The first clinical thermofiltration procedures (plasma filtration near and above normal physiologic temperature) were performed on a secondary hypercholesterolemia patient. The intent of this filtration scheme is the selective removal of low density lipoprotins (LDL) and maintenance of high density limpoproteins (HDL) and other essential plasma proteins including albumin. The EVAL 4A filter, at a filtration temperature of 37°C, was selected for on-line thermofiltration. The sieving coefficients of LDL + very low density lipoproteins (VLDL) cholesterol, HDL cholesterol, and albumin were 0.02, 0.75, and 0.78, respectively. These results were comparable to in vitro filtration tests using the plasma of the same patient. This system is simple to apply and does not require replacement solution. Compared with plasma exchange, a high return of HDL and other essential plasma solutes can be returned to the patient by this method. The recognition of macromolecule abnormalities in various metabolic or immunologic related disease states has led to the use of plasma exchange for therapeutics. Limitations of plasma exchange, however, exist. Membrane plasma filtration provides a practical alternative. Solute removal can be made selective by the choice of operating conditions as temperature and the selection of the membrane/module design. The optimal temperature range is a function of the plasma type, solute concentrations and membrane system. Operation at below physiologic temperature (cryofiltration) is particularly suited for the removal of cold aggregative solutes, while operation at near or above physiologic temperature is more suited for the separation of solutes having large size differences at normal physiologic temperature. Membrane filtration is simple and safe to perform, is more selective than plasma exchange, does not require plasma product infusion and is more versatile than selective sorption in its applications.

循環疾患々者にたいする運動負荷試験を,客観性,再現性のあるものとするため,定量負荷型自転車エルゴメーターを試作した.このエルゴメーターは,他励式直流発電機,制御用トランジスタ一回路,増速機,直流定電圧電源回路からなり,ペダルの回転速度が運動負荷中に, 55～70回転/分の範囲内で変化しても,発電機の出力電圧が自動的に調制化されることによつて,あらかじめる設定した負荷量は±5%の変動を示すにすぎない.また,ペダルの回転によつて生じる騒音をへらし,運動負荷中の心音描記を可能とした.かようなエルゴメーターを用い,健常ならびに,循環疾患々者の心予備力を検討するため, 0.5W/kgの負荷量から順次に負荷量を増して, Reindellのいう最大エルゴスターゼの状態を求め,そのさいの運動負荷中,および,後の心力学的数値変動を調べ,考察した.終りにエルゴメトリーの国際標準化にかんする1966年の国際委員会の公示を紹介した.

We report the treatment outcomes of 5 cases of adult-onset Ewing sarcoma(ES)managed between 2011 and 2014. We examined prognostic factors including the primary lesion, tumor size, metastatic status, and serum LDH levels.The locations of the primary lesions included the limbs in 1 case and the trunk in 4; the cases in the trunk had a worse prognosis than that in the limbs. Tumor size was greater than 8 cm in only 1 patient, who also displayed evidence of metastases at presentation and high LDH levels. All the patients received chemotherapy consisting of alternating vincristine, doxorubicin, and cyclophosphamide(VDC)and etoposide and ifosfamide(IE). Surgery was selected for the treatment of 4 patients, and radiotherapy was administered to 1 patient for local treatment of the tumor. A median follow-up duration of 31.6 months revealed the 2-year overall survival rate and progression-free survival rate to be 80.0%.The prognosis of patients with adult-onset ES is poor; however, combined modality therapy, including VDC-IE, was demonstrated to improve the outcome of patients in the present study. Nevertheless, the patient with tumor size exceeding 8 cm, metastasis, and high LDH levels, relapsed 1 year after treatment, as reported previously. Further investigation is required to clarify the factors affecting prognosis in adults, and to develop effective therapies for patients with a poor prognosis.
