We report a case of retroperitoneal emphysema caused by a renal abscess. A 45-year-old man with underlying type 2 diabetes mellitus visited the emergency department with right flank pain and a fever. On physical examination, right costovertebral tenderness in the ipsilateral flank was noted. Leukocytosis and high inflammatory marker levels were observed. Urinalysis showed pyuria and glucosuria. Urine culture was positive for Streptococcus agalactiae. A computed tomography scan of the abdomen showed a focal, lowattenuation lesion in the right kidney with a 3 cm, exophytic, high-attenuation lesion in the right kidney upper pole and gas-containing fluid collection within the retroperitoneal space. The diagnosis was retroperitoneal emphysema caused by a renal abscess. As the vital signs were stable and the patient refused puncture, we decided on a course of antibiotics alone with follow-up without percutaneous drainage or surgery. The patient improved without any complications. This is a rare case of a renal abscess penetrating the renal fascia and progressing to a posterior paranephric emphysema. The patient was treated with antibiotics alone and cured successfully. Early diagnosis and proper treatment are needed, and percutaneous drainage or urgent surgery would be beneficial for such cases depending on the patient's condition.

Melioidosis, an infectious disease caused by Burkholderia pseudomallei, is endemic in specific regions, including Southeast Asia and Northern Australia. In Japan, where no autochthonous has been reported to date, melioidosis is a rare infectious disease. Herein, we report a case of melioidosis in a 68-year-old Japanese man with renal abscess and bacteremia, but without pneumonia. The patient presented to our hospital and was admitted for fever and chills that have persisted for two months. It was speculated that he was infected in Thailand, where his family lives because he shuttled between Thailand and Japan. Blood cultures on admission identified Burkholderia species; however, the species was unidentifiable by matrix-assisted laser desorption ionization-time of flight mass spectrometry. Further re-examination, including culture, loop-mediated isothermal amplification, and multiplex polymerase chain reaction methods, finally identified Burkholderia pseudomallei. We treated the patient with intravenous ceftazidime for four weeks. In addition to the antibiotics administration, puncture drainage of the renal abscess was performed, and he gradually became afebrile. Intravenous ceftazidime was switched to oral sulfamethoxazole/trimethoprim on post-admission day 32, and he was discharged. After five months of oral sulfamethoxazole/trimethoprim, no recurrence was observed one year after discharge. To diagnose melioidosis, especially in non-endemic areas, a precise and thorough understanding of its epidemiology, presentation, and identification methods is necessary.

Xanthogranulomatous pyelonephritis (XGPN) is a rare disorder affecting the kidney which can fistulise to the colon in exceptional cases. We herein report a case of XGPN with renocolic fistula and large vessel thrombosis presenting with sepsis and pulmonary embolism. Preoperative diagnosis and strategic planning resulted in successful management. A 64-year-old woman presented to the emergency department with abdominal pain and a septic condition, corroborated by venous thromboembolism. Workup diagnosed a left renal abscess with calicocolic fistula. Scintigraphy confirmed a nonfunctioning left kidney. The patient underwent inferior vena cava filter placement and staged surgery. The first, damage control procedure was a loop ileostomy. Ten days later, when the patient’s conditions improved, she underwent left nephrectomy and left colectomy with primary anastomosis. Finally, a year later, the ileostomy was closed. At follow-up, the patient was well, with unremarkable renal function. Scrupulous diagnostics, multidisciplinary decision making, and staged intervention have been key to optimal outcome.

Renal and pararenal abscesses are rare but serious diseases that result from infection of kidneys or tissues around them. This article examines a clinical case of renal and paranephric abscess, presents modern data on the etiology of the disease, types of abscesses, and describes the mechanisms and risk factors for the development of renal and pararenal abscesses, as well as the main diagnostic measures.

Background: Imaging plays a very important role in the diagnosis of renal infection. It assists diagnosis and differentiates between different types of renal infection. It also helps to detect and manage the associated complications through image-guided drainage and aspiration. Contrast-enhanced CT is the most useful study. A contrast-enhanced study is essential for a complete evaluation of patients with renal inflammatory disease to demonstrate alterations in the renal parenchymal perfusion and excretion of the contrast material, which occur as a result of the inflammatory process. Objective: To detect the role of multidetector CT in diagnosis of renal infection. Materials and methods: This prospective study was conducted on sixty patients. The study was carried out at Diagnostic Radiology Departments of Menoufia University Hospital. The patients were presenting with fever (< 38°C) mainly and loin pain. Results: From 60 patients with renal infection, 18 cases were diagnosed as pyonephrosis, 16 cases were diagnosed as renal abscess, 12 cases were diagnosed as pyelonephritis, 4 cases were diagnosed as xanthogranulomatous pyelonephritis, 4 cases were diagnosed as perinephric abscess, 2 cases were diagnosed as emphysematous pyelonephritis, 2 cases were diagnosed as fungal ball, 2 cases were diagnosed as urinoma. The mean age was (39.4 ± 15.7). Conclusion: Contrast enhanced computed tomography is a perfect diagnostic modality to assess the severity of inflammatory process and reaching the diagnosis of renal infection. It also helps to reach the cause, predisposing factors, associations, complications and the extent of the disease.

Acute Lobar Nephronia (ALN) is a rare infective condition of the kidney currently described only in case reports and small case series. The diagnosis of ALN is made by characteristic clinico-radiological findings. Differentiation from acute pyelonephritis, renal abscess and renal tumor is crucial for proper management and to avoid unnecessary diagnostic interventions. Herein, we report a 58-year-old woman with an uncontrolled diabetes mellitus, who was diagnosed clinically as acute pyelonephritis and treated with standard duration of antibiotics but had recurrence of symptoms. On evaluation, she was found to have ALN which was treated successfully with prolonged antibiotic course.

Renal abscess and nephronia are uncommon diseases in children and with unknown global prevalence. Nephronia represents an intermediate state between pyelonephritis and renal abscess. Prompt diagnosis is important to prevent morbidity and mortality (sepsis, renal injury, death). Scientific advances have made these entities more evident, although they may still be underdiagnosed. Patients with this disease require prolonged intravenous antibiotic therapy and potentially surgical resolution, mostly when their size is bigger than 3 - 5 cm when therapy with intravenous antibiotics alone fails and they are accessible to percutaneous drainage. We describe five cases of pediatric patients from four private hospitals in Quito, Ecuador followed during a one-year period. In every case, the diagnosis was suspected mostly because of persistent fever, abdominal pain, severe leukocytosis and/or elevated inflammation biomarkers. Only one of them had a previous history of urinary tract malformation while in another one malformation was revealed in his actual admission. Every case had microbiologic isolation. All of them recovered successfully. To our knowledge, in our country, there are no previous reports of these diseases in pediatric patients, and worldwide, there is scarce data. Our aim is to alert doctors who work with children to be aware of this condition.

Background and Objective: Urinary tract infections (UTIs) are one of the most commonly reported infections worldwide. The diagnosis of these infections is primarily based on clinical symptoms and biochemical data. Diagnostic imaging plays an important role in the management of UTIs. The main objective of this prospective study was to evaluate and compare of sensitivity and specificity of ultrasonography and computed tomography examination in the diagnosis of UTIs.Patients and Methods: In this study, we examined 100 cases (48 males and 52 females of varying age groups) referred with clinical suspicion of urinary tract infection to the department of radiodiagnosis, KIMS Hospital, Bangalore. After history taking and clinical examination all patients underwent urine analysis, ultrasound, and computed tomography (CT) examination. Results: Uncomplicated cystitis followed by acute pyelonephritis (APN) was the most common type of UTI as per the results. The sensitivity and specificity of ultrasound in diagnosing uncomplicated cystitis were 88.57% and 100%, respectively, whereas, that of CT was 74.29% and 100%. The sensitivity and specificity of ultrasound in diagnosing APN were 64.51% and 100%, respectively, whereas, that of CT were 90.32% and 100%. Sensitivity and specificity of CT in diagnosing chronic pyelonephritis, renal abscess, and emphysematous pyelonephritis remained 100% throughout.Conclusion: Ultrasound was found to be a more sensitive investigation in cases of uncomplicated cystitis than CT. CT on the other hand was found to be more sensitive and specific in the diagnosis of UTI and its complications other than uncomplicated cystitis.

Acute pyelonephritis is a bacterial infection of the renal parenchyma and collecting system. Diagnosis is based on clinical findings of fever, flank pain, and urinary tract infection. Computed tomography findings include renal enlargement with wedge-shaped heterogeneous areas of decreased enhancement, known as a "striated nephrogram." Imaging is primarily used to diagnose complications such as emphysematous pyelonephritis, renal abscess, and pyonephrosis. Chronic pyelonephritis can have varying appearances on imaging ranging from xanthogranulomatous pyelonephritis or, in extreme cases, renal replacement lipomatosis.

Xanthogranulomatous pyelonephritis (XGP) is a rare form of chronic pyelonephritis, which is challenging to diagnose because its clinical presentation mimics other entities and is commonly associated with a history of urinary tract obstruction. We report a case of XGP in a young adult without nephrolithiasis and urinary tract obstruction. A 23-year-old woman presented with intermittent abdominal pain in the right upper quadrant persisting for the last ten months. The pain was dull, poorly localized, and started spreading to the right back, right shoulder, and right thigh in the last three months. Other complaints included fever, chills, pain during urination, and nausea. The patient had a history of infrequent urination, recurrent urinary tract infections (UTIs), and a low fluid intake. A physical examination revealed that the patient had right upper quadrant abdominal tenderness and right costovertebral angle tenderness. Laboratory findings showed leukocytosis and neutrophilia. The radiological examination revealed a round mass in the superior pole of the right kidney with mixed cystic and solid components, and a well-defined margin. It further enlarged from 4.5 cm to 10.6 cm in diameter in three months. The urologist performed a total right nephrectomy. The histopathological examination showed XGP with renal abscess. Proteus mirabilis was identified from the pus specimen culture. XGP should be considered in the diagnosis of patients having chronic UTI presented with or without the findings of urinary tract obstruction.

(1) Introduction and Aims: Little is known about the relationship between renal pathology and gallbladder pathology, although the two organs (the gallbladder and the right kidney) are in close proximity to one another. If a renal abscess disseminates, the gallbladder would be one of the secondary organs involved. As the bile provides a favorable environment for the development of pathogenic germs, it allows for the development of acute cholecystitis, even if calculi are absent, thus resulting in the development of acute acalculous cholecystitis. The aim of our study was to analyze the association between acute acalculous cholecystitis (AAC) and renal abscesses. (2) Methods: A department-wide retrospective cohort observational study including 67 patients with renal abscesses, with a mean age of 34.5+/−16.21 years and with five males and 62 females, was conducted. All of the patients were examined by an abdominal ultrasound. The lab tests included CBC with differential liver enzymes and serum bilirubin (in order to assess alterations in the liver function which can be associated with AAC) and serum creatinine (in order to assess the renal function). Blood culture and urine culture tests were also performed. (3) Results: Of the 67 patients with renal abscesses, eight (11.94%) were associated with acute cholecystitis: four cases (5.97%) of acalculous cholecystitis and four cases (5.97%) of calculous cholecystitis, two of which presented biliary sludge (acute micro-calculous cholecystitis). All four cases of acute acalculous cholecystitis presented with sepsis, and there was one case of septic shock at onset. We did not observe an impairment in renal function in the patients presenting with acute acalculous cholecystitis, and hepatic impairment was inconstant and moderate. All of the cases had a favorable outcome after a prompt initiation of intensive antibiotic therapy; both the renal abscess and the acute acalculous cholecystitis receded without further complications. (4) Conclusions: The association of acute acalculous cholecystitis with renal abscesses could be related to the possibility of germ dissemination from the infectious focus. In the case of a renal abscess, careful clinical, lab, and imaging exams of the gallbladder are recommended in order to ensure early therapeutic intervention in the event of an association with acute acalculous cholecystitis.

Wilms' tumor (WT) is the most common renal malignancy in children. Its clinical and radiologic presentation may mimic other pediatric renal diseases, including pyonephrosis or renal abscess. The authors report a case of a 3-year-old girl presenting with pyelonephritis and right renal mass suggestive of a renal abscess, not responding to antibiotics. Further investigations were conducted, including a percutaneous renal needle core biopsy. A stage I fully necrotic WT was finally diagnosed. This amazing case of a fully necrotic WT at diagnosis demonstrates the importance of anatomopathologic analyses in pediatric renal masses, even when the infection is suspected.

Diabetes is very common predisposing factor for UTI. The fact that asymptomatic bacteriuria is more common in females with diabetes as compared to males with diabetes. Asymptomatic bacteriuria (ASB) is defined as When a bacterial count of same species over10^5 per ml in mild stream clean catch specimen of urine on two occasion is detected without the symptom of urinary infection. Significant bacteriuria detected by urinary culture, without symptoms attributable to urinary tract like burning micturition, frequent micturition, urinary incontinence, urgency, painful micturition, suprapubic pain, flank pain or fever. Asymptomatic and symptomatic bacteriuria are more common in females with diabetes. Asymptomatic bacteriuria may be precursor for symptomatic bacteriuria. The UTI ranges from asymptomatic bacteriuria to lower cystitis, pyelonephritis, xanthogranulomatous pyelonephritis, renal abscess, perinephric abscess, and papillary necrosis. Asymptomatic bacteriuria seems to be non eradicable, recurring in case of diabetic females.

Urinary tract infection (UTI) affects patients of all ages and is a diagnosis that emergency physicians might make multiple times per shift. This article reviews the evaluation and management of patients with infections of the urinary tract. Definitions of asymptomatic bacteriuria, uncomplicated UTI, and complicated UTI are presented, as well as techniques for distinguishing them. The pathophysiology and clinical and laboratory diagnoses of UTI are described. Treatment of UTI is reviewed, with attention to bacteriuria and special populations, including pregnant, elderly/geriatric, and spinal cord injury patients.

We report the first case of microabscesses detected by polymerase chain reaction (PCR) amplification of nucleic acid from ultrasound-guided aspirated fluid in a three-year old boy with acute lymphoblastic leukemia and febrile neutropenia during induction chemotherapy. Fever persisted despite effective antifungal treatment. The addition of corticosteroid therapy successfully controlled the suspected immune reconstitution inflammatory syndrome (IRIS). This case highlights the utility of PCR and adjunctive corticosteroid in the approach of Candida tropicalis renal microabscesses in leukemic patients undergoing chemotherapy. The treatment of patients with renal intra-diverticular stones with minimal invasive surgery is always a challenge for the surgeon. We present here a report of a case of pure laparoscopic nephrolithotomy and another with a robotic approach for multiple calculi in renal calycenal diverticulum in two young female patients, previously treated by unsuccessful ureteroscopy in addition to two previous lumbotomies in one of the patients due to renal abscess.

A 5-year-old boy was diagnosed with left acute focal bacterial nephritis (AFBN) complicated with renal abscess (RA) on magnetic resonance imaging (MRI). MRI is useful for diagnosing AFBN and RA complications. He was administered antibiotics for 3 weeks on evaluation of MRI findings. Evaluation of apparent diffusion coefficient values over time may be useful as an index of treatment of RA.

Staphylococcus aureus bacteriuria (SABU) can occur in patients with S. aureus bacteremia (SAB). However, little is known on the (molecular) pathomechanisms of the renal passage of S. aureus. This review discusses the epidemiology and pathogenesis of SABU in patients with SAB and identifies knowledge gaps. The literature search was restricted to the English language. The prevalence of SABU in patients with SAB is 7.8%–39% depending on the study design. The main risk factor for SABU is urinary tract catheterization. SABU in SAB patients is associated with increased mortality. Given present evidence, hematogenous seeding—as seen in animal models—and the development of micro-abscesses best describe the translocation of S. aureus from blood to urine. Virulence factors that might be involved are adhesion factors, sortase A, and coagulase, among others. Other potential routes of bacterial translocation (eg, transcytosis, paracytosis, translocation via “Trojan horses”) were identified as knowledge gaps.

Background Red blood cell distribution width (RDW) is associated with the severe morbidity and mortality of some Gram-negative associated chronic diseases. We designed this retrospective study to identify whether RDW can be used as a factor to provide reference for the treatment of renal abscess. Methods All patients with renal abscess in our institution between April 2016 and November 2019 were collected, and 38 cases with the characteristic of initially diagnosis and no prior treatment were enrolled. Conservative treatment group and surgical intervention group were set based on the treatment of patients. Univariate analysis and the regression analysis were adopted to identify the difference between these two groups. Results Significant differences were found in Diabetes mellitus, RDW and size of renal abscess in both univariate and multifactor analysis. The median value of RDW in all patients was 13.62%, with 13.15% in conservative treatment group and 14.20% receiving surgical intervention (OR: 9.48, 95% CI: 1.15~78.02). Diabetes was present in 42.11% of patients, with the ratio of 20.0% in conservative treatment group and 66.67% in surgical intervention group (OR: 35.22, 95% CI: 1.55~799.05). The cut-off of RDW and size of renal abscess were 14.07% and 3.9 cm respectively. Conclusions RDW as an independent predictor of mortality in patients with Gram-negative bacteremia showed differences among patients with renal abscess who received different treatments. Since the relationship between RDW and Gram-negative bacteremia related to mortality, higher initial RDW value may represent a higher severity of renal abscess and a greater likelihood of the need of surgical intervention.

The condition is usually an asymptomatic normal variant. However, abnormal anatomic variants such as hydronephrosis, vesicoureteral reflux (VUR), and ureterocele are sometimes observed in a patient with a duplicated kidney. These abnormal variants usually lead to diagnostic challenges. Here, we report a case of congenital left duplex kidney with hydronephrosis that presented as an isolated left renal cyst in a 29-year-old woman. Case Report: We present the case of a 29-year-old woman who had left-side abdominal pain and fever for 1 day. Left-side flank throbbing pain was also noted. Laboratory investigations showed leukocytosis, pyuria and bacteriuria. Renal ultrasound revealed a huge hypoechoic mass around the left kidney, which was suspected to be a huge renal cyst or renal abscess. Under the impression of acute pyelonephritis with abscess formation, the patient was admitted for antibiotic treatment. The following abdominal computed tomography (CT) revealed a left duplex kidney with severe hydronephrosis and hydroureter. A percutaneous nephrostomy was then performed. Next, following a discussion with the patient, she underwent a robot-assisted left heminephrectomy. Conclusions: A duplex kidney and collecting system should be considered when chronic urologic problems occur. This report shows that because duplex kidney is a relatively common congenital abnormality, it should be considered in the differential diagnosis in young patients who present with renal cyst. This case also shows that patients can be managed effectively using robot-assisted heminephrectomy.

Aim Renal abscesses are relatively uncommon in Paediatrics when compared to the adult population. This may reflect the diagnostic difficulties of such a rare differential in children (in the developed world); its variable clinical presentations, reliance on imaging and non-specific clinical signs. This case series aims to understand the clinical presentation of children with renal abscesses, their possible risk factors, subsequent management and overall outcome. Methods A retrospective chart review of 8 patients (median, 3½ years) from the last 13 years (2005–18) with a diagnosis of renal abscess in terms of their potential unifying risk factors, clinical presentation, ultrasound findings, biochemical parameters, microbiological results, management options (surgical drainage vs. medical management) and overall time to recovery. Results In our cohort, the female to male ratio was 1.6:1. Commonest risk factors were vesicoureteric reflux (25%) and urinary tract infections (25%). The common presenting symptoms were fever (87.5%), flank pain (50%) and generalized abdominal pain (12.5%). Right sided renal abscesses commonly involved the upper pole (75%) while left sided abscesses were both upper (50%) and lower poles (50%). Mean CRP at presentation was 130 (range, 47–283) mg/L with neutrophilic leukocytosis on blood count. Mean creatinine was 49 (range, 20–139) µmol/L; urea 4.2 (range, 1.6–11) mmol/L. Commonest pus culture isolate included E.coli (25%) and S.aureus (25%). Average time to radiological resolution for those managed surgically was 4.3 months and 2.5 months for those managed medically (prolonged course of intravenous antibiotics). Conclusion Clinical presentation of renal abscesses in children is often non-specific. Ultrasound can be used as an initial screening tool. Commonest organisms isolated are S.aureus and E.coli. The time to radiological resolution of abscesses is variable and not improved by surgical intervention. Conservative management should be considered over invasive surgical procedures if the latter can be avoided.

Background Radiation dose reduction is a major concern in patients who undergo computed tomography (CT) to follow liver and renal abscess. Objectives The purpose of this study is to investigate the feasibility of ultralow-dose CT with iterative reconstruction (IR) to follow patients with liver and renal abscess. Methods This prospective study included 18 patients who underwent ultralow-dose CT with IR to follow abscesses (liver abscesses in 10 patients and renal abscesses in 8 patients; ULD group). The control group consisted of 14 patients who underwent follow-up standard-dose CT for liver abscesses during the same period. The objective image noise was evaluated by measuring standard deviation (SD) in the liver and subcutaneous fat to select a specific IR for qualitative analysis. Two radiologists independently evaluated subjective image quality, noise, and diagnostic confidence to evaluate abscess using a five-point Likert scale. Qualitative parameters were compared between the ULD and control groups with the Mann-Whitney U test. Results The mean CT dose index volume and dose length product of standard-dose CT were 8.7 ± 1.8 mGy and 555.8 ± 142.8 mGy·cm, respectively. Mean dose reduction of ultralow-dose CT was 71.8% compared to standard-dose CT. After measuring SDs, iDose level 5, which showed similar SD to standard-dose CT in both the subcutaneous fat and liver (P = 0.076, and P = 0.124), was selected for qualitative analysis. Ultralow-dose CT showed slightly worse subjective image quality (P < 0.001 for reader 1, and P = 0.005 for reader 2) and noise (P = 0.004 for reader 1, and P = 0.001 for reader 2) than standard-dose CT. However, the diagnostic confidence of ultralow-dose CT for evaluating abscess was comparably excellent to standard-dose CT (P = 0.808 for reader 1, and P = 0.301 for reader 2). Conclusions Ultralow-dose CT with IR can be used in the follow-up of liver and renal abscess with comparable diagnostic confidence.

Resumen Introducción. Los abscesos renales tras una infección urinaria en pediatría no son frecuentes. Reporte de los casos. Se trata de dos niñas con infección urinaria, de 13 y 8 meses, que desarrollan abscesos renales. En ambas, después de un uroanálisis sugestivo de infección urinaria, se inicia manejo empírico con amikacina. El reporte del urocultivo muestra crecimiento de Escherichia coli (E. coli) >100 000 UFC/ml, sensible a dicho antibiótico, con ecografía renal inicial normal en el primer caso y en el segundo con nefromegalia derecha. A pesar del manejo antibiótico persistió la fiebre por más de tres días, por lo que se sospecha una complicación supurada. En el primer caso, la ecografía renal de control refleja imágenes sugestivas de absceso renal, pero en el segundo, a pesar de ecografías seriadas, solo se reportó la nefromegalia, que llevó realizar una tomografía abdominal con contraste para confirmar el diagnóstico. El antibiótico inicial, a pesar de la sensibilidad in vitro, no fue capaz de controlar la formación de abscesos renales. En el primer caso, el proceso infeccioso se controló utilizando amikacina y ceftriaxona, pero en el segundo fue necesario meropenem y amikacina. En ningún momento se alteró la función renal. Es de anotar lo infrecuente que es el absceso renal en niños en nuestra institución, ya que no se ha encontrado ningún caso en varios años. Conclusiones. El absceso renal en pediatría no es frecuente, se desarrolla principalmente tras una infección urinaria. Son necesarios una alta sospecha y un adecuado diagnóstico para orientar su manejo, ya sea solo médico o asociado a drenaje quirúrgico. Summary Introduction: Renal abscesses are not common in pediatrics after urinary tract infections. Case reports: The two cases involve two 8 and a 13 month old girls with urinary tract infections, who also develop renal abscesses. In both cases, after a urinalysis shows signs of urinary tract infections, empirical management with amikacin is initiated. The uroculture report shows a growth of Escherichia coli (E. coli) >100,000 CFU/ml, sensitive to the given antibiotic, with a normal initial renal ultrasound in the first case and right nephromegaly in the second case. Despite the antibiotic treatment, the fever persisted for more than three days, which is why a suppurative complication is suspected. In the first case, the renal ultrasound from the control suggests the presence of a renal abscess, but in the second case, despite serial ultrasounds, only nephromegaly was reported; therefore, an abdominal tomography with contrast was performed in order to confirm the diagnosis. The initial antibiotic, despite in vitro sensitivity, was not able to control the formation of renal abscesses. In the first case, the infectious process was controlled using amikacin and ceftriaxone, but in the second case, meropenem and amikacin were necessary. At no point in time did the renal function change. It is important to note how infrequent renal abscesses in children are in our institution, given that there have not been any reported cases for several years. Conclusions: Renal abscesses in pediatrics are not frequent; they develop mainly after a urinary tract infection. A high level of suspicion along with an adequate diagnosis is needed in order to guide its management, be it only medical or associated with surgical drainage. Resumo Introdução. Os abscessos renais após uma infeção urinária em pediatria não são frequentes. Relatório dos casos. Trata-se de duas meninas com infeção urinária, de 13 e 8 meses, que desenvolvem abscessos renais. Em ambas, após uma análise de urina sugestiva de infeção urinária, se inicia manejo empírico com amika-cina. O relatório da cultura de urina mostra crescimento de Escherichia coli (E. coli) >100 000 UFC/ml, sensível a este antibiótico, com ultrassonografia renal inicial normal no primeiro caso e no segundo com nefromegalia direita. Apesar do manejo antibiótico persistiu a febre por mais de três dias, motivo de suspeita de uma complicação supurada. No primeiro caso, a ecografia renal de controle reflete imagens sugestivas de abscesso renal, mas no segundo, apesar de ultrassonografias seriadas, só foi reportada a nefromegalia, que levou a realizar uma tomografia abdominal com contraste para confirmar o diagnóstico. O antibiótico inicial, apesar da sensibilidade in vitro, não foi capaz de controlar a formação de abscessos renais. No primeiro caso, o processo infecioso foi controlado utilizando amikacina e ceftriaxona, mas no segundo foi necessário meropenem e amikacina. Em nenhum momento foi alterada a função renal. Vale anotar que o abscesso renal em crianças em nossa instituição é pouco frequente, já que não há registro de caso algum em vários anos. Conclusões. O abscesso renal em pediatria não é frequente, se desenvolve principalmente após uma infeção urinaria. É necessário uma alta suspeita e um adequado diagnóstico para orientar seu tratamento, seja somente médico ou associado à drenagem cirúrgica.

Background Renal abscess in children is a rare and severe form of infectious kidney disease that is responsible for several serious complications. In this report, we describe a previously healthy 5-year-old girl with a renal abscess caused by extended-spectrum β-lactamase (ESBL)-producing Escherichia coli (E. coli), which led to bacteremia and renal scarring. Case presentation The patient presented to our department with high fever, headache, vomiting for 2 days and high inflammatory response. We diagnosed her with a urinary tract infection and initiated treatment with ampicillin and cefotaxime. Gram-negative bacilli bacteremia was noted on day 3. On day 4, her fever persisted, and a computed tomography (CT) scan revealed a renal abscess in the left kidney. After identifying the bacteria as ESBL-producing E. coli from the blood culture, we switched to the antibiotic meropenem and continued treatment for 3 weeks. The renal abscess was not drained. Although the renal abscess was successfully treated and it disappeared, a low-density area remained in same lesion on subsequent CT scans and a dimercaptosuccinic acid renal scan performed 4 months after onset revealed renal scarring. Conclusion Given the increasing prevalence of ESBL-producing microorganisms, clinicians should be aware of the possibility of renal abscesses caused by community-acquired ESBL-producing organisms even in previously healthy children. Once a renal abscess is suspected, early diagnosis and management are important for reducing the risk of life-threating complications and renal scarring.

Urogenital tuberculosis is the commonest extrapulmonary tuberculosis after lymphadenitis, with the kidneys being the most frequently affected organ. The clinical manifestation of renal tuberculosis is frequently nonspecific, leading to many overlooked cases especially in a developed country. Herein, we report a case of renal tuberculosis in a 79-year-old Caucasian lady who presented with recurrent renal abscess refractory to antibiotics. Level of evidence: Level 5

Pyelonephritis refers to infection involving the renal parenchyma and renal pelvis. In most patients, uncomplicated pyelonephritis is diagnosed clinically and responds quickly to appropriate antibiotic treatment. If treatment is delayed, the patient is immunocompromised, or for other reasons, microabscesses that form during the acute phase of pyelonephritis may coalesce, forming a renal abscess. Patients with underlying diabetes are more vulnerable to complications, including emphysematous pyelonephritis in addition to abscess formation. Additionally, diabetics may not have the typical flank tenderness that helps to differentiate pyelonephritis from a lower urinary tract infection. Additional high-risk populations may include those with anatomic abnormalities of the urinary tract, vesicoureteral reflux, obstruction, pregnancy, nosocomial infection, or infection by treatment-resistant pathogens. Treatment goals include symptom relief, elimination of infection to avoid renal damage, and identification of predisposing factors to avoid future recurrences. The primary imaging modalities used in patients with pyelonephritis are CT, MRI, and ultrasound. The American College of Radiology Appropriateness Criteria are evidence-based guidelines for specific clinical conditions that are reviewed annually by a multidisciplinary expert panel. The guideline development and revision include an extensive analysis of current medical literature from peer reviewed journals and the application of well-established methodologies (RAND/UCLA Appropriateness Method and Grading of Recommendations Assessment, Development, and Evaluation or GRADE) to rate the appropriateness of imaging and treatment procedures for specific clinical scenarios. In those instances where evidence is lacking or equivocal, expert opinion may supplement the available evidence to recommend imaging or treatment.

Background. Renal abscess is a relatively uncommon yet debilitating and potentially fatal disease. There is no clearly defined, objective risk stratification tool available for emergency physicians’ and surgeons’ use in the emergency department (ED) to quickly determine the appropriate management strategy for these patients, despite early intervention having a beneficial impact on survival outcomes. Objective. This case control study evaluates the performance of Mortality in Emergency Department Sepsis Score (MEDS), Modified Early Warning Score (MEWS), Rapid Emergency Medicine Score (REMS), and Rapid Acute Physiology Score (RAPS) in predicting risk of mortality in ED adult patients with renal abscess. This will help emergency physicians, surgeons, and intensivists expedite the time-sensitive decision-making process. Methods. Data from 152 adult patients admitted to the EDs of two training and research hospitals who had undergone a contrast-enhanced computed tomography scan of the abdomen and was diagnosed with renal abscess from January 2011 to December 2015 were analyzed, with the corresponding MEDS, MEWS, REMS, RAPS, and mortality risks calculated. Ability to predict patient mortality was assessed via receiver operating curve analysis and calibration analysis. Results. MEDS was found to be the best performing physiologic scoring system, with sensitivity, specificity, and accuracy of 87.50%, 88.89%, and 88.82%, respectively. Area under receiver operating characteristic curve (AUROC) value was 0.9440, and negative predictive value was 99.22% with a cutoff of 9 points. Conclusion. Our study is the largest of its kind in examining ED patients with renal abscess. MEDS has been demonstrated to be superior to MEWS, REMS, and RAPS in predicting mortality for this patient population. We recommend its use for evaluation of disease severity and risk stratification in these patients, to expedite identification of critically ill patients requiring urgent intervention.

Acute pyelonephritis might be complicated by the formation of renal and perirenal abscesses and very rarely by renal vein thrombosis, which is a life-threatening condition. The main causative agents of acute pyelonephritis are enterobacteriaceae with the incidence of extended-spectrum beta-lactamase (ESBL)-producing strains increasing worldwide. We present the case of a 71-year-old Greek man with history of diabetes mellitus and recent hospitalization, who suffered from severe pyelonephritis with renal abscesses formation and associated bacteraemia caused by ESBL-producing Escherichia coli, complicated by extensive thrombosis of the ipsilateral renal vein and its branches, protruding also in the inferior venal cava. Our patient was effectively treated with anticoagulants and targeted antibiotic therapy, respectively, consisted of low molecular weight heparin transitioned to oral acenocoumarol for 3 months and 2-week course of intravenous meropenem followed by oral fosfomycin for additional 3 weeks as quidded by clinical and computed tomographic follow-up. In conclusion, in complicated urinary infections, caused by ESBL-producing enterobacteriaceae, oral fosfomycin might represent an effective option for step-down therapy of carbapenems, allowing the shortness of the duration of patient’s hospitalization and carbapenem use.

We report the case of a 58-year-old man diagnosed as having right renal stones and who had undergone shock wave lithotripsy several times. However, residual stones persisted and resulted in pyelonephritis and renal abscess. Percutaneous nephrolithotomy (PCNL) was performed. However, bile content discharge from the PCNL wound was observed later. Abdominal computed tomography revealed multiseptated right renal abscess with the right psoas muscle involvement and obscure plane between the right kidney and the duodenum. A fistulogram demonstrated the presence of duodeno-reno-cutaneous fistula. He underwent right nephrectomy and wedge resection of the duodenum for the closure of the fistula. The postoperative course was smooth, and the PCNL wound healed spontaneously during the admission. He remained asymptomatic 6 months after the operation. This report includes a literature review on the etiology, diagnosis, and treatment of the renoduodenal fistula.

Purpose The aim of this study was to evaluate the possibilities of ultrasound, computed tomography (CT) and magnetic resonance imaging (MRI) in diagnosing acute pyelonephritis (AP) and renal abscess. Patients and Methods Two hundred and seven patients with AP were followed up from 2010 throughout 2015. All the patients were divided into three groups. Group 1 included 113 (54.6%) patients with acute nonobstructive pyelonephritis; group 2 included 33 (15.9%) patients with acute obstructive pyelonephritis; and group 3 included 61 (29.5%) pregnant female patients with AP. All 207 patients with AP underwent ultrasound examination of the kidneys. Computed tomography (CT) was performed in 87 patients (42.0%). MRI was performed in 14 patients (6.7%). Results We identified the ultrasound (US), magnetic resonance (MR), and CT-signs of acute renal inflammation at different stages of the process. The main us-signs were decreased mobility of the kidney, its enlargement, thickened parenchyma, hydrophilic parenchyma and an impairment of corticomedullary differentiation. The typical CT-signs of AP were enlargement of the kidney with its thickened parenchyma and an impairment of corticomedullary differentiation. The main MR-signs of AP were enlargement of the kidney (>12 cm lengthwise), thickened parenchyma (Conclusions Assessment of the structural and functional state of renal parenchyma and the upper urinary tract using techniques such as ultrasonography, CT, MRI contributes to more efficacious treatment of patients at different stages of AP and timely drainage with properly adjusted pathogenetic therapy at the infiltrative stage is instrumental in preventing purulent destructive forms of AP.

Renal and perinephric abscesses are usually confined within the Gerota's fascia and treated with conservatively. We herein present a case with peritonitis caused by the spillage of perinephritic abscess into the abdomen through the Gerota’s fascia, which is clinical outcome. A 41-year-old female patient was admitted to emergency room with complaints of abdominal pain, bloating, fever, and fatigue. During the operation, an abscess of approximately 20x15cm in size was observed to extend into the abdominal cavity through the Gerota's fascia, causing secondary peritonitis. On the 20th postoperative day, the patient was uneventfully discharged. Delay in diagnosis of renal and perinephric abscesses can lead to fatal complications. Laparotomy is a life-saving approach in cases that cannot be treated with percutaneous drainage, when the abscess is large and causes complications such as strictures, fistulas, or secondary peritonitis, by extension through the Gerota's fascia, as in our case., Renal ve perinefrik apseler genellikle Gerota fasyasında sınırlı kalırlar ve konservatif yöntemlerle tedavi edilirler. Perinefritik apselerin tanısında gecikme olması öldürücü komplikasyonlara yol açabilir. Gerota fasyasını aşan, striktür, fistül veya bizim olgumuzda olduğu gibi sekonder peritonitin geliştiği ve perkütan drenajla tedavi edilemeyen geniş apselerde laparotomi hayat kurtarıcıdır.

An abscess is a collection of infected fluid and inflammatory debris that has been walled off by the body’s immune system. Any inflammatory process in the abdomen or pelvis such as appendicitis, diverticulitis, and Crohn’s disease can lead to an abscess. Traumatic puncture wounds and foreign bodies can also result in abscess formation. Common locations for an abdominal abscess are the subphrenic and subhepatic spaces. Abscesses can also develop in organs, such as the liver (intrahepatic abscess) or kidney (renal abscess). An abscess must be promptly treated with antibiotics and either percutaneous or surgical drainage to prevent systemic infection or sepsis.

Sex differences in urinary tract infection (UTI) susceptibility and severity are known, but have historically focused on anatomic differences between males and females. Until recently, experimental UTI has been limited to female animals due to ease of transurethral bladder catheterization. Olson and colleagues have developed a model of experimental UTI independent of sex that relies on direct bladder inoculation and thus permits investigation of sex differences in UTI susceptibility. They now build upon their prior work in this model by implicating androgens as drivers of tubular invasion by uropathogenic Escherichia coli, which form luminal bacterial communities preceding renal abscess formation.

Ureaplasma species are small, fastidious bacteria that frequently colonize the lower reproductive tract of asymptomatic hosts. These organisms have been well described to cause chorioamnionitis, neonatal infection, and urethritis, and to a lesser degree surgical site infection and infection in transplant recipients. Outside of these settings, invasive Ureaplasma infections are rare. We describe the case of a young woman receiving rituximab for multiple sclerosis who presented with fever and bilateral renal abscesses due to Ureaplasma spp., which was successfully treated with oral doxycycline. We searched the literature for cases of invasive Ureaplasma infection and found a patient population that predominates with humoral immunodeficiency, either congenital or iatrogenic. Diagnostic and therapeutic interventions are discussed.

Introduction Kidney abscess is defined as a suppurate lesion collected in the renal parenchyma. It is a rare pathology in pediatrics. We report four cases of renal suppuration. Material and method This is a retrospective study over a period of 9 years (2010-2018) collaging all cases of renal abscess having been hospitalized in the pediatric ward, emergency and pediatric resuscitation of Sfax. Results We collected 4 cases of renal abscess. These were two boys and two girls. The average age was 5 ½ years with extremes ranging from 3 ½ years to 10 years. The delay of diagnosis was estimated at an average of 19 days. Fever and lumbar pain were reported in three cases. In the other case, it was a respiratory chart. The urinary and digestive signs were variable or absent in one case. Hyperleukocytosis with a positive CRP were constant. The cytobacterioligical urine exam practiced systematically was positive in 2 cases by isolating an Enterobacter Cloacoe in one case and an E Coli in another case, it revealed aseptic leukocyturia in one case and it showed no abnormality in the other case. The diagnosis of abscess was made thanks to the ultrasound-CT abdominal pairing performed in all patients. All patients were treated with intravenous antibiotic therapy. Percutaneous drainage was performed in one patient due to the inefficacy of antibiotic alone. The evolution was marked by apyrexia and improvement of other clinical and biological signs. Conclusion The treatment of the kidney abscess is based on antibiotic therapy with or without percutaneous or surgical drainage depending on the size of the abscess and the clinical course.

URS is considered a safe method of stone treatment. Complications most often do not exceed grade II of the Clavien Dindo classification. Although it is considered safe, in some cases major complications may occur from grade III onwards. A 50-year-old man underwent f-URS for a 2.7 × 1.2 cm left medium caliceal stone; after surgery patient developed an intraparenchymal renal abscess and concomitant pulmonary and hepatic septic emboli. Despite attempts to treat it conservatively, nephrectomy was the only effective treatment. The f-URS is minimally invasive and effective procedure, although it is not free from complications.

Background: Acute lobar nephronia (ALN) is an underdiagnosed urological condition which has scarce publications available. It is described as an acute inflammatory and bacterial infection of the kidney without liquefaction, and in fact likely represents a mid-point between acute pyelonephritis and renal abscess. Patients usually present with fevers, septic shock, and occasionally abdominal and flank pain. To date there is no consensus on its optimal management. Objective: The aim of this study was to describe the presentation, evaluation, multidisciplinary involvement and management, and follow-up of ALN. Method: An observational study was performed utilising four patients from our single centre urology department. Data collected included basic demographics and diagnosis and management. All four patients are presented below. Observations and comparisons were made between all patients to aid in understanding the commonality between their presentations and managements. Conclusion: ALN is an underdiagnosed condition. Its diagnosis and management is difficult and requires a multidisciplinary approach. Many patients require prolonged inpatient management and close follow-up. An underlying renal mass is always an important differential diagnosis to be considered. To establish a routine workflow for in vivo magnetic resonance imaging (MRI) of mice infected with bacterial biosafety level 2 pathogens and to generate a mouse model for systemic infection with Staphylococcus aureus suitable for monitoring by MRI. A self-contained acrylic glass animal bed complying with biosafety level 2 requirements was constructed. After intravenous infection with 105 colony-forming units (CFU) (n = 3), 106 CFU (n = 11) or 107 CFU (n = 6) of S. aureus strain Newman, female Balb/c mice were whole-body scanned by 7T MRI. Abdominal infections such as abscesses were visualized using a standard T2-weighted scan. Infection monitoring was performed for each animal by measurements at 1, 3, and 7 days after infection. Intravenous pathogen application led to a dose-dependent decrease in survival probability (p = 0.03). In the group with the highest infectious dose the 7-day survival rate was 33 %. An intermediate S. aureus dose showed a survival rate of 80 %, whereas at the lowest infection dose, none of the animals died. All animals with the highest infection dose exhibited hepatic abscesses 4 days after inoculation, 80 % developed renal abscesses on the 3rd day. Mice obtaining the intermediate S. aureus load reached a plateau at day 4 with 72 % liver and 60 % renal abscess probability. No abscesses were observed in other abdominal organs at any time point. The implemented experimental setup provides a suitable and reliable in vivo MRI method to study murine abdominal infection models using BSL-2 pathogen. Systemic Staphylococcus aureus infection leads to a dose-dependent development of hepatic and renal abscesses.

Case series summarySix cats were diagnosed with renal abscesses. Common clinical findings were lethargy, dehydration, abdominal pain and nephromegaly. Fever was noted in half of the cases. Diagnosis was established by ultrasonography, cytological examination and bacterial culture of abscess aspirates. At least one possible contributing factor could be identified in all cases. Antibiotics were consistently used and in two cats the abscess was surgically drained. The short-term outcome was fair but the long-term outcome was dependent on the underlying condition.Relevance and novel informationThe results of this small case series suggest that renal abscess should be considered when nephromegaly and/or abdominal discomfort are noted. Diagnosis of renal abscess is straightforward when ultrasonography and fine-needle aspirate analysis can be performed. Medical treatment is assumed to be preferable but surgical treatment may be warranted on a case-by-case basis. Given that almost every affected cat was diagnosed with at least one comorbidity, a thorough evaluation is recommended for all cats with renal abscesses.
