The regeneration of hierarchical osteochondral units is challenging due to difficulties in inducing spatial, directional and controllable differentiation of mesenchymal stem cells (MSCs) into cartilage and bone compartments. Emerging organoid technology offers new opportunities for osteochondral regeneration. In this study, we developed gelatin-based microcryogels customized using hyaluronic acid (HA) and hydroxyapatite (HYP), respectively for inducing cartilage and bone regeneration (denoted as CH-Microcryogels and OS-Microcryogels) through in vivo self-assembly into osteochondral organoids. The customized microcryogels showed good cytocompatibility and induced chondrogenic and osteogenic differentiation of MSCs, while also demonstrating the ability to self-assemble into osteochondral organoids with no delamination in the biphasic cartilage-bone structure. Analysis by mRNA-seq showed that CH-Microcryogels promoted chondrogenic differentiation and inhibited inflammation, while OS-Microcryogels facilitated osteogenic differentiation and suppressed the immune response, by regulating specific signaling pathways. Finally, the in vivo engraftment of pre-differentiated customized microcryogels into canine osteochondral defects resulted in the spontaneous assembly of an osteochondral unit, inducing simultaneous regeneration of both articular cartilage and subchondral bone. In conclusion, this novel approach for generating self-assembling osteochondral organoids utilizing tailor-made microcryogels presents a highly promising avenue for advancing the field of tissue engineering.

The detection of changes in synthetic aperture radar (SAR) images based on deep learning has been widely used in landslides detection, flood disaster monitoring, and other fields of change detection due to its high classification accuracy. However, the inherent speckle noise in SAR images restricts the performance of existing SAR image change detection algorithms by clustering analysis. Therefore, this paper proposes a novel method for SAR image change detection based on clustering fusion and deep neural networks. We first used hierarchical fuzzy c-means clustering (HFCM ) to process two different images to obtain HFCM classification results. Then a fusion strategy is designed to obtain the fused image from the two HFCM classified images as the pre-classification result. Furthermore, a lightweight deep neural network com posed of a decomposition convolution module and an auxiliary classification module was proposed; the former module could reduce network parameters by 28%, and the latter could reduce network parameters by 33.3%. To improve the recognition performance of the network, the classification layer was replaced by the regression layer at the outcome of the network. By comparing the experiments of different methods on five data sets, the performance of our proposed method is superior.

Currently, no literature specifically addresses the curative efficacy of laparoscopic duodenum-preserving total pancreatic head resection (LDPPHRt) in the treatment of pancreatic-head intraductal papillary mucinous neoplasm (IPMN). This study aimed to compare the short-term and long-term outcomes between LDPPHRt and laparoscopic pancreaticoduodenectomy (LPD) for pancreatic-head IPMN.We retrospectively reviewed patients undergoing LDPPHRt or LPD for pancreatic-head IPMN in our institution between September 2014 and October 2020. We collected, analyzed and compared preoperative, intraoperative, and perioperative data, including quality of life as assessed using EORTC QLQ-C30 and QLQ-PAN26 questionnaires.In total, 50 patients were incorporated into this study, which included 12 LDPPHRt patients and 38 LPD patients. Preoperative data was comparable in the two groups, and neither was there any significant difference in postoperative data. The incidence of exocrine and endocrine insufficiency was similar between the LDPPHRt and LPD groups (50% vs. 28.9%, p = 0.321; 8.3% vs. 7.9%, p = 1.000), but some differences between the two surgical procedures were apparent when evaluating postoperative quality of life. Specifically, patients in the LDPPHRt group reported higher physical functional and body image scores than the LPD group (96.1 ± 6.0 vs. 88.2 ± 13.9, p = 0.008; 65.3 ± 32.9 vs. 43.0 ± 31.4, p = 0.039), and LDPPHRt was also associated with lower digestive symptom scores (5.5 ± 12.9 vs. 28.9 ± 25.9, p = 0.004).Perioperative outcomes for LDPPHRt were comparable to those for LPD, but the quality of life after operation was better in the LDPPHRt group.
