Cyclophilin A (CypA), a cytosolic binding protein of cyclosporine A, is an immunosuppressive drug. In this study, CypA cDNA was cloned from the two-spotted cricket Gryllus bimaculatus (gCypA). The protein encoded by gCypA comprises 165 amino acids with a molecular mass of 19.23 kDa and an isoelectric point of 9.38 and possesses three N-glycosylation sites and 17 phosphorylation sites. The secondary and tertiary structures of gCypA were identified, and homology analysis revealed that it shares around 73%-81% sequence identities with other CypA proteins. When the researchers analyzed the expression levels of gCypA mRNA in various tissues, they found that the foregut exhibited nearly the same expression level as that of the dorsal longitudinal flight muscle (the control). However, gCypA mRNA expression in the fat body, Malpighian tubes, and midgut was less than half of that in the dorsal longitudinal flight muscle. Under endoplasmic reticulum stress conditions, gCypA mRNA expression was highest in Malpighian tubules (about two times higher than the expression in the control). Under starvation conditions, gCypA mRNA expression increased to three times that of the dorsal longitudinal flight muscle 6 days after starvation. Nonetheless, its expression levels decreased in Malpighian tubules under all starvation conditions. This study provides insights into the physiological role of gCypA in G. bimaculatus.

Background: Complex orbital fractures are impure orbital fractures because they involve the orbital walls and mid-facial bones. The author reported an orbital wall restoration technique in which the primary orbital wall fragments were restored to their prior position in complex orbital fractures in 2020. As a follow-up to a previous preliminary study, this study retrospectively reviewed the surgical results of complex orbital wall fractures over a 4-year period and compared the surgical outcomes by dividing them into groups with and without balloon restoration.Methods: Data of 939 patients with facial bone fractures between August 2018 and August 2022 were reviewed. Of these, 154 had complex orbital fractures. Among them, 44 and 110 underwent reduction with and without the balloon technique respectively. Pre- and postoperative Naugle exophthalmometer (Good-Lite Co.) scales were evaluated. The orbital volume and orbital volume ratio were calculated from preoperative and 6 months postoperative computed tomography images.Results: Among 154 patients with complex orbital fractures, 44 patients underwent restoration with the balloon technique, and 110 patients underwent restoration without it. The Naugle scale did not differ significantly between the two groups, but the orbital volume ratio significantly decreased by 3.32% and 2.39% in groups with and without the balloon technique and the difference in OVR was significantly greater in patients in the balloon restoration group compared with the control group. Postoperative balloon rupture occurred in six out of 44 cases (13.64%). None of the six patients with balloon rupture showed significant enophthalmos at 6 months of follow-up.Conclusion: The balloon rupture rate was 13.64% (6/44 cases) with marginal screw fixation, blunt screws, and extra protection with a resorbable foam dressing. Furthermore, we restored the orbital wall with primary orbital fragments using balloon support in complex orbital wall fractures.

Background/Aim: Abdominal ultrasonography (USG) is recommended as a surveillance test for high-risk groups for hepatocellular carcinoma (HCC). This study aimed to analyze the current status of the national cancer surveillance program for HCC in South Korea and investigate the effects of patient-, physician-, and machine-related factors on HCC detection sensitivity.Methods: This multicenter retrospective cohort study collected surveillance USG data from the high-risk group for HCC (liver cirrhosis or chronic hepatitis B or C >40 years of age) at eight South Korean tertiary hospitals in 2017.Results: In 2017, 45 experienced hepatologists or radiologists performed 8,512 USG examinations. The physicians had a mean 15.0±8.3 years of experience; more hepatologists (61.4%) than radiologists (38.6%) participated. Each USG scan took a mean 12.2±3.4 minutes. The HCC detection rate by surveillance USG was 0.3% (n=23). Over 27 months of follow-up, an additional 135 patients (0.7%) developed new HCC. The patients were classified into three groups based on timing of HCC diagnosis since the 1st surveillance USG, and no significant intergroup difference in HCC characteristics was noted. HCC detection was significantly associated with patient-related factors, such as old age and advanced fibrosis, but not with physician- or machine-related factors.Conclusions: This is the first study of the current status of USG as a surveillance method for HCC at tertiary hospitals in South Korea. It is necessary to develop quality indicators and quality assessment procedures for USG to improve the detection rate of HCC.

In reproduction, mitochondria produce bioenergy, help to synthesize biomolecules, and support the ovaries, oogenesis, and preimplantation embryos, thereby facilitating healthy live births. However, the regulatory mechanism of mitochondria in oocytes and embryos during oogenesis and embryo development has not been clearly elucidated. The functional activity of mitochondria is crucial for determining the quality of oocytes and embryos; therefore, the underlying mechanism must be better understood. In this review, we summarize the specific role of mitochondria in reproduction in oocytes and embryos. We also briefly discuss the recovery of mitochondrial function in gametes and zygotes. First, we introduce the general characteristics of mitochondria in cells, including their roles in adenosine triphosphate and reactive oxygen species production, calcium homeostasis, and programmed cell death. Second, we present the unique characteristics of mitochondria in female reproduction, covering the bottleneck theory, mitochondrial shape, and mitochondrial metabolic pathways during oogenesis and preimplantation embryo development. Mitochondrial dysfunction is associated with ovarian aging, a diminished ovarian reserve, a poor ovarian response, and several reproduction problems in gametes and zygotes, such as aneuploidy and genetic disorders. Finally, we briefly describe which factors are involved in mitochondrial dysfunction and how mitochondrial function can be recovered in reproduction. We hope to provide a new viewpoint regarding factors that can overcome mitochondrial dysfunction in the field of reproductive medicine.

Owing to its non-invasive feature and excellent therapeutic effect, photodynamic therapy has received considerable interest in cancer therapy.

The purpose of this study was to explore the variables of foot pressure according to glycated hemoglobin level during walking in diabetic patients with pes rectus. The subjects of this study were 39 people (78 feet) in their 40s and 60s, and they were divided into 11 pre-diabetic patients (22 feet) and 14 diabetic patients (28 feet) based on glycated hemoglobin; 14 patients without diabetes (28 feet) was used as a control. Foot pressure was measured using the EMED-LE Measurement system. The sole of the foot was divided into 12 parts, each of which was analyzed for pressure, force, contact area, and grounding time. For statistics, nonparametric Kruskal–Wallis and Wilcoxon signed rank test were used. Prediabetic and diabetic patients had lower hindfoot and second and third metatarsal foot pressure but higher first and third toe pressure compared to those without diabetes. In addition, the maximum and minimum force were high in all parts of the sole, and the contact time was long, signifying strong downward force for a long duration. Measurement of foot pressure by parts allowed detailed analysis of abnormal foot pressure and is valuable as basic data for diagnosis, prediction, and treatment of diabetic foot. Based on these data, maximum and minimum pressure better explain the problem of plantar pressure distribution rather than mean maximum pressure.

Domestic wastewater treatment facilities(WTF) in Korea consume a lot of energy and generate greenhouse gases in the process of treating contaminated sewage. In response to climate change and the government's carbon neutrality, measures for energy reduction in WTF is an important issue along with water quality management, and simultaneously, diversification of renewable energy based on treatment facilities is essential. The energy independence and renewable energy utilization by facility capacity were analyzed based on sewage statistics (2019) and operation data of the National Sewage Information System. The amount of energy consumed by WTF was analyzed to be 83.6 million TOE, and the renewable energy produced was 10.8 million TOE. In consequence of analysis, the energy independence of domestic WTF in Korea was 12.9%. Among renewable energy, the contribution of biogas was the highest at 83.6%, but the current status of energy independence is somewhat insufficient to energy independence plan of WTF, requiring support for alternatives and policies to produce and secure additional renewable energy. In order to increase and diversify the use of renewable energy, the integrated digestion biogas of organic waste resources, the use of cooling and heating energy based on sewage heat, and the development of salinity gradient power generation were reviewed. Based on this, it was intended to contribute to the preparation of measures to achieve carbon neutrality in sewage treatment facilities.

Purpose: This study evaluated radiosensitivity and the tumor microenvironment (TME) to identify characteristics of breast cancer patients who would benefit most from radiation therapy.Methods: We analyzed 1,903 records from the Molecular Taxonomy of Breast Cancer International Consortium cohort using the radiosensitivity index (RSI) and gene expression deconvolution algorithms, CIBERSORT and xCell, that estimates the TME composition of tumor samples. In this study, patients were stratified according to TME and radiosensitivity. We performed integrative analyses of clinical and immuno-genomic data to characterize molecular features associated with radiosensitivity.Results: Radiosensitivity was significantly associated with activation of antitumor immunity. In contrast, radioresistance was associated with a reactive stromal microenvironment. The immuno-genomic analysis revealed that estrogen receptor (ER) pathway activity was correlated with suppression of antitumor immunity. In ER-negative disease, the best prognosis was shown in the immune-infiltrated and radiosensitive group patients, and the lowest was in the immune-excluded and radioresistant group patients. In ER-positive disease, immune signature and radiosensitivity had no prognostic significance.Conclusions: Taken together, these results suggest that tumor radiosensitivity is associated with activation of antitumor immunity and a better prognosis, particularly in patients with ER-negative breast cancer.

In this study, we developed and validated a high-performance liquid chromatography/photodiode array method for the quantification of glycine-proline-hydroxyproline (GPH) as a marker compound in skate skin collagen peptides (SCPs). The accurate molecular mass and amino acid sequence of this marker were determined using a QTOF mass spectrometer. Chromatographic separations were conducted using a 95:5 isocratic mobile phase of 0.1% trifluoroacetic acid (A) and acetonitrile/methanol (1:4 v/v, B) at a flow rate of 0.3 mL/min; the separated compounds were detected at 214 nm using a Jupiter® 4 μm Proteo 90Å column. The method was validated for specificity, linearity, limit of detection (LOD), limit of quantitation (LOQ), accuracy, and precision. It was found to be linear in the range of 1.0-50.0 μg/mL, with a good correlation coefficient (R2=0.9999) and excellent specificity. The LOD and LOQ were 0.07 and 0.22 μg/mL, respectively. A recovery study determined the accuracy of the method, with an average recovery of 103.76%, 100.35% and 103.97% of the marker at 10, 15 and 25 μg/mL, respectively, and a precision study showed a relative standard deviation of less than 2%. Additionally, the DPPH and ABTS radical scavenging activities of the SCPs increased in a concentration-dependent manner, and UV-protective effect was confirmed by human keratinocyte (HaCaT cell) viability. Our study thus provides the foundation for developing functional foods or cosmetics using SCPs.

Purpose: This study aimed to identify the educational needs and analyze the priorities of infection control education content related to COVID-19 among nurses at a medium-sized hospital.Methods: In total, 110 nurses at a general hospital in G metropolitan city participated in the survey. Data were collected using structured questionnaires from February 14 to 21, 2022. The priorities of COVID-19 infection control education content were identified using a three-step analysis method, including the paired t-test, Borich priority formula, and the Locus for Focus model.Results: Seven content items with high educational needs were identified using the Locus for Focus model and the Borich priority formula. These included “use of personal protective equipment (PPE) when conducting a COVID-19 test”, “criteria and methods for ventilation of the test space”, “method of disinfecting the test space”, “facility management”, “isolated patient management”, “how to transfer patients within medical institutions”, and “how to transfer patients to other medical institutions”. These content items, as educational needs, should be considered as priorities among the 31 content items of COVID-19 infection control education.Conclusion: The results are significant in that this study investigated the educational needs for responding to and preparing for COVID-19 in nurses, who are directly engaged in conducting COVID-19 infection control activities at a small and medium-sized hospital. Furthermore, a priority analysis of needs for COVID-19 infection control education content was performed to realistically reflect the educational needs. These findings can be used for the development of infection control education programs for nurses.

Purpose: This study aimed to identify the factors that influence nursing performance for patients after attempted suicide among clinical nurses.Methods: The participants in this study were 200 clinical nurses working at hospitals in Korea. Data was collected from February 22, 2022 to March 18, 2022 through a structured online survey that included general characteristics, suicide literacy, attitudes towards suicide, social stigma toward suicidal people, and nursing performance for patients after attempted suicide. Purpose: In the Republic of Korea, the use of trauma centers was recently adopted and is expected to have better outcome for severely injured patients. This study aimed to evaluate the clinical outcomes and treatment methods for unstable pelvic bone fractures in trauma centers. Methods: The annual number of patients, clinical outcomes, and treatment methods of unstable pelvic bone fractures in three trauma centers from 2016 to 2020 were retrospectively reviewed. The patients were dichotomized into survivors and deceased, and demographic data, treatment, and clinical outcomes were compared. Multivariable analysis was performed to identify the factors associated with survival.Results: Among 237 patients, 101 (42.6%) were deceased. Mortality was lower in the later period (2019- 2020) compared with the early period (2016-2018; 33.6% vs. 50.0%, p = 0.011). Direct admission of an increasing number of patients to trauma centers reduced prehospital time. Although the use of angioembolization in treating pelvic bone fracture (p < 0.001), and the use of other treatment methods did not change significantly (2016-2020). Lower age, lowest systolic blood pressure in the trauma bay, and higher lactate level, international normalized ratio, the amount of packed red blood cell transfusion at 24 hours were positively associated with mortality in the multivariate analysis.Conclusion: Increasingly more patients with unstable pelvic bone fracture were admitted to trauma centers; mortality improved. Angioembolization increased significantly and multi-disciplinary modality for early bleeding control was still essential.

BACKGROUND: Endothelial activation and insult may contribute to the aggressive clinical course of small-cell lung cancer (SCLC); however, no predictive biomarker for this pathogenesis has been identified. OBJECTIVE: To evaluate the clinical impact of the endothelial activation and stress index (EASIX) in SCLC. METHODS: In this retrospective study, the EASIX was calculated from measurements of serum lactate dehydrogenase, creatinine, and platelet levels. A total of 264 patients with SCLC treated with platinum-based chemotherapy were stratified into high and low EASIX groups. RESULTS: Complete and objective response rates in the limited-stage (LD) were 19.5% vs. 33.3% (P= 0.050) and 85.4% vs. 97.9% (P= 0.028) in the high and low EASIX groups, respectively. There was no significant difference in the response rate between the two groups in the extensive-stage (ED). The median overall survival was 9.8 vs. 40.5 months in LD (P< 0.001) and 7.2 vs. 11.9 months in ED (P< 0.001) in the high and low EASIX groups, respectively. In multivariate analyses, a high EASIX level was an independent prognostic factor for worse progression-free and overall survival irrespective of stage. CONCLUSION: EASIX may be a potential predictive biomarker of SCLC.

PURPOSE: To investigate the effect of a 4-week training using a laboratory index on the competition record of elite female middle-distance runners.METHODS: A female, middle-distance, trained runner with the following characteristics: age, 20 years; height, 168.7 cm; weight, 64.3 kg; 27.2% fat; and VO2max, 56.4 VO2 mL/kg/min; volunteered to participate in this study. Before the training program, the participant took part in a 1,500-m track and field national event in April and the onset of blood lactate threshold was analyzed using (OBVA/ vOBLA), VO2max/vVO2max, and tVO2max tests. After completing the 4-week training program, the participant again took part in the 1,500-m track and field national event in May and her parameters were reassessed using the OBVA/vOBLA, VO2max/vVO2max, and tVO2max tests.RESULTS: Pre- and post-training indicate that vVO2max did not improve; however, tVO2max (206 seconds pre-training vs. 251.51 seconds post-training) and VO2max (56.4 vs. 59.3 VO2 mL/kg/min)significantly. However, this improvement in relative VO2 was due to weight loss, and the absolute value of VO2 (3.63 vs. 3.62 L/min) did not change. The 1,500-m race record in track and field events decreased significantly from a pre-training value of 5 minutes 03 seconds to a post-training value of 4 minutes 52 seconds. CONCLUSIONS: The results of this study indicate that utilizing laboratory indicators including vOBLA, vVO2max, and tVO2max may be extremely valuable when prescribing training programs for middle-distance runners.

While sevoflurane and desflurane have been regarded as inhalation agents providing rapid induction and emergence, previous studies demonstrated the superiority of desflurane-anesthesia compared to sevoflurane-anesthesia in the postoperative recovery in obese and geriatric patients. We investigated whether a short-term switch of sevoflurane to desflurane at the end of sevoflurane-anesthesia enhances patient postoperative recovery profile in non-obese patients. We randomly divide patients undergoing elective surgery (n = 60) into two groups: sevoflurane-anesthesia group (Group-S, n = 30 ) and sevoflurane-desflurane group (Group-SD, n = 30 ). In Group-S, patients received only sevoflurane-anesthesia until the end of surgery (for >2 hours). In Group-SD, sevoflurane was stopped and switched to desflurane-anesthesia before the completion of sevoflurane-anesthesia (for approximately 30 minutes). We assessed the intergroup differences in the times to get eye-opening, extubation, and a bispectral index of 80 (BIS-80). Group-SD showed significantly shorter times to get eye-opening ( 438 ± 101 vs. 295 ± 45 s; mean difference, 143 s; 95% confidence interval [CI], 101–183; p < 0.001 ), extubation ( 476 ± 108 vs. 312 ± 42 s; mean difference, 164 s; 95% CI, 116–220; p < 0.001 ), and BIS-80 ( 378 ± 124 vs. 265 ± 49 minutes; mean difference, 113 s; 95% CI, 58–168 p < 0.001 ) compared to Group-S. There was no between-group difference in postoperative nausea, vomiting, and hypoxia incidences. Our results suggested that the short-term (approximately 30 minutes) switch of sevoflurane to desflurane at the end of sevoflurane-anesthesia can facilitate the speed of postoperative patient recovery.
