Purpose: This clinical study sought to evaluate the possible clinical effectiveness and practicality of URINO, an innovative, incisionless, and disposable intravaginal device, designed for patients suffering from stress urinary incontinence.Methods: A prospective, multicenter, single-arm clinical trial was carried out, involving women diagnosed with stress urinary incontinence who used a self-inserted, disposable intravaginal pessary device. Comparisons were made between the results of the 20-minute pad-weight gain (PWG) test at baseline and visit 3, where the device was applied. After 1 week of device usage, compliance, satisfaction, the sensation of a foreign body, and adverse events were assessed.Results: Out of 45 participants, 39 completed the trial and expressed satisfaction within the modified intention-to-treat group. The average 20-minute PWG of participants was 17.2±33.6 g at baseline and significantly dropped to 5.3±16.2 g at visit 3 with device application. A total of 87.2% of participants exhibited a reduction ratio of PWG by 50% or more, surpassing the clinical trial success benchmark of 76%. The mean compliance was recorded as 76.6%±26.6%, the average visual analogue scale score for patient satisfaction was 6.4±2.6, and the sensation of a foreign body, measured on a 5-point Likert scale, was 3.1±1.2 after 1 week of device use. No serious adverse events were reported; there was 1 instance of microscopic hematuria and 2 cases of pyuria, all of which recovered.Conclusions: The investigated device demonstrated significant clinical effectiveness and safety for patients with stress urinary incontinence. It was easy to use, showing favorable patient compliance. We propose that these disposable intravaginal pessaries could potentially be an alternative treatment for patients with stress urinary incontinence who are seeking nonsurgical options or are unable to undergo surgery.Trial Registration: The study was registered as a clinical trial (KCT0008369).

Fire extinguishers are considered essential safety items to be installed in residential spaces to improve the fire safety in buildings. However, they are usually placed in inconspicuous locations owing to design and practical constraints, making their immediate use difficult in a fire scenario. This study identifies problems regarding the application of existing fire extinguishers in residential spaces, and investigates factors influencing user preferences in recognizing fire extinguishers. A draft fire extinguisher design that aims to address the problems associated with existing fire extinguishers in terms of design and practicality was derived. Using this draft, a survey was conducted to analyze the factors influencing user preference while considering the improvement of practical problems with existing fire extinguishers. A life-friendly product design is proposed to ensure its use at all times in daily life; an indoor air quality measurement sensor and fire detection/alarm system are installed. Results of the survey confirmed that aesthetic factors and novelty had the most influence on overall likeability towards preferring a fire extinguisher. This study is expected to contribute to the future development of fire extinguishers by verifying the preference factors for users to be willing to keep fire extinguishers indoors.

Gastric cancer (GC) usually metastasizes to locoregional lymph nodes, the peritoneum causing ascites, or to the liver and lungs but rarely to the brain. Few cases of brain-only GC metastases have been reported, and the prognosis is very poor. We present a case of brain-only metastasis 2 years after curative GC surgery and chemotherapy, who visited the ER due to a headache. The patient underwent operative tumor resection of the brain with additional chemotherapy and is still alive and disease free 10 years after initial curative treatment. This case shows that brain-only metastasis may have a better prognosis than that of systemic metastasis; thus, aggressive treatment with radiotherapy or surgery with chemotherapy may be needed.

This research was conducted to determine the biodiversity of higher fungi at the supersite of Mt. Jeombong from July to October 2017 during the second long-term ecology research by the National Institute of Ecology under the management of the Ministry of Environment of Korea. Forty-three families, 74 genera, and 130 species of higher fungi were found within the permanent square area of Mt. Jeombong and a 10 m radius of the ecological flux tower. Russulaceae (54 species, 21.9%), Amanitaceae (17 species, 6.9%), and Boletaceae (17 species, 6.9%) were the top three species taxa found in mycorrhizal mushrooms. Mycenaceae (nine species, 8.5%), Polyporaceae (11 species, 4.5%), and Strophariaceae (11 species, 4.5%) were the top three species taxa found in saprophytic mushrooms. These results were analyzed and compared with those of previous fungal mushroom studies.

Objectives: The coronavirus disease 2019 (COVID-19) pandemic has led to a global shortage of medical resources; therefore, we investigated whether COVID-19 impacted the quality of non-COVID-19 hospital care in Korea by comparing hospital standardized mortality rates (HSMRs) before and during the pandemic.Methods: This retrospective cohort study analyzed Korean National Health Insurance discharge claim data obtained from January to June in 2017, 2018, 2019, and 2020. Patients’ in-hospital deaths were classified according to the most responsible diagnosis categories. The HSMR is calculated as the ratio of expected deaths to actual deaths. The time trend in the overall HSMR was analyzed by region and hospital type.Results: The final analysis included 2 252 824 patients. In 2020, the HSMR increased nationwide (HSMR, 99.3; 95% confidence interval [CI], 97.7 to 101.0) in comparison to 2019 (HSMR, 97.3; 95% CI, 95.8 to 98.8). In the COVID-19 pandemic zone, the HSMR increased significantly in 2020 (HSMR, 112.7; 95% CI, 107.0 to 118.7) compared to 2019 (HSMR, 101.7; 95% CI, 96.9 to 106.6). The HSMR in all general hospitals increased significantly in 2020 (HSMR, 106.4; 95% CI, 104.3 to 108.5) compared to 2019 (HSMR, 100.3; 95% CI, 98.4 to 102.2). Hospitals participating in the COVID-19 response had a lower HSMR (HSMR, 95.6; 95% CI, 93.9 to 97.4) than hospitals not participating in the COVID-19 response (HSMR, 124.3; 95% CI, 119.3 to 129.4).Conclusions: This study suggests that the COVID-19 pandemic may have negatively impacted the quality of care in hospitals, especially general hospitals with relatively few beds. In light of the COVID-19 pandemic, it is necessary to prevent excessive workloads in hospitals and to properly employ and coordinate the workforce.

OBJECTIVE Stereotactic radiosurgery (SRS), combined with contemporary targeted therapies and immunotherapies, has improved the overall survival of patients with lung adenocarcinoma (ADC). Given that histological subtypes reflect prognosis in patients with primary ADC, it is important to integrate pathological biomarkers to predict clinical outcomes after SRS in patients with brain metastases from lung ADC. Therefore, the authors investigated the prognostic relevance of various biomarkers of primary lung ADC for clinical outcomes after SRS. METHODS A total of 95 patients with 136 brain metastases (1–4 oligometastases) who were treated with Gamma Knife radiosurgery between January 2017 and December 2020 were included. The Kaplan-Meier method and univariate and multivariate analyses using Cox proportional hazard regression models were used to identify prognostic factors for local control, survival, and distant brain control. RESULTS Multivariate analysis revealed thyroid transcription factor 1 as an independent prognostic factor for local control (HR 0.098, 95% CI 0.014–0.698, p = 0.0203) and napsin A as a significant predictor of overall survival after SRS (HR 0.080, 95% CI 0.017–0.386, p < 0.01). In a subset analysis of epidermal growth factor receptor (EGFR) mutation, patients with EGFR exon 19 mutations showed better distant brain control than those with EGFR exon 21 mutations (p < 0.01). CONCLUSIONS Pathological biomarkers of primary cancer should be considered to predict clinical outcomes after SRS in patients with lung ADC. Use of such biomarkers may help to provide personalized treatment to each patient, improving clinical outcomes after SRS.

Objectives & Methods: The entire ≪Lingshu·Jingjin≫ content was analyzed to find repetitive patterns, as well as to analyze commonly used terms. Through this, the guidelines for interpretation were presented.Results & Conclusions: ≪Lingshu·Jingjin≫ is not a question-and-answer format, but a simple enumerated narrative format. There is no part corresponding to the introduction, only the descriptions of 12 individual Meridian-muscle exist. Each Meridian-muscle is divided into ‘Flow of Muscle Mechanics(流́走)’ part and ‘Diseases’ part, and ‘Flow of Muscle Mechanics’ of each Meridian-muscle can be divided into ‘Main Flow’ and ‘Branch’. ‘Main Flow’ is the most central ‘Flow’ in each ‘Meridian-muscle’, and ‘Branch’ is the part where ‘Main Flow’ ends and the description of another ‘Flow’ begins. ‘Branch’ has different expressions according to the importance. The expression also varies depending on the dynamics. ‘Meridian-muscle’ should be interpreted based on this.

Through this, the purpose of this study is to examine the various effects of not wearing a mask in recent years by examining the effect of makeup behavior and the expected value on appearance due to not wearing a mask, and examining the relationship between makeup behavior and purchase intention of color cosmetics. In order to achieve this objective, 300 women in their 20s and 40s who are sensitive to their appearance were selected from August 26, 2022 to August 30, 2022, using a non-probability sampling method called Convenience Sampling. Data were collected through an Internet-specialized survey company and used in the final thesis. Frequency analysis, factor analysis, reliability test, descriptive statistical analysis, and linear regression analysis were performed using SPSS 18.0 to examine the effect on the expected value of appearance management, makeup behavior, and purchase intention of color cosmetics. As a result of examining the effect of appearance management expectations on makeup behaviors, self-satisfaction expected values are in line with conformity-seeking makeup behaviors, fashion-seeking makeup behaviors, personality-seeking makeup behaviors, change-seeking makeup behaviors, and relationship-seeking makeup behaviors. In pursuit of makeup behavior, fashion-seeking makeup behavior, change-seeking makeup behavior, show-off makeup behavior, relationship-seeking makeup behavior, and conformity-seeking makeup behavior, fashion-seeking makeup behavior, individuality-seeking makeup behavior, show-off makeup behavior, these were found to have an effect on relationship-seeking makeup behavior. Next, as a result of examining the effect of makeup behavior on purchase intention of color cosmetics, fashion-seeking makeup behavior affects both base makeup and point makeup. In conclusion, we can confirm that it is possible to predict the change in the color cosmetics market as not wearing a mask increases the expected value of appearance and affects the makeup behavior. Accordingly, in the beauty industry, it is necessary to develop detailed services and various products that meet the needs of consumers. Furthermore, as the expectations of men's appearance are increasing in line with the expectation of not wearing a mask as a future study, it seems that a study comparing men and women should be conducted.

Background and Objectives This study aimed to investigate the protective effect of polydeoxyribonucleotide (PDRN) against skin flap necrosis in a murine skin flap model.Materials and Method Twenty mice with rectangular skin flaps on the dorsum were randomly divided into the PDRN (n=10) and pentobarbital sodium (PBS) (n=10) injection groups. PDRN (8 mg/kg) was subdermally injected at 12 different points immediately after the operation. Background: Lipomas are common benign tumors of mesenchymal origin that are composed of mature adipocytes. Giant lipomas have a diameter ≥ 10 cm in one or more dimensions or weigh at least 1,000 g. The surgical excision of a giant lipoma requires extensive dissection, increasing the risk of a seroma, which can cause surgical site complications such as wound infection and necrosis. Sclerotherapy with Abnobaviscum (Viscum album extract) is a relatively new technique used to reduce malignant pleural effusion. In this study, we evaluated the effectiveness of prophylactic sclerotherapy using Abnobaviscum to decrease seroma after giant lipoma excision.Methods: We conducted a retrospective medical record review of patients who underwent surgical excision for giant lipoma of the neck from January 2019 to December 2022. Sclerotherapy was performed on the first postoperative day in patients who consented to the procedure, and Abnobaviscum was instilled through the existing Hemovac drain. We compared the clinical course between those who underwent postoperative sclerotherapy and those who did not.Results: Among the 30 patients who underwent giant lipoma excision, we applied sclerotherapy with Abnobaviscum to 15 patients. The average time from surgery to Hemovac removal was statistically shorter in patients who underwent sclerotherapy (p= 0.004). Furthermore, seroma formation was significantly reduced in patients receiving sclerotherapy (p= 0.003).Conclusion: In patients undergoing giant lipoma excision, sclerotherapy using Abnobaviscum helps reduce postoperative seroma formation during the initial postoperative period. Penetrating wounds to the face are cosmetically devastating and can be life-threatening. If the foreign body causing the penetrating wound is a piece of wood, small remnants might be left behind after the initial treatment. A 33-year-old male patient presented to the emergency center after a piece of lumber pierced his face as a passenger in a traffic accident. The patient’s vital signs were stable, and emergency surgery was performed to remove the foreign body and repair the soft tissue. No noteworthy complications were seen after open reduction and internal fixation of the facial bone fractures. Seven months after the accident, the patient underwent scar revision along with full-thickness skin grafting for post-traumatic scars. After the surgery, pus-like discharge which was not previously present was observed, and the graft did not take well. A residual foreign body, which was the cause of graft failure, was found on computed tomography and the remaining foreign body was removed through revision surgery. The patient is receiving outpatient follow-up without any complications 6 months after surgery. This case demonstrates the importance of performing a careful evaluation to avoid missing a residual foreign body, especially if it is of wooden nature.
