To evaluate the microvasculature during remission in patients with pars planitis (PP-IU) and multiple sclerosis-associated intermediate uveitis (MS-IU) using optical coherence tomography angiography (OCT-A). Single-center, descriptive, case-control study was conducted. Adult patients (≥16 years) with IU in remission (PP-IU and MS-IU) and healthy age-sex matched healthy controls (HC) were enrolled to the study. Demographic/clinical features, best-corrected visual acuity (BCVA), ocular findings, neurological symptoms and preferred treatments were recorded. The presence of cystoid macular edema (CME) during follow-up was recorded. All IU patients in remission and HC subjects were scanned with OCT-A. Foveal avascular zone (FAZ) areas of superficial and deep capillary plexus (SCP/DCP) and vascular densities of SCP, DCP and choriocapillaris were obtained from OCT-A and compared between the groups. Sixty-nine eyes of 37 IU patients in remission and 20 HC were included (44 eyes/23 patients in PP-IU, 25/14 in MS-IU, 40/20 in HC). No statistically significant differences were observed in terms of demographic or clinical characteristics of the patients. The vascular density in the SCP was significantly reduced in the PP-IU and MS-IU groups compared to the HC group (p <.05). Nevertheless, there were no significant changes in any of the OCTA parameters between the IU groups. Uveitis duration was found to be correlated with enlargement of the FAZ area in PP-IU (p =.039). OCTA may not be useful in differentiating between PP-IU and MS-IU. Nevertheless, the primary implication in SCP potentially elucidates the pathogenesis of these two subtypes of IU, which are characterized by a shared pathogenesis. To evaluate the ocular features, course, and prognosis of uveitis in the ocular sarcoidosis (OS) subgroups without previously known systemic sarcoidosis. Ninety-one eyes of 49 patients were included. Definite OS patients were classified as group 1 (n = 15), presumed OS patients as group 2 (n = 15), and probable OS patients as group 3 (n = 19). The most common presentation of OS was panuveitis in all groups. During the follow-up, bilateral ocular involvement was observed in 85.7% of the patients. The most common extraocular involvement was pulmonary involvement, which was detected in 61.2% of the patients at the time of diagnosis. Ophthalmologists should be aware that uveitis may be the first sign of systemic sarcoidosis. In all subgroups of OS, uveitis tends to be bilateral and the most common type is panuveitis. To report the presenting features and outcomes in patients who developed bilateral acute iris transillumination (BAIT) or bilateral acute depigmentation of iris (BADI) following acute COVID-19 infection. Thirty two eyes of 16 patients were reviewed retrospectively. The severity of COVID-19 infection, use of antibiotics, time of onset of ocular symptoms; ocular signs, the course and surgical procedures were recorded. 24 eyes of 12 BAIT and eight eyes of four consecutive BADI patients were included. The mean time between infection and onset of ocular symptoms was 2.5±1.1 weeks. Nine patients were treated with oral moxifloxacin for COVID-19 prior to presentation. Trabeculectomy was performed in 7 eyes (21.8%) of 5 BAIT patients; in the postoperative follow-up, IOP was controlled without medication in 6 eyes, with medication in 1 eye. To compare the quantitative measurements of the anterior chamber angle (ACA) and iris parameters in patients with juvenile open-angle glaucoma (JOAG), pigmentary glaucoma (PG), and healthy controls using anterior segment optical coherence tomography (AS-OCT).Methods: This was a retrospective, cross-sectional study of 25 eyes with JOAG, 25 eyes with PG, and 25 control eyes. Anterior chamber depth, angle-opening distance 500 and 750, trabecular-iris space 500 and 750, scleral spur angle, iris thickness (IT, measured at the thickest part), and iris bowing were obtained using AS-OCT (Visante" OCT 3.0 Model 1000, Carl Zeiss Meditec, Inc).Results: The quantitative ACA parameters were found to be significantly higher in JOAG and PG patients compared to healthy controls (P < 0.001); there was no significant difference between the eyes with JOAG and PG (P > 0.05). In eyes with JOAG and PG, there was significantly backward bowing of the iris in temporal and nasal angles compared to control subjects (P < 0.001). Median iris bowing was not significantly different between the patients with JOAG and PG (P > 0.05). The temporal and nasal angle iris thickness were significantly thinner in eyes with JOAG than the eyes with PG (P < 0.001) and age-matched control subjects (P < 0.001). The median IT did not differ between the patients with PG and control subjects (P > 0.05). In patients with JOAG, the intraocular pressure (IOP) was inversely correlated with IT (r = -0.43, P < 0.05).Conclusion: AS-OCT provided quantitative data on the ACA and iris parameters in JOAG and PG. The evaluation of the ACA and iris structures using AS-OCT revealed higher ACA measurements and posterior bowing of the iris in patients with JOAG and PG. Purpose: To develop an algorithm for the diagnosis of Behçet's disease (BD) uveitis based on ocular findings. Methods: Following an initial survey among uveitis experts, we collected multi-center retrospective data on 211 patients with BD uveitis and 207 patients with other uveitides, and identified ocular findings with a high diagnostic odds ratio (DOR). Subsequently, we collected multi-center prospective data on 127 patients with BD uveitis and 322 controls and developed a diagnostic algorithm using Classification and Regression Tree (CART) analysis and expert opinion. Results: We identified 10 items with DOR >5. The items that provided the highest accuracy in CART analysis included superficial retinal infiltrate, signs of occlusive retinal vasculitis, and diffuse retinal capillary leakage as well as the absence of granulomatous anterior uveitis or choroiditis in patients with vitritis. Objectives: This study used ultrasound biomicroscopy (UBM) imaging to evaluate the anterior segment structure and surgical area of patients with open angle glaucoma following a trabeculectomy procedure. Methods: Consecutive patients with primary open angle or pseudoexfoliative glaucoma who underwent a trabeculectomy were included. UBM was used to examine the anatomical parameters of anterior chamber depth, anterior chamber angle, angle opening distance at 500 µm (AOD500), trabecular ciliary process distance (TCPD), iris ciliary process distance, scleral ciliary process angle (SCPA), and ciliary process thickness, as well as the internal ostium opening, ostium-iris distance, and scleral flap dimensions. Bleb morphologies were evaluated qualitatively using the Yamamoto classification and quantitatively according to the presence of drainage from the medial, lateral, and posterior borders. Results: Twenty-five eyes of 23 patients (4 female, 19 male) were included in the study. The TCPD and SCPA measurements were significantly greater in the postoperative third month (1.04±0.12 mm vs 1.09±0.16 mm, p<0.05; 58.6±3.9° vs 60.8±3.8°, p<0.05, respectively). Cases with a longer ostium-iris distance had a significantly greater AOD500, TCPD, and SCPA (p<0.05). Bleb drainage and morphological classification were correlated (p=0.001). Conclusion: UBM is a sound and efficient method to investigate anterior segment variations and the surgical bleb following a trabeculectomy. TCPD and SCPA values increased after surgery. Purpose/Aim of the study: The aim of this study was to compare the lamina cribrosa (LC) measurements obtained by Spectral-Domain Optical Coherence Tomography (SD-OCT) and Swept-Source Optical Coherence Tomography (SS-OCT) in the same eye and we also investigate how the differences in measurement will change in the presence of pseudoexfoliation glaucoma (PEG). Materials and Methods: A total of 30 patients from one institution have accepted. Detailed medical case histories and clinical examination, optic nerve head imaging using the SD-OCT (Heidelberg Engineering, Heidelberg, Germany) and SS-OCT (Triton; Topcon Medical Systems, Tokyo, Japan) for patients with pseudoexfoliation glaucoma and healthy volunteer. Results: Fifty-three eyes of the 30 patients (13 female, 17 male) were included in the study. Twenty-nine of eyes were healthy and 24 of eyes had pseudoexfoliation glaucoma. The mean age was 62 ± 7.3 years (range, 50–86 years). The difference between the mean Bruch's membrane opening distance measured by SD-OCT (1504.7 ± 154.2 µm) and by SS-OCT (1568.6 ± 193.3 µm) was statistically significant (p =.009). The difference between LC depth and LC thickness measurements between two OCT devices were not statistically significant. There was a negative correlation between the difference of two devices Bruch's membrane opening distance measurements and the cup/disc ratio (p =.007). Conclusion: Generally accepted belief is that lamina cribrosa visibility would be worse in healthy subjects who have thick prelaminar tissues, and better in individuals with glaucoma who had suffered the loss of prelaminar tissue. The difference between Bruch's membrane opening distance measurements taken with two devices was greater in normal eyes, and this difference was decreasing in glaucomatous eyes in our study. Purpose: The purpose of this study is to compare the thickness and depth measurements of the lamina cribrosa (LC) obtained using a swept-source optical coherence tomography (SS-OCT) device in idiopathic intracranial hypertension (IIH) patients and healthy subjects.Methods: This retrospective, cross-sectional observational study included 16 eyes with IIH and 20 control eyes. The LC measurements with serial horizontal B scans of the optic nerve head were obtained using SS-OCT (Topcon 3D DRI OCT Triton). The anterior lamina surface (ALS) depth, posterior lamina surface (PLS) depth, and LC thickness measurements were evaluated.Results: In patients with IIH, the mean ALS depth was 225.00 ± 58.57 μm and the mean PLS depth was 449.75 ± 63.50 μm. In the IIH control group, the corresponding values were 359.40 ± 105.38 and 570.10 ± 99.41 μm (P < 0.05). The difference in LC thickness between the IIH and control subjects was not statistically significant.Conclusion: LC can be evaluated using an SS-OCT device. LC was displaced anteriorly in patients with IIH compared with normal controls. Objectives. To evaluate the visual performance of Toris K soft contact lenses in patients with moderate-to-advanced keratoconus and also to compare the results according to cone types, cone location, and severity of keratoconus. Materials and Methods. Sixty eyes of 40 participants were included in this retrospective study. Uncorrected visual acuity (UCVA), best-spectacle corrected visual acuity (BCVA), best-contact lens corrected visual acuity (BCLCVA), and comfort rating via visual analogue scales (VAS) were measured. Results. The mean age was 27.3 ± 8.6 years (range: 18 to 54). The mean logMAR UCVA, BCVA, and BCLCVA were 0.85 ± 0.38 (range: 0.30–1.30), 0.47 ± 0.27 (range: 0.10–1.30), and 0.16 ± 0.20 (range: 0–1.00). There were significant increases in visual acuities with contact lenses (p<.05). BCLCVA was significantly better in oval type than globus type (p=.022). UCVA and BCLCVA were significantly better in moderate keratoconus group (p=.015, p=.018). The mean line gain in Snellen was 3.6 ± 1.8 lines (range: 0–7 lines). The mean line gain was higher in central cone group than paracentral cone group and oval group than globus group (p=.014, p=.045). The mean VAS score was 8.14 ± 1.88 (range: 6–10). Conclusions. A 47-year-old male presented with bilateral 4 + circulating pigment in the anterior chamber, diffuse iris transillumination, dilated pupils unresponsive to light, and high intraocular pressure (IOP) levels in both eyes. Visual acuity and IOP improved bilaterally with topical steroid and antiglaucomatous therapy. In the 10th month, bilateral cystoid macular edema (CME) was developed and resolved after subtenon triamcinolone injections. CME recurred after cataract surgery in the right eye which was treated with intravitreal dexamethasone implant injection. CME was recurred in the left eye and treated with intravitreal dexamethasone implant at the same setting with cataract surgery. CME can be seen in the course of bilateral acute iris transillumination (BAIT). This is the first BAIT case presenting with bilateral CME. [ABSTRACT FROM AUTHOR]

Background. To investigate the efficacy of selective laser trabeculoplasty (SLT) for lowering intraocular pressure (IOP) in patients with open angle glaucoma (OAG) secondary to emulsified silicone oil (SO). Methodology/Principal Findings. Prospective, interventional, consecutive case series of 11 eyes with sustained elevation of IOP after SO removal.The mean IOP at baseline, week 1, month 1, month 3, and month 6 was evaluated. The mean baseline IOP was significantly decreased from 25 ± 2.7mmHg to 18.4 ± 5.5mmHg at week 1 (ρ = 0.01), 17.9 ± 3.1mmHg at month 1 (ρ = 0.008), 15.8 ± 3.9mmHg at month 3 (ρ = 0.003), and 16.2 ± 4.7mmHg at month 6 (ρ = 0.004). IOP < 21mmHg was achieved in 91% of the eyes without a significant complication at month 6. Conclusion/Significance. Purpose: To evaluate the changes in macular and peripapillary retinal nerve fiber layer (RNFL) thickness with optic coherence tomography (OCT) during active inflammation period and after recovery in eyes with acute anterior uveitis (AAU) and to compare them with the patients' healthy eyes. Material and Method: Twenty-two consecutive patients diagnosed with unilateral AAU in the Uvea Department of our hospital were included in this study. OCT measurements were taken with 'Stratus OCT fast macular thickness map' program and 'fast RNFL thickness' program. Mean thicknesses of fovea centered 1 mm, 3 mm, 6 mm circles, total macular volume (TMV) and mean RNFL thickness in 3.4 mm peripapillary circle were compared between eyes with AAU and healthy eyes. Also, the measurements during the acute attack were compared with measurements of the same eyes after recovery. Results: There were 14 women and 18 men with a mean age of 37 (range, 20-65) years. Total macular volume, peripapillary RNFL thickness and macular thicknesses in 1 mm, 3mm, 6 mm circles were significantly elevated in acutely inflamed eyes as compared with healthy eyes (p<0,05). There were significant differences in the macular thicknesses in the circles of 3 and 6 mm and TMV between acute inflammation period and after treatment (p<0.05). However, there were no significant differences in the peripapillary RNFL thickness measurements (p>0.05). When measurements after recovery were compared with the patients' healthy eyes, the circle of 3 mm thickness and TMV were still significantly higher in the recovered eyes. Discussion: Although there were no inflammation findings in the posterior segment of eyes with AAU, increased peripapillary RNFL and macular thickness can be detected with OCT. Background/ObjectivesSubjects/MethodsResultsConclusionsTo investigate the long-term efficacy and safety of adalimumab(ADA) in the treatment of patients with serpiginous choroiditis (SC) refractory to conventional therapy through quantitative parameters.A retrospective analysis was conducted on patients diagnosed with SC clinically and through fundus autofluorescence(FAF). Patients receiving ADA treatment were included. Demographic and clinical characteristics of the patients, association with tuberculosis (TB) infection, number of immunosuppressive therapies, recurrences, best corrected visual acuity (BCVA) change, and ADA-related side effects were recorded. The progression rate before and after ADA was calculated based on the area involved by FAF.Sixteen eyes of 8 patients (3 female/5 male) were enrolled to the study. The median (IQR) age was 53.5 (16.5) years. Diagnosis was SC in 4, ampiginous choroiditis in 3, and TB-related serpiginous-like choroiditis in 1 patient. Peripapillary involvement was present in 10 of 16 eyes. The area involved by FAF continued to progress under ADA treatment, however the progression rate was decreased (p = 0.143).The BCVA was preserved (p = 0.772). The number of systemic and local treatments decreased with ADA (p = 0.025 and 0.019, respectively). Additionally, the number of recurrences was reduced with ADA (p = 0.002). Median (IQR) follow-up was 45(28.75) months. Two patients experienced ADA-related side effects (pulmonary TBand rash).Our findings suggest a promising role for ADA in halting the progression of SC and have implications for improving outcomes. • Lamina cribrosa is the firstly affected area in the development of optic nerve damage due to glaucoma. • Glaucoma clinic progresses more aggressively in the presence of pseudoexfoliation. • Optical coherence tomography (OCT)- enhanced deep imaging (EDI) mode allows the optic nerve head and lamina cribrosa to be evaluated more clearly. • The presence of thinning and posterior dislocation in the lamina cribrosa can be observed among the findings of glaucomatous damage. This study aimed to compare lamina cribrosa (LC) parameters obtained by spectral-domain optical coherence tomography (SD−OCT) of eyes with exfoliation syndrome (PXS), exfoliation glaucoma (PXG) and healthy subjects. In this cross-sectional comparative study, 206 eyes of 206 subjects were included. The Bruch's membrane opening distance (BMOd), the anterior and posterior borders of the LC (LC thickness) and the anterior laminar depth (ALD) were imaged using the enhanced depth imaging (EDI) mode of SD−OCT. There were 96 eyes in the PXG group, 55 eyes in the PXS group, and 55 eyes in the control group. The LC thickness was the thinnest in the PXG group (151.10 ± 51.18 μm), followed in the PXS group (158.76 ± 49.62 μm), and the thickest in the control group (181.00 ± 39.10 μm) (p = 0.002). In PXG cases where LC was observed in the deepest location, the ALD value was highest (423.92 ± 111.75 μm) in the PXG group, followed by the control group (403.08 ± 63.56 μm), and PXS group (357.43 ± 80.87 μm) (p < 0.001). The BMOd values were largest in the PXG group (1542.43 ± 152.99 μm), followed by the control group (1506.52 ± 169.09 μm) and PXS group (1435.74 ± 141.06 μm) (p < 0.001). In the PXG group, peripapillary retinal nerve fiber layer (pRNFL) thickness, BCVA, and cup to disc (C/D) ratio were also statistically different from the other groups (p < 0.001). We found thinner LC thickness in PXG and PXS cases relative to the control group. To evaluate retinal microvasculature and blood flow during Behcet's uveitis (BU) remission via optical coherence tomography angiography (OCT-A), and compare the results with those from healthy eyes. A total of 35 eyes of 35 BU patients presenting during the remission period of BU involving the posterior segment were enrolled, as were 30 normal eyes of 30 age and sex-matched healthy participants. Optical coherence tomography angiograms were analyzed. Foveal avascular zone (FAZ), vessel densities, and flow areas were calculated and compared. Thirty-five eyes of 35 BU patients and 30 normal eyes of 30 age and sex-matched subjects were included in the study. The superficial FAZ was slightly higher in the BU group than in the control group (p = 0.01), but deep FAZ was substantially higher (p < 0.001). Deep foveal and parafoveal vessel densities were significantly lower in BU eyes (p < 0.05). Flow areas in superficial and deep plexuses were lower in BU eyes (p < 0.05), however choroidal flow was similar (p > 0.05). BU can cause permanent damage especially in the deep capillary plexus and OCT-A provides a detailed view of the retinal microvasculature which helps us to visualize vascular damage in these patients. • Choroidal granuloma of ocular Tuberculosis can be the first sign of Tuberculosis. • Choroidal granuloma in tuberculosis can be diagnosed with the use of OCT. • OCT-A may provide useful information about the retinal microvascular involvement in patients with choroidal tuberculoma. Choroidal granuloma is rarely seen as the first sign of tuberculosis(TB). Here we report a case of a 34-year old male with a choroidal mass which was associated with serous retinal detachment on EDI-OCT and the "contact sign" between the neurosensory retina and the retinal pigment epithelium. We analysed the macular microvascular features of tuberculoma with Optical Coherence Tomography Angiography(OCTA). After 3 months of antituberculosis treatment, we observed the shrinkage of granuloma with permanent changes of macular microvasculature in deep capillary plexus and choriocapillaris. PurposeMethodsResultsConclusionTo investigate the association of clinical findings and indocyanine green angiography (ICGA) score with inflammatory markers derived from complete blood count (CBC) parameters in patients with Vogt-Koyanagi-Harada (VKH) to determine the diagnostic and predictive role.Demographic characteristics, presenting complaints, ocular findings, optical coherence tomography findings, ICGA scores and best corrected visual acuity were recorded in treatment-naive VKH patients at presentation. Patients were divided into two groups as acute stage and chronic recurrent stage. CBC parameters were noted in patients at presentation and healthy controls (HC, n = 25). Neutrophil-lymphocyte-platelet-monocyte counts, neutrophil/lymphocyte (NLR), platelet/lymphocyte (PLR), monocyte/lymphocyte and systemic immune-inflammation index (SII) were recorded. The association between these markers and clinical severity were evaluated.Thirty-two patients with VKH (23 females/9 males) with a mean age of 34.1 ± 14.6 years were included in the study. There was an increase in neutrophil count, NLR and SII in patients with VKH compared to HC (p < 0.001). The cut-off values for these three parameters were 4.37, 2.24 and 562.35, respectively. Twenty-six patients presented in the acute stage and six patients presented in the chronic recurrent stage. Choroidal thickness, early stromal hyperfluorescence and total ICGA scores were higher in patients presenting in the acute stage (p < 0.001, 0.001 and 0.025, respectively). Patients with higher disease severity at presentation were treated earlier. Early stromal vessel hyperfluorescence and choroidal vasculitis scores were correlated with decreased lymphocyte count, increased NLR, PLR and SII (p < 0.05).CBC-derived inflammatory parameters indicate that VKH is a systemic inflammation. Purpose: To evaluate outcomes of patients with uveitic glaucoma secondary to Behçet disease (BD) who underwent Ahmed glaucoma valve (AGV) implantation., Patients and Methods: A retrospective chart review of 14 eyes of 10 patients with uveitic glaucoma associated with BD who underwent AGV implantation at a tertiary referral center. Treatment success was defined as intraocular pressure (IOP) between 6 and 21 mm Hg with or without antiglaucoma medication, without further additional glaucoma surgery or loss of light perception. The main outcome measures were IOP, best-corrected visual acuity measured with Snellen charts, and number of glaucoma medications., Results: Mean duration of postoperative follow-up was 18.2±6.6 months (range, 6 to 31 mo). Of the 14 eyes, 10 (71.4%) were pseudophakic and 5 (35.7%) had primary AGV implantation without a history of previous glaucoma surgery. At the most recent follow-up visit, 13 of the 14 eyes had an IOP between 6 and 21 mm Hg. Mean IOP was significantly reduced during follow-up, as compared with preoperative values (P≤0.005). The cumulative probability of surgical success rate was 90.9% at 18 months based on Kaplan-Meier survival analysis. The mean number of antiglaucoma medications required to achieve the desired IOP decreased from 3.4±0.5 preoperatively to 1.0±1.1 postoperatively (P≤0.05). Visual acuity loss of >2 lines occurred in 4 eyes (28.5%) due to optic atrophy associated with retinal vasculitis. Temporary hypotony developed during follow-up in 4 eyes (28.5%) at first postoperative week., Conclusions: For the management of uveitic glaucoma associated with BD, AGV implantation is a successful method for glaucoma control but requires additional surgical interventions for high early hypotony rates.

Purpose: The pupil diameter under low ambient illumination is diagnostically valuable for refractive surgery. The aim of study was to compare the NeurOptics® Pupillometer, Sirius®, and Ocular Wavefront Analyser® in determining scotopic pupil diameter., Materials and Methods: A total of 96 eyes of 48 subjects were included. The scotopic pupil size was measured with 3 instruments and the measurements repeated following instillations of 1% cyclopentolate. Agreement between the instruments was assessed., Results: The mean measurement obtained by Sirius was significantly larger than Ocular Wavefront Analyser and NeurOptics. The Ocular Wavefront Analyser measured significantly smaller than the others. The mean cycloplegic pupillary diameters (7.73±0.70 mm with NeurOptics, 7.42±0.45 mm with Ocular Wavefront Analyser, and 8.06±0.76 mm with Sirius) were significantly different obtained by 3 instruments (p<0.001, for each; one-way analysis of variance and paired t tests)., Conclusions: The differences between measured pupil diameters with or without cycloplegia obtained by the NeurOptics, Sirius, and Ocular Wavefront Analyser were significant and have unacceptable levels of disagreement. These results may not be related to illumination and accommodation only, but also to measurement algorithms and technical differences of the devices.
