivares, por isso torna-se tão importante um diagnóstico precoce e correto tratamento com o intuito de evitar metástases.

Il est d'usage, dans toute une série de travaux sociolinguistiques, de distinguer deux traditions, deux directions de la politique linguistique d'un Etat. La première, conventionnellement appelée «orientale», pose la diversité des langues et des traditions culturelles comme naturelle et même désirable. L'idéal de la seconde tradition, appelée «occidentale», est un système unique de valeurs, une seule foi, une seule langue. On retrouve les sources de la première tradition chez les prophètes hébreux et chez les philosophes grecs de l'Antiquité ; la deuxième est historiquement liée avec l 'Empire Romain et l'Eglise catholique. Il existe une lutte constante entre ces deux traditions ; et, en règle générale, les différents pays suivent soit l'une soit l'autre. Le but du présent article est d'étudier la façon dont ces tendances ont coexisté et été en concurrence et en alternance dans la politique linguistique soviétique envers les petits peuples du Nord dans les années 1920-1930. Nous avons utilisé les périodiques de cette époque, et en particulier les publications du Comité du Nord, ainsi que les publications officielles du Narkompros et les publications non officielles de linguistes et de personnalités publiques.

Sans nous interroger sur les causes et les conséquences globales de ce discours de Staline, nous allons voir plus particulièrement comment sa manière de penser la question nationale a influencé, dans les publications de l 'époque, la façon de présenter le travail sur la latinisation et la création des alphabets. Dans Marxisme et philosophie du langage, Vološinov critique la phonétique expérimentale de son époque qu’il accuse d’empirisme phonétique. Le même reproche avait déjà été formulé par les deux leaders de l’«édification linguistique», à savoir Polivanov et Jakovlev. La question qui sert de fil rouge à cet article est donc : «Comment ces critiques ont-elles amené ces linguistes à revoir les définitions existantes du phonème» ? J’analyse ainsi les conceptions phonologiques développées en URSS au début dès années 1920, notamment celle de L. Ščerba et celle de N. Jakovlev. Vološinov ne cite dans son livre aucun de ces noms (les renvois non critiques à d’autres auteurs sont rares dans ce texte), mais on remarque que ses critiques de l’empirisme doivent beaucoup à Ščerba, dont il reprend mot à mot le raisonnement en changeant les exemples. Au-delà des pistes de recherches qu’offre ce sujet – le rapport entre Vološinov et Jakovlev, la comparaison de la phonologie saussurienne avec la phonologie soviétique des années 1920 – une lecture croisée de ces deux textes a été fructueuse dans le sens que chacun d’entre eux sert de filtre de lecture à l’autre. En outre, ma recherche rajoute un trait au «portrait» scientifique de l’auteur de Marxisme et philosophie du langage. On tient là un nouvel argument en faveur de l’hypothèse selon laquelle il s’agit bien de Valentin Vološinov : un auteur qui était au courant des recherches en phonétique expérimentale menées par Ščerba, Jakovlev et Polivanov, et qui y a probablement participé.

41. Marr et Jakovlev : deux projets d’alphabet abkhaz

- Abstract
Notre article se fonde sur une comparaison du projet d'«alphabet analytique abkhaz» que N. Marr élabora au tournant du XIXe et du XXe siècle avec l'alphabet abkhaz qui fut proposé 20 ans plus tard par son compatriote N. Jakovlev. L'article prend comme point de départ l'hypothèse que, malgré les différences fondamentales qui divisent les deux linguistes au sujet des principes linguistiques de l'élaboration d'un alphabet, leurs points de vue méthodologiques sont très proches. Après une comparaison de ces points de vue, nous constatons que leur affinité repose sur le fait qu'ils appartiennent à une même époque de l'étude des langues de l'URSS en général, et des langues caucasiennes en particulier. Nous concluons ensuite que Marr et Jakovlev ont en commun le désir d'élaborer une nouvelle linguistique fondée sur l'étude des langues non écrites et peu connues. Mais, passionné par la nouveauté de sa nouvelle doctrine sur le langage, Marr ne remarque pas la nouveauté de l'approche «phonologique» de l'alphabet avancée par Jakovlev et reste prisonnier de son approche «phonétique».

- Abstract
La notion de style fonctionnel a été introduite dans les «Thèses du cercle linguistique de Prague» (1929) et, nous semble-t-il, n'a pas laissé de traces significatives dans la linguistique occidentale. Dans la linguistique soviétique, en revanche, dès le début des années 1950, elle a joui d'une popularité surprenante jusqu'à devenir la base d'une discipline fort active, à savoir la stylistique fonctionnelle. Dans les années 1950, 1960 et 1970 paraissent par dizaines, puis par centaines, des articles et des livres sur le sujet, on organise des discussions, d'innombrables conférences . . . Cette discipline a eu un grand succès également en R.D.A., en Tchécoslovaquie, en Pologne, pays de «démocratie populaire». C'est une première coïncidence, la coïncidence spatiale : le concept et la discipline se trouvent localisés dans une aire géopolitique déterminée qu'on appelle le camp socialiste. Une deuxième coïncidence : à la fin des années 1980 - début des années 1990, après un épanouissement de plus de trente ans, la stylistique fonctionnelle quitte la scène, et nous pouvons affirmer aujourd'hui que cette discipline, tout comme le concept qui en a constitué la base, sont pratiquement oubliés. En d'autres mots, la fin de l'ère de la stylistique fonctionnelle coïncide (à un petit décalage près) avec l'effondrement du régime communiste. Cette double coïncidence, spatiale et temporelle, ne peut manquer de nous amener à l'idée que la stylistique fonctionnelle a été d'une certaine façon liée à l'idéologie dominante et à la pratique administrative de cette époque. Le but du présent article consiste à vérifier cette hypothèse, à essayer de déterminer ne serait-ce qu'hypothétiquement les facteurs subjectifs et objectifs internes et externes qui ont présidé à l ' instauration, à l'établissement, et à la chute de cette discipline. Il s'agira ici uniquement de la variante soviétique de la stylistique fonctionnelle et uniquement de la période d'après guerre, sans aborder sa préhistoire.

Les conceptions ontologiques de N. Marr ne s'accordaient pas vraiment avec ses conceptions gnoséologiques, ni ces dernières avec sa méthode de recherche. L'analyse pragmatico-fonctionnelle de la philosophie du langage de Marr permet de faire apparaître des échos des théories de W. von Humboldt, de H. Steinthal, des néo-platoniciens russes, de même que des fondateurs du fonctionnalisme et du pragmatisme en linguistique : A. Potebnja, H. Schuchardt, E. Cassirer, J. Baudouin de Courtenay, F. de Saussure, L. Ščerba, V. Mathesius, N. Troubetzkoy, etc. On a pu ainsi mettre en évidence certaines positions-clés de la philosophie du langage chez Marr : Dans son ontologie : un psychosociologisme métaphysique (« de classe »), un dualisme transcendental, une combinaison de réalisme socio-économique et d'idéalisme ethno-sociologique, un activisme historique, un téléologisme objectif et un préformisme socio-ethnique, un organicisme, un émergentisme, un globalisme prospectif, un pluralisme et un empirisme rétrospectifs, un verbalisme, un sémanticisme, un aposteriorisme linguo-génétique et un antinaturalisme. Dans sa gnoséologie : un objectivisme et un maximalisme, un phénoménologisme, un eïdétisme, une approche explicative, un herméneutisme, un diachronisme dynamique, un holisme, un anti-scientisme, anti-rationalisme et antiintellectualisme, un factualisme naturaliste. Des deux points de vue, les conceptions de Marr sont totalement éclectiques. Une bonne partie des malentendus suscités par les jugements sur Marr proviennent de la non-compréhension de ses conceptions méthodologiques et de la tentative de les interpréter à partir de positions scientifiques, et non philosophico-méthodologiques. Marr n'était pas un linguiste, mais un philosophe du langage.

With the global COVID-19 pandemic, governments from many countries in the world implemented various restrictions to prevent the SARS-Cov-2 virus's spread, including social distancing measures, quarantine, in-home lockdown, and the closure of services and public spaces. This led to an in-creased use of social media platforms to make people feel more connected, but also to maintain physical activity while self-isolating. Concerns about physical appearance and the desire to keep or reach a muscular and toned ideal body, might have further reinforced the engagement in fitness-related social media activities, like sharing progresses in training achievements or following more fitness contents on popular profiles. To better understand the underlying relation among these factors, the present study investigates 729 responses to the Exercise Addiction Inventory (EAI), the Appearance Anxiety Inventory (AAI), the Self-Compassion Scale (SCS) and their association to social media usage and compares the results cross-culturally in five countries (Spain, Lithuania, United Kingdom, Japan, and Hungary). Findings highlight significant differences between males and females, espe-cially in regard to the time spent online (U = 477.5, p = 0.036). Greater levels of appearance anxiety were associated with the exposure to fitness-related contents on social media. These results strongly confirm the previously highlighted association between fitspiration media and body image anxiety predominantly in females. Clinical implications and future considerations in terms of prevention and treatment in a situation of global emergency are also discussed. Context Coronavirus disease 2019 (COVID-19) pandemic has caused increased interest in the application of telehealth to provide care without exposing patients and physicians to the risk of contagion. The urological literature on the topic is sparse. Objective To perform a systematic review of the literature and evaluate all the available studies on urological applications of telehealth. Evidence acquisition After registration on PROSPERO, we searched PubMed and Scopus databases to collect any kind of studies evaluating any telehealth interventions in any urological conditions. The National Toxicology Program/Office of Health Assessment and Translation Risk of Bias Rating Tool for Human and Animal Studies was used to estimate the risk of bias. A narrative synthesis was performed. Evidence synthesis We identified 45 studies (11 concerning prostate cancer [PCa], three hematuria management, six urinary stones, 14 urinary incontinence [UI], five urinary tract infections [UTIs], and six other conditions), including 12 randomized controlled trials. The available literature indicates that telemedicine has been implemented successfully in several common clinical scenarios, including the decision-making process following a diagnosis of nonmetastatic PCa, follow-up care of patients with localized PCa after curative treatments, initial diagnosis of hematuria, management diagnosis and follow-up care of uncomplicated urinary stones and uncomplicated UTIs, and initial evaluation, behavioral therapies, and pelvic floor muscle training in UI patients, as well as follow-up care after surgical treatments of stress urinary incontinence or pelvic organ prolapse. The methodological quality of most of the reports was good. Conclusions Telehealth has been implemented successfully in selected patients with PCa, UI, pelvic organ prolapse, uncomplicated urinary stones, and UTIs. Many urological conditions are suitable for telehealth, but more studies are needed on other highly prevalent urological malignant and benign conditions. Likely, the COVID-19 pandemic will give a significant boost to the use of telemedicine. More robust data on long-term efficacy, safety, and health economics are necessary. Patient summary The diffusion of coronavirus disease 2019 (COVID-19) infections has recently increased the interest in telehealth, which is the adoption of telecommunication to deliver any health care activity. Context The coronavirus disease 2019 (COVID-19) pandemic raised concerns about the safety of laparoscopy due to the risk of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) diffusion in surgical smoke. Although no case of SARS-CoV-2 contagion related to surgical smoke has been reported, several international surgical societies recommended caution or even discouraged the use of a laparoscopic approach. Objective To evaluate the risk of virus spread due to surgical smoke during surgical procedures. Evidence acquisition We searched PubMed and Scopus for eligible studies, including clinical and preclinical studies assessing the presence of any virus in the surgical smoke from any surgical procedure or experimental model. Evidence synthesis We identified 24 studies. No study was found investigating SARS-CoV-2 or any other coronavirus. About other viruses, hepatitis B virus was identified in the surgical smoke collected during different laparoscopic surgeries (colorectal resections, gastrectomies, and hepatic wedge resections). Other clinical studies suggested a consistent risk of transmission for human papillomavirus (HPV) in the surgical treatments of HPV-related disease (mainly genital warts, laryngeal papillomas, or cutaneous lesions). Preclinical studies showed conflicting results, but HPV was shown to have a high risk of transmission. Conclusions Although all the available data come from different viruses, considering that the SARS-CoV-2 virus has been shown in blood and stools, the theoretical risk of virus diffusion through surgical smoke cannot be excluded. Specific clinical studies are needed to understand the effective presence of the virus in the surgical smoke of different surgical procedures and its concentration. Meanwhile, adoption of all the required protective strategies, including preoperative patient nasopharyngeal swab for COVID-19, seems mandatory. Patient summary In this systematic review, we looked at the risk of virus spread from surgical smoke exposure during surgery. Although no study was found investigating severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) or any other coronavirus, we found that the theoretical risk of virus diffusion through surgical smoke cannot be excluded.

Purpose To investigate the effects of different antibiotic prophylaxis regimens in patients with diabetes mellitus (DM) candidates to trans-rectal ultrasound-guided prostate biopsy (TRUSPB). Methods 143 outpatients with DM who underwent TRUSPB during the period 2018–2020 were selected from a cohort of 1150 patients in 3 different institutions. Exclusion criteria were allergies, concomitant anti-platelet therapies and uncontrolled DM. Different antibiotic prophylaxis regimens were adopted. Bacterial resistance levels to fluoroquinolones into the different communities were also collected. Univariable and multivariable binomial logistic regression analyses were used to assess the odds ratio (OR) with 95% confidence intervals (CIs) testing the risk of infective complications' occurrence after adjusting for clinical covariates. Results Overall, DM patients were significantly associated with infective complications' occurrence (p 37 °C were found in 9.1% and 1.5% (p Conclusion Fluoroquinolones were active in antibiotic prophylaxis of diabetic patients who had undergone to TRUSPB independently from the level of bacterial resistance found in the community. These results conflict with the recent European warning and support the Japanese and American guidelines on the topic.

Background: The aim of this study was to investigate the long-term effects of ureteral stenting and the exact timing of stent removal in favor of surgery in patients with idiopathic retroperitoneal fibrosis (IRF). Summary: Medline research terms of “idiopathic retroperitoneal fibrosis” AND “ medical therapy” OR “ureteral stenting” OR “surgical treatment” were done. Systematic reviews and observational and clinical studies were analyzed to obtain indication regarding the objective of the study for a narrative review. Ninety-two papers were analyzed. The treatment of IRF includes the monitoring of retroperitoneal fibrotic process spread and the prevention of abdominal organs entrapment. Treatment of ureteral obstruction includes medical therapy and ureteral stenting (US) or percutaneous nephrostomy (PNS) to overcome the worsening of renal function. Up to now, the timing of US or PNS removal is not yet clear, both for the complexity of evaluating the efficacy of the medical therapy and demonstrating the resolution of obstructive nephropathy. Moreover, it is not yet clear if the long-term ureteral stent placement or PNS is able to maintain an efficient renal function. Ureterolysis with a laparoscopic robot-assisted approach is now considered as an ultimate treatment for ureteral obstruction, limiting the progression of kidney impairment and improving the quality of life of patients, although nephrologists are generally abdicant regarding the potential switch toward the surgical approach. Key Messages: Prospective studies regarding the long-term effects of US on the renal function impairment in patients with IRF should be structured to obtain adequate information on the exact timing for the surgical approach.
